筋ジストロフィー(MD)は、運動を制御する骨格筋の進行性の衰弱および変性によって特徴付けられる、30を超える遺伝子疾患の一群である。デュシェンヌ型筋ジストロフィー(DMD)は、男児のおよそ5000人に1人が罹患するMDの最も重度の形態の1つであり、進行性の筋衰弱および早期死亡によって特徴付けられる。心筋症および心不全が、DMDの共通する不治性かつ致死性の特徴である。この疾患は、細胞表面にあるジストログリカン複合体を基底細胞骨格と連結することによって収縮中の筋細胞膜の完全性を維持する大きな細胞内タンパク質であるジストロフィンをコードする遺伝子(DMD)の変異によって引き起こされる。ジストロフィン遺伝子の変異は、ジストロフィンの発現の喪失をもたらし、その結果、筋膜脆弱化および進行性筋消耗を引き起こす。
CRISPR/Cas9によるゲノム編集は、疾患を引き起こす変異を修正または軽減するための有望な新規アプローチである。デュシェンヌ型筋ジストロフィー(DMD)は、X連鎖ジストロフィン遺伝子(DMD)の3000を超える異なる変異によって引き起こされる心筋および骨格筋の致死的変性を伴う。これらの変異の大部分は、「ホットスポット」に集合している。本明細書の実施例に記載するように、変異ホットスポット内またはその近傍で最も一般的な変異体またはアウトオブフレームDMDエクソンのスキッピングを潜在的に可能にする、12個のエクソン中の保存されたRNAスプライス部位を無効にする挿入/欠失(インデル)変異を非相同末端結合によって導入することができる、最適なガイドRNAについてスクリーニングを実施した。エクソンスキッピングによるDMD変異の修正は、本明細書において、「ミオエディティング(myoediting)」と称される。概念実証研究において、ミオエディティングを、DMD遺伝子内に大きな欠失、点変異または重複を有する複数の患者由来の代表的な人工多能性幹細胞において実施して、誘導心筋細胞におけるジストロフィンタンパク質発現を効率的に回復した。3次元人工心筋(EHM)では、DMD変異のミオエディティングは、ジストロフィン発現および対応する機械的収縮力を回復した。心筋細胞の一部(30〜50%)の修正だけで、変異体EHM表現型を正常に近い対照レベルまで救済するのに十分であった。したがって、ミオエディティングを通じて、保存されたRNAスプライシングアクセプター/ドナー部位を無効にし、スプライシング機構を変異体またはアウトオブフレームエクソンのスキッピングに指向させることで、DMDに関連する心臓の異常の修正が、根底にあるこの疾患の遺伝学的基礎を取り除くことによって可能になることが示される。
DMDは、ヒトにおいて特定されている4,000を超える独立した変異を有する新たな変異症候群である(dmd.nlのワールドワイドウェブ)。患者の変異の大部分は、ホットスポットに集合している欠失を含み、したがって、ある特定のエクソンをスキッピングする治療的アプローチが大規模な患者集団に適用される。エクソンスキッピングアプローチの論理的根拠は、DMD患者とベッカー型筋ジストロフィー(BMD)患者との間の遺伝的差異に基づく。DMD患者では、ジストロフィンmRNAのリーディングフレームが分断された結果、早期切断された非機能的ジストロフィンタンパク質をもたらす。BMD患者は、DMD遺伝子においてリーディングフレームを維持する変異を有しており、これによって、内部で欠失しているが部分的に機能的なジストロフィンの産生が可能となり、このことで、DMD患者と比較してはるかに軽度の病状を導く。
CRISPR(クラスター化して規則的な配置の短い回文配列反復)は、塩基配列の短い反復を含有するDNA遺伝子座である。各反復の後に、ウイルスへの事前曝露由来の「スペーサーDNA」の短いセグメントが続く。CRISPRは、配列決定された真正細菌ゲノムの約40%および配列決定された古細菌の90%に見いだされる。CRISPRは、CRISPRに関連するタンパク質をコードするCas遺伝子と関連していることが多い。CRISPR/Casシステムは、プラスミドおよびファージなどの外来遺伝エレメントに対する耐性を付与し、かつ獲得免疫の一形態を提供する、原核生物の免疫系である。CRISPRスペーサーは、真核生物におけるRNAiのように、これらの外因性遺伝エレメントを認識およびサイレンシングする。
