Claims (1)
- 【特許請求の範囲】 1. 手術用ファスナにおいて、 先端及び基端を持つ実質的に均等な直径の中実シャフトであって、前記シャ フトの前記先端には丸味が付けてあり、切断縁部及び貫入点が全くない中実シ ャフトと、 前記シャフトの前記基端に設けられた、前記シャフトから外方に延びるバー と、 前記先端の基端側で前記シャフトから外方に延びるフィンとを有し、このフ ィンは、前記シャフトから外方に且つ基端方向に傾いた先端縁部及び前記シャ フトから外方に且つ基端方向に傾いた基端縁部を有する、ことを特徴とする手 術用ファスナ。 2. 前記バーの側方への厚さは前記シャフトの直径以下である、請求項1に記載 のファスナ。 3. 前記フィンは、前記シャフトの軸線方向に延びる二つの平行な側部を含み、 前記フィンは前記バーと整合している、請求項2に記載のファスナ。 4. 前記フィンの前記側部間の前記フィンの厚さは、前記バーの前記側方への厚 さ以下である、請求項3に記載のファスナ。 5. 前記フィンは前記バーと整合している、請求項1に記載のファスナ。 6. 前記バー及び前記フィンが設けられた側部と反対側の前記シャフトの側部に は、前記シャフトの軸線方向で互いに整合した一対のニブが設けられている、 請求項5に記載のファスナ。 7. 前記バー及び前記フィンが設けられた側部と反対側の前記シャフトの側部か ら延びる折れ易いタブで前記シャフトに連結されたグリップ部分を更に有し、 前記タブを切断して前記グリップ部分を前記シャフトから分離するときに前記 タブの一部が前記シャフトに残り、前記シャフトの軸線方向で互いに整合した 一対のニブを前記シャフトの前記反対側に沿って形成する、請求項5に記載の ファスナ。 8. 前記バー及び前記フィンが設けられた側部と反対側の前記シャフトの側部に は、前記シャフトの軸線方向で整合した複数のニブが設けられている、請求項 4に記載のファスナ。 9. 前記バー及び前記フィンが設けられた側部と反対側の前記シャフトの側部か ら延びる折れ易いタブで前記シャフトに連結されたグリップ部分を更に有し、 前記タブを切断して前記グリップ部分を前記シャフトから分離するときに前記 タブの一部が前記シャフトに残り、前記シャフトの軸線方向で互いに整合した ニブを前記シャフトの前記反対側に沿って形成する、請求項4に記載のファス ナ。 10.前記先端の基端側に配眞された前記フィンの基端側に前記フィンと整合して 前記シャフト上に配置された第2フィンを更に有し、この第2フィンは、前記 先端の基端側の前記フィンの前記側部と整合した側壁を有する、請求項1に記 載のファスナ。 13.前記シャフトは、前記バーの第1部分及び第2部分と隣接しており且つこれ らの部分の中央に配置された第1基端シャフト部分と、前記フィンが延びる第 2先端シャフト部分と、前記第1及び第2のシャフト部分を相互連結し、前記 一方の側と反対側の前記第2シャフト部分の側部が前記バーの第2部分の端部 と同一平面内にあるように前記第1及び第2のシャフト部分をずらす第3シャ フト部分とを有する、請求項12に記載のファスナ。 14.前記バーは、先端方向に前記シャフトと全体に平行に延びる部分を含む、請 求項1に記載のファスナ。 15.前記ファスナは、先端及び基端を持つ実質的に均等な直径の第2中実シャフ トを更に有し、 前記バーは、前記第2シャフトの前記基端で前記第2シャフトまで延び且つ 前記第2シャフトに固定されており、 前記第2シャフトの前記先端には丸味が付けてあり、切断縁部及び貫入点が 全くなく、 また前記ファスナが、前記第2シャフトの前記先端の基端側で前記第2シャ フトから外方に延びる第2フィンを有し、この第2フィンは、前記第2シャフ トから外方に且つ基端方向に傾いた先端縁部及び前記第2シャフトから外方に 且つ基端方向に傾いた基端縁部を有し、 前記シャフトの前記直径は、互いにほぼ同じである、請求項1に記載のファ スナ。 