Abstract
El dispositivo para trombectomía comprende un catéter (1) y una endoprótesis vascular (2), definiendo un extremo distal y un extremo proximal, pudiéndose colocar el dispositivo en una posición retraída y en una posición extendida, y se caracteriza porque el dispositivo también comprende un recubrimiento (3) exterior a dicha endoprótesis vascular (2), siendo el diámetro del extremo distal de la endoprótesis vascular (2) mayor en la posición extendida que en la posición retraída.#Permite parar el flujo de sangre en la arteria, e incluso invertirlo cuando se combina con vacío. De esta manera, se consigue aspirar la totalidad del trombo y evitar el desprendimiento de fragmentos durante el procedimiento o durante la retirada del dispositivo.

DESCRIPCION
Dispositivo para trambectomia
La presente invention se refiere a un dispositivo para trombectomia, que permite la extraction de trombos a nivel vascular.
Antecedentes de la invencion
El tratamiento endovascular del ictus se realiza desde la decada de los 90. Su crecimiento en numero de pacientes tratados ha sido lento pero constante. Las principales dificultades para su generalization es la necesidad de un sistema coordinado a diferentes niveles que permita que el paciente este en un centro capacitado para administrar estos tratamientos de alta complejidad en menos de 6-8 horas desde el inicio de los smtomas.
Recientemente se ha publicado el primer estudio (Berkhemer O.A. et al. "A randomized trial of intraarterial treatment for acute ischemic stroke”. N Engl J Med. Enero 2015; 372: 11-20. doi: 10.1056/NEJMoa1411587. Epub 17 diciembre de 2014. PubMed PMID: 25517348), que demuestra la eficacia de la trombectomia frente al tratamiento medico convencional en la reduction de la discapacidad de los pacientes a los 3 meses del ictus. Ademas, se preve que en los proximos meses aparezcan publicados ensayos clmicos que confirmen definitivamente la superioridad de la trombectomia con dispositivos de ultima generation en la reduccion de la discapacidad en comparacion con otros tratamientos medicos.
En lo que refiere al tratamiento endovascular, inicialmente la estrategia era la perfusion de un agente fibrinolrtico a nivel local mediante cateter directamente sobre el trombo. A principios de la decada de 2000 aparecio un dispositivo que pareda ser mas eficaz que la fibrinolisis intra-arterial. Se trataba de una espiral que se desplegaba alrededor del trombo, facilitando su extraccion (MERCi®).
A partir de 2006 se empezo a popularizar un sistema que basicamente se basa en acercar un cateter de gran calibre al trombo para poder aspirarlo. El cateter se conecta a una bomba de aspiracion continua (Penumbra®).
Este sistema ha ido evolucionando con los anos, tratando de conseguir un cateter con un diametro cada vez mayor, capaz de navegar hasta el trombo.

Aproximadamente en el ano 2009 se empezo a utilizar los denominados "stent-retrievers”. Su uso consiste en atravesar el trombo con un microcateter, seguidamente se avanza la endoprotesis vascular por el microcateter. Una vez el extremo distal del dispositivo ha alcanzado la parte mas distal del trombo, se procede a desenvainar la endoprotesis, que se expande a nivel del trombo, capturandolo. Se recomienda esperar unos minutos con la endoprotesis expandida para incrementar la incorporation del trombo y entonces retirar la endoprotesis expandida con la intention de que arrastre el trombo. Este ultimo paso se puede hacer mientras se aspira por el cateter para intentar invertir el flujo y asi incrementar las probabilidades de recuperar el trombo.
Los dispositivos de tipo "stent-retrievers” por su gran eficacia y rapidez han desplazado totalmente a los dispositivos de primera generation indicados anteriormente.
Ademas, al usar un "stent-retriever” se suele utilizar un cateter guia con balon. Este cateter se avanza unicamente hasta la carotida extracraneal (lejos de los trombos situados en las arterias intracraneales). Una vez hinchado el balon situado en el extremo del cateter, este es capaz de parar el flujo en el segmento arterial distal al balon, donde se encuentra el trombo que queremos extraer. Si se aspira por el podemos invertir el flujo en el segmento arterial distal al balon con el fin de facilitar la retirada del trombo junto al "stent-retriever”.
En resumen, actualmente existen dos tendencias en el uso de dispositivos para trombectomia: los denominados "stent-retrievers” (con o sin cateter con balon), y la aspiration mediante cateter (manual con jeringa o mediante bomba de aspiration). Ambas tecnicas se pueden combinar.
Un ejemplo de un dispositivo para trombectomia se describe en el documento WO201182319, que tiene una variedad de puntas intercambiables en funcion de las necesidades que se encuentra el intervencionista durante la trombectomia. En todos los casos el dispositivo descrito en este documento ha de atravesar el trombo, por su interior o pasar entre el trombo y la pared arterial antes de ser expandido y se debe fragmentar el trombo para que se disuelva por medios naturales o artificiales, de manera que no permite la retirada del trombo de una forma que asegure la no embolization distal por liberation de fragmentos.
Ademas, a diferencia del dispositivo de la presente invention, no tiene la punta recubierta y, por lo tanto, no es capaz de parar el flujo de la arteria, ni mucho menos invertirlo.

