Description

【発明の詳細な説明】 技術分野 本発明は、活性成分として、下記式によって表される
化学構造を有するドロロキシフェンまたはそほ医薬とし
て受容し得る塩からなる前立腺疾患、子宮内膜症および
肥満の処置薬に関する： 背景技術 ドロロキシフェン、米国特許第5,047,431号に開示さ
れた公知化合物であって、該特許には特に胸部癌の処置
と予防のための抗腫瘍剤として開示されている。ドロロ
キシフェンはまた、月経停止後または卵巣摘出後の女性
においてしばしば認められる、エストロゲンなどの欠乏
による骨の疾患の軽減にも有用である。米国特許第5,25
4,594号。
Gill−Sharmaら、J.Reproduction and Fertility
（1993）99,395は200および400mg/kg/日のタモキシフェ
ンは雄ラットの睾丸および二次性徴器官の重量を低下さ
せることを開示している。
Neubauerら、The Prostate23;245（1993）は雄ラッ
トをラロキシフェンで処置すると下腹部前立腺の後退を
生じたことを教示している。
発明の総括 本発明は、哺乳動物の子宮内膜症、肥満、良性の前立
腺肥大および前立腺癌から選択される症状または疾患を
処置する方法であって、該症状または疾患を処置するに
有効な量のドロロキシフェンまたはその医薬として受容
し得る塩を上記哺乳動物に投与することからなる方法を
提供する。
発明の詳細な説明 ドロロキシフェン（１−［４′−（２−ジメチルアミ
ノエトキシ）フェニル］−１−（３′−ヒドロキシフェ
ニル）−２−フェニルブト−１−エン）およびその医薬
として受容し得る塩はここに参考文献として記載した米
国特許第5,047,431号に記載されている。
この出願において使用されているように、“前立腺疾
患”とは良性の前立腺肥大または前立腺癌を意味する。
“処置”とは症状を治療し、軽減し、あるいは疾患また
は症状の発病を予防することを意味する。
本発明の前立腺疾患、子宮内膜症および肥満の処置薬
は活性成分としてのドロロキシフェンまたはその塩から
なる。ドロロキシフェンの医薬として受容できる塩は、
有機酸（たとえば蟻酸、酢酸、クエン酸、マレイン酸、
酒石酸、メタスルホン酸、ベンゼンスルホン酸またはト
ルエンスルホン酸）との塩、無機酸（例えば、塩酸、臭
化水素酸、硫酸またはリン酸）、およびアミノ酸（たと
えば、アルパラギン酸またはグルタミン酸）との塩のよ
うな普通に使用される無毒の塩である。これらの塩は通
常の化学者に公知の方法によって製造できる。
本発明の上記疾病および症状の処置薬はヒトを含む動
物に、カプセル、マイクロカプセル、錠剤、顆粒、粉
末、トローチ、ピル、座薬、注射剤、懸濁剤およびシロ
ップのような従来からの剤形で経口的あるいは非経口的
に投与できる。
本発明の上記疾病および症状の処置薬は従来の補助剤
（たとえば、蔗糖、澱粉、マンニトール、ソルビトー
ル、ラクトース、グルコース、セルロース、タルク、リ
ン酸カルシウムまたは炭酸カルシウム）、結合剤（たと
えば、セルロース、メチルセルロース、ヒドロキシメチ
ルセルロース、ポリプロピルピロリドン、ポリビニルピ
ロリドン、ジェラチン、アラビアゴム、ポリエチレング
リコール、蔗糖またでんぷん）、崩壊剤（たとえば、で
んぷん、カルボキシメチルセルロース、ヒドロキシプロ
ピルスターチ、低度置換ヒドロキシプロピルセルロー
ス、重炭酸ナトリウム、リン酸カルシウムまたはクエン
酸カルシウム）、滑剤（たとえば、ステアリン酸マグネ
シウム、軽量無水ケイ酸、タルクまたはラウリル硫酸ナ
トリウム））、付香剤（たとえば、クエン酸、メントー
ル、グリシンまたはオレンジ粉末）、防腐剤（たとえ
ば、安息香酸ナトリウム、二硫化ナトリウム、メチルパ
ラベンまたはプロピルパラベン）、安定化剤（たとえ
ば、クエン酸、クエン酸ナトリウムまたは酢酸）、懸濁
化剤（たとえば、メチルセルロース、ポリビニルピロリ
ドンまたはステアリン酸アルミニウム）、分散剤（たと
えば、ヒドロキシプロピルメチルセルロース）、希釈剤
（たとえば、水）および基剤ワックス（たとえば、ココ
アバター、ホワイトペトリウムまたはポリエチレングレ
コール）のような有機または無機添加物である。上記医
薬用組成物の活性成分の量は、所望の治療効果を奏する
