A. 背景
デュシェンヌ型筋ジストロフィー(DMD)は劣性X連鎖型筋ジストロフィーであり、3,500人の少年のうち約1人が罹患し、筋肉変性および早期死亡をもたらす。この障害は、タンパク質ジストロフィンをコードするヒトX染色体上に位置する遺伝子ジストロフィンの変異によって引き起こされる。ジストロフィンは、細胞膜のジストログリカン複合体(DGC)に構造的安定性を与える筋組織内の重要な成分である。どちらの性別も変異を保有するが、女性は骨格筋形態の疾患の影響を受けることはめったにない。
症状は通常、2〜3歳の男児に現れ、早期乳児期に見られることもある。早期乳児期までは症状が現れなくても、検査室検査では、出生時に活性変異を保有する幼児を特定することができる。筋肉量の喪失に関連する脚および骨盤の近位筋の進行性の衰弱が最初に観察される。最終的にこの衰弱は腕、首、および他の領域に広がる。初期の徴候には、仮性肥大(ふくらはぎおよび三角筋の肥大)、低持久力、人の手を借りずに起立するのが難しいこと、または階段を上がれないことが含まれうる。病状が進行するにつれて、筋組織は消耗を起こし、最終的には脂肪および線維性組織に置き換えられる(線維症)。10歳までには、歩行を補助するために支持具が必要とされることがあり、ほとんどの患者は、12歳までに車椅子に頼っている。その後の症状には、脊椎の湾曲を含めて、骨格の変形をもたらす異常な骨の発達が含まれうる。進行性の筋肉劣化によって、運動の喪失が起こり、最終的には麻痺に至る。知的障害があるかもしれないし、またはないかもしれないが、存在しても、子供が年を取るにつれて次第に悪化することはない。DMDに罹患している男性の平均余命は25歳前後である。
・ ぎこちない歩き方、足踏みのし方、または走り方− (患者は、ふくらはぎの筋肉の緊張のため、足の前部で歩く傾向がある。また、つま先歩きは、膝伸筋の弱さに対する代償的適応である。)
・ 頻繁な転倒

デュシェンヌ型筋ジストロフィー(DMD)は、X染色体の短腕に位置する遺伝子座Xp21のジストロフィン遺伝子の変異によって引き起こされる。ジストロフィンは、多くのサブユニットを含むタンパク質複合体を通じて、各筋線維の細胞骨格を、基礎となる基底層(細胞外マトリックス)に接続する役割を担う。ジストロフィンが存在しないことで、過剰なカルシウムが筋線維鞘(細胞膜)に浸透することが可能になる。カルシウムおよびシグナル伝達経路の変化は、水をミトコンドリアに侵入させ、ミトコンドリアは破裂する。
障害の家族歴を有する人には、遺伝カウンセリングが勧められる。デュシェンヌ型筋ジストロフィーは、妊娠中に行われる遺伝子検査によって約95%の精度で検出することができる。
ジストロフィン遺伝子の筋肉特異的アイソフォームは79個のエクソンから構成され、DNA検査および分析は通常、影響を受けるエクソンの特定のタイプの変異を同定することができる。DNA検査はほとんどの場合に診断を確定する。
DNA検査で変異を見つけられない場合は、筋生検検査が行われうる。小さな筋組織サンプルを抽出し(通常は針の代わりにメスで)、ジストロフィンの存在を明らかにする色素を適用する。タンパク質の完全な欠如は、その病状を示す。
DMDはX連鎖劣性遺伝子によって運ばれる。男性はX染色体を1つしか持たないので、変異した遺伝子の1コピーでDMDを引き起こす。父親はX連鎖形質を息子に渡すことができないので、変異は母親によって伝達される。
DMDの現行の治療法はなく、規制当局によって継続的な医学的必要性が認識されている。ある種の変異に対するエクソンスキッピング処理を用いた第1〜2a相試験は、歩行の減退を止め、わずかな臨床的改善をもたらした。処置は一般に、生活の質を最大限にするために症状の発症を制御することを目的とし、以下を含む。
・ プレドニゾロンおよびデフラザコートのようなコルチコステロイドは、エネルギーおよび強度を高め、いくつかの症状の重症化を遅らせる。
・ ランダム化対照試験は、β2アゴニストが筋力を増加させるが、疾患の進行を改変しないことを示している。β2アゴニストに対するほとんどのRCTの経過観察時間は、わずか12ヶ月前後であり、ゆえにその時間枠を超えて結果を外挿することはできない。
・ 水泳などの、軽度な不快感のない身体活動が推奨される。不活動であること(たとえば、ベッドでの静養)は筋肉疾患を悪化させうる。
・ 理学療法は筋力、柔軟性、および機能を維持するのに役立つ。
・ 矯正装具(支持具および車椅子のような)は、移動性および自己ケア能力を改善しうる。睡眠中に足首を適切な位置で支える、体にぴったりした取り外し可能な脚支持具は、拘縮の発症を遅らせることができる。
