위장관 또는 기타 부위에서 약물이 흡수되기 위하여는 분자형태의 약물이 장벽 막을 통과하여야 한다. 먼저 약물이 용해되고 약물이 원하는 생물약학적 특성을 가지면 고농도 영역에서 저농도 영역으로 막을 통과하여 혈액 또는 대순환에 진입될 것이다. 모든 생물학적 막은 주성분으로 지질을 함유한다. 막 형성에 중요한 역할을 하는 분자들은 모두 강한 극성의 인산염-함유 머리 그룹 및, 대부분의 경우, 두 개의 강한 소수성의 탄화수소 꼬리를 가진다. 막은 이중층이고, 친수성 머리그룹은 어느 한쪽의 수용성 영역을 향한다. 강한 친수성 약물은 막의 소수성 층을 통과할 수 없고 강한 소수성 약물은 유사성으로 인하여 막의 일부로서 소수성 층에 체류될 것이고 세포질 내부로 효율적으로 진입될 수 없다.
본 발명의 목적은 위액에서 NSAIA 용해도를 높여 경구 투여 가능하게 하며 막 및 피부 장벽을 통한 NSAIA 침투율을 높여 경피 투여가 (국소적 적용) 가능하게 하여 NSAIA의 부작용을 회피하는 것이다. 본 발명의 가장 중요한 목적은 피부, 세포막 특히 뇌세포 및 신경세포막을 매우 효율적으로 침투하여 대순환에 휠씬 짧은 동안 체류하여 모약물보다 수백 배 더 효능을 가질 수 있고 통상 약물 투여량보다 수십 분지일 또는 수백 분지일 정도의 투여량만이 요구되어 부작용이 거의 없는 NSAIA 약물전구체를 설계하는 것이다. 이러한 발명은 단지 경피 약물전달만이 아니라 기타 약물전달체계 (경구, 피하, 정맥내, 흡입 또는 비강)에서도 유리하고 모약물로 치료될 수 없는 많은 병태들을 치료할 수 있다. 이러한 새로운 NSAIA 약물전구체는 공통적인 두 가지 구조적 특징들을 가진다: 이들은 친유성 부분 및 생리적 pH에서 양성자화 형태 (친수성 부분)로 존재하는 1급, 2급 또는 3급 아민 그룹을 가진다. 이러한 친수성-친유성 균형은 막 장벽을 거치는 효율적 통과에 필요하다 [Susan Milosovich 등. J. Pharm. Sci., 82, 227(1993)]. 양전하의 아미노 그룹들은 약물의 수용해도를 크게 높인다. 이들 약물전구체 아미노 그룹의 양전하는 막의 인산염 머리 그룹의 음전하와 결합될 것이다. 따라서, 막 또는 피부 외부의 국부적 농도는 매우 높아서 이들 약물전구체들은 고농도 영역에서 저농도 영역으로 통과하기 용이할 것이다. 이러한 결합은 막을 약간 교란시킬 것이며 약물전구체의 친유성 부분을 위한 약간의 공간을 만들 수 있다. 막 분자들이 움직일 때, 막은 약물전구체 결합으로 약한 균열된다. 이로써 약물전구체는 막 내부로 삽입된다. pH 7.4에서, 아미노 그룹의 약 99%만이 양성자화 된다. 아미노 그룹이 양성자화 되지 않을 때, 약물전구체의 아미노 그룹 및 막의 인산염 머리 그룹 사이 결합은 끊어지고, 약물전구체는 막으로 완전히 진입될 것이다. 약물전구체의 아미노 그룹이 막의 다른 측으로 들어가고 따라서 양성자화 되고, 이후 약물이 반-액상 농축 수용액 또는 현탁액인 세포질 내로 끌려 들어온다. GI 관에서 짧게 체류되므로, 약물전구체는 위점막 손상을 일으키지 않는다. 새로운 약물전구체의 인간 피부 침투율은 시험관내에서 인간 넓적다리 앞 및 뒤 영역의 피부조직(360-400 ㎛ 두께)에서 분리된 변형 Franz 세포를 이용하여 측정되었다. 생리식염수 중의 2% 소혈청 알부민 10ml로 이루어진 수용 유체는 600rpm으로 교반되었다. 시간 경과 피부 진입 약물전구체 및 약물의 축적 함량들은 소정의 고성능 액체 크로마토그래피로 결정되었다. NSAIA 약물전구체 겉보기 유동 수치는 0.1-50 mg/cm2/h이다. 결과는 약물전구체는 모약물보다 최소한 백배 이상 신속하게 인간 피부를 통하여 분산된다는 것을 보여준다. 이 결과는 디알킬아미노에틸 그룹상의 양전하가 막 및 피부 장벽을 거쳐 약물이 통과하는데 있어 매우 중요한 역할을 한다는 것을 보여준다.
