Description
Foreliggende oppfinnelse vedrører anvendelse av karboksamidiner for fremstilling av medikament for behandling av sykdom.
Ribavirin (1-(J-D-ribofuranosyl-1,2,4-triazol-karboksamid) er en nukleosid-analog som har vist effektivitet med hensyn til å behandle virussykdommer både som monoterapi (respiratorisk syncytial-virus, Hall, C.B.; McBridge, J.T.; Walsh, E.E.; Bell, D.M.; Gala, CL; Hildreth, S.; Teten Eyck, L.G.; W.J. Hall. Aerosolisert ribavirin-behandling av spedbarn med respiratorisk syncytial-virusinfeksjon. N. Engl. J. Med. 1983, 308, 1443-1447), og i kombinasjonsterapi med interferon-a (hepatitt C-virus, Reichard, O.; Norkrans, G.; Fryden, A.; Branconier, J-H.; Sonnerborg, A.; Weiland, O. Nylig rapportering studier indikerer at den in vivo nytte av ribavirin kan resultere ikke bare fra direkte inhibering av viral replikasjon, men også fra dets evne til å forsterke T-celle-mediert immunitet (Hultgren, C; Milich, D.R.; Weiland, O.; Sållberg, M. Induksjonen av en Type 1-cytokin-bias ved ribavirin, er funksjonelt signifikant in vivo i murine systemer (Tam, RC; Lim, C; Bard, J.; Pai, B. Kontakt-hypersensitivitetsresponser etter ribavirin- behandling in vivo blir influert av Type 1-cytokinpolarisasjon, regulering av IL-10-ekspresjon og kostimulerende signalisering. J. Pattedyr-immune systemer inneholder to hovedklasser av lymfocytter: B-lymfocytter (B-celler), som har sitt opphav i benmargen; og T-lymfocytter (T-celler) som har sitt opphav i thymus. B-celler er stort sett ansvarlige for humoral immunitet (det vil si antistoffproduksjon), mens T-celler stort sett er ansvarlige for celle-mediert immunitet.
T-celler anses generelt å falle innen to underklasser, hjelper-T-celler og cytotoksiske T-celler. Hjelper-T-celler aktiverer andre lymfocytter, inklusive B-celler og cytotoksiske T-celler, og makrofager, ved å frigi løselige protein-mediatorer kalt cytokiner som er involvert i cellemediert immunitet. Som brukt her, er lymfokiner et undersett av cytokiner.
Hjelper-T-celler er også generelt antatt å falle innen to underklasser, Type 1 og Type 2. Type 1-celler produserer interleukin 2 (IL-2), tumor-nekrosefaktor (TNFa) og interferon-y (IFNy), og er ansvarlige primært for celle-mediert immunitet slik som forsinket-type-hypersensitivitet og antiviral immunitet. I motsetning til dette, produserer Type 2-celler interleukiner, IL4, IL-5, IL-6, IL-9, IL10 og IL-13, og er primært involvert i å assistere humorale immunresponser slik som dem som sees i respons til allergener, for eksempel IgE og lgG4-antistoff-isotype-dreining (Mosmann, 1989, Annu Rev Immunol, 7:145-173).
Som nevnt her, menes uttrykkene Type 1- og Type 2-"responser" å inkludere hele området av effekter som resulterer fra induksjon av Type 1- og Type 2-lymfocytter, respektive. Blant annet, inkluderer slike responser variasjon i produksjon av de tilsvarende cytokiner via transkripsjon, translasjon, sekresjon og muligens andre mekanismer, øket proliferasjon av de tilsvarende lymfocytter og andre effekter assosiert med øket produksjon av cytokiner, inklusive motilitets-effekter.
Tidligere søknader (09/426 714, 09/291 097, 09/291 093, 09/471 513, 60/164 365, 60/164 3656, 60/172, 097, 60/175 111), vedrører aspekter av våre nylig foretatte oppdagelser som involverer effekten av diverse nukleosider (som her er definert å inkludere derivater og analoger av native nukleosider) på selektivt modulerende lymfocyttresponser relativt til hverandre. Blant annet har vi vist at enhver av Type 1- og Type 2-responser kan bli selektivt undertrykket, mens den annen enten blir indusert eller blir relativt upåvirket, og hver av Type 1- eller Type 2-responser kan bli selektivt indusert, mens den annen enten blir undertrykket eller er relativt upåvirket. Vi har også oppdaget det overraskende faktum at noen nukleosider som er effektive med hensyn til selektivt å modulere Type 1 - og Type 2-responser relativt til hverandre, har tendens til å ha en bimodal effekt. Blant annet har noen nukleosider som har tendens til generelt å undertrykke eller indusere både Type 1- og Type 2-aktivitet ved en relativt høy dose, tendens til selektivt å modulere Type 1 og Type 2 relativt til hverandre ved relativt lave doser.
Viramidin™ (1-p-D-ribofuranosyl-1,2,4-triazol-3-karboksamidinhydroklorid) har vist seg aktiv i ti forskjellige vimser som er sammenlignbare med ribavirin.