・ 疾患の進行に応じて適切な呼吸補助が重要である。
理学療法士は、患者が最大限の身体的可能性に達することを可能とすることに携わっている。彼らの目的は以下を行うことである。
・ 必要に応じてストレッチおよび運動のプログラムを開発することによって拘縮および変形の発症を最小限に抑えること
・ 固定具および耐久性のある医療機器を推薦することによって、身体的な性質の他の二次性合併症を予測し、最小限に抑えること
・ 呼吸機能を監視し、呼吸運動を補助するための技法および分泌物を取り除く方法について助言すること
筋ジストロフィーに関連した呼吸障害を有する人の処置において、呼吸のたびに調節可能な体積(量)の空気を人に送達する最新の「従量式酸素吸入器/人工呼吸器」が有益である。酸素吸入器は、空気を直接送達する侵襲性の気管内チューブまたは気管切開チューブを必要とすることもあるが、一部の人にとっては、フェイスマスクまたはマウスピースによる非侵襲性の送達で十分である。この後者の方法においては気道陽圧機器、特に二相性の気道陽圧機器が用いられることもある。呼吸具は、携行性のために外部バッテリを備えた電動車椅子の底面または背面の酸素吸入器トレイに容易に適合しうる。
デュシェンヌ型筋ジストロフィーは、最終的に全ての随意筋に影響を及ぼし、後期に心筋および呼吸筋に影響を与える進行性疾患である。平均余命は現在25歳前後であると推定されているが、これは患者によって異なる。最近の医学の進歩は、罹患者の生活を拡大させている。全ての筋疾患に焦点を当てた指導的立場の英国慈善団体である、The Muscular Dystrophy Campaignは、「高い基準の医学的ケアにより、デュシェンヌ型筋ジストロフィーを有する若年男性は30代を優に超えるまで生きることが多い」と述べている。
A. CRISPR/CAS
CRISPR (Clustered Regularly Interspaced Short Palindromic Repeat)は、塩基配列の短い反復を含んだDNA遺伝子座である。各反復の後に、ウイルスへの事前曝露由来の「スペーサーDNA」の短いセグメントが続く。CRISPRは、配列決定された真正細菌ゲノムの約40%および配列決定された古細菌の90%に見出される。CRISPRは、CRISPRに関連するタンパク質をコードするcas遺伝子と関連していることが多い。CRISPR/Casシステムは、原核生物の免疫系であり、プラスミドおよびファージのような外来遺伝要素に対する耐性を付与し、後天性免疫の一形態を提供する。CRISPRスペーサーは、真核生物におけるRNAiのような、これらの外因性遺伝要素を認識かつサイレンシングする。
Cas9はヌクレアーゼ、つまりDNAを切断するために特化した酵素であり、二重らせんの各鎖に対して1つずつの、2つの活性切断部位を有する。チームによって、標的DNAを狙った場所に位置付けるCas9の能力を保持しながら、一方または両方の部位を無効にできることが実証された。Jinekら(2012)はtracrRNAおよびスペーサーRNAを組み合わせて「単一のガイドRNA」分子にし、これはCas9と混合されて、正しいDNA標的を見出し、切断しえた。Jinekら(2012)は、そのような合成ガイドRNAを遺伝子編集に使用できる可能性があることを提唱した。
RNAガイドタンパク質として、Cas9はDNA標的の認識を指示する短いRNAを必要とする(Mali et al., 2013a)。Cas9は、PAM配列NGGを含むDNA配列を選択的に調べ、プロトスペーサー標的なしでここに結合することができる。しかしながら、Cas9-gRNA複合体は、二本鎖切断をもたらすためにgRNAに近い一致を必要とする(Cho et al., 2013; Hsu et al., 2013)。細菌中のCRISPR配列は複数のRNAにおいて発現され、次いでRNAのためのガイド鎖を作るためにプロセッシングされる(Bikard et al., 2013)。真核生物系はCRISPR RNAをプロセッシングするために必要なタンパク質のいくつかを欠いているため、RNAポリメラーゼIII型プロモーターU6で発現される単一のRNAにCas9標的化に不可欠なRNA断片を組み合わせるために合成構築体gRNAを作った(Mali et al., 2013a, b)。合成gRNAは、最小長で100 bpをわずかに上回り、PAM配列NGGの直前の20個のプロトスペーサーヌクレオチドを標的とする部分を含む; gRNAはPAM配列を含まない。
