Description
本出願は、2007年10月1日に出願された米国の同時係属特許出願第11/865,662号の優先権および恩恵を主張しており、その特許出願の全体は参照により本明細書に組み込まれる。
高強度の集束超音波(約20KHzよりも大きい周波数、より一般的には50KHz〜5MHzの周波数の音波)により生成されたような熱エネルギーを用いて、患者の体内の内部組織を治療的に処置することができる。例えば、超音波を用いて腫瘍(例えば、乳房の腫瘍および子宮筋腫)を除去することができ、それにより侵襲的手術の必要性が無くなる。このため、超音波エネルギーを生成するために電気信号により駆動されるトランスデューサ素子を有するフェイズドトランスデューサアレイは、患者の体外に配置され得るが、除去されるべき標的組織塊に非常に接近して配置される。超音波エネルギーが患者の体内の標的組織塊に対応する「焦点域(focal zone:震源域)」に集束されるように、トランスデューサは、幾何学的に形づくられて配置される。組織にわたる波動伝搬の間、超音波エネルギーの一部が、好適には焦点域の標的組織塊において吸収されて、温度上昇を生じ、最終的には細胞壊死につながる。トランスデューサの焦点域は、トランスデューサ素子のそれぞれに入力される電気信号の振幅と位相を別個に調整することにより迅速に移動され得る。超音波トランスデューサの焦点サイズ及び焦点距離は、超音波周波数、トランスデューサの焦点深度および開口サイズに依存する。標的組織塊のサイズが焦点域のサイズよりも大きいことが多いので、トランスデューサは、標的組織塊内の多数の標的部位に連続的に焦点合わせされて活性化され、標的体積を完全に凝固させることができる。連続的な「音波破砕」を用いて、所望のサイズ及び形状の、腫瘍のような全組織構造の凝固壊死を生じさせる。
本発明の第1の態様によれば、画像誘導治療システムが提供される。そのシステムは、治療エネルギービームを放射するように構成された熱治療装置を含む。一実施形態において、熱治療装置は、患者にエネルギービームを経皮的に放射するように構成された外部熱治療装置である。別の実施形態において、熱治療装置は、超音波トランスデューサであり、その場合、エネルギービームは超音波エネルギービームとなる。例えば、超音波トランスデューサは、多数の別個に制御されるトランスデューサ素子からなることができ、熱照射線量の特性は、個々のトランスデューサ素子に供給される個々の駆動信号の位相、周波数、及び振幅の1つ又は複数を調整することにより制御され得る。システムは更に、標的組織塊および熱治療装置の画像を取得するように構成されたイメジーング装置(例えば、磁気共鳴画像診断(MRI)装置)を含む。一実施形態において、熱治療装置およびイメジーング装置は異なる装置であるが、熱治療装置およびイメジーング装置は同じ装置とすることができる。
概して図1を参照すると、本発明の一実施形態に従って構成された画像誘導治療システム10がここで説明される。システム10は、患者14の体内領域内の標的組織塊16(例えば、腫瘍)を治療するために、システムの操作者12により操作されるように設計される。システム10は概して、集束熱治療サブシステム18、患者テーブル20、イメジーングサブシステム22、計画装置24、ユーザインターフェース26、及び動き補償プロセッサ28を含む。留意すべきは、図1に示された要素は本質的に機能的であり、これら機能を任意の方法で実行する構造に制限することは意図されていない。例えば、機能ブロックの幾つかは、単一の装置で具現化されることができ、又は機能ブロックの1つは、複数の装置で具現化され得る。また、機能は、ハードウェア、ソフトウェア、又はファームウェアで実施され得る。
Claims (1)
- 標的組織塊と熱治療装置との間の非周期的相対運動にもかかわらずに前記標的組織塊に熱治療を適用するための画像誘導治療システムであって、
治療エネルギービームを放射するように構成された前記熱治療装置と、
走査ウィンドウ内で前記標的組織塊および前記熱治療装置の画像を取得するように構成された磁気共鳴イメジーング装置と、
前記標的組織塊および前記熱治療装置の前記取得された画像に基づいて、共通の座標系において前記熱治療装置および前記標的組織塊の個々の位置を追跡し、且つ(i)前記熱治療装置および前記標的組織塊の個々の位置および(ii)前記治療エネルギービームの焦点域と前記熱治療装置の位置との間の相関関係に基づいて前記治療エネルギービームの焦点域と前記標的組織塊との間の相対移動を間接的に追跡するように構成されたプロセッサと、
前記熱治療装置から離れて前記標的組織塊に前記治療エネルギービームを集束するために、前記熱治療装置を制御するように構成されたコントローラとを含み、
前記コントローラが、前記非周期的相対運動にもかかわらずに前記標的組織塊に前記治療エネルギービームの焦点を自動的に維持するように前記プロセッサに応じ、
前記熱治療装置が、フェイズドアレイの超音波トランスデューサであり、前記治療エネルギービームが超音波エネルギービームであり、
前記走査ウィンドウの位置が、前記プロセッサからの制御信号に基づいて、前記標的組織塊の位置と一致するように調整される、画像誘導治療システム。