Robins, J.W. Seegmiller, Molecular Pharmacology, 18, 287-295, 1980). Videre antyder preliminære toksikologiske studier at Viramidin™ er mindre toksisk enn ribavirin (D.Y. Pifat, R.W. Sidwell, P.G. Canonico, Antiviral Research, 9, 136, 1988). Dessuten har nylige studier ved vårt laboratorium (R. Tam, K. Ramasamy, ICN Pharmaceuticals, Inc., upubliserte resultater, 1999) avslørt at Viramidin™ og rabavirin oppviste lignende immunomodulatoriske egenskaper. Disse resultater koblet med lav biotilgjengelighet og toksisitet assosiert med ribavirin påskynder oss ikke bare til å utvikle Viramidin™ for andre virussykdommer, men også å fremstille andre derivater av Viramidin™ inklusive syntesen av prodroger for Viramidin™ og å klassifisere dem som potensielle anti-vi rusmidler.
Effekten av andre nukleosid-analogforbindelser på selektivt modulerende lymfocyttresponser relativt til hverandre er tidligere ikke blitt studert eller dokumentert. Vi har oppdaget at den bimodale effekt, eller selektiv modulasjon av Type 1- og Type 2-responser relativt til hverandre også inntreffer etter administrering av andre nukleosid-analogforbindelser, slik som prodrogeformer av forbindelsene.
Det er mange barrierer å overvinne med hensyn til å utvikle biologisk aktive forbindelser til klinisk nyttige midler. Mange potente biologisk aktive forbindelser blir aldri klinisk nyttige midler på grunn av sine uønskede biofarmasøytiske egenskaper som inkluderer lav biotilgjengelighet på grunn av lav permeabilitet gjennom biologisk barrierer, slik som blodhjernebarrieren (BBB) og tarmbarrieren. Selv om mange faktorer påvirker biotilgjengeligheten av en droge, er de uønskede fysikokjemiske egenskaper (for eksempel ladning, lipofilisitet, hydrogenbindingspotensiale, størrelse) av mange droger sannsynligvis én av de vanligste faktorer å treffe på som hindrer gjennomtregning av droger gjennom biologiske barrierer. Derfor er optimalisering av de fysikokjemiske karakteristikker (ladning, lipofilisitet, hydrogenbindingspotensiale, størrelse) av en droge sannsynligvis den mest sannsynlige generelle strategi å forenkle transporten av droger gjennom slike membranbarrierer.
For å optimalisere de fysikokjemiske egenskaper ved droger, er én mulig strategi den med prodroger (H. Bundgaard, Design of Produgs, Elsevier, Amsterdam, 1985; N. Bodor, L, Prokai, W.M. Wu, H. Farag, S. Jonalagadda, M. Kawamura, J. Simpkins, Science, 257,1698-1700,1992; H.E. Taylor, K.B. Sloan, J. Pharm. Scij., 87, 5-20,1998). Betegnelsen prodroge anvendes for å beskrive et middel som må gjennomgå kjemisk eller enzymatisk transformasjon til den aktive eller opphavsdrogen etter administrering, slik at det metaboliske produkt eller opphavsdrogen etterpå kan oppvise den ønskede farmakologiske respons. Ved derivatisering av visse polare funksjonelle grupper i små organiske molekyler ubestandig og bioreversibelt, er de uønskede fysikokjemiske karakteristikker (for eksempel ladning, hydrogenbindingspotensiale) hos disse grupper blitt "maskert" uten permanent endring av de farmakologiske egenskaper ved molekylene. Denne strategi er blitt anvendt med stort hell i slike tilfeller hvor prodroge-derivatiseringen involverer omdannelse av en karboksyl- eller en hydroksyl-funksjonell gruppe til en ester, som lett kan bli hydrolysert in vivo enten kjemisk eller enzymatisk. Det lovende prodrogekonsept gjør at vi forventer at innføring av andre grupperinger i opphavsdrogen ville øke biotilgjengeligheten, adsorpsjonen og de antivirale effekter.
Til tross for eksistensen av hittil udefinerte mekanismer, har vi oppdaget at enorme potensielle goder kan stamme fra selektiv modulering av Type 1- og Type 2-responser relativt til hverandre. Vi har for eksempel konkludert med at spesifikk modulasjon av Type 1 i forhold til Type 2 kan være nyttig ved behandling av et stort utvalg av tilstander og sykdommer, varierende fra infeksjoner, plager, tumorer og hypersensitiviteter til autoimmune sykdommer.
Disse oppdagelser er spesielt signifikante fordi moderne behandlings-strategier for mange av de ovenfor opplistede sykdommer har begrenset effektivitet, signifikante bivirkninger, eller begge deler. Behandling av autoimmun sykdom, for eksempel, er hyppig begrenset til palliative forholdsregler, fjerning av toksiske antistoffer (som i myasthenia gravis) og administrering av risikofylte droger inklusive kortikosteroider, klorokinderivater og antimetaboliske eller antitumordroger, samt slike droger som cyklosporiner som er rettet mot immunsystemceller.
Foreliggende oppfinnelse er rettet mot anvendelse av karboksamidiner for fremstilling av medikament for behandling av sykdom.
I ett aspekt av oppfinnelsen, tilveiebringes anvendelse av karboksamidin som angitt i formel 1, eller et farmasøytisk akseptabelt salt derav for fremstilling av et medikament for behandling av en hepatitt C-virus (HCV) viral infeksjon eller en hepatitt B-virus (HBV) viral infeksjon,
hvor forbindelsen er i D-konfigurasjon.