새로운 NSAIA 약물전구체는 피부 장벽, 혈액-뇌 장벽 및 혈-유 장벽을 침투할 수 있다. 약물전구체 및 모약물의 무손상 랫트 피부를 통한 생체내 침투율이 비교되었다. 도너는 1ml 에탄올 중의 20% 약물전구체 또는 모약물로 이루어지고 랫트 (~200g) 등 부위 5cm2 영역에 적용되었다. 4시간 후, 랫트를 죽이고, 메탄올 5ml를 1ml 혈액, 1g 간, 1g 신장 또는 1g 뇌 (간, 신장 또는 뇌는 pH 7.2 완충액으로 3회 세척되었다)에 첨가하였고, 혼합물은 균질화되었다. 시료들은 5분간 원심 분리되고 HPLC로 분석하였다. 결과는 표 1-5에 도시된다.
그리고 1ml 에탄올 중의 20% 아스피린, 디클로페낙, 케토프로펜, 페노프로펜 또는 이부프로펜은 랫트의 등 부위 5cm2 영역에 적용되었다. 4시간 후, 랫트를 죽이고, 메탄올 5ml를 1ml 혈액, 1g 간, 1g 신장 또는 1g 뇌 (간, 신장 또는 뇌는 pH 7.2 완충액으로 3회 세척되었다)에 첨가하였고, 혼합물은 균질화되었다. 이들 약물 중 어떤 것도 랫트 조직 또는 혈장에서 발견되지 않았다. 이러한 결과는 NSAIA 약물전구체는 피부 장벽, 혈액-뇌 장벽 및 기타 막 장벽을 매우 높은 비율로 침투할 수 있지만 모 NSAIA는 피부 장벽을 검출 가능한 정도로 침투할 수 없다는 것을 보인다.
일반식 (1) 구조 1의 약물전구체는 본 발명자들 특허 (국제출원번호들: PCT/IB2006/052732, PCT/IB2006/052318, PCT/IB2006/052815, PCT/IB2006/052563, PCT/IB2006/052575, PCT/IB2006/053741, PCT/IB2006/053091, PCT/IB2006/053090, PCT/IB2006/052549)에서 소염, 진통, 해열, 및 항류마티스 활성을 보였다. 본 발명자들은 일반식 (2a, 2b, 2c, 또는 2d) "구조 2a, 2b, 2c, 또는 2d"의 모든 약물전구체들은 소염, 진통, 해열, 및 항류마티스 활성을 보인다는 것을 알았다. 본 발명의 주요 목적은 NSAIA 약물전구체의 의료적 용도이다.
염증 및 암과의 관계는 공지되어 있다. Thea D. Tlsty 박사는 연설에서 (Keystone Symposia: Inflammation and Cancer, Breckenridge, Colorado, USA, Feb. 27-March 3, 2005) 사이클로옥시게나제-2 (COX-2)는 아로마타제 활성, 혈관형성, 증식, 침입 및 프로스타글란딘 합성을 자극한다고 발표하였다. 프로스타글란딘 증가는 세포자멸사를 억제한다. 아스피린 및 기타 SNAIA는 COX-1 COX-2를 억제한다. 장기간 아스피린을 복용한 사람들은 대장암, 식도암, 난소암 또는 기타 암에 대한 총체적 상대위험도는 감소된다. 그러나, 암세포는 막 구조를 변형시켜 NSAIA가 암세포로 진입되지 않도록 한다. 본 발명의 새로운 약물전구체는 임의의 막 장벽을 침투할 수 있고 암 부위 피부 영역 외부로 국부적으로 적용될 수 있어 다량의 약물전구체가 거의 전신 노출 없이 암세포로 들어갈 것이다.
