Abstract

Description
技术领域
本发明涉及在胃内形成内褶部分的器械及其方法。
背景技术
多年来,肥胖症已经成为一个难题。已经试图用多种方法和装置来治疗患者的肥胖症,包括胃旁路术和植入装置。生物外科手术粘合剂以各种方式用在多种治疗步骤中。在美国专利公开2004/0190975中公开了一种示例性的粘合剂,该文献通过引用结合入本文。尽管已有几种系统和方法被用于解决肥胖症,并且粘合剂已经被用在多种其他的治疗步骤中,但是在本发明人之前没有人设计和使用所附权利要求中描述的发明。
发明内容

如图1所示,示例性的内褶形成系统10包括抽吸装置20和内窥镜式粘合剂施放器40。在本例中,抽吸装置20设置在胃100内部;而粘合剂施放器40设置在胃100外部。如图1至3所示和下面将要更加详细说明的,可对内褶形成系统10进行操作,从而在胃部100中形成多个内褶部分102。形成这样的内褶部分102可以最终减小胃100的容积。
抽吸装置20包括与柔性管24相连的头部22。如图所示,所述头部22具有环形的、向外张开的边缘26,并且头部22与柔性管24之间流体连通。设计边缘26是用来与胃的内表面104密封地接合。如图2所示,在边缘26与胃的内表面104结合的条件下,能够通过柔性管24提供真空,将胃100的一部分吸入头部22,由此形成胃100的一个内褶部分102。应当理解的是,所示的头部22的构造仅仅是示例性的,头部22可选择的具有各种可选形状或构造。
如图1所示,本实施例的粘合剂施放器40的构造适合内窥镜50的工作通道。本实施例的粘合剂施放器40是传统的内窥镜式粘合剂施放器,但是可以使用其他任何适当的可选施放器。操作粘合剂施放器40以将粘合剂60分散至胃100的外表面106(例如浆膜表面)上。粘合剂60可以包含氰基丙烯酸盐、异氰酸酯、或其他任何合适的物质。
如图5所示,在一种示例性应用中,在施加粘合剂60之前,基质布(matrix cloth)70可以被施加到胃100的外表面106上。基质布70可以包括多个单元72,例如可以是蜂巢型构造(仅仅是举例)。每个单元72可以含有能与粘合剂60反应的活化剂物质。尤其是可以对粘合剂60进行设计,使得只有粘合剂60与活化剂物质接触时,才能对粘合剂60进行操作而令人满意地粘合组织。可选择的是,基质布70可以不含有活化剂物质。仅仅是举例,在使用基质布70的范围内,基质布70可以含有聚酯或任何其他合适的一种或多种材料。
在另一种实施例中,基质布70含有粘合剂60,因此不需要单独的施放器40来施加粘合剂60。例如,单元72可以含有粘合剂60。可选择的是,基质布70的材料本身可以是粘合剂,而单元72中不需要具有粘合剂60。在这种变化下,基质布70的部分可以粘合到外表面106和/或基质布70的其他部分。这可以允许相邻外表面部分106通过空单元72提供的空间接触,这会使得该相邻外表面部分106结合。可选择的是,基质布70可以用任何适合的其他替代物替代,也可以具有其他的结构或物质,或将其完全省略。在使用基质布70的范围内,适合的施加基质布70的方法对本领域普通技术人员来说是可以想到的。
在本实施例中,如图2所示,将头部22放到与施加的粘合剂60和基质布72相对的胃部100。当通过柔性管24和头部22进行抽吸以产生胃100的内褶部分102时,粘合剂60和基质布72同内褶部分102一起被吸入。如图2和图6所示,粘合剂60被吸进内褶部分102的内部。可以理解的是,粘合剂60将提供足够的粘合力以保持内褶部分102的构造。在另一种实施方式中,在形成内褶部分102之后再施加粘合剂60。例如,可以将粘合剂施放器40压入到外表面106的由内褶部分102形成的凹陷中。施加粘合剂60直到粘合剂从凹陷中溢出或施加到期望的程度。
