Abstract
Implante intersomático apto para contener, al menos, un injerto óseo y para ser insertado dentro del espacio intervertebral según una dirección predeterminada, que comprende: - una primera parte (10) de forma general anular, que rodea una escotadura o cavidad (11) y presenta un primer borde superior (12) y un primer borde inferior (14), y - una segunda parte (16) de forma alargada según su eje principal (A), que presenta un segundo borde inferior (18) y un segundo borde superior (20), y que une según dicha dirección predeterminada dos porciones opuestas enfrente de dicha primera parte (10), dividiendo dicha cavidad o escotadura (11) en dos alojamientos aptos para contener dicho injerto óseo, caracterizado por el hecho de que, al menos, una porción de dichos segundos bordes, inferior y superior sobresale fuera del espacio definido por dichos primeros bordes inferior (14) y superior (12) de dicha primera parte (10) y son de forma curvilínea y sus extremos juntan dichos primeros bordes inferior (14) y superior (12) de dicha primera parte (10) respectivamente, de tal manera que los segundos bordes respetan la forma bicóncava del espacio intersomático y constituyen unos puntos de apoyo suplementarios del implante contra las placas vertebrales en su parte central.
Description
Implante intersomático o intervertebral.
La presente invención se refiere a un implante
intersomático destinado especialmente a inmovilizar un injerto óseo
entre dos vértebras, y más particularmente una jaula apta para
contener los injertos.
Los ámbitos de aplicación de la invención son,
en especial, los tratamientos quirúrgicos de los discos
degenerativos.
Estos tratamientos comportan generalmente una
intervención tendente a bloquear al mismo tiempo dos vértebras
contiguas, y esto interponiendo entre ellas unos injertos óseos,
que después de que ha prendido el injerto, las vuelven solidarias.
Sin embargo, habida cuenta de los esfuerzos mecánicos ejercidos
sobre los injertos por las dos vértebras, éstos últimos corren el
riesgo especialmente de ser expulsados o de hundirse.
Para paliar este inconveniente se conoce el
sistema de inmovilizar los injertos por medio de jaulas, cuyos
extremos superiores e inferiores, contra los bordes de los cuales,
están abiertos de manera a permitir el contacto de los injertos,
con la placa vertebral superior y la placa vertebral inferior.
El bloqueo de dos vértebras al mismo tiempo
mediante un elemento óseo presenta la ventaja de conferir unas
propiedades mecánicas homogéneas al conjunto así obtenido, y en
particular la deformación elástica de dicho conjunto que se
caracteriza por su módulo de elasticidad.
Ahora bien, las jaulas generalmente utilizadas
son de aleación de titanio y el módulo de elasticidad de estas
aleaciones es, al menos, diez veces superior al módulo de
elasticidad del hueso de las vértebras. Así, las jaulas
intersomáticas, que quedan por permanecer entre las vértebras,
crean en sí una discontinuidad de las propiedades mecánicas en el
conjunto formado por las dos vértebras soldadas.
A fin de conservar una deformación elástica
homogénea se ha propuesto utilizar unas jaulas de materiales
macromoleculares, cuyos módulos de elasticidad son del orden de los
del hueso. Sin embargo, las jaulas, de forma general anular, de
materias plásticas, son más frágiles que las jaulas de aleación de
titanio y su introducción con fuerza entre las vértebras puede
provocar la rotura de su borde. La solución, que consiste en
ensanchar o ampliar su borde, disminuye el volumen disponible para
los injertos y, por consiguiente, la fuerza de unión entre las dos
vértebras. Además, siendo su dureza inferior a las de las jaulas de
aleación de titanio, sus bordes penetran de manera menos importante
en las placas vertebrales, de suerte que su inmovilización con
respecto a las vértebras queda menos asegurada.

intersomático conforme al preámbulo de la reivindicación 1.

intersomática, en la cual dos paredes internas tienen unos bordes
inferior y superior en forma de alerón para anclar en el hueso
esponjoso.