レベル、たとえば、経口的あるいは非経口的投与の両方
について単位剤形当たり約1mgないし100mgでよい。
上記活性成分は通常一日当たり一回ないし４回ヒト患
者に単位投与量0.25mgないし100mg投与できるが、上記
投与量は患者の年齢、体重および医学的状態ならびに投
与のタイプに応じて適当に変えられる。１日当たり一回
投与が好ましい。
下記実施例は請求の範囲に定義された本発明を説明す
るがそれらに限定するものではない。
実施例１ 前立腺重量に対する作用 生後３カ月の雄のスプラグドーリーラットに１日当た
り、媒体（10％エタノール水溶液）、エストラジオール
（30μg/kg）、テストステロン（1mg/kg）またはクエン
酸ドロロキシフェン（10mg/kg）を14日間皮下注射した
（ｎ＝6/群）。14日後、動物を殺し、前立腺を摘出し、
湿時前立腺重量を測定した。平均重量を決定し統計的有
意性（ｐ＜0.05）を決定しスチューデントのｔ−テスト
を使用して媒体処理群と比較した。
10mg/kgのクエン酸ドロロキシフェンは媒体に比べて
有意に（ｐ＜0.05）前立腺重量を低下させた。テストス
テロンはいかなる効果も示さなかったが、エストロゲン
は30μg/kgで有意に前立腺重量を低下させた。
これらのデータはクエン酸ドロロキシフェンが良性前
立腺肥大および前立腺癌の処置に有用であることを示し
た。
実施例２ 体の脂肪塊に対する効果 約450gの体重の生後10カ月の雌スプラグドーリーラッ
トを疑似手術（sham）または卵巣摘出（OVX）し、媒
体、17αエチニルエストラジオール30μg/kg/日あるい
はクエン酸ドロロキシフェン1.0,2.5または5mg/kg/日で
経口的に８週間処理した。各サブグループ６ないし７ラ
ットであった。研究最後の日に、すべてのラットの体組
成を、全身スキャンソフトウエアを備えた二重のエネル
ギーｘ線吸光光度法（Hologic QDR−1000/W）を使用し
て測定した。Faulknerら、Calcified Tissue 53、７
（1993）参照。この結果、エチニルエストラジオールあ
るいはドロロキシフェンが脂肪のない部分の体重（ｇ）
には影響を及ぼさないが、疑似手術すなわちOVX対照に
比べて、エチニルエストラジオールあるいはドロロキシ
フェン（すべての投与レベル）で処理されたOVXラット
では脂肪量（ｇ）を有意に低減させる（40−60％）こと
を示した。これらの結果はドロロキシフェンが肥満の処
置に有用な薬剤であることを示した。
実施例３ 子宮内膜症の抑制および予防 子宮内膜症を外科的に誘発させるプロトコールはJone
s,Acta Endocrinol （Copenh）106:282−８に記載の
ものと同一である。成チャールスリバースプラグドーリ
ーCDR雌ラット（200−240g）を使用した。腹側斜め下切
開を体壁の皮膚と筋肉組織に施した。右子宮角の断片を
切開し、子宮内膜から子宮筋層を分離した。その断片を
長軸方向に切った。体壁に沿って上皮で裏打ちされた子
宮内膜の５×5mm切片をポリエステルの組みひも（Ethif
lex,7−0R）を使用してその４隅を筋肉に縫い付けた。
生存可能な移植片の基準は、エストロゲン刺激の結果と
して子宮に生じるものに類似の流体の蓄積である。
子宮内膜組織の移植３週間後（＋３週）ラットを側腹
切開し、外植片の容量（長さ×巾×高さ）をmmでカリパ
スで測定し、処理を始めた。ラットに３週間10ないし10
00μg/kg/日の式Ｉの化合物を皮下注射した。子宮内膜
外植片を有するラットに３週間とうもろこし油0.1ml/日
を皮下注射して対照として役立てた。３週間の処理期間
の終わりに（＋６週）、ラットを側腹切開し、外植片の
容量を測定した。処理停止の８週後（＋14週）、ラット
を殺し、外植片を再測定した。外植片容量の統計学的分
析は分散分析による。
実施例４ クエン酸ドロロキシフェン錠剤 クエン酸ドロロキシフェン 100g 乳糖 1190g 低度置換ヒドロキシプロピルセルロース 250g ポリビニルピロリドン 50g ステアリン酸マグネシウム 10g 上述の化合物を通常の方法で一緒に混合し、得られた
混合物を打錠して各錠が10mgのクエン酸ドロロキシフェ
ンを含む10,000錠を製造した。