上記のように、ある種の態様において、その後の精製および細胞/対象への送達のため、または遺伝子に基づく送達アプローチにおいて直接使用するため、転写因子産物を発現させるように発現カセットが利用される。発現は、適切なシグナルがベクター内に提供され、細胞における関心対象の遺伝子の発現をもたらす、ウイルスおよび哺乳動物両方の起源からのエンハンサー/プロモーターなどのさまざまな調節要素を含むことを必要とする。宿主細胞におけるメッセンジャーRNAの安定性および翻訳可能性を最適化するようにデザインされた要素も定義される。産物を発現する永久安定細胞クローンを樹立するためのいくつかのドミナント薬物選択マーカーの使用のための条件も提供され、薬物選択マーカーの発現をポリペプチドの発現と関連付ける要素も提供される。
本願の全体にわたって、「発現カセット」という用語は、核酸コード配列の一部または全部が転写され翻訳されうる、すなわち、プロモーターの制御下にある、遺伝子産物をコードする核酸を含有している任意のタイプの遺伝子構築体を含むものとする。「プロモーター」とは、遺伝子の特異的な転写を開始させるために必要とされる、細胞の合成機構により認識されるか、または合成機構へ導入されるDNA配列をいう。「転写制御下」という語句は、プロモーターが、RNAポリメラーゼ開始および遺伝子の発現を制御するため、核酸に対して正確な位置および方向にあることを意味する。「発現ベクター」とは、複製能を有する遺伝子構築体に含まれており、したがって、複製起点、転写終結シグナル、ポリA領域、選択可能マーカー、および多目的クローニング部位のうちの1つまたは複数を含む発現カセットを含むものとする。
本発明者は、昆虫ウイルスであるThosea asigna由来の2A様自己切断ドメイン(TaV 2Aペプチド)を利用する(Chang et al., 2009)。これらの2A様ドメインは、真核生物全体で機能することが示されており、アミノ酸の切断を、2A様ペプチドドメイン内で同時翻訳的に起こさせる。それゆえ、TaV 2Aペプチドを含めることで、単一のmRNA転写産物からの複数のタンパク質の発現が可能とされる。重要なことに、真核生物系で試験した場合、TaVのドメインは、99%を超える切断活性を示した(Donnelly et al., 2001)。
発現ベクターを細胞へ導入できるいくつかの方法が存在する。本発明のある種の態様において、発現構築体は、ウイルス、またはウイルスゲノムに由来する操作された構築体を含む。ある種のウイルスは、受容体により媒介されるエンドサイトーシスを介して細胞に侵入することができ、宿主細胞ゲノムへ組み込まれ、安定的かつ効率的にウイルス遺伝子を発現することができるため、哺乳動物細胞へ外来遺伝子を移入するための魅力的な候補である(Ridgeway, 1988; Nicolas and Rubenstein, 1988; Baichwal and Sugden, 1986; Temin, 1986)。遺伝子ベクターとして用いられた最初のウイルスは、パポーバウイルス(サルウイルス40、ウシパピローマウイルス、およびポリオーマ) (Ridgeway, 1988; Baichwal and Sugden, 1986)ならびにアデノウイルス(Ridgeway, 1988; Baichwal and Sugden, 1986)を含むDNAウイルスであった。これらは、比較的低い外来DNA配列の収容量を有し、限定された宿主スペクトルを有する。さらに、許容細胞における発がんの可能性および細胞変性効果のため、安全性に問題がある。それらは、8 kBまでの外来遺伝物質しか収容しえないが、種々の細胞株および実験動物へ容易に導入されうる(Nicolas and Rubenstein, 1988; Temin, 1986)。
臨床適用が企図される場合、薬学的組成物は、意図された適用のために適切な形態で調製されると考えられる。一般に、これは、発熱性物質およびヒトまたは動物にとって有害でありうるその他の不純物を本質的に含まない組成物を調製することを要すると考えられる。
以下の実施例は、本開示の好ましい態様を実証するために含まれる。当業者は、以下の実施例において開示される技法が、本開示の実践において良好に機能すると本発明者によって発見された技法を表しており、したがってその実践のために好ましい様式を構成すると考えられうることを理解するはずである。しかしながら、当業者は、本開示に照らして、本開示の趣旨および範囲から逸脱することなく、開示されている具体的な態様において多くの変更を行うことができ、それでも同様のまたは類似の結果を得ることができるものと理解するはずである。
プラスミド
ヒトコドン最適化Cas9遺伝子を含んだhCas9プラスミド(Addgeneプラスミド41815)およびsgRNAの骨格を含んだgRNAクローニングベクタープラスミド(Addgeneプラスミド41824)を、Addgeneから購入した。sgRNAのクローニングは、Church Lab CRISPRプラスミド取扱説明書(addgene.org/crispr/church/のワールドワイドウェブ)にしたがって行った。
T3プロモーター配列をPCRによってhCas9コード領域に付加した。T3-hCas9 PCR産物をゲル精製し、製造元の取扱説明書にしたがってpCRII-TOPOベクター(Invitrogen)にサブクローニングした。直線化したT3-hCas9プラスミドを、mMESSAGE mMACHINE T3転写キット(Life Technologies)を用いたインビトロ転写の鋳型として用いた。T7プロモーター配列をPCRによってsgRNA鋳型に付加した。ゲル精製したPCR産物を、MEGAshortscript T7キット(Life Technologies)を用いたインビトロ転写の鋳型として用いた。hCas9 RNAおよびsgRNAをMEGAclearキット(Life Technologies)によって精製し、ヌクレアーゼを含まない水(Ambion)で溶出した。RNAの濃度は、NanoDrop機器(Thermo Scientific)によって測定した。
ssODNをHDR鋳型として用い、Integrated DNA TechnologiesからUltramer DNA Oligonucleotideとして購入した。ssODNを精製せずに直接Cas9 mRNAおよびsgRNAと混合した。ssODNの配列を表S1に記載する。
全ての動物手順は、テキサス大学サウスウエスタン医療センター(University of Texas Southwestern Medical Center)の動物管理使用委員会(Institutional Animal Care and Use Committee)によって承認された。B6C3F1 (C57BL/6NCr雌性×C3H/HeN MTV雄性)、C57BL/6NCrおよびC57BL/10ScSn-Dmdmdx/Jは、卵母細胞ドナーとして用いられた3つのマウス系統であった。過剰排卵させた雌性B6C3F1マウス(6週齢)をB6C3F1種付け雄と交配させた。過剰排卵させた雌性C57BL/6NCr雌(12〜18グラム)をC57BL/6NCr雄と交配させ、過剰排卵させた雌性ホモ接合体C57BL/10ScSn-Dmdmdx/J (12〜18グラム)をヘミ接合体C57BL/10ScSn-Dmdmdx/J種付け雄と交配させた。受精卵を収集し、M16培地(100 U/mlのペニシリンおよび50 mg/mlのストレプトマイシンを含む卵子培養のためのブリスター培地)中37℃で1時間維持した。受精卵をM2培地(M16培地および20 mM HEPES)に移し、hCas9 mRNA、sgRNAおよびssODNを注入した。Cas9/sgRNAを前核のみ(Nucと呼ぶ)または前核および細胞質(Nuc+Cytと呼ぶ)に注入した。異なる用量のCas9 mRNA、sgRNAおよびssODNをNucまたはNuc+Cytで受精卵に注入した(表S2に詳述の通り)。注入済の受精卵をM16培地中37℃で1時間培養し、次いで偽妊娠ICR雌性マウスの卵管に移入した。
尾生検を25 mM NaOH/0.2 mM EDTA溶液100 μlに添加し、95℃に15分間置いた後、室温に冷却した。40 mM Tris-HCl (pH 5.5) 100 μlの添加後、チューブを15,000×gで5分間遠心分離した。DNAサンプルは数週間4℃で保管、または長期間貯蔵の場合-20℃で保管した。TRIzol (Life Technologies)を用い製造元の取扱説明書にしたがって筋肉からゲノムDNAを単離した。
PCRアッセイにはGoTaq (Promega) 2 μl、5×Green GoTaq反応緩衝液20 μl、25 mM MgCl2 8 μl、10 μMプライマー(DMD729FおよびDMD729R) (表S1) 2 μl、10 mM dNTP 2 μl、ゲノムDNA 4 μl、およびddH2O 〜100 μlを含めた。PCR条件は、2分間94℃; 32×(15秒間94℃、30秒間59℃、1分間72℃); 7分間72℃; その後4℃であった。直接配列決定のためPCR産物を2%アガロースゲル電気泳動によって分析し、QIAquick PCR Purification Kit (Qiagen)を用いてゲルから精製した。これらのPCR産物を、製造元の取扱説明書にしたがってpCRII-TOPOベクター(Invitrogen)にサブクローニングした。個々のクローンを選び、DNAを配列決定した。
