Claims (58)
- 1.生きている患者の腫瘍を治療する外科治療方法において、 (a)前記腫瘍の少なくとも一部を外科的に除去して患者の残りの組構内にキャ ビティを形成する工程と、 (b)前記キャビティに膨張自在の治療装置を配置する工程と、 (c)前記キャビティを囲む残りの組織を前記治療装置により治療する工程とを 備えることを特徴とする外科治療方法。
- 2.前記工程(c)は膨張自在の装置が前記キャビティを占めることにより治療 流体を前記キャビティを囲む前記残りの組織に極めて接近して配置するように前 記膨張自在の装置を前記治療流体で膨張させることを特徴とする請求の範囲第1 項に記載の方法。
- 3.前記腫瘍は脳腫瘍であり、 前記工程(c)は前記膨張自在の装置を前記キャビティの容積よりも大きくない 体積に膨張させて正常な脳組織の圧縮またはゆがみを防止することを特徴とする 請求の範囲第2項に記載の方法。
- 4.前記工程(c)は患者に更に外科切開を行なうことなく前記治療が非侵略的 に行なわれることを特徴とする請求の範囲第1項に記載の方法。
- 5.前記工程(c)は前記キャビティを囲む前記残りの組織に前記治療装置によ り熱を加えることを特徴とする請求の範囲第1項に記載の方法。
- 6.前記工程(c)は前記キャビティを囲む前記残りの組織に前記治療装置によ り放射線を当てることを特徴とする請求の範囲第1項に記載の方法。
- 7.前記工程(c)は前記キャビティを囲む前記残りの組織に前記治療装置によ り化学治療を施用することを特徴とする請求の範囲第1項に記載の方法。
- 8.前記工程(c)は前記キャビティを囲む前記残りの組織に前記治療装置によ り熱と放射線を同時に当てることを特徴とする請求の範囲第1項に記載の方法。
- 9.前記工程(c)は前記キャビティを囲む前記残りの組織に前記治療装置によ り熱と化学治療とを同時に施すことを特徴とする請求の範囲第1項に記載の方法 。
- 10.前記工程(c)は前記キャビティを囲む前記残りの組織に前記治療装置に より放射線と化学治療とを同時に施すことを特徴とする請求の範囲第1項に記載 の方法。
- 11.前記工程(c)は前記キャビティを囲む前記残りの組織に前記治療装置に より熱と放射線と化学治療とを同時に施すことを特徴とする請求の範囲第1項に 記載の方法。
- 12.前記工程(b)では前記膨張式装置がカテーテルにより注入受け器に接続 され、 前記方法は更に前記注入受け器を皮下に植え込む工程を更に備えることを特徴と する請求の範囲第1項に記載の方法。
- 13.前記工程(c)は治療流体を前記皮下注入受け器に経皮注入し、次いで前 記治療流体を前記注入受け器から前記カテーテルを介して前記膨張式装置へ流し て前記膨張自在の装置を前記治療流体で膨張させることにより前記治療流体を前 記キャビティを囲む前記残りの組織に極めて接近して配置することを特徴とする 請求の範囲第12項に記載の方法。
- 14.前記注入は皮下針により行なうことを特徴とする請求の範囲第13項に記 載の方法。
- 15.前記腫瘍は脳腫瘍であり、 前記注入受け器の前記植え込みは前記患者の頭蓋骨と頭皮との間に前記注入受け 器を植え込み、前記カテーテルを患者の頭蓋骨の穴に挿通することを特徴とする 請求の範囲第12項に記載の方法。
- 16.前記注入受け器は患者の頭蓋骨の座ぐりに収容されることを特徴とする請 求の範囲第15項に記載の方法。
- 17.患者の頭蓋骨にX線を当てることにより前記注入受け器を配置する工程を 更に備えることを特徴とする請求の範囲第16項に記載の方法。
- 18.患者の身体にX線を当てることにより前記注入受け器を配置する工程を更 に備えることを特徴とする請求の範囲第12項に記載の方法。
- 19.生きている患者の腫瘍を治療する外科治療方法において、 (a)前記腫瘍の少なくとも一部を外科的に除去する工程と、 (b)放射線、熱および化学治療よりなる群から選ばれる少なくとも2つの治療 法を前記腫瘍が除去された場所を囲む残りの組織に同時に施用する工程とを備え る外科治療方法。
- 20.前記腫瘍は脳腫瘍であることを特徴とする請求の範囲第19項に記載の方 法。
- 21.前記工程(b)は患者に更に外科切開を行なうことなく非侵略的に行なわ れることを特徴とする請求の範囲第19項に記載の方法。
- 22.前記工程(b)は前記工程(a)において植え込まれた膨張性カテーテル の流体を前記腫瘍を除去した前記場所に配置することにより行なうことを特徴と する請求の範囲第21項に記載の方法。
- 23.前記カテーテルと連通する受け器に前記流体を皮下注入することにより前 記流体を前記膨張性カテーテルに配置することを特徴とする請求の範囲第22項 に記載の方法。
- 24.前記工程(b)は前記治療法の3つ全てを同時に施用することを特徴とす る請求の範囲第19項に記載の方法。
- 25.生きている患者から腫瘍を外科的に除去することにより残されたキャビテ ィを囲む組織の治療を行なう植え込み式治療装置において、 前記キャビティ内に配置されるように構成された膨張自在のバルーンと、 治療流体の経皮注入を受ける治療流体受け器手段と、 前記治療流体を前記受け器手段から前記膨張自在のバルーンへ運ぶように前記受 け器手段と前記バルーンとの間に接続されたカテーテル手段とを備えることを特 徴とする植え込み式治療装置。
- 26.前記受け器手段と前記バルーンとの間における前記治療流体の流れを制御 するように前記バルーンと前記受け器手段との間に接続されたバルブ手段を更に 備えることを特徴とする請求の範囲第25項に記載の装置。
- 27.前記膨張自在のバルーンは前記キャビティの容積よりも大きくない膨張体 積を有し、前記キャビティを囲む正常な組織の圧縮またはひずみを防止する手段 を備えることを特徴とする請求の範囲第25項に記載の装置。
- 28.前記受け器手段は皮下植え込み式受け器手段であることを特徴とする請求 の範囲第25項に記載の装置。
- 29.前記バルーン手段は外側バルーン壁内に収容される内側バルーン壁を有す る二重壁バルーンであり、前記内側バルーン壁は放射性流体を内部に収容して保 持するように非孔質であり、前記外側バルーン壁は前記内側バルーン壁と外側バ ルーン壁との間のスペースに化学治療流体を収容しかつ該化学治療流体を分散さ せるように多孔質であることを特徴とする請求の範囲第25項に記載の装置。
- 30.前記受け器手段は前記放射性流体を受ける第1の受け器手段であり、前記 カテーテル手段は前記第1の受け器手段と前記内側バルーン壁との間に接続され た第1のカテーテル手段であり、 前記装置は更に 前記化学治療流体の経皮注入を受ける第2の治療流体受け器手段と、 前記第2の受け器手段と、前記内側壁および前記該側壁の間の前記スペースとの 間に接続された第2のカテーテル手段とを更に備えることを特徴とする請求の範 囲第29項に記載の装置。
- 31.前記バルーン手段は前記内側壁と前記外側壁との間にスポンジの層を有す ることを特徴とする請求の範囲第29項に記載の装置。
- 32.前記治療流体が前記バルーンにあるときに前記治療流体を非侵略的に加熱 するように前記バルーンと作動連係された加熱手段を更に備えることを特徴とす る請求の範囲第25項に記載の装置。
- 33.前記バルーン手段にある前記治療流体の温度を監視する監視手段を更に備 えることを特徴とする請求の範囲第32項に記載の装置。
- 34.前記監視手段は温度に関連する振動周波数を有する植え込み式結晶オシレ ータと、該オシレータの前記振動周波数を感知する外部手段とを有することを特 徴とする請求の範囲第33項に記載の装置。
- 35.前記加熱手段は超音波エネルギを前記バルーン内の前記治療流体に非侵略 的に伝送して前記治療流体を加熱する外部超音波伝送手段からなることを特徴と する請求の範囲第32項に記載の装置。
- 36.前記加熱手段は無線周波数電磁エネルギを前記バルーン内の前記治療流体 に非侵略的に伝送して前記治療流体を加熱する外部無線周波数電磁エネルギ伝送 手段からなることを特徴とする請求の範囲第32項に記載の装置。
- 37.前記加熱手段は、 外部マイクロ波送信機と、 昇温するようにマイクロ波エネルギを非侵略的に受ける皮下植え込み式マイクロ 波受信機と、該受信機手段から前記バルーンの前記治療流体に熱を導くように前 記受信機から前記バルーンの中へ延びる導体手段とを備えることを特徴とする請 求の範囲第32項に記載の装置。
- 38.前記受け器手段は少なくとも一部がX線不透過性でありかつ患者の頭蓋骨 内に座ぐりされるように構成されており、 前記装置は受け器手段を覆う患者の頭皮に一時的に取着されるように構成された 金属クリッドを更に備え、該金属クリッドは患者の頭蓋骨にX線を照射すること により前記金属クリッドに対する前記受け器手段の位置を定めることができるよ うになっていることを特徴とする請求の範囲第25項に記載の装置。
- 39.生きている患者から腫瘍を外科的に除去することにより残されたキャビテ ィを囲む組織を治療する植え込み式装置において、 前記キャビティの少なくとも一部を充填する膨張自在のバルーン手段と、 放射線、熱および化学治療よりなる群から選ばれる少なくとも2つの治療法を前 記キャビティを囲む残りの組織に同時に非侵略的に施用するように前記バルーン 手段に作動連係された治療手段とを備えることを特徴とする植え込み式治療装置 。
- 40.前記治療手段は、 放射性治療流体の経皮注入を受ける治療流体受け器手段と、 前記受け器手段から前記膨張自在のバルーン手段へ前記放射性治療流体を運ぶよ うに前記受け器手段と前記バルーン手段との間に接続されたカテーテル手段を更 に備えることを特徴とする請求の範囲第39項に記載の装置。
- 41.前記バルーン手段は外側バルーン壁内に収容される内側バルーン壁を有す る二重壁バルーンであり、前記外側バルーン壁は多孔質であることにより前記内 側壁と外側壁との間のスペースに収容された化学治療流体を前記多孔質外側壁を 介して分散させる手段を提供することを特徴とする請求の範囲第39項に記載の 装置。
- 42.前記治療手段は前記バルーン手段に治療流体を充填する手段と、 前記治療流体が前記バルーン手段に含まれているときに前記治療流体を非侵略的 に加熱するように前記バルーン手段と作動連係された加熱手段とを更に備えるこ とを特徴とする請求の範囲第39項に記載の装置。
- 43.前記バルーン手段にある前記治療流体の温度を監視する監視手段を更に備 えることを特徴とする請求の範囲第42項に記載の装置。
- 44.前記治療手段は前記治療法の3つ全てを同時に非侵略的に施用する手段で あることを特徴とする請求の範囲第39項に記載の装置。
- 45.生きている患者から脳腫瘍を外科的に除去することにより残されたキャビ ティを囲む残りの組織の治療を行なう放射線治療装置において、 前記キャビティに配置されかつ内部の放射性治療流体を前記残った組織に極めて 接近して保持するように構成された膨張自在の溜めと、 該溜めが前記キャビティ内に配置されてから前記放射性治療流体を前記溜めに導 くカテーテル手段とを備えることを特徴とする放射線治療装置。
- 46.前記放射性治療流体の経皮注入を受けるように配設されかつ前記カテーテ ルに接続された皮下植え込み式治療流体受け器手段を更に備えることを特徴とす る請求の範囲第45項に記載の装置。
- 47.前記膨張自在の溜めは、前記キャビティの容積よりも大きくない膨張容積 を有することにより、前記残りの組織の圧縮またはゆがみを防止する手段を構成 することを特徴とする請求の範囲第45項に記載の装置。
- 48.生きている患者から脳腫瘍を外科的に除去することにより残されたキャビ ティを囲む残った組織を治療する熱治療装置において、 前記キャビティに配置されかつ前記残った組織に極めて接近して内部に治療流体 を保持するように構成された膨張自在の溜めと、 該溜めが前記キャビティに配置されてから前記溜めに前記治療流体を導くカテー テル手段と、前記治療流体が前記溜めにあるときに前記治療流体を加熱するよう に前記溜めに作動連係された加熱手段とを備えることを特徴とする熱治療装置。
- 49.前記治療流体の経皮注入を受けるように配設されかつ前記カテーテル手段 に接続された皮下植え込み式治療流体受け器手段を更に備えることを特徴とする 請求の範囲第48項に記載の装置。
- 50.前記バルーン手段にある前記治療流体の温度を監視する監視手段を更に備 えることを特徴とする請求の範囲第48項に記載の装置。
- 51.前記監視手段は植え込み式結晶オシレータと、該オシレータの振動周波数 を感知する外部手段とを有することを特徴とする請求の範囲第50項に記載の装 置。
- 52.前記加熱手段は超音波エネルギを前記溜めの前記治療流体に伝送して前記 治療流体を加熱する外部超音波伝送手段を備えることを特徴とする請求の範囲第 48項に記載の装置。

- 54.前記加熱手段は 外部マイクロ波送信機と、 昇温するようにマイクロ波エネルギを受ける皮下植え込み式マイクロ波受信機手 段と、 前記受信機手段から前記溜めの前記治療流体へ熱を導くように前記受信機手段か ら前記溜めの中へ延びる導体手段とを備えることを特徴とする請求の範囲第48 項に記載の装置。
- 55.生きている患者から脳腫瘍を外科的に除去することにより残されたキャビ ティを囲む残った組織を治療する化学治療装置において、 前記キャビティ内に配置されかつ化学治療流体を内部に保持するように構成され 、しかも前記残った組織と接触するように前記化学治療流体を分散させる多孔質 外側壁を有する膨張自在の溜めと、 前記溜め手段が前記キャビティ内に配置されてから前記化学治療流体を前記溜め 手段へ導くカテーテル手段とを備えることを特徴とする化学治療装置。
- 56.前記化学治療流体を一時的に保持するように前記溜め手段に配置されたス ポンジ手段を更に備えることを特徴とする請求の範囲第55項に記載の装置。
- 57.前記溜め手段は前記多孔質外側壁に収容される非孔質の内側壁を更に備え 、前記カテーテル手段は前記化学治療流体を前記スペースに導くように前記内側 壁と前記外側壁との間に画定されたスペースに接続されていることを特徴とする 請求の範囲第55項に記載の装置。
- 58.前記カテーテル手段は第1のカテーテル手段であり、 前記装置は 前記溜め手段の前記内側壁内に画定された内部に第2の流体を導く第2のカテー テル手段と、前記化学治療流体および前記第2の流体の経皮注入を受けるように 前記第1および第2のカテーテル手段にそれぞれ接続され第1および第2の皮下 植え込み式流体受け器手段とを備えることを特徴ヒする請求の範囲第57項に記 載の装置。
