Ejemplo de referencia 1
Puesto que el exón 45 es uno de los exones que con más frecuencia están delecionados en DMD, en un principio el objetivo fue inducir la omisión específica del exón 46 (figura 1). Esto produciría la distrofina más corta, pero en gran medida funcional, que se encuentra en los pacientes BMD que portan una deleción de los exones 45 y 46. El sistema en principio se constituyó para la modulación del corte y empalme de pre-ARNm de distrofina del gen de distrofina de ratón. Después el objetivo fue el gen de distrofina humano, con la intención de restablecer el marco de lectura traduccional y la síntesis de distrofina en células musculares procedentes de pacientes DMD afectados por una deleción del exón 45.
Diseño de mAON y hAON
Se diseñó un serie de AON específicos de ratón y humano (mAON y hAON) dirigidos a una parte interna del exón 46 que contiene un tramo de secuencias ricas en purina, y se establece la hipótesis de que desempeña un papel regulador putativo en el proceso de corte y empalme del exón 46 (figura 2). Para la selección inicial de los AON en los ensayos de desplazamiento de la movilidad en gel (véase a continuación), se utilizaron oligonucleótidos de ADN no modificados (sintetizados por EuroGentec, Bélgica). Para los experimentos de transfección reales en células musculares, se emplearon oligorribonucleótidos de 2'-O-metil-fosforotioato (también sintetizados por EuroGentec, Bélgica). Se sabe que estos oligonucleótidos de ARN modificados son resistentes a endonucleasas y ARNasaH, y que se unen al ARN con alta afinidad. Las secuencias de estos AON que fueron finalmente eficaces y que se aplicaron a células musculares in vitro se muestran a continuación. Los correspondientes AON específicos de ratón y humano son altamente homólogos, pero no completamente idénticos.

Ensayos de desplazamiento de la movilidad en gel
Se determina la eficacia de los AON mediante su afinidad de unión a la secuencia diana. A pesar de las recientes mejoras en los programas de simulación por ordenador para la predicción del plegamiento del ARN, resulta difícil especular cuáles de los AON diseñados serán capaces de unirse a la secuencia diana con una afinidad relativamente alta. Por tanto, se realizaron ensayos de desplazamiento de la movilidad en gel (según los protocolos descritos en Bruice et al., 1997). El fragmento de ARN del exón 46 diana se generó mediante la transcripción con T7 in vitro de un fragmento de PCR (amplificado a partir de ARNm de músculo murino o humano utilizando un cebador sentido que contenía la secuencia del promotor de T7) en presencia de 32P-CTP. La afinidad de unión de los AON individuales (0,5 pmol) por los fragmentos de la transcripción diana se determinó mediante hibridación a 37 °C durante 30 minutos y una posterior electroforesis en gel de poliacrilamida (al 8%). Estos ensayos se realizaron para la selección de AON específicos de ratón y humanos (figura 3). Al menos 5 AON específicos de ratón diferentes (mAON#4, 6, 8, 9 y 11) y cuatro correspondientes AON específicos de humano (hAON#4, 6, 8, y 9) generaron un desplazamiento en la movilidad, lo cual demuestra su afinidad de unión por el ARN diana.
Transfección en cultivos de células musculares
Los AON específicos del exón 46 que mostraron la afinidad de unión por la diana más alta en los ensayos de desplazamiento de la movilidad en gel se seleccionaron para el análisis de su eficacia para inducir la omisión de células musculares in vitro. En todos los experimentos de transfección se incluyó un AON no específico como control negativo para la omisión específica del exón 46. Tal como se mencionó, el sistema primero se constituyó en células musculares de ratón. Se emplearon cultivos de mioblastos en proliferación y miotubos postmitóticos (que expresan niveles mayores de distrofina) derivados de la línea de células musculares de ratón C2C12. Para los posteriores experimentos en cultivos de células musculares derivadas de humano se emplearon cultivos de células musculares primarias aisladas de biopsias de músculo procedentes de un individuo no afectado y dos pacientes DMD no relacionados que portan una deleción del exón 45. Estos cultivos heterogéneos contenían aproximadamente 20-40% de células miogénicas. Se transfectaron los diferentes AON (a una concentración de 1 µM) en las células utilizando el polímero catiónico PEI (MBI Fermentas) a una proporción equivalente de 3. Los AON transfectados en estos experimentos contenían un grupo fluoresceína 5' que permite determinar las eficacias de transfección contando el número de núcleos fluorescentes. Generalmente, más del 60% de las células mostraron una captación nuclear específica de los AON. Para facilitar el análisis de RT-PCR, el ARN se aisló 24 horas después de la transfección utilizando RNAzol B (CamPro Scientific, Países Bajos).
