Abstract
Implant expansible (1, 101) permettant de participer à une restauration d'une anatomie osseuse humaine ou animale, caractérisé en ce qu'il comprend :- un plan d'expansion unique (2, 102) déterminé, intrinsèque au dit implant,- des moyens de positionnement (3, 103) de l'implant expansible dans ledit os (60) pour faire correspondre ledit plan d'expansion intrinsèque au dit implant avec un plan de restauration osseuse,- des moyens de déploiement (4, 104) de l'implant expansible dans ledit plan d'expansion intrinsèque au dit implant, en vue de former une cavité (72) à l'intérieur dudit os.
Description
Implant expansible prothétique osseux
La présente invention se rapporte au domaine de la chirurgie, et des implants prothétiques, plus particulièrement aux implants prothétiques osseux permettant avantageusement de mettre en oeuvre des procédés de restauration d'une anatomie osseuse humaine ou animale.
Diverses causes peuvent être -'à l'origine d'une compression osseuse, notamment l'ostéoporose qui engendre par exemple un tassement vertébral naturel sous le poids de l'individu, mais également les traumatismes, parfois les deux causes étant combinées. Ces compressions osseuses peuvent affecter essentiellement les vertèbres, mais concernent également d'autres os, comme le radius ou le fémur par exemple.
Plusieurs techniques de vertébroplastie sont connues pour effectuer un redressement vertébral, c'est à dire pour faire reprendre à une vertèbre sa forme ou anatomie initiale, ou une forme voisine de celle-ci, la plus proche possible.
On connaît par exemple une technique consistant à introduire dans une vertèbre un ballonnet gonflable, puis à envoyer un fluide sous pression dans le ballonnet placé dans la vertèbre afin de forcer l'enveloppe corticale de la vertèbre, et notamment les plateaux vertébraux inférieur et supérieur à reprendre une forme redressée sous l'effet de la pression. Cette technique est connue sous le nom de kyphoplastie. Une fois l'enveloppe corticale osseuse redressée, le ballonnet est ensuite dégonflé, puis retiré de la vertèbre, afin de pouvoir injecter dans cette dernière un ciment destiné à conférer à la vertèbre redressée une résistance mécanique suffisante pour que le redressement ait une durée significative dans le temps. Un inconvénient notable d'une telle méthode réside dans ses nombreuses manipulations, notamment gonflage, et dans la nécessité de retirer le ballon hors du corps du patient. En outre, l'expansion d'un ballon est relativement mal contrôlée car elle est volumique, c'est à dire multi-directionnelle; or, il peut être nettement préjudiciable à l'intervention et donc au patient, d'exercer une pression importante dans des directions inappropriées, pouvant entraîner un risque d'éclatement de la corticale, notamment la partie latérale de celle-ci reliant les plateaux inférieur et supérieur d'une vertèbre.
Il existe en outre des implants vertébraux visant à remplir une cavité dans une vertèbre, mais ces derniers adoptent un principe d'expansion radiale obtenue par la formation d'une pluralité de pointes se dressant normalement à l'axe longitudinal de l'implant sous l'effet d'une contraction de ce dernier. De tels implants imposent une pression trop élevée sur l'extrémité des pointes, susceptible de transpercer la matière sur laquelle les pointes s'appuient. En outre, cette pression très élevée peut provoquer un éclatement des tissus ou des parois d'organes, comme une corticale osseuse par exemple. De ce fait, ces implants ont pour fonction de remplir une cavité déjà formée car ils ne peuvent former de cavité, et sont utilisés entourés d'une maille, d'un tissu ou d'un ballon, ce qui rend leur usage d'autant plus compliqué. En outre, l'expansion radiale de tels implants ne permet pas de privilégier une direction particulière d' expansion.
La présente invention permet de pallier ces inconvénients et d'apporter d'autres avantages. Plus précisément, elle consiste en un implant expansible permettant de participer à une restauration d'une anatomie osseuse humaine ou animale, caractérisé en ce qu'il comprend: - un plan d'expansion unique déterminé, intrinsèque au dit implant, - des moyens de positionnement de l'implant expansible dans ledit os pour faire correspondre ledit plan d'expansion intrinsèque au dit implant avec un plan de restauration osseuse, - des moyens de déploiement de l'implant expansible dans ledit plan d'expansion intrinsèque au dit implant, en vue de former une cavité à l'intérieur dudit os.
L'implant selon l'invention permet de créer une cavité infra-osseuse grâce à son expansion, en utilisant une direction d'expansion privilégiée. Le matériau de remplissage destiné à remplir la cavité pourra être injecté avec une pression relativement faible grâce à l'implant qui reste en place, et qui a permis la création d'une cavité nette et précise, et la conservation des dimensions de la cavité ainsi créée.
