Description

加えて、CRF2Rは、骨格筋萎縮の調節及び肥大の誘導に関与する。骨格筋は、作業又は代謝要件についての生理学的要求の変化に容易に適応する形成組織である。肥大とは、骨格筋量の増大をいうが、骨格筋萎縮とは、骨格筋量の減少をいう。急性の骨格筋萎縮の様々な原因としては:外科手術、ベッド上療養又は骨折に起因する非活動;脊椎損傷、自己免疫疾患又は感染性疾患に起因する除神経/神経障害;無関係の状態でのグルココルチコイドの使用;感染又は他の原因に起因する敗血症;疾病又は飢餓に起因する栄養制限;及びスペーストラベルが挙げられるが、これらに限定されない。骨格筋萎縮は、正常な生物学的プロセスを通じて生じるが、特定の医学的状況では、この正常な生物学的プロセスは結果として、筋萎縮のレベルを減弱することになる。例えば、急性の骨格筋萎縮は、整形処置に付随する固定が挙げられるが、それに限定されない固定のある患者のリハビリテーションに大きな限界を呈する。このような場合、骨格筋萎縮を逆転させるのに必要なリハビリテーション期間は、もとの傷害から回復するために必要な期間よりもかなり長いことが多い。このような急性の非活動性萎縮は、筋肉の機能及び量の実質的な加齢関連欠損を既に被っている可能性のある高齢者では特に問題である。なぜならこのような萎縮は、永続的な身体障害及び早発性の死亡をもたらし得るからである。
筋ジストロフィーとは、骨格筋衰弱の選択的分布によって臨床的に識別される、ある群の遺伝性、進行性の筋障害を包含する。筋ジストロフィーの2つの最も一般的な形態は、デュシェンヌ(Duchenne)型筋ジストロフィー及びベッカー(Becker)型筋ジストロフィーであり、その各々はXp21遺伝子座に位置するジストロフィー遺伝子における変異の遺伝から生じる。他のジストロフィーとしては、以下が挙げられるがそれらに限定されない:p94カルパイン、アドハリン(adhalin)、γサルコグリカン及びβサルコグリカンの遺伝子座を含む多数の遺伝子座の変異から生じる肢帯型筋ジストロフィー;顔面肩甲上腕筋ジストロフィー(ランドゥジー・デジュリン(Landouzy-Dejerine)筋ジストロフィー)、筋強直緊張性ジストロフィー及びエメリードレイフュス(Emery-Dreifus)筋ジストロフィー。ほぼ独占的に男性で生じるデュシェンヌ型筋ジストロフィーの症状としては、アヒル歩行、つま先歩行、脊柱前弯症、頻繁な転倒、ならびに起立及び着席の困難性が挙げられる。症状は3〜7歳で始まり、ほとんどの患者は10〜12年間車椅子に拘束されて、多くは呼吸の合併症によってほぼ20歳で死ぬ。デュシェンヌ型筋ジストロフィーに対する現在の治療としては、治癒的ではないが、筋の強度の低下を遅らせて、身体障害を遅らせる、プレドニゾン(コルチコステロイド薬)の投与が挙げられる。プレドニゾンのようなコルチコステロイドは、疾患から生じる筋繊維障害によって誘発される免疫細胞の活性化及び浸潤をブロックすることによって作用すると考えられる。残念なことに、コルチコステロイド治療は骨格筋の萎縮をも生じ、これはこれらの患者の免疫応答をブロックする潜在的利益の一部を打ち消してしまう。従って、筋ジストロフィーを有する患者において筋繊維障害を遅らせて、身体障害の発生を遅らせるが、現在の治療法よりも骨格筋萎縮を生じる程度が低い治療薬剤を同定する必要性は依然として存在する。
本発明は、CRF2Rアゴニストである、単離されたペプチドを提供する。具体的に本発明は、CRF、ウロコルチンI、ウロコルチンII、ウロコルチンIII、ソーバジン、ウロテンシンI又は関連のペプチド誘導体である、単離されたペプチド又はこのペプチドをコードする核酸を提供する。本発明また、本発明の単離されたペプチドの安全且つ有効な量、及び薬学的に受容可能な賦形剤を含む薬学的組成物を提供する。本発明はさらに、単位投薬形態の単離されたペプチド、及び使用説明書を備えるキットを提供する。

表1は、CRFレセプターに結合する種々のタンパク質及びタンパク質フラグメントの配列を記載する。これらの選択された配列は、対応するジーンバンク(Genbank)又はダーウェント(Derwent)のアクセッション番号及びそれが報告されている動物種、ならびに同一又はほぼ同一のアミノ酸配列をコードする関連のヌクレオチド配列のアクセッション番号とともに含まれる。本発明のこれらの公知の配列及び新規な配列をさらに配列表に示す。
以下は、本明細書において用いた用語の定義のリストである。
本発明は、式(I)
α−β−γ−δ−ε−ζ−η−θ (I)
に従う、単離された非天然型ペプチドを包含する。
