デュシェンヌ型筋ジストロフィー(DMD)は、筋細胞膜の完全性に不可欠な大きい細胞骨格タンパク質であるジストロフィンをコードする遺伝子の変異によって引き起こされるX連鎖劣性疾患である。DMDは、進行性の筋衰弱を引き起こし、30歳までに若年死(一般的には心筋症から)に至る。この疾患には有効な処置法がない。組換えアデノ随伴ウイルス(rAAV)による短縮型ジストロフィンまたはユートロフィンの送達ならびにアンチセンスオリゴヌクレオチドおよび低分子を用いた変異体エクソンのスキッピングを含め、DMDにおけるジストロフィン発現を回復させるための数多くのアプローチが試みられている。しかしながら、これらのアプローチは、DMD変異を修正することも、ジストロフィン発現を永続的に回復させることもできない。したがって、DMD変異を修正してこの疾患の根本原因に対処し、それによってジストロフィン発現を永続的に回復させる、DMDを処置するための組成物および方法が当技術分野において必要とされている。

デュシェンヌ型筋ジストロフィーは、遺伝的原因による他の多くの疾患と同様に、困難な治療シナリオを提示する。CRISPR-Casシステムは、多様な遺伝的欠陥の修正のためのアプローチを示す。CRISPR(clustered regularly interspaced short palindromic repeat、クラスター化して規則的な配置の短い回文配列反復)システムは、ファージ感染を防御する細菌および古細菌の適応免疫系として機能する。このシステムでは、単一ガイドRNA(sgRNA)によってエンドヌクレアーゼが特異的なゲノム配列にガイドされ、プロトスペーサー隣接モチーフ(PAM)配列の近くのDNA切断をもたらす。以前は、CRISPR-Cas9を使用して、マウスおよびヒト細胞においてDMD変異を修正していた。しかしながら、多くの課題が対処されずに残っている。例えば、これまで最も広く使用されてきたCas9エンドヌクレアーゼである化膿レンサ球菌(Streptococcus pyogenes)のCas9(SpCas9)は、ATリッチ領域のゲノム編集を許容しないGリッチPAM要件(NGG)を有する。加えて、SpCas9の大きなサイズは、アデノ随伴ウイルス(AAV)ベクターなどの小容量ウイルスベクターへのパッケージングおよび送達の効率を低減する。黄色ブドウ球菌(Staphylococcus aureus)由来のCas9エンドヌクレアーゼ(SaCas9)は、SpCas9よりもサイズは小さいが、より長くかつより複雑なPAM配列(NNGRRT)を有し、したがって、そのゲノム標的の範囲を制限している(Ran et al., 2015)。認識配列の柔軟性がより大きくかつ切断効率が同程度のより小さいCRISPR酵素が、とりわけ翻訳用途に対して、正確な遺伝子編集を容易にするであろう。
A. 背景
デュシェンヌ型筋ジストロフィー(DMD)は、男児のおよそ3,500人に1人が罹患する、筋肉変性および早期死亡をもたらす劣性X連鎖型の筋ジストロフィーである。この障害は、以下に再現する配列のタンパク質ジストロフィン(GenBankアクセッション番号AAA53189; SEQ ID NO.383)をコードする、ヒトX染色体上に位置する遺伝子ジストロフィン(GenBankアクセッション番号NC_000023.11を参照のこと)の変異によって引き起こされる。
症状は、通常、2〜3歳の男児に現れ、早期幼児期に見られることもある。早期幼児期まで症状が現れなくても、検査室試験により、出生時に活性変異を保有する子供を特定することができる。筋肉量の損失に関連する脚および骨盤の進行性の近位筋衰弱が最初に観察される。最終的に、この衰弱は、腕、首、および他の領域に広がる。初期の徴候は、仮性肥大(ふくらはぎおよび三角筋の腫脹)、低持久力、および、助けなしで起立することが困難、または階段を上ることができないことを含み得る。病状が進行するにつれて、筋組織は、消耗を経験し、最終的には脂肪および線維性組織に置き換えられる(線維症)。10歳までに歩行を補助するために支持具が必要となることがあり、ほとんどの患者は、12歳までに車椅子に頼る。その後の症状は、脊椎湾曲を含め、骨格の変形を導く異常な骨発達を含み得る。進行性の筋肉劣化に起因して、運動の消失が起こり、最終的に麻痺に至る。知的障害が存在する可能性も、存在しない可能性もあるが、存在しても、子供が年を取るにつれて次第に悪化することはない。DMDに罹患している男性の平均余命は25歳前後である。