Selon une caractéristique avantageuse, l'implant selon l'invention comprend des moyens de réglage d'une valeur déterminée d'expansion, comprise entre une épaisseur minimale de l'implant avant toute expansion de celui-ci et une épaisseur maximale de l'implant après son expansion maximale.
Cette caractéristique permet de contrôler la valeur d'expansion de l'implant, par 20 exemple en vue d'un redressement donné de vertèbre.
Selon une caractéristique avantageuse, l'implant selon l'invention comprend un premier et un deuxième plateaux opposés, aptes à former respectivement une première et une deuxième surfaces d'appui dans ledit os destinées à s'éloigner l'une de l'autre suivant ledit plan d'expansion unique lors de l'expansion de l'implant.
Cette caractéristique permet de réduire la pression exercée par l'implant sur les tissus en contact avec celui-ci, en augmentant la surface de contact ou d'appui sur lesdits tissus, et d'écarter plus aisément ces derniers en vue de former la cavité sans les transpercer.
Selon une caractéristique avantageuse, lesdits premier et deuxième plateaux opposés forment des première et deuxième surfaces d'appui aptes à se déplacer suivant une perpendiculaire à un axe longitudinal dudit implant expansible lors de l'expansion de ce dernier.
Selon une caractéristique avantageuse, lesdites première et deuxième surfaces d'appui dans ledit os possèdent respectivement une longueur sensiblement égale à la 35 longueur dudit implant.
Cette caractéristique permet d'optimiser le rapport de la longueur d'appui sur les tissus sur la longueur de l'implant. Plus ce rapport est proche de un, et plus l'implant sera utilisable dans des endroits de faible longueur en vue de créer une cavité la plus précise possible. Un intérêt de l'implant selon l'invention étant de permettre la création d'une cavité la plus grande possible pour une longueur d'implant donnée, afin de permettre un redressement osseux dans des parties exiguës d'un os, et afin de permettre l'introduction d'un matériau de remplissage selon une pression d'injection la plus faible possible, ceci en vue d'éviter l'infiltration du matériau de remplissage dans des tissus non appropriés, comme des parois de vaisseaux sanguins par exemple.
Selon une caractéristique avantageuse, lesdits premier et deuxième plateaux 05 forment des première et deuxième surfaces d'appui continues sur leur longueur, respectivement.
Cette caractéristique, du fait de la continuité des surfaces d'appui, permet de contribuer à la réduction de la pression sur les tissus repoussés, et à la création d'une cavité dont les contours sont continus apportant ainsi sa contribution dans la réduction de la pression d'injection du matériau de remplissage.
Selon une caractéristique avantageuse, lesdits premier et deuxième plateaux forment respectivement des surfaces d'appui continues lisses.
Selon une caractéristique avantageuse, lesdits premier et deuxième plateaux forment respectivement des surfaces d'appui partiellement cylindriques dont une 15 génératrice est parallèle à un axe longitudinal dudit implant expansible.
Selon une caractéristique avantageuse, l'implant selon l'invention comprend un premier et un deuxième appuis pour chacun desdits premier et deuxième plateaux, situés sous ces derniers respectivement.
Cette caractéristique permet d'augmenter le rapport correspondant à la longueur des surfaces d'appui sur la longueur de l'implant et de le faire tendre vers un, comme cela sera expliqué plus en détail plus loin avec la description d'un exemple de mode de réalisation de l'invention. En outre, cette caractéristique permet de répartir sous le plateau les efforts de poussée afin de diminuer le porte-à-faux.
Selon une caractéristique avantageuse, lesdits premier et deuxième plateaux comportent respectivement une première et une deuxième ailes en porte-à-faux dont les zones d'attache respectives sont situées au niveau desdits premier et deuxième appuis.
Selon une caractéristique avantageuse, lesdites première et deuxième ailes en porte-à-faux possèdent respectivement une longueur correspondant sensiblement à la valeur de déplacement maximale d'un desdits premier ou deuxième plateau dans ledit plan d'expansion unique.
Selon une caractéristique avantageuse, lesdits moyens de positionnement de l'implant expansible dans ledit os pour faire correspondre ledit plan d'expansion intrinsèque au dit implant avec un plan de restauration osseuse, comprennent des moyens de prise permettant d'orienter ledit implant angulairement autour d'un axe longitudinal.
D'autres caractéristiques et avantages apparaîtront à la lecture qui suit de deux exemples de mode de réalisation d'un implant expansible selon l'invention et de deux exemples d'utilisation d'un tel implant, accompagnée des dessins annexés, exemples donnés à titre illustratif non limitatif.
