Description

手掌激活型药物递送装置
本申请是于2012年4月16日进入中国国家阶段的PCT专利申请PCT/US2010/052894(申请日为2010年10月15日,中国发明专利申请号为201080047681.7,发明名称为“手掌激活型药物递送装置”)的分案申请。
根据35 USC§ 119和/或§120的规定,本专利申请要求于2010年7月7日在先提交的美国临时申请序列号61/361,983和提交于2009年10月16日的61/252,378的优先权,所述专利申请全文以引用方式并入本专利申请中。
技术领域
本发明整体涉及用于肠胃外药物递送的方法和装置。该装置通过确认药物递送过程的完成,实现辅助人工药物递送。该装置提供了一种系统,该系统具有改良的安全性和易用性,并能向使用者提供听觉反馈或其他形式的反馈以指示何时药物递送正在进行、何时已完成、或两者均给予指示,从而避免出现不完全给药和药物浪费这两种情况中的一种或两种,同时提供具有改良的安全性和易用性的系统。
背景技术
多年来,用于肠胃外药物递送的公认方法一直是通过采用注射器和针头进行的。注射器装有一些药物,这些药物要么是预先填充于注射器内销售的药物,要么是通过将药物从药瓶或其他容器吸入注射器内而引入注射器的药物。由于制造成本低并且设计简单、有效,注射器已被广为接受。然而,对于使用者而言,注射器和针头具有许多缺点。
一个缺点是许多患者对针头有恐惧感。在需要自我药疗的情况(例如需要多次、每天注射的情况)下,由于对针头的恐惧感、通常与注射关联的疼痛、通过针头和注射器适当给药所需的敏捷度、或其他类似的因素,他们可能不会按照医嘱方案进行药疗。对于视觉、敏捷度、或意识受损的一些患者,通过针头和注射器进行自行给药可能面临另外的困难,从而阻止他们接受他们所需的药物。
除了与针头和注射器药物治疗方案相关的社会烙印以外,不仅对于患者,而且对于他们周围的人,还有与针头和注射器相关的安全性和处置方面的问题,这些问题可由污染的针头、意外穿刺、交叉污染等等引起。然而,尽管存在这些缺点,由于能够控制针头插入并在按压注射器内柱塞时控制药物递送的速度,进而控制与此类药物注射相关的疼痛感和不适感,因此许多患者仍被鼓励采用针头和注射器递送药物。
为了使自行给药更为方便,多年来已取得若干进展。这些进展包括具有改进的针尖几何形状的更小的针头以减少疼痛。采用了在使用前、使用后、或者使用前和使用后套住针头的安全注射器,以使对于针头出现意外穿刺的顾虑最小化。注射器设计中改进的人类工程学也有助于使通过针头和注射器进行精确和安全的自行给药所需的敏捷度得以降低。开发了具有类似于钢笔的形状因子的预填充的一次性装置,以改善剂量准确性,并且采用了自动注射器,通过回缩针头或在针头的周围设置护罩,对患者隐藏针头,以减少恐惧感和安全顾虑。
虽然这些进展已改善了基于针头和注射器的药物递送方式,但是人类工程学设计、钢笔形状以及自动注射器都保留了与最初的针头和注射器概念的实质相似性,从而限制了需要自行给药的患者对它们的接受。当前的系统采用表明“抓握和刺入”通用注射技术的形状因子,其中使用者将装置抓握在手掌中并且将拇指置于激活按钮之上。
当前的自动注射器将药物递送到机体内的控制传送给机械系统。因为此系统高度依赖于自动注射器特定的机械设计,患者可能需要专门的培训,才能使用装置,并且仍然存在剂量不准的风险。这种情况在递送可能仅每周一次施用或甚至更罕见施用的极昂贵药物时问题重重。
当前自动注射器的典型使用方法包括在装置处于药物递送的过程中,患者将装置紧贴皮肤持握几秒钟。许多使用者,尤其是老年使用者,可能经历胳膊或手的疲劳,这导致他们将装置紧贴在皮肤上的施压不均,或者他们可能过早移除装置。任何一种情况可导致剂量不准、药物浪费、不适加重等等。在任何一种这些情况下,包括或演化自传统注射器和针头系统的当前装置和方法的缺点是减弱了医嘱给药方案的功效。
