Abstract
혈소판 수의 감소를 요하는 사람에게 유효량의 하기 화학식 (I)의 화합물 또는 제약학적으로 허용되는 그의 염 또는 용매화물을 투여하는 것을 포함하는, 사람에 있어서 혈소판 수를 안전하게 감소시키는 방법을 제공한다.

Description
혈소판은 혈액에서 발견되는 주요 세포형이고, 정상적인 지혈을 유지시키기 위해 중요하다. 혈소판은 골수 간세포로부터 유도되며, 보통 혈류에서 발견되거나 비장에서 격리된다. 사람 혈액 중 혈소판의 정상적인 수는 약 150,000 내지 400,000개/㎖이고, 혈소판 수가 실질적으로 이 수준 미만으로 떨어지는 상태를 혈소판감소 상태로 부르고, 반대로 혈소판의 과다는 혈소판증가증 또는 혈소판혈증 상태로 부른다.

본 명세서에서 기재하는 화합물의 정의에 사용되는 일반적인 용어는 그들의 통상의 의미를 갖는다. 예를 들면, "C1-C6알킬"은 메틸, 에틸, 프로필, 이소-프로필, n-부틸, 헨틸 및 헥실 등을 포함하는, 탄소 원자수 1 내지 6의 직쇄 또는 분지쇄 지방족을 가르킨다.
용어 "치환된 페닐"은 페닐기 자체나, 또는 C1-C4알킬, C1-C4알콕시, 히드록시, 니트로, 클로로, 플루오로 또는 트리(클로로 또는 플루오로)메틸로 이루어진 군 중에서 선택되는 하나 이상의 치환기를 갖는 페닐기를 가르킨다. "OC1-C4알킬"은 메톡시, 에톡시, n-프로폭시 및 이소-프로폭시 등과 같은, 산소 브릿지를 통해 부착된 C1-C4알킬기를 가르킨다.

따라서, 본 발명의 바람직한 실시태양인 랄록시펜 염산염은 R1및 R3이 모두 수소이고, R2가 피페리디닐 고리인 화학식 I의 화합물의 염산염이다. 랄록시펜 염산염은 [2-(4-히드록시페닐)-6-히드록시벤조[b]티엔-3-일][4-[2-(1-피페리디닐)에톡시]페닐]메타논 염산염이다.
본 발명의 방법과 제형에 사용되는 모든 화합물은 각각 본원에 참고로 인용한 미국 특허 제4,133,814호 및 동 제4,418,068호에 상세히 기재된 바와 같은 절차에 따라 제조할 수 있다. 일반적으로, 이 방법에서는 6-히드록실기 및 2-(4-히드록실페닐)기를 갖는 벤조[b]티오펜을 출발 물질로 사용한다. 출발 화합물을 보호하고, 알킬화시키고, 탈보호하여 화학식 I의 화합물을 형성한다. 카르복실산 에스테르인 화학식 I의 화합물은 본원에 참고로 인용한 미국 특허 제5,393,763호에 기재된 방법으로 제조할 수 있다.

본 발명의 방법에 사용되는 용어 "혈소판 수의 감소"는 심각한 출혈 사건을 촉진시킬 가능성이 있을 수 있는 혈소판감소증의 수준 (<100,000개/㎖)에 도달시키지 않으면서, 순환 혈소판의 수를 감소시키는 것을 의미한다.

본 발명에 따라 혈소판 수를 안전하게 감소시키기 위해 필요한 화학식 I의 화합물의 구체적인 투여량은 구체적인 증상과 심각도에 따를 것이다. 투여량, 투여 경로 및 투여 빈도와 같은 고려사항은 담당의사에 의해 가장 잘 결정된다. 일반적으로, 경구 또는 비경구 투여에 대해 허용되는 유효 투여량은 10 ㎎ 내지 800 ㎎, 더 전형적으로는 20 ㎎ 내지 100 ㎎일 것이다. 특히 바람직한 투여량은 특히, 폐경기 여성에서 경구 경로를 통해 60 ㎎/일이다. 이러한 투여량은 치료를 요하는 환자에게 1일 1회 내지 3회 또는 혈소판 수를 효과적이고 안전하게 감소시키기 위해 요구되는 만큼 자주 투여될 것이다.

2,043명의 건강한 폐경기 여성을 선별하였다. 이들 환자를 584명의 환자들의 대조군과 다음 3가지 시험군: 랄록시펜 HCl 30 ㎎/일 - 288명의 환자들; 랄록시펜 HCl 60 ㎎/일 - 584명의 환자들; 랄록시펜 HCl 120 또는 150 ㎎/일 - 590명의 환자들에 무작위로 배치하였다. 대조군의 환자들에게 일일 경구 플라시보를 투여하였다. 임상 시험 기간은 2년이었다. 환자들의 전체적인 평가의 일부로서, 다양한 임상 시험 및 파라미터들을 일정 간격에서 측정하였다. 주기적 파라미터 중에서 혈소판 수를 측정하였다.
초기 혈소판 수를 종료점 값과 비교하면 대조군에 비해 시험군의 환자들에서 순환 혈소판이 7-9% 감소한 것을 나타낸다. 연구 기간 동안 비정상적인 출혈 에피소드를 경험하지 않았으며, 혈소판 수는 100,000개/㎖ 미만으로 떨어지지 않았다.
Claims (4)
- 혈소판 수의 감소를 요하는 사람에게 유효량의 하기 화학식 I의 화합물 또는 제약학적으로 허용되는 그의 염 또는 용매화물을 투여하는 것을 포함하는, 사람에 있어서 혈소판 수를 감소시키는 방법.<화학식 I>상기 식에서, R1및 R3은 독립적으로 -H, -CH3, -CO(C1-C6알킬) 또는 -COAr (여기에서, Ar은 임의로 치환된 페닐임)이고;R2는 피롤리딘, 피페리딘 및 헥사메틸렌이미노로 이루어진 군 중에서 선택된다.