La figure 1 représente une vue en perspective d'un premier exemple de mode de 40 réalisation d'un implant expansible selon l'invention, en position de repos, c'est à dire non déployé.

L'implant expansible 1 représenté sur les figures 1 à 7 permettant de participer à 25 une restauration d'une anatomie osseuse humaine ou animale, comprend: - un plan d'expansion unique 2 déterminé, intrinsèque à l'implant, - des moyens de positionnement 3 de l'implant expansible dans l'os permettant de faire correspondre le plan d'expansion intrinsèque à l'implant avec un plan de restauration osseuse, - des moyens de déploiement 4 de l'implant expansible dans le plan d'expansion unique 2, en vue de former une cavité à l'intérieur de l'os, et avantageusement: des moyens de réglage 5 d'une valeur déterminée d'expansion, comprise entre une épaisseur minimale A de l'implant avant toute expansion de celui-ci et une épaisseur maximale B de l'implant après son expansion maximale, - un premier 6 et un deuxième 7 plateaux opposés, aptes à former respectivement une première 8 et une deuxième 9 surfaces d'appui dans l'os destinées à s'éloigner l'une de l'autre suivant le plan d'expansion 2 unique lors de l'expansion de l'implant 1.
L'implant 1 s'inscrit de préférence dans une forme cylindrique à section extérieure transversale circulaire, et peut avantageusement être fabriqué à partir d'un corps tubulaire 24, en matériau bio-compatible, par exemple en titane.
L'implant 1 comprend une première 20 et une deuxième 21 extrémités, adoptant respectivement la forme d'une section transversale du corps tubulaire 24, destinées à être rapprochées l'une de l'autre en vue de permettre un déploiement de l'implant, comme représenté sur la figure 2. A cet effet, les deux extrémités 20, 21 sont reliées entre elles par un premier 22 et un deuxième 23 bras rectilignes parallèles lorsque l'implant est non déployé, formés longitudinalement dans le corps tubulaire 24 et aptes à se plier sous l'effet d'un rapprochement des extrémités 20 et 21 l'une vers l'autre, tout en écartant de l'axe longitudinal 10 du corps tubulaire 24 les premier 6 et deuxième 7 plateaux opposés.
Comme représenté sur les figures 4 et 5, afin de permettre leurs déploiements dans un plan unique 2, les bras 22 et 23 sont diamétralement opposés et déterminent ainsi un plan d'expansion unique 2 passant par l'axe longitudinal 10 du corps tubulaire 24.
Les bras 22, 23 sont formés à partir d'un évidement 40 transversal du corps tubulaire, traversant le corps tubulaire 24 de part en part, et s'étendant sur la longueur du corps tubulaire comprise entre les deux extrémités 20 et 21 de l'implant 1. Comme représenté sur la figure 5, les bras 22, 23 reliant les deux extrémités 20 et 21 adoptent respectivement une section transversale comprise entre un arc de cercle 26 de la surface extérieure du corps tubulaire 24 et la corde 27 qui définit cet arc de cercle 26 et qui est comprise dans la paroi 25 formant l'évidement 40. L'évidement 40 est par exemple symétrique par rapport à l'axe longitudinal 10.
Chaque bras 22, 23 est divisé en trois parties successives rigides articulées entre elles et sur les extrémités 20 et 21 de l'implant 1, comme suit: pour le bras 22 supérieur: une première partie rigide 28 est liée par une extrémité de celle-ci à l'extrémité 20 de l'implant 1 au moyen d'une articulation 29 et est liée par son autre extrémité à la première extrémité de la deuxième partie rigide 30 voisine au moyen d'une articulation 31; la deuxième partie rigide 30 étant liée par son autre extrémité à la troisième partie rigide 32 au moyen d'une articulation 33, l'autre extrémité de la troisième partie rigide étant liée à l'extrémité 21 de l'implant 1 au moyen d'une articulation 34. Les articulations 29, 31, 33, et 34 sont des articulations à un degré de liberté en rotation, agissant respectivement autour d'axes perpendiculaires au plan d'expansion 2 de l'implant 1. Les articulations 29, 31, 33, sont avantageusement formées par un amincissement de la paroi formant le bras dans la zone de l'articulation concernée, comme représenté sur les figures 1, 2, ou 3.