ゲノムPCR産物20 μl、10×NEB緩衝液CS 3 μlおよびTseI (New England BioLabs) 1 μlからなる消化反応物を65℃で1時間インキュベートし、2%アガロースゲル電気泳動によって分析した。野生型DNAからの消化PCR産物は581 bpであり、その一方でF0マウスからのHDR媒介ゲノム編集DNAは約437 bpのさらなる産物を示す。
ミスマッチ二重鎖DNAは、以下の条件を用いてゲノムPCRサンプル25 μlの変性/再生により得た: 10分間95℃、95℃から85℃(-2.0℃/秒)、1分間85℃、85℃から75℃(-0.3℃/秒)、1分間75℃、75℃から65℃(-0.3℃/秒)、1分間65℃、65℃から55℃(-0.3℃/秒)、1分間55℃、55℃から45℃ (-0.3℃/秒)、1分間45℃、45℃から35℃ (-0.3℃/秒)、1分間35℃、35℃から25℃ (-0.3℃/秒)、1分間25℃、4℃に保持。変性/再生後、以下のものをサンプルに添加した: 10×NEB緩衝液2 3 μl、T7E1 (New England BioLabs) 0.3 μlおよびddH2O 〜30 μl。消化反応液を37℃で1時間インキュベートした。消化されていないPCRサンプルおよびT7E1消化PCR産物を、2%アガロースゲル電気泳動によって分析した。消化されていないPCR産物は729 bpであり、その一方でミスマッチDNAを有するF0マウスからのゲノムDNAは約440 bpおよび290 bpの2つのさらなる消化産物を示した。
筋力は、UT Southwestern Medical CenterのNeuro-Models Core Facilityによって実施された握力行動課題によって評価された。ケージからマウスを取り出し、体重を測定し、尾部を持ち上げて、前腕が握力計(Columbus Instruments)に接続されたプルバーアセンブリをつかむようにした。把持できなくなるまでマウスをセンサから遠ざけるように真っすぐ引っ張り、力のピーク量をグラム単位で記録した。これを5回繰り返した。
血液を顎下静脈から採取し、血清CKレベルをVITROS Chemistry Products CK Slidesにより測定して、VITROS 250 Chemistry Systemを用いCK活性を定量的に測定した。
野生型マウス、mdxマウス、および修正されたmdx-Cマウス由来の骨格筋を個別に解剖し、ガムトラガカント粉(Sigma-Aldrich)と組織凍結培地(TFM) (Triangle Bioscience)の1:2体積混合物中で凍結包埋した。心臓をTFM中で凍結包埋した。-155℃に過冷却されたイソペンタン熱抽出剤中で全ての包埋物を急速凍結した。得られたブロックを-80℃で終夜貯蔵した後に、切片化した。骨格筋の8ミクロンの横断切片、および心臓の前面切片をLeica CM3050クリオスタット上で調製し、同日染色前に風乾した。確立された染色プロトコルにしたがってH&E染色を行い、MANDYS8モノクローナル抗血清(Sigma-Aldrich)を用い製造元の取扱説明書に改変を加えてジストロフィン免疫組織化学を行った。簡単に説明すると、クリオスタット切片を解凍し、1%トライトン/リン酸緩衝生理食塩水, pH 7.4 (PBS)中で再水和/脱脂した。脱脂後、切片を洗浄してトライトンを除去し、マウスIgGブロッキング試薬(M.O.M. Kit, Vector Laboratories)とともにインキュベートし、洗浄し、連続してMOMタンパク質濃縮物/PBSで平衡化し、MANDYS8をMOMタンパク質濃縮物/PBS中で1800分の1に希釈した。4℃で終夜の一次抗体インキュベーションの後、切片を洗浄し、MOMビオチン化抗マウスIgGとともにインキュベートし、洗浄し、Vectorフルオレセイン-アビジンDCSのインキュベーションで検出を完了した。Vectashieldでのカバースリッピングの前に、ヨウ化プロピジウム(Molecular Probes)で核を対比染色した。
