Description
Foreliggende oppfinnelse angår anvendelse av retinsyrereseptor- (RAR) agonister, spesielt en retinsyrereseptor-agonist som er RARy-selektiv for fremstilling av medikamenter inneholdende én eller flere slike agonister for behandling av emfysem.
Kronisk obstruktiv pulmonal sykdom (COPD) er en hovedårsak til sykdom og dødelighet, rangert tre og fire som dødsårsak i henholdsvis den Europeiske Union og Nord-Amerika. COPD er karakterisert ved redusert maksimal ekspiratorisk strøm som ikke forandres over mange måneder og vedvarer i 2 eller mer påfølgende år. Pasienter med den alvorligste form av COPD har generelt en betydelig grad av emfysem.
Emfysem er definert anatomisk ved permanent luftromforstørrelse distalt til de terminale bronkioler og det er karakterisert ved gradvis tap av lunge-tilbakeslag, alveolær destruksjon, redusert alveolært overflateareale og gassutveksling, hvilket fører til redusert FEV1 (American Thoracic Society: Am. J. Resp. and Critical Care 152: S77 - S124,1995). Svekket gassutveksling og reduksjon i ekspiratorisk strøm er karakteristiske fysiologiske abnormaliteter som emfysempasienter lider av. Hovedsymptom for alvorlig rammede emfysempasienter er kortpustethet under minimal fysisk aktivitet.
Selv om andre potensielle miljøtoksiner kan bidra, er den mest vanlige årsak til emfysem sigarettrøking. Disse skadelige midler aktiverer destruktive prosesser i lungen, omfattende frigjøring av aktive proteaser og friradikal oksydanter i overskudd i forhold til beskyttende mekanismer. Den ukontrollerte frigjøring av aktive proteaser skaper en ubalanse i protease/antiprotease-nivåer i lungene som fører til elastin-matriksdestruksjon, svekking av elastisk tilbakeslag, vevskade og kontinuerlig lungefunksjon-reduksjon. Progresjonen av denne skaden kan sinkes ved å fjerne de skadelige agenser (for eksempel ved å slutte med røking); imidlertid blir skadede alveolære strukturer ikke reparert og lungefunksjon ikke gjenvunnet.
All-trans retinsyre (ATRA) er en multifunksjonell modulator av cellulær oppførsel, som har et potensiale for å endre både ekstracellulær matriks metabolisme og normal epitelial differensiering. I lungene er ATRA vist å modulere forskjellige aspekter av lungedifferensiering ved å interagere med spesifikke retinsyrereseptorer (RAR) som er selektivt uttrykt i tid og rom. Koordinert aktivering av RARB og RARy er forbundet med lungeforgrening, alveolisering/tverrdeling ("septation") og genaktivering av tropoelastin i neonatale rotter.
Under alveolær tverrdeling øker retinsyre-lagringsgranuler (retinyl-estere) i det fibroblastiske mesenkym som omgir alveolære vegger (Liu et at, Am. J. Physiol. 265: L430 - L437, 1993; McGowan et a/Am. J. Physiol. 269: L463 - L472,1995) og RARy-ekspresjon i lungen toppes (Ong, D. E. og Chytil, F., Proe. Nati. Acad. of Sciences 73: 3976 - 3978,1976; Grummer, M. A., Thet, L. og Zachman, R. D., Pediatr. Pulm. 17: 234 - 238,1994). Uttømming av disse retinylester-lagere skjer parallelt med avsetning av ny elastin matriks og tverrdeling. Til støtte for dette konseptet demonstrerte Massaro og Massaro (Massaro, D. og Massaro, G., Am. J. Physiol. 270, L305 - L310,1996) at postnatal administrering av retinsyre øker antallet alveoler i rotter. Behandling av nyfødte rotteunger med dexamethason hemmer prosessen med tverrdeling i lungene. Denne effekten kan overvinnes ved supplerende behandling med retinsyre. Videre ble evnen til dexamethason til å forhindre ekspresjonen av CRBP og RARB mRNA og påfølgende alveolær tverrdeling i rottelunge under utvikling, opphevet med ATRA.