RNAにガイドされるタンパク質として、Cas9は、DNA標的の認識を指令する短いRNAを必要とする。Cas9は、PAM配列NGGを含有するDNA配列を選択的に調べるが、それはプロトスペーサー標的なしでここに結合することができる。しかしながら、Cas9-gRNA複合体は、二本鎖切断を生じるためにgRNAとの厳密な一致を必要とする。細菌におけるCRISPR配列は、複数のRNAで発現され、次いでRNAのガイド鎖を生じるためにプロセシングされる。真核生物系は、CRISPR RNAをプロセシングするために必要なタンパク質の一部を欠いているため、RNAポリメラーゼIII型プロモーターU6で発現される単一RNAにCas9ターゲティングに不可欠なRNA片を組み合わせるために合成構築物gRNAが作られた。合成gRNAは、最小長で100bpをわずかに上回り、PAM配列NGGの直前の20のプロトスペーサーヌクレオチドを標的とする部分を含む;gRNAは、PAM配列を含まない。
Casヌクレアーゼ
CRISPR関連(cas)遺伝子は、CRISPR反復-スペーサーアレイに関連していることが多い。2013年現在、40種を超える異なるCasタンパク質ファミリーが記述されている。これらのタンパク質ファミリーのうち、Cas1は、異なるCRISPR/Casシステムの間で遍在しているようである。8つのCRISPRサブタイプ(Ecoli、Ypest、Nmeni、Dvulg、Tneap、Hmari、Apern、およびMtube)を定義するためにcas遺伝子および反復構造の特定の組み合わせが使用されており、その一部は、RAMP(repeat-associated mysterious protein)をコードする追加の遺伝子モジュールと関連している。単一のゲノム内に2つ以上のCRISPRサブタイプが存在し得る。CRISPR/Casサブタイプの散在的分布は、システムが微生物の進化中に遺伝子水平伝播を受けることを示唆している。
プレボテラおよびフランシセラ1由来のクラスター化して規則的な配置の短い回文配列反復、すなわちCRISPR/Cpf1は、CRISPR/Cas9システムといくつかの類似性を有するDNA編集技術である。Cpf1は、クラスII CRISPR/CasシステムのRNAガイドエンドヌクレアーゼである。この獲得免疫機構は、プレボテラ細菌およびフランシセラ細菌に見いだされる。それは、ウイルスによる遺伝子損傷を防止する。Cpf1遺伝子は、CRISPR遺伝子座に関連しており、ガイドRNAを使用してウイルスDNAを発見および切断するエンドヌクレアーゼをコードする。Cpf1は、Cas9よりも小さくかつ単純なエンドヌクレアーゼであり、CRISPR/Cas9システムの制限の一部を克服する。
適応:Cas1およびCas2タンパク質が、小さなDNA断片のCRISPRアレイへの適応を容易にする;
crRNAの形成:Casタンパク質をガイドする成熟crRNAを産生する、プレ-cr-RNAのプロセシング;および
干渉:Cpf1がcrRNAに結合して、二成分複合体を形成し、標的DNA配列を同定および切断する。
Cas9は、DNAを切断するために2つのRNA分子を必要とする一方で、Cpf1は1つ必要とする。これらタンパク質はまた、DNAを異なる場所で切断し、編集部位を選択する際に研究者により多くの選択肢を提供する。Cas9は、DNA分子において両方の鎖を同じ位置で切断し、「平滑」末端を残す。Cpf1は、一方の鎖を他方よりも長く残し、扱いやすい「付着」末端を生じる。Cpf1は、Cas9と比較して、切断部位に新たな配列をより多く挿入できるようである。CRISPR/Cas9システムは遺伝子を効率的に無効にすることができるが、遺伝子を挿入することもノックインを生成することも困難である。Cpf1は、tracrRNAを欠き、TリッチPAMを利用し、DNAを突出型のDNA DSBを介して切断する。

上で考察した通り、ある特定の態様において、その後の精製および細胞/対象への送達のため、または遺伝子に基づく送達アプローチで直接使用するため、発現カセットを用いて転写因子産物を発現させる。Cas9またはCpf1およびジストロフィンスプライス部位を標的とする少なくとも1つのDMDガイドRNAをコードする1つまたは複数の核酸を含有する発現ベクターが本明細書に提供される。いくつかの態様において、Cas9またはCpf1をコードする核酸および少なくとも1つのガイドRNAをコードする核酸は、同じベクター上に提供される。さらなる態様において、Cas9またはCpf1をコードする核酸および少なくとも1つのガイドRNAをコードする核酸は、別々のベクター上に提供される。