Betegnelsene "nukleosid" og "nucleosidanalogforbindelse" kan brukes om hverandre og refererer til en forbindelse sammensatt av enhver pentose- eller modifisert pentoseandel festet til en spesifikk posisjon i en heterocyklus, aromatisk heterocyklus eller til den naturlige posisjon i et purin (9-posisjon) eller pyrimidin (1-posisjon) eller til den ekvivalente posisjon i en analog.
Betegnelsen "nukleotid" refererer til en fosfatester substituert på 5'-posisjonen i et nukleosid.
Betegnelsen "heterocyklus" refererer til et enverdig mettet eller umettet karbocyklisk radikal som minst ett heteroatom, slik som N, O eller S, i ringen hver tilgjengelig posisjon eventuelt substituert, uavhengig, med for eksempel hydroksy, okso, amino, imino, lavere alkyl, brom, klor og/eller cyano. Inkludert innen denne klasse av substituenter er puriner, pyrimidiner.
Betegnelsen "purin" refererer til nitrogenholdige bicykliske heterocykluser.
Betegnelsen "pyrimidin" refererer til nitrogenholdige monocykliske heterocykluser.
Betegnelsen "D-nukleosider" refererer til de nukleosidforbindelser som har en D-ribose-sukkerandel (for eksempel adenosin).
Betegnelsen "L-nukleosider" refererer til de nukleosidforbindelser som har en L-ribose-sukkerandel.
Betegnelsene "L-konfigurasjon" og "D-konfigurasjon" anvendes i forbindelse med hele foreliggende oppfinnelse for å beskrive den kjemiske konfigurasjon av ribofuranosylandelen i forbindelsene som er forbundet med den pyrrolo-pyrimidon-andel av molekylet.
Betegnelsen "C-nukleosider" anvendes gjennom hele beskrivelsen for å beskrive den bindingstype som er dannet mellom ribose-sukkerandelen og den heterocykliske base. I C-nukleosider stammer bindingen fra C-1-posisjonen i ribose-sukkerandelen og knytter seg til karbonet i den heterocykliske base. Den binding som dannes i C-nukleosider er av typen karbon-til-karbon.
Betegnelsen "N-nukleosider" anvendes gjennom hele beskrivelsen for å beskrive den bindingstype som dannes mellom ribose-sukkerandelen og den heterocykliske base. I N-nukleosider stammer bindingen fra C-1-posisjonen i ribose-sukkerandelen og knytter seg til nitrogenet i den heterocykliske base. Den binding som danner seg i N-nukleosider er av typen karbon-til-nitrogen.
Betegnelsen "beskyttende gruppe" refererer til en kjemisk gruppe som adderes til, oksygen eller nitrogenatom for å forhindre dets ytterligere reaksjon under forløpet av derivatisering av andre andeler i molekylet hvor oksygenet eller nitrogenet er beliggende. Et stort utvalg av oksygen- og nitrogenbeskyttende grupper er kjent for fagmannen på området organisk syntese.
Betegnelsen "lavere alkyl" refererer til metyl, etyl, n-propyl, isoproyl, n-butyl, t-butyl, i-butyl eller n-heksyl. Denne betegnelse er ytterligere eksemplifisert som en cyklisk, forgrenet eller rett kjede med én til seks karbonatomer.
Betegnelsen "aryl" refererer til et enverdig umettet aromatisk karbocyklisk radikal som en enkelt ring (for eksempel fenyl) eller to kondenserte ringer (for eksempel naftyl), som eventuelt kan være substituert med hydroksyl, lavere alkyl, klor og/eller cyano.
Betegnelsen "heterocyklus" refererer til et enverdig mettet eller umettet karbocyklisk radikal som har minst ett heteroatom, så som n, O, S Se eller P, i ringen, hvor hver tilgjengelige posisjon av disse eventuelt kan være substituert eller usubstituert, uavhengig, med hydroksy, okso, amino, imino, lavere alkyl, brom, klor og/eller cyano.

Betegnelsen "effektiv mengde" refererer til mengden av en forbindelse av formel (1) som vil gjeninnsette immunfunksjon til normale nivåer, eller øke immunfunksjon over normale nivåer for å eliminere infeksjon.
Forbindelsene av formel 1 kan ha flere asymmetriske sentre. Følgelig kan de bli fremstilt enten i optisk aktiv form eller som en racemisk blanding. Omfanget av oppfinnelsen som beskrevet og krevet beskyttelse for, inkluderer anvendelse av alle de individuelle optiske isomerer og ikke-racemiske blandinger derav, så vel som de racemiske former av forbindelsene av formel 1.
Betegnelsene "a" og "p" indikerer den spesifikke stereokjemiske konfigurasjon av en substituent ved et asymmetrisk karbonatom i en kjemisk struktur som opptegnet.
Betegnelsen "enantiomerer" refererer til et par av stereoisomerer som er ikke-dekkende speilbilder av hverandre. En blanding av et par av enantiomerer, i forhold 1:1, er en "racemisk" blanding.
Betegnelsen "isomerer" refererer til forskjellige forbindelser som har den samme formel. "Stereoisomerer" er isomerer som avviker bare på den måte at atomene er arrangert i rommet.