Nyere undersøkelser demonstrerte at ATRA kan fremkalle dannelse av nye alveoler og returnere elastisk tilbakeslag til nær normalt i dyremodeller av emfysem (Massaro, D. og Massaro, G., Nature Med. 3: 675 - 677,1997; "Strategies to Augment Alveolization," National Heart, Lung and Blood Institute, RFA: HL-98-011,1998.). Imidlertid er mekanismen ved hvilken dette skjer, fortsatt uklar.
Retinoider er en klasse forbindelser strukturelt beslektet med vitamin A, omfattende naturlige og syntetiske forbindelser. Mange serier av retinoider er funnet klinisk anvendelige ved behandling av dermatologiske og onkologiske sykdommer. All-trans retinsyre (ATRA) og dens andre naturlig forekommende retinoid-analoger (9-cis retinsyre, all-trans 3-4 didehydro retinsyre, 4-okso-retinsyre og retinol) er pleiotropiske regulatoriske forbindelser som modulerer strukturen og funksjonen av en rekke inflammatoriske, immune og strukturelle celler. De er viktige regulatorer av epitelcelleproliferasjon, differensiering og morfogenese i lungen. Retinoider utøver deres biologiske effekter gjennom en serie av nukleære reseptorer som er ligand-induserbare transkripsjonsfaktorer som tilhører steroid/thyroid-reseptor superfamilien.
Retinoidreseptorene er klassifisert i to familier, retinsyrereseptorer (RAR) og retinoid X reseptorer (RXR), hver bestående av tre distinkte undertyper (a, p og y). Hver undertype av RAR-genfamilien koder for et variabelt antall isoformer som oppstår fra differensiell spleising av to primære RNA-transkripter. ATRA er det fysiologiske hormon for retinsyrereseptorer og binder med omtrent lik affinitet til alle de tre RAR-undertyper. ATRA binder ikke til RXR-reseptorene; i stedet er for disse reseptorer 9-c/s retinsyre den naturlige ligand.
I mange ikke-pulmonale vev har retinoider antiinflammatoriske effekter, endrer progresjonen av epitelcelle-differensiering og hemmer stromalcelle-matriksproduksjon. Disse egenskaper har ført til utvikling av topiske retinoide terapeutiske midler for dermatologiske lidelser så som psoriasis, acne og hypertrofiske hud-arr. Andre applikasjoner omfatter kontroll av akutt promyelocyttisk leukemi, adeno- og platecelle-karsinom og hepatisk fibrose. Imidlertid er terapeutisk anvendelse av retinoider utenom for kreft, begrenset på grunn av de relative toksisiteter observert med de naturlig forekommende retinoider, ATRA og 9-cis RA. Disse naturlige ligander er ikke-selektive og har derfor pleiotropiske effekter gjennom hele kroppen, som ofte er toksiske. Nylig er forskjellige retinoider beskrevet som interagerer selektivt eller spesifikt med RAR- eller RXR-reseptorer eller med spesifikke undertyper (a, p, y) innen en klasse. Ved anvendelse av disse nye retinoider blir transrepresjon av AP-1 og transaktiveringsaktiviteter av retinoider separert. (Li, J. I tillegg har de reseptorselektive forbindelser vist redusert generell toksisitet in vitra og in vivo. (Chandraratna, R., J. Am. Acad. Dermatology, 39: S149 - S152, 1998; Look, J. et al., Am. J. Physiol. 269: E91 - E98, 1995).
I ett aspekt tilveiebringer foreliggende oppfinnelse følgelig anvendelse av en RAR-agonist som er i det minste gamma-selektiv og er RARa-sparende for fremstilling av medikamenter inneholdende én eller flere slike agonister for behandling av emfysem. Anvendelse av retinioder som er minst RARy-selektive og RARa- sparende vil fremme reparasjon uten å fremkalle ugunstige effekter på nivåer av plasmatriglycerider.
Som anvendt her betyr betegnelsen ED effektiv dose og blir anvendt i forbindelse med dyremodeller. Betegnelsen EC betyr effektiv konsentrasjon og blir anvendt i forbindelse med in vitro modeller.
Som anvendt her betyr betegnelsen "ED50" for et retinoid for en retinsyrereseptor, den molare konsentrasjon av retinoidet i en dyremodell som transaktiverer den spesielle aktuelle retinsyrereseptor med 50% av maksimal transaktivering som kan oppnås med det retinoid. Som anvendt her betyr betegnelsen "EC5o" for et retinoid for en retinsyrereseptor, den molare konsentrasjon av retinoidet i en in vitro modell som transaktiverer den spesielle aktuelle retinsyrereseptor med 50% av maksimal transaktivering som kan oppnås med det retinoid.
