VO 1538
Werkwijze voer het oereiden van laag-moleculaire heparinen, begiftigd met verhoogde farmacologische eigenschappen, alsmede het aldus verkregen •produkt._._' ____
De uitvinding heeft betrekking op een werkwijze voor het bereiden van laag-moleculaire heparinen, begiftigd met farmacologische,therapeutische eigenschappen van bijzonder belang, uitgaande van normaal hepa-rine.
5 Heparine is een vel bekend en stabiel een- of tveevaardig zout van het instabiele heparine^-zuur. Het is een polymeer van een disaccharide eenheid gevormd door hexuron (D-glucuron en L-iduron) zuren en glucosamine ρΰ- en N-sulfaat gebonden door a t - h glycoside bindingen.
Zijn anti-bloedstollende eigenschappen zijn reeds veie jaren bekend 10 sedert zijn ontdekking en isolering uit veefsels in 1917·
Heparine is aanvezig in verschillende vormen in vele veefsels en cellen, meer of minder los gebonden aan een proteïnedeel. In de huid en, ten dele in de longen van verschillende speciën is heparine aanvezig in . r een hoog-moleculaire vorm, die gevoelig is voor de inwerking van ascor-15 bine zuur of enkele enzymen die in het darmslijmvlies aanvezig worden geacht, lia behandeling met ascorbinezuur of met darmslijmvlies homoge-naten kunnen deze macromoleculaire vormen van long- of huid-heparine worden verkleind tot dezelfde moleculaire grootte als het heparine dat uit het darmslijmvlies kan worden geïsoleerd.
20 Het heparine geïsoleerd uit darmslijmvlies met een gemiddeld mole- cuulgevicht (MW) van 15.000 Dalton, of heparinen uit andere bronnen verminderd tot dit (MW) door enkele der bovenbeschreven conventionele methoden stellen de minimale grootte van de natuurlijke moleculen van heparine voor. Volgens de uitvinding wordt een zodanig type heparine gede-25 finieerd als normaal heparine. Er bestaan namelijk in het lichaam geen enzymen,de het heparine uit het darmslijmvlies tot lage moleculaire fracties kunnen splitsen,noch zijn tot dusver geen chemische methoden ontwikkeld , waarmede het normale heparine molecuul zonder verlies van zijn biologische activiteit kan worden gedepolymeriseerd.
30 Slechts bepaalde microbiële enzymen, bijvoorbeeld heparinase uit
Flavobacterium henarinum zijn in staat het heparine-molecuul geheel tot gemodificeerde öisaccharide-eenheden te dissociëren. Deze methode is toe-gepast in structuurstudie' s doch is ongeschikt gebleken voor het ver-l' krijgen van nieuwe biologisch actieve moleeulen.
I 35 Doel van de uitvinding is het verschaffen van een werkvijze,waar- I! mede het mogelijk is uitgaande van normaal heparine lager moleculaire II 81 00 35 4 -2- heparinefracties te verkrijgen, die begiftigd zijn met hoge farmaceutische activiteit.
Hiertoe wordt -volgens de onderhavige uitvinding een -werkwijze voorgesteld, gekenmerkt door de volgende trappen: 5 a) aanzuren van normaal heparine ter verkrijging van heparinezuur, b) het depolymeriseren van genoemd heparinezuur door verhitting in een autoclaaf bij aanwezigheid van peroxyden ter verkrijging van een lager moleculair heparamine, c) het sulfateren van genoemd heparamine ter verkrijging van het over-10 eenkomstig laag-moleculaire heparine.
Een reacties chema is af geheeld op het formuleblad. Hierin is m ^ n,en 74, m 4 22 wanneer ni=26,7 wat de n waarde is corresponderend met heparine van MN ss· 15 · 000 D.
Zij wordt berekend als volgt: 15

562 waarin 562 het MW van de disaccharide-eenheid is (van de chemische formule voorgesteld door de formules 1 en k van het 20 formuleblad.