・ぎこちない歩き方、足踏み方、または走り方(患者は、ふくらはぎの筋肉緊張の増大のため、足の前部で歩く傾向がある。また、つま先歩きは、膝伸筋の衰弱に対する代償的適応である。)
・頻繁な転倒
・疲労
・運動技能(ランニング、ホッピング、ジャンピング)の障害
・股関節屈筋の短縮を導くおそれのある、腰椎の脊柱前彎過度。これは、姿勢全体、および歩き方、足踏み方、または走り方に影響を及ぼす。
・アキレス腱および膝腱の筋拘縮は、結合組織において筋線維が短くなり線維形成を起こすので、機能を損なう。
・進行性の歩行困難
・筋線維の変形
・舌およびふくらはぎの筋肉の仮性肥大(腫脹)。筋組織は最終的に脂肪および結合組織により置き換えられるため、仮性肥大という語を用いる。
・脳内のジストロフィンの非存在または機能不全の結果であると考えられる、神経行動障害(例えば、ADHD)、学習障害(失読症)、および特定の認知技能(特に短期言語記憶)の非進行性衰弱の高いリスク
・歩く能力の最終的損失(通常は12歳までに)
・骨格の変形(場合によっては脊柱側彎を含む)

デュシェンヌ型筋ジストロフィー(DMD)は、X染色体の短腕に位置する遺伝子座Xp21のジストロフィン遺伝子の変異によって引き起こされる。ジストロフィンは、多くのサブユニットを含有するタンパク質複合体を通じて、各筋線維の細胞骨格を、基礎となる基底層(細胞外マトリックス)に接続する役割を担う。ジストロフィンが存在しないと、過剰なカルシウムが筋線維鞘(細胞膜)に浸透する。カルシウムおよびシグナル伝達経路の変化により水がミトコンドリアに侵入し、次いでミトコンドリアが破裂する。
この障害の家族歴を有する人には、遺伝カウンセリングが推奨される。デュシェンヌ型筋ジストロフィーは、妊娠中に行われる遺伝子検査によって約95%の精度で検出できる。
ジストロフィン遺伝子の筋肉特異的アイソフォームは、79のエクソンから構成され、通常、DNA検査および分析で、影響を受けるエクソンの特定のタイプの変異を同定することができる。ほとんどの場合に、DNA検査が診断を確定する。
DNA検査で変異を見つけられない場合は、筋生検検査が行われることがある。小さな筋組織サンプルを抽出し(通常は針の代わりにメスで)、ジストロフィンの存在を明らかにする色素を適用する。そのタンパク質の完全な非存在は、その病状を示唆する。
DMDは、X連鎖劣性遺伝子によって運ばれる。男性はX染色体を1つしか持たないので、変異した遺伝子の1コピーがDMDを引き起こす。父親はX連鎖形質を息子に譲り渡すことができないので、変異は母親によって伝達される。
DMDに対する現行の治癒法はなく、継続的な医学的必要性が規制当局によって認識されている。ある特定の変異に対するエクソンスキッピング処置を用いた第1〜2a相試験は、歩行の減退を中断し、わずかな臨床的改善をもたらした。処置は、一般に、生活の質を最大限にするために症状の発症を制御することを目的とし、以下を含む:
・プレドニゾロンおよびデフラザコートなどのコルチコステロイドは、エネルギーおよび強度を高め、いくつかの症状の重症化を遅らせる。
・ランダム化比較試験は、ベータ-2-アゴニストが筋力を増加させるが、疾患の進行を改変しないことを示している。ベータ2-アゴニストでのほとんどのRCTの経過観察時間は、わずか12ヶ月前後であり、よってその時間枠を超えて結果を推定することはできない。
・水泳などの、軽度な不快でない身体活動が推奨される。不活動であること(ベッドでの静養など)は、筋肉疾患を悪化させ得る。
・理学療法は、筋力、柔軟性、および機能を維持するのに役立つ。
・矯正装具(支持具および車椅子など)は、移動性および自己ケア能力を改善し得る。睡眠中に足首を適切な位置で支える、体にぴったり合った取り外し可能な脚支持具は、拘縮の発症を遅らせることができる。