Por lo tanto, el dispositivo de acuerdo con la presente invention pretende sustituir a los dispositivos de aspiration, mejorando su eficacia, puesto que permitira aspirar desde una distancia muy cercana al trombo y con una boca grande, que permitira ocluir la arteria parando el flujo y aspirar el trombo entero sin fragmentarlo.
Description de la invencion
Con el dispositivo de la invencion se consiguen resolver los inconvenientes citados, presentando otras ventajas que se describiran a continuacion.
El dispositivo para trombectomia de acuerdo con la presente invencion comprende un cateter y una endoprotesis vascular, definiendo un extremo distal y un extremo proximal, pudiendose colocar el dispositivo en una position retraida y en una position extendida, y se caracteriza porque el dispositivo tambien comprende un recubrimiento exterior a dicha endoprotesis vascular, siendo el diametro del extremo distal de la endoprotesis vascular mayor en la posicion extendida que en la posicion retraida.
La endoprotesis vascular tiene un diametro variable pudiendo alcanzar el diametro de la arteria en ese punto y asi poder parar el flujo y admitir en su interior el trombo causante de la oclusion entero, sin necesidad de fragmentarlo. La endoprotesis podra ser autoexpandible o expandirse mediante otros mecanismos como un balon hinchado en su interior.
De acuerdo con una realization preferida, dicho recubrimiento es de politetrafluoretileno expandido (ePTFE) con las fibras del polimero orientadas de forma paralela a la direction de extrusion, es decir, la longitud del cateter, y de esta manera no ofrece casi resistencia a la expansion y su elasticidad es muy baja para no provocar que la endoprotesis vascular se vuelva a cerrar.
Ventajosamente, dicha endroprotesis vascular esta colocada en el interior del cateter en la posicion retraida y esta colocada al menos parcialmente en el exterior del cateter en la posicion extendida.
Preferentemente, el recubrimiento esta sujeto mediante una estructura exterior, que comprende, por ejemplo, al menos un alambre, un anillo en uno de sus extremos y un aro abierto en otro de sus extremos, sujetando por ejemplo dicho aro abierto el recubrimiento en su extremo distal.

Ademas, dicho recubrimiento esta ventajosamente unido a la endoprotesis vascular mediante dicha estructura exterior en su extremo proximal, de manera que se muevan a la vez y siempre se mantengan el recubrimiento y la endoprotesis vascular en paralelo.
De acuerdo con una realization preferida, dicha endoprotesis vascular es de nitinol, y comprende una marca radiopaca en su extremo distal.
El extremo distal expandible y recubierto del dispositivo de acuerdo con la presente invention permite parar el flujo de sangre en la arteria, e incluso invertirlo cuando se combina con vado. De esta manera, se consigue aspirar la totalidad del trombo y evitar el desprendimiento de fragmentos durante el procedimiento o durante la retirada del dispositivo.
Ademas, el dispositivo objeto de la presente invencion no atraviesa ni comprime el trombo. De esta forma, presenta la ventaja de que asegura la retirada de la totalidad del trombo, evitando asi una posible embolization distal.
Breve description de los dibujos
Para mejor comprension de cuanto se ha expuesto, se acompanan unos dibujos en los que, esquematicamente y tan solo a trtulo de ejemplo no limitativo, se representa un caso practico de realizacion.

Descripcion de una realizacion preferida
En las figuras 1 y 2 se puede apreciar el dispositivo de trombectomia de acuerdo con la presente invencion, que comprende un cateter 1 y una endoprotesis vascular 2.

Para facilitar la description, los extremos del cateter 1 y de la endoprotesis vascular 2 se definen como extremo distal y extremo proximal, siendo el extremo distal mas alejado al usuario que manipula que dispositivo de acuerdo con la presente invention y, por lo tanto, el mas cercano al trombo que se desea retirar (indicado con el numero de referencia 10), y el extremo proximal es el extremo mas cercano al usuario y, por lo tanto, el mas alejado al trombo 10.
El dispositivo de trombectomia de acuerdo con la presente invencion puede colocarse en una position retraida (mostrada en la figura 1) y en una position extendida (mostrada en la figura 2), tal como se explicara a continuation, siendo el diametro del extremo distal de la endoprotesis vascular 2 mayor en la posicion extendida que en la posicion retraida.
En la posicion retraida dicha endroprotesis vascular 2 esta colocada en el interior del cateter 1, mientras que en la posicion extendida esta colocada al menos parcialmente en el exterior del cateter 1.
De acuerdo con una realization preferida, dicha endoprotesis vascular 2 es de nitinol y comprende una marca radiopaca 8 en su extremo distal.
Ademas, el dispositivo de trombectomia de acuerdo con la presente invencion tambien comprende un recubrimiento 3, que es exterior a dicha endoprotesis vascular 2, tal como se puede apreciar en las figuras.
Dicho recubrimiento 3 es preferiblemente de politetrafluoretileno expandido (ePTFE) y puede comprender un revestimiento hidrofflico, y esta sujeto mediante una estructura exterior 4, que, de acuerdo con una realizacion preferida, comprende al menos un alambre 5, un anillo 6 en uno de sus extremos y un aro abierto 7 en otro de sus extremos.
Dicho recubrimiento 3 esta ventajosamente unido a la endoprotesis vascular 2 mediante dicha estructura exterior 4 en su extremo proximal, y dicho aro abierto 7 esta sujeto al recubrimiento 3 en su extremo distal.
El dispositivo de trombectomia es de calibre intermedio y permite avanzar hasta el nivel donde se encuentra el trombo 10. Una vez avanzado, el extremo distal de la endoprotesis vascular 2 se expandira hasta alcanzar el diametro de la arteria en ese punto. Esto permitira ocluir el flujo proximal, invertir mediante aspiration el flujo en el segmento arterial distal, una

