Abstract
Description
本出願は、2014年10月8日に出願された「腫瘍治療に用いるためのコルテキソロンの17αモノエステルおよび17α,21−ジエステル」と題する欧州特許出願第14188063.3号に対する米国特許法第119条に基づく優先権の利益を主張し、当該文献の全開示は、参照により本明細書の一部をなすものとする。
腫瘍すなわち新生物は、その周囲の構造とは独立的に存続および成長し、生理学的用途を持たない新規の組織の塊であると定義される(非特許文献1参照)。
別段の規定が無い限り、本明細書において用いられる技術、表記、およびその他の科学用語は、本開示内容が属する技術分野の通常の知識を有するものが一般的に理解する意味を有することを意図する。いくつかの場合において、一般的に理解されている意味を有する用語を、明瞭性および/または手早い参照のために、本明細書中で定義する。よって、本明細書中のそのような定義の包含は、当該技術において一般的に理解されている意味と実質的な相違を表すものと解釈するべきではない。
今や、いくつかのコルテキソロン誘導体が、腫瘍、好ましくは上皮性腫瘍および/またはホルモン依存性腫瘍に対する、治療的に興味深い抗腫瘍特性を有することが見出された。
本明細書中に記載される化合物は、遊離形態で存在してもよいし、適切な場合には塩として存在してもよい。薬学的に許容される塩は特に重要である。なぜなら、医療目的のために下記の化合物の投与において有用であるからである。薬学的に許容可能でない塩は、単離および精製の目的のために、および特定の場合には、本発明の化合物またはその中間体の立体異性形態の分離における使用のために、製造プロセスにおいて有用である。
本発明の別の対象は、式(I)の化合物の薬物としての利用により表される。
本明細書に記載される化合物を、薬学的に許容可能なキャリア、希釈剤、補助剤またはビヒクルをさらに含む薬剤組成物中に配合することができる。1つの実施形態において、本発明は、本明細書に記載される化合物、および薬学的に許容可能なキャリア、希釈剤、補助剤またはビヒクルを含む薬剤組成物に関する。1つの実施形態において、本発明は、有効量の本明細書に記載される化合物またはその薬学的に許容可能な塩、および薬学的に許容可能なキャリア、希釈剤、補助剤またはビヒクルを含む薬剤組成物に関する。薬学的に許容可能なキャリアは、たとえば、目的とする投与形態に関して適当に選択され、かつ慣用の薬学的実施に整合する、薬学的希釈剤、賦形剤またはキャリアを含む。
本明細書に記載された化合物および組成物は、経口的、非経口的、吸入噴霧により、局所的、経直腸的、経鼻的、経頬的、経膣的、または移植された容器を経由して、投与されてもよい。本明細書で用いられる際に、用語「非経口的」は、皮下、静脈内、筋肉内、関節腔内、滑液包内、胸骨内、髄腔内、肝臓内、病変内および頭蓋内の注射または輸液技術を含むが、それらに限定されるものではない。
別の実施形態によれば、化合物、組成物、および薬剤組成物は、同時、別個または順次的投与のための組み合わせとして、少なくとも別の活性成分、好ましくは化学療法活性成分を含有してもよい。
前述のように、本発明の別の対象は、薬物としての使用のための、下式で表されるコルテキソロン17α−バレレート(「05」または「CB−03−05」とも呼称される)
本試験は、それぞれアンドロゲン受容体(AR)発現陽性または陰性の前立腺癌細胞系の代表であるLNCaP(AR+)およびPC3(AR-)におけるコルテキソロン17α−ベンゾエートのin vitro抗腫瘍活性を試験し、画定させるために実施された。試験方法は、以下の工程を含んだ。
1. 96ウェルの平底プレート中、2%チャコール処理済ウシ血清を含有する完全培地中に、3000個の癌細胞を播種した。