最后,对于任何健康护理相关的装置或服务,治疗方案中任何常用元件的成本必须加以考虑。虽然在药瓶中提供药物(用于在或大致在患者的药物治疗时间填充空注射器)可提供最便宜的解决方案,但它增加了昂贵药物额外的浪费或损失的机率。如果药物需要冷藏,在填充注射器之前和之后每次取出和重新插入冷藏装置时,药物可能发生降解,如果药瓶含有一些在长时间段递送的药物,则可导致低于预期的药物功效。虽然预填充的注射器在可靠性和便利性方面均具有优势,但此类装置仍具有之前列举的固有缺点。
就诸如预填充自动注射器的装置而言,此装置最常见的是针对广泛的药物而制造,而并非专为任何一种药物所定制。因为此类装置依赖采用弹簧控制药物注射速度的机械系统,许多不同粘度的药物或需要冷藏并且由于温度变化而明显改变粘度的药物,可因为自动注射器设计中预定的弹簧力而被过快或过慢递送。在许多情况下,弹簧力太低可导致不完全药物递送,还未完成递送便移除装置,或由于延长注射装置在体内插入的时间而导致对使用者造成过度的疼痛和不适。然而,弹簧力太高可导致药物递送过于迅速而使药物降解,或者可给患者带来注射力疼痛,所述注射力疼痛是因酸性药物的快速递送或因皮下或血管内的压力梯度的诱导而引起的。
因此,在间歇性肠胃外药物递送领域拥有许多机会取得进展,使其可以克服“针头恐惧”、减少患者的疼痛及增加许多药物治疗方案的安全性、可靠性和功效。

具体实施方式
下面的详细描述应参考附图来阅读,其中不同附图中的相同构件被相同地编号。附图未必按比例绘制,仅出于说明的目的示出示例性的实施例,并且并非旨在限制本发明的范围。详细描述以举例的方式而不是限制性方式说明了本发明的原理。
本发明为药物递送装置及其使用方法,该装置克服了常规的注射器和针头以及自动注射器型装置的许多局限性和缺点。为克服之前装置的缺点和局限性并满足本领域未达成的需求,本发明所公开的装置和方法的实施例包括被构造为使得使用者看不到并且无法接触到针头的装置,从而减少了针头恐惧以及针头污染的可能性。这包括药物递送后对针头自动防护。
装置的实施例具有人类工程学形状因子,允许单手操作并适宜替代位点的注射,例如腿、手臂、或腹部。在包括压敏触发器的实施例中,护针器闩锁抑制针头的运动。这样,装置包括如果没有按压在注射位点上将不允许针头暴露的安全机构。
图1A-1D举例说明了本发明装置的一个实施例,其包括窗口104以在使用前观察药物。在装置已使用后,有色指示标记可显示在窗口上,以向使用者提供是否装置内的药物已用尽的视觉指示。另外,在药物递送后,还提供增加的安全性并减少意外针头穿刺的可能性。
为确保使用者知道药物递送的状态以及是否递送已完成,本发明的此实施例包括棘爪和棘轮(诸如图4和图7所示的棘爪117和棘轮116所描绘的),在注射完成时两者接合产生一声或多声可听到的咔嗒声。这种机构可通知使用者剂量已递送并且装置可从皮肤移除,从而防止装置过早从注射位点撤回。因此,使用者在整个递送过程中积极参与,这与传统的自动注射器不同,对于传统的自动注射器,使用者可能需要等待几秒钟以确保全部剂量已施用。
为向使用者提供更大的反馈,本发明所公开的棘爪和棘轮系统还提供可听到的咔嗒声以及装置在递送过程中的运动以指示注射正在进行。在另一个实施例中,在递送结束时更大声的咔嗒声单独或与视觉指示标记结合提供反馈,以确认递送已完成。
此外,本发明具有友好、悦人的设计和操作方法,这与传统针头安全装置和自动注射器不同,传统针头安全装置和自动注射器令使用者联想起注射器并造成不适感。另外,不同于传统的自动插入器,使用者控制针头的插入和药物的注射,如下文所述。
图1至图9示出了本发明的示例性装置。图1A至1D示出了装置在药物注射之前不同阶段的实施例,并且图2A至2C示出了药物注射过程中和注射后的实施例。图1A示出了处于使用前构型的装置100,其可为使用者所收到的构型。在此松弛状态下,上壳体101部分覆盖下壳体102的近侧部分或最上部分。在描述装置各实施例时,术语“近侧”是相对于装置底部表面使用的。例如,在图1B中,“近侧”是相对于装置100的底部表面或底部131使用的。