・疾患の進行に応じて適切な呼吸補助が重要である。
理学療法士は、患者が身体的潜在性を最大限に発揮できるようにすることに携わる。彼らの目的は、以下を行うことである:
・必要に応じてストレッチおよび運動のプログラムを開発することによって拘縮および変形の発症を最小限に抑えること
・固定具および耐久性のある医療機器を推薦することによって、身体的な性質の他の二次的合併症を予測し、最小限に抑えること
・呼吸機能を監視し、呼吸運動を補助するための技法および分泌物を取り除く方法について助言すること。
呼吸のたびに調節可能な体積(量)の空気を人に送達する最新の「従量式酸素吸入器/人工呼吸器」が、筋ジストロフィーに関連した呼吸障害を有する人の処置において有益である。酸素吸入器は、空気を直接送達する侵襲性の気管内チューブまたは気管切開チューブを必要とすることもあるが、一部の人には、フェイスマスクまたはマウスピースによる非侵襲性の送達で十分である。この後者の方法においては、気道陽圧装置、特に二相性の気道陽圧装置が使用されることもある。呼吸具は、携行性のために外部バッテリを備えた電動車椅子の底面または背面の酸素吸入器トレイに容易に適合し得る。
デュシェンヌ型筋ジストロフィーは、最終的に全ての随意筋に影響を及ぼしかつ後期に心筋および呼吸筋を侵す、進行性疾患である。平均余命は、現在25歳前後と推定されているが、これは患者によって異なる。最近の医学の進歩は、罹患者の生存を延長させている。全ての筋疾患に焦点を当てた有力な英国慈善団体であるThe Muscular Dystrophy Campaignは、「高い水準の医学的ケアにより、デュシェンヌ型筋ジストロフィーを有する若年男性は、30代を優に超えるまで生きることが多い」と述べている。
A. CRISPR
CRISPR(クラスター化して規則的な配置の短い回文配列反復)は、塩基配列の短い反復を含有するDNA遺伝子座である。各反復の後に、ウイルスへの事前曝露由来の「スペーサーDNA」の短いセグメントが続く。CRISPRは、配列決定された真正細菌ゲノムの約40%および配列決定された古細菌の90%に見いだされる。CRISPRは、CRISPRに関連するタンパク質をコードするcas遺伝子と関連していることが多い。CRISPR/Casシステムは、プラスミドおよびファージなどの外来遺伝エレメントに対する耐性を付与し、かつ獲得免疫の一形態を提供する、原核生物の免疫系である。CRISPRスペーサーは、真核生物におけるRNAiのように、これらの外因性遺伝エレメントを認識およびサイレンシングする。
CRISPR関連(cas)遺伝子は、CRISPR反復-スペーサーアレイに関連していることが多い。2013年現在、40種を超える異なるCasタンパク質ファミリーが記述されている。これらのタンパク質ファミリーのうち、Cas1は、異なるCRISPR/Casシステムの間で遍在しているようである。8つのCRISPRサブタイプ(Ecoli、Ypest、Nmeni、Dvulg、Tneap、Hmari、Apern、およびMtube)を定義するためにcas遺伝子および反復構造の特定の組み合わせが使用されており、その一部は、RAMP(repeat-associated mysterious protein)をコードする追加の遺伝子モジュールと関連している。単一のゲノム内に2つ以上のCRISPRサブタイプが存在し得る。CRISPR/Casサブタイプの散在的分布は、システムが微生物の進化中に遺伝子水平伝播を受けることを示唆している。
プレボテラおよびフランシセラ1由来のクラスター化して規則的な配置の短い回文配列反復、すなわちCRISPR/Cpf1は、CRISPR/Cas9システムといくつかの類似性を有するDNA編集技術である。Cpf1は、クラスII CRISPR/CasシステムのRNAガイドエンドヌクレアーゼである。この獲得免疫機構は、プレボテラ細菌およびフランシセラ細菌に見いだされる。それは、ウイルスによる遺伝子損傷を防止する。Cpf1遺伝子は、CRISPR遺伝子座に関連しており、ガイドRNAを使用してウイルスDNAを発見および切断するエンドヌクレアーゼをコードする。Cpf1は、Cas9よりも小さくかつ単純なエンドヌクレアーゼであり、CRISPR/Cas9システムの制限の一部を克服する。

