Description
- Hintergrund der Erfindung
- Die vorliegende Erfindung bezieht sich allgemein auf Gewebeablationsvorrichtungen und insbesondere Vorrichtungen, die geeignet sind, Linien bzw. Bahnen von Gewebe zu ablatieren, beispielsweise zur Verwendung in Verbindung mit einer elektrochirurgischen Variante des Maze-Verfahrens.
- Das Maze-Verfahren ist ein chirurgischer Eingriff bei Patienten mit einem chronischen Vorhofflimmern (VHF), das gegen andere medizinische Behandlungen resistent ist. Die Operation verwendet Schnitte in den rechten und linken Vorhof, welche die Vorhöfe in elektrisch isolierte Abschnitte teilen, was wiederum einen geregelten Durchlauf der Depolarisationswellenfront vom Sinoatrialknoten (SA-Knoten) zum Atrioventrikularknoten (AV-Knoten) zur Folge hat, während gleichzeitig eine Ausbreitung einer neueintretenden Wellenfront verhindert wird. Obwohl mit dem Maze-Verfahren Vorhofflimmern erfolgreich behandelt wird, ist es ziemlich kompliziert und wird derzeit von einer begrenzten Anzahl von hoch spezialisierten Herzchirurgen in Verbindung mit anderen Maßnahmen am offenen Herzen durchgeführt. Aufgrund der Kompliziertheit des operativen Eingriffs gibt es ein zunehmendes Interesse an Verfahren, die elektrochirurgische Instrumente oder andere Typen von Ablationsvorrichtungen, z. B. zur thermischen Ablation, Mikrowellenablation, Kryoablation oder dergleichen, verwenden, um Geweben entlang von Bahnen zu ablatieren, welche die Schnitte des Maze-Verfahrens approximieren. Elektrochirugische Systeme zum Durchführen solcher Verfahren sind in dem
US-Patent Nr. 5,916,213 , erteilt an Hiassaguerre u. a., demUS-Patent Nr. 5,957,961 , erteilt an Maguire u. a., und demUS-Patent Nr. 5,690,661 beschrieben. Kryoablationssysteme zum Durchführen solcher Verfahren sind in demUS-Patent Nr. 5,733,280 , erteilt an Avitall, beschrieben. - Im Zusammenhang mit der Verwendung von elektrochirurgischen Ablationsvorrichtungen sind verschiedene Steuermechanismen entwickelt worden, um die Abgabe von Ablationsenergie so zu steuern, dass das gewünschte Ergebnis der Ablation erreicht wird, d. h. dass Zellen am Ablationsort zerstört werden, während die Grundstruktur des zu ablatierenden Organs intakt gelassen wird. Solche Steuersysteme umfassen die Messung von Temperatur und Impedanz an oder neben dem Ablationsort, wie in dem
US-Patent Nr. 5,540,681 , erteilt an Struhl u. a., und inEP-A-0 870 473 offenbart ist. - Außerdem ist eine substantielle Arbeit geleistet worden, um sicherzustellen, dass das Ablationsverfahren vollständig ist, d. h. dass sich die Ablation durch die Dicke des zu ablatierenden Gewebes erstreckt, bevor die Aufbringung von Ablationsenergie beendet wird. Dieses angestrebte Ergebnis wird mitunter als "transmurale" Ablation bezeichnet. Beispielsweise ist die Erfassung eines gesuchten Abfalls der elektrischen Impedanz am Elektrodenort als Hinweis auf Transmuralität in dem
US-Patent Nr. 5,562,721 , erteilt an Marchlinski u. a., offenbart. - Alternativ sind die Erfassung einer Impedanzzunahme oder einer Impedanzzunahme im Anschluss an eine Impedanzabnahme in dem
US-Patent Nr. 5,558,671 , erteilt an Yates, bzw. in demUS-Patent Nr. 5,540,684 erteilt an Hassler, offenbart. - Da ablatiertes Herzgewebe nekrotisch ist, depolarisiert es nicht und trägt deshalb nicht zum Depolarisationssignal bei. Diese Tatsache hat jüngst dazu geführt, dass einige Ärzte die Amplitude eines lokal gewonnenen Elektrogrammsignals verwenden, um zu bestimmen, ob eine Läsion total ist. Beispielsweise kann der Arzt während der Versorgung mit HF-Energie bei einem konstanten Leistungspegel die Amplitude der Elektrogramme überwachen, die unter Verwendung von Elektroden neben dem Ablationsort erhalten werden, und in Reaktion auf einen definierten Abfall, z. B. 75 %, die Versorgung mit HF-Energie beenden.