16.前記フィンは互いにほぼ平行である、請求項15に記載のファスナ。 17.前記フィンは互いに向き合っている、請求項15に記載のファスナ。 18.前記シャフトのうちの第1シャフトの前記基端に設けられた前記バーの一部 が前記第1シャフトから延びる前記フィンと整合しており、前記第2シャフト の前記基端に設けられた前記バーの一部が前記第2フィンと整合している、請 求項15に記載のファスナ。 19.手術用ファスナにおいて、 先端及び基端を持つ実質的に均等な直径の中実シャフトであって、前記シャ フトの前記先端には丸味が付けてあり、切断縁部及び貫入点が全くない中実シ ャフトと、 前記シャフトから半径方向外方に前記シャフトの周りに等しく延びる、前記 シャフトの前記基端に設けられたボールと、 前記先端の基端側で前記シャフトから外方に延びるフィンとを有し、このフ ィンは、前記シャフトから外方に且つ基端方向に傾いており先端縁部と、前記 シャフトから外方に且つ基端方向に傾いており且つ前記フィンの先端縁部と交 差する基端縁部とを有する、ことを特徴とする手術用ファスナ。 20.前記フィンは、前記シャフトの軸線方向に延びる平らな平行な側部を備え、 前記フィンの厚さは、前記シャフトの直径以下である、請求項19に記載のフ ァスナ。 21.設置工具のキャリヤ部分と係合するようになった手術用ファスナ用ディスペ ンサにおいて、該ディスペンサが前記ファスナを保持するための収納室を有し 、 この収納室には、前記ファスナが通過できる開口部が設けられており、又前記 ディスペンサが前記収納室の開口部からディスペンサの側壁の開口部まで延び 、前記キャリヤ部分を持つ前記設置工具の先端を前記収納室まで案内するよう に形成された壁構造を有し、前記設置工具の残りの部分は前記ディスペンサの 外側に残り、これによって、前記設置工具の前記キャリヤ部分を前記ファスナ と係合でき、前記設置工具の先端を前記ディスペンサの開口部から引き出すこ とによって前記ファスナを前記ディスペンサから引き出すことができる、こと を特徴とするディスペンサ。 22.シャフトと、前記シャフトの基端に設けられた、前記シャフトから外方に延 びるバーと、前記シャフトの先端の基端側で前記シャフトから外方に延びるフ ィンとを有し、前記フィン及び前記バーは前記シャフトの側部に沿って互いに 整合しており、前記シャフトの反対側に接合されたタブによって前記シャフト に連結されたグリップ部分を更に有するファスナ用の、ディスペンサにおいて 、 前記ファスナを保持するためのチャンバを内部に備え、前記ファスナを受け 入れて保持するようになったキャリヤ部分を備えた設置工具を受け入れるため の入口行路を有し、前記キャリヤ部分には切断手段が設けられている、ハウジ ングを有し、 前記入口行路及び前記チャンバは、前記設置工具のキャリヤ部分の切断手段 を案内し、前記ファスナの脚部と切断係合させ、前記グリップ部分を前記シャ フトから切断し、前記キャリヤ部分を前記ファスナの周囲で摺動させ、前記フ ァスナと容易に係合させるように形成されており、 これによって、前記設置工具を前記ハウジングから引き出すとき、前記グリ ップ部分のない前記ファスナが前記ディスペンサから取り出され、前記設置工 具のキャリヤ部分に配置される、ことを特徴とするディスペンサ。 23.