プロモーターという用語は、RNAポリメラーゼIIのための開始部位の周辺に密集した転写制御モジュールの一群を指すために本明細書において使用される。どのようにプロモーターが組織化されるかについての考えの大部分は、HSVチミジンキナーゼ(tk)およびSV40初期転写単位のためのものを含む、いくつかのウイルスプロモーターの分析に由来する。より最近の研究によって拡大されたこれらの研究は、プロモーターが個別の機能的モジュールから構成されることを示しており、機能的モジュールは各々、およそ7〜20bpのDNAからなり、転写活性化タンパク質または抑制タンパク質のための1つまたは複数の認識部位を含有している。
真核生物では、RNAポリメラーゼIII(Pol IIIとも呼ばれる)は、DNAを転写して、リボソーム5S rRNA、tRNAおよび他の低分子RNAを合成する。RNA Pol IIIによって転写される遺伝子は、「ハウスキーピング」遺伝子のカテゴリーに分類され、その発現は、すべての細胞タイプおよびほとんどの環境条件において必要とされる。それゆえ、Pol III転写の調節は主に、細胞成長の調節および細胞サイクルに関わっており、したがって、RNAポリメラーゼIIよりも少ない調節タンパク質しか必要としない。しかしながら、ストレス条件下では、タンパク質Maf1は、Pol III活性を抑制する。

デュシェンヌ型筋ジストロフィー(DMD)は、筋肉変性および早期死亡をもたらす、男児のおよそ5000人に1人が罹患する劣性X連鎖型の筋ジストロフィーである。この障害は、タンパク質ジストロフィン(GenBankアクセッション番号AAA53189; SEQ ID NO:5)をコードする、ヒトX染色体上に位置する、遺伝子ジストロフィン(GenBankアクセッション番号NC_000023.11を参照)の変異によって引き起こされる。
症状は、通常、2〜3歳の男児に現れ、早期幼児期に見られることもある。早期幼児期まで症状が現れなくても、検査室試験により、出生時に活性変異を保有する子供を特定することができる。筋肉量の損失に関連する脚および骨盤の進行性の近位筋衰弱が最初に観察される。最終的に、この衰弱は、腕、首、および他の領域に広がる。初期の徴候は、仮性肥大(ふくらはぎおよび三角筋の腫脹)、低持久力、および、助けなしで起立することが困難、または階段を上ることができないことを含み得る。病状が進行するにつれて、筋組織は、消耗を経験し、最終的には脂肪および線維性組織に置き換えられる(線維症)。10歳までに歩行を補助するために支持具が必要となることがあり、ほとんどの患者は、12歳までに車椅子に頼る。その後の症状は、脊椎湾曲を含め、骨格の変形を導く異常な骨発達を含み得る。進行性の筋肉劣化に起因して、運動の消失が起こり、最終的に麻痺に至る。知的障害が存在する可能性も、存在しない可能性もあるが、存在しても、子供が年を取るにつれて次第に悪化することはない。DMDに罹患している男性の平均余命は25歳前後である。

以下の表は、本明細書に開示される組成物および方法と併せて使用するための例示的なプライマー、gRNAおよびゲノムターゲティング配列を提供する。
以下の実施例は、本開示の好ましい態様を実証するために含まれる。以下の実施例において開示される技術が、本開示の実践において良好に機能すると本発明者によって発見された技術を代表するものであり、したがって、これがその実践のための好ましい様式に相当すると見なすことができると、当業者は理解するはずである。しかしながら、当業者は、本開示に照らして、本開示の精神および範囲から逸脱することなく、開示されている具体的な態様において多くの変更を行うことができ、それでも同様のまたは類似の結果を得ることができるものと理解するはずである。