- Es sind drei grundlegende Methoden angewendet worden, um mit elektrochirurgischen Vorrichtungen lang gestreckte Läsionen hervorzurufen. Die erste Methode besteht einfach darin, eine Reihe von kurzen Läsionen mit einer Kontaktelektrode hervorzurufen, indem sie die Oberfläche der zu ablatierenden Organwand entlangbewegt wird, um eine lineare Läsion hervorzurufen. Dies kann erreicht werden, indem entweder eine Reihe von Läsionen hervorgerufen wird, wobei die Elektrode zwischen Läsionen bewegt wird oder indem die Elektrode die Oberfläche des zu ablatierenden Organs entlanggezogen wird und kontinuierlich Ablationsenergie aufgebracht wird, wie in dem
US-Patent Nr. 5,897,533 , erteilt an Mulier u. a., beschrieben ist. - Die zweite grundlegende Methode, um lang gestreckte Läsionen hervorzurufen, besteht einfach darin, eine lang gestreckte Elektrode zu verwenden und diese längs der gewünschten Läsionslinie entlang dem Gewebe zu platzieren. Diese Methode ist in dem vorerwähnten
US-Patent Nr. 5,916,213 beschrieben. Die dritte grundlegende Methode, um lang gestreckte Läsionen hervorzurufen, besteht darin, eine Reihe von Elektroden bereitzustellen und die Reihe von Elektroden entlang der gewünschten Läsionslinie anzuordnen. Die Elektroden können einzeln oder der Reihe nach aktiviert werden, wie in dem ebenfalls schon erwähntenUS-Patent Nr. 5,957,961 offenbart ist. Bei Mehrelektroden-Vorrichtungen kann auch eine individuelle Regelung der Ablationsenergie, die über die Elektroden aufgebracht wird, angewendet werden. Es wird die Meinung vertreten, dass die vorliegende Erfindung in Verbindung mit allen drei Methoden zweckmäßig ist. - Zusammenfassung der Erfindung
- Die vorliegende Erfindung zielt auf ein verbessertes System wie im unabhängigen Anspruch definiert zum Hervorrufen von Läsionen und Beurteilen ihrer Totalität bzw. Vollständigkeit oder Transmuralität ab. In der bevorzugten Ausführungsform wie offenbart ist die Vorrichtung zum Erzeugen der Läsionen ein elektrochirurgisches Instrument, insbesondere eine mit physiologischer Lösung gespülte, bipolare elektrochirurgische Zange. Jedoch wird die Meinung vertreten, dass der Mechanismus zum Beurteilen der Transmuralität der Läsion, der durch die vorliegende Erfindung bereitgestellt wird, in anderen Kontexten, einschließlich der unipolaren HF-Ablation und der HF-Ablation unter Verwendung von Kathetern oder in der Hand gehaltenen Sonden, zweckmäßig ist. Der Mechanismus zum Beurteilen der Transmuralität kann auch im Kontext anderer Typen von Ablationssystemen, einschließlich jener, bei denen die Ablation in Verbindung mit einem hervorgerufenen Anstieg der Gewebetemperatur stattfindet, wie etwa jenen, die Ablationsenergie in der Form von Mikrowellen, Licht (Laserablation) oder Wärme (thermische Ablation) aufbringen, nützlich sein. Außerdem kann die Erfindung in Verbindung mit anderen Typen der Ablation, einschließlich der Kryoablation, der Ultraschallablation und der chemischen Ablation, angewendet werden.
- Gemäß der vorliegenden Erfindung erfolgt die Beurteilung der Transmuralität einer Läsion durch Überwachen der Depolarisationssignalamplitude in einem lokalen Elektrogramm, das mit Elektroden aufgenommen wird, die sich neben dem zu ablatierenden Gewebe befinden. Im Kontext der HF-Ablation kann die Messung der Elektrogrammamplitude mit Ablationselektroden erfolgen oder kann mit speziellen Elektroden neben den Ablationselektroden erfolgen. Im Kontext anderer Typen der Ablation, wie schon erörtert, würde eine Elektrogrammmessung üblicherweise mittels eines speziellen Satzes Messelektroden ausgeführt werden.
- Im Anschluss an den Beginn der Aufbringung von Ablationsenergie auf Herzgewebe fällt die Amplitude eines lokalen Elektrogramms, das mit Elektroden gemessen ist, die sich neben dem zu ablatierenden Gewebe befinden, zunächst allmählich ab und stabilisiert sich dann, was darauf schließen lässt, dass das Gewebe, das überwacht wird, keinen Beitrag mehr zu dem erfassten Elektrogramm liefert. Durch die vorliegende Erfindung ist es möglich, den Amplitudenabfall (ΔEGM) oder das anschließende Amplituden-Plateau "P" allein oder zusammen als Indikatoren für die Transmuralität zu verwenden. Der Amplitudenabfall kann mit einem vorher festgesetzten Wert verglichen werden (ΔEGM ? a). Das Plateau "P" kann in Reaktion auf eine Bestimmung, dass über eine Reihe von Amplitudenmessungen oder über eine definierte Dauer (|dA/dt| = b) die Änderungsgeschwindigkeit der Amplitude niedriger als ein definierter Wert ist, erfasst werden. Bei einigen Ausführungsformen kann die Erfassung einer hohen Geschwindigkeit des Abfalls der Amplitude (dA/dt = d) als ein Indikator dafür verwendet werden, dass der Ablationsvorgang zu schnell fortschreitet, und kann verwendet werden, um eine Verringerung des Betrags der aufgebrachten Ablationsenergie auszulösen. Bei weiteren Ausführungsformen kann die Er fassuNg einer zu geringen Geschwindigkeit der Abnahme der Amplitude (dA/dt = d) als ein Indikator dafür verwendet werden, dass der Ablationsvorgang zu langsam fortschreitet, und kann verwendet werden, um eine Erhöhung des Betrags der aufgebrachten Ablationsenergie auszulösen.