Un objeto de la presente invención es proveer un
implante intersomático de materiales macromoleculares de tipo
jaula, que presenta una abertura superior y una abertura inferior
del mismo orden de tamaño que los de las jaulas de aleación de
titanio, de manera a conservar una superficie de injerto
prácticamente igual y cuyo mantenimiento en posición fija
con
respecto a las vértebras se mejora.

A este efecto, la presente invención propone un
implante intersomático caracterizado por el hecho de que, al menos,
una porción de dichos segundos bordes inferior y superior
sobresalen fuera del espacio definido por dichos primeros bordes
inferior y superior de dicha primera parte y son de forma
curvilínea y sus extremos juntan respectivamente dichos primeros
bordes inferior y superior de dicha primera parte de tal manera que
los segundos bordes respetan la forma bicóncava del espacio
intersomático y constituyen unos puntos de apoyo suplementarios del
implante contra las placas vertebrales en su parte central.
Así, una característica del implante
intersomático reside en el refuerzo de dicha primera parte de forma
general anular mediante una segunda parte alargada situada en la
dirección predeterminada de hundimiento de dicho implante entre las
vértebras. De este modo, a pesar de las tensiones o esfuerzos
ejercidos sobre la primera parte en el momento de su impactación
entre las vértebras, y que tienden a deformarla según su plano
medio de manera a aproximar las dos porciones prácticamente
opuestas, dicha segunda parte conserva su forma general anular.
Además, las superficies de abertura superior e inferior, divididas
en dos por dicha segunda parte son del mismo orden de tamaño que
las superficies de un implante intersomático de aleación de
titanio, lo que permite obtener una superficie de injerto
prácticamente idéntico.
Además, los segundos bordes inferior y superior
sobresalen fuera del espacio definido por dichos primeros bordes
inferior y superior de dicha primera parte, de suerte que, no
solamente el implante se mantiene entre las vértebras mediante
contacto de los bordes de dicha primera parte contra las placas
vertebrales, sino que se mantiene igualmente por dicha segunda
parte de forma alargada, que se apoya contra las placas vertebrales
de forma bicóncava.
Además, dichos segundos bordes inferior y
superior de dicha segunda parte son de forma curvilínea y sus
extremos juntan respectivamente dichos primeros bordes inferior y
superior de dicha primera parte. Así, la forma de la segunda parte
coincide prácticamente con la forma del espacio disponible situado
entre las vértebras.
Según un primer modo de realización de la
invención, dicha primera parte y dicha segunda parte se unen al
mismo tiempo de manera a formar una sola y misma pieza, de suerte
que son perfectamente solidarias.
Según un modo preferente de ejecución, dicha
primera parte está perforada lateralmente según dicho eje principal
de dicha segunda parte de manera a practicar un orificio ciego, que
se prolonga en dicha segunda parte, siendo apto dicho orificio
ciego para recibir una herramienta de inserción.
Como se explicará más detalladamente, según este
modo preferente de ejecución, la inserción del implante se hace con
fuerza entre dos vértebras adyacentes y es necesario controlar
perfectamente la dirección de la inserción. A fin de conservar un
ángulo de impacto constante, la herramienta que se aplica contra
dicho implante, se introduce en el orificio ciego, que se prolonga
en la segunda parte. Así, el extremo de la herramienta aplicado
contra el implante queda prisionero de dicho implante durante el
impacto y no corre el riesgo de perforar un tejido situado en el
entorno de la intervención.
De manera ventajosa dichos bordes inferiores y
superiores de dichas primera y segunda partes presentan unas
acanaladuras o estrías aptas para constituir unos puntos de anclaje
en dichas vértebras.
Así, la porción aguda o viva de las acanaladuras
penetra en la pared de las placas vertebrales. De este modo, el
implante se mantiene entre las vértebras sin posibilidad de juego
lateral, lo que es favorable a la toma o presa de los injertos
óseos.
De manera preferencial dicha primera parte de
forma general anular presenta, al menos, un eje de simetría y
ventajosamente dicha segunda parte une dichas dos porciones
opuestas de dicha primera parte según dicho eje de simetría.
