背景技术
人眼以其最简单的术语解释是通过使光传输通过称为角膜的清澈外部并经由晶状体在视网膜上聚焦成像来提供视力。聚焦成像的质量取决于包括眼的大小和形状,角膜和晶状体的透明度在内的许多因素。
当年龄或疾病使得晶状体变得不那么透明时,视力会由于能够传输到视网膜的光变暗而劣化。眼晶状体内的这一缺陷在医学上被称为白内障。对这一病况的一种被接受的疗法是手术摘除晶状体并由起晶状体作用的人工晶体(IOL)替代。

发明内容

图1例示了带有近端114及相对于尖端柄108弯曲的切割边缘部112的晶状体乳化平衡尖端100。平衡尖端100可以包括基本笔直的柄108以及在切割边缘部112内的至少两个弯曲(第一弯曲102和第二弯曲104)。也可以考虑其他数量的弯曲(例如,3个、4个或7个弯曲等等)。平衡尖端100可以与晶状体乳化机头204结合使用(参见图2)。在结合机头204使用时,平衡尖端100可以纵向振动和/或扭曲振动,即由尖端100围绕其轴来回旋转。弯曲102/104可以例如在距远端沿平衡尖端100长度的约5至25%处定位(这可以是在距尖端远端点106约5mm到距尖端远端点106约12mm之间的位置,上述距离是沿着尖端轴116测得的)。还可以考虑长度的其他部分。

如图6所示,平衡尖端100的超声扭曲振动可以导致至少两种运动:1)从平衡尖端100的均衡位置垂直于与笔直柄部分共线的轴116(轴116可被指示为“z轴”)和垂直于尖端弯曲的轴(在图1中被指示为y轴)的平衡尖端100的侧向位移(y轴和z轴形成包括了所述弯曲的平面);2)沿着平衡尖端100的z轴的扭转角。在扭曲振动的作用下,非平衡尖端会沿尖端长度发生明显的弯曲(特别是在柄内)。通过本文描述的平衡尖端,可以在减小沿着尖端100的柄的侧向位移的同时增加尖端100的远端点106处的侧向位移。扭转振动可以在尖端100内呈现(例如,沿着相对于z轴的扭转角来回扭转),由此导致远端点106的相对较大的侧向位移,作为柄108的侧向位移的附加或在柄108的该侧向位移不存在的情况下。

图8a-b例示了根据以上提供的建模方程的输入尖端形状和沿着该尖端长度的对应位移和扭转角。在某些实施例中,可以基于诸如人机工程学和制造考虑的各种因素选择第一弯曲102的位置和曲率。可将第二弯曲104布置地更靠近平衡尖端100的切割边缘106。随后可以使用以上提供的建模方程的预测来选择该弯曲的曲率。其后可以通过执行有限元分析模拟来验证和/或调整所得的尖端形状。理想曲率可以是使得平衡尖端100的扭转振动模式和弯曲振动模式解耦。平衡尖端100在扭曲力下的运动可以与其扭转振动模式相同。在某些实施例中,可以定位尖端弯曲(例如,102/103)使得平衡尖端100内的超声扭曲振动能量可以处于沿着柄108的主要部分(带有减小的侧向运动)的扭转振动模式内。在某些实施例中,还可以调整柄108的长度以调谐所述扭转振动模式,使得该扭转振动与超声驱动机构(例如,与操纵杆结合的机头内的压电元件227)共振以增加远端点106处的扭转位移。
在某些实施例中,平衡尖端100的远端点侧向位移的幅度可以取决于扭曲驱动力和扭转振动模式之间的共振。虽然驱动频率可由扭曲操纵杆设计来设定,但是扭转模式的频率例如可以通过选择平衡尖端100的圆锥部110的长度来调整。可以选择圆锥部110的长度以最大化平衡尖端100的扭转振动,由此实现远端点106的最大扭转位移。其他尖端特征也可以变化。
在某些实施例中,平衡尖端100可以具有范围在约0.5mm至2mm(例如,1.5mm)之间的直径。在某些实施例中,平衡尖端100可以在尖端顶端处具有约1.5mm的直径,而在尖端的远端附近具有0.9mm的直径(也可以考虑其他的直径和配置)。在一个实施例中,平衡尖端100可以具有约一又八分之三英寸的长度,而弯曲102和103则分别位于距远端约1/8和2/8英寸处。同样可以考虑其他尺寸。在某些实施例中,第一弯曲102可以在约-10至30度的范围内,而第二弯曲可以在约20至50度的范围内。同样可以考虑其他弯曲角。切割边缘部112可以具有喇叭形、锥形和/或斜面形的端部(在某些实施例中,该切割边缘部112可以是平坦的)。平衡尖端100可由不锈钢或钛制成(也可以使用其他材料)。平衡尖端100可以具有在约0.50英寸和1.50英寸之间的总长度(例如,约1.20英寸)。同样可以考虑其他长度。平衡尖端100还可以使用其他常规的金属加工技术制成,并且可以是电抛光的。柄108可以大致呈管状,具有在0.005英寸到0.100英寸之间的外径以及在0.001英寸和0.090英寸之间的内径(也可以考虑其他尺寸)。
图9例示了根据一个实施例的用于确定尖端几何结构的方法的流程图。在流程图中提供的各元素仅是示例性的。提供的各元素可被省略,附加的元素可被添加,和/或可以按下述不同的次序执行各元素。
在901,可将尖端几何结构输入系统。例如,可将几何结构输入存储在输入文件内。在某些实施例中,尖端几何结构可以包括随后定义作为各尖端切片处的几何结构值的一个或多个(例如,尖端可被分割成500个切片并且在每个切片处的尖端几何结构特征可被存储在分配给各自几何结构变量的分开的数组中)。例如,尖端切片的几何结构特征可以包括曲率(例如,以度为单位)、扭曲刚度(例如,C(z))、围绕x轴的惯性力矩(例如,I(z))、横截面积(例如,S(z))、围绕y轴的切片以控制尖端弯曲刚度的惯性力矩(例如,Iy(z))、尖端距z轴的距离(例如,l(z))。同样可以考虑其他输入。