NSAIA 약물전구체의 항-종양 활성을 평가하기 위하여, 인간 유방암세포 (BCAP-37, 2-3 mm3의 종양조직이 각각의 생쥐에 사용되었다)이 누드 생쥐(BALB, 12 군들, 각 군당 7마리 생쥐)에 피하로 이종 이식되었다. 14 일 후, 종양은 5010 mm3 (0.05 ml) 크기로 성장되었다. 이후 30l의 5% (약물전구체 1.5mg와 동등) 디에틸아미노에틸 아세틸살리실레이트. 아세틸살리실산염 (P-1, 아세톤 중); 1-피페리딘프로필 2[(2,6-디클로로페닐)아미노]벤젠아세테이트. 두 번째 항-종양 실험에서, 인간 결장암세포 (LS174J, 2-3 mm3 의 종양조직이 각각의 생쥐에 사용되었다)이 누드 생쥐(BALB)에 피하로 이종 이식되었다. 7일 후, 종양은 5510 mm3 (0.055 ml)크기로 성장되었다. 이후 약 30μ의 5% (약물전구체 1.5 mg과 동등) 디에틸아미노에틸 아세틸살리실레이트. 아세틸살리실산염 (P-1, 아세톤 중); 1-피페리딘프로필 2[(2,6-디클로로페닐)아미노]벤젠아세테이트. 결과는 NDAIA 약물전구체는 매우 강한 항-종양 활성을 가지며 부작용이 거의 또는 전혀 없음을 보여준다.
살리실산염의 저혈당효과는 100년 전 독일의사들 (Edmund J. Hengesh, Principles of medicinal chemistry, 4th ed., pg 591, Williams & Wilkins, 1995)에 의해 처음 관찰되었다. 살리실산염은 글루코스-자극 인슐린 분비를 촉진하며 젖산 및 알라닌에 의해 글루코스 생성을 억제한다 (H. F. Woods 등., Clin. Exp. Pharmacol Physiol., 1, 534(1974). 소정의 살리실산염은 유리 지방산, 트리글리세리드 및 콜레스테롤의 혈장 수준을 감소시킨다. 높은 수준의 혈장-유리 지방산은 글루코스 활용을 억제하므로 이들 농도 감소는 저혈당작용에 기여한다. 그러나, 혈당수준 및 혈액지질수준을 적당히 조절하기 위하여 많은 살리실산염 투여 함량 (매일 5g)이 필요하다. 이러한 투여수준에서는 위세척, 구역질, 구토 및 귀울림과 같은 여러 부작용들이 발생한다. 본 발명의 새로운 약물전구체는 매우 높은 피부 및 막 침투율을 가진다. 이들은 매우 신속하게 작용부위에 도달될 수 있으며 이들 약물전구체의 약리효과 및 임상반응은 크게 증진되며, 따라서 훨씬 소량 (필요한 모약물의 백 분지일 내지 십 분지일만)의 약물 투여로 족하고 부작용은 훨씬 감소될 것이다.
본 발명의 약물전구체는 랫트 모델 (SLAC/GK, 타입 2 당뇨병, n=7) 혈당수준을 낮춘다. 디에틸아미노에틸 아세틸살리실레이트. 아세틸살리실산염 50% 아세톤 용액 (P-1, 아세톤 중); 4-아세트아미도페닐 살리실일디메틸아미노부틸레이트.HCl (P-6), 디에틸아미노에틸 5-(2,4-디플루오로페닐) 아세틸살리실레이트.5-(2,4-디플루오로페닐) 아세틸살리실산염 (P-8), 디에틸아미노에틸 살리실살리실레이트.AcOH (P-9), 디에틸아미노에틸 살리실레이트.AcOH (P-10), 디에틸아미노에틸 5-아세트아미도-아세틸살리실레이트 (P-58), 디에틸아미노에틸 아세틸살리실살리실레이트. 아세틸살리실살리실산염 (P-59), 디에틸아미노에틸 아세틸살리실살리실레이트. 아세틸살리실살리실살리실산염 (P-60) (NSAIA 20mg/kg 와 동등)이 랫트 (모피가 제거된) 등 부위에 (약 1.5 cm2) 매일 1회 (8 am) 5주 동안 경피 투여되었다. 혈당수준은 2주부터 5주까지 매 3일 1회 3 pm (공복없슴)에 측정되었다. 결과는 표 10에 도시된다. 혈액지질수준은 5주째 측정되었다. 결과는 표 11에 도시된다.