本発明のペプチドは、CRF2Rによって又はCRF2R活性によって調節される様々な疾患、障害及び状態の処置のために有用である。本明細書において用いる場合、「疾患(disease)」、「障害(disorder)」及び「状態(condition)」という用語は、交換可能に用いられる。本明細書において用いる場合、「CRF2Rによって調節された(modulated by CRF2R)」又は「CRF2R活性によって調節された(modulated by CRF2R activity)」という用語によって記載される障害は、障害、状態又は疾患であって、CRF2R活性が、この障害、又は疾患もしくは障害の生物学的兆候の1つ以上を緩和する有効な手段であるか;又は障害をもたらすかもしくは基礎にある障害の原因であるかのいずれかである生物学的カスケードの1つ以上の点を妨害する;又はこの障害の1つ以上の症状を緩和する、障害、状態又は疾患をいう。従って、「調節(modulation)」に供される障害としては、以下に関する障害が挙げられる:(1)CRF2R活性が、感染によって、照射によって、内部刺激によって、又は他のいくつかの原因によって遺伝的に変更されていようといまいと、CRF2R活性の欠失が、この障害又は1つ以上の生物学的兆候の「原因(cause)」である障害;(2)この疾患もしくは障害、又はこの疾患もしくは障害の観察可能な兆候(単数又は複数)が、CRF2R活性によって緩和される障害(CRF2R活性の欠失は、この疾患もしくは障害、又はその観察可能な兆候に原因として関連している必要はない);(3)CRF2R活性が、その疾患もしくは障害を生じるか又はその疾患もしくは障害に関連する生化学的カスケード又は細胞的カスケードの一部を妨害する障害。これに関して、CRF2R活性は、このカスケードを変更し、それによってこの疾患、状態又は障害を制御する。
さらに別の実施形態において、CRF2R調節性障害の治療としては、肥満及び糖尿病を含む代謝性疾患が挙げられる。
さらに別の実施形態において、CRF2R調節性障害の治療としては、以下が挙げられる:疼痛軽減;腫脹軽減;アレルギー反応;アレルギー;体温低下;食欲減退;うっ血性心不全;ストレス及び不安;望ましくない低レベルの副腎皮質刺激ホルモン(「ACTH」)分泌の変更;食欲、覚醒状態及び認知機能の制御;ならびに不安障害、拒食症及び憂鬱性抑うつのようなストレスの長期間の影響を防止すること。
本発明はさらに、式(I)のペプチド、及びその変異体をコードする核酸分子を、好ましくは単離型で提供する。本明細書において用いる場合、「核酸(nucleic acid)」は、上記のように本発明のペプチドをコードするか、このようなペプチドをコードする核酸配列に相補的である、RNA又はDNAとして規定される。特に意図されるのは、ゲノムDNA、cDNA、mRNA及びアンチセンス分子、ならびに別の骨格に基づく核酸であり、又は天然の供給源に由来しようと、もしくは合成されようと代替的な塩基を含む。
本発明のペプチドは、CRF2R調節性障害の治療のための薬学的試薬としての使用が挙げられるがこれらに限定されない、様々な使用のために調製され得る。本発明の特定の実施形態については、精製されたペプチドが最も有用であるが、他の実施形態ではこのペプチドを発現する細胞株が最も有用であることが当業者には明白である。
本発明のペプチドの1つ以上のエピトープを選択的に認識する抗体も、本発明に包含される。このような抗体としては、例えば、ポリクローナル抗体、モノクローナル抗体、キメラ抗体、ヒト抗体、単鎖抗体、Fabフラグメント、F(ab’)2フラグメント、Fab発現ライブラリーを用いて生成された分子、トランスジェニックマウスで生成されたヒト抗体(ポリクローナル抗体又はモノクローナル抗体)、及び上記のいずれかのエピトープ結合フラグメントが挙げられる。
この抗体を遺伝子治療技術と組み合わせて利用して、例えば、これらの遺伝子が導入されている細胞で、又は患者組織で直接、本発明のペプチドの発現を評価することができる。
本発明のペプチドの薬理学的活性及び選択性は、公開された試験手順を用いて決定することができる。例えば、米国特許出願第09/799978号を参照のこと。CRF2R及びCRF1Rは、相同なタンパク質であるので、CRF2Rのアゴニストの特定の割合がまたCRF1Rのアゴニストとして機能することが予想される。上記で考察したように、CRF1Rの活性は、HPA系の活性化を誘導する。なぜならコルチコステロイド産生の増大は、骨格筋萎縮をもたらすからである。筋量又は筋機能の増大が所望されるほとんどの場合、HPA系を活性化することは所望されない。筋ジストロフィーに関連しない、CRF2R調節性障害の治療に有用なペプチドを選択する場合、このペプチドがCRF2Rに選択性であることが好ましい。好ましくは、このペプチドは、CRF2R対CRF1Rについて10倍の選択性(すなわち、CRF1Rに対してよりもCRF2Rに対して10倍大きい活性)、より好ましくは100倍の選択性、そして最も好ましくは1000倍以上の選択性を示す。