"Farmasøytisk akseptable salter" kan være hvilke som helst salter som stammer fra uorganiske og organiske syrer eller baser.
En eksempelsyntese av inkluderte forbindelser (her: Viramidin™) kan følge en metode som er beskrevet nedenunder og vist i figur 1.
3-cyano-1 -(2,3,5-tri-0-acetyl-p-D-ribofuranosyl)-1,2,4-triazol (7):
en blanding av 3-cyano-1,2,4-triazol (18,8 g, 200 mmol) (6), 1,2,3,5-tetra-O-acetyl-p-D-ribofuranose (63,66 g, 200 mmol) og bis(p-nitrofenyl)fosfat (1 g) ble anbragt i en rundbunnet 500 ml kolbe. Kolben ble anbragt i et forhåndsoppvarmet oljebad ved 165-175°C under vann-aspirator-vakuum med omrøring i 25 minutter. Eddiksyren som ble forskjøvet, ble oppsamlet i en iskald felle som er anbragt mellom aspiratoren og den rundbunnede kolbe. Kolben ble tatt fekk fra oljebadet og fikk avkjøle seg. Da temperaturen i kolben nådde grovt sett 60-70<*>C, ble EtOAc (300 ml) og mettet NaHC03(150 ml) innført og ekstrahert i EtOAc. Det vandige sjikt ble ekstrahert igjen ved 200 ml EtOAc. Den kombinerte EtOAc-ekstrakt ble vasket med 300 ml mettet NaHC03, 200 ml vann og 150 ml saltløsning. Den organiske ekstrakt ble tørket over vannfritt Na2S04, filtrert og filtratet inndampet til tørrhet. Resten ble oppløst i 100 ml eter som ved avkjøling ved 0°C i 12 timer, ga fargeløse krystaller. Faststoffet ble filtrert, vasket med minst mulig kald EtOH (20 ml) og tørket ved høyt vakuum over fast NaOH. Utbytte: 56,4 g (80%), smp. 96-97°C.<1>HMR (CDCI3) 8: 2,11 (s, 3H, COCH3), 2,13 (S, 3H, COCH3), 2,14 (s, 3H, COCH3), 4,22 (dd, 1H), 4,46 (m, 2H), 5,52 (t, 1H, J=6,0 Hz), 5,70 (m, 1h), 6,01 (d, 1H, C1H, J=3,6 Hz) og 8,39 (s, 1H, C5H). Analyse beregnet for C14H16N407(352,30): C, 47,73; H, 4,58; N, 15,90. Funnet: C, 47,70; H, 4,63; N, 16,01.
1 -p-D-ribofuranosyl-1,2,4-triazol-3-karboksamidin (Viramidin™) hydroklorid (8): En blanding av (7) (14,08 g, 40,0 mmol), NH4CI (2,14 g, 40,0 mmol) og 150 ml vannfri ammoniakk ble oppvarmet i en stålbombe ved 85"C i 18 timer. Stålbomben ble avkjølt, åpnet og innholdet inndampet til tørrhet. Inn-dampningsresten ble krystallisert utfra MeCN-EtOH slik at man fikk 10,6 g (95%) av 8, smp. 177-179°C.<1>HMR (DMS-d6) 8 3,44-4,2 (m, 3H), 4,40 (m, 2H), 5,04 (t, 1H), 5,29 (m, 1H), 5,74 (m, 1H), 5,87 (d, 1H, dH), 8,96 (bs, 3H)og9,17(s, 1H, C5H). Analyse beregnet for C8H14CIN5O4(279,68); C, 34,35; H, 5,05; N, 25,04; Cl, 12,69. Funnet: C, 34,39; H, 5,10; N, 25,14; Cl, 12,71.
Alternativt kan syntesen foregå ut fra kommersielt tilgjengelig Ribavirin™ som følger: 2\3',5'-tri-0-acetyl-1 -p-D-ribofuranosyl-1,2,4-triazol-3-karboksamid (9): En suspensjon av 1-p-D-ribofuranosyl-1,2,4-triazol-3-karboksamid (Ribavirin™)
(28,4 g, 116,4 mmol) (5) i 200 ml eddiksyreanhydrid og 50 ml pyridin ble omrørt ved romtemperatur natten over. Den resulterende klare løsning ble konsentrert i vakuum og ga et klart skum (43,1 g, kvantitativt). Dette skum var homogent på TLC og ble anvendt direkte for neste trinn uten rensing. En liten mengde ble renset ved flash-kromatografi og ga da en analyseprøve;<1>HMR (300 MHz), DMSO-de) 8 2,01, 2,08, 2,09 (3s, 9H, COCH3), 4,10 (m, 1H), 3,52 (m, 2h), 5,58 (t,
1H), 5,66 (m, 1H), 6,33 (d, 1H, J=3,0 Hz, CiH), 7,73 7,92 (2s, 2H, CONH2), 8,86 (s, 1H, C5H trizol). Analyse (CioH^Oe) C, H, N.