RT-PCR y análisis de la secuencia
El ARN se sometió a transcripción inversa utilizando la polimerasa de C. therm. (Roche) y un cebador inverso específico del exón 48. Para facilitar la detección de la omisión del exón 46 de distrofina, el ADNc se amplificó mediante dos rondas de PCR, que incluyen una amplificación anidada empleando cebadores en los exones 44 y 47 (para el sistema humano), o los exones 45 y 47 (para el sistema de ratón). En los cultivos celulares de mioblastos y miotubos de ratón, se detectó un producto truncado cuyo tamaño se corresponde con el exón 45 directamente cortado y empalmado para producir el exón 47 (figura 4). Posteriores análisis de la secuencia confirmaron la omisión específica del exón 46 de estas transcripciones de distrofina de ratón. La eficacia de la omisión de exón fue diferente para los AON individuales, mostrando mAON#4 y #11 la mayor eficacia. Después de estos resultados prometedores, los inventores se centraron en inducir una modulación similar del corte y empalme de la distrofina en cultivos de células musculares derivadas de humano. Así, se detectó un producto truncado en las células musculares control, que se corresponde con el exón 45 cortado y empalmado para producir el exón 47. De modo interesante, en las células musculares derivadas de pacientes, se detectó un fragmento más corto que consiste en el exón 44 cortado y empalmado para producir el exón 47. La omisión específica del exón 46 de las transcripciones de distrofina humana se confirmó mediante los datos de la secuencia. Esta modulación del corte y empalme en la transcripción de distrofina de ratón y humana no se observó en los cultivos celulares no transfectados ni en los cultivos transfectados con un AON no específico.
Análisis inmunohistoquímico
Los inventores quisieron inducir la omisión del exón 46 en células musculares procedentes de pacientes que portan una deleción del exón 45, para restablecer la traducción y la síntesis de una proteína de distrofina. Omisión simultánea de múltiples exones de distrofina
La omisión de un exón adicional, tal como el exón 46 o el exón 51, restablece el marco de lectura para un número considerable de diferentes mutaciones de DMD. La gama de mutaciones en las que puede aplicarse esta estrategia puede aumentar mediante la omisión simultánea de más de un exón. Por ejemplo, en pacientes DMD con una deleción del exón 46 para producir el exón 50, solo la omisión de los exones 45 y 51 que flanquean la deleción permite el restablecimiento del marco de lectura traduccional.
Ejemplo de referencia: Omisión de exón independiente de ERS
Una mutación en el exón 29 conduce a la omisión de este exón en dos pacientes con distrofia muscular de Becker (Ginjaar et al., 2000, EJHG, vol. 8, pp. 793-796). Los inventores estudiaron la viabilidad de dirigir la omisión del exón 29 dirigiendo los AON al sitio de mutación. La mutación está localizada en un tramo rico en purina que puede asociarse con actividad ERS. Los inventores diseñaron una serie de AON (véase a continuación) dirigidos a secuencias dentro (h29AON#1 a h29AON#6) y fuera (h29AON#7 a h29AON#11) de la ERS hipotética. Se realizaron ensayos de desplazamiento de la movilidad en gel (según se describe) para identificar los AON con la mayor afinidad por el ARN diana (figura 8). Después, h29AON#1, #2, #4, #6, #9, #10, y #11 se transfectaron en cultivos de miotubos control humanos utilizando el reactivo de transfección PEI. El ARN se aisló 24 horas después de la transfección, y se generó el ADNc utilizando un cebador inverso específico del exón 31. Se realizó una amplificación con PCR de la región diana utilizando diferentes combinaciones de cebadores que flanquean al exón 29. La RT-PCR y el posterior análisis de la secuencia (figura 8 B, C) revelaron que los inventores fueron capaces de inducir la omisión específica del exón 29 de la transcripción de distrofina humana. Sin embargo, los AON que facilitan esta omisión se dirigieron a secuencias dentro y fuera de la ERS hipotética (h29AON#1, #2, #4, #6, #9, y #11). Estos resultados sugieren que la omisión del exón 29 aparece independientemente de que el exón 29 contenga o no una ERS y que, por tanto, es muy probable que la unión de los AON al exón 29 inactive una señal de inclusión de exón, en lugar de una ERS. Esta prueba de una omisión de exón independiente de ERS puede extender la aplicabilidad global de esta terapia a exones sin ERS.

Ejemplo de referencia: Omisión del exón 46 inducida por AON in vivo en tejido muscular murino
Después de los prometedores resultados en células musculares cultivadas, los inventores ensayaron los diferentes AON específicos del exón 46 de distrofina de ratón in vivo inyectándolos, unidos a polietilenimina (PEI), en el músculo gastrocnemio de ratones control. Con mAON#4, #6, y #11, que previamente habían demostrado ser eficaces en células musculares de ratón in vitro, los inventores fueron capaces de inducir la omisión del exón 46 en tejido muscular in vivo, según se determina mediante RT-PCR y análisis de la secuencia (figura 9). La omisión in
vivo del exón 46 fue dependiente de la dosis, produciéndose las mayores eficacias (hasta 10%) después de la inyección de 20 µg por músculo diarios durante dos días consecutivos.