在907,用户(或系统)可以第二尖端几何结构(或修改第一尖端几何结构),并重新计算沿着该尖端长度的侧向位移(ux)和扭转角(φ)。也可以使用其他建模技术。例如,有限元分析(FEA)可被用于确定各种振动下沿着各种几何结构的尖端长度的侧向位移(ux)和扭转角(φ)。此外,还可以使用其他方程(例如,可以为方形尖端建模使用不同的方程)。
在909,可以根据901-907计算不同几何结构的若干尖端的特征,并对其进行比较以选择一个尖端几何结构(或生成新的尖端几何结构用于分析)。选择一个尖端几何结构可以包括基于哪个尖端几何结构在晶状体乳化程序期间于尖端柄被配置为沿着眼内切口的部分的侧向位移更小来选择尖端几何结构。在某些实施例中,如果(被分析尖端几何结构的)在晶状体乳化程序期间于尖端柄被配置为沿着眼内切口的部分(例如,贯穿所述柄从柄的近端(诸如,圆锥部的端部)延伸到切割边缘部的第一弯曲的部分)的侧向位移大于远端点106的侧向位移的约5%-25%(例如15%,也可以使用其他阈值,例如1微米、2微米、100微米、2mm等),则可生成另一个尖端几何结构,可以建模该新的尖端几何结构的侧向位移并将其与第一或第二尖端几何结构的侧向位移中的至少一个相比较以供在所述第一、第二和新几何结构之间的进一步的选择(此时,可以选择一个尖端,也可以生成另一个尖端几何结构用于比较)。
在某些实施例中,生成新的几何结构可以包括修改已在前测试过的几何结构以用于附加的建模。在某些实施例中,用户可以进一步修改选定的尖端几何结构以根据附加规则调谐该几何结构。例如,用户可以修改圆锥部110的长度(或诸如柄长的其他几何结构特征)以增加平衡尖端100的扭转振动,从而通过远端点106的更大的侧向位移。在某些实施例中,用户可以尝试一个或多个弯曲的不同位置和曲率以在增加朝向尖端远侧端的侧向位移的同时降低尖端近侧端的侧向位移。所述修改还可以用于第三、第四尖端等,并且将其结果与在前尖端相比较能够优化对尖端几何结构特征的选择。
图10例示了根据一个实施例的用于确定尖端几何结构的另一方法的流程图。在流程图中提供的各元素仅是示例性的。提供的各元素可被省略,附加的元素可被添加,和/或可以按下述不同的次序执行各元素。
在1001,物理构造或建模具有第一几何结构的第一尖端(例如,使用有限元分析)。在某些实施例中,第一尖端可以具有圆形或方形的横截面,或者是沿着尖端轴变化的横截面。
在1003,第一尖端可以在与晶状体乳化程序类似的条件下振动(例如,通过如图2-3所示的那样固定晶状体乳化机头并以约31kHz的频率振动和/或使用诸如ANSYS的建模软件“振动”)。还可以考虑其他频率(例如,在约10kHz和60kHz之间)。在某些实施例中,将第一尖端固定至要被振动的晶状体乳化机头。在某些实施例中,尖端可被固定至不同的装置(例如,测试夹具)用以施加振动。在某些实施例中,可将第一尖端的端部放于水中或具有类似特征的其他材料中(例如,玻璃体液,也可以使用其他液体)。在某些实施例中,第一尖端可以包括两个弯曲(例如,102和103)。同样可以考虑其他数量的弯曲。
在1005,可以在振动期间分析第一尖端。例如,可以使用热成像、动态镜检查、位移的物理测量结果等来确定尖端的侧向位移(ux)和/或扭转角(φ)(或指示侧向位移(ux)和/或扭转角(φ)的特征)。例如,在振动尖端的热扫描中,沿着尖端长度的更高热的定位可以指示更大的侧向位移(ux)。
