Definitions

  • the invention relates to a system and method for administering medicaments to a patient.
  • patients in particular intensive care units, are supplied with medicaments and optionally artificial food by means of one or more delivery devices, for example intravenously or by means of a stomach tube.
  • the delivery device may be an insulin delivery device that maintains the insulin value present within the patient's bloodstream at a predeterminable level in response to a previously measured blood glucose level in the patient's bloodstream.
  • feeding devices for at least one nutritional value of a food directly or indirectly supplied to the blood circulation with at least one food supply device can be administered to the patient.
  • such delivery devices even when incorporated into a drug and / or nutritional delivery system, require that the physician or other clinical personnel enter values underlying delivery by the delivery device. For example, quantity values, time intervals in which the supply should take place, interval-like feeds, etc., can be entered as the basis for the subsequent delivery of, for example, insulin.
  • This delivery is preceded by a mostly hand-drawn blood sample in the patient, which requires the interposition of clinical personnel.
  • additional clinical personnel are required who have the necessary expertise in the input functions of the delivery device, such as the infusion pump, to subsequently perform this delivery.
  • such delivery devices such as infusion pumps
  • a delivery rate represented as volume per unit time (ml / hr).
  • the dosage unit is used for an added drug solution. Accordingly, it is necessary that the dosage unit be converted into a delivery rate of the pump, which is the task of the doctor to be treated.
  • a disadvantage of such a conversion can often be computational errors, which leads to incorrect entries of the delivery rate and thus to an incorrect administration of insulin for the patient.
  • the input of the dosage unit is made possible with an infusion pump.
  • this requires the consideration of information on the concentration of the drug of the drug to be administered and on the nature of the drug. Both when specifying the concentration of the active ingredient and when entering the dose unit and the conversion of those in the delivery rate so far only the main active ingredients of the drug are taken into account. This is often sufficient if only or primarily a specific drug is to be administered.
  • Feeding or conveying devices for the supply of a drug solution mixture in a body are known several times.
  • devices / systems having a plurality of infusion and / or syringe pumps, each of which delivers a solution having at least one specific drug active to a body and thereby creating a drug-solution mixture are known.
  • the infusion pumps have the characteristics of continuous, accurate dosing of the medication in its delivery.
  • the pumps are integrated in a common order system, which usually has a central control unit, an operating unit and an alarm unit.
  • a common order system usually has a central control unit, an operating unit and an alarm unit.
  • the transfer of previously prepared medicaments, such as may be present within, for example, a syringe, to the actual infusion pump carries the risk of confusion with regard to the medication and the patient.
  • syringes with drugs contained therein of a predetermined dose assigned by the clinical staff of the wrong infusion pump and thus administered to the wrong patient are examples of a predetermined dose assigned by the clinical staff of the wrong infusion pump and thus administered to the wrong patient.
  • the central idea of the invention is that in a system for administering medicaments to a large number of patients, in particular a selected patient, the following features are present:
  • An identification element generating means for generating at least a first identification element with personal, drug and / or treatment-related first data to attach them to the medicament container; at least one reading device for reading the first data from / from the first identification element;
  • a feeding device for supplying the drug from the drug container inserted into the feeder in the patient;
  • the reading means is also adapted for reading second data mounted on / in a second identification element associated with the patient, and connected to the feeding means;
  • a control unit within the feeder sends a first control signal for activating the feed operation to one of a plurality of feed units of the feeder, provided that the first and second data are at least partially coincident.
  • Such a system is advantageously characterized by the fact that it provides a high level of security in the transfer of a prepared medicament in a dedicated medicine container to a respective delivery device of a patient for the correct assignment, because of the use of identification elements with or on it attached data is no longer possible confusion of such drug containers with the drugs contained therein. Rather, confusion of drugs is ruled out due to reading and comparing different data.
  • the feeder for comparing the first and second data with respect to the personal data match.
  • the person / patient can be compared with the personal data of the second data, which is a wristband or the like associated with the patient, according to the first data already used in drug preparation as an associate for the manufactured drug , can be taken.
  • the control unit would not give a release signal for activating the delivery process.
  • the supply device which may be an insulin pump, can not be activated. Rather, for example, the insulin pump can only be activated if it gets the right drug with the associated drug container, such as a syringe, used and a Identification of personal data on the syringe and on the patient.