如图所示,装置的表面可见的特征包括上壳体101、下壳体102、顶盖103、窗口104、互锁按钮105、夹圈106、上壳体101的底部边缘111、以及剂量指示标记107。图3为本发明此实施例的部件的分解图。
使用装置的预备步骤是移除顶盖103,如图1B所示,该顶盖可移动地附接于下壳体102。移除顶盖103的同时移除针头护罩113,并且暴露出护针器108。窗口104和护针器狭槽109(各自优选地同时出现在装置的两侧)允许使用者观察和检查封装在内的注射器118及其所含药物。
在使用中,通过将手掌置于上壳体101的顶部来抓握住装置,类似于如何抓握落地安装的汽车变速杆。夹圈106为使用者提供如何抓握装置的视觉提示。在一个实施例中,夹圈106被覆盖、或包覆、或由适当的弹性体材料制成,弹性体材料包括(但不限于)氯丁橡胶、氨基甲酸酯、聚氨酯、硅树脂、天然橡胶、热塑性弹性体(“TPE”)、或它们的组合,以提供防滑和舒适的抓握表面。
使用者通过手掌在夹圈106和互锁按钮105上的向下压力,将装置压在身体的期望注射位置,典型地为大腿的上部或侧面、腹部、或上臂的侧面或背面。手掌在互锁按钮105上的压力导致其向下偏转,如图1C所示,继而打开图5所示的护针器闩锁124,允许护针器108向上滑动,并且暴露出针头110(注意,出于举例说明目的,一些装置元件已从图5移除)。护针器闩锁124与上壳体套管120的部分远端一体地形成。上壳体套管120是中空的圆柱体,当装置处于松弛状态时,其部分存在于上壳体101内并且部分存在于下壳体102内。上壳体套管120被固定地附接于上壳体101,并且执行闭锁功能以及将偏置元件119夹在下壳体102上,如下文更详细描述的。
护针器闩锁124包括向内(相对于装置的纵向中心轴线A-A’)倾斜表面127和在其最上端的阻挡件130。为打开护针器闩锁124,与表面127互补的向外倾斜表面128(其形成互锁按钮延伸元件123的远端)接合护针器闩锁124上的倾斜表面127。表面127和128的接合导致护针器闩锁124相对于中心轴线向外偏转,从而移除阻挡件130,使护针器108的向上运动不受阻碍。闭锁机构和护针器108优选地构造成使得除非互锁按钮105被完全按压,否则阻止护针器108的向上运动。这防止了由于接触其他表面而导致的针头的污染和损伤,保护使用者使其免遭意外的针头穿刺,并且将针头屏蔽于视线之外。
当使用者继续在上壳体101上向下按压时,护针器108向上移动,从而暴露出针头110并允许其刺入使用者的皮肤,并且当下壳体102的底部表面131基本上与皮肤齐平的时候停止。一旦护针器108超过阻挡件130,使用者可释放互锁按钮105,或选择不释放,不会影响剩余的注射步骤。当释放互锁按钮105时,回弹部件121使互锁按钮105返回至上部位置。运动导杆132的作用是确保互锁按钮笔直地向上和向下行进。
本文所述的针头插入方法对插入到使用者进行了控制。此特征允许使用者利用经常被胰岛素依赖性糖尿病患者采用的常用方法:如果针头接触到皮肤并且停留在那儿不刺穿皮肤,几秒后使用者将不再感觉到针头的存在,此时针头可通过增加施加于其上的压力无痛的插入。
如图2A至2C所示,在针头110已被插入使用者之后,注射过程通常开始。参考图6,壳体闩锁122(其为下壳体102的部分)以特写细节示出,并且在装置的使用前状态下防止上壳体101相对于下壳体102移动(注意,出于举例说明目的,一些装置部件已从图6移除)。当护针器108已完成其向上的行程时,护针器108上的倾斜表面133接触表面134的倾斜部分(其形成壳体闩锁122的末端),导致壳体闩锁122向内偏转,从而允许上壳体101和上壳体套管120向下移动。
在针头110插入身体后,使用者保持施加在上壳体101上的压力。如图3、4、7和8所示,柱塞杆115推进柱塞112。柱塞杆115固定地连接于上壳体101,并且注射器118固定到或保持在下壳体102内形成的圆柱体内。因此,当上壳体101相对于下壳体102并且在其之上向下移动时,柱塞杆115和柱塞112在注射器118内向下移动,使注射器110内的药物通过针头110递送给患者。