CRISPR/Cas9によるゲノム編集は、疾患を引き起こす変異を修正または軽減するための有望な新規アプローチである。デュシェンヌ型筋ジストロフィー(DMD)は、X連鎖ジストロフィン遺伝子(DMD)の3000を超える異なる変異によって引き起こされる心筋および骨格筋の致死的変性に関連する。これらの変異の大部分は、「ホットスポット」に集合している。真核生物のスプライスアクセプターおよびスプライスドナー配列と原核生物のCRISPR/Cas9標的遺伝子の認識および開裂を支配するプロトスペーサー隣接モチーフ配列との間には偶然の対応がある。この対応を利用して、変異ホットスポット内またはその近傍で最も一般的な変異体またはアウトオブフレームDMDエクソンのスキッピングを潜在的に可能にする、12個のエクソン中の保存されたRNAスプライス部位を無効にする挿入/欠失(インデル)変異を非相同末端結合によって導入することができる、最適なガイドRNAをスクリーニングした。エクソンスキッピングによるDMD変異の修正は、本明細書において、「ミオエディティング」と称される。概念実証研究において、DMD遺伝子内に大きな欠失、点変異または重複を有する複数の患者由来の代表的な人工多能性幹細胞においてミオエディティングを実施して、誘導心筋細胞におけるジストロフィンタンパク質発現を効率的に回復した。3次元人工心筋(EHM)では、DMD変異のミオエディティングは、ジストロフィン発現および対応する機械的収縮力を回復した。心筋細胞の一部(30〜50%)の修正だけで、変異体EHM表現型を正常に近い対照レベルまで救済するのに十分であった。したがって、ミオエディティングを通じて、保存されたRNAスプライシングアクセプター/ドナー部位を無効にし、スプライシング機構を変異体またはアウトオブフレームエクソンのスキッピングに指向させることで、DMDに関連する心臓の異常の修正が、根底にあるこの疾患の遺伝学的基礎を取り除くことによって可能になる。
スキップされたときにDMD変異のホットスポット領域のほとんどでジストロフィンオープンリーディングフレームを潜在的に回復することができる、上位12個のエクソンの一覧を表5に示す。ヒトDMD変異の大部分をエクソンスキッピングによって修正することへの初期段階として、ヒトDMD遺伝子の上位12個のエクソンを標的とするガイドRNAのプールをスクリーニングした(図1Aおよび1B)。各エクソンの3'スプライス部位または5'スプライス部位それぞれを標的とする3〜6個のPAM配列(NAGまたはNGG)を選択した(図1Aおよび表5)。これらのガイドRNAをプラスミドSpCas9-2A-GFPにクローニングした。必須のスプライスドナーまたはアクセプター配列を除去するインデルは、対応する標的エクソンのスキッピングを可能にする。公知のDMD変異の頻度に基づいて、これらのガイドRNAは、DMD患者の最大60%においてジストロフィン機能を救済することができると予測される。
エクソンスキッピングによって異なるタイプのヒトDMD変異を修正する単一ガイドRNAの有効性を評価するために、代表的なタイプのDMD変異を有する3種のDMD iPSC系統を入手した:大きな欠失(Delと呼ばれる;エクソン48〜50を欠く)、偽エクソン変異(pExと呼ばれる;イントロン点変異によって引き起こされる)、および重複変異(Dupと呼ばれる)。簡単に述べると、全血から得た末梢血単核細胞(PBMC)を培養し、次いで、再プログラム化因子を発現する組換えセンダイウイルスベクターを使用してiPSCに再プログラム化した。エクソン48の大きな欠失を有するDMD患者由来のiPSC系統(別称Del)に対するiPSCミオエディティングにおいて変異を修正または回避するためのCas9およびガイドRNAを、ヌクレオフェクションによって細胞に導入した。次いで、処置された細胞のプールまたは単一クローンを、標準条件を使用して誘導心筋細胞(iCM)に分化させた。精製したiCMを使用して、3D-EHMを作製し、機能性アッセイを実施した(図2A)。