crRNAの形成:Casタンパク質をガイドする成熟crRNAを産生する、プレ-cr-RNAのプロセシング;および
干渉:Cpf1がcrRNAに結合して、二成分複合体を形成し、標的DNA配列を同定および切断する。
RNAにガイドされるタンパク質として、Cas9は、DNA標的の認識を指令する短いRNAを必要とする。Cas9は、PAM配列NGGを含有するDNA配列を選択的に調べるが、それはプロトスペーサー標的なしでここに結合することができる。しかしながら、Cas9-gRNA複合体は、二本鎖切断を生じるためにgRNAとの厳密な一致を必要とする。細菌におけるCRISPR配列は、複数のRNAで発現され、次いでRNAのガイド鎖を生じるためにプロセッシングされる。真核生物系は、CRISPR RNAをプロセッシングするために必要なタンパク質の一部を欠いているため、RNAポリメラーゼIII型プロモーターU6で発現される単一RNAにCas9ターゲティングに不可欠なRNA片を組み合わせるために合成構築物gRNAが作られた。合成gRNAは、最小長で100bpをわずかに上回り、PAM配列NGGの直前の20のプロトスペーサーヌクレオチドを標的とする部分を含む;gRNAは、PAM配列を含まない。
Cas9は、DNAを切断するために2つのRNA分子を必要とする一方で、Cpf1は1つ必要とする。これらタンパク質はまた、DNAを異なる場所で切断し、編集部位を選択する際に研究者により多くの選択肢を提供する。Cas9は、DNA分子において両方の鎖を同じ位置で切断し、「平滑」末端を残す。Cpf1は、一方の鎖を他方よりも長く残し、扱いやすい「付着」末端を生じる。Cpf1は、Cas9と比較して、切断部位に新たな配列をより多く挿入できるようである。CRISPR/Cas9システムは遺伝子を効率的に無効にすることができるが、遺伝子を挿入することもノックインを生成することも困難である。Cpf1は、tracrRNAを欠き、TリッチPAMを利用し、DNAを突出型のDNA DSBを介して切断する。
CRISPR/Cpf1を使用してDMD遺伝子を編集する際の最初の工程は、ゲノム標的配列を同定することである。本開示のgRNAのためのゲノム標的は、ジストロフィン遺伝子内の任意の約24ヌクレオチドのDNA配列であることができるが、但し、その配列がゲノムの残部と比較してユニークであるものとする。いくつかの態様において、ゲノム標的配列は、ヒトジストロフィン遺伝子のエクソン51、エクソン45、エクソン44、エクソン53、エクソン46、エクソン52、エクソン50、エクソン43、エクソン6、エクソン7、エクソン8、および/またはエクソン55中にある。いくつかの態様において、ゲノム標的配列は、ヒトジストロフィン遺伝子のエクソン51、エクソン45、エクソン44、エクソン53、エクソン46、エクソン52、エクソン50、エクソン43、エクソン6、エクソン7、エクソン8、および/またはエクソン55の5’または3’スプライス部位である。いくつかの態様において、ゲノム標的配列は、ヒトジストロフィン遺伝子のエクソン51、エクソン45、エクソン44、エクソン53、エクソン46、エクソン52、エクソン50、エクソン43、エクソン6、エクソン7、エクソン8、および/またはエクソン55のすぐ上流または下流にあるイントロンである。例示的なゲノム標的配列を表Dに見いだすことができる。
上で考察した通り、ある特定の態様において、その後の精製および細胞/対象への送達のため、または遺伝子に基づく送達アプローチで直接使用するため、発現カセットを用いて転写因子産物を発現させる。Cpf1およびジストロフィンスプライス部位を標的とする少なくとも1つのDMDガイドRNAをコードする1つまたは複数の核酸を含有する発現ベクターが本明細書に提供される。いくつかの態様において、Cpf1をコードする核酸および少なくとも1つのガイドRNAをコードする核酸は、同じベクター上に提供される。さらなる態様において、Cpf1をコードする核酸および少なくとも1つのガイドRNAをコードする核酸は、別々のベクター上に提供される。