Som anvendt her er betegnelsen "retinoid" hvilken som helst forbindelse som kan transaktivere hvilken som helst eller alle a-, B- eller y-RAR- eller RXR-reseptorer med en ED50 på 1000 nm eller mindre.
Som anvendt her angir betegnelsen "transaktivering" evnen til et retinoid til å aktivere transkripsjonen av et gen hvor gentranskripsjonen blir initiert ved binding av en ligand til den spesielle retinsyrereseptor som testes, dvs. RARa, RARB eller RARy. Bestemmelse av evnen til en forbindelse til å transaktivere en retinsyrereseptor kan utføres ved metoder kjent for fagfolk på området. Eksempler på slike metoder finnes i Bernard et al., Biochem. Som anvendt her angir betegnelsen "RARy-selektiv agonist" en forbindelse som selektivt kan binde til RARy-reseptoren og fremme RARy-aktivering. RARy-selektive agonister vil binde til RARy-reseptoren i betydelig lavere konsentrasjoner (>10 ganger selektivitet, foretrukket 50 til 100 ganger selektivitet) enn RARa- og RARB-reseptorer. Den foretrukne aktivitetsprofil vil spare aktivering av RARa-reseptorer, hvilket fører til mer selektive biologiske responser.
Som anvendt her angir betegnelsen "RARy/B-selektiv agonist" én som selektivt binder til RARy- og RARB-reseptorer, fremmer både RARy- og RARB-aktivering og sparer aktivering av RARcc-reseptorer.
Som anvendt her angir betegnelsen "RAR-agonist som er i det minste gamma-selektiv og er RARa-sparende" én som er RARy-selektiv eller RARy/B-selektiv.
Som anvendt her angir betegnelsen "RAR pan agonist" én som binder til RARa-, RARB- og RARy-reseptorer med lignende affinitet, og fremmer RARa-, RARB- og RARy-aktivering.
"Prodrug" betyr hvilken som helst forbindelse som frigjør et aktivt stam-medikament in vivo når et slikt prodrug blir administrert til et pattedyr. Prodrug blir fremstilt ved modifikasjon av funksjonelle grupper til stede i det aktive medikament på en slik måte at modifikasjonene kan spaltes in vivo for frigjøring av stamforbindelsen. Prodrug omfatter forbindelser hvor en hydroksygruppe i det aktive medikamentet er bundet til hvilken som helst gruppe som kan spaltes in vivo for å regenerere den frie hydroksylgruppen. Eksempler på prodrug omfatter, men er ikke begrenset til estere (f .eks. acetat-, formiat- og benzoat-derivater), karbamater (f.eks. N,N-dimetylaminokarbonyl) og etere av hydroksy-funksjonelle grupper i aktive medikamenter og lignende. Slike forbindelser blir rutinemessig fremstilt av fagfolk på området ved acylering eller foretring av hydroksygruppen i stam-medikamentet.
Oppfinnerne har oppdaget at RARy-selektive agonister transaktiverer tropoelastin genekspresjon i humane lungefibroblaster. Tilsvarende er RARy-selektive forbindelser vist å fremme reparasjon og/eller regenerering av rottelunge-alveoler. Mens RARy-selektive retinoidagonister fremkalte tropoelastin-ekspresjon i humane lungefibroblaster, gjorde hverken RARB- eller RARa-selektive agonister dette.
Imidlertid vil det forstås av fagfolk på området at foreliggende oppfinnelse omfatter anvendelse av alle RARy-selektive agonister og er ikke begrenset til de RARy-selektive agonister beskrevet i referansene ovenfor eller de som nå er kjent på området. Generelt er alle forbindelser som har minst RARy-selektiv agonistaktivitet nyttige for anvendelsene og metodene ifølge foreliggende oppfinnelse.
En familie av RARy-selektive agonister som er anvendelige her er beskrevet i U.S. patent nr. 5.700.836, bevilget 23. desember 1997 og er representert ved Formel I.