在一种变化方式中,如图3所示,可以在靠近每个内褶部分102的根部施加一根或多根缝线80,以进一步帮助保持内褶部分102的构造(和/或为了其他目的)。缝线80可能会阻断每个内褶部分102的血液循环,导致形成每个内褶部分102的组织的死亡。当然,可以使用缝线80的任何合适的替代方式,包括但不限于带、夹具、缝钉等等。可选择的是,根据需要缝线80可以被替代、可以被补充、或完全不用。
在一种实施方式中,如图4所示,内褶部分102可能最终会从胃100脱离(例如,至少一部分形成内褶部分102的组织坏死或死亡后)。此后,这些脱离的内褶部分102会穿过胃肠道。在每个内褶部分102曾处的部位108处,残留的粘合剂60可以防止形成穿过胃100的开口。此外,或可选择的是,疤痕组织或其他组织的融合可以阻止形成这样的开口。最终,在本例中,尽管内褶部分102从各自的部位108脱离,胃100仍能保持结构的完整性。可理解的是,内褶部分102的脱离会导致能产生胃酸的面积的减少。在内褶形成系统10用以治疗肥胖症的范围内,能产生胃酸的面积的减少,会进一步的使营养吸收受到阻碍。这种阻碍营养吸收的效果是由胃容积减少达到的,胃容积的减少是由用以治疗肥胖症的形成内褶的方法实现的。
图7至9描绘了一种可以替代抽吸装置20的抓持器械120。如图所示,抓持器械120具有轴122和一对抓持用钳口构件124。轴122是大致柔性的,能够通过患者的食道例如以内窥镜等方式将该轴导入到患者的胃内。如下将进行的更详细描述,轴122进一步具有腔(没有显示出),通过该腔可以提供真空。
抓持用钳口构件124与轴122通过枢轴连接,因此,抓持用钳口构件124可以选择性地具有张开或关闭位置。每个抓持用钳口构件124都具有多个齿126。设计齿126用以夹住组织,例如胃的内表面104。可以通过缆索(没有显示出)操作抓持用钳口构件124的张开和关闭。本例中缆索穿过轴122延伸连到使用者的输入装置中(没有显示出)。当然,任何合适的缆索替代方式都可以用来张开和/或关闭抓持用钳口构件124。
本例中的抓持器械120进一步具有真空端口140。真空端口140可以具有类似抽吸装置20上的头部22的结构。在本例中,真空端口140的直径小于抽吸装置20上的头部22的直径。真空端口140与轴122的真空腔流体连通。真空端口140用于例如在抓持用钳口构件124用于夹住胃内表面104之前相对于抓持用钳口构件124固定胃的内表面104。
在使用中,基质布70和粘合剂60被施加到胃外表面106。抓持器械120被放置在紧邻胃内表面104处,其位置基本上与基质布70和粘合剂60相对。在抓持用钳口构件124处于打开位置时,通过真空端口140施加真空,确保胃内表面104相对于抓持用钳口构件124固定,如图7所示。然后将抓持用钳口构件124致动到基本关闭的位置,用齿126夹住胃内表面104,如图8至9所示。由此,在胃100内产生了内褶部分202。在本例中,内褶部分202与内褶部分102相似,只是内褶部分202在一个尺寸上更宽。因此,可以理解,与形成内褶部分102相比,所产生的内褶部分202能更大程度地减少胃100容积。也可以理解,可以进一步用一根或多根缝线、带、夹具、缝钉等等对内褶部分202的根部进行固定。
图10至12描绘了抓持器械120的示例性变化。在这种变型中,抓持器械220具有轴222和一对抓持用钳口构件224。轴222是大致柔性的,并且能够用于通过患者的食道以例如内窥镜等方式将该轴导入到患者的胃100内。如下将进行的更详细描述,轴222进一步具有腔250(没有显示出),通过该腔可以提供真空。
抓持用钳口构件224与轴222通过枢轴连接,因此,抓持用钳口构件224可以选择性地具有张开或关闭位置。每个抓持用钳口构件224都具有多个齿226。设计齿226用以夹住组织,例如胃内表面104。可以通过缆索230操作抓持用钳口构件224的张开和关闭。本例中缆索230穿过轴222延伸连到使用者的输入装置中(没有显示出)。当然,任何合适的缆索230替代方式都可以用来张开和/或关闭抓持用钳口构件224。