  • a second comparison unit within the delivery device is provided for comparing the first medication-related data of the first data with the second medication-related data contained in a storage unit of the delivery device.
  • This allows pre-stored drug compositions within the storage unit of the delivery device to be compared to those drug compositions of the syringe used, thus precluding mis-prepared drugs having unrealistic drug compositions from being administered to the patient.
  • the second comparison unit of the feeder allows the second comparison unit of the feeder to a feeding process only if the first drug-related data of the first data, which are mounted on the identification element of the drug container, within a parameter or data range, which is permissible and suitable for the second drug-related Data is stored within the memory unit.
  • each composition of the previously prepared medicament can be automatically transferred to the identification element by the identification element generating device, so that it is ensured that no components or components are forgotten by the treating person.
  • a selection unit of the delivery device is arranged for at least partially selecting the first and / or second medication-related data.
  • This selector should be used to select the correct drug-related data.
  • parameters of the selected second drug-related data or selected first drug-related data may be altered to be able to administer a modified drug composition at other dosages based on recent events.
  • a disable unit for inhibiting the start of a feed operation is arranged on receipt of a second control signal of the control unit, wherein the control unit transmits this second control signal when the first and second data are not at least partially coincident.
  • Such a blocking unit has the task that the follow-up of the process, which should start a delivery process of a drug to the patient, from the side of the feeder is not possible if non-matching data are detected. This means that the process is automatically aborted by the device unless matching data is detected. This is intended to provide a high degree of security for a correct allocation of the medicament to the respective patient.
  • the identification element is advantageously a barcode label, a data matrix code label and / or a transponder.
  • Data matrix code label is printed from the identification element generating device and in the presence of a transponder, the data is stored by means of a writer on the transponder. This transponder can then be read at the feeder by means of a reader, whereas the barcode label and
  • Datamatrix code label can be read by means of a barcode scanner or data matrix code scanner.
  • the delivery device is an insulin pump
  • the delivery unit is one of a plurality of insulin pump units
  • the medicament container is a syringe.
  • Activating a delivery process for delivering the medicament to the patient with a delivery device by means of a first control signal of a control unit of the delivery device.
  • Such a method allows the automatic or automatic comparison of read data to ensure that only in the presence of a match of the read data, the process continues to be driven and thus a delivery process of the drug to the patient is possible. This results in a high degree of a secure allocation of the right drug to the right patient in order to avoid the administration of wrong medication and thus further unwanted symptoms of the patient.
  • a preferred embodiment of the inventive method is in a step of reading third data, which are attached to / in a third identification element, which is associated with a patient-handling, ensures that only the authorized person to be treated, ie a person from the clinical Personnel performing administration of the drug with proper assignment to the patient. Other persons thus have no access to the administering device, which represents the feeder. As a result, protection against unauthorized operation of the feeder and the delivery of drugs can be achieved.
  • a step of comparing first drug-related data of the first data with second drug-related data stored in a storage unit of the delivery device is provided.
  • the first medication-related data stored on the identification element can be compared with the second medication-related data stored in the storage unit and thus it can be determined whether the drug composition within the medication container corresponds to an approved medication composition range within which the drugs to be administered have to lie.
  • FIGS. 2a and 2b show the method according to the invention according to an embodiment of the invention.
  • Fig. 1 the system according to the invention an Ausstoryunsform of the invention is shown in a schematic representation.
  • a computer device 1 the medicament composition with the associated data relating to the person and the type of treatment is entered by the medical staff by means of a keyboard 2.
  • a barcode 5a is printed, which contains at least parts of the personal, drug and / or treatment-related first data.
  • This barcode is printed as a label and includes, for example as personal data, a patient identification number, as drug-related data, the name of the drug and its concentration, and as treatment-related data, various dosage parameters, such as the duration of the dosage.
  • the barcode label may have the words “Hans Müller”, “adrenaline”, “2mg / 50” in addition to the barcode or the Datamatrix code, meaning that a dosage of 2 mg / 50 for the drug adrenaline, which is within
  • the medicament is prepared in a composition in a medicament preparation station 4 and added to the syringe 5.
  • the barcode 5a is adhesively bonded to the syringe 5 by means of an adhesive surface.
  • the barcode label 5a is read in a reading device 6 by means of a barcode scanner 7, whereby the personal, drug-related and treatment-related data are read in within a feeder 11.