- Es wird die Meinung vertreten, dass die Erfindung im Kontext der HF-Ablation in Verbindung mit einer Ablationsvorrichtung nützlich ist, die einzeln aktivierbare Elektroden oder Elektrodenpaare aufweist, die entlang einer gewünschten Läsionslinie anzuordnen sind. In diesem Kontext kann der Mechanismus zum bestimmen der Transmuralität von Läsionen neben einzelnen Elektroden oder Paaren verwendet werden, um einzelne Elektroden oder Elektrodenpaare zu deaktivieren, wenn die Läsionen in dem Gewebe neben diesen einzelnen Elektroden oder Elektrodenpaaren total sind. Dies ermöglicht das Hervorrufen einer im Wesentlichen gleichmäßigen Läsion entlang der Linie der Elektroden oder Elektrodenpaare ungeachtet der Unterschiede in der Gewebedicke neben den einzelnen Elektroden oder Elektrodenpaaren. Außerdem wird die Meinung vertreten, dass die Erfindung im Zusammenhang mit der Beurteilung der Transmuralität von Läsionen, die durch Vorrichtungen mit nur einer einzigen Elektrode oder nur einem einzigen Elektrodenpaar hervorgerufen werden, zweckmäßig ist. Ähnliche Erwägungen betreffen die Anwendung der vorliegenden Erfindung in den Kontexten der anderen Typen von Ablationen, die oben aufgeführt sind.
- Im Folgenden werden bevorzugte Ausführungsformen beispielhaft, mit Bezug auf die Zeichnung beschrieben.

1 ist ein Grundriss einer bipolaren, mit physiologischer Kochsalzlösung gespülten elektrochirurgischen Gefäßklemme eines Typs, der in Verbindung mit der vorliegenden Erfindung verwendet werden kann. Die Gefäßklemme ist mit lang gestreckten Griffen11 und12 und einem Verriegelungsmechanismus14 ähnlich einer herkömmlichen chirurgischen Gefäßklemme versehen. Die Griffe sind durch einen Zapfen oder ein Drehgelenk16 miteinander verbunden und setzen sich distal in Form von lang gestreckten Klemmbacken18 und19 fort. Die Klemmbacken18 und19 tragen eine lang gestreckte Elektrode bzw. Reihen von Elektroden24 ,25 , auf die Ablationsenergie, z. B. HF-Energie mit Hilfe von Stromleitern21 und22 aufgebracht wird. Die Elektroden sind dafür eingerichtet, dass sie über ihre Länge mit physiologischer Kochsalzlösung oder einem anderen leitfähigen Fluid, das über Einlassstutzen20 und23 geliefert wird, gespült werden. Im Betrieb wird zu ablatierendes Gewebe zwischen den Klemmbacken zusammengepresst, und zwischen der Elektrode oder den Elektrodensätzen24 ,25 wird HF-Energie aufgebracht, wie allge mein in demUS-Patent Nr. 6,096,037 , erteilt an Mulier u. a., beschrieben ist. -
2a zeigt eine erste Ausführungsform einer Elektrodenanordnung für eine Gefäßklemme, im Großen und Ganzen wie in1 veranschaulicht. Die dargestellten Einzelteile entsprechen identisch nummerierten Einzelteilen in1 . In dieser Ausführungsform nehmen die Elektroden24 und25 die Form von lang gestreckten Wickelelektroden30 und32 an, die um poröse Hülsen34 und36 herum, durch welche physiologische Kochsalzlösung oder ein anderes leitfähiges Fluid abgegeben wird, angebracht sind. Die Anordnung der Elektroden kann auch umgekehrt sein, wobei beispielsweise Wicklungen30 ,32 im Inneren von lang gestreckten porösen Hülsen34 und36 platziert sind, um ein ähnliches Ergebnis zu erreichen. Alternativ kann jede andere Anordnung zum Bereitstellen einer lang gestreckten Elektrode und Abgeben von physiologischer Kochsalzlösung über der Länge der Elektrode eingesetzt werden. Die besondere Gestaltung der Elektrode ist für die vorliegende Erfindung unkritisch. Beispielsweise können auch gespülte Elektroden eingesetzt werden, die jenen entsprechen, die in demUS-Patent Nr. 6,096,037 erteilt an Mulier u. a., demUS-Patent Nr. 5,876,398 , erteilt an Mulier u. a., demUS-Patent Nr. 6,017,378 , erteilt an Brucker u. a. oder demUS-Patent Nr. 5,913,856 , erteilt an Chia u. a., beschrieben sind. Außerdem sei angemerkt, dass zwar das Elektrodensystem wie in2a veranschaulicht ein bipolares System ist, trotzdem die Erfindung in Verbindung mit unipolaren Elektroden und/oder in der Form einer Sonde oder eines Katheders angewendet werden kann. Bei einigen Ausführungsformen kann das Spülen der Elektroden unterlassen werden. -
