Opfindelsen angår en analogifremgangsmåde til fremstilling af en hidtil ukendt gruppe acylderivater af carnitin (β-hydroxy-Y-butyrobetain). Mere specielt angår opfindelsen en sådan fremgangsmåde til fremstilling 5 af acylderivater af carnitin med den almene formel I (se kravet), hvori X betegner en anion, fortrinsvis en halogenid- eller sulfatanion, og R betegner et radikal af en af følgende organiske syrer: (a) acrylsyre, vinyleddikesyre eller allyleddikesyre, 10 (b) 3-cyclohexencarboxylsyre eller 2-cyclopenteneddi- kesyre, (c) methoxyeddikesyre eller ethoxyeddikesyre, (d) 3-carbomethoxypropionsyre eller 4-carbomethoxysmørsyre, eller 15 (2) 2-formylpropionsyre eller formylisosmørsyre, i optisk aktiv eller racemisk form, eller farmaceutisk acceptable syreadditionssalte deraf.
Forbindelserne med formlen I kan fremstilles enten som sådanne eller i form af deres salte med mineralsyrer 20 eller med mono- eller multi-carboxyliske alifatiske og aromatiske syrer eller med sulfon- og sulfonamidsyrer.
Fremgangsmåden ifølge opfindelsen er ejendommelig ved det i kravets kendetegnende del anførte.
Forbindelserne med formlen I og deres farmakolo-25 gisk acceptable salte har generelt vist bemærkelsesværdige cardiotrope, hyperlipoproteinæmiske og hyper-lipidæmiske egenskaber.
Fra fremlæggelsesskrift 148 300 er det kendt, at visse acylderivater af carnitin er egnede som lægemidler 30 til behandling af hjertelidelser og forstyrrelser af li-pidmetabolismen. Forbindelserne, som fremstilles ifølge opfindelsen er imidlertid hidtil ukendte acylderivater, som med hensyn til deres kemiske struktur er helt forskellige fra de fra nævnte fremlæggelsesskrift kendte 35 forbindelser.
Forbindelserne med formlen I fremstilles sædvanligvis i form af deres chlorider.
Som opløsningsmiddel ved fremgangsmådens trin 2

(2) anvendes sædvanligvis trifluoreddikesyre eller en af syrerne (a)-(e), anført under formlen I ovenfor, svarende til det anvendte syrechlorid. Når syrechloridet er fast og ikke let opløses i nævnte organiske opløsningsmiddel 5 er det muligt at forbedre dets opløselighed i så høj grad, at der kan opnås en homogen fase ved at tilsætte en lille mængde af et chloreret opløsningsmiddel, såsom chloroform eller vandfri methylenchlorid.
Der må drages særlig omsorg for at opretholde det 10 vandfri miljø ved at beskytte reaktionssystemet ved hjælp af passende dehydratiseringsmidler, f.eks. CaCl2-holdige rør.
Ved reaktionens afslutning afkøles blandingen og behandles sædvanligvis med acetone. Det faste stof, der 15 herved eventuelt udskilles, fjernes, medens et bundfald, som fremkommer ved tilsætning af ethylether, opsamles. Bundfaldet kan renses ved krystallisation fra ethylether. Sædvanligvis er en eller to krystallisationer tilstrækkelig til opnåelse af et produkt med stor renhed, hvilkel 20 let kan kontrolleres ved tyndtlagschromatografi (T.L.C.) under anvendelse af silicaplader og forskellige elue-ringsmidler, såsom CHCl-j-MeOH-conc.NH^OH (50:30:8 efter volumen) eller n-BuOH-eddikesyre-I^O (60:20:20 efter volumen).
25 I almindelighed giver reaktionen udbytter fra 60-85%,forudsat at den udbytteformindskelse, der eventuelt fås ved krystallisationsrensningen, ikke tages i betragtning.
Fremgangsmåden ifølge opfindelsen illustreres 30 nærmere ved hjælp af følgende eksempler.
Eksempel 1
Fremstilling af vinylacetyl-carnitinchlorid.

og under omrøring sat 1,8 ml, 0,02 mol, vinylacetylchlo-rid, fremstillet ud fra vinyleddikesyre og thionylchlorid.
Reaktionsblandingen blev derpå opvarmet til 50°C natten over. Derpå blev den afkølet til stuetemperatur og udhældt i 200 ml ethylether og omrørt i 20 minutter.
5 Etherfasen blev dekanteret, og det således opnåede bundfald blev optaget i acetonitril og den resulterende opløsning henstillet i 2 timer. Ikke omsat carnitin, der udskiltes i form af et krystallinsk stof, blev filtreret fra, og til filtratet blev sat ethylether.
10 Det bundfald, der dannedes herved var et hvidt krystallinsk stof.