Chaque bras 22, 23, lorsque les extrémités 20 et 21 de l'implant sont rapprochées l'une de l'autre se déploie de sorte que la partie rigide centrale 30 s'éloigne de l'axe 10 longitudinal de l'implant poussée par les deux parties rigides voisines 28 et 32. Comme plus particulièrement représenté sur la figure 3, afin d'initier le mouvement du bras dans la bonne direction lors du rapprochement des extrémités 20 et 21 l'une de l'autre, il y a lieu de créer un couple de rotation approprié des diverses parties du bras. A cet effet, les parties rigides d'extrémités 28, 32 du bras supérieur 22 sont articulées sur les extrémités 20 et 21 respectivement de l'implant 1 en partie basse du voile de matière constituant ces parties rigides, et sont articulées sur la partie rigide centrale 30 dans une partie supérieure dudit voile de matière constitutif des parties rigides 28, 32, le décalage des articulations, créant, dès lors qu'un effort de rapprochement des extrémités 20 et 21 est appliqué suivant l'axe longitudinal 10 de l'implant, un couple de rotation sur les parties rigides d'extrémités 28 et 32, tendant à faire pivoter ces dernières vers l'extérieur de l'implant en sorte d'éloigner la partie rigide centrale 30 de l'axe longitudinal 10 de l'implant 1.
Le bras inférieur 23 est construit de la même manière que le bras supérieur et est avantageusement symétrique du bras supérieur 22 par rapport à un plan perpendiculaire au plan d'expansion 2 passant par l'axe longitudinal 10.
Les articulations des bras supérieur 22, et inférieur 23 sont donc constituées par des zones de fragilisation réalisées par des rainures 81, respectivement, définissant un voile mince de matière constitutive du corps tubulaire 24, dont l'épaisseur est déterminée par la profondeur des rainures 81, comme représenté sur les figures, de manière à permettre une déformation plastique du matériau sans qu'il se rompe, avantageusement jusqu'à ce que les parties rigides d'extrémités 28 et 32 du bras supérieur 22, et leurs symétriques sur le bras inférieur 23 puissent adopter une position, dite d'expansion extrême, dans laquelle les parties rigides visées sont perpendiculaires à l'axe longitudinal 10 de l'implant 1, lorsque les extrémités 20 et 21 de l'implant 1 sont rapprochées l'une de l'autre de sorte que ce dernier soit déployé jusqu'à sa capacité d'expansion maximale. La largeur des rainures 81 sera déterminée de manière à permettre avantageusement un tel débattement des parties des bras supérieur et inférieur et également de manière à conférer un rayon de courbure approprié aux voiles en vue d'assurer une déformation plastique sans rupture de la matière.
Les premier 6 et deuxième 7 plateaux opposés sont respectivement avantageusement formés dans les bras supérieur 22 et inférieur 23, comme suit: pour le bras supérieur 22 par exemple, le plateau 6 rigide est formé d'une part par ladite partie rigide centrale 30 et d'autre part par une extension de matière s'étendant de part et d'autre de la partie centrale 30 dans les parties d'extrémités 28 et 32. Pour réaliser un plateau 6 rigide, les parties de ce dernier s'étendant dans les parties d'extrémités 28 et 32 sont dissociées de celles-ci par la réalisation de deux fentes 35 et 36 transversales situées dans l'épaisseur de chaque partie d'extrémité 28 et 32 et s'étendant longitudinalement sur la longueur de celles-ci, comme représenté sur les figures 3 et 4. Les articulations 31 et 33 et les parties d'extrémités 28 et 32 forment respectivement un premier 12 et un deuxième 13 appuis pour le premier 6 plateau, situés sous ce dernier. Il en est de même pour le deuxième plateau 7 par symétrie.
Ainsi, les premier 6 et deuxième 7 plateaux comportent respectivement une première 16, 18 et une deuxième 17, 19 ailes en porte-à-faux dont les zones d'attache respectives sont situées au niveau des premier 12, 14 et deuxième 13, 15 appuis. Comme représenté sur les figures 1, 2, ou 3, les premières 16, 18 et deuxièmes 17, 19 ailes en porte-à-faux possèdent respectivement une longueur correspondant sensiblement à la valeur de déplacement maximale d'un des premier 6 ou deuxième 7 plateau dans le plan d'expansion unique 2.
Les premier 6 et deuxième 7 plateaux opposés forment avantageusement des première 8 et deuxième 9 surfaces d'appui, possédant respectivement une longueur sensiblement égale à la longueur de l'implant, et aptes à se déplacer perpendiculairement à l'axe longitudinal 10 de l'implant expansible 1 lors de l'expansion de ce dernier. Il est à noter que les premier 6 et deuxième 7 plateaux forment avantageusement des première 8 et deuxième 9 surfaces d'appui continues et lisses sur leur longueur, respectivement. Du fait de la réalisation préférentielle de l'implant 1 dans un corps tubulaire 24, les premier 6 et deuxième 7 plateaux forment respectivement avantageusement des surfaces d'appui partiellement cylindriques dont une génératrice 11 est parallèle à l'axe longitudinal 10 de l'implant 1 expansible.