2. 24時間後、培養物に対して、10nMの、抗アンドロゲン化合物を有するまたは有さないDHT(ジヒドロテストステロン)、またはDMSOビヒクル(陰性の比較対照)を添加した。
3. 3日後、ATP−依存増殖アッセイを用いて、生存可能細胞数を定量化した。
結果は、Prizm統計分析ソフトウェアにおけるS字状投与量応答曲線にフィッティングした。
以下の結果は、試験1のものに対する追加の試験を含む。結果は、Prizm統計分析ソフトウェアにおける非線形回帰最小二乗カーブフィッティングにより分析した。

オスのヌードマウスにおける膵臓移植片の腫瘍成長に対するコルテキソロン17α−ベンゾエートの活性を、最も強力な抗アンドロゲンステロイドであるシプロテロンアセテート(CPA)と比較して評価した。

細胞系生存可能性試験において試験される化合物の安全性を評価するために、細胞の生存および生存可能性に影響する全ての要素を考慮すべきである。この意味において、化合物および比較剤の固有毒性の評価は、真に重要である。末梢血単核細胞(PBMC)における化合物のIC50および癌細胞系のIC50に由来する比は治療指数とされ、試験される化合物のより安全なものが何かを示す。
刺激された(被刺激) − 活発に分裂している細胞

本試験は、上皮性腸腫瘍の代表である細胞系、すなわちHT29におけるコルテキソロン17α−ベンゾエートおよびコルテキソロン17α−バレレート−21−プロピオネート(CB−03−10)のin vitro抗癌活性を試験し、画定させるために実施された。試験方法は、以下の工程を含んだ。
1. 96ウェルのプレート中に、単層のHT−29細胞を2×104細胞/mLの濃度で蒔いた。蒔いた細胞を5%CO2中37℃に維持し、24時間にわたって放置して付着させた。
2. その後、それぞれ0.16mM、0.8mM、4mM、20mM、100mM、および500mMの濃度の試験化合物とともに、72時間にわたって細胞をインキュベートした。
3. 72時間の処理後、MTT比色分析アッセイを行った。
細胞系生存可能性試験において試験される化合物の安全性を評価するために、細胞の生存および生存可能性に影響する全ての要素を考慮すべきである。この意味において、化合物および比較剤の固有毒性の評価は、真に重要である。PBMCにおける化合物のIC50および癌細胞系のIC50の間の比は治療指数とされ、安全な条件下における生成物の有効性を定義するのに重要なパラメータである。

以下の工程を含む合成スキームにしたがって、コルテキソロン17α−ベンゾエートを調製した。
以下の合成スキームにしたがって、コルテキソロン17α−バレレート−21−プロピオネート(CB−03−10)を調製した。
in vitroで確立された癌細胞系の成長を阻害するCB−03−10の能力を試験した。
癌細胞系におけるAR発現とCB−03−10の癌細胞成長を阻害する能力との間の関係をより良好に理解するために、第I表で試験した前立腺細胞系および膵臓細胞系に対して、FACSアッセイを行った。
CB−03−10の細胞毒性活性がAR発現と相関しないように見えるため、充実性腫瘍のより大きな試料を試験した。以前のパネルに、MCF7(乳癌細胞系(AR+/-))、より高度なAR発現を有する追加の膵臓細胞系(BxPC3)、および腸癌細胞系(HT29(AR-))を追加した。結果を第III表に示す。
治療指数(TI)(治療ウインドウまたは安全性ウインドウ、時として治療係数とも呼称される)は、毒性をもたらす量に対する、治療効果をもたらす治療薬の量の比較である。化合物のIC50は、ヒト血液から単離される新鮮な細胞(PBMC)において決定される。化合物の毒性は下式で決定される
(治療指数)=(安全性)/(効力)=被刺激PMBCのIC50/癌細胞のIC50
結果を第IV表に示す。