2b veranschaulicht eine alternative Ausführungsform eines Elektrodensystems für eine Gefäßklemme, im Großen und Ganzen wie in1 veranschaulicht. In diesem Fall sind statt eines einzigen Paares Elektroden mehrere Elektrodenpaare vorgesehen. Die Elektrodenpaare umfassen Wickelelektroden40 und42 ,44 und46 ,48 und50 ,52 und54 sowie56 und58 . Es könnten jedoch andere Paarungen von Elektroden eingesetzt werden, beispielsweise Paarungen der Elektroden50 und44 , der Elektroden48 und52 oder dergleichen. Bei dieser Ausführungsform sind die Elektrodenpaare um poröse Kunststoffhülsen60 und62 angebracht, durch welche physiologische Kochsalzlösung oder ein anderes elektrisch leitfähiges Fluid abgegeben wird. Wie in dem Fall mit der Ausführungsform von2a kann die Anordnung dieser Elektroden ohne weiteres umgekehrt werden, wobei die Elektroden im Innenraum der Kunststoffhülse60 oder62 platziert werden, wobei auch jede andere Anordnung, die mehrere gespülte Elektroden bereitstellt, eingesetzt werden kann. Wie im Fall der Ausführungsform von2a könnten die mehreren bipolaren Elektrodenpaare, wie veranschaulicht, durch unipolare Elektroden ersetzt werden und/oder die Erfindung könnte in Verbindung mit einer Mehrelektrodensonde oder einem Mehrelektrodenkatheder ausgeübt werden. - Im Gebrauch ist die Gefäßklemme so angeordnet, dass sich das zu ablatierende Gewebe zwischen den Klemmbacken
18 und19 befindet, und es wird Druck ausgeübt, um das Gewebe zwischen den Klemmbacken leicht zusammenzudrücken, damit ein guter elektrischer Kontakt sichergestellt ist. Alle Elektrodenpaare können einzeln aktiviert werden und können einzeln deaktiviert werden, wenn die Läsionen zwischen den einzelnen Elektrodenpaaren vollständig transmural sind. Alternativ könnten die Elektrodenpaare nacheinander aktiviert werden, wobei ein Paar nach einem Erfassen einer totalen Läsion zwischen den beiden Elektroden deaktiviert wird, gefolgt von der Aktivierung des nächstfolgenden Elektrodenpaares. Eine entsprechende Anwendung der Erfindung in Verbindung mit einer Reihe von unipolaren Elektroden, beispielsweise Elektroden längs einer der zwei Klemmbacken zusammen mit einer entfernten Masse-Platte oder einer ähnlichen Reihe von einzeln aktivierbaren Elektroden an einem Katheder oder einer Sonde zusammen mit einer Masse-Platte entsprechend, ist ebenfalls möglich. -
3 ist ein Diagramm, das die lokale Elektrogrammamplitude "A" gegen die Zeit, gemessen über Gewebe, das sich zwischen den Elektroden einer gespülten, bipolaren Gefäßklemme wie in1 gezeigt befindet, veranschaulicht.3 veranschaulicht den Abfall der Elektrogrammamplitude, gefolgt von einem Amplituden-Plateau. Durch die vorliegende Erfindung ist es möglich, den Amplitudenabfall (ΔEGM) oder das anschließende Amplituden-Plateau "P" allein oder zusammen als Indikatoren für die Transmuralität zu verwenden. Bei einigen Ausführungsformen kann die Erfassung einer hohen Geschwindigkeit des Abfalls der Amplitude (dA/dt = d) als ein Indikator dafür verwendet werden, dass der Ablationsvorgang zu schnell fortschreitet, und kann verwendet werden, um eine Verringerung des Betrags der aufgebrachten Ablationsenergie auszulösen. Bei weiteren Ausführungsformen kann die Erfassung einer zu geringen Geschwindigkeit der Abnahme der Amplitude (dA/dt = d) als ein Indikator dafür verwendet werden, dass der Ablationsvorgang zu langsam fortschreitet, und kann verwendet werden, um eine Erhöhung des Betrags der aufgebrachten Ablationsenergie auszulösen. -
4 ist ein Funktionsblockdiagramm, das eine Ausführungsform eines HF-Generatorsystems zur Verwendung in Verbindung mit der vorliegenden Erfindung veranschaulicht. Bei dieser Ausführungsform sind separat steuerbare HF-Ausgänge für einzelne Ablationselektroden oder -elektrodenpaare an einer zugeordneten HF-Ablationsvorrichtung, beispielsweise wie in2B , vorgesehen. Der HF-Generator könnte selbstverständlich auch mit Ablationsvorrichtungen verwendet werden, die wie in2A nur eine einzige Elektrode oder nur ein einziges Elektrodenpaar aufweisen. Mit der Ausnahme der nachstehend erörterten Elektrogrammamplitudenmessschaltungen entspricht der Generator im Großen und Ganzen jenem, der in Verbindung mit16 des oben angeführten '961-er Patents, erteilt an Maguire u. a., beschrieben wurde. Der in dem '961-er Patent offenbarte HF-Generator bietet eine Regelung der HF-Leistung anhand entweder der gemessenen Leistung (konstante Leistung) oder der gemessenen Temperatur. Die vorliegende Erfindung ist in Verbindung mit der Betriebsart konstanter Leistung etwas einfacher umzusetzen, sie kann aber auch an eine temperaturregulierte Betriebsart oder an einen anderen Leistungsregelungsmechanismus angepasst werden. - Die Anzeige