每个抓持用钳口构件224具有大致球形的关节部分228。关节部分228的构造如下,当抓持用钳口构件224处在张开位置时,关节部分228在轴222内互相重叠。每个抓持用钳口构件224还分别具有杯状结构,杯状结构被构造成提供真空室240。所述真空室240与腔250流体连通,由此,可以通过腔250在真空室240内提供真空。在另一变化中,可以在每个真空室240上分别设置具有多个孔的盖子(没有显示出)。可选择的是,抓持用钳口构件224可使用任何其他结构变型。
尽管腔250一般被描绘成轴222的内部,但是可以理解,可用插入轴222的单独元件(未示出)可选择地提供腔250。对于本领域技术人员来说,其他的变化也将是可以想到的。
在一种示例性应用中,抓持器械220的使用方法与上述抓持器械120的用法相似。然而,与通过真空端口140提供抽吸力不同的是,本例中的抽吸力是通过抓持用钳口构件224的真空室240提供。本例中的抽吸力可以在齿226夹紧胃内表面104前或夹紧的同时提供,并且协助齿226夹紧胃内表面104。在抓持用钳口构件224进入关闭位置的过程中,可以持续提供抽吸力,于是形成了内褶部分202。

本发明的具体实施方式应用在传统的内窥镜和开放式外科手术仪器中,同时也应用在机器人辅助外科手术中。
本文公开的实施装置可以被设计成一次使用后就处理掉,或设计成可多次使用的。在以上两种情况下,都可以在至少一次使用后将装置修复后重新使用。修复可以包括所述装置的拆卸步骤的任意组合,然后清洗或者更换特定零件,和随后重新组装。特别是,将实施装置拆卸,可任意组合地将装置的任一特定零件或部分选择性更换或去除。在清洁和/或换掉特定部份后,为了随后的使用,可以在修复装置中重新组装实施装置,或者在即将进行外科手术前由外科手术小组成员组装。本领域技术人员可以理解,装置的修复可能使用各种各样的技术进行拆卸、清洁/或更换、和组装,而这些技术及其修复装置都在本申请的范围内。
仅作为举例的方式,本处描述的具体方式可以在外科手术前进行。首先,获得新的或使用过的仪器,如果需要可以进行清洁。然后可以将仪器消毒。在一种消毒技术中,将仪器放到封闭的容器中,例如塑料袋或TYVEK袋。将容器和仪器放入射线场内,该射线能穿透容器,例如伽马射线、X射线、或高能电子。射线可以杀死容器中的和仪器上的细菌。然后将消毒后的仪器储存在无菌容器中。封闭的容器可以保持仪器无菌,直到在医药室中打开该容器。可以使用本领域任何其他已知的技术对装置消毒,包括但不限于β或伽马射线、环氧乙烷或蒸汽。

Claims (10)
1.一种在胃内形成内褶部分的系统,该系统包括:
(a)抽吸装置,其中该抽吸装置包括:
(i)能够提供抽吸力的抽吸头部,
(ii)轴,所述抽吸头部与轴相连,和
(iii)真空腔,该真空腔与抽吸头部流体连通,其中抽吸装置的结构能够使得抽吸头部靠近组织放置,其中所述抽吸头部和真空腔能够在靠近组织处产生真空,其中所述抽吸装置还能够使组织产生内褶部分;和
(b)固定构件,其中,所述固定构件能够基本上保持组织内褶部分的构造。
2.如权利要求1所述的系统,其中,所述抽吸头部具有环形的向外张开的边缘。
3.如权利要求1所述的系统,其中,所述抽吸头部还具有多个抓持用钳口构件,所述抓持用钳口构件能够处于张开或关闭位置。
4.如权利要求3所述的系统,其中,所述抽吸头部还具有位于抓持用钳口构件附近的真空端口。
5.如权利要求3所述的系统,其中,每个抓持用钳口构件分别具有真空室。
6.如权利要求3所述的系统,其中,每个抓持用钳口构件分别具有一个大致球形的关节部分,其中,当抓持用钳口构件处在张开位置时,关节部分至少部分重叠。
7.如权利要求1所述的系统,其中,所述固定构件含有粘合剂。
8.如权利要求7所述的系统,其中,所述粘合剂包含氰基丙烯酸盐、异氰酸酯中的一种或两种。
9.如权利要求7所述的系统,其中,所述固定构件还包括活化剂,用于粘合剂的活化。
10.如权利要求7所述的系统,其中,所述固定构件还包括基质,其中基质内含有活化剂。