タブによって一体的に相互連結されたシャフト部分及びグリップ部分を含む ファスナ用のディスペンサにおいて、 前記ファスナを保持するためのチャンバを内部に備えたハウジングを有し、 前記チャンバは、前記ファスナのシャフト部分を保持するための第1チャンバ 部分及び前記グリップ部分を保持するための第2チャンバ部分を含み、前記ハ ウジングには、設置工具を前記第1チャンバ部分内に案内して前記ファスナの シャフト部分を取り囲むため、及び前記設置工具の切断縁部を案内して前記タ ブと順次係合させ、前記タブを切断して前記ファスナのシャフト部分を前記グ リップファスナ部分から分離し、前記グリッブ部分を前記ディスペンサ内に残 して前記シャフト部分を前記ディスペンサから引き出すことができるようにす るための入口行路が設けられている、ことを特徴とするディスペンサ。 24.シャフト、このシャフトの基端に設けられた、前記シャフトから外方に延び 前記設置工具は、前記キャリヤ部分及びこのキャリヤ部分内の前記ファスナ を組織体内に延ばし、前記キャリヤ部分を前記組織から引き出しすように操作 でき、この引き出し時に前記フィンが前記ファスナの引き出しに抵抗し、前記 ファスナは、前記設置工具の引き出し時に前記組織内に止まる、ことを特徴と する設置工具。 26.前記キャリヤ部分はチャンバを有し、このチャンバは、少なくとも一部が前 前記シャフトの先端の基端側で前記シャフトから半径方向外方に延びるフィ ンとを有し、前記バー及び前記フィンは、前記シャフトの側部に沿って互いに 整合しており、 前記ディスペンサは、 前記ファスナを保持するためのチャンバが内部に配置されており、前記ファ スナを受け入れて保持するようになったキャリヤ部分を持つ設置工具を受け入 れるための入口行路を有するハウジングを有し、 前記入口行路及びチャンバは、前記キャリヤ部分を前記ファスナに滑り込ま せて前記ファスナと係合させることを容易にするように形成されており、 前記設置工具を前記ハウジングから引き出すとき、前記ファスナが前記ディ スペンサから取り出され、前記設置工具のキャリヤ部分に配置され、 前記設置工具は、 細長い剛性チューブを有し、 前記キャリヤ部分は、前記チューブ内の第1位置と前記チューブの先端から 前記シャフトの前記基端に設けられた、前記シャフトから外方に延びるバー と、 前記先端の基端側で前記シャフトから外方に延びるフィンとを有し、このフ ィンは、前記シャフトから外方に且つ基端方向に傾いた先端縁部及び前記シャ フトから外方に且つ基端方向に傾いた基端縁部を有する、ことを特徴とする手 術用ファスナを提供する工程と、 前記ファスナを収容するための設置工具を提供する工程であって、この設置 工具は、開放した先端及び開放した側部を有し、この開放した側部を通って前 記ファスナの前記バー及び前記フィンが延びており、前記ファスナの前記先端 は、前記設置工具の前記切断縁部から引っ込んでいる、設置工具を提供する工 程と、 前記ファスナを内部に収容した状態で、前記設置工具を、前記第1組織片を 通して前記第2組織片内に、前記ファスナバーが前記第1組織片の表面と係合 するまで駆動する工程と、 前記設置工具を前記組織から引き出す工程とを有し、この引き出し時に、前 記第2組織片と係合した前記フィンが前記ファスナの基端方向への移動に抵抗 することにより、前記設置工具は、前記ファスナが前記組織に食い込んだまま で、前記ファスナから引き出される、ことを特徴とする方法。 30.一片の組織を別の組織片に取り付けるための方法において、 第1組織片と係合するまで移動する工程と、 前記設置工具の制御手段を操作し、前記キャリヤ部分を基端方向に前記チュ ーブ内に移動し、このとき、前記キャリヤ部分が前記ファスナから引き出され るとき、前記フィンが前記ファスナからの引き出しに抵抗する、工程とを有し 、 これによって、前記キャリヤ部分を前記ファスナから引き出し、前記ファス ナを前記組織に残す、ことを特徴とする方法。 