804 und die Steuerungen bzw. Regler802 sind mit einem digitalen Mikroprozessor800 verbunden, der eine Schnittstelle zwischen dem Benutzer und den übrigen elektrischen Bauelementen des Systems ermöglicht. Der Mikroprozessor800 arbeitet unter der Steuerung durch eine gespeicherte Programmierung, die seinen Betrieb festlegt, eingeschlossen eine Programmierung, die seinen Betrieb entsprechend der vorliegenden Erfindung steuert, wie nachstehend ausführlicher erörtert ist. Der Mikroprozessor800 liefert über den Adress/Daten-Guss806 Steuerungsausgaben an die restliche Schaltungsanordnung, von der er auf gleichem Wege Eingangssignale empfängt. Insbesondere sorgt der Mikroprozessor800 für die Überwachung der Leistung, des Stroms, der Spannung, der Elektrogrammamplitude und der Temperatur. Nach Bedarf wird der Mikroprozessor diese Informationen an die Anzeige804 liefern. Außerdem ermöglicht der Mikroprozessor800 dem Benutzer, die Regelungsart (entweder Temperatur oder Leistung) auszuwählen und den Leistungssollwert, den Temperatursollwert und eine Zeitvorgabe in das System einzugeben. Die Primärquelle der Leistung für den Hochfrequenzgenerator kann eine 12-V-Batterie mit nominell 7,2 Amperestunden sein, oder die Vorrichtung kann wechselstromgespeist sein. Außerdem kann eine (nicht gezeigte) Stützbatterie, wie etwa eine Lithiumzelle, bereitgestellt sein, um den Mikroprozessor260 mit ausreichend Energie zu versorgen, damit gewünschte Speicherfunktionen aufrechterhalten werden, wenn die Hauptenergieversorgung unterbrochen ist. - Das Energieversorgungssystem wie veranschaulicht umfasst eine gewünschte Anzahl "M" von einzeln steuerbaren HF-Energieversorgungen und empfängt Temperatureingaben von einer gewünschten Anzahl "N" von Temperaturmesseinrichtungen in der Ablationsvorrichtung, schematisch als
838 dargstellt, und empfängt Elektrogrammamplitudeneingaben von einer gewünschten Anzahl "M" von Elektrogrammüberwachungsschaltungen. Jede HF-Energieversorgung umfasst einen Transformator (822 ,824 ,826 ), eine Leistungssteuerschaltung (810 ,812 ,814 ) und eine Leistungsmessschaltung (816 ,818 ,820 ). Ein quarzgesteuerter Hochfrequenzoszillator264 erzeugt die Schaltimpulse, die sowohl die Leistungstransformatoren (822 ,824 ,826 ) als auch die Leistungssteuerschaltungen (810 ,812 ,814 ) treiben. Die Leistungssteuerschaltungen (810 ,812 ,814 ) können analoge Steuereinheiten sein, die mittels Pulsbreitenmodulation arbeiten, indem sie ein Leistungssollwertsignal vom Mikroprozessor800 mit einem Istleistungssignal vergleichen, das von einer Leistungsmessschaltung (816 ,818 ,820 ) erzeugt ist, die beispielsweise einen Ringtransformator enthalten kann, der an die Leistungsabgabeseite des zugeordneten Transformators (822 ,824 ,826 ) gekoppelt ist. Die Leistungsmessschaltungen (816 ,818 ,820 ) vervielfachen den Ausgangsstrom und die Ausgangsspannung und liefern das resultierende Istleistungssignal sowohl an die Leistungssteuereinheiten (810 ,812 ,814 ), als auch an den Mikroprozessor800 . - Die HF-Leistungsabgabe der Transformatoren (
822 ,824 ,826 ) wird an eine Schnittstellenkarte808 geliefert und wird dadurch an die Ablationselektrode oder -elektroden an der Ablationsvorrichtung838 geliefert. Außerdem können separate analoge Komparatorschaltungen (nicht dargestellt) für ein Überwachen der Ausgabe der Leistungsmessschaltungen (816 ,818 ,820 ) bereitgestellt sein, um den Strom zu den Ausgangstransformatoren (822 ,824 ,826 ) zu unterbrechen, falls die Leistung eine Grenze von üblicherweise 55 Watt überschreitet. Die Leistungstransformatoren (822 ,824 ,826 ) können Mittelabgriffe aufweisen, welche die Ausgaben der Leistungssteuereinheiten (810 ,812 ,814 ) empfangen. Sekundärwicklungen der Transformatoren (822 ,824 ,826 ) können für eine kontinuierliche Überwachung der anliegenden Spannung sorgen, um die Leistungsberechnungen durch die Leistungsmessschaltungen (816 ,818 ,820 ) zu ermöglichen. - Das veranschaulichte HF-Leistungserzeugungssystem verwendet eine softwaregesteuerte Temperaturverarbeitung, die durch den Mikroprozessor
800 ausgeführt wird, der die "N" Temperatureingangssignale von Temperaturmessschaltungen (828 ,830 ,832 ) empfängt, wovon jede mittels eines elektrischen Verbinders, schematisch unter836 dargestellt, und einer Schnittstellenschaltung834 an einen entsprechenden Temperatursensor in der Ablationsvorrichtung838 gekoppelt ist. Wenn der Prozessor800 so programmiert ist, dass er in der temperaturgeregelten Betriebsart arbeitet, empfängt er die "N" Temperatursignale und legt anhand der angegebenen Temperaturen Leistungssollwerte für jede der Leistungssteuerschaltungen (810 ,812 ,814 ) fest, die in der schon beschriebenen Weise die Leistungs pegel steuern, die durch die Schnittstelle834 auf die Elektroden am Katheter aufgebracht werden. Außerdem kann der Prozessor800 in Reaktion auf externe Steuersignale von den Reglern802 oder in Reaktion auf erfasste anomale Temperaturbedingungen wahlweise jede der "M" bereitgestellten HF-Energieversorgungen ein- oder ausschalten. - Zusätzlich zu der Schaltungsanordnung wie schon beschrieben und in dem '961-er Patent an Maguire u. a offenbart, umfasst die Vorrichtung von