distancia de arrastre del trombo muy corta y la capacidad de admitir en su interior el trombo entero sin fragmentacion, facilitando su extraccion.
La colocacion del dispositivo se realiza mediante fluoroscopia y, por lo tanto, la punta del dispositivo comprende dicha marca radiopaca 8.
La expansion del extremo distal de la endoprotesis vascular 2 se realiza al ser desenvainada, si se trata de una protesis autoexpandible, es decir, si la endoprotesis esta hecha de nitinol (NiTi) tendria memoria de forma y seria autoexpandible, y en el momento que sale del cuerpo del cateter a la temperatura corporal su tendencia natural seria de expandirse. Debe indicarse, sin embargo, que esta expansion puede realizarse mediante otros sistemas, tal por ejemplo, el hinchado en su interior de un balon del diametro requerido.
Para aspirar el trombo 10, se aplica vado a traves del cateter extractor 1 mediante una jeringa en el otro extremo del cateter. Al aplicar vado, el flujo de sangre se invierte y el trombo queda aspirado dentro del dispositivo.
En este punto solo queda retirar el dispositivo junto con el trombo 10, manteniendo el vado dentro del dispositivo. Esto es posible debido a que el sistema arterial va aumentando su diametro a medida que vamos bajando, por ejemplo desde las arterias cerebrales hasta la aorta.
A pesar de que se ha hecho referencia a una realizacion concreta de la invencion, es evidente para un experto en la materia que el dispositivo para trombectomia descrito es susceptible de numerosas variaciones y modificaciones, y que todos los detalles mencionados pueden ser sustituidos por otros tecnicamente equivalentes, sin apartarse del ambito de proteccion definido por las reivindicaciones adjuntas.
Claims (9)
- 5101520253035ES 2 577 288 A1REIVINDICACIONES1. Dispositivo para trambectomia, que comprende un cateter (1) y una endoprotesis vascular (2), definiendo un extremo distal y un extremo proximal, pudiendose colocar el dispositivo en una posicion retraida y en una posicion extendida, caracterizado porque el dispositivo tambien comprende un recubrimiento (3) exterior a dicha endoprotesis vascular (2), siendo el diametro del extremo distal de la endoprotesis vascular (2) mayor en la posicion extendida que en la posicion retraida.
- 2. Dispositivo para trombectomia de acuerdo con la reivindicacion 1, en el que dicho recubrimiento (3) es de politetrafluoretileno expandido (ePTFE).
- 3. Dispositivo para trombectomia de acuerdo con la reivindicacion 1, en el que dicha endroprotesis vascular (2) esta colocada en el interior del cateter (1) en la posicion retraida y esta colocada al menos parcialmente en el exterior del cateter (1) en la posicion extendida.
- 4. Dispositivo para trombectomia de acuerdo con la reivindicacion 1, en el que el recubrimiento (3) esta sujeto mediante una estructura exterior (4).
- 5. Dispositivo para trombectomia de acuerdo con la reivindicacion 4, en el que dicha estructura exterior comprende al menos un alambre (5), un anillo (6) en uno de sus extremos y un aro abierto (7) en otro de sus extremos.
- 6. Dispositivo para trombectomia de acuerdo con la reivindicacion 4 o 5, en el que dicho recubrimiento (3) esta unido a la endoprotesis vascular (2) mediante dicha estructura exterior (4) en su extremo proximal.
- 7. Dispositivo para trombectomia de acuerdo con la reivindicacion 5, en el que dicho aro abierto (7) esta sujeto al recubrimiento (3) en su extremo distal.
- 8. Dispositivo para trombectomia de acuerdo con la reivindicacion 1, en el que dicha endoprotesis vascular (2) es de nitinol.
- 9. Dispositivo para trombectomia de acuerdo con la reivindicacion 1, en el que dicha endoprotesis vascular (2) comprende una marca radiopaca (8) en su extremo distal.