3-cyano-2',3',5'-tri-0-acetyl-1-p-D-ribofuranosyl-1,2,4-triazol (10): Til en løsning av 9 (43,1 g, 116,4 mmol) i 500 ml kloroform ble det tilsatt 244 ml trietyl-amin, og blandingen ble avkjølt til 0°C i et is/saltbad. Fosforoksyklorid (30,7 ml, 330 mmol) ble tilsatt dråpevis med omrøring, og løsningen fikk oppvarme seg til romtemperatur. Etter at blandingen var omrørt ved romtemperatur i 1 time, indikerte TLC (heksan/aceton 3:1) fullstendig forsvinning av utgangsmateriale. Den brune reaksjonsblanding ble konsentrert til tørrhet i vakuum, og resten ble oppløst i 500 ml kloroform. Denne organiske løsning ble vasket med mettet vandig natriumbikarbonat (3 x 200 ml), tørket over vannfritt natriumsulfat og konsentrert i vakuum. Resten ble kromatografert over silisiumdioksydgel (flash-kromatografi) med 20% aceton i heksan og ga da 33,14 g (81% fra ribavirin) av ren 10 som et amorft faststoff. Dette faststoff var i alle henseender identisk med en autentisk prøve: smp. 101-103°C; IR (kalsiumbromid) v 2250 (CN), 1750 (C=0), cm"<1>,<1>HMR (300 MHz, CDCI3) 5 2,04, 2,06, 2,07 (3s, 9H, acetylmetyler), 4,15 (dd, 1H), 4,40 (m, 1H), 5,47 (t, 1H), 5,63 (dd, 1H), 5,95 (d, 1H, J=3,2 Hz, CiH), 8,34 (s, 1H, C5H triazol).
1 -p-D-ribofuranosyl-1,2,4-triazol-3-karboksamidinhydroklorid (8):
Til en suspensjon av 10 (4,0 g, 11,4 mmol) i 100 ml metanol ble det tilsatt 12 ml av en molar løsning av metanolisk natrium-metoksyd, og blandingen ble omrørt ved romtemperatur natten over. Løsningen ble surgjort til pH 4 med metanol-vasket Dowex H+-harpiks, harpiksen ble filtrert, og filtratet konsentrert til tørrhet i vakuum. Resten ble oppløst i en minimal mengde av metanol (15 ml) og overført til en trykkflaske. Ammoniumklorid (0,61 g, 11,4 mmol) og en løsning av metanol mettet ved 0°C med tørr ammoniakkgass (75 ml), ble tilsatt, flasken ble forseglet, og løsningen omrørt ved romtemperatur natten over. Løsningen ble konsentrert til tørrhet i vakuum, og det resulterende restmateriale krystallisert ut fra acetonitril/- etanol og ga da 8 som et krystallinsk fast stoff (2,95 g, 93%). Denne prøve var identisk med en autentisk prøve i alle henseender.
I visse farmasøytiske doseringsformer, foretrekkes prodrogeformen av forbindelsene, spesielt inklusive acylerte (acetylerte eller andre) derivater, pyridin-estere og diverse saltformer, og kan administreres i en metode for behandling av en tilstand hos en pasient. Fagmannen på området vil erkjenne hvordan han lett skal modifisere de her beskrevne forbindelser til prodrogeformer for å forenkle utlevering av aktive forbindelser til et målsted i vertsorganismen eller pasienten. Fagmannen på området vil også trekke fordel av gunstige farmakokinetiske parametere av prodrogeformene, der hvor det er gjørlig, ved levering av de her beskrevne forbindelser til et målpunkt i vertsorganismen eller pasienten for å maksimalisere den tilsiktede effekt av forbindelsen.

For spesifikk utlevering av droger til leveren og gallesystemet, er det endogene gallesyre-transportsystem en attraktiv kandidat. Fremstilling av beskyttede 5'-monofosfatderivater er vist nedenunder. Ved å beskytte de negative ladninger av fosfater med nøytrale substituenter, ville det dannes mer lipofile derivater som forventes å vende tilbake til de tilsvarende monofosfater så snart de er inne i cellen.
Nukleotid-derivater er en annen klasse av prodroger eller prodrogeformer. Fremstilling av beskyttede 5'-monofosfatderivater er vist nedenunder. Ved å beskytte de negative ladninger av fosfater med nøytrale substituenter, ville det dannes mer lipofile derivater som forventes å vende tilbake til de tilsvarende monofosfater så snart de er inne i cellen.

Anvendelser
Omfattet av oppfinnelsen er anvendelse av karboksamidin som angitt i formel 1, eller et farmasøytisk akseptabelt salt derav for fremstilling av et medikament for behandling av en hepatitt C-virus (HCV) viral infeksjon eller en hepatitt B-virus (HBV) viral infeksjon.
Det er spesielt tatt sikte på at Viramidin™ (1,39 ug/ml) øker ekspresjonen og syntesen av Type 1-cytokiner i (fortrinnsvis aktiverte) T-lymfocytter, og resultater fra diverse forsøk er vist i figurene 2-5. Figur 2 viser effekten av 5 jiM Viramidin (en forbindelse i henhold til formel 1), Ribavirin og levovirin på Type 1-cytokin-syntese i SEB-aktiverte human-T-celler (n = 5 donorer), hvor Viramidin viser en klar økning i Type 1-respons sammenlignet med kontrollprøven med Triazol. Figur 3 er en grafisk representasjon av en doser-responseffekt av Viramidin i området 0,625-10 jiM på Type 1-cytokinsyntese i SEB (Staphylococcal Enterotoxin B) -aktiverte human T-celler (data representerer 4 individuelle donorer). In wVo-effekten av en øket Type 1-respons i en kontakt-hypersensitivitets (CHS) -assay av Viramidin er klart demonstrert i figur 4, og figur 5 viser en sammenligning mellom Viramidin og Levovirin/Ribavirin med hensyn til toppresponsnukleosidkonsentrasjon og toppområde av responser (y-akse viser antall respondører i et spesielt forsøk).