在壳体闩锁122脱离后,围绕上壳体套管120远端的偏置元件119从紧张状态摆脱,通过对上壳体套管120(在其最上端固定地附接于上壳体101)施加向下的力而对上壳体101施加向下的力。偏置元件119还可用于提供辅助柱塞杆115和柱塞112前进的能量,同时使用者提供所需的附加力,进而使药物得以注射,或者偏置元件119提供的能量仅足够推进柱塞杆15和柱塞112。在本发明的另一个实施例中,偏置元件119提供用以注射药物的足够的力,使用者不需要施加附加力,从而提供针头手动插入而药物自动注射的注射装置。偏置元件可为能够对上壳体套管120施加向下力以达到所需程度的任何元件,并且可为(但不限于)弹簧、压缩气体致动器、液压驱动、蜡致动器、电化学致动器、形状记忆合金等等以及它们的组合。在图1至图9描述的实施例中,使用者提供所需的附加力,以通过对上壳体101向下按压,推进柱塞杆115和柱塞112。因此,使用者注射药物所需的力得以减少,其方式类似于汽车内的动力转向装置减少了驾驶员转动方向盘所需的力的方式。与使用者提供了注射所需的力的传统的自动注射器不同,而本发明提供使用者对药物注射速度的控制。
参见图4和图7,分别示出了本发明实施例在药物递送开始前和开始后的剖视图。当药物正被递送时,附接于上壳体套管120上的棘爪117沿着附接于下壳体102上的棘轮116移动。棘爪117和棘轮116可具有至少下列两种功能。首先,防止上壳体101和下壳体102被拉开而分离。其次,棘爪117沿着棘轮116的移动产生柔和的咔嗒响声,从而向使用者提供上壳体101正在移动以及药物正在递送的反馈。另外,如图8所示,在上壳体101的行程结束时,棘爪117可构造为在棘轮116内接合更深的凹槽,从而产生声音更大的咔嗒声(这种声音可为使用者提供行程终点已到达以及药物已完全递送的听觉信号),并且还将上壳体101锁定在原位以防止装置的复位或重新使用。
参见图2B和图8,当药物被完全注射并且上壳体101抵达行程终点时,上壳体101的底部边缘111覆盖剂量指示标记107。剂量指示标记107是位于下壳体102的远侧部分的圆周有色环。这为使用者提供药物递送已完成的视觉提示。
在使用之前,患者可通过窗口104观察药物以检查其澄清度和颗粒。使用后,柱塞112可在窗口104内观察到,表明装置已被使用。作为另外一种选择,窗口可设计为使得注射完成后柱塞杆115也可看到。柱塞112和柱塞杆115可具明亮色彩以向患者提供装置已被使用的清晰指示。
参见图2C和图9,注射完成后,使用者从皮肤移除装置100,并且护针器返回元件114使护针器108延伸覆盖针头110,从而保护使用者和其他人免遭意外针头穿刺。护针器返回元件可为能够使护针器108延伸覆盖针头110的任何元件,包括(但不限于)弹簧、压缩气体致动器、液压驱动、蜡致动器、电化学致动器、形状记忆合金等等以及它们的组合。一旦护针器108完全延伸,护针器锁125接合护针器108内的狭槽,从而防止护针器108回缩。护针器锁125为从上壳体套管120的内表面向内延伸的悬臂闩锁。下壳体肋126(下壳体102的部分)可构造为通过阻塞狭槽防止在递送过程中护针器锁125过早接合护针器108内的狭槽。在本发明另一个实施例中,如果装置100在递送完成之前被移除,护针器108可延伸并且锁定在原位以防止装置的重新使用或共享。
采用本发明提供的辅助递送方法时,使用者在整个递送过程中积极参与。这有别于传统自动注射器的激活过程,后者在按压按钮后,使用者要被动等待几秒钟,等待药物被递送,有时会疑惑注射是否在进行。
本发明的辅助激活方法具有另外的优势,因为使用者通过改变施加于上壳体101的力来控制递送速度,所以该方法减少了为递送不同药物而更改注射装置的相关开发时间和成本。如果柱塞被轻微卡住,使用者可施加稍多一点的力,这不同于传统的自动注射器,传统的自动注射器必须针对最坏情况的力要求进行设计,这个力依据药物、药筒、柱塞、针头、以及机构内的摩擦力而变化。
