Claims (96)
- 心臓の卵円孔開存を治療する方法であって、該方法は、
卵円孔開存を治療するために、カテーテルデバイスを心臓の或る位置まで前進させる工程と、
カテーテルデバイスを使用して、卵円孔開存に隣接する組織にエネルギを印加し、実質的に卵円孔開存を急激に閉鎖する工程とを含む、方法。 - 前記カテーテルデバイスを使用して、組織を少なくとも部分的に並置する工程を更に含んでいることを特徴とする、請求項1に記載の方法。
- エネルギを印加する前に、組織が並置されることを特徴とする、請求項2に記載の方法。
- エネルギを印加している間に、組織を並置したまま保持する工程を更に含んでいることを特長とする、請求項3に記載の方法。
- エネルギを印加した後で、組織を並置したまま保持する工程を更に含んでいることを特徴とする、請求項4に記載の方法。

- 前記露出された拡張部材は、卵円孔開存に隣接している組織に横方向の力を供与することを特徴とする、請求項15に記載の方法。
- 前記露出された拡張部材は、心臓の左心房の中に伸びて入らずに、前記横方向の力を供与することを特徴とする、請求項16に記載の方法。
- 前記露出された拡張部材は、組織と接触することにより、前記カテーテルデバイスの断面中心を卵円孔開存の断面中心と概ね整合させることを特徴とする、請求項15に記載の方法。
- 一次中隔と二次中隔のうちの少なくとも一方の左心房表面を前記拡張部材の遠位部と接触させる工程と、
拡張部材を後退させて、卵円孔開存に隣接している組織を並置する工程とを更に含んでいることを特徴とする、請求項15に記載の方法。 - 前記拡張部材を前記ガイドカテーテル内の或る位置まで後退させる工程を更に含んでいることを特徴とする、請求項19に記載の方法。
- 前記拡張部材が前記ガイドカテーテル内に後退した後で、前記カテーテルデバイスを通してガイドカテーテルを取出す工程を更に含んでいることを特徴とする、請求項20に記載の方法。
- 前記ガイドカテーテルは、エネルギを印加する前に取出されることを特徴とする、請求項21に記載の方法。
- 前進させる前記工程は、卵円孔開存を通って延びているガイドワイヤの上を伝ってカテーテルを前進させる処置を含んでおり、ガイドワイヤはその長尺に沿った拡張部を有して、卵円孔開存の内側に張出すようにしたことを特徴とする、請求項1に記載の方法。
- 一次中隔と二次中隔のうち少なくとも一方の左心房表面を前記ガイドワイヤの遠位部と接触させる工程と、
前記ガイドワイヤを後退させて、卵円孔開存に隣接している組織を並置する工程とを更に含んでいることを特徴とする、請求項23に記載の方法。 - エネルギを印加する前記工程は、高周波エネルギ、極低温エネルギ、抵抗加熱エネルギ、超音波エネルギ、マイクロウエーブエネルギ、および、レーザエネルギのうちの少なくとも1つを印加する処置を含んでいることを特徴とする、請求項1に記載の方法。
- モノポーラ高周波エネルギとバイポーラ高周波エネルギのうちの少なくとも一方が印加されることを特徴とする、請求項25に記載の方法。
- エネルギを印加する前記工程は、前記カテーテルデバイスの一の導電性部材に電力投入する処置を含んでいることを特徴とする、請求項25に記載の方法。
- エネルギを印加する前記工程は、前記カテーテルデバイスの多数の導電性部材に電力投入することを特徴とする、請求項25に記載の方法。
- エネルギを印加する前記工程は、組織中のコラーゲンを変性させる処置を含んでいることを特徴とする、請求項1に記載の方法。

- 心臓の卵円孔開存を治療する方法であって、該方法は、
卵円孔開存を治療するために、カテーテルデバイスを心臓の或る位置まで前進させる工程と、
カテーテルデバイスを使用して卵円孔開存に隣接している組織からエネルギを除去し、実質的に卵円孔開存を急激に閉鎖する工程とを含む、方法。 - 前記カテーテルデバイスを使用して、組織を少なくとも部分的に並置する工程を更に含んでいることを特徴とする、請求項36に記載の方法。
- 前記組織は、エネルギを除去する前に並置されることを特徴とする、請求項37に記載の方法。
- エネルギを除去している最中に組織を並置する工程を更に含んでいることを特徴とする、請求項37に記載の方法。
- エネルギが供与された後で組織を並置する工程を更に含んでいることを特徴とする、請求項39に記載の方法。
- 組織を少なくとも部分的に並置する前記工程は、前記カテーテルデバイスの遠位端に隣接している組織カバー部材と組織を接触させる処置を含んでいることを特徴とする、請求項37に記載の方法。