Fremstilling av human-T-celler og aktivering in vitro
Periferale blod-mononukleærceller ble isolert fra friske donorer eller reumatoid artritt-pasienter ved densitetsgradient-sentrifugering fulgt av T-celle-anriking ved anvendelse av Lymphokwik (One Lambda, Canoga Park CA). Kontaminerende monocytter ble fjernet ved adherens til plast. Rensede T-celler var >99% CD2+, <1% HLA-DR+ og <5% CD25+ og ble opprettholdt i RPMI-AP5 (RPMI-1640 medium som inneholdt 20 mM HEPES buffer, pH 7,4, 5% autologt plasma, 1% L-glutamin, 1% penicillin/streptomycin og 0,05% 2-merkaptoetanol).
For bestemmelse av cytokinproteinivåer, ble T-celler (1 x 10<6>celler i et volum av 1 ml) aktivert ved tilsetning av 10 ng PMA + 0,5 jxg ionomycin (begge fra Calbiochem, La Jolla, CA) og inkubert i plater med 24 brønner i nærvær av 0-20 HM nukleosid i opp til 48 timer ved 37°C og 5% CO2i en fuktig inkubator. Etter aktivering, ble supernatantene analysert med hensyn på cellederivert cytokin-produksjon. For proliferasjons- og levedyktighetsstudier ble protokollen som ovenfor modifisert til et format med plate med 96 brønner ved anvendelse av 0,2 x 10<6>celler i et volum av 0,2 ml og aktivering med 2 ng PMA og 0,1 ug ionomycin. I separate forsøk, ble 5 x 106 T-celler i 2 ml aktivert med 20 ng PMA + 1 jig ionomycin. Alternativt kan celler bli aktivert in vitro med SEB ved å følge publiserte metoder. Her ble totalt RNA isolert fra T-celler etter 6-24 timers inkubasjon og analysert ved RT-PCR for å bestemme mRNA-nivåer av diverse cytokiner og inflammatoriske mediatorer. Også i separate forsøk, ble human T-celler renset videre (ved anvendelse av celleanrikningsreagenser fra Stem Cell Technologies, Vancouver, BC) for å gi rene populasjoner av CD4+ (<1% CD8+ ved anvendelse av RosetteSep human CD4+ T-celleisolasjonsreagns), og CD8+
(<1% CD4+ med RosetteSep human CD4+ T-celleisoleringsreagens) T-celle-subsets, hvoretter 1 x 10<6>celler pr. ml ble aktivert med PMA og ionomycin, som i de totale T-celle-eksperimenter.
Ekstracellulære cytokin-analyser
Human-cytokin-nivåer ble bestemt i cellesupernatanter, etter passende fortynning, ved anvendelse av ELISA-sett som var spesifikke for IL-2, IFNg, TNFa, IL-4 og IL-5 (Biosource International, Camarillo, CA). Murin-cytokin-nivåer ble bestemt ved anvendelse av ELISA-sett som var spesifikke for murin IFNg og IL-4 (R og D Systems, Minneapolis, NM). Alle ELISA-resultater ble uttrykt som pg/ml. Noen data er vist som prosent av aktivert kontrollprøve, beregnet som forholdet mellom aktivert T-cellecytokin-nivå i nærvær av testnukleosid over cytokin-nivået av ubehandlede aktiverte T-celler x 100%. Null effekt på cytokin-nivåer ved testnukleosider ville gi en prosentdel av aktivert kontrollprøveverdi på 100%. Alternativt ble data vist som prosent endring fra aktivert kontrollprøve ([(test pg/ml - aktivert kontrollprøve pg/ml)/aktivert kontrollprøve pg/ml] x 100%). Null effekt på cytokin-nivåer ved testnukleosider ville være 0%.
Kontakt-hypersensitivitet (CHS)
Reaktivitet mot kontaktallergenet, DNFB, ble bestemt, i BALB/c-mus, som tidligere beskrevet (Ishii, N., K. Takahashi, H. Nakajima, S. Tanaka, P.W. Askenase. 1994. DNFB-kontaktsensitivitet (CS) i BALB/c og C3H/He-mus. J. Invest. Dermatol. 102:321). Kort sagt ble mus sensitivisert ved påføring av 20 pj av 0,3% DNFB i aceton:olivenolje, 4:1 på de barberte underliv hos naturlige mus. For optimal fremkallelse av CHS, ble musene utfordret på begge sider av hvert øre med 20 ul av 0,12% DNFB, fem dager etter sensitivisering. Usensitiviserte mus ble også utfordret og anvendt som kontrollprøver i hvert forsøk. Etter 24 timer, ble øretykkelsesmålinger foretatt, og respons på DNFB ble fastslått ved å subtrahere verdier etter utfordring fra verdier før utfordring. Der hvor det ble indikert, ble 7-|5-D-ribofuranosyl-4-oksopyrrolo[2,3-d]pyrimidin-5-karboksamidin, i en dose av 6,2 pg i 50 ul PBS (0,3 mg/kg) eller 12,4 ug i 100^il PBS (0,6 mg/kg), administrert ved i.p.-injeksjon ved tidspunktet for utfordring med DNFB. Disse doser av 7-|}-D-ribofuranosyl-4-oksopyrrolo[2,3-d]pyrimidin-5-karboksamidin ga maksimal effekt i preliminære optimaliseringsstudier. Etter de endelige øretykkelsesmålinger, ble mus avlivet ved servikal dislokasjon, og armhule/lateral armhule-lymfeknuter ble fjernet. Etter isolasjon av total cellulær RNA fra isolerte lymfeknuteceller, ble RT-PCR og Southern Blot-analyser utført for å overvåke muse-IFNg, IL2 og IL-10 mRNA-nivåer.