上記の試験は、異なる由来の腫瘍から誘導される癌細胞系におけるCB−03−10の強い細胞毒性活性を例証した。この細胞毒性活性は、試験された癌細胞の抗アンドロゲン受容体発現と完全には相関しない。この証拠に基づいて、ARに対する化合物の親和性を試験するためのアッセイを設計した。野生型ARに対するCB−03−10の相対的結合親和性を決定するために、life Technologies由来のPolar Screen kitを用いる競争アッセイを用いた。簡単に言えば、蛍光性アンドロゲンリガンド(Fluormone(商標) AL Green)に対してARを添加し、複合体AR−LBDを形成した。競争剤は、AR−LBDから蛍光性のFluormone(商標) AL Greenを置換し、蛍光性リガンドがその蛍光寿命中に速やかに崩壊し、低い偏光値をもたらす。非競争剤は、複合体からの蛍光性リガンドを置換せず、偏光値は高いままとなる。試験される化合物の存在下における偏光値のシフトを用いて、AR−LBDに関する試験される化合物の相対親和性を決定する。AR受容体に関するCB−03−10の親和性は1.1×10-6(IC50、モル濃度)であり、同一アッセイにおいて、ジヒドロテストステロン(AR受容体の強力な結合剤)の親和性は1.1×10-8であった。DHTに比較した際に、AR受容体に関するCB−03−10の結合親和性は低く、CB−03−10をARの潜在的結合剤であると特徴づける。
アンドロゲンおよびグルココルチコイドホルモンは、細胞、組織および動物において、相違する効果、しばしば反対の効果を引き出す。広範囲の生理学的および分子生物学的証拠が、これらの効果を媒介する受容体、アンドロゲン受容体およびグルココルチコイド受容体(それぞれ、ARおよびGR)は、互いの転写活性に影響することを示唆している。CB−03−10のアンタゴニスト活性およびアゴニスト活性を、in vitroアッセイにおいて試験した。簡単に言えば、発光に基づくレポーター分子が結合したGR結合部位を含有するDNA構造を、ヒト腎臓上皮細胞にトランスフェクションした。24時間後、細胞をアンタゴニストモードまたはアゴニストモードの下で処理した。さらなる24時間の日に、GRアゴニスト転写活性に比例する発光を定量化した。
現在使用されている細胞毒性抗癌薬の大部分は、影響を受ける細胞のアポトーシスを誘起することが示されている。異種の目標に相互作用する共通点のない薬剤がある共通の特徴を有する細胞死を誘起するという事実は、細胞毒性がこのいわゆる「プログラムされた」細胞死に関わる細胞の能力によって決定されることを示唆する。癌細胞の細胞毒性の機構がアポトーシスおよび細胞周期静止によって媒介されるか否かを決定するために,CB−03−10を評価した。癌細胞系を6ウェルの平底プレートに播種した。24時間後、試験化合物またはDMSOビヒクル(陰性の比較対照)を添加した。さらなる24時間の後に、細胞を掻き取り、フルオレセインが結合したアネキシンVおよびプロピジウムヨージドにより染色し、フローサイトメトリーにより分析した。
以前の研究から、MiaPaca2細胞においてアネキシンV染色を用いて、CB−03−10がアポトーシスを誘起することが決定された。現象をより良好に分析するため、カスパーゼ8(外因性経路に関する開始剤カスパーゼ)、カスパーゼ9(内因性経路に関する開始剤カスパーゼ)およびカスパーゼ3および7(エフェクターカスパーゼ)の酵素活性を測定した。
ヒトおよびラット血清中におけるCB−03−10の代謝に関するいくつかの見通しを得るために、特殊なアッセイを設計した。簡単に言えば、37℃のヒト血清およびラット血清中で、異なる時間にわたって化合物をインキュベートした。インキュベーション後、無傷の化合物の存在下、試料を液体クロマトグラフィーで試験した。時間経過および濃度を図13に示す。
静脈内投与(IV)、皮下投与(SC)および経口投与(PO)後のマウスの血清中で、CB−03−10の薬物動態を評価した。