4 mehrere Elektrogrammüberwachungsschaltungen EGM1, EGM2, ...EGMM (843 ,845 bzw.847 ), die einen oder mehrere Spitzenwertdetektoren enthalten können, die an Abtast-Halte-Schaltungen gekoppelt sind, und im Großen und Ganzen wie in demUS-Patent Nr. 6,266,566 , erteilt an Nichols u. a., demUS-Patent Nr. 6,029,986 , erteilt an Kim u. a., demUS-Patent Nr. 6,095,150 , erteilt an Panescue u. a., oder demUS-Patent Nr. 5,685,315 , erteilt an McClure u. a., beschrieben arbeiten. - Die Elektrogrammüberwachungsschaltungen messen Elektrogrammamplituden, die unter Verwendung der Elektroden an der HF-Ablationsvorrichtung erfasst werden. Die gemessenen Amplituden können Spitze-Spitze-Messungen der Depolarisationswellen-Amplituden oder absolute Spitzenwertmessungen, positive Spitzenwerte oder negative Spitzenwerte sein. Die Amplitudenmesswerte, die bei der Ausübung der Erfindung verwendet werden, können einzeln gemessene Werte oder digital gefilterte Werte, die durch Mitteln von einer Reihe von einzeln gemessenen Amplituden erhalten werden, sein. Die Elektrogrammsignale von den Elektroden an der Ablationsvorrichtung
838 können zunächst durch Tiefpassfilter F1, F2, ...FN (842 ,844 ,846 ) gefiltert werden und können zwischen den Ablationselektroden oder zwischen Elektroden, die sich neben den Ablationselektroden befinden, gemessen sein. Die Messungen werden vor zugsweise während Unterbrechungen bei der Abgabe von Ablationsenergie an die Ablationselektroden vorgenommen, um Probleme bezüglich der Erfassung von Störungen zu minimieren. Optional kann eine EGM-Triggerschaltung880 , die an Oberflächen-EGM-Elektroden852 und854 gekoppelt ist, die Elektrogrammmessung triggern. - Einzelne bzw. individuelle Elektrogrammamplitudenmesswerte, die durch die Messschaltungen
843 ,845 und847 erzeugt sind, werden an den Adress/Daten-Bus806 und von dort an den Mikroprozessor800 für eine Analyse geliefert, um zu bestimmen, ob der Verlauf der gemessenen Elektrogrammamplitude über der Zeit angibt, dass die Läsion, die den gemessenen Amplituden zugeordnet ist, vollständig transmural ist. Wie nachstehend ausführlicher erörtert ist, kann eine Bestimmung der Transmuralität in Reaktion auf eine Erfassung eines definierten Abfalls der Elektrogrammamplitude und/oder einer Reihe von Amplitudenmesswerten, die über eine gewünschte Zeitdauer oder über eine definierte Anzahl von aufeinanderfolgenden Amplitudenmessungen relativ konstant sind, erfolgen. Bei einigen Ausführungsformen kann ein abrupter Abfall der Elektrogrammamplitude außerdem verwendet werden, um den Leistungspegel der Ablationsenergie, die an das überwachte Gewebe abgegeben wird, herabzusetzen. - In Fällen, in denen eine alternative Ablationsenergie erzeugende Vorrichtung verwendet wird, insbesondere jenen, in denen ein Anstieg der Gewebetemperatur herbeigeführt wird, z. B. bei der Laser-, Mikrowellen- oder thermischen Ablation, würde die HF-Generatorschaltungsanordnung von
4 durch eine entsprechende alternative Ablationsenergie erzeugende Vorrichtung ersetzt werden. Die Messung der Elektrogrammamplitude und ihre Verwendung gemäß der Erfindung können in Verbindung mit diesen alternativen Ablationsenergie erzeugenden Systemen jedoch nach wie vor zweckmäßig sein. Ebenso können die Messung der Elektrogrammamplitude und ihre Verwendung gemäß der vorliegenden Erfindung auch in Verbindung mit anderen Formen der Ablation, wie etwa der Kryoablation, der Ultraschallablation und der chemischen Ablation, zweckmäßig sein. -
5 ist ein Funktionsablaufplan, der die Funktionsweise einer Vorrichtung wie in4 gemäß der vorliegenden Erfindung veranschaulicht. Der Ablaufplan von5 veranschaulicht die Funktionsweise der Vorrichtung von4 , um die Versorgung einer einzelnen Elektrode oder eines einzelnen Elektrodenpaares mit HF-Energie zu steuern. Falls mehrere Elektroden oder Elektrodenpaare verwendet werden, würde die Steuermethodik von5 auf jede Elektrode oder jedes Elektrodenpaar individuell angewendet werden, wie nachstehend in Verbindung mit10 und11 ausführlicher erörtert ist. - Der Ablaufplan von
5 veranschaulicht ein Verfahren zum Beurteilen der Transmuralität und Beenden der Abgabe von Ablationsenergie an eine Elektrode oder ein Elektrodenpaar in Reaktion auf eine Erfassung eines spezifizierten Abfalls der Elektrogrammamplitude, z. B. eines 75 %-Abfalls. Im Anschluss an die Erfassung des gesuchten Amplitudenabfalls kann die Vorrichtung eine festgelegte Zeitdauer warten, um die Vollendung der Läsion sicherzustellen, und dann die Aufbringung von Ablationsenergie auf das zugeordnete Elektrodenpaar beenden. Alternativ kann das Beenden der Aufbringung von Ablationsenergie gleichzeitig mit der Erfassung des gesuchten Amplitudenabfalls erfolgen. Das Messen der Elektrogrammamplitude im Gewebe neben der Ablationselektrode oder dem Ablationselektrodenpaar kann unter Verwendung der Ablationselektroden selbst oder unter Verwendung von Elektroden, die sich in der Nähe der Ablationselektroden befinden, beispielsweise jenen entsprechend, die zur Messung der Impedanz verwendet werden, wie in dem '671-er Patent an Yates beschrieben, erfolgen. - Nach einer Initialisierung bei