本出願全体を通して、「発現カセット」という用語は、核酸コード配列の一部または全てが転写および翻訳されることができる、すなわち、プロモーターの制御下にある、遺伝子産物をコードする核酸を含有する任意のタイプの遺伝子構築物を含むことを意味する。「プロモーター」は、遺伝子の特異的な転写を開始するために必要とされる、細胞の合成機構または導入された合成機構によって認識されるDNA配列を指す。「転写制御下」という語句は、プロモーターが、RNAポリメラーゼ開始および遺伝子の発現を制御するため、核酸に対して正確な位置および方向にあることを意味する。「発現ベクター」は、複製可能な遺伝子構築物に含まれており、したがって、複製起点、転写終結シグナル、ポリA領域、選択可能マーカー、および多目的クローニング部位の1つまたは複数を含む、発現カセットを含むことを意味する。
本発明者は、昆虫ウイルスであるトセア・アシグナ(Thosea asigna)由来の2A様自己切断ドメイン(TaV 2Aペプチド; SEQ ID NO.444; EGRGSLLTCGDVEENPGP)を利用する。これらの2A様ドメインは、真核生物全体で機能することが示されており、アミノ酸の切断を、2A様ペプチドドメイン内で同時翻訳的に起こさせる。それ故、TaV 2Aペプチドを含めることで、単一のmRNA転写物から複数のタンパク質の発現が可能となる。重要なことに、真核生物系で試験したとき、TaVのドメインは、99%を超える切断活性を示した(Donnelly et al., 2001)。他の許容される2A様ペプチドは、ウマ鼻炎Aウイルス(ERAV)2Aペプチド

発現ベクターを細胞に導入できる多数の方法が存在する。ある特定の態様において、発現構築物は、ウイルス、またはウイルスゲノムに由来する改変された構築物を含む。受容体媒介性エンドサイトーシスを介して細胞に侵入し、宿主細胞ゲノムに組み込まれ、ウイルス遺伝子を安定的かつ効率的に発現することができるある特定のウイルスは、哺乳動物細胞に外来遺伝子を移入するための魅力的な候補である。これらは、比較的低い外来DNA配列の収容量を有し、限定された宿主範囲を有する。さらに、許容細胞における発がん潜在性および細胞変性効果は、安全性の問題を提起する。これらは、8kBまでの外来遺伝物質しか収容できないが、種々の細胞株および実験動物に容易に導入することができる。
臨床適用のために、薬学的組成物が、意図する適用に適した形態で調製される。一般に、これは、発熱性物質およびヒトまたは動物にとって有害であり得る他の不純物を本質的に含まない組成物を調製することを要する。
以下の表は、本明細書に開示の組成物および方法と一緒に使用するための例示的なプライマー配列およびgRNA配列を提供する。

pLbCpf1-2A-GFPプラスミドおよびpAsCpf1-2A-GFPプラスミドの作製
ヒトコドン最適化LbCpf1およびAsCpf1を、それぞれ、Feng Zhang(Addgene plasmid # 69988)からの贈与であるpY016プラスミド(Zetsche et al., 2015)(pcDNA3.1-hLbCpf1)およびFeng Zhang(Addgene plasmid # 69982)からの贈与であるpY010プラスミド(Zetsche et al., 2015)(pcDNA3.1-hAsCpf1)からPCR増幅させた。Cpf1 cDNAおよびT2A-GFP DNA断片を、Feng Zhang(Addgene plasmid # 48138)からの贈与である、AgeI/EcoRIで切断してSpCas9(BB)-2A-GFPを除去したpSpCas9(BB)-2A-GFP(PX458)プラスミド(Ran et al., 2015)の骨格にクローニングした。In-Fusion HDクローニングキット(Takara Bio)を使用した。ヒトDMDまたはマウスDmd遺伝子座を標的とするCpf1ガイドRNA(gRNA)を、新たに作製されたpLbCpf1-2A-GFPプラスミドおよびpAsCpf1-2A-GFPプラスミドに、BbsI消化およびT4ライゲーションを使用してサブクローニングした。詳細なプライマー配列を表Cに見いだすことができ、ゲノム標的配列を表Dに見いだすことができ、gRNA配列を表Eに見いだすことができる。