결과는 NSAIA의 약물전구체는 당뇨 랫트 모델에서 혈당수준을 매우 효과적으로 낮추고 정상 랫트의 혈당수준에는 영향을 미치지 않는다는 것을 보여준다. 가장 흥미로운 사실은 치료를 중지하고 30일 동안 랫트 혈당수준이 정상수준(7-8 mmol/L, 공복없슴)을 유지하였다는 것이다. 이것은 약물전구체는 혈당수준을 낮출 뿐 아니라 당뇨병을 치료할 수 있다는 것을 의미한다.
결과는 NSAIA 약물전구체는 당뇨 랫트모델에서 혈액지질수준(총 콜레스테롤 및 트리글리세리드)을 낮추지만 HDL 수준에는 영향을 미치지 않는다는 것을 보였다.
위액 pH는 1-3이다. 막의 인산염 머리 그룹 음전하는 양성자에 의해 중화되고 약물전구체의 아미노 그룹 양전하는 막의 인산염 머리 그룹과 결합될 수 없어, 약물전구체는 위벽을 통과할 수 없고 위를 손상 또는 위연동이상항진(upset)시키지 않을 것이다. 십이지장의 pH는 약 5-7이고 약물전구체는 십이지장 점막을 통과할 수 있다. 췌장이 가까이 있어 다량의 약물전구체는 이들이 대사되는 간, 신장 및 대순환에 들어가기 전에 여기로 진입되므로 이들 약물전구체의 매우 적은 투여함량만이 필요하고 부작용은 거의 없거나 낮을 것이다. 디에틸아미노에틸 아세틸살리실레이트. 아세틸살리실산염 20% 아세톤 용액 (P-1, 아세톤 중); 4-아세트아미도페닐 살리실일디메틸아미노부틸레이트.HCl (P-6), 디에틸아미노에틸 5-(2,4-디플루오로페닐) 아세틸살리실레이트.5-(2,4-디플루오로페닐) 아세틸살리실산염 (P-8), 디에틸아미노에틸 살리실살리실레이트.AcOH (P-9), 디에틸아미노에틸 살리실레이트.AcOH (P-10), 디에틸아미노에틸 5-아세트아미도-아세틸살리실레이트 (P-58), 디에틸아미노에틸 아세틸살리실살리실레이트. 아세틸살리실살리실산염 (P-59), 디에틸아미노에틸 아세틸살리실살리실레이트. 아세틸살리실살리실살리실산염 (P-60) (NSAIA 15mg/kg 와 동등)이 사료와 혼합되어 랫트(SLAC/GK, 타입2 당뇨병, n=7)에 5주 동안 경구 투여되었다. 혈당수준은 2주부터 5주까지 매 3일 1회 3 pm (공복없슴)에 측정되었다. 결과는 표 12에 도시된다. 혈액지질수준은 5주째 측정되었다. 결과는 표 13에 도시된다.

본 발명의 약물전구체는 생쥐 모델 (SLAC: NOD-IDDM, 타입 1 당뇨병, n=7) 혈당수준을 낮춘다. 디에틸아미노에틸 아세틸살리실레이트. 아세틸살리실산염 50% 아세톤 용액 (P-1, 아세톤 중); 4-아세트아미도페닐 살리실일디메틸아미노부틸레이트.HCl (P-6), 디에틸아미노에틸 5-(2,4-디플루오로페닐) 아세틸살리실레이트.5-(2,4-디플루오로페닐) 아세틸살리실산염 (P-8), 디에틸아미노에틸 살리실살리실레이트.AcOH (P-9), 디에틸아미노에틸 살리실레이트.AcOH (P-10), 디에틸아미노에틸 5-아세트아미도-아세틸살리실레이트 (P-58), 디에틸아미노에틸 아세틸살리실살리실레이트. 아세틸살리실살리실산염 (P-59), 디에틸아미노에틸 아세틸살리실살리실레이트. 아세틸살리실살리실살리실산염 (P-60) (NSAIA 30mg/kg 와 동등)이 생쥐 (모피가 제거된) 등 부위에 (약 1.5 cm2) 매일 1회 (8 am) 7주 동안 경피 투여되었다. 혈당수준은 4주부터 7주까지 매 3일 1회 3 pm (공복없슴)에 측정되었다. 결과는 표 14에 도시된다.