- 前記組織カバー部材と組織を接触させる前記工程は、心臓の左心房内の血圧により組織を少なくとも部分的に並置させるようにしたことを特徴とする、請求項41に記載の方法。
- 組織を並置する前記工程は、前記組織カバー部材を使用して組織に真空力を供与する処置を含んでいることを特徴とする、請求項42に記載の方法。
- カテーテルデバイスを前進させる前記工程は、心臓の右心房にカテーテルデバイスの遠位端を設置する処置を含んでいることを特徴とする、請求項36に記載の方法。
- 前進させる前記工程は、卵円孔開存の中に延びているガイドカテーテルの上を伝ってカテーテルデバイスを前進させる処置を含んでいることを特徴とする、請求項36に記載の方法。
- 前記ガイドカテーテルは、卵円孔開存を通して心臓の左心房の中に延びることを特徴とする、請求項45に記載の方法。
- 前記ガイドカテーテルのシース部を後退させて、卵円孔開存の内側で拡張部材を露出させる工程を更に含んでおり、露出された拡張部材は卵円孔開存に隣接している組織を少なくとも部分的に並置させることを特徴とする、請求項45に記載の方法。
- 前記露出された拡張部材は卵円孔開存に隣接している組織に横方向の力を供与することを特徴とする、請求項47に記載のカテーテルデバイス。
- 前記露出された拡張部材は、心臓の左心房の中に延びることなく、横方向の力を供与することを特徴とする、請求項48に記載の方法。
- 前記露出された拡張部材は、組織に接触することにより、前記カテーテルデバイスの断面中心を卵円孔開存の断面中心と概ね整合させることを特徴とする、請求項47に記載の方法。
- 一次中隔と二次中隔のうちの少なくとも一方の左心房表面を前記拡張部材の遠位端と接触させる工程と、
拡張部材を後退させて、卵円孔開存に隣接している組織を並置する工程とを更に含むことを特徴とする、請求項47に記載の方法。 - 前記拡張部材を前記ガイドカテーテルの内側の或る位置まで後退させる工程を更に含んでいることを特徴とする、請求項51に記載の方法。
- 前記拡張部材が前記ガイドカテーテルの内部に後退させられた後で、カテーテルデバイスを通してガイドカテーテルを除去する工程を更に含んでいることを特徴とする、請求項52に記載の方法。
- 前記ガイドカテーテルは、エネルギを除去する前に除去されることを特徴とする、請求項53に記載の方法。
- 前進させる前記工程は、卵円孔開存を通って延びるガイドワイヤの上を伝ってカテーテルデバイスを前進させる処置を含み、ガイドワイヤはその長尺に沿った拡張部を有して、卵円孔開存の内側で拡張させるようにしたことを特徴とする、請求項36に記載の方法。

- エネルギの量を監視する前記工程は、組織の温度を関しする処置を含んでいることを特徴とする、請求項58に記載の方法。
- エネルギの量を監視する前記工程は、組織のインピーダンスを監視する処置を含んでいることを特徴とする、請求項58に記載の方法。
- 組織から十分な量のエネルギが除去されて、実質的に卵円孔開存を急激に閉鎖したのがいつであるかを判定する工程を更に含んでいることを特徴とする、請求項58に記載の方法。
- 十分な量のエネルギが除去されてしまうと、エネルギの供与を中断する工程を更に含んでいることを特徴とする、請求項61に記載の方法。
- 前記カテーテルデバイスと連結された少なくとも一の視覚化装置を使用して、卵円孔開存と前記組織を直接的に視覚化する工程を更に含んでいることを特徴とする、請求項36に記載の方法。
- 心臓の卵円孔開存を治療する装置であって、該装置は、
近位端および遠位端を設けたカテーテルデバイスと、
遠位端に隣接した位置にあって、卵円孔開存に隣接している組織にエネルギを供与したり、該組織からエネルギを除去することで、実質的に卵円孔開存を急激に閉鎖するための少なくとも一のエネルギ伝達部材とを備えている、装置。 - 前記遠位端に隣接した位置にあって、組織を少なくとも部分的に一緒にする少なくとも一の組織並置部材を更に備えていることを特徴とする、請求項64に記載の装置。
- 前記少なくとも一のエネルギ伝達部材は前記少なくとも一の組織並置部材と連結されていることを特徴とする、請求項65に記載の装置。
- 前記少なくとも一のエネルギ伝達部材は前記少なくとも一の組織並置部材と相対的に移動させることができることを特徴とする、請求項66に記載の装置。
- 前記少なくとも一の組織並置部材は組織カバー部材を備えていることを特徴とする、請求項65に記載の装置。
- 前記組織カバー部材は卵円孔開存を覆って、心臓の左心房の血圧により組織を一緒にすることができるようにしたことを特徴とする、請求項68に記載の装置。