Foreliggende oppfinnelse vedrører følgelig anvendelse ifølge krav 3, hvor RAR-agonisten er valgt fra forbindelser med formel I:

RARy-agonistselektivitet av en forbindelse kan bestemmes ved rutinemessige ligandbindingsforsøk kjent for en fagmann på området så som beskrevet i C. Apfel et al., Proe. Nat. Sei. Acad. (USA), 89: 7129-7133 (1992); M. Teng et al., J. Med. Chem., 40: 2445-2451 (1997); og PCT publikasjon WO 96/30009.
RARy-selektive agonister, som beskrevet her, er anvendelige for å fremme reparasjon av skadede alveoler og tverrdeling av nye alveoler. Som sådanne er disse forbindelser anvendelige for behandling av sykdommer så som emfysem.
Den spesielle dose av en RAR-agonist eller en RARy-selektiv agonist nødvendig for å fremme alveolær reparasjon/regenerering ifølge foreliggende oppfinnelse vil avhenge av alvorlighetsgraden av lidelsen, administreringsveien og beslektede faktorer som vil bestemmes av behandlende lege. Typisk vil den orale dosen være i området mellom ca. 1,0 mg/kg kroppsvekt pr. dag (mg/kg/dag) til 0,01 mg/kg/dag, fortrinnsvis fra ca. 0,1 til ca. 0,05 mg/kg kroppsvekt pr. dag. For et 50 kg menneske er den daglige orale dose av aktiv bestanddel fra ca. 50 til ca. 0,5 mg, fortrinnsvis fra ca. 5 til ca. 2,5 mg. Denne dosen kan leveres i et konvensjonelt farmasøytisk preparat ved en enkelt administrering, ved multiple applikasjoner eller ved kontrollert frigjøring, som nødvendig for å oppnå de mest effektive resultater. Det er forventet at lokal aerosol-levering av medikamentet til pulmonale luftrom vil redusere den effektive dosen 10 til 100 ganger (10 u.g/kg/dag til 0,1 u.g/kg/dag). Dosering vil fortsette så lenge det er medisinsk indikert, som, avhengig av alvorlighetsgraden av sykdommen, kan være i området fra noen få uker til mange måneder.
Typisk blir et farmasøytisk akseptabelt preparat, så som et salt eller prodrug av RAR-agonisten i en farmasøytisk akseptabel bærer eller fortynningsmiddel, administrert. I sammenheng med foreliggende oppfinnelse omfatter farmasøytisk akseptable salter hvilket som helst kjemisk egnet salt kjent på området retinoider som kan anvendes for administrering til humane pasienter. Eksempler på konvensjonelle salter kjent på området omfatter alkalimetallsaltene så som natrium- og kaliumsalter, jordalkalimetallsaltene så som kalsium- og magnesiumsalter og ammonium- og alkylammoniumsalter. Spesielt foretrukne prodrugpreparater av RARy-agonister omfatter hydrolyserbare esterderivater så som aromatiske og benzylestere eller lavere alkylestere f.eks. etyl, t-butyl, cyklopentyl og lignende.
Representative leveringsregimer omfatter oral, parenteral (omfattende subkutan, intramuskulær og intravenøs), rektal, buckal (omfattende sublingual), transdermal, pulmonal og intranasal. Én metode for pulmonal administrering involverer aerosolfremstilling av en vandig løsning av en RAR-agonist. Aerosoliserte preparater kan omfatte forbindelsen pakket i reverse miceller eller liposomer. Andre metoder for pulmonal levering omfatter tørre pulvere levert med tørrpulver-inhaleringsanordninger og flytende preparat av retinoid-agonisten med polyetylenglykoler eller vandig etanol levert med elektrohydrodynamiske inhaleringsanordninger. Typiske pulmonale og respiratoriske leveringssystemer er beskrevet i U.S. patent nr. 5.607.915, 5.238.683, 5.292.499 og 5.364.615 og "Aerosol delivery of liposomal ATRA to the lungs" Parthasarathy, R., Gilbert, B.M. og Mehta, K., Cancer Chemotherapy Pharmacol, 43: 277-283,1999.
Medikamenter kan også anvendes , f.eks. i form av et farmasøytisk preparat for systemisk administrering av RARy-agonister i samtidig eller sekvensiell kombinasjon med en ytterligere aktiv bestanddel for å forbedre slimfjerning av luftveisslim eller reduksjon av sl im viskositet. Representative aktive bestanddeler for å forbedre slimoppløsning omfatter, for eksempel natriumkanal-blokkere (f.eks. amilorid) eller antibiotika (f.eks. duramycin, nisin eller subtilin). Representative aktive bestanddeler for reduksjon av slimviskositet omfatter N-acetylcystein, homocystein og fosfolipider.