L'implant 1 représenté sur les figures 1 à 7 peut donc être avantageusement 10 obtenu à partir de l'usinage d'un tronçon de tube, par exemple selon les formes et géométries décrites ci-dessus.
Les moyens de positionnement 3 de l'implant expansible dans l'os permettant de faire correspondre le plan d'expansion 2 intrinsèque à l'implant avec un plan de restauration osseuse, comprennent des moyens de prise permettant d'orienter l'implant angulairement autour de l'axe longitudinal 10, par exemple deux méplats 37, 38 formés sur la surface cylindrique à section circulaire de l'extrémité 20 par exemple, en vue de permettre une prise en rotation de l'implant 1.
Les moyens de déploiement 4 de l'implant expansible dans le plan d'expansion unique 2, en vue de former une cavité à l'intérieur de l'os, comprennent les parties d'extrémités 28 et 32 du bras supérieur 22 ainsi que les parties d'extrémités symétriques sur le bras inférieur 23, permettant de déployer les plateaux supérieur 6 et inférieur 7, ainsi qu'un porte implant 71 permettant de rapprocher les extrémités 20 et 21 de l'implant lorsqu'il est implanté, et comme cela sera décrit plus loin avec la description d'un exemple de procédé de restauration osseuse décrivant un exemple de mode d'utilisation de l'implant. Le porte implant 71 doit permettre, en prenant appui sur l'extrémité 20 par exemple, de rapprocher l'extrémité 21 en tirant sur celle-ci, ou en prenant appui sur l'extrémité 21 par exemple, de rapprocher l'extrémité 20 en poussant sur celle-ci. A cet effet, l'extrémité 21 distale par exemple comporte avantageusement un trou taraudé 39 suivant l'axe longitudinal 10 pour permettre la mise en prise du porte implant 71.
L'extrémité 20 proximale comporte quant à elle un alésage 80 suivant l'axe longitudinal 10 afin de permettre le passage d'une âme du porte implant 71 comme cela sera expliqué plus loin.
Les moyens de réglage 5 sont conférés par le porte implant qui comporte avantageusement des moyens de contrôle millimétrique du rapprochement des extrémités 20 et 21 l'une vers l'autre de l'implant 1, de préférence au moyen d'un procédé par vissage (non représenté), permettant de stopper l'expansion à chaque instant en fonction des besoins. D'autre part, les moyens de réglage sont également conférés par les articulations des bras 22 et 23, plus spécifiquement par l'épaisseur des voiles de matière définissant celles-ci, qui, se déformant dans le domaine plastique, permettent à l'expansion de conserver sensiblement une position déterminée de déploiement des bras, au retrait élastique près qui est très faible en pratique.
On remarquera que l'expansion des plateaux 6 et 7 de l'implant, et leur stabilisation une fois déployés, peut se faire en s'adaptant à la géométrie osseuse rencontrée car l'implant 1 permet un déplacement non parallèle des plateaux 6 et 7 et, en fin de déplacement, permet une position définitive des plateaux dans un état non parallèle si nécessaire en fonction de l'anatomie osseuse.
Sur les figures 8 à 16 relatives à la description d'un deuxième exemple de mode de réalisation d'un implant expansible 101 selon l'invention, les éléments fonctionnellement identiques à ceux des figures 1 à 7 relatives au premier exemple d'implant 1 comportent respectivement la même référence numérique additionnée du nombre 100 et ne seront donc pas plus décrits. On se reportera à la description qui précède pour ces éléments.
L'implant 101 représenté se différencie de l'implant 1 par l'absence d'aile sur les plateaux 106 et 107, comme représenté plus particulièrement sur la figure 9, et par la présence d'un système à parallélogramme 141 sur une des parties d'extrémités 128 ou 132 de chacun des bras 122 et 123, dans l'exemple représenté sur la partie d'extrémité 128 du bras supérieur 122 lié à l'extrémité 120 de l'implant 101 et de son symétrique sur le bras inférieur 123. Les systèmes à parallélogramme ont pour fonction de garantir un déplacement des plateaux respectivement de chacun des bras 122 et 123, parallèle à l'axe longitudinal 110 de l'implant. A cet effet, comme représenté sur la figure 9, la partie d'extrémité 128 du bras 122 et de son symétrique 123 est dédoublée, de même que ses articulations 131 et 129 respectivement sur la partie centrale 130 et sur l'extrémité 120 de l'implant, en sorte de former un parallélogramme déformable lors du déplacement du plateau correspondant.