- 前記組織カバー部材は、真空力を組織に付与して組織を一緒にする少なくとも一の吸引部材を備えていることを特徴とする、請求項68に記載の装置。
- 前記組織カバー部材は第1の搬送寸法から第2の治療寸法まで拡張することができることを特徴とする、請求項68に記載の装置。
- 前記組織カバー部材と前記カテーテルデバイスの間に配置されて、使用中に組織カバー部材が望ましくない運動をするのを防止する可撓性の隔絶部を更に備えていることを特徴とする、請求項68に記載の装置。
- 前記少なくとも一のエネルギ伝達部材は前記組織カバー部材と連結されていることを特徴とする、請求項68に記載の装置。
- 卵円孔開存を通って前進するガイド部材を更に備え、前記カテーテルデバイスはガイド部材の上に滑動自在に配置されていることを特徴とする、請求項64に記載の装置。
- 前記ガイド部材は、
ガイドカテーテルと、
ガイドカテーテルの内側に配置される少なくとも一の拡張可能部材とを備えており、ガイドカテーテルは後退させられることで拡張可能部材を露出させ、拡張可能部材が卵円孔開存の内部で拡張することができるようにしたことを特徴とする、請求項74に記載の装置。 - 前記拡張可能部材は、互いに離れて拡張することで、卵円孔開存に隣接している組織に横方向の力を供与する少なくとも2個の部材を備えていることを特徴とする、請求項75に記載の装置。
- 前記露出された拡張部材は、心臓の左心房の中に延びることなく、横方向の力を供与することを特徴とする、請求項76に記載の装置。
- 前記拡張可能部材はバネ式部材を備えていることを特徴とする、請求項75に記載の装置。
- 前記拡張可能部材は形状記憶素材から構成されていることを特徴とする、請求項75に記載の装置。
- 前記ガイド部材は、その長尺の一部にそって分割されたガイドワイヤを備えており、分割部は拡張可能な形状記憶部材を有していることを特徴とする、請求項74に記載の装置。
- 前記ガイド部材は、卵円孔開存に隣接している組織の左心房表面に接触する少なくとも一の先端部を備えていることを特徴とする、請求項74に記載の装置。
- 前記少なくとも一の先端部は左心房表面に従っていることを特徴とする、請求項81に記載の装置。
- 前記ガイド部材は後退させられることで、前記少なくとも一の先端部を左心房表面と係合させることができることを特徴とする、請求項81に記載の装置。
- 前記ガイド部材は、前記少なくとも一のエネルギ伝達部材のうちの少なくとも1個を備えていることを特徴とする、請求項74に記載の装置。
- 前記ガイド部材は卵円孔開存の内側で拡張する拡張可能部材を備えており、拡張可能部材は少なくとも一の高周波エネルギ伝達部材を備えていることを特徴とする、請求項84に記載の装置。
- 前記少なくとも一のエネルギ伝達部材は、高周波エネルギ、極低温エネルギ、抵抗加熱エネルギ、超音波エネルギ、マイクロウエーブエネルギ、および、レーザエネルギのうちの少なくとも1種を伝達することを特徴とする、請求項64に記載の装置。
- 前記少なくとも一のエネルギ伝達部材は少なくとも一のモノポーラ高周波伝達部材を備えていることを特徴とする、請求項86に記載の装置。
- 前記少なくとも一のエネルギ伝達部材は少なくとも2個のバイポーラ高周波伝達部材を備えていることを特徴とする、請求項86に記載の装置。
- 前記少なくとも一のエネルギ伝達部材は少なくとも一の曲線形状の高周波伝達部材を備えていることを特徴とする、請求項86に記載の装置。
- 前記少なくとも一のエネルギ伝達部材は、メッシュ素材と編組素材のうち少なくとも一方から構成されていることを特長とする、請求項86に記載の装置。
- 前記少なくとも一のエネルギ伝達部材は、卵円孔開存を通って前進するガイド部材を備えていることを特徴とする、請求項64に記載の装置。
- 前記ガイド部材が、卵円孔開存内で膨張し少なくとも卵円孔開存に隣接する組織を接合する少なくとも1つの膨張可能部分を含む、請求項91に記載の装置。
- カテーテル装置に連結され前記少なくとも1つのエネルギ伝達部材から組織に伝達されたエネルギの量を検出する少なくとも1つのセンサを備えている、請求項64に記載の装置。
- 前記少なくとも1つのセンサが、赤外線検出装置、サーミスタ、および熱電対のからなる群から選択されてセンサである、請求項93に記載の装置。
- 前記少なくとも1つのセンサに接続され検出されたデータを処理し伝達されたエネルギの量がいつ所望量に達したかを決定するたマイクロプロセッサを備えている、請求項93に記載の装置。