以前の研究において、CB−03−10が、MiaPaca2膵臓細胞系(AR+/-)のin vitro成長を強く阻害することが観察された。この結果をin vivo移植片膵臓癌モデルに移すことができるか否かに関する調査を実施した。よく知られている抗アンドロゲンであるCPAを比較対照として用いた。簡単に言えば、matrigelに懸濁させた1×106個のMiaPaca2細胞を6週齢のオスの無胸腺ヌードマウスに皮下(SC)注射した。4日毎にデジタルキャリパーを用いて腫瘍を測定した。腫瘍体積を、式0.5236(r1)2(r2)(式中、r1<r2)にしたがって計算した。腫瘍が50mm3に到達した後に、CB−03−10および比較対照化合物による治療を開始した。DMSO/2−ヒドロキシプロピルβ−デキストリン(ビヒクル)中に希釈された化合物を、連続する28日間にわたり、40mg/Kgの濃度において、毎日、皮下的に注射(マウス当たり100μL)した。図15は、ビヒクルと比較して、CB−03−10のSC注射後のin vivo移植片モデルにおける平均腫瘍増加を示す。
以前の研究から、CB−03−10がLNCaP前立腺癌細胞系のin vitro成長を阻害することにも有効であることが観察された。この結果をin vivo移植片前立腺癌モデルに移すことができるか否かに関する調査を実施した。matrigelに懸濁させた3×106個のLNCaP細胞を6週齢のオスの無胸腺ヌードマウスに皮下注射(右脇腹)した。4日毎にデジタルキャリパーを用いて腫瘍を測定した。腫瘍を前述のように測定した。腫瘍が50mm3に到達した後に、CB−03−10および比較対照化合物による治療を開始した。投与のための調合物を、20mMクエン酸緩衝液(pH4)中の、15%ビタミンE−TPGSおよび65%の0.5%v/vCMC溶液において調製した。経口投与を、連続する28日間にわたって毎日実施した(200μL/マウスにおいて100mg/Kg)。結果を、治療の開始を基準として、腫瘍体積の平均変化としてプロットした。図16は、CB−03−10の経口投与後のin vivo移植癌モデルから得られた結果を示す。新規かつ強力な抗アンドロゲンであるエンザルタミドを、陽性の比較対照として用いた。
前立腺特異抗原、すなわちPSAは、前立腺の細胞によって生成されるタンパク質である。PSA試験は、ヒト血中のPSA濃度を測定する。前立腺癌を有する人間において、PSAの血中濃度はしばしば上昇し、PSAの血中濃度はヒト集団における前立腺癌の進行を試験するための代理マーカーとして用いられる。CB−03−10が前立腺癌の成長をin vivoで阻害できることが観察された後に、当該化合物の癌細胞からのin vitroPSA分泌を阻害する能力が決定された。96ウェル平底培養プレート中、10nMのDHTを有するまたは有さないチャコール処理済血清中にLNCaP細胞を播種した。24時間後、細胞に対して試験化合物を添加した。DMSOを陰性の比較対照として用い、エンザルタミドを陽性の比較対照として用いた。試験化合物の48時間のインキュベーションの後、上澄みを採取し、PSAに関するELISAアッセイを用いて試験し、ならびに、同細胞を細胞生存可能性評価のために溶解させた。
トリプルネガティブ乳癌(TNBC)は、新たに診断される侵襲性乳癌の約20%を占める。このサブタイプの癌は、ホルモン、エストロゲンおよびプロゲステロンに支持されていないし、多すぎるHER2受容体の存在にも支持されていない。この理由により、患者は、慣用の治療法(たとえば、タモキシフェンまたはハーセプチン)に応答しない。したがって、この癌は、化学療法に抵抗性であり、患者の生存期間が短いと特徴づけられる。
MCF7乳癌細胞(ER+、PR+、Her2+、GR+/-)