Les articulations du parallélogramme 141 déformable sont réalisées de la même manière que les autres articulations 131, 133, 134 du bras 122, comme représenté sur les figures 8 à 16, et selon une géométrie appropriée comme expliqué plus haut, plus particulièrement comme représenté sur les figures 11 à 14, en vue de créer les couples d'effort sur les différentes parties 129, 130, 132 du bras permettant les déplacements souhaités lors du rapprochement des extrémités 120 et 121 de l'implant 101.
Afin d'obtenir un parallélogramme 141 déformable, la partie d'extrémité 128 du bras est divisée en trois leviers longitudinaux, à savoir deux leviers 142 latéraux et un levier central 143 formant deux côtés du parallélogramme déformable 141, les deux autres côtés du parallélogramme étant constitués respectivement par un prolongement 144 de la partie centrale du bras 122, placé dans l'axe de prolongement du levier central 143, et par un prolongement double 145 de l'extrémité 120 de l'implant 101, s'étendant parallèlement à l'axe longitudinal 110 de l'implant et placé dans l'axe de prolongement des deux leviers 142 latéraux, comme représenté sur la figure 8 par exemple.
Il est à noter que les bras 122 et 123 sont symétriques par rapport à un plan perpendiculaire au plan d'expansion 102 passant par l'axe longitudinal 110 de l'implant 101, afin d'obtenir, lors de l'expansion de l'implant, le déplacement des deux plateaux 106 et 107 de manière parallèle à l'axe longitudinal 110 de l'implant 101.
L'implant 101 pourra avantageusement être doté de deux plateaux rapportés (non représentés), par exemple adoptant la forme, notamment la longueur, des plateaux 6 et 7 de l'implant 1 décrit plus haut comportant des ailes s'étendant au delà de la partie centrale des bras 22 ou 23, en vue d'allonger la longueur de la partie centrale des bras 122 ou 123 sans allonger pour autant la longueur de l'implant. Ainsi, l'implant 101 adopterait une configuration similaire à celle de l'implant 1 et ne se distinguerait plus de lui que par sa capacité supplémentaire à contraindre les plateaux à se déployer parallèlement à l'axe longitudinal de l'implant, ce qui trouve des applications dans des réductions de fractures spécifiques pour lesquelles les fragments d'os doivent être écartés de manière parallèle.
Les plateaux rapportés peuvent être associés à leur partie centrale de bras respective par soudure, par exemple.
Un premier exemple de procédé de restauration d'une anatomie osseuse humaine, qui utilise un implant expansible selon l'invention, notamment tel que décrit plus haut, va maintenant être décrit avec l'aide des figures 17 à 29. Il s'agit plus précisément d'un procédé de restauration osseuse d'une vertèbre par une voie postérolatérale, avec réduction de fracture.
Le procédé comprend les étapes suivantes: - introduire, dans une vertèbre 60 dont la forme doit être restaurée, un implant 1 expansible selon un plan déterminé 2 unique intrinsèque à l'implant: pour réaliser cette 20 opération, il y a lieu de: * mettre en place de manière percutanée unebroche 61, par exemple de type broche de Kirschner, comportant une extrémité 62 filetée, par une voie postérolatérale, de manière à ce que l'extrémité filetée 62 vienne se visser dans la corticale 63 opposée à la corticale 64 traversée par la broche, comme représenté sur la figure 17; *introduire un premier tube dilatateur 65 autour de la broche 61, jusqu'à ce que l'extrémité du premier tube 65 vienne prendre appui sur la surface extérieure de la corticale 64, comme représenté sur la figure 18; *introduire un deuxième tube dilatateur 66 autour du premier tube dilatateur 65, jusqu'à ce que l'extrémité du deuxième tube 66 vienne prendre appui sur la surface extérieure de la corticale 64, comme représenté sur la figure 19; *introduire un troisième tube dilatateur 67 autour du deuxième tube dilatateur 66, jusqu'à ce que l'extrémité du troisième tube 67 vienne prendre appui sur la surface extérieure de la corticale 64, comme représenté sur la figure 20; le tube 67 possède à son extrémité en contact avec la corticale osseuse 64 des dents 68 afin d'obtenir un ancrage du tube dans la corticale 64; * retirer les premier et deuxième tubes dilatateurs 65 et 66, comme représenté sur la figure 21, ne laissant en place que la broche 61 entourée du tube 67 séparés l'un de l'autre par un espace tubulaire 68; * percer la corticale proximale 64 et la matière spongieuse 70 constituant l'intérieur de la vertèbre 60 au moyen d'un foret 69 guidé par la broche 61, comme représenté sur la figure 22, la matière spongieuse étant percée jusqu'au tiers distal environ, puis retirer le foret 69 et la broche 61; *fixer l'implant 1 sur un porte implant 71 et introduire l'ensemble dans le tube 67 jusqu'en butée à fond de perçage, comme représenté sur la figure 23; à cet effet, le porte implant pourra comporter une gaine à une extrémité de laquelle sera fixée l'extrémité proximale 20 de l'implant avec ses moyens 3 de prise en rotation, et à l'intérieur de laquelle une âme est apte à coulisser, âme dont l'extrémité distale est destinée à se visser dans l'extrémité distale 21 de l'implant 1; le porte implant comporte en outre des moyens de manoeuvre avec contrôle millimétrique de l'âme par rapport à la gaine, appelés système de compression, permettant faire coulisser l'âme dans la gaine en sorte de rapprocher les extrémités de l'implant l'une de l'autre; - positionner l'implant 1 expansible dans la vertèbre 60 pour faire correspondre le plan d'expansion 2 intrinsèque à l'implant avec un plan de restauration osseuse, c'est à dire dans le cas d'espèce, faire tourner le porte implant 71 avec l'implant 1 fixé à son extrémité dans la vertèbre 60, jusqu'à ce que le plan d'expansion 2 soit parallèle à l'axe mécanique de la vertèbre, comme représenté sur la figure 24; - déployer l'implant 1 expansible dans le plan de restauration osseuse, en vue de former une cavité 72 à l'intérieur de la vertèbre 60, en vissant le système de compression (non représenté) visant à rapprocher les extrémités 20 et 21 de l'implant l'une de l'autre, de manière à déployer les plateaux 6 et 7 opposés, formant avantageusement respectivement une première 8 et une deuxième 9 surfaces d'appui, dans la vertèbre 60, continues sur leur longueur sensiblement égale à la longueur de l'implant 1, comme représenté sur la figure 25; au cours de l'expansion, contrôler la réduction de la fracture grâce aux moyens de contrôle millimétrique, et après avoir obtenu l'expansion désirée, par exemple d'une valeur déterminée comprise entre une épaisseur minimale de l'implant avant toute expansion de celui-ci et une épaisseur maximale de l'implant après son expansion maximale, libérer ensuite le porte implant 71 en le dévissant de l'implant 1, puis l'extraire du tube 67, comme représenté sur la figure 26, seul l'implant en position déployée restant en place dans la vertèbre 60; l'expansion de l'implant dans la vertèbre se fait par appui sous les plateaux permettant de répartir la charge de poussée sur la longueur des plateaux sous ces derniers, permettant ainsi de proposer une longueur suffisante de plateaux tout en limitant un dimensionnement excessif de l'épaisseur de ces derniers pour résister en flexion; on notera ainsi, que l'implant selon l'invention adopte un rapport encombrement en longueur (non déployé) sur longueur de plateau élévateur extrêmement optimisé, permettant d'utiliser au mieux les espaces limités intra-osseux en vue d'une réduction de fracture par exemple; - injecter dans la cavité contenant l'implant 1 un matériau de remplissage 74, par exemple un ciment ionique, notamment un ciment phosphocalcique, un ciment acrylique ou un composé de ceux-ci en vue de combler la cavité; pour cela, introduire l'aiguille de l'injecteur 73 dans le tube 67 jusqu'à ce que l'extrémité de l'aiguille atteigne l'orifice distal 39 de l'implant 1, comme représenté sur la figure 27; injecter alors le matériau de remplissage en sorte d'obtenir un remplissage optimal de la cavité 72 créée par l'expansion de l'implant; continuer l'injection de manière rétrograde jusqu'à l'orifice proximal 64 de la vertèbre 60, comme représenté sur la figure 28; puis retirer l'aiguille de l'injecteur 73 et le tube 67, comme représenté sur la figure 29.
Un deuxième exemple de procédé de restauration d'une anatomie osseuse humaine, qui utilise un implant expansible selon l'invention, notamment tel que,décrit plus haut, va maintenant être décrit avec l'aide des figures 30 à 32. Il s'agit plus précisément d'un procédé de restauration osseuse d'une vertèbre par une voie transpédiculaire, avec réduction de fracture.