標本は、落射蛍光照射、CRIカラーホイール、およびZeiss Axiocam単色CCDカメラを備えたZeiss Axioplan 2iE直立顕微鏡写真機で再調査した。OpenLab 4.0取得および制御ソフトウェア(Perkin-Elmer)を用いて4倍、10倍および20倍の対物倍率視野をとらえ、それをさらに用いて、インデックス付きの擬似カラー表示および重ね合わせ画像オーバーレイを適用した。画像をAdobe Photoshop CS2でピークレベル調整し、画像解析のために保存した。ImageJ 1.47を用いて立体的形態計測ランダム化グリッドオーバーレイを適用し、このソフトウェアの計数機能を用いて、各筋肉群からのジストロフィン陽性および陰性免疫染色につき約500の凝集筋線維をマークかつスコア化した(最小3区間切片から)。各遺伝子型のヒラメ筋のH&E染色切片を、サイズおよび特徴についてImageJ 1.47でさらに分析した。簡単に説明すると、115+立体的ランダム化筋線維の筋線維鞘境界を手作業で描写し、それらの断面積を計算し、中心核表現型を記録した。
筋肉を解剖し、液体窒素中で急速凍結した。タンパク質抽出およびウエスタンブロット分析を、記述(Nicholson et al., 1989 and Kodippili et al., 2014)のように改変を加えて行った。サンプルを、10% SDS, 62.5 mM Tris, 1 mM EDTAおよびプロテアーゼ阻害剤(Roche)を含有するサンプル緩衝液400 μL中で2×20秒間ホモジナイザー(POLYTRON System PT 1200 E)によりホモジナイズした。BCA Protein Assay Kit (Pierce)を用いてタンパク質濃度を測定した。各筋肉サンプル由来のタンパク質50マイクログラムを勾配SDS-PAGE (Bio-Rad)にロードした。ゲルを2.5時間100 Vで泳動した。分離されたタンパク質をコールドルーム(4℃)中で終夜35 VにてPVDF膜に転写した。PVDF膜を2%ポンソーレッド(Ponceau Red)により全タンパク質について染色し、その後、穏やかに振盪しながら25℃で5% w/v脱脂粉乳、1×TBS、0.1% Tween-20 (TBST)により1時間ブロッキングした。ブロッキングされた膜をマウス抗ジストロフィンモノクローナル抗体(MANDYS8, Sigma-Aldrich, 5%ミルク/TBST中で1,000分の1希釈)とともに4℃で終夜インキュベートし、その後、TBST中で洗浄した。次いで、ブロットを西洋ワサビペルオキシダーゼ結合ヤギ抗マウスIgG二次抗体(Bio-Rad, 10,000分の1希釈)とともに25℃で1時間インキュベートした。TBSTで洗浄した後に、ブロットをウエスタンブロッティングルミノール試薬(Santa Cruz Biotech)に1分間曝露してシグナルを検出した。タンパク質ロードを、抗GAPDH抗体(Millipore, 10,000分の1希釈)によってモニターした。
(A) mdx (B) mdx+Cas9 (C) WTおよび(D) WT+Cas9に関して、表S1に記載されているプライマーを用いてPCRによりオフターゲット遺伝子座を増幅させた。PCR産物をMinElute PCR精製キット(QIAGEN)により精製し、同じ濃度(10 ng/μL)に調整し、群ごとに等量(5 μL)を合わせた。ライブラリ調製を製造元の取扱説明書(標準的PCRライブラリ増幅モジュールを有するKAPA Library Preparation Kit, Kapa Biosystems)にしたがい実施した。配列決定をIlluminaのHiseq 2500で実施し、Rapid Mode 150PE chemistryを用いて行った。配列決定の読み取りをBWA (bio-bwa.sourceforge.net/)によりマッピングした。変種発見のため、マッピング品質を満たす30超の読み取りを保持した。全領域および全サンプルにわたる平均の読み取り深度は2570倍であった。SAMtools (samtools.sourceforge.net/)に加えてカスタムスクリプトを用いて変種を呼び出した。各領域において、3塩基対またはそれ以上の挿入および欠失を、Cas9潜在的切断部位を中心とする50 bpのウインドウにおいて計数した。
腓腹筋の凍結包埋物からの凍結切片を、Pax-7免疫組織化学の同日セットアップのためポリエチレン膜フレームスライド(Leica Microsystems PET-Foil 11505151)上にマウントした。PET-箔膜フレームスライドで作業する場合には(Gjerdrum et al., 2001)、記述(Murphy et al., 2011)に抗原回復のための改変を加えて、モノクローナルPax-7抗体(Developmental Studies Hybridoma Bank)を用いた。簡単に説明すると、腓腹筋凍結切片を風乾し、4%パラホルムアルデヒドで固定し、トライトン-X100で脱脂し、抗原回復緩衝液(クエン酸ナトリウム緩衝液Ph 6.0)中65℃で20時間インキュベートした。