결과는 NSAIA의 약물전구체는 당뇨 (타입 I) 생쥐 모델에서 혈당수준을 매우 효과적으로 낮춘다는 것을 보였다.
3.0 내지 3.5kg의 중국 화이트 래빗 18마리 (연령 6-7 개월)을 선택하여 3군으로 나누었다 (대조, P-1 및 P-10 군, n=6). 실험 1시간 전, 정맥혈액(1ml)을 무균 병으로 흡입시켜 응고시킴으로써 혈전을 만들었다. 균열 및 서서히 용혈되는 것을 방지하기 위하여, 자가혈괴는 10분 동안 온도-제어 (70℃) 증류수에서 안정화시켰다. 마취 후, 대퇴정맥을 노출시켜 원위 분리시키고, 미리 대퇴정맥에 배치된 유치도관 (20GA)을 통하여 자가혈괴 (0.05 g/kg)를 주입시켰다. 디에틸아미노에틸 아세틸살리실레이트. 아세틸살리실산염(P-1, 아세톤 중, 20 mg/kg)의 50% 아세톤 용액 및 디에틸아미노에틸 아세틸살리실살리실레이트. 아세틸살리실살리실산염 (P-59, 20 mg/kg) 이 래빗 등 부위에 국소 투여되었다. 2일 후, 래빗을 소듐 아모발비탈 (60 mg/kg)을 과량 정맥내 주사하여 래빗을 안락사시켰다. 폐동맥에 혈전이 존재하는지를 확인하기 위하여 폐 및 심장을 적출하였다. 폐는 10% 포르말린에 24 시간 담가두었다. 폐쇄 폐동맥을 따라 광범위(Consecutive) 횡단면이 파라핀-함몰되고 헤마톡실린-에오신으로 염색되었다. 대조군에서는, 혈소판 혈전 및 혼합 혈전이 대형혈관에도 존재하는 주입 응고체를 포위하였고 혈관벽에 근위 및 원위 양방향으로 이어졌다. 이들 혈관에는 내피세포 및 섬유모세포의 과잉증식이 있었다. 또한 급성폐울혈이 있었다. P-1 및 P-59 군에서, 폐 조직 및 혈관벽은 정상이었다. 이들 결과는 혈전 활성 및 색전형성-관련 혈전 전파는 NSAIA 약물전구체로 예방될 수 있다는 것을 보였다. 이들 약물전구체는 뇌졸증, 심근경색, 및 장기이식거부의 주 원인인 혈괴 예방 및 치료에 매우 유용할 수 있다.
본 발명의 약물전구체는 래빗 모델에서의 상처 치료 및 벤 곳 및 화상으로 인한 흉터 연화 및 축소에 조력할 수 있다. 약물전구체로 치료된 래빗의 평균 흉터 크기는 중국 화이트 래빗 모델에서의 동일 크기 벤 상처 흉터의 1/3이고 흉터는 정상적인 비-흉터 조직과 같이 연화되었다.
COX-1 및 COX-2는 동물의 면역-반응에 매우 중요한 역할을 한다. NSAIA는 COX-1 및 COX-2를 억제한다. 본 발명의 NSAIA 약물전구체는 건선, 원판상 홍반루푸스, 전신홍반루푸스 (SLE), 및 기타 자가면역성 질환 치료에 매우 유용할 수 있다. 전풍균 농도가 진한 부유액[Rosenberg, E.W. 등, Mycopathologia, 72, 147-154 (1980)]이 중국 화이트 래빗 (n=4 x 6) 제모 등 피부에 매일 2회 (7 am 및 7pm) 2주 동안 투여되어, 건선과 유사한 환부가 형성되었다. 이후 3-피페리딘메틸 2-(-이소부틸페닐) 프로피오네이트. AcOH (P-15), 디에틸아미노에틸 2-아미노-3-(4-브로모-벤조일)벤젠아세테이트. AcOH (P-17) 디에틸아미노에틸 3-클로로-4-(2-프로페닐옥시)벤젠아세테이트. AcOH (P-18), 디에틸아미노에틸 1-(4-클로로벤조일-5-메톡시-2-메틸-1H-인돌-3-아세톡시아세테이트. AcOH (P-20), 디에틸아미노에틸 4-(4-chloro페닐)-2-페닐-5-티아졸아세테이트. AcOH (P-21), 디에틸아미노에틸 3-(4-chloro페닐)-1-페닐-1H-피라졸-4-아세테이트. AcOH (P-22)이 전풍균이 진한 부유액 투여 3시간 후 (10 am 및 10 pm) 동일 영역에 제공되었다 이들 약물전구체 투여 10 일 후, 환부가 치료되었다.