DMD症例の約60〜70%が1つまたは複数のエクソンの大きな欠失によって引き起こされると推定される。ホットスポットにエクソン48〜50の大きな欠失を有するDMD患者由来のiPSC系統に対してミオエディティングを実施した。図2Bに示すように、大きな欠失は、フレームシフト変異を生み出し、エクソン51に未熟な終止コドンを導入する。エクソン51におけるスプライスアクセプターの破壊は、原理上は、エクソン47〜52のスプライシングを可能にし、それによって、オープンリーディングフレームが再構成される(図2Bおよび図6B)。理論上は、エクソン51のスキッピングは、DMD患者の約13%を潜在的に修正することができる。最適化されたガイドRNA Ex51-g3およびCas9(図2C)をこのiPSC系統にヌクレオフェクトした結果、ゲノム配列決定によって実証された通り、スプライスアクセプターの破壊またはNHEJによるエクソン51の再構成、および、オープンリーディングフレームの回復が成功した(図6B)。ミオエディティングされたDMD iPSC(Del-Cor.)のプールをiCMに分化させ、そして、エクソン47〜52の増幅からのRT-PCR産物の配列決定によってインフレームジストロフィンmRNA発現の救済が確認された(図2Dおよび図6C)。
このアプローチを希少な変異にさらに拡張するために、イントロン47に自然発生点変異(c.6913-4037T>G)を有するDMD患者由来のiPSC内の点変異(別称pEx)を修正しようと試みた。この点変異は、新規のRNAスプライシングアクセプター部位(YnNYAG)を生成し、未熟な終結シグナルをコードするエクソン47Aの偽エクソンをもたらす(図2E)。変異を正確に標的とするように2個のガイドRNA(Ex47A-g1およびEx47A-g2)を設計した(図2Fならびに図7Aおよび7B)。図2Gに示すように、ミオエディティングは、潜在スプライスアクセプター部位を無効にし、偽エクソンを永続的にスキップし、それによって、修正された細胞(pEx-Cor.)において完全長ジストロフィンタンパク質が回復した。これらのDMD iCMにおいてRT-PCRによってエクソンスキッピングの有効性を試験した(図2G)。RT-PCR産物の配列決定は、エクソン47がエクソン48にスプライシングされたことを裏付けた(図7C)。
エクソン重複は、DMDを引き起こす特定された変異の約10〜15%を占める。ジストロフィンオープンリーディングフレームを分断する大きな重複(エクソン55〜59)を有するDMD患者由来のiPSC系統(別称Dup)に対してミオエディティングを試験した(図2H)。この患者由来の細胞における全ゲノムシーケンシングおよびコピー数変動プロファイルの解析を実施し、イントロン54における正確な挿入部位を特定した(図2H)。この挿入部位(In59-In54接合部)をPCRによって確認した(図8Aおよび表4)。
次に、処置されたiCMの単一コロニーおよびプールにおけるジストロフィンタンパク質の回復および安定発現を、免疫細胞化学(図3A〜3C、ならびに図6D、7Dおよび8F)およびウェスタンブロット解析(図24、D〜F)によって確認した。クローン選択および拡大なしでも、Del-Cor.、pEx-Cor.、およびDup-Cor.におけるiCMのほとんどは、ジストロフィン陽性であった(図3A〜3C、ならびに図6D、7Dおよび8F)。ミオエディティングされたDel iPSCの混合物から、2つのクローン(#16および#27)を選び出し、心筋細胞に分化させた。より高いジストロフィン発現レベルを有するクローン#27を後続の実験のために選択した(別称Del-Cor-SC)。修正されたpExについての1つの選択されたクローン(#19)をさらなる研究に使用した(別称pEx-Cor-SC)。修正されたDupについての2つの選択されたクローン(#26および#6)をiCMに分化させた。クローン#6を機能性アッセイの実験に使用した(別称Dup-Cor-SC)。修正されたiCMのジストロフィンタンパク質発現レベルは、免疫細胞化学およびウェスタンブロット解析によってWT心筋細胞と同等であると推定された(50〜100%)(図3)。
ジストロフィンmRNAおよびタンパク質発現を生化学的方法によって測定することに加えて、正常DMDおよび修正されたDMD iCMに由来する3D-EHMを使用したマクロスケールの機能性解析を使用した。簡単に述べると、グルコース除去することによってiPSC由来心筋細胞を代謝精製した。精製した心筋細胞をヒト包皮線維芽細胞(HFF)と70%:30%の比で混合した。細胞混合物をウシコラーゲンおよび無血清培地の混合物中で再構成した。培養下で4週間後、収縮実験を実施した(図4A)。
DMD遺伝子は、260万個の塩基対を包含し、79個のエクソンをコードする、ヒトゲノムにおける最も大きな公知の遺伝子である。DMD遺伝子の大きなサイズおよび複雑な構造は、その高い自然発生変異率に寄与する。大きな欠失または重複(約77%)、小さなインデル(約12%)および点変異(約9%)を含む約3000の変異がヒトにおいて立証されている。これらの変異は主にエクソンに影響を及ぼすが、pEx変異についてここで示すように、イントロン変異がスプライシングパターンを変更し、疾患を引き起こし得る。
プラスミド
2A-EGFPを有するヒトコドン最適化SpCas9遺伝子およびガイドRNA骨格を含有するpSpCas9(BB)-2A-GFP(PX458)プラスミドは、F. Zhangからの贈与であった(プラスミド#48138, Addgene)。ガイドRNAのクローニングを、Feng Zhang Lab CRISPRプラスミド説明書(addgene.org/crispr/zhang/)に従って行った。
製造業者の説明書に従ってリポフェクタミン2000トランスフェクション試薬(Thermo Fisher Scientific)によって細胞をトランスフェクトし、細胞を合計48〜72時間インキュベートした。細胞選別を、テキサス大学(UT)サウスウェスタン医学センターのフローサイトメトリー共同研究施設で実施した。トランスフェクトされた細胞をトリプシン−EDTA溶液を使用して解離させた。混合物を37℃で5分間インキュベートし、10%ウシ胎児血清を補充した温かいダルベッコ変法イーグル培地2mlを加えた。再懸濁した細胞を15mlのファルコンチューブに移し、穏やかに20回トリチュレートした。細胞を室温で5分間1300rpmにて遠心分離した。培地を除去し、2%ウシ血清アルブミン(BSA)を補充したリン酸緩衝食塩水(PBS)500mlに細胞を再懸濁した。細胞をメッシュカップに通してセルストレーナーチューブに濾過した。選別された単一細胞を、マイクロフュージチューブ中に、GFP+細胞集団とGFP−細胞集団に分離した。
DMD iPSC系統Delは、細胞バンク理化学研究所バイオリソースセンターから購入した(cell no. HPS0164)。WT iPSC系統は、D. Garry(ミネソタ大学)からの贈与であった。他のiPSC系統(pExおよびDup)は、UTサウスウェスタンウェルストンミオエディティング共同研究所で作製し、維持した。簡単に述べると、DMD患者の全血から得られたPBMCを培養し、次いで、再プログラム化因子を発現する組換えセンダイウイルスベクター(Cytotune 2.0, Life Technologies)を使用してiPSCに再プログラム化した。iPSCコロニーを、免疫細胞化学、マイコプラズマ検査およびテラトーマ形成によって検証した。ヒトiPSCを、mTeSRTM1培地(STEMCELL Technologies)中で培養し、およそ4日ごとに継代した(継代比率1:18)。ヌクレオフェクションの1時間前に、iPSCを10mM ROCK阻害剤(Y-27632)で処理し、Accutase(Innovative Cell Technologies Inc.)を使用して単一細胞に解離させた。細胞(1×106)をSpCas9-2A-GFPプラスミド5mgと混合し、P3 Primary Cell 4D-Nucleofector X kit(Lonza)を製造業者のプロトコルに従って使用してヌクレオフェクトした。ヌクレオフェクション後、iPSCを、10mM ROCK阻害剤、ペニシリン-ストレプトマイシン(1:100)(Thermo Fisher Scientific)およびプリモシン(100mg/ml;InvivoGen)を補充したmTeSRTM1培地中で培養した。ヌクレオフェクションの3日後、上記のように蛍光活性化細胞選別によってGFP+およびGFP-を選別し、PCRおよびT7E1アッセイに供した。
プロテアーゼK(20mg/ml)をDirectPCR Lysis Reagent(Viagen Biotech Inc.)に終濃度1mg/mlまで加えた。細胞を4℃で10分間6000rpmにて遠心分離し、上清を廃棄した。氷上で維持された細胞ペレットを、DirectPCR/プロテアーゼK溶液50〜100mlに再懸濁し、55℃で>2時間または塊が観察されなくなるまでインキュベートした。粗溶解物を85℃で30分間インキュベートし、次いで、10秒間スピンした。NaClを終濃度250mMまで加え、続いて、0.7倍体積のイソプロパノールを加えてDNAを沈殿させた。DNAを4℃で5分間13,000rpmにて遠心分離し、上清を廃棄した。DNAペレットを70% EtOH 1mlで洗浄し、水に溶解した。NanoDrop装置(Thermo Fisher Scientific)を使用してDNA濃度を測定した。
PCRアッセイは、GoTaqポリメラーゼ(Promega)2ml、5×緑色GoTaq反応緩衝液20ml、25mM MgCl2 8ml、10mMプライマー2ml、10mMデオキシヌクレオチド三リン酸2ml、ゲノムDNA 8mlを含有し、二重蒸留H2O(ddH2O)を加えて100mlとした。PCR条件は、以下の通りであった:94℃で2分間、32×(94℃で15秒間、59℃で30秒間、および72℃で1分間)、72℃で7分間、次いで、4℃で保持した。PCR産物を2%アガロースゲル電気泳動によって分析し、ダイレクトシーケンシングのためにQIAquick PCR Purification kit(Qiagen)を使用してゲルから精製した。これらのPCR産物を、製造業者の説明書に従ってpCRII-TOPOベクター(Invitrogen)にサブクローニングした。個々のクローンを選び出し、DNAを配列決定した。
ゲノムPCR試料25mlの以下の条件を使用した変性/復元によってミスマッチ二重鎖DNAを得た:95℃で10分間、95°〜85℃(-2.0℃/秒)、85℃で1分間、85°〜75℃(-0.3℃/秒)、75℃で1分間、75°〜65℃(-0.3℃/秒)、65℃で1分間、65°〜55℃(-0.3℃/秒)、55℃で1分間、55°〜45℃(-0.3℃/秒)、45℃で1分間、45°〜35℃(-0.3℃/秒)、35℃で1分間、35°〜25℃(-0.3℃/秒)、25℃で1分間、次いで、4℃で保持した。
全ゲノムシーケンシングは、血液試料をNovogene Corporationに送ることによって実施された。精製されたゲノムDNA(1.0mg)をDNA試料調製用のインプット材料として使用した。TruSeq Nano DNA HT Sample Preparation kit (Illumina)を製造業者の説明書に従って使用してシーケンシングライブラリーを作製した。簡単に述べると、音波処理によってDNA試料を350bpのサイズまで断片化した。DNA断片を平滑末端化し、Aテール付加し、そして、完全長アダプターとライゲーションして、さらにPCR増幅しながらIlluminaシーケンシングを行った。ライブラリーをIlluminaシーケンシングプラットフォームで配列決定し、ペアエンドリードを作製した。
TRIzol RNA単離試薬(Thermo Fisher Scientific)を製造業者の説明書に従って使用して細胞からRNAを単離した。
iPSCを、PBS被覆プレート内のTESR-E8(STEMCELL Technologies)中1:120 Matrigel上で適合および維持し、EDTA溶液(Versene, Thermo Fisher Scientific)を使用して週2回継代した。心臓分化のために、iPSCを5×104〜1×105細胞/cm2でプレーティングし、RPMI、2% B27、200mM L-アスコルビン酸-2-リン酸セスキマグネシウム塩水和物(Asc;Sigma-Aldrich)、アクチビンA(9ng/ml;R&D Systems)、BMP4(5ng/ml;R&D Systems)、1mM CHIR99021(Stemgent)、およびFGF-2(5ng/ml;Miltenyi Biotec)で3日間誘導した;RPMI培地でさらに洗浄後、4日目〜13日目まで2% B27および200mM Ascを補充したRPMI中で5mM IWP4(Stemgent)と共に細胞を培養した。心筋細胞を、13日目〜17日目まで、2.2mM乳酸ナトリウム(Sigma-Aldrich)、100mM b-メルカプトエタノール(Sigma-Aldrich)、ペニシリン(100U/ml)およびストレプトマイシン(100mg/ml)を含むグルコース不含RPMI(Thermo Fisher Scientific)中でグルコース除去することによって代謝精製した。心筋細胞の純度は、15回の独立した分化実行(細胞系統ごとに1〜3回)から、92±2%であった。
規定の無血清EHMを作製するために、精製した心筋細胞をHFF(American Type Culture Collection)と70%:30%比で混合した。細胞混合物を、pH中和医療用ウシコラーゲン(1 EHM当たり0.4mg;LLC Collagen Solutions)と濃縮無血清培地[2×RPMI、インスリン不含の8% B27、ペニシリン(200U/ml)およびストレプトマイシン(200mg/ml)]の混合物中で再構成し、インスリン不含の4% B27、1%非必須アミノ酸、2mMグルタミン、300mMアスコルビン酸、IGF1(100ng/ml;AF-100-11)、FGF-2(10ng/ml;AF-100-18B)、VEGF165(5ng/ml;AF-100-20)、TGF-b1(5ng/ml;AF-100-21C;成長因子はすべてPeproTech社製)、ペニシリン(100U/ml)およびストレプトマイシン(100mg/ml)を含むIscove培地中で3日間培養した。3日間の凝縮期間の後、増張力性収縮を支持するためにEHMをフレキシブルホルダーに移した。4週間の全EHM培養期間の後に解析を行った。
37℃のオルガンバス内、ガス供給(5% CO2/95% O2)タイロード液(120mM NaCl、1mM MgCl2、0.2mM CaCl2、5.4mM KCl、22.6mM NaHCO3、4.2mM NaH2PO4、5.6mMグルコース、および0.56mMアスコルビン酸塩を含有する)中、等尺性条件下で収縮実験を実施した。200mAの5-ms方形波を用いて1.5 HzでEHMを電気刺激した。フランク・スターリングの法則に従って最大収縮期力振幅(FOC)が観察されるまで、EHMを125mm間隔で機械的に伸縮させた。漸増細胞外カルシウム(0.2〜4mM)に対する応答を調べて、最大変力性能を判定した。表示されている場合は、力を筋肉含量(フローサイトメトリーによって判定された場合のサルコメアa-アクチニン陽性細胞含量)に対して正規化した。
EHMの単一細胞懸濁液を既述の通り調製し、70%氷冷エタノール中で固定した。固定した細胞をヘキスト3342(10mg/ml;Life Technologies)で染色して、細胞二重体を除外した。サルコメアa-アクチニン染色(clone EA-53, Sigma-Aldrich)によって心筋細胞を特定した。細胞をLSRII SORPサイトメーター(BD Biosciences)に流し、DIVAソフトウェアを使用して分析した。1試料当たり少なくとも10,000の事象を分析した。
iPSC由来心筋細胞をアセトンで固定し、免疫染色に供した。固定した心筋細胞を血清カクテル(2%正常ウマ血清/2%正常ロバ血清/0.2% BSA/PBS)でブロッキングし、0.2% BSA/PBS中のジストロフィン抗体(1:800;MANDYS8, Sigma-Aldrich)およびトロポニン-I抗体(1:200;H170, Santa Cruz Biotechnology)とインキュベートした。4℃で一晩インキュベートした後、これらを二次抗体[ビオチン化ウマ抗マウス免疫グロブリンG(IgG)(1:200;Vector Laboratories)およびフルオレセインコンジュゲートロバ抗ウサギIgG(1:50;Jackson ImmunoResearch)]と1時間インキュベートした。ヘキスト33342(Molecular Probes)で核を対比染色した。
ヒトiPSC由来心筋細胞に対するウェスタンブロット解析を、ジストロフィンに対する抗体(ab15277, Abcam; D8168、Sigma-Aldrich)、グリセルアルデヒド-3-リン酸デヒドロゲナーゼに対する抗体(MAB374, Millipore)、および心臓ミオシン重鎖に対する抗体(ab50967, Abcam)を使用して実施した。ヤギ抗マウスおよびヤギ抗ウサギホースラディッシュペルオキシダーゼコンジュゲート二次抗体(Bio-Rad)を記載の実験に使用した。

iPSCを心筋細胞へと分化させる段階;ならびに
心筋細胞を対象に投与する段階