在1007,可以构造第二尖端(例如,可以修改第一尖端的几何结构)。可以对以上提供的不同几何特征做出修改。例如,可以修改第二弯曲103的位置和/或曲率。
在1009,第二尖端可以在与第一尖端类似的条件下振动。
在1011,可以在振动期间分析第二尖端以确定与为第一尖端确定的特征相类似的特征(例如,侧向位移和扭转角)。
在1013,可以比较第一尖端和第二尖端的特征,并且可以选择第一和第二尖端几何结构之一或者生成新的尖端几何结构以供比较。例如,可以基于哪个尖端几何结构在晶状体乳化程序期间于尖端柄被配置为沿着眼内切口的部分的侧向位移更小来选择第一尖端几何结构或第二尖端几何结构。在某些实施例中,如果在晶状体乳化程序期间于尖端柄被配置为沿着眼内切口的部分(例如,贯穿所述柄从柄的近端(诸如,圆锥部的端部)延伸到切割边缘部的第一弯曲的部分)的侧向位移大于该尖端的远端点106的位移的约5%-25%(例如,15%)(如上所述,也可以使用其他阈值),则可生成并测试第三尖端。可以确定该第三尖端几何结构的侧向位移和/或扭转角,并将其与第一或第二尖端几何结构的侧向位移和/或扭转角相比较以供在所述第一、第二和第三尖端几何结构之间的进一步的选择(此时,可以选择一个尖端,也可以生成另一个尖端几何结构用于比较)。
图11例示了六个可能的平衡尖端实施例(其他实施例也是可能的)。平衡尖端100可以具有根据图11表格中提供的一组所述参数的几何结构。平衡尖端100可以具有OD英寸的外径;ID英寸的内孔径;从尖端100的毂(尖端100的被配置用于附接至超声操纵杆的近端点)到切割边缘106的L的总长度。尖端100的圆锥部110可以从毂延伸Lc。尖端100的第一弯曲102可以具有α1度的角度并在距远端点106x1和x2英寸的距离的点之间延伸。第二弯曲104可以具有α2度的角度并在距远端点106y1和y2英寸的距离的点之间延伸切割边缘部112可以如表格中所指示的具有面向上30或45度或是面向下30度的斜角(作为一例,图1中示出的斜角是面朝下的)边缘(即,在切割边缘部的最远边缘处)。
在某些实施例中,建模系统可以包括一个或多个处理器。处理器可以包括单个处理设备或者多个处理设备。这类处理设备可以是微处理器、控制器(可以是微控制器)、数字信号处理器、微型计算机、中央处理单元、现场可编程门阵列、可编程逻辑器件、状态机、逻辑电路、控制电路、模拟电路、数字电路、和/或基于操作指令操纵信号(模拟和/或数字)的任何设备。耦合至和/或嵌入处理器的存储器可以是单个存储器设备或是多个存储器设备。这类存储器设备可以是只读存储器、随机存取存储器、易失性存储器、非易失性存储器、静态存储器、动态存储器、闪速存储器、高速缓冲存储器、和/或存储数字信息的任何设备。注意到当处理器经由状态机、模拟电路、数字电路和/或逻辑电路执行其功能的一个或多个时,存储相应操作性指令的存储器可以嵌入或外连至包括所述状态机、模拟电路、数字电路和/或逻辑电路的电路。与关联于附图(例如图9和图10)示例和描述的元素的至少一部分相对应的操作指令可由所述存储器执行,并可由所述存储器执行。
本领域普通技术人员可以多本申请中的实施例做出各种修改。本发明的其他实施例对于考虑了说明书并实现了本发明公开的本领域技术人员而言同样是显而易见的。本说明及其示例旨在被认为仅是由所附权利要求及其等效方案指明的本发明真实范围和精神的说明。