Volgens het reactieschema geeft de formule 1 het hatriumzout van normaal heparine aan; eenvoudigheidshalve toont deze formule slechts D-glucuronzuur en niet het deel van het molecuul dat L-iduronzuur bevat. Evenzo kan elk ander een- of tweewaardig kation in plaats van het na-25 triumkation aanwezig zijn.
Het genoemde heparine 1 wordt aangezuurd ter verkrijging van zijn vrij zuur, het .· heparinezuur van de formule 2 volgens bovenbeschreven trap a).
Het heparinezuur van de formule 2 wordt gedepolymeriseerd vol-30 gens trap b), met verlies van het merendeel van de N-sulfaat-groepen en met vorming van een heparamine van de formule 3.

Tenslotte wordt het heparamine van de formule 3 behandeld volgens trap c) door herstel van de N-sulfaat-groepen ter verkrijging van het eindprodukt volgens de uitvinding, namelijk de heparinen van formule 35 die worden omschreven als LMW (laag-moleculair gewicht) heparinen of ΘΒϋ (gereconstitueerde gedepolymeriseerde) heparinen.

Volgens een meer specifieke beschrijving van de werkwijze vol-| gens de uitvinding wordt het genoemde heparinezuur verkregen uit normaal I heparine 1 door eenvoudig aanzuren of bij voorkeur door het behandelen » 8100354 -3- met kati on uitwis s elhar s en.
Het heparinezuur 2 gaat dan via de trap b) Tan depolymerisatie die wordt uitgevoerd in een autoclaaf bij aanwezigheid van een peroxyde, bijvoorbeeld Ηρ0^, door verhitten onder een druk van bij voorkeur tussen 5 1 en 2 atn., waarbij de autoclaafbehandeling op vooraf bepaalde tijden kan worden gestopt (bijvoorbeeld na 15» 30, 60, 120 en 2U0 minuten).
Op deze wijze is het mogelijk eindprodukten te verkrijgen, namelijk IMW of RD.heparinen, met verschillende gemiddelde molecuulgewichten MW in het gebied van ongeveer tussen J2.000 en t.000 D, uitgaande van normaal 10 heparine van MW = 15*000 D. De opbrengsten van genoemde depolymerisatie zijn dezelfde ( ongeveer 65 gew.jS) in alle gevallen. De afname in MW wordt gevolgd door titratie van de eindreducerende groepen (Somogji’s methode) of door afname van de specifieke viscositeit.
De produkten van genoemde depolymerisatie zijn laag-moleculaire 15 heparinen 3 , waarvan het molecuulgevicht kleiner is dan dat van het overeenkomstig uitgangsheparine 1 naar beneden toe tot ongeveer 1/3 van de voorafbepaalde verschillende autoclaveringstijden.
Daar in genoemde depclymeriseringstrap en als eerder is ‘vermeld het grootste deel van de aan de stikstof gebonden sulfaatgroepen ver-20 loren gaat, wordt het heparamine 3 tenslotte na afkoelen van het reae-tiemengsel en verhogen van de pH aan herstel van de actieve zwavelzuur— groepen onderworpen voor het verkrijgen van het eindprodukt ^ volgens genoemde trap c) volgens bekende methoden zoals bijvoorbeeld door reactie met sulfotrioxyden van stikstof-bevattende basen (bijvoorbeeld pyri-25 dine sulfotriosyde, trimethylamine sulfotrioxyde enz.) bij aanwezigheid van alkali-earbonaten of door reactie met chloorsulfonzuur en stikstof-bevattende basen.
De opbrengsten van de eindprodukten ongeacht de autoclaveringstijden zijn steeds dezelfde in gewicht uitgedrukt betrokken op de uit-30 gangshoeveelheid heparine.
De uitvinding wordt nader toegelicht aan de hand van de onderstaande voorbeelden.
Voorbeeld I.