在另一个实施例中,互锁按钮105和互锁弹簧121可从设计中省略。在此实施例中,上壳体101在碰到阻挡件之前自由的向下移动。此移动用于采用类似于上述的互锁机构来开启护针器108,从而允许护针器108回缩。一旦护针器108完全回缩,其可脱离另一个闩锁,该闩锁允许上壳体101中止向下移动并且以上述类似的方式注射药物。
图10至图18描述了本发明的另一个实施例。图10A示出了具有上壳体205、下壳体202、以及它们之间的中壳体201的装置200。上壳体205包括抓握顶盖228。在松弛状态下,上壳体205部分覆盖中壳体201的近侧部分。中壳体201的最远侧部分固定地安放于下壳体202之内。图10A还示出了上壳体底部边缘211、行进脊216和窗口204。窗口204优选地固定于下壳体202的近侧部分之内。未示出的第二个窗口优选地存在于装置上与窗口204相对的侧面。
顶盖203可拆卸地附接于下壳体202,并且图10B示出了其从装置200上移除以暴露出针头护罩213、针头护罩夹具217和护针器208。在顶盖203移除期间,针头护罩夹具217抓住并同时移除针头护罩213,从而将护针器208暴露给使用者。当装置使用者紧贴皮肤按压护针器208时,此动作导致护针器208向上滑动,从而暴露出针头210,如图10C所示。
图12为装置200的分解图。抓握顶盖228包括将抓握顶盖228固定地紧固于上壳体205上的抓握顶盖装配销230。装配销230与上壳体205中的孔242配合。优选地,装配销230的剖面为具有圆角的正方形,从而在装配销230的角和孔242之间提供过盈表面。导杆233和柱塞杆215与所示的抓握顶盖228的内表面为一体并且从该内表面向下延伸。柱塞杆215在其远端包括阻尼器221。还示出了带有柱塞212的注射器218和针头护罩213。
在一个优选的实施例中,抓握顶盖228的外表面用能够由为使用者提供柔软、防滑握把的材料包覆或由其形成,或整个抓握顶盖228由其形成。包覆或形成抓握顶盖的适当的材料包括(但不限于)弹性体材料,例如氯丁橡胶、氨基甲酸酯、聚氨酯、硅树脂、天然橡胶、TPE等等和它们的组合。
上壳体205包括卡扣闩锁220、手柄肋导向器238和底部边缘211。对于卡扣闩锁220以及装置中采用的其他闩锁,优选地采用至少两个闩锁,并且同样的闩锁相对于彼此对称设置,以有利于装置的平滑移动和操作。
图12示出了带有主体207以及在主体207的近侧部分的外表面上带有手柄导向狭槽239的中壳体201。当装置在使用时,手柄肋导向器238(其为上壳体205的一体部分)与手柄导向狭槽239接合并在其内滑动,以保持药物递送过程中上壳体205的平滑且受控的运动。
主体207可起到剂量指示标记的作用,因为当装置被激活时,上壳体205下降到主体207上。如图11C所示,当全部的药物剂量已递送完时,主体207被上壳体205完全掩盖。优选地主体207具有色彩,更优选地具有明亮的色彩,或具有图案以向使用者提供正在给药或已完成给药的易观察到的视觉反馈。任选地,在主体207上可包括刻度以在视觉上对已递送的药物或有待递送的药物进行定量。
参考图13,中壳体201还包括抓握闩锁224、卡扣闩锁捕获狭槽236、以及护针器闩锁237。抓握闩锁224为大致矩形的元件,其可移动地在其最远侧部分附接于中壳体201的内表面243上,以使得其在施加外力后能够朝着内表面243向外移动。抓握闩锁224还包括止动表面245和三角形阻挡件244,该阻挡件从最顶端部分的一个角朝着装置的中心向内延伸。在装置静止时,使用前状态的抓握闩锁224防止上壳体205相对于中壳体201移动(由于阻挡件245干扰抓握顶盖228的导杆233的向下行进)。
参考图12和图13,所示下壳体202带有下壳体基座206、行进脊216的末端、窗口204、壳体闩锁229、导向狭槽227以及注射器保持器夹片235。顶盖203通过顶盖保持环234可移动地附接于下壳体基座206。在使用中,下壳体基座206接触使用者的皮肤,因而优选地由任何适用于抓握顶盖228的柔韧材料制成。
窗口204在下壳体202中具有开口,以观察注射器218的内容物。窗口204被布置为使得注射器218的底部对使用者可见,以允许使用者验证柱塞212已到达通往注射器底部的行程的终点。窗口204可为任何便利的尺寸和形状,并且优选地为椭圆形,其长轴与装置和注射器的长轴对齐以使得注射器的期望长度暴露于视线之内。
导向狭槽227保持以下三个不同组件的对齐:抓握顶盖228的导杆233;抓握闩锁释放元件231;以及护针器延伸元件241。导向狭槽227通过保持上壳体202和护针器208的对齐和垂直行程以及抓握闩锁231的可靠闭锁和打开,确保装置的平滑激活。向外延伸的壳体闩锁229通过接合中壳体201的内表面243中的凹槽(未示出)将中壳体201紧固到下壳体202上。在装置的非可重复使用的实施例中,闩锁229和凹槽的形状可使得中壳体和下壳体不可分离。对于可重复使用的实施例,凹槽和闩锁被构造为使得中壳体和下壳体能够拉开。
参见图12,护针器208包括护针器狭槽209,其一侧由抓握闩锁释放元件231形成以及另一侧由护针器延伸元件241形成。抓握闩锁释放元件231包括倾斜表面240。参见图14和图15,抓握闩锁释放元件231的倾斜表面240面向外部,并且当抓握闩锁231向上行进时,接合抓握闩锁224的倾斜表面244(其面向内部),导致抓握闩锁224向外偏转,从而去除对导杆233和205的向下移动的阻碍。
当柱塞在其向下行程的终点在注射器上活动时,护针器狭槽209允许窗口204用于观察注射器和柱塞。另外,护针器返回元件214位于抓握闩锁释放元件231和护针器延伸元件241形成的空间内并且在其底部。
装置200的创新方面在于注射器218悬置于装置内的方式。参考图12、图13、和图17,注射器218被保持在针头护罩213和阻尼器221(两者均为柔性组件)之间,以便在装置200掉落或以其他方式错误操作的情况下保护注射器218。当组装装置时,注射器218通过保持器夹片235宽松地保持在下壳体202的腔体246内。根据注射器218内药物的体积,当装置处于使用中时,在阻尼器221接触柱塞212之前,上壳体205可能要行进一段,并且在此初始向下行进期间,阻尼器221作为空气活塞,压缩在柱塞杆215和柱塞212的末端之间形成的空隙内的空气,空气对握把的初始向下运动提供速度依赖性阻力。当阻尼器221快速移动时,空气无法足够快的逸出以减少空气压力的累积。阻尼器221可任选地在其中包括通孔(未示出),以允许空气通过阻尼器221泄漏。作为另外一种选择,要使阻尼器产生摩擦式阻力同时没有压力累积,可使用其中没有泄漏和没有速度依赖的阻尼器,或者可使用它们的组合。在阻尼器221接触柱塞212后,阻尼器221向内朝着柱塞杆215塌缩,以减少在阻尼器221和腔体246内表面之间的摩擦力。
参考图10和图11,当使用者期望使用装置200时,使用者从下壳体202移除顶盖203,该动作同时移除针头护罩213并且暴露出护针器208。使用者通过上壳体205抓握装置200,将手掌置于抓握顶盖228之上,并且当紧贴身体上期望的注射位点保持装置200时,在抓握顶盖228上向下按压,该按压动作使护针器208向上滑动,暴露出针头210。对抓握顶盖228继续施加压力,导致针头210穿透使用者的皮肤和真皮下组织,并且当下壳体基座206接触皮肤表面时或当边缘245到达或护针器208到达下壳体202内其行程终点时针头停止。
参考图15,当护针器208在下壳体202内到达其向上行程的终点时,抓握闩锁释放元件231的倾斜表面240接触中壳体201的抓握闩锁224的相对面向的互补倾斜表面244,导致抓握闩锁224朝着中壳体201的内壁243偏转。此动作移除了抓握闩锁224的止动表面245,避免其干扰抓握顶盖228的导杆233的向下行程,导杆233得以释放并允许上壳体205向下并在中壳体201上移动。