Ytterligere forsøk
Det er generelt tatt sikte på at et skifte av en immunrespons mot en Type 1-respons er gunstig. Følgelig er det tiltenkt at forbindelsene over kan være spesielt nyttige ved behandling av virussykdommer (fortrinnsvis i virusinfeksjoner hvor Type 1-respons blir redusert eller undertrykket). For å bekrefte effektiviteten av å modulere en immunrespons, er det blitt utført diverse forsøk, og det følgende er en eksempelvis oppsummering av noen av forsøkene utført med tilsiktede forbindelser: In vitro - Viramidin-inhibert Punta Toro-virus-infeksjon av LLC-MK2 (rhesus ape-nyreceller) med EC50 av 8 mg/ml (Adames-stamme) og 12 mg/ml (Balliet-stamme) - CC50 var 320 mg/ml (0,1-1,2 virus-rating).
In vivo - Administrering s.c. eller oralt viramidin resulterte i 100% overlevelse (10 C57BL/6 mus/gp) av PTV injisert s.c. (Adames-stamme).
For 24 timers postinfeksjon ved PTV in vivo, var den minimale effektive s.c. dose av 32 mg/kg for ribavirin og for viramidin 96 mg/kg gitt s.c. b.i.d i 5 dager. For 24 timers postinfeksjon ved PTV in vivo, var den minste effektive p.o. dose på 20 mg/kg for ribavirin og for viramidin 40 mg/kg gitt p.o. b.i.d i 5 dager.
Generelt er de mest foretrukne anvendelser i henhold til oppfinnelsen, slike hvor de aktive forbindelser er relativt mindre cytotoksiske mot de ikke-målrettede vertsceller og relativt mer aktive mot målet. I dette henseende kan det også være fordelaktig at L-nukleosider kan ha øket stabilitet i forhold til D-nukleosider, som kunne føre til bedre farmakokinetikk. Dette resultat kan oppnås fordi L-nukleosider muligens ikke blir erkjent av enzymer, og derfor kan ha lengre halvtid.
Forbindelsene over vil bli administrert in vivo, in vitro eller ex vivo i enhver passende farmasøytisk sammensetning, og under enhver passende protokoll. Derfor kan administrering finne sted oralt, parenteralt (inklusive subkutane injeksjoner, intravenøst, intramuskulært, ved intrasternal injeksjon eller infusjonsteknikker), ved inhaleringsspray, eller rektalt, topisk og så videre, og i doseringsenhetsblandinger som inneholder konvensjonelle ikke-toksiske farmasøytisk akseptable bærere, adjuvanter og vehikler.
Som eksempel er det påtenkt at forbindelser over kan settes sammen i blanding med en farmasøytisk akseptabel bærer. For eksempel kan forbindelsene i henhold til oppfinnelsen bli administrert oralt som farmakologisk akseptable salter. Fordi forbindelsene i henhold til oppfinnelsen for det meste er vannløselige, kan de administreres intravenøst i fysiologisk saltløsning (for eksempel bufret til en pH-verdi på ca. 7,2-7,5). Konvensjonelle buffere, for eksempel fosfater, bikarbonater eller citrater, kan anvendes for dette formål. Naturligvis kan fagmannen på området modifisere blandingene innen læren av beskrivelsen for å tilveiebringe tallrike blandinger for en spesiell administreringsmåte uten å gjøre sammensetningene i henhold til oppfinnelsen ustabile eller å gå til kompromiss med hensyn til dere terapeutiske aktivitet. Spesielt kan modifiseringen av de her beskrevne forbindelser for å gjøre dem mer løselige i vann eller annen vehikkel, for eksempel lett foretas ved små modifikasjoner (saltblanding, forestring, osv.) som er godt innenfor det fagmannen har kjennskap til. Det er også godt innenfor det tekniske område å modifisere administreringsmåten og doseringsregimet for en spesiell forbindelse i den hensikt å mestre farmakokinetikken til de her beskrevne forbindelser for maksimal velgjørende effekt hos pasienten.