ヒトiPSCをmTeSR(商標)1培地(STEMCELL Technologies)中で培養して、およそ4日毎に継代した(継代比率1:18)。ヌクレオフェクションの1時間前に、iPSCを10μM ROCK阻害剤(Y-27632)で処理して、Accutase(Innovative Cell Technologies, Inc.)を使用して単一細胞に解離させた。1×106のiPSC細胞をpLbCpf1-2A-GFPプラスミドまたはpAsCpf1-2A-GFPプラスミド5μgと混合し、P3 Primary Cell 4D-Nucleofector X kit(Lonza)を製造業者のプロトコルに従って使用してヌクレオフェクトした。ヌクレオフェクション後、10μM ROCK阻害剤、ペニシリン-ストレプトマイシン(1:100)(ThermoFisher Scientific)および100μg/ml Primosin(InvivoGen)を補充したmTeSR(商標)1培地中でiPSCを培養した。ヌクレオフェクションの3日後、GFP(+)細胞およびGFP(-)細胞をFACSによって選別して、T7E1アッセイに供した。GFP(+)から単一クローンを得た。iPSCを採取して配列決定した。過去に記載されている通り(Burridge et al. 2014)、心筋細胞へ分化するようにiPSCを誘導した。
Quick-gDNA MiniPrep kit(Zymo Research)を製造業者のプロトコルに従って使用して、マウス10T1/2線維芽細胞およびヒトiPSCのゲノムDNAを単離した。
TRIzol(ThermoFisher Scientific)を製造業者のプロトコルに従って使用してRNAを単離した。iScript Reverse Transcription Supermix(Bio-Rad Laboratories)を製造業者のプロトコルに従って使用してcDNAを合成した。DMDエクソン47および52に隣接するプライマーを使用してRT-PCRを実施した。
過去に記載されている通り(Long et al., 2014)、ウサギ抗ジストロフィン抗体(Abcam, ab15277)およびマウス抗心臓ミオシン重鎖抗体(Abcam, ab50967)を使用してウエスタンブロット分析を実施した。
iPSC由来心筋細胞をアセトンで固定し、血清カクテル(2%正常ウマ血清/2%正常ロバ血清/0.2%ウシ血清アルブミン(BSA)/PBS)でブロッキングし、ジストロフィン抗体(MANDYS8, 1:800, Sigma-Aldrich)およびトロポニン-I抗体(H170, 1:200, Santa Cruz Biotechnology)と0.2% BSA/PBS中でインキュベートした。4℃で一晩インキュベートした後、これらを二次抗体(ビオチン化ウマ抗マウスIgG, 1:200, Vector Laboratories, フルオレセインコンジュゲートロバ抗ウサギIgG, 1:50, Jackson Immunoresearch)と1時間インキュベートした。Hoechst 33342(Molecular Probes)で核を対比染色した。
過去に記載されている通り(Zechner et al., 2010)、Trizolを使用してゲノムおよびミトコンドリアDNAを単離し、続いて逆抽出した。KAPA SYBR FAST qPCR kit(Kapa Biosystems)を使用してリアルタイムPCRを実施し、ミトコンドリアDNAのコピー数を定量的に決定した。ヒトミトコンドリアND1遺伝子を、プライマー(フォワード:

)を使用して増幅させた。ヒトゲノムLPL遺伝子を、プライマー(フォワード:

過去に記載されている通り(Baskin et al., 2014)、XF24 Extracellular Flux Analyzer(Seahorse Bioscience)を製造業者のプロトコルに従って使用して、ヒトiPSC由来心筋細胞において酸素消費率(OCR)を決定した。