항-홍반루푸스 활성을 평가하기 위하여 5% 디에틸아미노에틸 아세틸살리실레이트. 아세틸살리실산염 (P-1, 아세톤 중, 30 mg/kg) 또는 3-피페리딘메틸 2-(-이소부틸페닐) 프로피오네이트. AcOH (P-5, 물 중, 30 mg/kg)이 원판상 홍반루푸스 및 전신 홍반루푸스가 있는 생쥐 (MRL/LPR, n=5 x 3) 등 피부에 매일 2회 적용시켰다. 6주 후, 약물전구체로 치료된 생쥐에서는 모든 피부 환부 및 루푸스신장염은 치료되었으나, 대조군 생쥐 병태는 더욱 악화되었다.
이러한 결과는 NSAIA 약물전구체는 인간의 건선, 원판상 홍반루푸스, 전신홍반루푸스 (SLE), 다발경화증 (MS) 및 기타 자가면역성 질환 치료를 위한 잠재적 약제임을 제안하는 것이다.
근육위축성 측삭경화증 (ALS) 에서의 세포사 발병기전은 글루탐산염-매개 흥분독성, 산화 손상, 및 세포자멸사를 포함한다. 척수 뉴런 및 별아교세포에 존재하는 사이클로옥시게나제-2는 프로스타글란딘 E2 합성에 촉매 역할을 한다. 프로스타글란딘 E2는 별아교세포로부터 글루탐산염 방출을 자극하고, 또한 사이클로옥시게나제-2는 프로-염증성 시토카인, 반응 산소종, 및 자유 라디칼 생성에 중요한 역할을 한다. 선택적인 사이클로옥시게나제-2 억제제, 셀레콕시브로 치료하면 ALS 생쥐 척수에서 프로스타글란딘 E2 생성이 억제된다. 셀레콕시브 치료는 체력 약화 및 체중 감소 발병을 상당히 지연시키고 생존율을 25% 연장시킨다. 치료된 ALS 생쥐 척수는 척수 뉴런의 상당히 보존되어 있고 성상교세포증 및 미세아교활성이 감소된다 (Merit. E. Cudkowicz, et al., Annals of neurology, 52, 771-778, 2002). 이러한 결과는 사이클로옥시게나제-2 억제는 ALS 환자에게 유리하다는 것을 제안한다. 본 발명의 NSAIA 약물전구체는 피부 및 신경 세포막 장벽을 매우 높은 비율로 침투할 수 있고 (대부분의 NSAIA는 신경세포를 효과적으로 침투할 수 없다) GI 관을 손상시키지 않고 경피 투여될 수 없어, 이들 약물전구체는 다발경화증 (MS), 크론병, 및 기타 자가면역성 질환, 근육위축성 측삭경화증(ALS), 눈인두근육퇴행위축증(OPMS), 근육긴장퇴행위축증(MD), 뒤시엔느 근위축증(DMD), 다발근육염 (PM), 피부근육염(DM), 봉입체근염 (IBM), 및 기타 근육장애를 치료용 잠재적 약제이다.