Ved beregning af integralerne af ovenstående karakteristika var det muligt at vurdere mængden af nævnte isomer til at være tilstede i produktet i en 25 mængde på ca. 15-20%.
Eksempel 2
Fremstilling af 3-cyclohexenylcarboxylcarnitinchlorid.
30 (a) Fremstilling af 3-cyclohexen-carboxylsyrechlorid.
3 1,2 cm , 0,01 mo3,3-cyclohexencarboxylsyre blev 3 blandet med 0,9 cm , 0,01 mol, oxalylchlorid og den resulterende blanding omrørt ved stuetemperatur i 3,5 timer. Opløsningen blev derpå koncentreret under vakuum 35 (100 mm Hg, t=70°C). Det herved opnåede råprodukt blev anvendt som sådant i næsten trin.

(b) Omsætning af 3-cyclohexencarboxylchlorid med carni-tinchlorid.
1 g, 0,05 mol, carnitinchlorid blev opløst i 3 2 cm trifluoreddikesyre. Til den resulterende opløsning 5 blev langsomt under omrøring ved stuetemperatur sat 0,01 mol af det i foregående trin fremstillede chlorid.
Den resulterende reaktionsblanding blev omrørt i 15 timer. Derpå blev tilsat ethylether, hvorved der blev opnået et bundfald, der blev vasket to gange med ethyl-10 ether til fjernelse af overskud af syrechlorid.
Bundfaldet blev filtreret og tørret under vakuum. Udbytte 90%.

Til en opløsning af 4 g, 20 mmol, carnitinchlorid 3 i trifluoreddikesyre blev sat 3,0 cm , 24 mmol, 3-carbomethoxypropionylchlorid, og den resulterende reak-35 tionsblanding blev omrørt ved 45°C natten over. Blandingen blev derpå afkølet, og der blev tilsat ethylether.

Til en opløsning af 2,0 g, 10 mmol, carnitinchlorid 3 3 i 5 cm TFA blev tilsat 1,65 cm , 12 mmol, 4-carbo- methoxybutyrylchlorid, og den resulterende blanding 15 omrørt i 24 timer ved stuetemperatur. Til denne blanding blev sat vandfri ethylether. Der dannedes et stof, der blev filtreret fra og krystalliseret fra varm isopropa- nol. Smp. 130-132°C.

Eksempel 5
Fremstilling af methoxyacetylcarnitinchlorid.
En blanding af 3 ml, 0,04 mol, methoxyeddikesyre 30 og 3 ml, 0,03 mol, methoxyacetylchlorid, fremstillet ud fra methoxyeddikesyre og thionylchlorid, blev omrørt ved 30°C i 2 timer. Til denne blanding blev sat 4 g, 0,02 mol, forud tørret carnitinchlorid. Den resulterende blanding blev opvarmet til 40°C og omrørt i 7 døgn.
35 Blandingen blev derpå optaget i acetonitril og ikke omsat carnitinchlorid filtreret fra. Ved tilsætning af tert-butylmethylether udfældedes en olie. Olieudskillelsen blev gentaget fire gange med acetonitril/tert-

6 butylmethylether. Den til slut opnåede olie blev lyofiliseret. 4 g Methoxyacetylcarnitinchlorid blev opnået på denne måde. Udbytte 75%.

Eksempel 6
Fremstilling af ethoxyacetylcarnitinchlorid.
En blanding af 5 ml, 0,07 mol, ethoxyeddikesyre og 1,8 ml, 0,015 mol, etoxyacetylchlorid, der var frem-15 stillet ud fra ethoxyeddikesyre og thionylchlorid, blev omrørt i 2 timer ved 80°C. Til denne blanding blev sat 2 g, stillet som anført i trin (b) i eksempel 5 blev omrørt i 2 timer ved 80°C. Til denne blanding blev sat 2 g, 0,01 mol, carnitinchlorid, der forud var tørret. Den 20 resulterende blanding blev først holdt ved 80°C til fuldstændig opløsning af carnitin var opnået, og derefter ved 40°C i 8 døgn. En yderligere mængde ethoxy-acetylchlorid (0,9 ml, 0,007 mol) blev tilsat og den resulterende blanding holdt ved 40°C i 7 døgn. Ved 25 til blandingen at sætte tert-butylmethylether udskiltes en olie. Olieudskillelsen blev gentaget fire gange med acetonitril/tert-butylmethylether,indtil der var opnået en fuldstændig fjernelse af overskydende ethoxyeddikesyre. På denne måde blev opnået 1,5 g af den i over-30 skriften nævnte forbindelse. Udbytte 75%.