Een hoeveelheid van 20 g natrium- of calciumheparine (antibloed- 3 35 stollingsactiviteit = 150 Iü/mg) werd opgelost in 100 cm gedeïoniseerd 3 water en gebracht in een kolom bevattend 100 ca Amberlite (fT) IRC 120 in # _+ f h vorm .
it i 3 ^ e J De kolom werd gewassen met 100 cm gedeïoniseerd water en de vloei- 8100354 -ll·- stoffen werden opgevangen. De pH van de oplossing bedroeg 1 - 1,5· 20 cm^ van een verzadigde waterstofperoxyde-oplossing (36 %) werden toegevoegd en de vloeistof werd. verhit in een autoclaaf gedurende 15 minuten onder 1 atm. 5a afkoeling tot kamertemperatuur werd de pH verhoogd tot 5 7,2 met 2N NaOH; 16 cm pyridine en 16 g pyridine sulfotrioxyde werden onder roeren toegevoegd.
De reactie duurde 12 uren met een geringe toevoeging van pyridine om de pH tussen 5 en 6 te houden. Daarna werd de pH verhoogd tot 8 met . · · 3
NaOH en werd de oplossing geconcentreerd in vacuo tot 100 cm om de over- 10 maat pyridine te verwijderen. Zouten werden verwijderd:in een gemengd-bed ionuitwisselkolom en de oplossing werd steriel gefiltreerd en gevriesdroogd.
Opbrengst: 65 % als wit poeder met een anti-bloedstollingsactivi-teit van 85 - 100 IU/mg en een MW (Scmogji methode) die 2/3 van het oor-15 spronkelijke bedroeg.
Voorbeeld II.
20 g natrium- of calciumheparine, 150 IU/mg werden opgelost in 3 ... . . . . ' 100 cm water voor injectie en gedekatiomseerd op een kation-uitwissel- hars in H vorm.
20 Aan de oplossing (pH - 1 - 1,5) werd toegevoegd 20 cm perazijn- zuur (Uo % oplossing) en het verkregen mengsel werd daarna geautoclaveerd gedurende 60 minuten onder 1 atm.
Na afkoelen en verhogen van de pïï tot 7,2 werden 20 g trimethyl-amine sulfotrioxyde + 20 g Na^CO^ toegevoegd. Het reactiemengsel 25 werd vele uren geroerd bij een temperatuur die boven 60°C is en daarna door een Dowex Retardión (if) 11 A 8 kolom geleid. De· pH van de oplossing werd ingesteld op 6, de oplossing werd afgekoeld en 3 vol.dln ethanol werden toegevoegd. Het witte neerslag werd verzameld , gewassen met ethanol, methanol of aceton en in vacuo gedroogd.
30 Het eindprodukt bezat een anti-bloedstollingsactiviteit van 35 - 50 IU/mg en een MW (Somogji methode)die 1/3 is van het oorspronkelijke.
Voorbeeld III.
Door te werken als voorgeschreven in voorbeeld I, doch met de vol-^ 35 gende werkwijze-omstandigheden: autoclaveren gedurende 30 minuten met 2 atm .werd een eindprodukt verkregen met een anti-bloedstollingsactivi-\ teit van 35 - 50 IU/mg en een MW die ongeveer 1/3 van het oorspronkelijke \ bedraagt.

Het wordt voor het eerst vermeld, dat het produkt b. een chemische samenstelling (gehalte aan sulfaten, hexosaminen, hexuron zuren) en bepaalde fysisch-ehemische eigenschappen (specifieke optische rotatie)-bezit die identiek zijn aan die van het gezuiverde heparine 3, doch een 5 verschillende waarde geeft bij de titratie van de eindreducerende groepen (Somcgji methode), een verschillende elektroforetische-mobiliteit εν verschoven golflengte-absorptie maxima in de complexen gevormd met basische kleurstoffen, bijvoorbeeld met tolüIdine~blauw.
De LMW heparinen (RD heparine) verkregen volgens de uitvinding ver-TO tonen zeer belangwekkende farmacologische en therapeutische eigenschappen vergeleken met normaal heparine 1. De belangrijkste ervan is een gewijzigde verhouding hetzij in vitro of in vivo (in experimentele dieren of bij de mens), tussen de anti-thrombotische en de totale anti-bloedstol- lende activiteiten als bepaald door de verhouding--±j[?CT-“ waarin: 35 Anti-XA. test = Yin’s test; APTT = geactiveerde partiële thromboplastine-tijd.