알츠하이머병의 병인론적 연쇄반응에서 염증 기전은 매우 중요한 매개체로 제안되었다 (McGeer PL, McGeer EG. 알츠하이머병 및 기타 신경변성질환 치료를 위한 뇌의 염증반응시스템. Brain Res. Rev., 1995; 21: 195-218). in't Veld 등. (the New England Journal of Medicine, 2001; 345, 1515)에 의한 연구에서, 알츠하이머병 위험이 있는 약 7000명 환자를 거의 7년에 걸쳐 연구하였다. 결과는, NSAIA는 치매 발병 전 최소 2년 및 2년 이상 NSAIA를 계속적으로 사용한 환자들의 상대적 위험도를 감소시킨다는 것을 제안하였다. 만일 NSAIA의 신경보호능이 치매 발병 몇 년 전에 끝난다면, 이들 화합물은 질환 조짐이 있는 대부분 환자들 중 진행에 대한 보호를 제공하지 못할 것이다. 본 발명자들은 이러한 이유로 손상 신경세포 주위 조직들이 흉터를 형성하여 더 이상의 손상으로부터 신경세포를 보호할 것이라는 것이다. 대부분의 NSAIA는 뇌-혈액 및 신경세포 장벽 침투율이 매우 낮으므로 이러한 흉터 장벽을 침투하지 못할 것이다. 본 발명의 약물전구체들은 매우 높은 피부, 혈액-뇌, 신경세포막, 및 흉터장벽 침투율을 가지며 알츠하이머병, 파킨슨병 및 기타 진행성 신경변성질환 치료에 있어 매우 잠재적인 약제이다.
손상된 척수 주위의 보호 흉터로 인하여 치료가 중단된 척수손상 환자에게 이들 약물전구체는 도움이 된다. 대부분의 NSAIA는 흉터 장벽을 치료학적 유효량으로 침투할 수 없지만, 본 발명의 약물전구체는 흉터 장벽을 침투할 수 있고, 항-염증성 활성을 가지며 상처 치료에 조력할 수 있다.
NSAIA는 상기 병태 치료에 매우 효과적이지 않거나 세포막, 특히 뇌세포 및 신경세포에 매우 효과적으로 침투하지 못하고 대순환에 너무 오래 체류되어, 대부분 약물은 작용부위에 도달되기 전에 장관 점막, 간, 신장 및 폐에 의해 대사될 것이다. 이러한 상황은 매우 낮은 약리효과를 보일 뿐 아니라 장관 점막, 간, 신장, 폐 및 신체 기타 부위를 중독시킨다. 본 발명의 약물전구체는 피부, 뇌-혈액, 뇌세포, 신경세포 및 기타 막 장벽을 매우 잘 침투하고 모약물보다 수 백배 효능을 가지며 정상약물 투여량의 수십 분지일 또는 수백 분지일의 적은 함량만이 필요하며 부작용이 훨씬 덜 할 것이다. 이것은 경피약물전달뿐 아니라 임의 기타 약물전달체계 (경구, 피하, 정맥내, 흡입 및 비강 등)에도 유리하며 모약물로 치료되는 것보다 더욱 양호하게 많은 병태를 치료할 것이고 모약물로 치료될 수 없는 병태조차도 치료할 수 있다.
상기 일반식 (1, 2a, 2b, 2c, 또는 2d) "구조 1, 2a, 2b, 2c, 또는 2d" 화합물들은 NSAIA 및 N, N'-디사이클로헥실카보디이미드, N, N'-디이소프로필카보디이미드 또는 기타 커플링 시약과 반응하여 무수물을 형성하고 적합한 알코올, 티올 또는 아민과 반응하여 제조될 수 있다. 상기 일반식 (1, 2a, 2b, 2c, 또는 2d) "구조 1, 2a, 2b, 2c, 또는 2d" 화합물들은 NSAIA의 금속염, 유기염기염 또는 고정화 염기염 및 적합한 할라이드 화합물과 반응하여 제조될 수 있다.
활성 성분으로써의 일반식 (1, 2a, 2b, 2c, 또는 2d) "구조 1, 2a, 2b, 2c, 또는 2d" 화합물들 또는 일반식 (1, 2a, 2b, 2c, 또는 2d) "구조 1, 2a, 2b, 2c, 또는 2d" 화합물들의 경피 치료학적 적용은 인간 또는 동물의 임의 NSAIA-치료가능한 병태 및 본 발명에서 기재된 임의 병태를 치료함에 유용할 수 있다. 이들 시스템은 하나의 활성 물질-함유 기층 및 불투과성 지지층을 가지는 붕대 또는 첩포일 수 있다. 가장 바람직한 시스템은 피부를 향하여 투과성 바닥을 가지는 활성물질 저장체이다. 방출속도를 조절함으로써, 이러한 시스템은 NSAIA가 일정하게 최적의 치료 혈액수준으로 도달하도록 하여 효능을 증대할 수 있고 NSAIA 부작용을 줄일 수 있다. 이들 시스템은 손목, 발목, 팔, 다리 또는 기타 신체 부위에 착용될 수 있다.

[실시예]
디에틸아미노에틸 아세틸살리실레이트. 아세틸살리실산염 제조
180g의 2-아세틸살리실산이 1000ml의 클로로포름에 용해되었다. 혼합물은 5℃로 냉각되었다. 103g의 1,3-디사이클로헥실카보디이미드가 혼합물에 첨가되었다. 혼합물은 실온에서 2시간 교반되었다 고상물은 여과로 제거되었고 클로로포름으로 세척되었다 (3 x 300ml). 59g의 디에틸아미노에탄이 반응혼합물에 첨가되었다. 혼합물은 실온에서 3시간 교반되었다. 유기용액은 증발되었다. 건조 후, 220g의 원하는 생성물을 수득하였다 (96%). 원소분석: C24H29NO8; MW: 459.18. 계산 % C: 62.73; H: 6.36; N: 3.05; O: 27.86; 검출 % C: 62.70; H: 6.40; Cl: N: 3.01; O: 27.90.
1-피페리딘프로필 2[(2,6-디클로로페닐)아미노]벤젠아세테이트. AcOH 제조
31.8g (0.1mol)의 소듐 2[(2,6-디클로로페닐)아미노]벤젠아세테이트가 180ml의 클로로포름에 현탁되었다. 28.6g (0.1 mol)의 1-피페리딘프로필 브로마이드.HBr가 혼합물에 첨가되었고 혼합물은 실온에서 5시간 교반되었다. 혼합물은 5% Na2CO3 (1 x 300ml) 및 물 (3 x 100 ml)로 세척되었다. 혼합물은 무수 Na2SO4상에서 건조되었다. 황산나트륨은 여과로 제거되었고 클로로포름으로 세척되었다(3 x 50ml). 6g의 아세트산이 용액에 첨가되었다. 용액은 진공 중 100ml로 농축되었다. 이후 300ml 헥산이 용액에 첨가되었다. 고상 생성물은 여과로 수거되어 헥산으로 세척되었다 (3 x 100ml). 건조 후, 40g의 원하는 생성물을 얻었다 (86%). 원소분석: C24H30Cl2N2O4; MW:481.43 계산 % C: 59.88; H: 6.28; Cl: 14.73; N: 5.82; O: 13.29; 검출 % C: 59.83; H: 6.32; Cl: 14.71, N: 5.79; O: 13.35.
3-피페리딘메틸 2-(-이소부틸페닐) 프로피오네이트. AcOH 제조
60g의 고분자-결합 트리에틸아민 (3mmol/g, 100-200 메쉬)가 180ml의 클로로포름에 현탁되었다. 20.6g (0.1 mol)의 2-(-이소부틸페닐) 프로피온산이 교반 되면서 혼합물에 첨가되었다. 39g (0.15mol)의 3-피페리딘메틸 브로마이드.HBr이 혼합물에 첨가되었고 혼합물은 실온에서 5시간 교반되었다. 고분자는 여과되어 제거되었고 아세톤으로 세척되었다 (3 x 50 ml). 300ml의 5% Na2CO3이 용액에 교반되면서 첨가되었다. 혼합물은 30분 교반되었다. 클로로포름 용액은 물로 세척 (3 x 100 ml) 및 Na2SO4상에서 건조되었다. 황산나트륨은 여과되어 제거 및 클로로포름으로 세척되었다 (3 x 100 ml). 6g의 아세트산이 혼합물에 첨가되었다. 용액은 진공 중에서 100ml로 농축되었다. 이후 300ml 헥산이 용액에 첨가되었다. 고체 생성물은 여과로 수거되어 헥산으로 세척되었다 (3 x 100ml). 건조 후, 35g의 원하는 생성물을 수득하였다 (96%). 원소분석: C21H33NO4; MW: 363.49 계산 % C: 69.39; H: 9.15; N: 3.85; O: 17.61; 검출 % C: 69.35; H: 9.18; N: 3.83; O: 17.64.