I tillegg kan forbindelsene over administreres alene eller i kombinasjon med andre midler for behandling av de ovennevnte infeksjoner eller tilstander. Anvendelser ved kombinasjonsterapier i henhold til foreliggende oppfinnelse omfatter administrering av minst én forbindelse i henhold til oppfinnelsen eller et funksjonelt salt derav og minst én annen farmasøytisk aktiv ingrediens. Den eller de aktive ingredienser og farmasøytisk aktive midler kan administreres separat eller sammen, og når de administreres separat, kan dette foregå samtidig eller separat i hvilken som helst rekkefølge. Mengdene av den eller de aktive ingredienser og det eller de farmasøytisk aktive midler og de relative tidspunkter for administrering vil bli valgt i den hensikt å oppnå den ønskede kombinerte terapeutiske effekt. Fortrinnsvis involverer kombinasjonsterapien administrering av én forbindelse i henhold til oppfinnelsen eller et fysiologisk funksjonelt derivat derav og ett av de midler som er nevnt nedenunder.
Eksempler på andre droger eller aktive ingredienser ment å være effektive i kombinasjon med en modulator i henhold til formel 1, er anti-virusmidler, for eksempel interferon, inklusive a-interferon.
Med hensyn til dosering, vil fagmannen på området erkjenne at en terapeutisk effektiv mengde vil variere med infeksjonen eller den tilstand som skal behandles, dens grad av alvorlighet, behandlingsregimet som skal anvendes, farmakokinetikken til det anvendte middel, så vel som pasienten (dyr eller menneske) som behandles. Det er påtenkt at diverse alternative doseringer også er aktuelle, inklusive doseringer mellom 0,5 mg/kg og 0,1 mg/kg og mindre, men også doseringer mellom 0,5 og 1,0 mg/kg og mer. Det er videre påtenkt at selv om behandlingssuksess kan oppnås med noen virusinfeksjoner ved relativt lave plasmakonsentrasjoner av forbindelsene av formel 1, kan andre virusinfeksjoner kreve relativt høye doseringer. Det er imidlertid påtenkt at et aktuelt regime vil bli utviklet ved å administrere en liten mengde og deretter øke mengden inntil bivirkningene blir for alvorlige, eller til den påtenkte effekt er oppnådd.
Administrering av den aktive forbindelse kan variere fra kontinuerlig (intravenøst drypp) til flere orale administreringer pr. dag (for eksempel Q.I.D.) og kan inkludere oral, topisk, parenteral, intramuskulær, intravenøs, subkutan, transdermal (som kan inkludere et penetreringsforsterkende middel), bukkal og stikkpilleadministrering, blant andre administreringsmåter.
For å fremstille farmasøytiske preparater i henhold til oppfinnelsen, blandes en terapeutisk effektiv mengde av én eller flere av forbindelsene i henhold til oppfinnelsen fortrinnsvis intimt med en farmasøytisk akseptabel bærer i henhold til konvensjonelle farmasøytiske kompunderingsteknikker for å produsere en dose. En bærer kan ha et stort utvalg av former avhengig av den preparatform som ønskes for administrering, for eksempel oral eller parenteral. Ved fremstilling av farmasøytiske preparater i oral doseringsform, kan ethvert av de vanlige farmasøytiske medier benyttes. Således kan, for flytende orale preparater, så som suspensjoner, eliksirer og løsninger, egnede bærere og additiver inklusive vann, glykoler, oljer, alkoholer, smaksmidler, konserveringsmidler, fargemidler og lignende bli anvendt. For faste orale preparater, for eksempel pulver, tabletter, kapsler, og for faste preparater slik som stikkpiller, kan egnede bærere og additiver inklusive stivelser, sukkerbærer, for eksempel dekstrose, mannitol, laktose og beslektede bærere, fortynningsmidler, granuleringsmilder, smøremilder, bindemidler og desintegreringsmidler bli anvendt. Om ønsket kan tablettene eller kapslene bli enterisk belagt eller lages med forsinket frigivelse ved standardteknikker.
For parenterale blandinger, vil bæreren vanligvis omfatte sterilt vann eller vandig natriumkloridløsning, selv om andre ingredienser inklusive slike som hjelper på dispergering kan inkluderes. Naturligvis, hvis sterilt vann skal anvendes og holdes sterilt, må preparatene og bærerne også bli sterilisert. Injiserbare suspensjoner kan også fremstilles, og i dette tilfelle kan aktuelle flytende bærere og suspenderingsmidler bli anvendt.
Claims (6)
1. Anvendelse av karboksamidin som angitt i formel 1, eller et farmasøytisk akseptabelt salt derav for fremstilling av et medikament for behandling av en hepatitt C-virus (HCV) viral infeksjon eller en hepatitt B-virus (HBV) viral infeksjon,
hvor forbindelsen er i D-konfigurasjon.
2. Anvendelse som angitt i krav 1, hvor den virale infeksjonen er en HCV-infeksjon.
3. Anvendelse som angitt i krav 1 eller 2, hvor medikamentet er formulert for oral administrering.
4. Anvendelse som angitt i krav 1 til 3, hvor medikamentet formuleres til å inkludere en dose av forbindelsen mellom 0,1 mg pr. kg kroppsvekt hos pasienten og 40 mg pr. kg kroppsvekt hos pasienten.
5. Anvendelse som angitt i et hvilket som helst av kravene 1 til 4, hvor medikamentet ytterligere omfatter interferon.
6. Anvendelse ifølge krav 5, hvor interferonet er et alfa-interferon.