当上壳体205向下移动时,随着抓握顶盖228的柱塞杆215和阻尼器221向下推动注射器柱塞212,注射器218内的药物通过针头210递送。在药物递送结束时,主体207基本上完全被上壳体205所覆盖,并且上壳体205的底部边缘211已与下壳体202的互补形状的行进脊216配合。在窗口204内,柱塞杆215、阻尼器221和柱塞212也清晰可见。所有这些特征向使用者提供药物已递送的视觉确认,并且底部边缘211对着行进脊216的急停向使用者提供触觉确认。
另外,在药物递送结束时卡扣机构被激活以提供听觉反馈。参考图14,当其滑道247接触并滑过中壳体201顶部时,卡扣闩锁220向外偏转。当滑道247向下移动足够远时,滑道247与卡扣闩锁捕获狭槽236对齐,并且滑道247滑入捕获狭槽236内(该狭槽延伸穿过中壳体201近侧部分的壁),并且搭锁在中壳体201的主体207的外表面上,产生咔嗒声。在非可重复使用型装置中,卡扣闩锁220被捕获狭槽236永久捕获并且无法复位。在一个优选的实施例中,两个卡扣闩锁220定位在彼此180度相对的位置,以提供装置的平滑激活并提高咔嗒声响和锁定功能。
当使用者从皮肤移除装置200时,护针器返回元件214(图12中示为弹簧,其通过紧贴使用者皮肤按压装置200而被压缩)扩张,导致护针器208向下延伸覆盖针头210,从而保护使用者免于意外穿刺。除了弹簧以外,护针器返回元件还可为压缩气体致动器、液压驱动、蜡致动器、电化学致动器、形状记忆合金等等以及它们的组合。当护针器208完全伸展时,护针器保持器232接合下壳体202上的阻挡件248(如图13所示),防止护针器208与下壳体202分离。图16示出了护针器闩锁237,其远端可移动地附接于中壳体201的内表面243上。当护针器208向上行进时,护针器闩锁237向外偏转,而与导杆233或护针器延伸元件241的外表面接触。当护针器208向下行进并延伸覆盖针头210时,护针器闩锁237在护针器延伸元件241的顶部上滑动,防止护针器208重新回缩。
在使用前,抓握顶盖228的延伸导杆233将护针器闩锁237保持于向外偏转的位置,从而允许护针器208回缩以便插入针头210。优选地采用两个护针器保持器232和护针器闩锁237,并且围绕装置200的中心轴线相距180度定位。如果装置200在完成药物递送之前从皮肤移除,护针器208将延伸覆盖针头210并将其锁定以防止装置重新使用。在可供选择的可重复使用的实施例中,在完成药物递送之前装置200从皮肤移除的情况下,护针器208延伸,但不将其锁定原位。
图19为装置200可供选择的可重复使用的实施例的绘图,其中上壳体205和中壳体201可与下壳体202分离。在此实施例中,使用者将中壳体和下壳体分离,将注射器218插入下壳体,然后将中壳体和上壳体重新附接。
图20描绘了装置200又一个可供选择的实施例,其中包括辅助驱动219。辅助驱动219可能最适用于递送粘性药物。辅助驱动219在上壳体205和中壳体201之间施加力,从而对上壳体套管120施加向下力。这减少了使用者为注射药物必须对抓握顶盖228施加的向下力的量。辅助驱动219可为弹簧、压缩致动器、液压驱动、蜡致动器、电化学致动器、形状记忆合金等等以及它们的组合。作为另外一种选择,辅助驱动可提供足够的力以注射药物,无需使用者附加的力输入,从而提供其中针头被手动插入而药物以类似于传统的自动注射器的方式自动注射的注射装置。
图21描绘了装置200的下壳体202可供选择的实施例,其中包括用于可重复使用的装置的可复位的卡扣机构。在此实施例中,导向狭槽227接合卡扣222的导杆2225。卡扣装置222被护针器返回元件214偏置。为设置卡扣装置222,使用者紧压一个卡扣导杆225直至卡扣闩锁226延伸覆盖卡扣装置222,防止其上移。当抓握顶盖228向下移动时,在行程终点,导杆233接触卡扣闩锁226上的倾斜表面,导致其向内偏转并且释放卡扣222以在护针器返回元件214的力的作用下向上行进。当卡扣222的卡扣表面223接触下壳体202时,产生咔嗒声响,从而发出信号通知药物已被完全递送。当护针器208在药物注射过程中回缩时,增大了护针器返回元件214的压缩,从而增加了施加于卡扣装置的力和咔嗒声的音量。作为另外一种选择,当使用者在将新的注射器加载到装置中后将上壳体附接于下壳体时,卡扣机构可自动复位。