ヒトコドン最適化LbCpf1を、pLbCpf1-2A-GFPから、T7 プロモーター配列を含むようにPCR増幅させた(表S1)。mMESSAGE mMACHINE T7転写キット(ThermoFisher Scientific)を製造業者のプロトコルに従って使用して、PCR産物を転写した。合成したLbCpf1 mRNAを、E. coli Poly(A) Polymerase(New England Biolabs)でポリAテール化し、NucAwayスピンカラム(ThermoFisher Scientific)を使用して精製した。
ssODNをHDR鋳型として使用して、Integrated DNA Technologiesによって4nM Ultramerオリゴヌクレオチドとして合成した。ssODNを、精製なしで、LbCpf1 mRNAおよびgRNAと直接混合した。ssODNの配列は、以下のとおりである。
全ての動物の取り扱いは、テキサス大学南西医療センターの動物実験委員会によって承認された。注射手順を過去に記載の通り実施した(Long et al., 2014)。唯一の変更は、Cas9 mRNAおよびCas9 sgRNAをLbCpf1 mRNAおよびLbCpf1 gRNAに置き換えたことであった。
これらの方法を過去に公開されている通り実施した(Long et al., 2014)。
統計分析は、両側スチューデントのt検定によって評価した。データを平均±SEMとして示す。P<0.05の値を統計的に有意であると見なした。
Cpf1媒介性ゲノム編集によるDMD iPSC由来心筋細胞の修正
フレーム外エクソンを並列することによってオープンリーディングフレーム(ORF)を破壊する、ヒトDMD遺伝子のエクソン51の前のエクソン欠失は、最も一般的なタイプのヒトDMD変異である(Aartsma-Rus et al., 2009)。エクソン51のスキッピングは、原理上、エクソン欠失を有するDMD患者の13%でDMD ORFを回復させることができる(Cirak et al., 2011)。このタイプの「ホットスポット」変異を修正するCpf1の潜在性を試験するために、本発明者らは、エクソン48〜50の欠失を保有するDMD線維芽細胞由来iPSC(Riken HPS0164、略称Riken51)を使用して、エクソン51内に中途終止コドンを導入した(図1A)。
リフレーミング戦略を使用してCRISPR-Cpf1によって編集されたRiken51 iPSCを、心筋細胞へ分化するように誘導した(Burridge et al., 2014)(図2A)。エクソン47を標的とするフォワードプライマーおよびエクソン52を標的とするリバースプライマーを使用したRT-PCRによって、リフレーミングされたDMD遺伝子を有する心筋細胞を同定し、PCR産物を配列決定した(図2B〜C)。未修正のiPSC由来心筋細胞は、エクソン51によってコードされる最初の8つのアミノ酸の後に中途終止コドンを有し、これが中途停止コドンを生み出す(図2C)。エクソン47〜52の増幅からのRT-PCR産物の配列決定によって明らかなように、Cpf1編集Riken51 iPSCから分化した心筋細胞は、DMD ORFの回復を示した(図2C)。本発明者らはまた、ジストロフィン抗体を使用したウエスタンブロット分析および免疫細胞化学によって、ジストロフィンタンパク質発現の回復も確認した(図2D〜E)。驚いたことに、クローンの選択および増幅なしでも、Cpf1編集iPSC混合物から分化した心筋細胞は、WT心筋細胞に匹敵するジストロフィンタンパク質レベルを示した(図2D)。