Esta configuración permite adaptar perfectamente
el implante a la forma de las vértebras y especialmente de los
cuerpos vertebrales. Al quedar dispuesta la segunda parte según el
eje de simetría, se reparten uniformemente los esfuerzos ejercidos
sobre el implante en el momento de la impactación.
Según otro modo preferente de realización de la
invención, dicha segunda parte une dichas dos porciones opuestas de
dicha primera parte según una dirección, que forma un ángulo
comprendido entre 0 y 90° con dicho eje de simetría.
Así, la inserción del implante entre las
vértebras se puede hacer de manera más fácil cuando el nivel al
cual se realiza, no permite efectuar la intervención simétricamente
con respecto a la columna vertebral. En efecto, siempre es
necesario hundir el implante según el eje de dicha segunda parte
para no romper dicha primera parte, y algunos niveles vertebrales
no son accesibles más que lateralmente. En este caso, dicha segunda
parte se dispone de forma oblicua con respecto al eje de simetría
de dicho implante.
Con preferencia, dicha primera parte presenta
una forma general prácticamente hemicircular.
Según otro modo más, preferido de puesta en
ejecución de la invención, dicha pieza está moldeada a base de un
material macromolecular. Así, se produce en unas condiciones
económicas ventajosas. Preferentemente dicha pieza es de
poliéter-éter-cetona, que es fácilmente moldeable y
presenta unas propiedades elásticas próximas a las del hueso.
Según un segundo modo de realización de la
invención, dicha segunda parte comprende, además, unos medios de
anclaje, que desembocan en dicho segundo borde superior y en dicho
segundo borde inferior respectivamente. Así, dichos medios de
anclaje penetran con fuerza en la superficie de las placas
vertebrales, en el momento de la introducción del implante entre
las vértebras, de suerte que dicho implante queda completamente
inmovilizado en translación con respecto a las vértebras y no puede
deslizarse con respecto a ellas.
Según un modo particular de ejecución, dichos
medios de anclaje están constituidos por una pieza de anclaje, que
presenta dos extremos y por el hecho de que dicha segunda parte
presenta una escotadura o cavidad, que desemboca en dichos segundos
borde inferior y superior de manera que dicha pieza de anclaje,
insertada en dicha escotadura o cavidad, atraviesa dicha segunda
parte de parte a parte, desembocando el primer extremo en dicho
segundo borde superior y desembocando el segundo extremo en dicho
segundo borde inferior de dicha segunda parte.
De este modo, la pieza de anclaje es susceptible
de ser introducida en la segunda parte, en el momento de su
inserción entre las vértebras, si es absolutamente necesaria.
Además, al estar realizados los medios de anclaje a base de una
sola pieza, aumenta su rigidez y no se deforman en el momento de la
inserción.
Ventajosamente, dicha pieza de anclaje es
prácticamente de forma trapezoidal, siendo definidos sus dos
extremos por los dos lados no consecutivos y no paralelos entre sí.
De forma preferencial, presenta un plano medio, y se inserta en
dicha segunda parte de manera que dicho eje principal A de dicha
segunda parte corte las dos bases del trapecio y que su plano medio
sea prácticamente perpendicular al plano medio de dicha primera
parte. Así, gracias a la escotadura o cavidad que es prácticamente
de forma paralelepipédica rectángulo, las paredes de la pieza de
anclaje coinciden con las paredes de la escotadura o cavidad.
Con preferencia, una de las bases del trapecio
forma dos ángulos agudos con los otros dos lados no paralelos entre
sí, y por el hecho de que dicha pieza de anclaje se inserta en
dicha segunda parte de forma que la dirección de inserción de dicho
implante se efectúa de la mayor base hacia la menor base del
trapecio. De este modo, la pieza de anclaje presenta un punto
saliente en cada extremo, definido por el ángulo agudo, susceptible
de incrustarse más profundamente en el cuerpo vertebral y de ser
más solidario todavía. Además, la pieza de anclaje forma una
porción de flecha, cuando está encastrada entre las vértebras,
cuando el implante se hunde de la mayor base del trapecio hacia la
menor. De este modo, el desplazamiento de dicha pieza de anclaje en
la dirección opuesta se vuelve imposible mediante la incrustación
de los puntos salientes en las placas vertebrales. De forma
preferencial, la pieza de anclaje está realizada a base de aleación
de titanio y así su rigidez permite un mejor anclaje.