可设想附图中所不包括的本发明另外的实施例。这包括多剂量设计,其中当使用者按压时,上壳体和中壳体中的一者或两者升至部分高度并且递送部分注射器。

Claims (16)

1.一种用于给药的装置,所述装置包括:
下壳体,所述下壳体被构造为固定地装有注射器,所述注射器具有附接于其上的针头和设置于其中的药物;
柱塞杆;
上壳体,所述上壳体由所述下壳体可移动地支撑,并且被构造为朝所述下壳体移动,以使所述柱塞杆前进到所述注射器中并将所述药物从所述针头排出;以及
偏置元件,所述偏置元件被构造为产生偏置力,所述偏置力偏置所述上壳体朝所述下壳体移动,但不足以使所述上壳体相对于所述下壳体移动,所述偏置力被构造为在沿朝向所述下壳体的方向对所述上壳体施加外力时辅助所述外力,以便使所述上壳体朝所述下壳体移动。
2.根据权利要求1所述的装置,其特征在于,所述偏置元件的所述偏置力根据期望递送的所述药物的粘度来设定。
3.根据权利要求1所述的装置,其特征在于,所述上壳体被构造为沿着第一方向朝所述下壳体移动,并且所述外力沿着所述第一方向被施加到所述上壳体。
4.根据权利要求3所述的装置,其特征在于,所述外力使所述上壳体朝所述下壳体移动。
5.根据权利要求1所述的装置,其特征在于,所述装置限定所述装置沿所述方向的最大高度,并且所述上壳体朝所述下壳体的移动使所述装置的所述最大高度减小。
6.根据权利要求1所述的装置,其特征在于,所述上壳体的近端被暴露,以便接收施加到所述上壳体的所述外力。
7.根据权利要求1所述的装置,其特征在于,所述装置包括固定地附接于所述上壳体的上壳体套管,其中所述上壳体套管将所述偏置元件夹在所述下壳体上。
8.根据权利要求7所述的装置,其特征在于,所述偏置元件围绕所述上壳体套管的远端。
9.根据权利要求7所述的装置,其特征在于,当所述偏置元件从紧张状态摆脱时,所述偏置元件通过对所述上壳体套管施加向下的力而对所述上壳体施加向下的力。
10.根据权利要求1所述的装置,其特征在于,所述偏置元件包括弹簧、压缩气体致动器、液压驱动、蜡致动器、电化学致动器和形状记忆合金中的至少一种。
11.根据权利要求1所述的装置,其特征在于,所述下壳体还包括护针器,其中所述护针器被构造为当所述下壳体的底部表面紧压在皮肤表面上时向上移动,以暴露出针头。
12.根据权利要求11所述的装置,其特征在于,所述护针器还包括护针器返回元件,当所述下壳体的底部表面被从所述皮肤表面移除时,所述护针器返回元件能够使所述护针器延伸在所述针头上方。
13.根据权利要求12所述的装置,其特征在于,所述装置还包括护针器闩锁,在当所述下壳体的底部表面被从所述皮肤表面移除时而所述护针器延伸在所述针头上方之后,所述护针器闩锁防止所述护针器向上移动。
14.根据权利要求11所述的装置,其特征在于,所述上壳体被构造为朝着所述下壳体的底部表面从第一位置移动到第二位置,并且所述装置还包括将所述上壳体锁定在所述第一位置中的闩锁,其中所述护针器的向上移动偏置所述闩锁,以便允许所述上壳体朝所述第二位置移动。
15.根据权利要求1所述的装置,其特征在于,所述上壳体被构造为朝着所述下壳体的底部表面从第一位置移动到第二位置,并且所述装置还包括闩锁部件,所述闩锁部件被构造为在所述上壳体已经从所述第一位置移动到所述第二位置之后将所述上壳体锁定在所述第二位置中,以便防止所述上壳体从所述第二位置朝所述第一位置移动。
16.根据权利要求1所述的装置,其特征在于,所述装置包括附接于所述上壳体和所述下壳体上的中壳体,所述中壳体包括主体,当所述上壳体处于第一位置时,所述主体在所述上壳体与所述下壳体之间暴露出,当所述上壳体处于第二位置时,所述主体基本上被所述上壳体完全地覆盖。