Le deuxième exemple de procédé est similaire au premier et se différencie de celui-ci par la voie de pénétration de l'implant dans la vertèbre 60 qui est maintenant réalisée de manière transpédiculaire, comme représenté sur la figure 30, en remplacement de la voie postérolatérale utilisée pour le premier procédé. De ce fait, seules quelques étapes du deuxième procédé ont été représentées sur les figures 30 à 32 afin de montrer la voie différente utilisée pour l'introduction de l'implant 1 dans la vertèbre. Pour les figures 30 à 32, les éléments identiques à ceux du premier exemple de procédé possèdent les mêmes références numériques, et ces figures correspondent respectivement aux étapes des figures 24, 25, et 28 du premier exemple de procédé. Concernant l'étape représentée sur la figure 32, celle-ci diffère légèrement de la figure 28 par la position de l'aiguille de l'injecteur 73, plus proche de l'extrémité distale de l'implant sur la figure 32. 30 35
Claims (12)
1. Implant expansible (1, 101), permettant de participer à une restauration d'une anatomie osseuse humaine ou animale, caractérisé en ce qu'il comprend: - un plan d'expansion unique (2, 102) déterminé, intrinsèque au dit implant, - des moyens de positionnement (3, 103) de l'implant expansible dans ledit os (60) pour faire correspondre ledit plan d'expansion intrinsèque au dit implant avec un plan de restauration osseuse, - des moyens de déploiement (4, 104) de l'implant expansible dans ledit plan d'expansion intrinsèque au dit implant, en vue de former une cavité (72) à l'intérieur dudit os.
2. Implant expansible selon la revendication 1, caractérisé en ce qu'il comprend des moyens de réglage (5, 105) d'une valeur déterminée d'expansion, comprise entre une épaisseur minimale (A) de l'implant avant toute expansion de celui-ci et une épaisseur maximale (B) de l'implant après son expansion maximale.
3. Implant expansible selon la revendication 1 ou 2, caractérisé en ce qu'il comprend un premier (6, 106) et un deuxième (7, 107) plateaux opposés, aptes à former respectivement une première (8, 108) et une deuxième (9, 109) surfaces d'appui dans ledit os (60) destinées à s'éloigner l'une de l'autre suivant ledit plan d'expansion (2, 102) unique lors de l'expansion de l'implant (1, 101).
4. Implant expansible selon la revendication 3, caractérisé en ce que lesdits premier (6, 106) et deuxième (7, 107) plateaux opposés forment des première (8, 108) et deuxième (9, 109) surfaces d'appui aptes à se déplacer perpendiculairement à un axe longitudinal (10, 110) dudit implant expansible (1, 101) lors de l'expansion de ce dernier.
5. Implant expansible selon la revendication 3 ou 4, caractérisé en ce que lesdites première (8) et deuxième (9) surfaces d'appui dans ledit os possèdent respectivement une longueur sensiblement égale à la longueur dudit implant.
6. Implant expansible selon l'une quelconque des revendications 3 à 5, caractérisé en ce que lesdits premier (6, 106) et deuxième (7, 107) plateaux forment des première (8, 108) et deuxième (9, 109) surfaces d'appui continues sur leur longueur, respectivement.
7. Implant expansible selon la revendication 6, caractérisé en ce que lesdits premier (6, 106) et deuxième (7, 107) plateaux forment respectivement des surfaces d'appui continues lisses.
8. Implant expansible selon l'une quelconque des revendications 3 à 7, caractérisé en ce que lesdits premier (6) et deuxième (7) plateaux forment respectivement des surfaces d'appui partiellement cylindriques dont une génératrice (11) est parallèle à un axe longitudinal (10) dudit implant (1) expansible.
9. Implant expansible selon l'une quelconque des revendications 3 à 8, caractérisé en ce qu'il comprend un premier (12, 14) et un deuxième (13, 15) appuis pour chacun desdits premier (6) et deuxième (7) plateaux, situés sous ces derniers respectivement.
10. Implant expansible selon la revendication 9, caractérisé en ce que lesdits premier (6) et deuxième (7) plateaux comportent respectivement une première (16, 18) et une deuxième (17, 19) ailes en porte-à-faux dont les zones d'attache respectives sont situées au niveau desdits premier (12, 14) et deuxième (13, 15) appuis.
11. Implant expansible selon la revendication 10, caractérisé en ce que lesdites première (16, 18) et deuxième (17, 19) ailes en porte-à-faux possèdent respectivement une longueur correspondant sensiblement à la valeur de déplacement maximale d'un desdits premier (6) ou deuxième (7) plateau dans ledit plan d'expansion unique (2).
12. Implant expansible selon l'une quelconque des revendications 1 à 11, caractérisé en ce que lesdits moyens de positionnement (3, 103) de l'implant expansible dans ledit os pour faire correspondre ledit plan d'expansion intrinsèque (2, 102) au dit implant avec un plan de restauration osseuse, comprennent des moyens de prise (37, 38, 137, 138) permettant d'orienter ledit implant angulairement autour d'un axe longitudinal (10, 110) dudit implant (1, 101) expansible.