MDA−MB−231 TNBC細胞(ER-、PR-、Her2-、GR++)
であった。
これらの実施例は、特にコルテキソロン17α−バレレート−21−プロピオネート(CB−03−10)が他の既知のコルテキソロン誘導化化合物を超える優れた活性を有することを例示する。本発明者らは、in-vitroおよびin-vivoの両方において増大した結果、たとえば以下の結果を観察した。
I) 一般的in-vitro抗腫瘍活性;
II) AR発現と直接相関しないin-vitro抗腫瘍活性;
III) GR発現と直接相関するin-vitro抗腫瘍活性;
IV) 治療指数(TI);および
V) 膵臓腫瘍および前立腺腫瘍に対するin vivo抗腫瘍活性。
VI) 以下の第I表に示すデータから、コルテキソロン誘導化化合物が種々の濃度およびIC50で癌細胞を殺すことが明らかである。しかしながら、異種の由来の癌細胞系全体において、コルテキソロン誘導化系列中の他の化合物に比べて、CB−03−06およびCB−03−10が最良のIC50を示す。CB−03−05およびCB−03−10の代謝物でさえ、LNCaP前立腺癌細胞において良好なIC50値を示す(IC50 32μM)。より低いIC50は、より強力なin vitro抗腫瘍活性を証明する。

9. 別の実施形態において、前記腫瘍性疾病が、充実性腫瘍、好ましくは、前立腺カルシノーマ、乳カルシノーマ、膵カルシノーマ、肺カルシノーマ、結腸カルシノーマのような消化管カルシノーマ、腎カルシノーマ、甲状腺カルシノーマ、子宮カルシノーマ、および副腎カルシノーマのような上皮性腫瘍であることによって特徴づけられる、記載事項8の化合物である。
10. 別の実施形態において、前記上皮性腫瘍が、前立腺カルシノーマ、膵カルシノーマ、好ましくは膵臓外分泌部カルシノーマであることによって特徴づけられる、記載事項9の化合物である。
11. 別の実施形態において、本発明は、少なくとも1種の生理学的に許容可能な賦形剤とともに、少なくとも1種の記載事項1から5の何れかの式(I)の化合物を含む薬剤組成物である。
12. 別の実施形態において、本発明は、固体形態または液体形態にあることによって特徴づけられる、記載事項11の薬剤組成物である。
13. 別の実施形態において、本発明は、粉末、凍結乾燥粉末、顆粒、ペレット、錠剤またはカプセルであることによって特徴づけられる、固体形態にある記載事項12の薬剤組成物である。
14. 別の実施形態において、本発明は、溶液、乳化物、懸濁物またはシロップであることによって特徴づけられる、液体形態にある記載事項12の薬剤組成物である。
15. 別の実施形態において、本発明は、同時投与、別個投与または順次投与のための組み合わせとして、少なくとも1種の別の活性成分、好ましくは化学療法活性成分を含有することによって特徴づけられる、液体形態にある記載事項11から14のいずれかの薬剤組成物である。
16. 別の実施形態において、本発明は、前癌性病変、異形成病変、化生および腫瘍性疾病治療における使用のための、記載事項11から15のいずれかの薬剤組成物である。
17. 別の実施形態において、前記腫瘍性疾病が悪性新形成および転移を含むことによって特徴づけられる、記載事項17の薬剤組成物である。
18. 別の実施形態において、前記腫瘍性疾病が、充実性腫瘍、好ましくは、前立腺カルシノーマ、乳カルシノーマ、膵カルシノーマ、肺カルシノーマ、結腸カルシノーマのような消化管カルシノーマ、腎カルシノーマ、甲状腺カルシノーマ、子宮カルシノーマ、および副腎カルシノーマのような上皮性腫瘍であることによって特徴づけられる、記載事項17の薬剤組成物である。
19. 別の実施形態において、前記上皮性腫瘍が、前立腺カルシノーマ、膵カルシノーマ、好ましくは膵臓外分泌部カルシノーマであることによって特徴づけられる、記載事項18の薬剤組成物である。