小さいINDELを導入する単一gRNA媒介性リフレーミング法とは対照的に、エクソンスキッピングは、2つのgRNAを使用して、スプライス部位を破壊し、大きな欠失を生成する(図3A)。Riken51 iPSCにおいてジストロフィン発現を回復させる独立した戦略として、本発明者らは、イントロン50の3'端を標的とする2つのLbCpf1 gRNA(g2およびg3)を設計し、ヒト293T細胞において切断効率を試験した。T7E1アッセイは、g3と比較してg2がイントロン50内でより高い切断効率を有していたことを示した(図3B)。それ故、本発明者らは、エクソン51のスプライスアクセプター部位を破壊する目的で、LbCpf1、g2、およびg1(g1は、エクソン51の5'領域を標的とする)をRiken51 iPSCに共送達した。ゲノムPCRは、LbCpf1編集iPSCにおいてより低いバンドを示し(図3C)、配列決定データは、イントロン50とエクソン51との間に約200bpの欠失の存在を確認し、これが保存されたスプライスアクセプター部位を破壊した(図3D)。
インビボでのCpf1媒介性Dmd修正の潜在性をさらに評価するために、本発明者らは、LbCpf1を使用して、HDR媒介性修正またはNHEJ媒介性リフレーミングによって、mdxマウスの生殖細胞系における変異を永続的に修正した。mdxマウスは、CからTへの転換に起因して、Dmd遺伝子のエクソン23にナンセンス変異を保有する(図4A)。エクソン23を標的とする3つのgRNA(g1、g2、およびg3)をスクリーニングし、マウス10T1/2線維芽細胞において切断効率を試験した(図4B)。T7E1アッセイは、LbCpf1およびAsCpf1がDmdエクソン23において異なる切断効率を有していたことを明らかにした(図4C)。配列決定結果に基づくと、g2を使用したLbCpf1媒介性ゲノム編集は、g3と比較して、マウス線維芽細胞においてより大きなINDELの発生をもたらした(図6C)。
mdx接合体に、インビトロ転写されたLbCpf1 mRNA、インビトロ転写されたg2 gRNA、および180bp ssODNを共注射し、偽妊娠の雌に再移植した(図5A)。LbCpf1編集mdxマウスの3組の同腹仔を、T7E1アッセイおよびTseI RFLP(制限酵素断片長多型)によって分析した(図5B〜C)。TseI RFLPおよび配列決定によって検出された通り、産まれた24匹の仔のうち、12匹がT7E1陽性であり、5匹が修正された対立遺伝子(mdx C1〜C5)を保有していた(図5C〜D)。WTマウス、mdxマウス、およびLbCpf1修正されたmdx-Cマウス由来の骨格筋(前脛骨筋および腓腹筋-足底筋)を、4週齢のときに分析した。筋肉のヘマトキシリンおよびエオシン(H&E)染色は、mdx筋肉において線維化および炎症性浸潤を示したが、LbCpf1修正された(mdx-C)筋肉は、正常な筋肉形態を呈し、ジストロフィー表現型の兆候はなかった(図5Eおよび図7A〜B)。免疫組織化学は、mdxマウスの筋肉切片においてジストロフィン陽性線維が存在しないことを示したが、LbCpf1媒介性HDRによって修正されたmdx-C筋肉は、筋線維の大部分でジストロフィンタンパク質発現を示した(図5Fおよび図7A〜B)。これらの知見は、生殖細胞系DNAのLbCpf1媒介性編集が、マウスにおいて筋ジストロフィーを効果的に予防できることを示す。
この研究で、本発明者らは、新たに発見されたCRISPR-Cpf1ヌクレアーゼが、患者由来iPSCおよびmdxマウスにおいてDMD変異を効率的に修正でき、これによってジストロフィン発現の回復が可能となることを示す。DMDにおけるジストロフィンの欠如は、筋線維鞘の完全性を崩壊させ、ミトコンドリア機能不全および酸化ストレスを引き起こすことが示されている(Millay et al., 2008; Mourkioti et al., 2013)。本発明者らは、LbCpf1で修正されたiPSC由来心筋細胞において、未修正のDMD iPSC由来心筋細胞と比較して増加したミトコンドリアDNAおよびより高い酸素消費率を見いだした。また、Cpf1媒介性ゲノム編集によって、ヒトDMD iPSC由来心筋細胞の代謝異常も回復した。本発明者らの知見はまた、マウスゲノム編集におけるCpf1の堅固性および効率も実証した。HDR媒介性修正を使用することによって、マウスDmd遺伝子のORFは完全に回復し、線維化および炎症性浸潤などのジストロフィー表現型の病態生理学的特徴も回復した。