RARy-agonister vil typisk administreres som farmasøytiske preparater i blanding med en farmasøytisk akseptabel, ikke-toksisk bærer. Som nevnt ovenfor kan slike preparater fremstilles for parenteral administrering (subkutan, intramuskulær eller intravenøs), spesielt i form av væskeløsninger eller -suspensjoner; for oral eller buckal administrering, spesielt i form av tabletter eller kapsler; for intranasal administrering, spesielt i form av pulvere, nesedråper eller aerosol-preparater; og for rektal eller transdermal administrering. Hvilket som helst konvensjonelt bærermateriale kan anvendes. Bærermaterialet kan være hvilket som helst organisk eller uorganisk bærermateriale, så som vann, gelatin, gummi arabicum, laktose, stivelse, magnesiumstearat, talk, polyalkylenglykoler, vaselin og lignende.
Flytende preparater for parenteral administrering eller for oral administrering kan inneholde som tilsetningsmidler sterilt vann eller saltvann, alkylenglykoler så som propylenglykol, polyalkylenglykoler så som polyetylenglykol, oljer av vegetabilsk opprinnelse, hydrogenerte naftalener og lignende. De kan inneholde litt sure buffere i pH-områder på ca. 4 til ca. 6. Egnede buffere omfatter acetat, askorbat og citrat i konsentrasjoner i området fra ca. 5 mM til ca. 50 mM. For oral administrering kan preparatet forbedres ved tilsetning av gallesyresalter eller acylcarnitiner.
Preparater for buckal administrering kan være faste og kan inneholde som typiske tilsetningsmidler sukkere, kalsiumstearat, magnesiumstearat, pregelatinert stivelse og lignende.
Faste former for oral administrering omfatter tabletter, harde og myke gelatinkapsler, piller, luktepulvere, pulvere, granuler og lignende. Hver tablett, pille eller luktepulver kan inneholde fra ca. 5 til ca. 200 mg RARy-agonist, fortrinnsvis fra ca. 10 til ca. 50 mg. Foretrukne faste orale doseformer omfatter tabletter, todelte kapsler med hardt skall og myke elastiske gelatin- (SEG) kapsler. SEG-kapsler er av spesiell interesse fordi de gir klare fordeler fremfor de andre to former (se Seager, H., "Soft gelatin capsules: a solution to many tableting problems", Pharmaceutical Technology, 9: (1985). Noen av fordelene med anvendelse av SEG-kapsler er: a) doseinnhold-jevnhet er optimalisert i SEG-kapsler fordi medikamentet blir oppløst eller dispergert i en væske som kan doseres nøyaktig i kapslene; b) medikamenter formulert som SEG-kapsler viser god biotilgjengelighet fordi medikamentet blir oppløst, solubilisert eller dispergert i en vannblandbar eller oljeaktig væske og, når frigjort i kroppen, blir løsningene derfor oppløst eller emulgert og gir medikamentdispersjoner med stort overflateareale; og c) nedbrytning av medikamenter som er sensitive for oksydasjon under langvarig lagring blir forhindret på grunn av det tørre skallet.
Preparater for nasal administrering kan være faste og kan inneholde tilsetningsmidler, for eksempel laktose eller dekstran eller kan være vandige eller oljeaktige løsninger for anvendelse i form av nesedråper eller oppmålt spraypreparat. Spesielt omfatter nasale preparater tørre pulvere egnet for konvensjonelle tørrpulverinhalatorer (DPI), væskeløsninger eller suspensjoner egnet for forstøvning og drivmiddelpreparater egnet for anvendelse i inhalatorer med oppmålt dose (MDI).
Når formulert for nasal administrering kan absorpsjonen gjennom neseslimhinnen forbedres med overflateaktive syrer, så som for eksempel glykokolsyre, kolsyre, taurokolsyre, etokolsyre, deoksykolsyre, chenodeoksykolsyre, dehydrokolsyre, glykodeoksykolsyre, cyklodekstriner og lignende, i en mengde i området mellom ca. 0,2 og 15 vektprosent, fortrinnsvis mellom ca. 0,5 og 4 vektprosent, mest foretrukket ca. 2 vektprosent.