invención se pondrán de manifiesto con la lectura de la
subsiguiente descripción de modos de realización particulares de la
invención, dados a, título indicativo, pero no limitativo, en
referencia a los dibujos anexos, en los cuales:

La figura 1 ilustra el implante intersomático
insertado entre dos vértebras contiguas. Se referirá igualmente a
la figura 2 para describir el implante conforme a un primer modo de
realización de la invención y en el cual se inserta, de forma
preferente, entre las vértebras lumbares.
El implante intersomático presenta una primera
parte 10 vista de perfil en la figura 1. La vertiente anterior Ra
del raquis hace frente a la pared delantera del cuerpo y la
vertiente posterior Rp del raquis hace frente a la pared trasera
del cuerpo.
El implante intersómatico está destinado a
inmovilizar unos injertos óseos entre dos vértebras de manera a
sustituir el disco intervertebral. En la figura 1, el disco
intervertebral DIV1 se representa intacto entre las vértebras V2 y
V3.
El canal raquídeo, que se extiende a lo largo
del raquis según el eje R, contiene la médula espinal y se
comprende, en razón de la fragilidad de ésta última, que la
inserción del implante entre dos vértebras se hace, con
preferencia, según una vía de acceso anterior. Este es
especialmente el caso en la mayoría de las intervenciones para las
hernias discales cervicales.
En la figura 2, donde el implante intervertebral
se representa de perfil, se halla la primera parte 10 de forma
general anular y que define una cavidad o escotadura 11 dividida en
dos por una segunda parte 16 de forma alargada según un eje A
uniendo dicha segunda parte 16 dos porciones enfrente de la primera
parte 10. El primer borde superior 12 de la primera parte 10 está
prácticamente situado en un plano paralelo al plano definido por el
primer borde inferior 14 de la primera parte 10. Cada uno de estos
primeros bordes, 12, 14, cuando el implante se inserta entre las
vértebras, está en contacto con la placa vertebral que le
corresponda.
Las escotaduras o cavidades 11 se destinan a
recibir los injertos óseos, que están en contacto en su parte
superior con la placa vertebral inferior de la vértebra superior y
en su parte inferior, con la placa vertebral superior de la
vértebra inferior. Así, los injertos se contienen, y se inmovilizan
entre las vértebras de manera que permite al injerto prender y
solidarizar el conjunto. Estos injertos son generalmente unos
injertos corticales tomados en la cresta iliaca anterior, cuyo
espesor corresponde prácticamente al espacio intersomático. Es
evidente que, cuando la unión ósea entre las vértebras es estable,
los implantes intervertebrales permanecen y forman parte integrante
de la unión.
A fin de que el implante sea tan estable como
sea posible después de su inserción entre las vértebras, la segunda
parte 16, que une los dos bordes opuestos de la primera parte 10,
presenta unos segundos bordes inferior 18 y superior 20 de forma
curvilínea, que sobresalen fuera de la primera parte 10. Esta
geometría permite respetar la forma bicóncava del espacio
intersomático y constituir unos puntos de apoyo suplementarios del
implante contra las placas vertebrales en su parte central.
Previamente a la inserción del implante provisto de sus injertos
óseos, se extrae el disco intervertebral dañado de manera a
permitir el hundimiento del implante. A continuación se hunde el
implante en el espacio intervertebral según una dirección
predeterminada.
Esta dirección predeterminada depende del nivel
vertebral al cual se realiza la intervención, y de la indicación
terapéutica.
El implante intersomático tal como queda
representado en la figura 2 se destina a ser insertado sobre la
vertiente anterior del raquis. Presenta un orificio ciego 22
perforado en la pared de la primera parte 10 de forma anular y se
prolonga en la segunda parte 16 según su eje principal A, el cual se
confunde o coincide con el eje de simetría S de la primera parte
10. El extremo de la herramienta de inserción se introduce en el
orificio ciego 22 y se apoya en dicha segunda parte 16 de refuerzo
de manera a mantener y a guiar el implante en el momento de su
hundimiento en el espacio intersomático.
En efecto, una característica de la invención
reside en la inserción de la segunda parte 16 de forma alargada en
la primera parte a fin de permitir a ésta última resistir las
tensiones o esfuerzos, que se le aplican en el momento de la
impactación. Para que la resistencia del implante, a la deformación
sea óptima los impactos deben aplicarse según el eje A de la
segunda parte 16. Ahora bien, el orificio ciego 22 mantiene la
herramienta de impactación en una dirección constante con respecto
al implante, según el eje A de dicha pieza. Por otra parte, el
hecho de que el extremo de la herramienta de inserción sea
parcialmente solidaria del implante permite controlar la
trayectoria del implante entre los cuerpos vertebrales.
El bloqueo del implante entre las vértebras es
función de las fuerzas de frotamiento o roce que se ejercen en la
interfaz entre los bordes 12, 14, 18 y 20 de la primera parte 10 y
de la segunda parte 16 y las placas vertebrales. A fin de aumentar
estas fuerzas de rozamiento, los bordes 12, 14, 18 y 20 presentan
unas acanaladuras, que forman unos puntos de anclaje en las placas
vertebrales.
Según un modo particular de ejecución, las
acanaladuras están inclinadas en el sentido opuesto a la dirección
de inserción, de suerte que en el sentido de la inserción las
fuerzas de frotamiento o roce del implante contra las vértebras son
inferiores a las fuerzas de frotamiento, que aparecen en el sentido
opuesto a su dirección de inserción.
La primera parte 10 de forma anular tal como
queda representada en la figura 2 presenta un eje de simetría S, el
cual se confunde o coincide con el eje de simetría longitudinal de
la segunda parte 16, lo que permite una inserción perpendicular a
la vertiente anterior del raquis. Por otro lado, la forma general de
dicha primera parte 10 es prácticamente hemicircular de manera a
corresponder prácticamente a la forma de las placas vertebrales.
Así, la superficie de injerto definida por el implante es
óptima.
Según otro modo particular de puesta en
ejecución de la invención, ilustrado por la figura 3, la segunda
parte 16 es oblicua con respecto al eje de simetría S de la
primera, parte 10. Asimismo, el orificio ciego 22, que perfora el
borde lateral de la primera parte 10 de forma anular, se prolonga
en dicha segunda parte 16 según su eje A, oblicuo con respecto al
eje de simetría S.
Esta configuración permite una inserción del
implante según una dirección oblicua con respecto a la vertiente
anterior del raquis, lo que puede ser necesario para ciertos
espacios intervertebrales difícilmente accesibles.
Tal como queda representado en la figura 3 la
segunda parte alargada de eje A y el eje de simetría S de la
primera parte forman un ángulo prácticamente igual a 45°. Sin
embargo, la segunda parte se puede disponer de manera a formar un
ángulo comprendido entre 0 y 90° con dicho eje de simetría S.
Según un modo preferido de ejecución el implante
se realiza a base de un material macromolecular, que presenta unas
propiedades de deformabilidad elástica próximas a las del hueso y
más concretamente comprendidas entre las del hueso cortical y las
del hueso esponjoso.
El módulo de elasticidad o módulo de Young es
para el hueso esponjoso del orden de 0,1 GPa y para el hueso
cortical del orden de 12 GPa. Por consiguiente, el material
macromolecular se escoge entre los materiales sintéticos, que
presentan un módulo de elasticidad comprendido entre 0,1 y 12
GPa.
De manera preferencial el material presenta un
módulo de elasticidad comprendido entre 1 y 6 GPa de forma a
constituir un buen compromiso por el cual, cuando se inserta entre
dos vértebras, no causa ninguna discontinuidad en las propiedades
mecánicas elásticas del conjunto formado por las vértebras y el
implante.
Ventajosamente, el implante se moldea de una
sola pieza a base de poliéter-éter-cetona. Este
material presenta un módulo de elasticidad de 3,5 GPa.
El hecho de poder moldear el implante resulta
económicamente ventajoso y permite producir jaulas intersomáticas a
un coste inferior a los costes de producción de jaulas a base de
aleación de titanio.
Según algún otro modo particular de realización,
la superficie abierta del implante, que corresponde a la suma de
las superficies definidas por la intersección de un plano medio del
implante y de dichos dos alojamientos, dividida por la superficie
total definida por la intersección del plano medio con los
contornos de dicha primera parte corresponde a un ratio comprendido
entre 0,75 y 0,95.
Ahora se describirá otro modo particular de
realización de la invención, en referencia a las figuras 3, 4 y 5,
en el cual el implante se inserta preferentemente entre las
vértebras cervicales.
La figura 4 presenta un implante, que comporta
una primera parte 10 y una segunda parte 16, que unen dos bordes
opuestos de la primera parte 10. La segunda parte 16 presenta, de
manera análoga a la segunda parte representada en la figura 2, un
segundo borde inferior 18 y un segundo borde superior 20. En
cambio, está provista de una pieza de anclaje 40, la cual está
realizada preferentemente a base de aleación de titanio, de manera
a ser biocompatible y extremadamente resistente. La pieza de
anclaje 40 presenta un primer extremo 42, que desemboca en el
segundo borde superior 20, y un segundo extremo 44, que desemboca
en el segundo borde inferior 18.
Para hacer esto, la segunda parte 16 presenta
una escotadura o cavidad 46 de forma general paralelepipédica
rectángulo, que desemboca en el segundo borde inferior 18 y en el
segundo borde superior 20, y cuyas cuatro aristas formadas por las
paredes internas de la cavidad o escotadura 46 son prácticamente
perpendiculares al plano medio P de la primera parte 10. De este
modo, la pieza de anclaje 40, cuyas dimensiones son ligeramente
superiores a las de la cavidad o escotadura 46, es susceptible de
atravesar con fuerza la cavidad o escotadura 46 de parte a parte.
Así, se mantiene en posición fija con respecto a la segunda parte
16, mediante frotamiento o roce contra las paredes internas de la
cavidad o escotadura 46, las cuales se separan prácticamente las
unas de las otras.
Por supuesto, las dimensiones de la pieza de
anclaje 40, entre los dos extremos 42 y 44, son superiores a las
dimensiones de la cavidad o escotadura 46 de manera que los dos
extremos 42 y 44 sobresalen en cada uno de los segundos bordes 18,
20.
La figura 5 ilustra la pieza de anclaje 40 en
perspectiva. Es de forma trapezoidal y presenta dos primeros bordes
opuestos 48 y 50 paralelos entre sí, que forman las bases del
trapecio, y dos segundos bordes opuestos 52 y 54, que toman un
ángulo idéntico con el eje de simetría T de la pieza de anclaje 40.
Así, la pieza de anclaje 40 presenta dos esquinas 56 y 58 idénticas
con ángulo agudo susceptible de penetrar en la superficie de las
placas vertebrales.
Además, los dos segundos bordes opuestos 52 y 54
tienen un perfil agudo, para penetrar de manera todavía más fácil
en las placas vertebrales.
La pieza de anclaje 40 define un plano medio Pa,
y se inserta en la cavidad o escotadura 46, como ilustra la figura
4, de manera que el plano medio Pa sea perpendicular al plano medio
P de la primera parte 10. Además, se inserta de tal forma que la
dirección F de introducción del implante en el espacio
intervertebral corresponda a una dirección, que va del primer borde
50 de mayor dimensión hacia su borde opuesto 48 de menor dimensión.
Se comprende que esta disposición evita que la pieza de anclaje
frene la introducción del implante en el espacio intervertebral
cuando los extremos 42 y 44 se apoyan contra las placas vertebrales
de las dos vértebras. Se comprende igualmente que las dos esquinas
56 y 58 de la pieza de anclaje, que se incrustan en las placas
vertebrales en el momento de la introducción del implante,
constituyen unos medios de bloqueo importante de la pieza de
anclaje 40 y, por consiguiente, del implante, especialmente en la
dirección opuesta a la dirección de introducción. Así, el implante
se mantiene de forma más eficaz entre las dos vértebras.
La figura 6 muestra el implante visto de frente,
provisto de la pieza de anclaje 40, cuyo primer extremo 42 aparece
por encima de la primera parte 10 y el segundo extremo 44 por
debajo.
En el caso de este modo particular de
realización, donde el implante se introduce generalmente entre unas
vértebras cervicales, la primera parte 10 no es estrictamente
simétrica con respecto a su plano medio P. En efecto, como ilustra
la figura 6, el primer borde inferior 14 presenta unas porciones
encorvadas 60 sobre las partes laterales de la primera parte 10,
que hacen aparecer unos espacios libres 62 entre el plano tangente
Ptg de la primera parte 10 y el primer borde inferior 14. Además,
el primer borde superior de la primera parte, presenta una porción
convexa 64, donde la segunda parte 16 se une a la primera parte
10.
Gracias a esta configuración, el implante
intersomático coincide perfectamente con la geometría del espacio
intervertebral, especialmente a nivel de las vértebras
cervicales.
Habida cuenta de esta disimetría, el implante se
debe introducir en un sentido predeterminado. Y para hacerlo, un
orificio ciego principal 66 situado según el eje de simetría S es
susceptible de recibir la herramienta y un agujero de orientación
68 permite orientar el implante en el sentido predeterminado.
Según otro objeto la invención se refiere a una
pinza de implante intersomático destinada a insertar o a
desinsertar dicha pieza de anclaje 40 en dicha cavidad 46. Se
describirá la pinza en referencia a las figuras 7 y 8.
La pinza para implante comporta una primera
mordaza o zapata 70 susceptible de recibir el implante, que está
constituido por la primera parte 10, en la cual la segunda parte 16
une dos bordes opuestos, insertándose la pieza de anclaje 40 en
esta segunda parte 16. La primera zapata o mordaza 70 presenta una
cavidad o escotadura central 72 ilustrada en la figura 8, que
permite el paso de la pieza de anclaje 40 cuando se fuerza, a
través de la escotadura 46 de la segunda parte 16. La pinza
comporta una segunda mordaza 74 destinada a apoyarse contra la
pieza de anclaje 40, a fin de insertarla cuando las mordazas o
zapatas 70 y 74 se aproximan la una a la otra. Para hacerlo, cada
mordaza 70, 74 se prolonga por un mango, uniéndose los mangos al
mismo tiempo de manera a poder pivotar el uno con respecto al
otro.
Además, cuando la pieza de anclaje está
preinstalada en la segunda parte 16, es fácil extraerla con fuerza,
por medio de la pinza si la intervención no requiere la
introducción de un implante equipado con una pieza de anclaje.
Claims (16)
1. Implante intersomático apto para contener,
al menos, un injerto óseo y para ser insertado dentro del espacio
intervertebral según una dirección predeterminada, que
comprende:
- -
- una primera parte (10) de forma general anular, que rodea una escotadura o cavidad (11) y presenta un primer borde superior (12) y un primer borde inferior (14), y
- -
- una segunda parte (16) de forma alargada según su eje principal (A), que presenta un segundo borde inferior (18) y un segundo borde superior (20), y que une según dicha dirección predeterminada dos porciones opuestas enfrente de dicha primera parte (10), dividiendo dicha cavidad o escotadura (11) en dos alojamientos aptos para contener dicho injerto óseo, caracterizado por el hecho de que, al menos, una porción de dichos segundos bordes, inferior y superior sobresale fuera del espacio definido por dichos primeros bordes inferior (14) y superior (12) de dicha primera parte (10) y son de forma curvilínea y sus extremos juntan dichos primeros bordes inferior (14) y superior (12) de dicha primera parte (10) respectivamente, de tal manera que los segundos bordes respetan la forma bicóncava del espacio intersomático y constituyen unos puntos de apoyo suplementarios del implante contra las placas vertebrales en su parte central.
2. Implante intersomático según la
reivindicación 1, caracterizado por el hecho de que dicha
primera parte (10) y dicha segunda parte (16) se unen
simultáneamente de manera a formar una única pieza.