De verschillende RD heparinen vertonen toenemende waarden van , , anti-thrombotische activiteit ^ Λ de vernouoing . . -— -— -r-,—r— met afnemenae Mw totale anti-bloedstoilende activiteit ofschoon de specifieke anti-bloedstollingsactiviteit afneemt met afne-20mend MW'.
Steeds heeft een normaal heparine met MW - 15·000 met een totale anti-bloedstollende activiteit 150 lU/mg en een verhouding anii-Xa/APTT =3, terwijl RD heparinen volgens de uitvinding een totale anti-bloed-stollende activiteit ^ 3 50 IU/mg en een Anti-Xa/APTT 1 hebben. In het bij-25 zonder is de verhouding nagenoeg het tweevoudige voor verbinding k vergeleken met verbinding 1. Dit betekent dat op basis van dezelfde anti-bloedstollingsactiviteit de anti-thrombotische activiteit van verbinding • ktweemaal die van normaal heparine is, met andere woorden in termen van anti-bloedstollingsactiviteit is de helft van de dosis van verbinding t 30 of zelfs minder vereist om dezelfde anti-thrombotische werking te krijgen als wordt verkregen met een hele dosis normaal heparine.
Deze getallen zijn bevestigd bij proefdieren door het bepalen van de beschermende werking van intraveneus toegediende verbindingen 1 en b tegen de vorming van thrombine- en door thromboplastins opgewekte thromui.
35 Daarom is het in de familie van RD heparinen die kunnen worden ^jjj^ verkregen met de onderhavige werkwijze mogelijk het meest geschikte pro- \ dukt te kiezen in termen van specifieke anti-bloedstollende activiteit en - 1 anti-thrombotische , -· . , - ...
1 ————:— verhouding voor speciale toenassmg op het gebied van \ anti-bloedstolling ‘ de be- 8100354 -6- scherming tegen thrombose.
Bijvoorbeeld kan een patiënt in thrombogene toestand met een sterk bekorte bloedstollingstijd een anti-thrombcdsch middel vereisen met een betrekkelijk hoge anti-bloedstollingsactiviteit£ervij'l een pa-5 tiënt die aan een chirurgische ingreep-moet worden onderworpen, waarbij zowel de risico's van bloedingen als post-operatieve thrombose bestaan een anti-thrombotisch middel met lage bloedstollende activiteit kan vereisen.
Voorts blijft na subcutane toediening bij de mens verbinding 10 . k langer’in de bloedstroom dan verbinding ·]. Verschillende proeven, namelijk recalcificatietijd , geactiveerde partiële thromboplastine-tijd (APTT)jthrombinetijd, Anti-Xa activiteit, werden toegepast om na te gaan hoelang verbindingen van de formule i1 in de bloedstroom blijven.
15 Bovendien toont verbinding 11 enige orale absorptie bij bepaalde diersoorten (rat, muis, honden), terwijl verbinding 1 niet oraal wordt geabsorbeerd.

- 2. Werkwijze volgens conclusie 1, met het kenmerk, dat de depoly-10 merisatie wordt uitgevoerd in een autoclaaf.
- 3. Werkwijze volgens conclusie 1, met het kenmerk, dat het sulfa-teren geschiedt door "behandeling met sulfotrioxyden van stikstof-bevattende' organische, basen bij aanwezigheid van een base. k. Werkwijze volgens conclusie 3, met het kenmerk, dat genoemde 15 base de vrije stikstof-bevattende basen zijn corresponderend met genoemd sulfotricxyde.
- 5. Werkwijze volgens conclusie 3, met het kenmerk, dat genoemde base een alkalicarbonaat is.
- 6. Werkwijze volgens conclusie 1, met het kenmerk, dat genoemde 20 sulfatering wordt uitgevoerd door behandelen met chloorsulfonzuur en stikstof-bevattende organis che basen.
- 7. Laag-moleculare hepamen verkregen met de werkwijze volgens conclusies 3-6, met het kenmerk, dat zij de volgende eigenschappen bezitten:
