Description
米国特許第4,300,557号は、例えば、送達すべき薬物を充填することが可能なシリコーンカプセルの形態の眼内インプラントについて記載している。カプセルは、眼に切開を施し、カプセルを挿入し、切開部分を閉鎖することにより、眼の硝子体領域に挿入される。カプセルはしばらくの間その場所に止まり、眼に第2の外科的切開を加え、デバイスを取り出すことにより、除去することができる。カプセルは、眼の表面を通り、眼の外部へ延びる、その後の薬物の注入に便利な付着したチューブを有する。硝子体内では、デバイスは固定されておらず、自由に動き回ることができる。

図1〜3に示すとおり、送達デバイス1は、近位端3と遠位端4とを有するリザーバ2を備えている。リザーバ2の近位端3近傍に位置するのは、リザーバ2内への所望の剤の注入のための注入ポート5である。注入ポート5を通じて注入された剤は、処置部位に送達機構6を通じて送達される。
送達デバイス1は、一般的に以下の手順で用いられる。送達デバイス1はリザーバ2が空の状態、好ましくは図1A〜4Aに示すとおり圧縮された状態で用意される。中空体またはチューブ10がデバイスに備えられている場合、送達デバイス1が閉鎖のためにわずかの縫合を要するか、または縫合を要しないアクセス用の小切開を通じて移植および除去されることを可能にする薄型の形状を提供するために、リザーバ2は、好ましくは、図1Aに示すとおり、中空体またはチューブ10の周囲に圧縮される。治療剤の標的部位への直接送達のためにチューブ14が用いられている場合、チューブ14の第一端16はリザーバ2の遠位端4または中空体もしくはチューブ10の遠位端12に接続される。
その後、送達デバイス1は、手で、または、例えばシリンジ様デバイスなどの当業者に知られた種々の挿入デバイスを用いて、切開を通じて挿入される。ひとたび安全に眼の内部に入ると、本適用においてチューブ14が用いられている場合、チューブ14を処置位置に設置するために、マイクロ鉗子または類似の設置機構を用いてもよい。
本発明はまた、1または2以上の本発明のデバイスを含むキットであって、好ましくは無菌条件下で包装されたものを包含する。本発明のキットはまた、本デバイスとともに用いるための、好ましくは無菌条件で包装された、例えば1または2以上のチューブ14、1または2以上のリザーバ2、固定機構8を強膜に縫合または固定する手段など、および/または本デバイスおよびキットの他の構成要素を使用するための説明書を含むことができる。

Claims (40)
- 患者の眼の内部に剤を送達するための持続的送達デバイスであって、
近位端と遠位端とを有する硬質の中空体、
近位端と遠位端とを有するリザーバであって、リザーバが硬質の中空体を少なくとも部分的に収容し、硬質の中空体の近位端がリザーバの外部に延び、リザーバが、硬質の中空体周囲での折り畳み、巻回および/または圧縮を可能にする柔軟な材料で製造され、デバイスが、閉鎖するのにわずかな縫合を要するか、または縫合を要しない切開を通じて挿入できるようにする、前記リザーバ、
リザーバ中に剤を注入するための、硬質の中空体の近位端近傍の注入ポート、
硬質の中空体の内容物とリザーバの内容物との間の連通を提供する、硬質の中空体における少なくとも1つの開口部、および
剤をリザーバから患者の眼の内部に所望の速度で送達するための送達機構を備え、
該送達機構が、リザーバの、剤に透過性の材料で製造された部分を備え、該剤に透過性の材料で製造された部分のパーセンテージが剤の送達速度を制御する、前記送達デバイス。 - 送達機構が、リザーバに少なくとも1つの開口部を備えている、請求項1に記載の送達デバイス。
- 送達機構が、リザーバの遠位端に単一の開口部を備えている、請求項2に記載の送達デバイス。
- リザーバの少なくとも1つの開口部の大きさおよび/または数が、剤の送達速度を制御する、請求項2に記載の送達デバイス。
- リザーバの少なくとも1つの開口部を通じた剤の送達速度を制御するための機構をさらに備えている、請求項2または3に記載の送達デバイス。
- 治療剤の送達速度を制御するための機構が、リザーバの少なくとも1つの開口部をカバーする多孔性のライニングを備えている、請求項5に記載の送達デバイス。
- 治療剤の送達速度を制御するための機構が、リザーバの少なくとも1つの開口部をカバーするマトリックス材料、拡散制限材料または速度制限膜を備えている、請求項5に記載の送達デバイス。
- 送達機構が、リザーバの遠位端から延びる中空チューブをさらに含む、請求項1〜7のいずれか一項に記載の送達デバイス。
- 注入ポートの近傍に、送達デバイスを所定の場所に固定するためのハブをさらに備えた、請求項1〜8のいずれか一項に記載の送達デバイス。
- 硬質の中空体が近位端と遠位端とを有し、硬質の中空体の近位端がリザーバの外部に延び、かつ、注入ポートを備えている、請求項1〜9のいずれか一項に記載の送達デバイス。
- 硬質の中空体の遠位端がリザーバの外部に延び、かつ、さらなる送達機構を備えている、請求項1〜10のいずれか一項に記載の送達デバイス。
- リザーバに収められた硬質の中空体の部分に少なくとも1つの開口部をさらに備え、それにより、該少なくとも1つの開口部が、硬質の中空体の内容物とリザーバの内容物との間の連通を提供する、請求項11に記載の送達デバイス。
- リザーバが弾性材料で製造されており、それにより、剤が充填された場合にリザーバが拡張する、請求項1〜12のいずれか一項に記載の送達デバイス。