Aspectos y realizaciones de la descripción
En un primer aspecto, se proporciona un método para dirigir el corte y empalme de un pre-ARNm en un sistema capaz de realizar una operación de corte y empalme, que comprende poner en contacto dicho pre-ARNm en dicho 5 sistema con un agente capaz de inhibir específicamente una señal de inclusión de exón de al menos un exón en dicho pre-ARNm, y dicho método comprende también permitir el corte y empalme de dicho pre-ARNm.
Un método preferido comprende también permitir la traducción del ARNm producido por el corte y empalme de dicho pre-ARNm.
Un método preferido es en el que dicho ARNm codifica una proteína funcional.
10 Un método preferido es en el que dicha proteína comprende dos o más dominios, en los que al menos uno de dichos dominios está codificado por dicho ARNm como resultado de la omisión de al menos parte de un exón en dicho pre-ARNm.
Un método preferido es en el que dicho contacto produce la activación de un sitio de corte y empalme críptico en un exón contactado.
15 En un segundo aspecto, se proporciona un método para disminuir, al menos en parte, la producción de una proteína aberrante en una célula,
y dicha célula comprende un pre-ARNm que comprende exones que codifican dicha proteína, comprendiendo dicho método
proporcionar a dicha célula un agente capaz de inhibir específicamente una señal de inclusión de exón

Los métodos del primer y segundo aspecto son preferiblemente de modo que dicha señal de inclusión de exón comprende una secuencia de reconocimiento de exón.
Los métodos del primer y segundo aspecto son preferiblemente de modo que dicha señal de inclusión de exón está presente en un exón que comprende una pareja de donante/aceptor de corte y empalme fuerte.
Los métodos del primer y segundo aspecto son preferiblemente de modo que dicha traducción produzca una proteína de distrofina mutante o normal, más preferiblemente en el que dicha proteína de distrofina mutante es equivalente a una proteína de distrofina de un paciente de Becker. En este método preferido, dicha señal de inclusión de exón está presente en el exón número 2, 8, 43, 44, 45, 46, 50, 51, 52 o 53.
Los métodos del primer y segundo aspecto son preferiblemente de modo que dicho agente comprende un ácido nucleico o un equivalente funcional de este. Más preferiblemente, dicho ácido nucleico contiene entre 15-25 nucleótidos o un equivalente funcional de este.
Los métodos del primer y segundo aspecto son preferiblemente de modo que comprenden además proporcionar a dicha célula otro agente capaz de inhibir una señal de inclusión de exón presente en otro exón de dicho pre-ARNm.
En un tercer aspecto, se proporciona un método para determinar si un ácido nucleico, o un equivalente funcional de este, que comprende complementariedad con una parte de un exón, es capaz de inhibir específicamente una señal de inclusión de exón de dicho exón,
que comprende
proporcionar dicho ácido nucleico a una célula que porta un pre-ARNm que contiene dicho exón, cultivar dicha célula para permitir la formación de un ARNm a partir de dicho pre-ARNm, y determinar si dicho exón está ausente de dicho ARNm.
Un método preferido del tercer aspecto comprende además determinar in vitro la afinidad de unión relativa de dicho ácido nucleico, o un equivalente funcional de este, por una molécula de ARN que comprende dicho exón.
En un cuarto aspecto, se proporciona un ácido nucleico, o un equivalente funcional de este, que puede obtenerse mediante un método del tercer aspecto.
En un quinto aspecto, se proporciona un vehículo de transporte de un ácido nucleico, que comprende un ácido nucleico del cuarto aspecto o su complemento.
En un sexto aspecto, se proporciona un vehículo de transporte de un ácido nucleico capaz de expresar un ácido nucleico del cuarto aspecto.
En un séptimo aspecto, se proporciona un uso de un ácido nucleico del cuarto aspecto, o un vehículo de transporte de un ácido nucleico del quinto o sexto aspecto, para la preparación de un medicamento.
En un octavo aspecto, se proporciona un uso de un ácido nucleico del cuarto aspecto, o un vehículo de transporte de un ácido nucleico del quinto o sexto aspecto, para la preparación de un medicamento para el tratamiento de una enfermedad heredada o la predisposición a una enfermedad.
En un noveno aspecto, se proporciona un uso de un ácido nucleico, o un equivalente de este, que comprende una cualidad inhibidora de una señal de inclusión de exón, para la preparación de un medicamento.
En un décimo aspecto, se proporciona un animal no humano provisto de un ácido nucleico del cuarto aspecto. Preferiblemente, el animal no humano del décimo aspecto comprende además un ácido nucleico que codifica una proteína humana, o un equivalente funcional de este. Aún más preferiblemente, este animal no humano comprende además una mutación silenciadora en el gen que codifica un homólogo animal de dicha proteína humana.
