Abstract

Description
본 발명은 경피 피분석물 감지 또는 약제 전달용 장치 및 방법에 관한 것이다.
관련 출원
본 출원은 2007년 4월 7일자로 미국특허청에 출원하고 발명의 명칭이, "경피 피분석물의 감지 또는 경피 약제 전달을 위한 피부 침투용 장치" 인, 미국특허출원번호 제60/914,552호의 우선권을 주장한다.
일반적으로 피부를 통한 약제의 침투는 가능한 한 매우 느린 속도로 일어난다. 이 과정에서 주요한 비율 제한 단계는 화합물이 피부의 최외층을 통과하는 경로이고, 각질층이라 불린다. 각질층은 이 층의 어느 쪽으로도 물질이 침투할 수 없는 죽은 세포의 얇은 층이다. 각질층은 주로 피부의 장벽 기능을 제공한다. 각질층의 손실 또는 변경은 많은 물질의 침투가능성을 높이는 결과를 초래한다. 즉, 물질은 피부안으로 또는 외부로 보다 더 쉽게 확산될 수 있다는 사실을 오래동안 이해해왔다. 피부의 장벽 기능은 약물의 경피 투여 또는 체액의 피부수집에 관심이 있 는 제약 제조사에게 매우 중대한 문제를 제공한다.
인간의 피부를 통한 전기적 신호와 생물학적 물질의 전달 및 수용도 각질층에 의해 방해를 받는다. 예를 들면, 피부를 통하여 측정되는 생체전기의 전위와 전류의 신호 정확성은 각질층의 높은 저항(impedence)에 의해 떨어진다. 따라서, 높은 저항은 인간세포, 기관(organs),및 조직(tissue)에서의 생체전기의 이상적인 전달 및 측정을 통하여 수신하는 데 문제를 발생시킨다.
각질층을 제거하면 피부의 높은 저항을 감소시키고, 인간 세포안으로 또는 그로부터 전기신호 또는 생물학적 종의 전달을 더 나아지게 한다. 또한 전자기 에너지 유도된 각질층 변경이 물질로 침투성을 증가시킨다는 것이 입증되어 왔다(Yaegashi의 미국특허 제6,315,722호, 플록 등의 미국특허 제6,215,100호, 마치토등의 미국특허 제6,056,738호, 와너 등의 미국 특허 제5,643,252 참조). 각질층을 침투하기 위하여 통상 "침투 강화제(permeation enhancers)"라고 불리는 화합물이 어느 정도 성공적으로 사용될 수 있다. 전통적인 접근 방식은 샌드 페이퍼(sand paper), 브러시(brushes)로써 피부를 연마하고, 테이프나 독성 화학물질로 피부를 박피하고, 레이저 또는 열적 제거에 의한 각질층의 제거 또는 바늘로 피부를 천공하는 것이 요구된다. 이들 방법에 의한 피부 준비는 매우 가변적이고, 위험하며 환자에게 고통스럽고 대체적으로 불편하다.
약물 전달 또는 피부를 통한 피분석물의 추출을 위한 피부준비의 종래 방식은 피부 준비의 정도를 제어하기 위한 외부 피이드백 기구를 필요로 한다. 실제로, 전기 전도성 결합 매체, 복귀 전극(return electrode) 및/또는 수화겔 패치가 조절 된 피부 준비를 피이드백 기구를 가능하게 하기 위하여 필요하다(마르키토 등의 미국 공개특허 제20060100567호, 마르키토 등의 미국 공개특허 제20030204329호 참조). 그러한 장치와 시스템의 신뢰성은 복귀 전극이 충분한 전도성을 가진 피부측에 위치할 때에만 정밀한 피이드백을 제공할 수 있기 때문에 의문스러울 수가 있다. 불행히도, 피부의 전도성은 나이, 위치, 태양 노출, 로션의 사용, 습도, 및 주위 온도 등과 같은 다양한 조건에 따라 변화한다.
그러므로, 피부의 높은 저항을 감소하기 위한 개선 시스템이 필요하다.
본 발명의 목적은 피부의 높은 저항을 감소하기 위한 개선된 시스템을 제공함에 있다.
본 발명의 다른 목적은, 피부의 임피던스(impedence)를 측정하기 위한 개선된 방법에 제공함에 있다.
본 발명의 또 다른 목적은 개선된 경피 약물 전달 및/또는 피분석물 감지 시스템을 제공함에 있다.
피부의 침투성의 증가를 위한 장치, 시스템, 키트(kits) 및 방법은 이하에서 설명된다. 이들은 경피 약물 전달 및 피분석물 추출 및 측정에 사용될 수 있다. 조절된 연마 장치는 (i) 핸드피스(hand piece), (ii) 연마 팁(abrasive tip), (iii) 피이드백 제어기구(feedback control mechanism), (iv) 둘 이상의 전극, (v) 전기모터를 포함한다. 바람직하게는 상기 피이드백 제어기구는 내부 피이드백 제어이다. 이 실시예에서 연마 팁은 2개의 전극, 즉 소스 전극(source electrode)와 복귀 전극(return electrode)를 포함한다. 다른 실시예에서는, 상기 피이드백 제어기구는 외부 피이드백 제어이다. 바람직한 실시예에서, 상기 연마 장치는 2개의 전극의 동축 또는 동심 배치를 포함한다. 이 실시예에서 연마팁은 소스 전극을 포함하고, 복귀 전극은 핸드 피스의 선단에 위치한다. 연마팁은 피부를 연마할 수 있는 표면을 가진 어떠한 재료로도 만들어질 수 있다. 연마팁의 재료는 전도성 또는 비전도성 물질이다. 바람직한 실시예에서, 상기 연마팁의 재료는 전도성 물질이다. 선택적으로 상기 연마팁은 피부에 적용되기 전에 습윤 유체로 젖는다. 제어되는 연마장치는 키트(kit)로 제공될 수 있는데, 이 키트는 장치, 하나 또는 그 이상의 연마팁 및 선택적으로 습윤 유체를 포함한다. 일 실시예에서, 상기 연마팁은 습윤 유체로써 젖어지고, 팁안에 습윤 유체를 보유시키기 위하여 용기내에 밀봉된다. 다른 실시예에서 습윤 유체는 미리포장된 와이프(wipe)와 같은 별도의 용기 또는 물질안으로 공급된다. 피부의 침투성을 증가시키는 방법은 제어되는 연마장치를 짧은 시간동안, 예컨대 30초동안에 표피의 부분에 적용하는 것을 포함한다. 피부 임피던스 또는 전도성, 그 결과에 따른 처리된 부분의 침투성의 바람직한 수준은 소정의 값으로 정해질 수 있다.
피부 임피던스 또는 전도성의 수준은 피부 완전성(intergity), 환자가 불편함을 느끼는 정도, 또는 적용기간의 바람직한 수준에 기초하여 선정될 수 있다. 상기 연마장치는 피이드백 제어기구의 일부분으로서 피이드백 회로를 포함하고, 상기 피이드백 회로는 피부 침투성이 원하는 수준에 도달한 시점을 결정하는 전도성 정보에 기초한 적당한 알고리즘 또는 신호 처리를 사용한다. 원하는 수준의 피부 침투성이 도달하면, 연마장치 제거되고 약물 전달 조성물 또는 장치 또는 피분석물 센서가 처리된 부분에 적용된다.

이하 설명되는 장치, 시스템, 키트 및 방법은 편리하고, 신속하고, 경제적이며 최소한도의 침입 시스템과 피부 침투성을 증가시키기 위한 방법을 제공한다. 이들 장치,시스템, 키트 및 방법은 경피 약물전달 및/또는 피분석물 측정을 위하여 사용될 수 있다.
Ⅰ. 제어되는 연마 장치
제어되는 연마장치(10)가 도 1에 도시되어 있다. 이 제어되는 연마장치는 (i) 핸드피스(12), (ii) 연마팁(20), (ⅲ) 피이드백 제어기구(30), (ⅳ) 둘 또는 그 이상의 전극(40), (ⅴ) 전기모터(50)를 포함한다. 이 장치는 추가적인 제어 및/또는 사용자 인터페이스를 포함할 수 있다.
도 1 및 도 3a 내지 3D에 도시된 상기 연마장치는 외부 피이드백 제어기구를 가진다. 바람직하게는 상기 피이드백 제어기구는 내부 피이드백 제어기구이다. 도 2a에는 내부 피이드백 제어기구를 가진 제어되는 연마장치의 일 예가 도시되어 있다.
a. 연마팁
연마팁(20)은 재사용가능하거나 일회용일 수 있다. 만약 연마팁이 재사용가능하면, 사용과 재사용 사이에 세척되도록 설계된다. 바람직한 실시예에서, 연마팁은 일회용이다.
일회용 연마팁은 핸드 피스의 선단에 적합한 연결수단에 의해 부착할 수 있고 제거할 수 있다.
일회용 연마팁의 바람직한 실시예는 도 3a 내지 3D에 도시되어 있다. 바람직한 실시예에서, 상기 일회용 연마팁은 튜브(24), 바람직하게는 플라스틱 튜브에 부착된다. 튜브(24)는 플라스틱 컵 또는 콘(cone,27)안의 중앙 공간안으로 삽입되고, 상기 중앙 공간은 연마장치가 켜질 때 팁이 움직이도록 허용하면서 상기 튜브를 수용하도록 형성되어 있다(도 3D 참조). 상기 컵 또는 콘(27)은 유체가 핸드피스(12)에 접촉하는 것을 방지하도록 설계되어 있어, 사용후 핸드 피스의 세척을 최소화하거나 필요없게 한다. 바람직한 실시예에서, 컵 또는 콘(27)의 구멍(25)은 핸드 피스(12)의 선단(14)의 외측 벽(21) 내에 꼭 맞게 되어 있다(도 3D 참조). 바람직한 실시예에서, 외측 벽(21)은 복귀 전극(44)으로 작용하는 전도성 물질을 포함한다.
i. 재료
연마 팁은 피부를 샌드페이퍼, 대개 100% 의료용 등급의 나일론으로 만들어지고 복수개의 코팅제 및 연마제, 와이어 브러시, 탄소섬유, 또는 미세바늘을 가지고 화장용 미세 박피에 사용되는 피부용 등급 직물과 같은, 거친 직물과 같이 피부를 연마할 수 있는 표면을 가진 어떠한 재료로 만들어질 수 있다. 상기 재료는 전도성 또는 비전도성일 수 있다. 예를 들면, 비전도성 물질인 백색 산화알루미늄,이 의료용 등급으로 저 비용으로 쉽게 사용가능하다. 이 재료는 연마팁을 제조하기 위한 고 체적 결합/제조를 위하여 필요한 어떠한 유리화(verification) 공정에 존재하는 것과 같은, 상승 온도를 견딜 수 있다. 몇명 실시예에서, 상기 재료는 산화알루미늄보다 피부에 덜 자극적이도록 산화알루미늄보다 더 연질의 물질이 바람직하다. 폴리머 비즈(beads)는 산화알루미늄 대신에 연마재로서 사용될 수 있다. 일반적으로 폴리머 비즈는 산화알루미늄 피부에 덜 자극적이고 더 연질인 물질을 제공한다. 재료의 적합성은 처리될 특정 개인 및 그 처리의 목적에 달려 있다. 그리하여 다른 개인에게는 다른 재료가 위 목록 재료에 대체될 수 있다.
적절한 공학적 설계로써, 전도성 재료가 연마팁에 연마재로 사용될 수도 있다. 적합한 전도성 물질은 금속, 탄소, 전도성 폴리머 및 전도성 엘라스토머(elastomer)를 포함할 수 있지만, 여기에 한정되지 않는다.
바람직한 실시예에서, 상기 재료는 전도성 재료, 바람직하게는 금속, 가장 바람직하게는 복수개의 구멍 또는 천공(22a, b)을 가진 스테인리스 강판이다. 이실 시예의 예는 도 3b에 도시되어 있다. 상기 연마팁은 연마될 피부의 영역에 대응하는 직경을 가진 원반(disc)을 형성하도록 재료를 펀칭(punching)하여 형성될 수 있다. 그리고나서 원반은 돔(dome) 형상으로 되고 튜브, 바람직하게는 플라스틱 튜브에 부착된다.
ⅱ. 치수
연마팁은 어떤 적합한 두께와 직경을 가질 수 있다. 일 실시예에서, 연마입자는 아크리로니트릴 부타디엔 스틸렌(ABS)과 같은 플라스틱 베이스위에 코팅되고, 연마 코팅의 두께는 연마입자의 그리트(grit) 크기에 의해 정해진다. 바람직한 실시예에서, 연마입자는 약 120(약 0.0044인치, 약 120미크론의 직경)의 그리트 크기를 가진다. 전형적으로, 크기가 120보다 더 큰 그리트 크기를 가진 입자는 타박상(bruising)을 유발하는 것으로 알려져 왔으므로 그리트 크기는 120이거나 이보다 더 작다.
전형적으로 연마팁은 0.5미크론 내지 150미크론의 범위의 두께, 바람직하게는 15 내지 120 미크론의 두께를 가진다.
연마팁은 적합한 어떠한 형상 또는 모양을 가질 수 있다. 전형적으로 연마팁은 원형의 단면을 가진다. 연마팁의 크기는 연마에 의해 침투될 영역의 크기에 달려 있다. 예를 들면, 작은 영역이 침투될 것이 요구되는 적용의 경우에 수 마이크로미터, 예를 들면 1 내지 25 ㎛의 직경을 가질 수 있다. 더 큰 침투될 영역을 요구하는 적용의 경우, 연마팁은 수 인치, 예를 들면 0.1 내지 5 인치의 직경을 가질 수 있다.
ⅲ. 습윤 유체
연마팁 재료의 전기전도성에 의존하여 습윤 유체는 연마팁을 젖게 할 수 있거나 젖게 할 필요가 있고, 그로써 피부에 전도성 경로를 제공한다. 습윤 유체는 적합한 물질을 포함한다. 이 물질로는 물, 소금, 이온성 또는 비이온성 계면활성제, 보존제, 알콜, 글리세롤, 겔 및 다른 유사 물질이 있다. 이들 물질의 여러가지 혼합물은 원하는 적용에 의존하는 다양한 전도성 수준으로 습윤 유체와 제형될 수 있다. 여기서 사용된 바와 같이, "고 전도성 유체", 또는 "고 전도성을 가진 유체"는 약 1,000 내지 약 100,000 μsiemens/㎝의 전도성을 가진 유체를 말한다. 여기서 사용된 바와 같이, "저 전도성을 가진 유체"는 약 0.1 내지 약 999 μsiemens/㎝의 전도성을 가진 유체를 말한다. 예를 들면, 도 1에 도시된 바와 같이, 외부 피이드백 제어기구의 경우, 플라스틱 또는 그리트된(gritted) 재료와 같은 비전도성 재료로 만들어지면, 고 전도성 유체가 피부로 통하는 전도성 경로를 제공하는 데 필요하다. 연마팁이 금속과 같은 전도성 재료로 만들어지면, 고 전도성 또는 저 전도성을 가진 습윤 유체가 사용될 수 있다. 다른 대안으로서, 금속재의 연마팁 자체가 침투된 피부를 통한 전도성 경로를 제공하기에 충분히 전도성이라면, 시스템은 습윤 유체를 전혀 필요로 하지 않을 수 있다. 바람직한 실시예에서, 500 내지 50,000μsiemens/㎝의 전도성을 가진 습윤 유체가 외부 피이드백 제어기구로써 사용될 수 있다.
도 2a 및 2b에 도시된 바와 같은 내부 피이드백 제어기구의 경우, 저 전도성 을 가진 습윤 유체가 사용되어야 한다. 고 전도성을 가진 습윤 유체는 단락회로 및 부적합한 장치 기능을 유발하는 경향이 있으므로, 고 전도성의 습윤 유체를 피해야 한다. 도 2a 및 2b에 도시된 연마팁은 전형적으로 비전도성 재질로 만들어진다. 그러한 습윤 유체의 사용은 피부가 손상되지 않은 때 저 전도성 바세린을 제공하고, 연마장치로써 피부 측이 침투될 때 전도성에 있어서 현저한 증가가 수반된다.
바람직하게는, 습윤 유체는, 물, 염(salts), 알콜, 글리세롤, 비 이온성 계면활성제, 방부제, 폴리에틸렌 글리콜, 및/또는 이들의 혼합물을 포함한다. 고 전도성 습윤제의 일 예는 정제수안에 0.1-20%(wt/wt)의 염, 0-2%(wt/wt)의 이온성 계면활성제, 0-20%(wt/wt)의 알콜, 0-1%(wt/wt)의 방부제를 포함한다. 저 전도성 습윤 유체의 일예는, 정제수안에 0-2%(wt/wt)의 비 이온성 계면활성제, 0-50%(wt/wt)의 알콜, 0-1%(wt/wt)의 방부제를 포함한다.
선택적으로, 습윤 유체는 환자에 전달된 약물, 진단약 또는 예방약, 등과 같은 하나 또는 그 이상의 활성제를 포함한다. 그러한 습윤 유체는 특히 약물 전달 적용에 유용하다.
일 실시예에서, 연마팁은 비 전도성 재질로 만들어지고, 습윤 유체는 저 전도성을 가진 유체이다.
ⅳ. 전극
전형적으로 연마팁(20)은 침투될 조직상에 대상 부위체 전기적으로 그리고, 피이드백 제어회로로써 연속성을 제공하기 위하여 모터(50)와 전기적으로 교통하는 제1전극(42)(이하, "소스 전극"이라고도 한다)을 포함한다. 바람직한 일 실시예에서, 상기 연마팁은 소스 전극으로 작용하는 전도성 구성요소를 포함하거나 전도성 재료로 만들어져(도 3D참조) 소스 전극으로 작용하고, 소스 전극은 연마팁(20)에서 모터축으로 연속성을 제공하기 위하여 스프링(28)과 접촉한다. 도 3D는 소스 전극으로도 작용하는 연마팁의 사용을 보여주고 있지만, 동일한 스프링 형상이, 내부에 삽입된 적어도 하나의 전도성 구성요소를 포함한 비 전도성 재질로 만들어진 연마팁에 사용될 수 있다. 이 실시예에서 소스 전극은 연마팁의 외측 표면과 위치레벨에 있어서 연마팁(20) 내부에 위치한다.
도 3D에 도시된 스프링 형상과 동일한 형상이 도 2a에 설명된 장치와 같이 내부 제어 피이드백 기구를 포함하는 장치에 사용될 수 있다.
외부 피이드백 제어기구를 포함하는 연마장치의 몇몇 실시예에서, 연마팁은 전극을 포함하지 않는다. 이들 실시예에서, 제1전극(42)(또는 소스 전극)은 로케이팅 링(60)안에 위치할 수 있다(도 1 참조).
상기 전극은 예를 들면 금속 및 전도성 폴리머를 포함하는 어떠한 전도성 재질로 만들어질 수 있다. 또 두 전극들은 이들 전극들이 피부와 접촉하고 상기 피이드백 제어회로와 전기적으로 소통하도록 허용하는 형상이면 어떠한 형상으로도 설계될 수 있다.
보다 더 균질한 피부 침투를 달성하기 위하여 복수 전극들이 사용될 수 있다. 정확한 전기적 판독을 제공하기 위하여, 적어도 하나의 전극과 접촉하는 환자의 피부의 표면은 충분하게 침투되어야 한다. 즉, 전극이 적용되는 부위에 각질층 이 제거되어야 한다.
바람직한 실시예에서, 연마팁(20)은 내부 피이드백 제어기구로써 설계된다. 이 실시예에서, 연마팁은 2개의 전극을 포함하고, 이 전극들은 연마팁의 외측면과 동일한 높이의 위치로 연마팁 내부에 위치한다. 이 실시예에서, 연마팁은 제1전극(42, 소스전극)과 제2전극(44, 복귀전극)를 포함한다. 이 전극들은 적합한 전도성 재료, 예를 들면 금속 및 전도성 폴리머를 포함하는 전도성 재료로 만들어진다. 이 실시예에서 적합하게 기능하는 내부 피이드백 제어기구의 경우, 연마팁은 바람직하게는 비전도성 재료로 만들어진다. 습윤 유체가 연마팁에 적용되면, 습윤 유체는 저 전도성을 가진 유체인 것이 바람직하다.
외부 피이드백 제어기구를 가진 장치의 바람직한 실시예에서, 연마장치(10)의 끝단은 동축 또는 동심으로 배치되는 2개의 전극을 포함한다(도 3b 및 3c 참조). 이 실시예에서, 연마장치(10)의 끝단(14)는 제1전극(42, 소스전극)과 제2전극(44, 복귀전극)를 포함한다. 연마장치의 선단의 평면도에서 보면, 소스 전극은 연마장치의 끝단 중앙에 위치한다. 소스 전극은 공기(26)로 채워진 공간에 의해 둘러쌓이고, 복귀 전극(44)에 의해 둘러쌓인다. 도 3b는 연마팁이 전도성 재료로 만들어지고 소스 전극으로 작용하는 실시예를 보여준다. 도 3c는 비 전도성 재질로 만들어지고 연마장치의 내부에 와이어 형태로 된 소스 전극이 삽입되는 실시예를 보여준다.
동축 또는 동심 배치에서, 제1전극 또는 소스 전극(44)은 핸드피스의 끝단(14)의 외측 벽(21)에 위치한다. 연마장치의 선단의 평면도에서 보면, 복귀 전 극(44)는 연마장치의 외측 링을 형성한다(도 3b, 3c 참조).
외부 피이드백 제어기구를 가진 연마장치에 대한 다른 실시예에서 , 제2전극 또는 복귀전극(44)는 제어되는 연마장치에서 분리되어 있다(도 1 참조). 제2전극의 위치는 제1전극의 위치에 인접하거나 떨어져 있을 수 있다.
b. 피이드백 제어기구
피이드백 제어기구(30)는 (i) 피부 연마측 전기 전도성을 주기적으로 또는 계속적으로 측정하기 위하여 장차 연마되거나 연마되고 있는 피부 측에 위치한 제1전극(42), (ⅱ) 피부 연마 부위에서 떨어진 부위에 위치하여 피부 연마 부위와 접촉하거나 피부 연마 부위에 인접할 수 있는 적어도 하나의 제2전극(44), 및 (ⅲ) 컨트롤러(32)의 사용을 포함한다. 상기 컨트롤러는 전극(42,44)에 의해 제공되는 전도성 정보에 대한 적합한 알고리즘 또는 신호처리를 사용하는 수학적 분석을 수행하고 피부 전도성의 거동을 계산한다. 컨트롤러는 또한 연마장치(10)을 제어한다.
피부를 통한 전도성의 동적 변화는 연마장치가 피부에 적용되고 있는 동안에 실시간으로 측정된다. 신호 처리는 측정치에 기초하여 수용되고 피부 침투의 수준은 동적 수학적 분석을 수행함으로써 제어된다. 그 분석의 결과는 피부 침투성의 요구 수준을 얻기 위한 연마장치의 적용을 제어하기 위하여 사용된다. 피부 침투성의 요구 수준은 소정의 값으로 고정될 수 있다. 또 달리, 피부 침투성의 수준은 피부 순정도(intergrity), 환자의 불만족감, 적용 기간에 관한 요구 수준에 따라 선 택될 수 있다.
제어되는 피부 침투성에 대한 실시간 알고리즘의 예는 엘스트롬 등의 미국 특허 제6,887, 239호에 기술되어 있고, 엘스트롬 등의 미국 특허 제6,887,239호의 도 4 내지 7에 입증되어 있다. 어떤 부위가 침투강화 처리가 되고 있을 때 표피의 침투성을 제어하기 위한 일반적인 방법을 기술하고 있다.
도 4는 원하는 침투성 수준을 달성하기 위하여 표피의 영역의 연마를 제어하기 위한 방법의 흐름도이다. 단계 108로 표시된 피부침투장치는 여기서 설명되는 연마장치이다.
그러나, 다른 침투장치 및 방법이 이하 설명되는 제어되는 피이드백 제어기구를 사용하기 위하여 수정될 수 있다. 다른 침투 방법은 테이프 벗겨내기, 문지르기, 샌딩, 연마, 레이저 절제, 고주파 절제(RF ablation), 화학적, 소노포레시스(sonophoresis), 일렉트로폴레이션(electroporation), 및 열적 절제(thermal ablation)를 포함한다.
102 단계에서, 제1전극(소스 전극)은 침투가 요구되는 피부 제1 영역에 전기적 접촉상태로 결합한다.
다음에, 104단계에서, 제2전극(복귀 전극)은 피부의 제2영역에 전기적 접촉상태로 결합된다. 이 피부의 제2영역은 피부 연마 부위에서 떨어진 부위에 위치할 수 있고, 피부 연마 부위에 인접할 수도 있으며, 피부 연마부위 내에 위치할 수도 있다.
2개의 전극들이 적당히 위치되었을 때, 106 단계에서, 2개의 전극들 사이의 초기 전도성이 측정된다. 이것은 전극을 통한 피부의 영역에 전기신호를 인가함으로써 달성될 수 있다. 하나의 실시예에서, 전기신호는 충분한 강도(세기)를 가져 피부의 전기적 변수가 측정될 수 있으나, 적당하게 낮은 강도를 가짐으로써 전기적 신호는 피부에 영구적인 손상이나 다른 해로운 효과를 일으키지 않는다. 하나의 실시예에서, 10 내지 100㎐의 교류전원이 소스 전극과 복귀 전극사이의 전위차를 생성시키기 위하여 사용된다. 공급전압이 500㎷를 초과하지 않야 하고, 바람직하게는 100㎷를 초과하지 않아야 하고, 그렇지 않으면 피부에 손상을 줄 위험이 있다. 전류세기도 적당하게 제한될 수 있다. 초기 전도성 측정은 적당한 회로를 사용하여 전원이 적용된 후 이루어진다. 다른 실시예에서, 저항 센서는 10 내지 100㎐ 사이의 주파수에서 피부 영역의 저항(impedence)을 측정하는 데 사용된다. 다른 실시예에서, 이중 또는 다중 AC전원로써 이중 또는 다중 측정은 유사한 또는 비유사한 자극을 사용하여 이루어질 수 있다. 다른 실시예에서 1㎑ 전원이 사용된다. 다른 주파수의 전원도 가능하다.
108 단계에서, 연마장치는 제1부위에서 피부에 적용된다.
110단계에서, 2개의 전극사이의 전도성이 측정된다. 그 전도성은 주기적으로 측정될 수 있고, 또는 계속적으로 측정될 수도 있다. 감시 측정은 초기 전도성 측정을 하기 위하여 사용된 동일한 셋업 전극을 사용하여 이루어진다.
112단계에서, 수학적 분석 및/또는 신호처리는 피부 전도성 데이터의 시간 변화에 따라 수행될 수 있다. 피부 전도성은 설정된 시간 주기, 예를 들면 침투 처리도중에 매초마다 한번 혹은 계속적으로 측정될 수 있다.
전도성 데이터를 그린 후, 그래프는 'S형(sigmoidal)' 곡선과 유사하고, 아래 일반적인 s형 곡선식(수학식1)으로 표현될 수 있다.:

수학식 1에서 t*의 값은 내곡점(즉, 곡선의 기울기가 변화하는 지점)을 얻기위하여 요구되는 노출시간에 대응하고, 제1 미분의 피크에 대응하며, 도 4에 나타난 데이터에 기초한 635의 값을 가진다.
도 6은 침투 단계를 종료할 때를 결정하는 방법을 설명하는 흐름도이다. 도 7a, b, c는 도 6의 흐름도의 단계들에 대응하는 대표적인 그래프들이다. 도 6, 단계 302에서, A/D 변환은 전도성 데이터에 기초하여 수행된다. 이는 도 7a에 설명된 것과 유사한 그래프로 나타난다. 다음, 단계 304에서, 필터링은 디지털 데이터에 따라 수행된다. 도 7b에 도시된 바와 같이, 필터링된 데이터는 필터링되지 않은 도 7a의 데이터보다 보다 완만한 곡선을 가진다. 다음, 단계 306에서, 곡선의 기울기는 계산된다. 단계 308에서 그 기울기의 최대값이 저장된다. 후속 측정도안에 얻어진 기울기에 대한 전류값이 저장된 최대값보다 크지 않으면 피크값을 기다리기 위하여 단계 302로 복귀한다. 만약 기울기가 최대값보다 작거나 같으면, 단계 312에서 과정은 최대값, 또는 도 7c에서 "X"로 표시된 내곡점을 감지하고, 그리고나서 단계 314에서, 장치는 피부에 대한 연마력 적용을 종료한다.

C. 전기모터
전기모터(50)는 핸드 피스(12)안에 위치한다. 연마팁(20)은 모터(50)에 직접 또는 간접적으로 연결되어 제어되는 연마장치가 작동할 때 모터가 연마팁을 진동식 또는 회전식으로 움직이도록 한다.
전기모터는 AC모터 및 DC모터, 2가지 타입이 가능하다. 이들 모터는 회전식 또는 선형식이다.
바람직하게는 상기 전기모터(50)는 회전식 DC모터이다. 바람직한 실시예에서, 모터는 구조와 유용성에 있어서 값비싼 희토류 금속을 사용하는 "브러시리스"에 비교하여 표준 전력 공급원(예, 직류 배터리)으로 상대적으로 사용의 편리함 때문에 회전식, 브러시타입, DC모터이다. 그러나, 브러시리스 모터도 장치에 사용가능하다.
모터는 선형, 진동, 동심, 동축, 및 편축운동과 같은 다양한 운동 형태를 생성시킬 수 있다. 또한, 모터는 0.01-10,000 rps 또는 ㎐ 범위의 다양한 운동속도를 생성할 수 있다.
d. 연마팁에 힘을 제공하는 수단
바람직한 실시예에서, 제어되는 연마장치는 작동할 때 연마팁이 피부와 접촉 상태를 유지하는 것을 보장하도록 연마팁에 힘을 제공하는 하나 또는 하나 이상의 수단을 포함한다. 적합한 수단은 표피에 접촉하고 있을 때 연마팁에 하향(피부 표면쪽으로) 힘을 제공하기 위하여 모터축 또는 커플러(coupler)에 설치된 스프링(16)을 포함한다(도 3a참조).
도 3a에 도시된 바와 같이, 스프링(16)은 연마팁이 피부에 가압될 때 압축된다. 스프링이 압축되면, 핸드피스(12)의 끝단(14)은 피부의 표면쪽으로 움직여, 복귀 전극(44)이 피부에 접촉하게 한다. 그리하여 이 위치에서, 소스 전극(42), 연마팁(20) 및 복귀 전극(44)들은 피부의 표면에 접촉하게 된다.
e. 복귀 전극

Ⅱ. 피분석물 감지 시스템
이하 설명되는 제어되는 연마장치는 체액내에 존재하는 관심 대상의 하나 또는 그 이상의 피분석물의 농도를 검출하기 위한 피분석물 센서와 결합될 수 있다. 상기 체액은 다음에 기재된 힘과 이들 힘의 조합에 의해 추출될 수 있다: 물리적 힘, 화학적 힘, 생물학적 힘, 진공압력, 전기 힘, 삼투력, 확산력, 전자기력, 초음파 힘, 캐비테이션 힘, 기계적인 힘, 열적 힘, 모세관 힘, 피부를 통과하는 유체 순환 힘, 전자음향 힘, 자력, 자석 유체동력학적 힘, 음향 력, 대류에 의한 힘, 광 음향력, 체액을 피부에서 씻어냄.
체액은 다음의 방법을 포함하는 수집 방법에 의해 수집될 수 있다: 흡수, 흡착, 상분리, 기계적, 화학적, 전기적 유도, 및 이들의 조합. 피분석물의 존재는 다음 방법에 의해 감지될 수 있다: 전기화학적, 광학적, 음향적, 생물학적, 효소기술적, 및 이들의 조합에 의한 방법.
예를 들면, 피부 부위에서 침투성의 요구되는 수준의 침투성을 얻기위하여 제어되는 연마장치를 사용한 후, 포도당 센서장치와 같은 피분석물 센서는 연마시스템에 의해 처리된 피부 부위 위에 배치된다. 포도당 센서는 피부를 통하여 계속 포도당 플럭스를 수용함으로써 작동한다. 반응에 있어서, 센서장치는 나노 암페어(nA)의 전기신호를 제공하고, 이 전기신호는 핑거-스틱 포도당 메터(finger-sticks glucose meter)를 사용하여 환자의 기준 혈당(BG)치에 대하여 계산된다. 상기 혈당 센서와 제어되는 연마시스템의 조합은 아래에서 예로 설명된다.
위 예는 포도당 감지에 대하여 설명하지만, 다른 피분석물도 동일한 방법을 사용하여 분석될 수 있다. 피분석물은 혈액, 혈장, 혈청 또는 간질액과 같은 생물학적 유체에 존재하는 분자 또는 생물학적 종(species)일 수 있다. 감시될 피분석물은 포도당, 젖산, 혈액가스(예컨대, 이산화탄소, 또는 산소), 혈액 pH, 전해질, 암모니아, 단백질, 바이오마커(biomarker) 또는 체액내에 존재하는 다른 어떠한 생 물학적 종을 포함하는 관심 대상의 피분석물일 수 있다.
Ⅲ. 약물 전달시스템
이하 설명되는 제어되는 연마장치는 약물전달 조성물 또는 환자에 약물을 경피적으로 전달하는 장치와 결합될 수 있다. 약물은 어떠한 적합한 형태로 된 적합한 치료적, 예방적, 또는 진단적 분자 또는 약제일 수 있다. 약물은 액체내에 분해되거나 현탁되거나, 고체, 반고체,또는 캡슐화된 및/또는 분산된 마이크로 또는 나노입자, 유제, 리포좀, 또는 지질 소포일 수 있다. 약물 전달은 혈액, 림프, 지질 유체, 세포, 조직, 및/또는 기관, 또는 이들의 조합 속으로 일어날 수 있다. 약물은 전형적으로 시스템적으로 전달된다.
예를 들면, 피부 부위에서 요구되는 수준의 침투성을 얻기 위하여 제어되는 연마장치를 사용한 후, 투여될 약물을 포함한 연고, 크림, 겔, 또는 패치와 같은 약물 전달 조성물 또는 장치가 연마 시스템에 의해 처리된 피부 부위 위에 배치될 수 있다.
다른 방법으로, 약물은 연마팁에 적용된 습윤 유체안에 포함될 수 있다. 이 번 실시예에서, 약물은 피부 표면이 연마될 때 동시에 투여될 수 있다.
Ⅳ. 키트
제어되는 연마를 위한 키트는 위에서 설명된 연마장치와 하나 또는 그 이상의 연마팁을 포함한다. 선택적으로, 키트는 연마팁에 첨가될, 적당한 용기안에 포장된 습윤 유체를 포함한다. 다른 실시예에서, 습윤 유체는, 하나 또는 그 이상의 연마팁에 미리 조치되고 연마팁안에 습기를 유지하기 위하여 포장된다. 다른 실시예에서, 키트는 습윤 유체를 포함하는 하나 또는 그 이상의 예비적으로 함습된 와이프(wipe)를 포함한다.
만약, 연마장치가 일회용 연마팁을 사용하면, 키트는 하나 또는 그 이상의 일회용 플라스틱 컵 또는 콘(27)을 포함한다. 바람직하게는, 상기 일회용 연마팁은 해드 피스에 결합되고 연결되도록 설계된 튜브(24)에 부착된다.
연마장치가 외부 피이드백 제어기구를 포함하도록 설계된다면, 키트도 하나 또는 그 이상의 복귀 전극을 포함한다.
Ⅴ. 피부 저항을 감소시키는 방법
A. 제어되는 연마장치
이하, 설명되는 제어되는 연마장치는, 피부 침투처리 없이 정제수로 젖게 한 다음 측정된 피부 저항과 비교하여, 30초 또는 그 이상까지 피부 저항을 줄일 수 있도록 환자의 피부 표면에 적용될 수 있다. 적은 피부에 약 1㎝의 거리 이내에 2개의 전극을 배치하여 측정할 때, 피부 침투 조치없이 정제수로써 젖게 한 다음 피부 저항 측정은 약 300㏀ 또는 그 이상이다.
연마 팁은 1 내지 30초에서 바람직하게는 5 내지 25초에서 90초까지 동안의 짧은 시간동안에 적용된다. 피부 저항( 또는 전도성)의 요구 수준과 처리된 부위의 결과 침투성은 소정의 값에 고정될 수 있다. 한편, 피부 저항(전도성)의 수준은 피부의 순정도, 환자의 불만족감의 자각 또는 적용 기간의 요구수준에 기초하여 선 정될 수 있다.
침투성이 요구되는 수준에 도달하면, 연마 장치는 제거되고 약물 전달 조성물 또는 장치 또는 피분석물 센서가 처리된 부위에 적용된다. 약물 전달은 약물 전달시스템이 연마된 피부에 적용되자마자 즉시 진행될 수 있다. 유사한 방법으로, 피분석물은 피분석물 센서가 피부에 적용되는 즉시 신체에서 피분석물 센서로 확산된다. 그러나, 피분석물의 정확한 값은 "준비(warm-up)" 기간, 즉 경피 피분석물 플럭스가 평형상태에 도달하고 센서가 피부-뼈 피분석물과 가능한 다른 인터페이스 종을 소모하고 피부 측에 센서가 물리적으로 결합되는 것이 안정될 때까지 걸리는 시간 도중에는 대개 유용하지 않다. 준비 기간은 전형적으로 처리된 부위에 피분석물 센서를 적용한 다음 약 1시간 동안 지속된다.
연마장치의 적용후, 피부 부위는 24시간 동안까지 침투가능하게 유지되고 몇몇 실시예서는 72시간까지 침투가능하게 유지된다.
B. 다른 침투장치
다른 침투장치 및 기술은 피부 침투의 요구수준을 얻기 위하여 여기서 설명된 제어되는 연마장치 대신에 사용될 수 있다. 예를 들면, 피이드백 제어기구는 테이프 제거, 문지르기, 샌딩, 연마, 레이저 절제, 고주파 절제,화학적, 초음파 영동법(sonophoresis), 이온삼투압 방법, 일렉트로폴레이션(electroporation), 및 열적 절제(thermal ablation)과 같은 다른 피부 침투 방법과 결합될 수 있다.
실시예
실시예1: 2가지 피부 침투 방법의 비교: 초음파 영동법과 연마
6명의 환자에 있어서, 24시간동안 연구, 연마 방법의 성능은 도 4에 나타난 것과 같은 제어 알고리즘을 사용하는, 미국 특허 제6,887,239에 기재된 초음파 영동법과 비교하였다. 각 환자들은 가슴 부위 또는 복부 부위에 연마 처리된 한 부위와 초음파 처리된 한 부위를 가진다.
제어되는 연마시스템에서, 전도성 피이드백 역치가 얻어질 때까지(위 I.b항목의 피이드백 제어기구에서 설명된 바와 같이), 도 1에 설명된 연마장치가 5 내지 25초 동안 환자의 피부에 적용되었다.
제어되는 초음파 영동시스템의 경우, Sontra SonoPrep®의 초음파 피부 침투장치를 사용하여 55㎑의 초음파가 5 내지 50초 동안 환자의 피부에 적용되었다. 초음파는 전도성 피이드백 역치가 얻어질 때까지(위 I.b항목의 피이드백 제어기구에서 설명된 바와 같이) 적용되었다.
포도당센서 유닛이 제어되는 연마 또는 초음파 영동에 의해 처리된 2곳의 타겟 피부 부위의 각각에 배치되었다. 그 연구의 과정을 거쳐, 기준 핑거-스틱 혈당("BG") 샘플이 깨어 있는 시간동안에 시간 간격으로 또는 식사시간 부근에 15분 간격으로 채취되었고, 상기 센서의 전기 신호에 상관되었다.
이 상관의 분석은 장치 정밀도, 일관성, 성능의 유효 길이에 관한 정보를 제공한다.
도 8은 계속적인 경피 포도당 감지에 의해 피부를 침투하는 시험 대상 환자 에 연마시스템을 사용하여 얻은 결과 그래프이다. 표 1은 연속적인 포도당 감시를 위하여 피부 침투의 수단으로서 초음파 처리를 연마와 직접 비교한 결과를 보여준다. 표 1은 6명의 환자에서 얻은 데이터에 기초한 평균값을 보여준다.

기준 혈당(RefBG)값은 핑거스틱을 사용하는 상업적인 혈당 메터에 의해 측정되었다. 2번의 측정이 혈당센서에서 1.2 및 9.1 시간(도 8에 "측정점"으로 이름이 붙여져 있다)에서 RefBG 값에 기초하여 행해졌다. 센서 혈당 판독값(에측BG)은 기준 혈당(RefBG)에 거의 근접하는 것은 경피 포도당 센서의 양호한 정밀성을 나타낸다. RefBG와 예측BG사이의 24시간 평균 절대상대 편차(MARD)는 11.9mg/dl이었다.
제어되는 연마장치를 사용한 침투의 경우, 평균 24시간 MARD는 31%의 신호변이를 가진 11.7mg/dl있다. 제어되는 초음파 영동 시스템의 경우, 평균 24시간 MARD는 26%의 신호변이를 가진 13.1mg/dl있다. 그리하여 제어되는 연마 장치는, 준비기간(1시간), 정확도( 센서 예측 포도당과 기준 BG사이의 MARD, 변이(센서 예측 포도당과 기준 BG의 시간 종속 %변화), 및 Clarke Error Grid 분석에 기초한 "A영역"에서 데이터 분포의 퍼센터에 있어서, 초음파 영동시스템과 비교될 만한 또는 어떤 경우에는 더 나은 추적을 제공하였다.
실시예2: 연마장치의 적용에 이은 피부 저항의 낮춤
인간 피부가 정세수로 젖어 있을 때,2개의 전극을 젖은 피부위에 약 1㎝ 거리 이내에 배치되어 측정될 때, 저항값은 통상 300㏀ 혹은 그 이상이다. 그러나 도 1에 도시된 바와 같은 제어 알고리즘을 사용하여 제어되는 연자장치로써 동일한 영역이 처리되고, 피부위에 5 내지 25초 동안 연마장치를 두고 연마장치의 적용으로써 저항값을 얻었을 때, 그 저항값은 약 10㏀ 또는 그 이하로 현저히 감소되었다. 이 실험에서, 연마팁은 ABS 기판위에 코팅된 백색 산화알루미늄(120 그리트)를 포함하였다.