Levering av de fremstilte medikamentene ifølge foreliggende oppfinnelse til pasienten over forlengede tidsperioder, for eksempel i perioder på én uke til ett år, kan oppnås ved en enkelt administrering av et kontrollert frigjøringsystem inneholdende tilstrekkelig aktiv bestanddel for den ønskede frigjøringsperioden. Forskjellige kontrollerte frigjøringsystemer, så som monolittiske eller reservoir-type mikrokapsler, depot-implantater, osmotiske pumper, blærer, miceller, liposomer, transdermale plastere, iontoforetiske anordninger og alternative injiserbare doseformer kan anvendes for dette formål. Lokalisering til stedet hvor levering av den aktive bestanddel er ønsket, er et ytterligere trekk ved noen kontrollerte frigjøringsanordninger, som kan være fordelaktig ved behandling av visse lidelser.
De følgende er representative farmasøytiske preparater for anvendelse av RAR y-selektive agonister som beskrevet herfor å fremme elastinmediert matriks-reparasjon og alveolær tverrdeling.

EKSEMPEL 1:
RAR- transaktivering av stabilt tropoelastin- qen
Transaktivering av tropoelastin-genet i normale humane lungefibroblaster (CCD-16Lu) ble utført som beskrevet nedenfor. Celler ble dyrket til sammenflyting på hvilket tidspunkt friske media +/- ATRA eller selektivt retiniod ble satt til basalkulturen. ATRA og/eller selektive retinoider ble anvendt i en konsentrasjon på 1 x 10"<7> M. Cellelaget ble lyset ved anvendelse av en guanidinium-basert buffer (TRIZOL/Sigma) for RNA-ekstraksjon og analyse. Amplifisering av tropoelastin og GAPDH RNA ble utført ved standard kvantitativ RT-PCR (TAQMAN, Perkin/Elmer) ved anvendelse av passende selektive primere. Tropoelastin genekspresjon ble normalisert til ekspresjonen av standard husholdningsgen (GAPDH).
Prøver ble kjørt in triplo. Resultatene er vist nedenfor i Tabell II. Disse resultater demonstrerer at ATRA (RAR-agonist 4) fremkaller litt tropoelastin geninduksjon sammenlignet med kontrollen og at den RARy-selektive agonist (RAR-agonist 3) markert fremkaller tropoelastin geninduksjon. Imidlertid induserte RARa- og RARB-selektive agonister (RAR-agonister 1 og 2) ikke tropoelastingenet.
EKSEMPEL 2
RAR- selektive a<g>onister i rottelunae
All-trans retinsyre (ATRA) og RAR-selektive agonister ble evaluert for deres effekter på alveolær reparasjon i rottemodellen av elastase-fremkalt emfysem (Massaro, G. og Massaro, D., Nature, Vol. 3 Nr. 6: 675-677 (1997)). Dyr ble delt i behandlingsgrupper på omtrent åtte. Lungeinflammasjon og alveolær skade ble fremkalt i Sprague Dawley hann rotte r ved en enkel instillering av pankreatisk elastase (porcin-avledet, Calbiochem) 2 U/gram kroppsmasse.
Tre uker etter skade ble ATRA eller en RAR-selektiv agonist oppløst i DMSO (20 mg/ml) og lagret ved -20 C. Friske arbeidslagere ble fremstilt daglig ved fortynning i maisolje til en endelig konsentrasjon på 2 mg/ml. Dyr behandlet med ATRA og RAR-selektive agonister (0,5 mg/kg ip) ble dosert én gang daglig ved intraperitoneal injeksjon, som startet 21 dager etter skade. Kontrollgrupper ble utfordret med elastase og 21 dager senere behandlet med konstituent (DMSO/PBS) i 14 dager. Dyrene ble avlivet 24 timer etter siste dose ved blodtømming under dyp anestesi. Blod ble oppsamlet på tidspunktet for blodtømming for analyse av endringer i blodkjemi etter de respektive behandlinger.