200 veranlasst der Mikroprozessor800 (4 ) die Elektrogrammmessschaltungsanordnung, die der Elektrode oder dem Elektrodenpaar, die bzw. das beurteilt wird, zugeordnet ist, eine Basislinie oder einen Anfangsamplitudenwert EGMi aufzunehmen,202 . Der Mikroprozessor beginnt dann bei204 mit der Aufbringung von Ablationsenergie auf das überwachte Gewebe. Während der Aufbringung von Ablationsenergie erhält und speichert der Mikroprozessor in festgelegten Intervallen oder in Reaktion auf Triggersignale von einer EGM-Triggerschaltung (850 ,4 ) Elektrogrammmesswerte,206 . Die Abgabe von Ablationsenergie kann während der Elektrogrammmessperiode unterbrochen sein. Bei jedem erhaltenen Amplitudenmesswert bestimmt der Prozessor, ob der gesuchte Amplitudenabfall aufgetreten ist. Diese Bestimmung kann beispielsweise in Reaktion auf den ersten Amplitudenmesswert unterhalb eines vorgegebenen Wertes für den gesuchten Abfall, "a", z. B. –75 %, auf eine Reihe von erforderlichen gemessenen Amplituden unterhalb des Wertes für den gesuchten Abfall, z. B. 2 oder 3 Messwerte, oder auf ein gefordertes Verhältnis der gemessenen Amplituden unterhalb des Wertes für den gesuchten Abfall, z. B. 2 von 3 Messwerten, erfolgen. Wie schon erwähnt wurde, können alternativ bei jeder Messung gemittelte Amplituden berechnet werden, wobei in diesem Fall ein Abfall der gemittelten Amplitude verwendet werden kann, um Transmuralität zu erfassen. Die Ablation und die Elektrogrammmessung werden fortgesetzt, bis bei208 der gesuchte Abfall erfasst worden ist. Das Beenden der Aufbringung von Ablationsenergie auf das Gewebe, das überwacht wird, erfolgt dann bei210 . Das Beenden der Ablation kann gleichzeitig mit der Erfassung des gesuchten Abfalls oder nach einer vorgegebenen Zeitverzögerung, um eine vollständige Transmuralität sicherzustellen, erfolgen. -
6 ist ein Funktionsablaufplan, der die Funktionsweise einer Vorrichtung wie in4 gemäß einer zweiten Ausführungsform der vorliegenden Erfindung veranschaulicht. Der Ablaufplan von6 veranschaulicht die Funktionsweise der Vorrichtung von4 , um die Versorgung einer einzelnen Elektrode oder eines einzelnen Elektrodenpaares mit HF-Energie zu steuern. Falls mehrere Elektroden oder Elektrodenpaare verwendet werden, würde die Steuermethodik von6 auf jede Elektrode oder jedes Elektrodenpaar individuell angewendet werden, wie nachstehend in Verbindung mit10 und11 ausführlicher erörtert ist. - Der Ablaufplan von
6 veranschaulicht ein Verfahren zum Beurteilen der Transmuralität und Beenden der Abgabe von Ablationsenergie an eine Elektrode oder ein Elektrodenpaar in Reaktion auf eine Erfassung eines spezifizierten Abfalls der Elektrogrammamplitude, z. B. eines 75 %-Abfalls, in Verbindung mit der Erfassung eines Elektrogrammamplituden-Plateaus. Im Anschluss an die Erfassung des gesuchten Amplitudenabfalls und -plateaus kann die Vorrichtung eine festgelegte Zeitdauer warten, um die Vollendung der Läsion sicherzustellen, und dann die Aufbringung von Ablationsenergie auf das zugeordnete Elektrodenpaar beenden. Alternativ kann das Beenden der Aufbringung von Ablationsenergie gleichzeitig mit der Erfassung des gesuchten Amplitudenabfalls und -plateaus erfolgen. Das Messen der Elektrogrammamplitude im Gewebe neben der Ablationselektrode oder dem Elektrodenpaar kann unter Verwendung der Ablationselektroden selbst oder unter Verwendung von Elektroden, die sich in der Nähe der Ablationselektroden befinden, vorgenommen werden. - Nach einer Initialisierung bei
300 veranlasst der Mikroprozessor800 (4 ) die Elektrogrammmessschaltungsanordnung, die der Elektrode oder dem Elektrodenpaar, die bzw. das beurteilt wird, zugeordnet ist, eine Basislinie oder einen Anfangsamplitudenwert EGMi aufzunehmen,302 . Der Mikroprozessor beginnt dann mit der Aufbringung von Ablationsenergie auf das überwachte Gewebe,304 . Während der Aufbringung von Ablationsenergie erhält und speichert der Mikroprozessor Elektrogrammmesswerte,306 , wie schon in Verbindung mit5 erörtert. Bei jedem erhaltenen Amplitudenmesswert bestimmt der Prozessor, ob der gesuchte Amplitudenabfall aufgetreten ist,308 . Diese Bestimmung kann wie schon in Verbindung mit5 beschrieben durchgeführt werden. - Wenn bei