  • the reported data are then forwarded to a first comparison unit 15, which is preferably arranged within the feeder 11.
  • the comparison unit 15 compares the personal data, such as those arranged on the bracelet 10, for example by means of a bar code, with the personal data of the bar code label 5a, as originally produced by the computing unit 1 with reference to the drug produced by the drug preparation station 4 are. If, within the first comparison unit 15, a correspondence of the personal data but also, if necessary, of the drug-related data is determined, a corresponding signal is forwarded by the first comparison unit 15 to a control unit 16, which causes a first control signal 13 to be applied to the inserted infusion syringe by means of a control line 14 for actuating the feeding unit 12b for compressing the syringe 14 is sent.
  • syringes can be arranged in different delivery units 12a, 12b, 12c, 12d and 12e, which can be administered different medicaments for administration. to the patient.
  • the syringe 14 is the syringe 5 containing the drug as previously prepared.
  • the control unit 16 will send a corresponding signal to a blocking unit 20 in order to use this blocking unit to terminate the procedure and block the entire delivery device 11 to obtain.
  • a blocking unit 20 in order to use this blocking unit to terminate the procedure and block the entire delivery device 11 to obtain.
  • third data are read in by a nurse's bracelet containing the personal data of the nurse. These third data serve to compare them with permanently stored data, which may for example be contained in a memory unit 19, in order to establish that the nurse is authorized to operate this device or this supply device 11. If it is determined that this is other personal data, for example, because this nurse is not authorized to administer insulin, the ⁇ inhibit 20> is activated again in accordance with the signals of a control unit, which terminates the feeder operation 1 1 causes.
  • Medication-related data of the first data are compared with drug-related second data contained in the storage unit 19. This comparison takes place within a second comparison unit 18. If it is determined in such a comparison that deviations exist between the medicament-related first data read in by means of the barcode scanner 7 and the medicament-related second data from the storage unit 19, or if the first data lies outside a prescribed range for the second data by means of a display unit and input unit 17, the user or the nurse questions whether they still want to continue the treatment and what data they want to perform. As a result, the nurse may select the first medication-related data or the second medication-related data to continue the treatment.
  • the nurse can perform a change of the parameters on the delivery device 11 by means of the display and input unit 17 in order to change the dosage of the medication to be administered, for example. This is done in continuous data exchange with the control unit 16, which then in turn accordingly controls the feed unit 12b.
  • a corresponding failure signal is sent to the control unit 16 by means of a selection unit 21, which in turn controls the delivery unit 12b with the syringe 14 inserted therein in accordance with the selected data to start or optionally continue the feeding process.
  • a selection unit 21 controls the delivery unit 12b with the syringe 14 inserted therein in accordance with the selected data to start or optionally continue the feeding process.
  • an automated documentation of the administration parameters and of the entire delivery process as well as of the entire procedure is again automatically generated, this preferably taking place in the computer device 1. This can be done according to the arrow 22, for example by means of wireless LAN.
  • FIG. 2a and 2b the inventive method according to an embodiment of the invention is shown.
  • a step 25 the preparation of a drug is carried out according to parameters and data entered.
  • a step 26 an automated generation of a barcode label for application to a medicament container then takes place.
  • step 27 the application of the barcode label as an ID element is subsequently carried out by means of, for example, an adhesive surface.
  • reading 28 of the first data contained on the barcode label, preferably encrypted takes place.
  • a reading of second data 29, which are mounted on a bracelet in the form of a bar code label, preferably encrypted takes place.
  • a comparison between the first and second data is made for their match according to step 30, so as to bring about a termination of the entire process according to step 31 in the event of a mismatch.
  • reading 32 of third data takes place, which in turn is compared in a step 33 with data already stored in the insulin pump. This is data about the attending person to determine if this person is eligible. If this is denied, in a step 34 no admission to the treatment is allowed and a termination takes place.
  • first drug related data is compared to second drug related data for compliance. This is to ensure that not the drug-related data of the barcode, as it is attached to the syringe, unintentionally deviate from intended drug-related data areas that are already stored within the insulin pump. If such a comparison leads to a denial regarding the match, a termination of the process is carried out in a step 36.
  • step 35 If an affirmation of this comparison is determined according to step 35, a release signal is sent to one of the feed units and an activation of the feed operation in a step 37. Subsequently, in a step 38, display of feed parameters takes place.