Lungene ble blåst opp med 10% nøytral bufret formalin ved intratrakeal instillering med en konstant hastighet (1 ml/gram kroppsmasse/min). Lungen ble skåret ut og nedsenket i fiksativ i 24 timer før anvendelse. Standard metoder ble anvendt for å fremstille 5 um paraffin snitt. Snittene ble merket med Hematoxylin og Eosin (H%E). Datorisert morfometrisk analyse ble utført for å bestemme gjennomsnittet av alveolær størrelse og alveolært antall.
Resultatene er vist nedenfor i Tabell III. Disse resultater demonstrerer at ATRA (RAR-agonist 4) fremkaller alveolær reparasjon, og reverserer den alveolære destruksjon forårsaket av behandling med elastase. RARy- og RARy/B-selektive agonister (RAR-agonister 3, 5, 6 og 7) fremkaller markert alveolær-reparasjon. Imidlertid fremkalte RAR B- og RAR a-selektive agonister (RAR agonister 1, 2 og 9) ikke alveolær reparasjon.
I tillegg ble lysmikrografi av lungesnitt tatt. Mikrografier ble tatt av en normal rottelunge, en rottelunge skadet ved elastase og deretter behandlet med RAR-agonist 1 (a-selektiv), en rottelunge skadet ved elastase og deretter behandlet med RAR-agonist 2 (B-selektiv) og en rrottelunge skadet med elastase og deretter behandlet med RAR-agonist 3 (y-selektiv). Mikrografiene avslørte store strukturelle forskjeller åpenbare i lungene som fikk de tre forskjellige behandlingene. Når sammenlignet med mikrografiene av den normale rottelungen, viste mikrografi av lungen behandlet med den RAR y-selektive agonist alveolært område lignende det til den normale rottelungen. Mikrografi av lungen behandlet med den RAR a-selektive agonist og mikrografi av lungen behandlet med den RAR B-selektive agonist reverserte ikke forandringen forårsaket av elastasebehandling.
EKSEMPEL 3
Effekter av RAR pan aaonist oa RARy- oa RARy/ B- selektive retinoid- aaonister på plasma- triavlceridnivåer
Eksperimentelt emfysem ble fremkalt i rotter som beskrevet i Eksempel 2. Tre (3) uker etter skade ble dyrene behandlet med konstituent (Capmul løsning) eller retinoider fremstilt i Capmul og dosert oralt med 1, 3 og 10 mg/kg kroppsvekt. Dyrene ble behandlet i 14 dager før avslutning av studien. Lunge-preparering og blodoppsamling ble utført som beskrevet i Eksempel 2.
Kvantifisering av triglycerider inneholdt i rotteplasma ble utført etter anvendelse av etablerte prosedyrer i en kontakt klinisk laboratoriefasilitet. Kort sagt ble plasmatriglycerider omdannet til dihdroksyaceton og H2O2, ved sekvensiell behandling av plasma med lipase og glycerokinase i henhold til retningslinjene angitt av produsenten av triglycerider/GPO-sett (Boehringer Mannheim #1488872). H202 ble kvantifisert kolorimetrisk i en Hitachi 911 Chemistry Analyzer. I rotter er normale triglyceridnivåer 75-175 mg/dl.
Resultatene er vist nedenfor i Tabell IV. Disse resultater demonstrerer at RAR pan agonister (RAR-agonister 4 (ATRA), 8 og 10) forårsaker at triglyceridnivåer i rotter blir enten i den høye enden av normale, eller forhøyede. I motsetning forårsaker de RARy-selektive retinoider (RAR-agonister 3, 7 og 5) og RARy/B-selektive retinoider (RAR-agonist 6) ikke forhøyelse i triglycerid-nivåene; for disse RARa-sparende retinoider forblir triglyceridnivåene godt innenfor den middels til lave enden av det normale området. Det synes derfor som om RARa-selektiv aktivitet er bundet til forhøyede triglyceridnivåer.
EKSEMPEL 4
Klinisk forsøksstudie
Menn og kvinner inntatt i denne undersøkelsen vil være mellom 45-75 år, som har en historie med emfysem og vil ha sluttet med røking i en periode på minst 6 måneder før inntak i undersøkelsen. I tillegg må pasienten presentere minimum 2 av de 3 følgende pulmonale funksjonskriterier: post bronkodilator TLC (total lunge kapasitet) > 110% av forutsagt
(indikativ for hyperekspansjon)
post bronkodilator FEV1 (tvunget ekspiratorisk volum) < 70% av
forutsagt (indikativ for moderat luftstrømsobstruksjon).