- リザーバ内の剤に対する弾性材料の圧力が、送達機構を通じた剤の送達のための駆動力を提供する、請求項13に記載の送達デバイス。
- 患者に剤を送達するための持続的送達デバイスであって、
近位端と遠位端とを有する硬質の中空体、
硬質の中空体の近位端における、剤注入用の注入ポート、
硬質の中空体の少なくとも一部をカバーする柔軟なリザーバであって、閉鎖するのにわずかな縫合を要するかまたは縫合を要さない切開を通じて前記送達デバイスを挿入できるように、前記硬質の中空体の周囲で折り畳み、巻回または圧縮を可能にする柔軟な材料で製造されている、前記リザーバ、
硬質の中空体とリザーバとの間の連通を提供するための、硬質の中空体における少なくとも1つの開口部、および
患者に剤を送達するための送達機構
を備え、リザーバと連通する該送達機構によって、治療剤が注入ポートを通じて注入され、リザーバに移動し、そして、注入ポートを閉鎖すると、治療剤が送達機構を通じて所望の速度でリザーバから流出する、前記送達デバイス。 - 硬質の中空体の遠位端がリザーバの外部に延び、かつ、送達機構を備えている、請求項15に記載の送達デバイス。
- 送達機構を通じた剤の送達速度を制御する機構をさらに備えた、請求項15または16に記載の送達デバイス。
- 剤の送達速度を制御する機構が、該送達機構をカバーする多孔性ライニングを備えている、請求項17に記載の送達デバイス。
- 剤の送達速度を制御する機構が、該送達機構をカバーするマトリックス材料、拡散制限材料または速度制限膜を備えている、請求項17に記載の送達デバイス。
- 送達機構が硬質の中空体の遠位端を含み、かつ、剤の送達速度を制御する機構が、該硬質の中空体の遠位端に複数の開口部を形成することを含む、請求項17に記載の送達デバイス。
- 硬質の中空体の遠位端における複数の開口部の大きさおよび/または数が、剤の送達速度を制御する、請求項20に記載の送達デバイス。
- 送達機構が、リザーバの、剤に透過性の材料で製造された部分を少なくとも含む、請求項15に記載の送達デバイス。
- 送達機構が、リザーバにおける少なくとも1つの開口部を含む、請求項15に記載の送達デバイス。
- 送達機構が、リザーバの遠位端の単一の開口部を含む、請求項23に記載の送達デバイス。
- リザーバにおける少なくとも1つの開口部の大きさおよび/または数が、剤の送達速度を制御する、請求項23に記載の送達デバイス。
- 送達機構が、硬質の中空体の遠位端から延びている中空チューブである、請求項15に記載の送達デバイス。
- デバイスを処置領域に固定するためのハブをさらに備えた、請求項15〜26のいずれか一項に記載の送達デバイス。
- 硬質の中空体が円筒形の形状である、請求項15〜27のいずれか一項に記載の送達デバイス。
- 注入ポートが、注入可能な自己密封型の材料で製造されている、請求項15〜27のいずれか一項に記載の送達デバイス。
- 注入可能な自己密封型の材料が、シリコーンゴム、シリコーンエラストマーおよびポリオレフィンから選択される、請求項29に記載の送達デバイス。
- 空の送達デバイスの断面の最大寸法が、約0.25mm〜約1mmの範囲である、請求項15または30に記載の送達デバイス。
- 空の送達デバイスの断面の最大寸法が、約0.25mm〜約0.5mmの範囲である、請求項31に記載の送達デバイス。
- デバイスが、約0.5cm〜1.5cmの範囲の長さを有する、請求項1〜32のいずれか一項に記載の送達デバイス。
- 中空チューブが、約1cm〜約2.5cmの範囲の長さを有する、請求項8または26に記載の送達デバイス。
- 中空チューブが、約0.1mm未満の外径、および約0.001mm〜約0.007mmの範囲の内径を有する、請求項8、26または34に記載の送達デバイス。
- 中空チューブが、約0.001mm〜約0.005mmの範囲の内径を有する、請求項35に記載の送達デバイス。
- 折り畳み可能な材料がエラストマー性であり、それにより、リザーバのエラストマー性を利用して、剤をリザーバから処置部位へ所望の速度で送達する、請求項15に記載の送達デバイス。
- 持続的送達デバイスであって、
近位端と遠位端とを有する硬質の中空体;
前記硬質の中空体の近位端にある剤の注入のための注入ポート;
前記硬質の中空体の少なくとも一部をカバーする折り畳み可能な材料で製造されたリザーバであって、該硬質の中空体の遠位端が該リザーバの外部へ延びた、前記リザーバ;
前記硬質の中空体と前記リザーバとの間の連通を提供する、硬質の中空体における少なくとも一つの開口部;および
剤をリザーバから患者の眼の内部に所望の速度で送達するための送達機構
を備え、治療剤が、前記注入ポートを通じて前記硬質の中空体に注入され、前記リザーバに移動し、そして、前記注入ポートを閉鎖すると、前記送達機構を通じてリザーバの外部へ流出し、デバイスが、閉鎖するのにわずかな縫合を要するか、または縫合を要しない切開を通じて挿入できる、前記送達デバイス。 - 請求項1〜38のいずれか一項に記載の送達デバイスの1種または2種以上、ならびに
該デバイスとともに用いるための、1種または2種以上のチューブ、1種または2種以上のリザーバ、固定機構を縫合または固定する手段、および/または該デバイスおよび前記デバイスの使用のための説明書からなる群より選択される1種または2種以上の他の構成要素
を含む、医療デバイスキット。 - 1種または2種以上の送達デバイスが無菌条件下で包装されている、請求項39に記載のキット。