公表された研究では、筋ジストロフィーの治療におけるコルチコステロイド療法の利益が実証されているので、CRF2Rアゴニストは、筋ジストロフィーを治療するために用いられる場合、同じレベルのCRF1Rアゴニズムを保持しているという利点があるかもしれない。従って、筋ジストロフィーの治療のためには、同様の濃度範囲にわたって、CRF2R及びCRF1Rを活性化する選択性のさらに低いペプチドが好ましい。好ましくは、このペプチドは、CRF2R対CRF1Rについて100倍選択性であり、より好ましくはCRF2R対CRF1Rについて10倍選択性、そして最も好ましくは選択性でない(すなわち、この候補化合物の活性は実質的に、CRF2R及びCRF1Rについて同様である)。また、この場合、ペプチドがCRF2Rの完全なアゴニストであるが、CRF1Rの部分的なアゴニストであることがさらに好ましいかもしれない。従って、このようなペプチドは、コルチゾール上昇の最大程度及び筋萎縮の可能性に対する本来備わった限界を有するが、一方CRF2Rを通して調節された抗萎縮効果はこの用量を増大することによって強化することができる。当業者は、当該分野で既知の方法を用いてペプチドがCRF1R又はCRF2Rの完全なアゴニストであるか又は部分的アゴニストであるかを容易に決定することができる。
本発明のペプチドの薬理学的な活性を、公表された試験手順を用いて決定することができる。例えば、骨格筋の萎縮又は肥大のモデルとしては、骨格筋萎縮のインビトロ細胞培養モデル及びインビボ動物モデルの両方が挙げられる。
本発明の別の局面は、以下を含む組成物である:(a)本発明の安全且つ有効な量のペプチド;及び薬学的に受容可能なキャリア。レミントンの薬学書(Remington's Pharmaceutical Sciences)、マック・パブリッシング・カンパニー(Mack Publishing Company)、ペンシルバニア州、イーストン(Easton,Pa.)最終版に開示されるような、標準的な薬学的処方の技術が用いられる。
本発明のペプチドが注射される場合、好ましい薬学的に受容可能なキャリアは滅菌の生理食塩水であって、血液適合性のコロイド性懸濁化剤を有し、そのpHは約7.4に調節されている、生理食塩水である。
本発明の組成物は必要に応じて、他の活性剤を含んでもよい。活性剤の非限定的な例は、PCT国際公開特許WO99/15210に列挙される。
本発明はまた、被験体に安全且つ有効な量のペプチドを投与することによって、ヒト又は他の動物被験体において、CRF2R調節性障害を治療する方法を提供する。本発明の方法は、上記の障害を予防又は治療するのに有用である。
遺伝子治療の一部として細胞に本発明の核酸を導入するために、発現ベクターを用いてもよい。このようなベクターは一般に、核酸配列の挿入を行なうために、プロモーター配列の近位に配置される好都合な制限部位を有している。転写開始領域、標的遺伝子又はそのフラグメント、及び転写終止領域を含む転写カセットを調製してもよい。転写カセットは、様々なベクター、例えば、プラスミド、レトロウイルス、レンチウイルス、アデノウイルスなど中に導入することが可能であり、ここで、ベクターを細胞中で一過性に又は安定に、通常少なくとも約1日、より好ましくは通常少なくとも約数日〜数週間、維持することができる。
本発明は、CRF2R調節性障害を予防又は治療するためのキットを備え、このキットは以下:(a)単位用量形態の式(I)のペプチド;及び(b)使用説明書を備える。このようなキットは好ましくは、多数の単位投薬量を含む。このようなキットは、その意図する用途の順序で並べられた投薬量を記入したカードを備えてもよい。このようなキットの例は、「ブリスターパック(blister pack)」である。ブリスターパックは、包装産業では周知であり、薬学的な単位投薬形態を包装するために広範に用いられる。所望の場合、記憶補助が、例えば、数、文字、もしくは他の印の形で、又はカレンダーの挿入によって提供されて、これが投薬を行ない得る治療スケジュールの日にちを示してもよい。キットの例はPCT国際公開特許WO01/45636に記載される。治療スケジュールは、医療技術の当業者の権限の範囲内である。非限定的な例としては、毎日、毎週、隔週、毎月、又は隔月が挙げられる。
ソーバジン及び他の非選択性のCRFRアゴニストは一般にCRF2R調節性障害の治療において有効ではない。なぜなら、これらのアゴニストはまたCRF1Rを活性化し、それによって望ましくない副作用を生じるからである。
表3は、本発明の種々の実施形態の競合的CRF結合を示す。
(インビトロ有効性の増大)
本発明のペプチドは、公知のネイティブな配列、例えばUroIIペプチドフラグメント(配列番号4)に比べて、特に低用量の条件下で、拡大された生物学的可用性を示す。