スナを受け入れて保持するようになったキャリヤ部分を持つ設置工具を受け入 れるための入口行路を有し、前記キャリヤ部分には切断手段が設けられている 、ハウジングを有する、前記ファスナ用のディスペンサであって、 前記入口行路及び前記チャンバは、前記設置工具のキャリヤ部分を案内して 前記ファスナと係合するように形成されており、 これによって、前記設置工具を前記ハウジングから引き出すとき、前記ファ スナが前記ディスペンサから取り出され、前記設置工具のキャリヤ部分と係合 する、ディスペンサを提供する工程と、 前記チューブ内の第1位置と前記チューブの先端から延びる第2位置との間 で移動自在のキャリヤ部分を先端に有し、このキャリヤ部分は開放した側部を 有し、この開放した側部から前記バー及び前記フィンの端部分が延びている、 細長い剛性チューブと、 前記キャリヤ部分及びこのキャリヤ部分内の前記ファスナを組織体内に移動 し、前記キャリヤ部分を前記組織から引き出すための制御手段とを有する設置 工具を提供する工程と、 前記設置工具の前記キャリヤ部分を前記ディスペンサのハウジングのチャン バに挿入し、前記ファスナが前記設置工具のキャリヤ部分と係合した状態で前 記設置工具を前記ディスペンサから引き出す工程と、 前記チューブの前記先端を前記組織と隣接して前記組織の開口部の基端側に 配置する工程と、 前記設置工具の制御手段を操作し、前記キャリヤ部分を前記チューブの外に 先端方向に移動し、前記ファスナのバーが前記組織と係合するまで前記組織内 に移動する工程と、 前記設置工具の制御手段を操作し、前記キャリヤ部分を前記チューブ内に基 端方向に移動することによって前記キャリヤ部分を引き出し、この際、前記フ ィンが前記ファスナの引き出しに抵抗し、前記ファスナは前記設置工具を前記 組織から引き出すときに前記組織内に止まり、前記ファスナを前記組織内に残 す、ことを特徴とする方法。 33.第1対象物を第2対象物に取り付けるための方法において、 先端及び基端を持つ実質的に均等な直径の中実シャフトであって、前記シャ フトの前記先端には丸味が付けてあり、切断縁部及び貫入点が全くない中実シ ャフトと、 前記シャフトの前記基端に設けられた、前記シャフトから外方に延びるバー と、 前記先端の基端側で前記シャフトから外方に延びるフィンとを有し、このフ ィンは、前記シャフトから外方に且つ基端方向に傾いた先端縁部及び前記シャ フトから外方に且つ基端方向に傾いた基端縁部を有する、ことを特徴とする手 術用ファスナを提供する工程と、 前記ファスナを収容するための設置工具であって、この設置工具は開放した 先端及び開放した側部を有し、この開放した側部を通って前記ファスナの前記 バー及び前記フィンが延びており、前記ファスナの前記先端は、前記設置工具 の前記切断縁部から引っ込んでいる、設置工具を提供する工程と、 前記ファスナを内部に収容した状態で、前記設置工具を、前記第1対象物を 通して前記第2対象物内に、前記ファスナバーが前記第1対象物の表面と係合 するまで駆動する工程と、 前記設置工具を前記第2対象物から引き出し、次いで前記第1対象物から引 き出す工程とを有し、この引き出し時に、前記第2対象物と係合した前記フィ ンが前記ファスナの基端方向への移動に抵抗することにより、前記設置工具は 、前記ファスナが前記第2対象物及び前記第1対象物に食い込んだままで、前 記ファスナから引き出される、ことを特徴とする方法。 34.前記第1対象物は、メッシュ、フィラメント、及び組織片からなる群から選 択された一つの対象物からなる、請求項33に記載の方法。 35.前記第1対象物及び前記第2対象物は、両方とも、生体組織からなる、請求 項33に記載の方法。 36.前記第1対象物及び前記第2対象物は、両方とも、半月軟骨からなる、請求 項35に記載の方法。