Claims (20)

1.一种晶状体乳化尖端,包括:
具有近端和远端的柄;
位于所述柄的远端处的切割边缘部,其中所述切割边缘部包括至少第一弯曲和第二弯曲;
其中,柄以及所述至少第一弯曲和第二弯曲的几何结构被配置为使得在尖端进行超声扭曲振动期间,整个柄部分的垂直于所述柄的侧向位移小于所述尖端的远端点处的侧向位移的5-25%,其中所述柄部分从柄的近端延伸到切割边缘部的第一弯曲;
其中所述至少第一弯曲和第二弯曲被定位使得超声扭曲振动期间所述柄中的超声扭曲振动能量处于沿着所述柄的主要部分的扭转振动模式;
其中,选择柄长度以调谐所述扭转振动模式,使得所述扭转振动与超声驱动机构共振来增加所述柄的远端点处的扭转位移。
2.如权利要求1所述的晶状体乳化尖端,其中所述侧向位移(ux)至少部分使用方程 u ·· x = 1 ρ S ( z ) d 2 dz 2 ( EI y ( z ) ( d 2 u x dz 2 - φ d 2 l ( z ) dz 2 ) ) 限定,其中:

提供第二尖端几何结构;
建模第二尖端几何结构在所述尖端的超声扭转振动期间的侧向位移;
将第一尖端几何结构的侧向位移与第二尖端几何结构的侧向位移相比较;及
基于以下来选择第一尖端几何结构或第二尖端几何结构:
哪个尖端几何结构:
(a)具有更小的建模的侧向位移;以及
(b)在超声扭曲振动期间所述柄中的超声扭曲振动能量处于沿着所述柄的主要部分的扭转振动模式;
(c)具有这样的柄长度,所述柄长度调谐所述扭转振动模式使得所述扭转振动与超声驱动机构共振来增加所述柄的远端点处的扭转位移。
8.如权利要求7所述的方法,其中第一尖端几何结构至少包括:
距笔直几何结构的侧向位移l(z),
作为尖端材料密度的ρ,

14.一种确定晶状体乳化尖端的几何结构的方法,该晶状体乳化尖端包括柄,该方法包括:
提供带有第一尖端几何结构的第一尖端;
超声扭转振动该第一尖端几何结构;
确定在第一尖端的超声扭转振动期间沿所述尖端几何结构的侧向位移;
提供带有第二尖端几何结构的第二尖端;
超声扭转振动该第二尖端几何结构;
确定在第二尖端的超声扭转振动期间沿所述第二尖端几何结构的侧向位移;
将第一尖端几何结构的侧向位移与第二尖端几何结构的侧向位移相比较;以及
基于如下来选择第一尖端几何结构或第二尖端几何结构:
哪个尖端几何结构:
(a)具有更小的建模的侧向位移;以及
(b)在超声扭曲振动期间所述柄中的超声扭曲振动能量处于沿着所述柄的主要部分的扭转振动模式;
(c)具有这样的柄长度,所述柄长度调谐所述扭转振动模式使得所述扭转振动与超声驱动机构共振来增加所述柄的远端点处的扭转位移。
15.如权利要求14所述的方法,其中选择第一或第二尖端几何结构还包括确定第一尖端几何结构和第二尖端几何结构中的哪一个在从柄的近端延伸到所述柄长度的至少一半的整个柄部分上具有更小的尖端侧向位移。

18.如权利要求14所述的方法,还包括确定所述建模的侧向位移是否小于尖端远端点处的侧向位移的5-25%。
19.如权利要求18所述的方法,还包括如果所述建模的侧向位移大于尖端远端点处的侧向位移的5-25%,则生成第三尖端几何结构,建模该第三尖端几何结构的侧向位移,并将该第三尖端几何结构的侧向位移与第一和第二尖端几何结构的侧向位移中的至少一个侧向位移相比较。
20.如权利要求14所述的方法,其中确定尖端几何结构的侧向运动包括使用热成像、动态镜检查、或位移的物理测量结果中的至少一种。