Den farmakologiske virkning for forbindelserne med formlen I illustreres nedenfor.
Den akutte toxicitet af forbindelserne med den almene formel I blev undersøgt hos mus under anvendelse 5 af Weil's metode (Weil C.S., Biometr. J. <8, 249, 1952).
De i tabel 1 nedenfor anførte LDcn-værdier viser, at ou forbindelserne tåles godt.
Den cardiokinetiske virkning blev undersøgt på kaninhjerter isoleret efter Langendorff1 s metode. Kanin-10 hjerter isoleret efter denne metode blev perfuseret med oxygeneret Ringer-opløsning ved 38,2°C. De isometriske kontraktioner, elektrokardiogram og koronarstrømningen blev optegnet under anvendelse af en "Battaglia-Rangoni"-polygraf. Ved at fjerne oxygen fra perfusions-15 væsken blev der fremkaldt metabolisk skade i hjertemusklen til opnåelse af en 80%'s formindskelse af hjertets sammentrækningsstyrke.
Under disse betingelser af langvarig anoxi nedsættes myokardiets glycolyse med tilhørende dannelse 20 af syrekataboliter som følge af både ophobning af pyrodruesyre og dens omdannelse til mælkesyre, der ikke kan udnyttes som følge af undertrykkelse af pyridin-enzymer, såsom LDH (lactodehydrogenase). Dette virker tilbage på den anaerobe glycolyse og påvirker et sta-25 digt stigende antal enzymer ledsaget af fremadskridende og stadigt mere kritisk udmatning af myocardium. Således indtræder en hel serie træthedsniveauer for hjertemusklen, som kan iagttages ud fra de undersøgte parametre, nemlig sammentrækningskraften, koronarstrømningen, 30 hjertefrekvensen og hjerterytmen. Når sammentrækningskraften var reduceret med 80% blev perfusionsvæsken atter oxygeneret enten uden tilsætning af andre forbindelser (kontrolforsøg) eller med tilsætning af de forbindelser, der skulle undersøges.

Forbindelsernes anti-arytmiske virkning blev undersøgt hos mus efter metoden beskrevet af P.W.
Nwangwu og T. Holcslow (P.W. Nwangwu, T.L. Holeslow? 5 Arch. Int. Pharmacodyn. 229, 219 (1977)). Under anvendelse af 5 γ/ml aconitin som arytmogenisk middel blev ændringerne i hjerterytmen forstyrrende optegnet og tidspunktet for fremkomst af initial arytmi og/eller ventrikulær tachycardi blev anvendt som slutpunkt.
10 Resultaterne er anført i tabel III.
Forbindelsernes virkning på lipoproteinmønsteret såvel som på koncentrationerne af plasmakolesterol og -triglycerider ændrede ved oral administrering af olivenr olie blev undersøgt hos normalt fodrede rotter, behand-15 let oralt med olivenolie, 15 ml/kg 1 time før den orale administrering af forbindelsen og med forbindelserne i forskellige koncentrationer.
Den vigtigste virkning 2 timer efter administreringen af olivenolie, der medførte en forøgelse af 20 triglyceriderog kolesterol med reduktion af a-lipo-proteiner og forøgelse af β- og pre-β-fraktioner blev godt modvirket af nogle forbindelser, der, som vist i tabel IV, viste sig at være i stand til at genoprette foregående parametre til det normale, idet der ofte 25 blev nået grænserne for statistisk signifikans.

Claims (1)
- 5 CH3-_> N-CH2-CH-CH2-COOH i X~ OR hvori X betegner en anion, fortrinsvis en halogenid-eller sulfatanion, og R betegner et radikal af en af følgende organiske syrer: 10 (a) acrylsyre, vinyleddikesyre eller allyleddikesyre, (b) 3-cyclohexencarboxylsyre eller 2-cyclopenteneddikesyre, (c) methoxyeddikesyre eller ethoxyeddikesyre, (d) 3-carbomethoxypropionsyre eller 15 4-carbamethoxysmørsyre, eller (e) 2-formylpropionsyre eller formylisosmørsyre, i optisk aktiv eller racemisk form, eller farmaceutisk acceptable syreadditionssalte deraf, kendetegnet ved, at man 20 (1) på i og for sig kendt måde fremstiller chloridet af én af syrerne (a)-(e) og (2) omsætter det fremstillede chlorid med carnitin- chlorid under tilstedeværelse af et organisk opløsningsmiddel ved en temperatur mellem 0° C og 25 80°C, og om ønsket overfører det således fremstil lede acylderivat i et farmaceutisk acceptabelt syreadditionssalt. 30 35