DLCO (fortynning lunge karbonmonoksyd) < 65% av forutsagt (indikativ
for moderat til alvorlig destruksjon av alveolære strukturer.
I tillegg bør pasientene ha CT scanningsbevis på mild til moderat emfysem, tilstrekkelig renal og hepatisk funksjon og normal benmarg.
Pasienter vil være utelukket fra undersøkelsen hvis de oppfyller ett eller flere av de følgende kriterier: FEV1<0,8 liter, uforklart vekttap >10% vanlig vekt pr. år, tilbakevendende lungeinfeksjoner > 2 pr. år med sputum på over 3 spiseskjeer/dag, bronkiektasi, ustabil angina, hypertriglycerider >300 mg% , hypercholesteremi >220%, oral steroid-avhengighet, samtidig medisinering kjent for å gripe inn i P450 hepatiske systemer, akutt eller kronisk leversykdom med for høyt alkoholkonsum eller historie av allergi mot retinoider.
Studieutformning
Alle pasienter vil bli observert i en periode på 3 måneder etter fullføring av aktiv behandlingsfase for å bedømme resterende lungeforbedring eller toksisitet. Pulmonal funksjonstesting (PFT) og system-baserte spørreskjemaer blir utført hver 3 måneder. HRCT vil utføres bare ved begynnelsen av screeningen og etter fullføring av behandling. Individer som mottar høy dose eller potensielt lav dose av retinoid kan demonstrere én eller flere av de følgende responser: reduksjon av nedgangsgrad i FEV1 fra 63 ml/år til 31 ml/år; som viser initiell 5% forbedring i FEV i løpet av første år av behandling; forbedring i DLCo (5-10%); forbedring i livskvalitet som bestemt ved standard spørreskjema.
EKSEMPEL 5
Bestemmelse av pulmonal aassutvekslina
Pulmonale og arterielle blodgasser ble bestemt i elastase-skadede rotter retinoid behandling før avslutning av undersøkelsen. Rotter ble plassert under dyp anestesi ved anvendelse av pentobarbital (50 mg/kg, i.p. og en trakeal kanyle (PE 240) ble innsatt. Rottene ble kunstig ventilert (f= 90, TV= ca. 0,5 ml/100 g BW) ved anvendelse av en liten respiratorisk pump for dyr (Harvard). For hver rotte ble pumpeparametere regulert for å etablere et arterielt C02-nivå for den pulmonale arterie (PC02) på 30-35 torr. Arterielle blodprøver (ca. 0,2 ml) ble tatt fra abdominal aorta (A02) og umiddelbart analysert ved pHOx blodgass . Data er presentert som prosent (%) gjenvinning i forhold til elastase + konstituent behandlede rotter.
Som vist nedenfor forbedret behandling av elastase-skadede rotter med retinoider, gassutveksling. Spesielt var RARy-selektiv agonist 3 mer effektiv og kraftigere enn pan-agonisten (ATRA). Dette resultatet er i overensstemmelse med effektene på den strukturelle reparasjon av alveoler i de tidligere eksempler. Forbedret gassutveksling korrelerer med redusert kortpustethet forbundet med emfysem. Således vil behandling av en pasient med en RAR-gamma-selektiv agonist resultere i lindring av et av emfysemets vesentligste symptomer.

Claims (8)
1. Anvendelse av en RAR-agonist som er i det minste gamma-selektiv og er RARa-sparende for fremstilling av medikamenter inneholdende én eller flere slike agonister for behandling av emfysem.
2. Anvendelse ifølge krav 1, hvor RAR-agonisten er en RARy/B-selektiv agonist.
3. Anvendelse ifølge krav 1, hvor RAR-agonisten er en RARy-selektiv agonist.
4. Anvendelse ifølge krav 3, hvor den RARy-selektive agonist binder til RARy-reseptoren og transaktiverer med en EC50 på minst 200 nM.
5. Anvendelse ifølge krav 3, hvor selektiviteten til den RARy-selektive agonist for RARy-reseptoren er minst 20 ganger i forhold til RARa- og RARB-reseptorer.
6. Anvendelse ifølge krav 3, hvor RAR-agonisten er valgt fra forbindelser med formel I:
