Abstract
본 발명은 평균 분지도가 10 초과 내지 25 몰% 이고, 분자량 (Mw)가 40,000~800,000 달톤 (Dalton)인 고차분지형 아밀로펙틴 및 그의 유도체의, 인간 또는 동물 신체의 외과수술 또는 치료방법에서, 바람직하게는 혈장 증량제로서의 용도에 관한 것이다. 히드록시-에틸화 아밀로펙틴 기재의 혈장 증량제는 히드록시 에틸화의 결과로, 이제까지 불완전한 대사 및 그로 인하여 부작용과 연결되는 일시적인 조직 저장이라는 단점을 나타내었다. 본 발명에 따라, 상기와 같은 동일한 단점을 나타내지 않는 다당류 기재의 신규 혈장 증량제가 제공된다. 이로부터 수득될 수 있는 개선된, 완전 대사되는 혈장 증량제는, 적용가능한 고도의 분지화가 혈청-α-아밀라아제의 제어를 가능하게 하여 히드록시에틸화가 존재하지 않거나 매우 적은 정도로 요구되는 방식의 글리코실 전이반응에 의해 천연 식물의 아밀로펙틴을 변경시켜 수득될 수 있다.
Description
혈장 증량제의 개발 내력에 있어서, 혈청 중에 콜로이드-삼투압의 천연 담체, 즉 알부민의 구형 구조를 달성하기 위한 시도가 항상 있어왔다. 인간 유기체에서의 천연 보유 다당류로서 또한 발견되는 글리코겐이 이러한 구형 구조에 가장 가깝다. 글리코겐은 그의 매우 높은 분지도로 인해서 그러한 구형 구조를 갖는다. 구조적인 관점에서, 글리코겐은 선형 단면에서 α-1,4 글리코시드 결합을 갖는 글루코오스 다당류로, 상기 결합에 α-1,6 글리코시드 분지점이 고정되어 있다. 글리코겐 자체는 저렴한 원료 공급원으로서 유용하지 못하기 때문에, Wiedersheim 은 1957 년에 혈장 증량제 히드록시에틸 전분 (HES)의 생산을 위한 출발물질로서 덜 분지된 아밀로펙틴을 대신하여 사용하는 것을 제안하였다. 반면, 히드록시에틸 전분은 혈장 증량제로서 몇몇 상이한 유형으로, 매우 대규모로 사용되고 있다. 이러한 개발은, 최적의 증량 효과를 나타내는, 그렇지 않으면 예로서 응고 또는 조직 내 중간체 보유에 영향을 미치는 것과 같은 부작용이 최소인 히드록시에틸 전분류의 새로운 유형 (HES 형)을 이끌었다.

그러나, 화학적인 면에서, 히드록시에틸 에테르기는 대사적으로 극히 안정하여 히드록시에틸 에테르기를 갖는 히드록시에틸 전분의 무수글루코오스 단위는 실질적으로 대사되는 것이 불가능하다. 또한, 히드록시에틸 전분 중의 α-1,4 글리코시드성 결합들만이 비치환 글루코오스 단위에 의해 형성되는 혈청 α-아밀라아제에 의해 분할될 수 있다. 이러한 이유로, 최적화된 HES 형의 경우에도 최소이지만 여전히 현저한 조직잔류가 일정 기간에 걸쳐 확인됨을 발견하였다.

따라서, 아밀로펙틴 유도체 히드록시에틸 전분의 완전 대사능의 결핍이라는단점을 나타내지 않는 아밀로펙틴 기재의 신규한 개선된 혈장 증량제를 개발하는 과제가 존재한다. 동시에, 그러한 신규 혈장 증량제는 보다 구형 구조를 가지고, 이에 따라 비교적 낮은 점도의 용액을 형성할 수 있어야 한다.
특정 아밀로펙틴에 대한 추가의 적용 분야를 여는 것도 본 발명의 과제 중 하나로 고려될 수 있다.

본 발명은 고차분지형 아밀로펙틴의 용도에 관한 것이다.
특히, 본 발명은 특정 분지도 및 특정 분자량 Mw 을 나타내는 고차분지형 아밀로펙틴의 신규 용도에 관한 것이다.
평균 분지도가 10 초과 내지 25 몰% 이고, 평균 분자량 Mw 가 40,000 내지 800,000 달톤인 고차분지형 아밀로펙틴 및 필요한 경우 그의 유도체를 인간 또는 동물 신체의 외과수술 또는 치료방법 (포유동물의 외과수술 또는 치료방법 포함) 또는 진단 방법에 사용함으로써, 직접 예측될 수 없는 방식으로, 한편으로는 고차분지형 아밀로펙틴에 대한 의약 분야에서의 다수의 신규하고 흥미있는 적용을 발견해내는 것이 가능하다. 다른 한편으로는, 현재 가장 흔히 사용되는 전분 기재의 HES 생성물에 대한, 특히 '혈장 증량제' 부문에 있어서 가장 이상적인 대체물이 제공되어, 위험한 부작용을 보다 감소시킨다.
혈장 증량에 있어서, 예의 연구 및 조사 결과 실제로 본 발명과 관련하여 혈류 및 소변 중에 히드록시에틸 전분의 잔류 분획이, 본래 주입된 히드록시에틸 전분 (HES 제품)에 비해, 혈장 증량제 적용 후 수시간 또는 몇 일 후에도 분지도에있어서 강한 증가를 나타낸다는 것이 발견되었다. 분지도는 분지점을 갖는 무수글루코오스의 몰 % 로 나타내었으며, 적용한지 2 시간 후 약 5 몰 % 에서 7 몰% 초과로 증가하였고, 적용한지 7 시간 후에는 약 8 몰% 로 증가하였다. 동시에, 주입 후 48 또는 42 시간 후에 9 몰% 또는 10 몰% 의 보다 높은 분지도가 전체 소변 분획에서 명백하게 나타났다. 이러한 현상은 분자량, 적용된 히드록시에틸 전분의 치환도 또는 치환 패턴과 관계없이 관찰되었다. 이는 분해시, 이들 분획들이 글리코겐과 유사하게 되는 구조 및/또는 분지화에 대한 접근을 더욱 획득한다는 것을 의미하며, 이는 문헌에서 분지로서 높은 수준인 약 10 몰% 수준으로 나타난다.
놀랍게도, 아밀로펙틴 및 그의 유도체에서 α-(1,6) 분지의 상대적인 안정성은 우세한 α-아밀라아제 분해에 비해 아밀로펙틴의 분해를, 완전히 분해가능하지만, 약물동력학 및/또는 증량 효과 고려시 이상적인 혈장 증량제의 성질을 여전히 나타내는 다당류가 생성될 수 있는 정도로, 감소시키는데 활용할 수 있다는 것이 이제 발견되었다.
즉, 본 발명은 고차분지형 아밀로펙틴 및 그의 유도체의 의약 분야에서의 용도를 포함한다.
아밀로펙틴이라는 용어는 우선 일반적으로, 글루코오스 분자 사이에 α-(1,4) 및 α-(1,6) 결합을 갖는 모든 분지형 전분 또는 전분 생성물을 의미하는 것으로 이해되어야 한다. 사슬의 이러한 분지화는 α-(1,6) 결합을 통해 일어난다. 자연 발생적인 아밀로펙틴의 경우, 이들은 대략적으로 매 15~30 글루코오스 절편마다 불규칙한 방식으로 일어난다. 자연 발생적인 아밀로펙틴의 분자량은 매우 높아, 107내지 2×108달톤 범위이다. 아밀로펙틴 또한 특정 범위 내에서 나선형을 형성하는 것으로 추측된다.
아밀로펙틴의 분지도를 정의하는 것은 가능하다. 아밀로펙틴의 무수글루코오스 분자의 총 수에 대한 분지점 (α-(1,6) 결합)을 갖는 무수글루코오스 분자의 수의 비율은 분지의 척도이며, 이 비율은 몰로 표시된다. 자연 발생적인 아밀로펙틴은 약 4 몰% 의 분지도를 갖는다. 그러나, 단리시, 아밀로펙틴의 클러스터 (cluster) 및 분자 구역은 자연적인 평균 분지도에 비해 약간 더 높은 분지도를 나타낸다.
본 발명의 의미에 따른 고차분지형 아밀로펙틴은 자연으로부터의 아밀로펙틴에 대해 알려진 분지도를 현저히 초과하는 분지도를 나타낸다. 아밀로펙틴은 다중분산 물질이기 때문에, 상기 분지도는 임의의 경우에는 평균 값 (평균 분지도)이다.
이러한 고차분지형 아밀로펙틴은, 변경되지 않은 아밀로펙틴 또는 히드록시에틸 전분에 비해 현저히 높은, 분지화 무수글루코오스의 몰%로 표시되는 분지도를 나타내며, 결과적으로 이는 구조 면에서 글리코겐에 보다 유사하다.
본 발명에 따른 적용에 요구되는 고차분지형 아밀로펙틴의 평균 분지도는 10 초과 내지 25 몰% 의 범위이다. 이는 본 발명의 의미에 따른 유용한 아밀로펙틴이 평균 α-(1,6) 결합 상에서 결과적으로 대략적으로 매 10 내지 14 글루코오스단위마다 분지점을 나타낸다는 것을 의미한다. 분지도가 10 몰% 이하이면, 분지형 아밀로펙틴의 분해가 불충분하게 지체된다 (예로서, 이의 혈장 증량제로서의 용도의 경우). 분지도가 25 몰% 초과이면, 분해가 과다하게 지연되어, 예로서 이의 혈장 증량제로서의 용도가 불가능하게 된다.
의약 분야에서 바람직하게 사용될 수 있는 아밀로펙틴의 유형은 11 내지 16 몰% 의 분지도에 의해 특징된다.
다른 바람직한 고차분지형 아밀로펙틴은 13 내지 16 몰% 의 범위의 분지도를 나타낸다.
또한, 고차분지형 아밀로펙틴의 분자량 Mw 도 중요하다. 분자량 Mw 는 이러한 평균 값을 제공하는 관련 방법에 의해 측정가능한 중량 평균 분자량이다. 이들 방법은 수성 GPC, HPLC, 광산란 등을 포함한다.
본 발명에 따른 용도에 적합한 고차분지형 아밀로펙틴은 일반적으로 40,000 내지 800,000 달톤의 중량 평균 분자량을 나타낸다. 분자량 범위에 대한 하한치 Mw 는, 바람직한 적용의 경우에서 소위 "신장 역치" 로부터 본질적으로 수득되며, 이는 고차분지형 화합물의 경우 약 40,000 달톤으로 설정되어야 한다. Mw 가 40,000 달톤 미만인 경우, 분자들이 신장을 통해 너무 빨리 여과될 것이다. Mw 가 800,000 초과인 경우, 비록 구형 구조의 경우 한계 점도가 분자량에 더이상 의존하지 않지만, 언급할 만한 가치가 있는 추가적인 유용성이 달성되지 않을 것이다.
90,000 내지 300,000 달톤의 평균 분자량 Mw 가 혈장 증량제로서의 용도에바람직하며, 특히 120,000 내지 250,000 달톤의 분자량 Mw 가 특히 적절하다.
본 발명의 특정 구현예는, 평균 분지도가 11 내지 16 몰% 이고, 분자량 Mw 가 90,000 내지 300,000 달톤인 고차분지형 아밀로펙틴을 함유한다. 본 발명의 다른 적절한 구현예는, 평균 분지도가 13 내지 16 몰%이고, 분자량 Mw 가 120,000 내지 250,000 달톤인 고차분지형 아밀로펙틴을 포함한다.
상기 언급된 파라미터들, 즉 분지도 및 분자량은 목적하는 효과가 발현되도록하고, 이에따라 바람직한 약물동력학, 특히 바람직한 α-아밀라아제 분해가 달성되도록 설정된다. 아밀로펙틴의 분지도는 이와 관련하여 중요한 역할을 한다. 그러나, 분자량 또한 고려되는 동력학에 영향을 준다. 또한, 분지점의 분포를 변화시킴에 의해 바람직한 방향을 취하도록 아밀로펙틴 분해의 동력학에 영향을 주는 것도 가능할 것이다.
그러나, α-아밀라아제에 의한 아밀로펙틴의 분해 및 결과적으로 혈장 증량제의 작동을 위해서는 분지도가 중요하다. 높은 분지도의 결과, α-아밀라아제의 공격이 현저히 지연되고, α-아밀라아제의 접근이 불가능하기 때문에 높은 분지점 밀도를 갖는 분자 영역에서는 이러한 공격이 아주 배제된다. 그럼에도 불구하고 이러한 화합물들은 다른 효소에 의해 올리고당류로, 최종적으로 글루코오스로 분해될 수 있다.
필요한 경우, 본 발명에 따라 사용되는 고차분지형 아밀로펙틴은 유도체 형태로 수득될 수 있다. 이러한 유도체들은 예로서 화학 또는 생물기술적 전환에 의해 수득가능한 것들과 같은 아밀로펙틴의 화학 유도체들을 함유한다.
바람직한 고차분지형 아밀로펙틴의 유도체들은 히드록시에틸아밀로펙틴, 히드록시프로필 아밀로펙틴 및 아세틸 아밀로펙틴이다. 이들 중, 히드록시에틸아밀로펙틴이 특히 유리하게 사용된다. 아밀로펙틴 분해의 동력학은 결과적으로 유도체화에 의해 영향을 받을 수 있다. 그러나, 유도체화 정도, 예로서 히드록시에틸화도는, 정상적으로 분지된 아밀로펙틴으로부터 제조된 히드록시에틸 전분 (HES)에 비해 이에 필적할 만한 증량 효과 또는 유사한 약물동력학을 달성하기 위해, 이들 경우에서 현저히 낮은 것이 유리하다.
본 발명의 의미에 따라 (다른 것들 중, 바람직하게는) 혈장 증량제로서의 용도에 적합한 고차분지형 아밀로펙틴의 제조는, α-1,4 글리코시드 결합의 가수분해 및 이들의 α-1,6 글리코시드 화합물로의 전환을 촉매하는 소위 분지화 효소에 의한 효소적 전환과 같은 알려진 방법으로 수행한다. 이러한 소위 전이 (transfer) 효소는 공지의 방법으로, 예로서 PCT WO 0018893 호에 따라 해조류로부터 추출할 수 있다. 그러나, US 특허 4454 161 호 및 EP 0418945 호로부터 다른 글리코겐 분지화 효소가 알려져 있으며, 따라서 이 또한 사용할 수 있다. 효소적인 글리코실기전이 반응은 예로서, 찰옥수수 전분을 상응 효소와 함께 마일드한 조건하에서 pH 약 7.5, 온도 약 30 ℃ 에서 수용액 중에서 인큐베이션하는 것과 같은 공지의 방법에 의해 수행한다. 상기 반응 제조액에 대한 작업을 이어서 다시 공지된 방법으로 수행하고, 효소를 먼저 pH 변화에 의해 또는 여과 단계에 의해 불활성화 또는 제거시킨다.
이후 바람직하게는 염산에 의해 수행되는 가수분해 단계에서, 생성물의 바람직한 분자량을 조정한다. 약 3,000 달톤의 컷오프 (cut-off)를 갖는 막을 사용하여 투석여과함으로써, 생성물을 이어서 저분자량 화합물 및 산성 가수분해액 제조물의 중화 동안 형성된 일반염으로부터 분리한다. 생성물을 분무건조에 의해 단리한다.
혈장 증량제로서의 사용 외에도, 고차분지형 아밀로펙틴은 또한 다른 의약 영역에서 유용하게 사용될 수 있다. 즉, 고차분지형 아밀로펙틴은, 정상적 분지형의 전분을 기재로 한 표준 HES 제품이 사용될 수 있는 치료 및 외과수술분야에서의 모든 적용에 유용할 수 있다.
혈장 증량제로서의 용도 외에도, 이는 바람직하게는 미세순환의 개선에 대한 용도, 백혈구분반술 (leucapheresis)과 관련된 세포 분리에서의 침강 보조제로서의 용도, 또는 적혈구 또는 과립구와 같은 혈액 성분의 냉동 보존에 대한 용도를 포함한다.
실시예 1
상이한 분지형 α-1,4/α-1,6 글루코당류를 사용한 비교 분해 시험
SIGMA 사로부터의 굴 유래의 글리코겐을, DMSO 30 % 함량의 DMSO 와 물의 혼합물 중에서, 70 ℃ 및 pH 6.0 에서, NOVOZYMES 사로부터의 내열성 α-아밀라아제 BAN 480 L 에 의해 분해시켰다. 기체 크로마토그래피에 의해 분자량에서의 변화를 측정함으로써 반응 경과를 관찰하고, 효소 활성화를 위한 가성 소다 용액을 첨가함으로써 약 2 시간 후 반응을 중지시켰다. 중화 후, 생성물을 공칭 컷-오프 1,000 D 및 25,000 D 의 셀룰로오스 아세테이트 한외여과지를 사용하여 한외여과로 분획하여 저분자량 분획 및 잔류하는 고분자량 분획을 제거하였다. 생성물을 이어서 이온 교환기 Amberlite IR 200 C 및 활성탄으로 처리하고, 에탄올로 침전시키고, 80 ℃ 에서 건조시켰다.
1H NMR 분광법 (아노머성 양성자의 신호의 적분)에 의해 측정된 분지도는 15 몰% 였다. 평균 분자량 Mw 는 7,000 달톤이었다.
산분해 찰옥수수 전분 (thin boiling waxy maize starch) (≥95 % 아밀로펙틴) (Cerestar)을 상기 기재된 것과 동일한 방식으로 처리하였다. 분지 클러스터의 단리된 높은 분지 분획은 분지도가 11 몰% 이고, 평균 분자량 Mw 가 8,000 달톤이었다.
아밀로펙틴 및 글리코겐의 고분지된 클러스터 분획들을 이어서, pH 7.2 의 1 % 인산 버퍼 용액 중에서, 37 ℃ 에서 0.5 IU/㎖ 효소를 갖는 돼지의 췌장 α-아밀라아제 (Roche)를 사용하여 분해 시험에 투입하고, 겔 크로마토그래피에 의해 분자량에서의 변화를 측정함으로써 분해 동력학을 관찰하였다. 상용 히드록시에틸 전분 혈장 증량제를 사용하여 비교시험을 또한 실시하였다 (Voluven, Fresenius Kabi). 분해 동력학에서 명백한 차이가 수득되었다. 분자량의 반감기 (출발물질의 평균 분자량 Mw 가 초기 값의 절반으로 감소)는, 15 % 분지도의 분획의 경우 60 분이었으며, 결과적으로 Voluven 혈장 증량제에서와 동일한 조건 하에서 결정된 반감기에 도달하였다.
한편 평균 분지도가 11 몰% 인 분획의 반감기는 단지 25 분이었으며, 따라서 이는 현저히 단축된 것이다.
실시예 2
Ceresta 로부터의, NMR 로 측정시 평균 분지도가 4 몰% 인 산분해 찰옥수수 전분을 실시예 1 에 제공된 데이타에 따라 돼지 췌장 α-아밀라아제을 이용한 분해 시험에 투입하였다. 이를 위해, 1 % 용액을 pH 7.2 의 인산 버퍼 중에서 약 90 ℃ 로 살짝 가열하여 젤라틴화시키고, 냉각 후 효소를 생성된 0.5 IU/㎖ 의 양으로 제조액에 첨가하였다.
시험 온도는 37 ℃ 였다.
겔 크로마토그래피에 의해 분자량에서의 변화를 측정함으로써 분해 동력학을 관찰하였다. 출발 물질의 분자량은 실시예 1 과 동일한 조건 하에서 10 분 내에 반으로 줄어들었다.
실시예 1 의 α-1,4/α-1,6 글루코당류에 비해, 평균적으로 비교적 낮은 분지도를 갖는 산분해 찰옥수수 전분은 α-아밀라아제에 의해 너무 신속히 분해되어 혈장 증량제로서 사용할 수 없었다.
이는, 상기 2 개의 실시예 1 및 2 는 분자량이 낮은 경우라도, 분지가 보다 높은 수준이면 α-아밀라아제의 분해가 지연되고, 이러한 효과가 혈장 증량제의 제조에 사용될 수 있다는 것을 증명한다.
Claims (7)
- 인간 또는 동물 신체의 외과수술 또는 치료방법 또는 진단 방법에 적용되는, 평균 분지도가 10 초과 내지 25 몰% 이고, 중량 평균 분자량 Mw 가 40,000~800,000 달톤 (Dalton)인 고차분지형 아밀로펙틴 및 그의 유도체.
- 제 1 항에 있어서, 혈장증량제로서의 고차분지형 아밀로펙틴.
- 제 1 항에 있어서, 미세순환의 개선을 위한 고차분지형 아밀로펙틴.
- 제 1 항에 있어서, 백혈구분반술 (leucapheresis)과 관련된 세포 분리에서 침강 보조제로서의 고차분지형 아밀로펙틴.
- 제 1 항에 있어서, 과립구 또는 적혈구와 같은 혈액 성분의 냉동보존을 위한 고차분지형 아밀로펙틴.
- 제 1 항 내지 제 5 항 중 어느 한 항에 있어서, 평균 분지도가 11 내지 16 몰% 이고, 분자량 Mw 가 90,000 내지 300,000 달톤인 고차분지형 아밀로펙틴.
- 제 1 항 내지 제 5 항 중 어느 한 항에 있어서, 평균 분지도가 13 내지 16몰% 이고, 분자량 Mw 가 120,000 내지 250,000 달톤인 고차분지형 아밀로펙틴.