3. Implante intersomático según una cualquiera
de las reivindicaciones 1 y 2, caracterizado por el hecho de
que dicha primera parte (10) está perforada lateralmente según
dicho eje principal (A) de dicha segunda parte de manera a
practicar un taladro ciego (22), que se prolonga en dicha segunda
parte (16), siendo apto dicho taladro ciego (22) para recibir una
herramienta de inserción.
4. Implante intersomático según una cualquiera
de las reivindicaciones 1 a 3, caracterizado por el hecho de
que dichos bordes inferiores y superiores (12, 14, 18, 20) de
dichas primera y segunda partes presentan unas acanaladuras o
estrías aptas para constituir unos puntos de anclaje en dichas
vértebras.
5. Implante intersomático según una cualquiera
de las reivindicaciones 1 a 4, caracterizado por el hecho de
que dicha primera parte (10) de forma general anular presenta, al
menos, un eje de simetría (S).
6. Implante intersomático según la
reivindicación 5, caracterizado por el hecho de que dicha
segunda parte (16) une dichas dos porciones opuestas de dicha
primera parte (10) según dicho eje de simetría (S).
7. Implante intersomático según la
reivindicación 5, caracterizado por el hecho de que dicha
segunda parte (16) une dichas dos porciones opuestas de dicha
primera parte (10) según una dirección, que forma un ángulo
comprendido entre 0 y 90° con dicho eje de simetría (S).
8. Implante intersomático según una cualquiera
de las reivindicaciones 5 a 7, caracterizado por el hecho de
que dicha primera parte (10) presenta una forma general
prácticamente hemicircular.
9. Implante intersomático según una cualquiera
de las reivindicaciones 1 a 8, caracterizado por el hecho de
que dicha primera parte (10) y dicha segunda parte (16) están
moldeadas en una sola pieza a base de un material
macromolecular.
10. Implante intersomático según una cualquiera
de las reivindicaciones 1 a 9, caracterizado por el hecho de
que dicha primera parte (10) y dicha segunda parte (16) son de
poliéter-éter-cetona.
11. Implante intersomático según una cualquiera
de las reivindicaciones 1 a 10, caracterizado por el hecho
de que dicha segunda parte comprende, además, unos medios de
anclaje, que desembocan en dicho segundo borde superior y en dicho
segundo borde inferior respectivamente.
12. Implante intersomático según la
reivindicación 11, caracterizado por el hecho de que dichos
medios de anclaje están constituidos por una pieza de anclaje, que
presenta dos extremos; y por el hecho de que dicha segunda parte
presenta una escotadura o cavidad, que desemboca en dichos segundos
bordes inferior y superior, de forma que dicha pieza de anclaje,
insertada en dicha cavidad, atraviesa la segunda parte por
completo, desembocando el primer extremo en dicho segundo borde
superior y desembocando el segundo extremo en dicho segundo borde
inferior de dicha segunda parte.
13. Implante intersomático según la
reivindicación 12, caracterizado por el hecho de que dicha
pieza de anclaje es prácticamente de forma trapezoidal, siendo
definidos sus dos extremos por los dos lados no consecutivos y no
paralelos entre sí.
14. Implante intersomático según la
reivindicación 13, caracterizado por el hecho de que dicha
pieza de anclaje presenta un plano medio, y por el hecho de que se
inserta en dicha segunda parte de manera que dicho eje principal A
de dicha segunda parte corte las dos bases del trapecio y que su
plano medio sea prácticamente perpendicular al plano medio de dicha
primera parte.
15. Implante intersomático según la
reivindicación 13 ó 14, caracterizado por el hecho de que
una de las bases del trapecio forma dos ángulos agudos con los
otros dos lados no paralelos entre sí, y por el hecho de que dicha
pieza de anclaje se inserta en dicha segunda parte de manera que la
dirección de inserción de dicho implante se efectúa de la base
mayor hacia la base menor del trapecio.
16. Implante según una cualquiera de las
reivindicaciones 12 a 15, caracterizado por el hecho de que
dicha pieza de anclaje está fabricada a base de aleación de
titanio.