抗原回復後、切片をクエンチし、0.6%過酸化水素によって内因性ペルオキシダーゼを除去し、マウスIgGブロッキング試薬(M.O.M. Kit, Vector Laboratories)とともにインキュベートし、PBSで洗浄し、MOMタンパク質濃縮物/PBSとともにインキュベートし、その後に4℃でMOMタンパク質濃縮物/PBS中Pax-7抗体(2 μg/ml)との終夜インキュベーションを行った。切片をPBSで洗浄し、MOMビオチン化抗マウスIgG、ストレプトアビジン-ペルオキシダーゼ(Vector Laboratories)とともにインキュベートし、ジアミノベンジジン色素 (DAB, Dako)で発色させた。ヌクレアーゼを含まないMayerのヘマトキシリンで核を対比染色した。Pax-7陽性衛星細胞を顕微鏡で同定し、Leica AS-LMDを用いて63倍の対物倍率でレーザーマイクロダイセクションにより単離した。遺伝子型ごとに60〜70個のPax-7陽性衛星細胞を単離し、捕捉緩衝液(DirectPCR Lysis Reagent, Viagen Biotech Inc.) 10 μlにプールし、-20℃で保管した。標的ゲノム領域を、上記のように、プライマーDMD232_fおよびDMD232_r (表S1)を用いPCRによって増幅した。
この研究の目的は、インビボでのCRISPR/Cas9媒介ゲノム編集によって、mdxマウスのDmd遺伝子における遺伝子欠損を修正することであった。mdxマウス(C57BL/10ScSn-Dmdmdx/J)は、Dmd遺伝子のエクソン23にナンセンス変異を含む(14, 15) (図1A)。本発明者らは、筋原性前駆細胞を含む身体の全ての細胞における変異を修正する可能性を有する戦略である、生殖細胞系列における疾患原因遺伝子変異修正のために、Cas9、sgRNAおよびHDR鋳型をマウス受精卵に注入した(16, 17)。CRISPR/Cas9に基づく遺伝子治療の安全性および有効性も評価した。
出生後組織におけるゲノム編集に対する課題は、筋線維および心筋細胞のような分裂後細胞においてHDRが起こらないことである。しかしながら、NHEJは起こるので、突然変異誘発の精度を必要とせずに変異を破壊するために用いることができる。エクソンスキッピングは、遺伝子の一部を「スキップ」し、部分的に機能するジストロフィンを生じることを可能にする戦略である(Aartsma-Rus, 2012)。しかしながら、伝統的なアンチセンスオリゴヌクレオチド(AON)を介する一過性エクソンスキッピングは、オリゴヌクレオチドの組織取り込みの非効率性、オリゴヌクレオチドの生涯送達の必要性、および不完全なエクソンスキッピングに悩まされる。この課題を回避するために、本発明者らは、CRISPR/Cas9システムを用いて、DMD変異より前のエクソンスプライス部位を破壊するか、または変異体もしくはフレーム外エクソンを欠失させ、それにより周辺エクソン間のスプライシングが、変異を欠くインフレームジストロフィンタンパク質を再作製することを可能にした。DMDに関与する遺伝子損傷を永続的に修正することにより、ゲノム編集は、心臓または骨格筋への編集成分の必要性が一回の送達のみで済む。さらに、経時的な筋機能の漸進的な改善は、ゲノム編集が行われてから長い間、筋機能の継続的な回復を可能にする。
mdxマウスにおいてエクソン23変異のMyo編集を試験し始めるために、本発明者らは最初にエクソン23の5'末端および3'末端を標的とするsgRNA (sgRNA-LおよびR)のプールを作製した(図14A)。NHEJ媒介indelは、保存されたRNAスプライス部位をなくすか、エクソン23を欠失させることができる。これらのガイドRNAをプラスミドspCas9-2A-GFP (Addgene #48138)にクローニングした。最初に、本発明者らは、マウス10T1/2細胞におけるガイドRNAの効率を評価した。Myo編集効率は、以前に記述のT7E1アッセイ法によって検出された。エクソン23のsgRNA-R3標的3'末端は高い活性を示した。次いでそれらに、Cas9ならびにガイドRNA mdxおよびR3をmdx受精卵へHDR鋳型なしで共注入した(図14B)。著しくは、9匹の子孫マウスのうち7匹が3'ドナー部位にindelを含むか、またはエクソン23全体を欠失した(図14C)。以前の研究では、mdx仔マウスの約8%しかHDR媒介修正を含んでいなかった。新しい方法を用いることで、効率が10倍に大幅に高められた。エクソン23の標的部位由来のPCR産物をクローニングし、配列決定した。結果から、Myo編集がNHEJ媒介indel変異を効率的に作製して、ジストロフィン遺伝子の読み取り枠を救済しうることが示された。
AAVは、ヒト骨格筋および心臓への正確なMyo編集のためのCas9タンパク質およびガイドRNAの安全な送達に最も有望かつ適切なビヒクルの1つである。AAVによるCRISPR/Cas9精密Myo編集療法の送達の問題は、ウイルスの送達とゲノム工学のプロセスの両方の効率を最適化することに密接に関連していることを強調することが重要である。本発明者らは、インビボで筋細胞にCRISPR/Cas9ゲノム編集機構を送達するための最高力価のAAV9調製物の開発に焦点を当てた。これは集中的な国際研究領域である(Senis et al., 2014; Schmidt & Grimm, 2015)。
長期目標は、Myo編集を生後心筋および骨格筋細胞に適用し、このアプローチを活用してヒトのDMD変異を修正することである。本発明者らは今回、DMD患者由来iPSCのゲノムにおける変異体エクソンのスキッピングを操作することにより、マウスからヒトDMD患者由来の細胞へMyo編集を進展させた。患者におけるDMD変異は、遺伝子の特定の領域に固まって存在している(「ホットスポット」変異) (図22)。本発明者らは、潜在的にDMD患者の80%に適用可能な上位12のホットスポット変異エクソンを標的とするために、sgRNAのプールを用いた「ホットスポット」DMD変異のMyo編集を最適化した。本発明者らは、各エクソンの5'または3'末端を標的とするために3〜6個のPAM配列を選択した(表2)。Myo編集媒介indelは、保存されたRNAスプライスアクセプタ/ドナー部位をなくし、フレーム外エクソンを救済する。既知のDMD変異に基づいて、本発明者らは、大部分のDMD患者においてジストロフィン機能を救済するMyo編集のための最適な標的DMD配列を選択するためのオンライン情報源(Duchenne Skipper Database)を確立している。
これらの結果は、CRISPR/Cas9媒介ゲノム編集が、mdxマウスにおいて筋ジストロフィー(DMD)に関与する原発性遺伝子病変を修正し、この疾患に特有の特徴の発達を予防しうることを示す。生殖系列におけるゲノム編集によって、広範囲のモザイク現象(2〜100%)を有する遺伝的に修正された動物が生み出されたので、本発明者らは、正常な筋肉構造および機能の救済の程度とゲノム修正率を比較することができた。本発明者らは、インビボで修正された細胞のサブセットのみが完全な表現型の救済に十分であることを観察した。図3Cに図式化されるように、部分的に修正されたmdxマウスの組織学的分析により、3タイプの筋線維が明らかになった: 1) 正常ジストロフィン陽性筋線維; 2) ジストロフィー性ジストロフィン陰性筋線維; および3) 筋再生を示す中心核を含んだモザイクジストロフィン陽性筋線維。本発明者らは、後者のタイプの筋線維が、損傷した筋線維への修正された衛星細胞の動員から生じ、中心核を有するモザイク筋線維を形成することを提唱する。インビボ生着のための細胞源としてエクスビボで衛星細胞を増殖させる努力は、培養下のこれらの細胞の増殖能および再生能の喪失によって妨げられている(Montarras et al., 2005)。したがって、CRISPR/Cas9システムによるインビボでの衛星細胞の直接編集は、DMDにおける筋肉修復を促進する潜在的に有望な代替アプローチとなる。
Claims (11)
- Cas9をコードする第一の核酸および単一のDMDガイドRNA(gRNA)をコードする第二の核酸を含む、対象におけるDMD遺伝子欠損をエクソンスキッピングによって修正するための薬学的組成物であって、
該Cas9および該単一のDMDガイドRNA(gRNA)が、一度発現すると、Cas9-gRNA複合体を形成してDMD遺伝子のエクソン51の5’スプライスアクセプタ部位を破壊するように、該薬学的組成物が該対象における細胞に接触し、
該単一のDMD gRNAは、スペーサーRNAおよびtracrRNAを含み、かつDMD遺伝子のエクソン51の5’スプライスアクセプタ部位を標的とし、
該スペーサーRNAは20ヌクレオチドからなり、
該スプライスアクセプタ部位はSEQ ID NO: 28の配列を含み、かつ
該第一および第二の核酸は、1つまたは複数のアデノ随伴ウイルス(AAV)ベクターに含まれ、かつ該対象における細胞に送達され、それにより、エクソンスキッピングによってDMD遺伝子発現の欠失を救済する、

- 単一のDMDガイドRNAが、エクソン51中のプロトスペーサー隣接モチーフの隣のゲノム遺伝子座を標的とする、請求項1記載の薬学的組成物。
