Abstract
Una composición para la administración mediada por VAA de un producto terapéutico que tiene un sistema de ablación de expresión transgénica controlada, comprendiendo dicha composición (a) un vector VAA que contiene una molécula de ácido nucleico que comprende: (i) una secuencia de ácido nucleico que codifica un producto terapéutico unido operativamente a un promotor que controla la transcripción; y (ii) al menos un sitio de ablación de endonucleasa que comprende una secuencia de ácido nucleico de al menos de pares de bases reconocida específicamente por al menos diez (10X) dedos de cinc, localizándose dicho al menos un sitio de ablación de endonucleasa al menos en posición 5' con respecto a la secuencia que codifica el producto terapéutico; (b) al menos un ablacionador que comprende una endonucleasa quimérica que comprende al menos diez copias de un dominio de dedo de cinc, unida a un dominio catalítico de endonucleasa funcional en asociación operativa con un promotor, en el que la actividad de transcripción y/o ablación está inducida en respuesta a un agente farmacológico, reconociendo dicho dominio de al menos diez (10X) dedos de cinc específicamente dicha secuencia de al menos 30 pares de bases de (a) (ii) en dicho al menos un sitio de ablación de endonucleasa y que comprende al menos 10 hélices de reconocimiento.

Cuando se empaquetan en dos o más reservas víricas, las composiciones de virus con replicación defectuosa pueden administrarse de manera simultánea o secuencial. Cuando dos o más reservas de virus se administran secuencialmente, la última de las reservas de virus administrada puede administrarse uno, dos, tres o cuatro días después de la administración de la primera reserva de virus. Preferentemente, cuando se administran dos reservas víricas secuencialmente, la segunda reserva de virus administrada se administra uno o dos días después de la administración de la primera reserva de virus.
Para determinar el número de copias del genoma (CG) de las composiciones de virus con replicación defectuosa de la invención puede usarse cualquier método conocido en la técnica. Un método para realizar un ajuste del número de CG del VAA es el siguiente: primero se tratan muestras purificadas del vector VAA con DNasa para eliminar el ADN del genoma del VAA no encapsidado o ADN plasmídico contaminante del proceso de producción. Las partículas resistentes a DNasa se someten después a un tratamiento térmico para liberar el genoma de la cápside. Los genomas liberados se cuantifican después mediante PCR en tiempo real usando conjuntos de cebador/sonda que se dirigen a la región específica del genoma viral (normalmente señal poli A).
En una realización, las composiciones de virus con replicación defectuosa de la invención se administran por vía sistémica a través del hígado mediante inyección de un afluente mesentérico de la vena porta a una dosis de aproximadamente 3,0 x 1012 GC/kg. En otra realización, las composiciones de virus con replicación defectuosa de la invención se administran por vía sistémica a través del músculo mediante hasta 20 inyecciones intramusculares en cualquiera de los músculos cuádriceps o bíceps a una dosis de aproximadamente 5,0 x 1012 GC/kg. En otra realización, las composiciones de virus con replicación defectuosa de la invención se administran a la región prosencefálica basal del cerebro que contiene el núcleo basal de Meynert (NBM) mediante inyección bilateral estereotáctica a una dosis de aproximadamente 5,0 x 1011 GC/kg. En otra realización, las composiciones de virus con replicación defectuosa de la invención se administran al SNC por inyección bilateral intraputaminal y/o intranigral a una dosis en el intervalo de aproximadamente 1,0 x 1011 GC/kg a aproximadamente 5,0 x 1011 GC/kg. En otra realización, las composiciones de virus con replicación defectuosa de la invención se administran a las articulaciones mediante inyección intraarticular a una dosis de aproximadamente 1,0 x 1011 GC/ml de volumen articular para el tratamiento de artritis inflamatoria. En otra realización, las composiciones de virus con replicación defectuosa de la invención se administran al corazón por inyección mediante infusión intracoronaria a una dosis en el intervalo de aproximadamente 1,4 x 1011 GC/kg a aproximadamente 3,0 x 1012 GC/kg. En otra realización, las composiciones de virus con replicación defectuosa de la invención se administran a la retina mediante inyección en el espacio subretiniano a una dosis de aproximadamente 1,5 x 1010 GC/kg.

En un aspecto de la divulgación, un método para el tratamiento de la degeneración macular relacionada con la edad en un sujeto humano comprende la administración de una cantidad eficaz de una composición de virus con replicación defectuosa, en la que el producto terapéutico es un antagonista de VEGF.
En otro aspecto de la divulgación, un método para el tratamiento de la hemofilia A en un sujeto humano comprende administrar una cantidad eficaz de una composición de virus con replicación defectuosa de la invención, en la que el producto terapéutico es el Factor VIII o sus variantes, tal como la cadena ligera y cadena pesada del heterodímero y el dominio con B delecionado. Patente de Estados Unidos n.º 6.200,560 y patente de Estados Unidos n.º 6.221.349). El gen del Factor VIII codifica 2351 aminoácidos y la proteína tiene seis dominios, denominada desde el extremo amino al extremo carboxilo A1-A2-B-A3-C1-C2 [Wood et al, Nature, 312:330 (1984); Vehar et al., Nature 312:337 (1984); y Toole et al, Nature, 342:337 (1984)]. El Factor VIII humano se procesa dentro de la célula para producir un heterodímero que comprende principalmente una cadena pesada que contiene los dominios A1, A2 y B y una cadena ligera que contiene los dominios A3, C1 y C2.