308 der gesuchte Abfall erfasst worden ist, verwendet der Mikroprozessor800 bei310 die gespeicherten Elektrogrammamplitudenmesswerte, um dA/dt zu berechnen, was beispielsweise basierend auf der Nettoänderung der Elektrogrammamplitude über eine Reihe von 2 oder 3 Messwerten berechnet werden kann. Wie schon in Verbindung mit der Erfassung des gesuchten Amplitudenabfalls erörtert wurde, können auch gemittelte Amplitudenwerte verwendet werden, um dA/dt zu berechnen. Der Absolutbetrag von dA/dt, d h. |dA/dt|, kann verwendet werden, um bei310 zu beurteilen, ob ein Elektrogrammamplituden-Plateau erreicht worden ist, indem beispielsweise verifiziert wird, dass eine Reihe von Werten von |dA/dt| vollständig (z. B. 3 von 3) oder überwiegend (z. B. 2 von 3) unterhalb eines festgelegten Variabilitätswerts "b" ist. - Der Prozessor fährt fort, Amplitudenmesswerte zu erfassen, und führt Berechnungen durch, bis bei
310 ein Amplituden-Plateau erkannt wird und bei308 ein ausreichender Amplitudenabfall erkannt wird. Wenn diese beiden Krite rien erfüllt sind, erfolgt dann das Beenden der Aufbringung von Ablationsenergie auf das Gewebe, das überwacht wird, bei312 . Das Beenden der Ablation kann gleichzeitig mit der Erfassung des gesuchten Abfalls oder nach einer vorgegebenen Zeitverzögerung, um eine vollständige Transmuralität sicherzustellen, erfolgen. -
7 ist ein Funktionsablaufplan, der eine optionale, zusätzliche Menge von Operationen zum Ausführen eines Transmuralitätsmessverfahrens, das im Großen und Ganzen wie in6 ist, veranschaulicht. Die zusätzlichen Operationen sorgen für eine Erhöhung der Ablationsenergie in Reaktion auf eine Erfassung eines Elektrogrammamplituden-Plateaus bei310 (6 ), um zu verifizieren, dass die Ablation vollständig ist. Der Prozessor triggert eine Erhöhung der Leistung bei420 , beispielsweise um 5 bis 25 %, und startet bei422 eine Frist von einigen Sekunden. Das Messen und Berechnen von Elektrogrammamplituden wird bei424 fortgesetzt, bis entweder bei426 ein deutlicher Abfall der Amplitude erfasst wird oder bei428 der Zeitnehmer abgelaufen ist. Falls ein deutlicher Abfall erfasst wird, beispielsweise in der gleichen Größenordnung wie der Abfall, der bei310 eine Plateau-Erfassung verhindert hätte (6 ), startet der Prozessor den Prozess der Erfassung eines Plateaus bei306 (6 ) erneut. Wenn die Frist ohne einen weiteren deutlichen Abfall der Elektrogrammamplitude abläuft, beendet der Prozessor den Ablationsvorgang bei312 (6 ). -
8 ist ein Funktionsablaufplan, der die Funktionsweise einer Vorrichtung wie in4 gemäß einer weiteren alternativen Ausführungsform der vorliegenden Erfindung veranschaulicht. Der Ablaufplan von8 veranschaulicht die Funktionsweise der Vorrichtung von4 , um die Versorgung einer einzelnen Elektrode oder eines einzelnen Elektrodenpaares mit HF-Energie zu steuern. - Falls mehrere Elektroden oder Elektrodenpaare verwendet werden, würde die Steuermethodik von
8 auf jede Elektrode oder jedes Elektrodenpaar individuell angewendet werden, wie nachstehend in Verbindung mit10 und11 ausführlicher erörtert ist. - Der Ablaufplan von
8 veranschaulicht ein Verfahren zum Beurteilen der Transmuralität und Beenden einer Abgabe von Ablationsenergie an eine Elektrode oder ein Elektrodenpaar in Reaktion auf eine Erfassung eines Elektrogrammamplituden-Plateaus in Verbindung mit einer festgelegten minimalen Ablationsdauer. Im Anschluss an die Erfassung des gesuchten Plateaus kann die Vorrichtung eine festgelegte Zeitdauer warten, um die Vollendung der Läsion sicherzustellen, und dann die Aufbringung von Ablationsenergie auf das zugeordnete Elektrodenpaar beenden. Alternativ kann das Beenden der Aufbringung von Ablationsenergie gleichzeitig mit der Erfassung des gesuchten Amplitudenabfalls und -plateaus erfolgen. Das Messen der Elektrogrammamplitude im Gewebe neben der Ablationselektrode oder dem Elektrodenpaar kann unter Verwendung der Ablationselektroden selbst oder unter Verwendung von Elektroden, die sich in der Nähe der Ablationselektroden befinden, vorgenommen werden. - Nach einer Initialisierung bei
400 veranlasst der Mikroprozessor800 (4 ) die Elektrogrammmessschaltungsanordnung, die der Elektrode oder dem Elektrodenpaar, die bzw. das beurteilt wird, zugeordnet ist, eine Basislinie oder einen Anfangsamplitudenwert EGMi aufzunehmen,402 . Dann, bei404 , startet der Mikroprozessor ein Zeitintervall und beginnt bei406 mit der Aufbringung von Ablationsenergie auf das überwachte Gewebe. Während der Aufbringung von Ablationsenergie erhält und speichert der Mikroprozessor Elektrogrammmesswerte,408 , wie schon in Verbindung mit5 erörtert. Bei jedem erhaltenen Amp litudenmesswert bestimmt der Prozessor800 bei410 in der schon in Verbindung mit6 beschriebenen Weise, ob ein Amplituden-Plateau aufgetreten ist. Wenn ein Plateau erfasst worden ist, prüft der Prozessor bei412 , ob die geforderte minimale Ablationszeit, z. B. 10 Sekunden, verstrichen ist. - Der Prozessor fährt fort, Amplitudenmesswerte zu erfassen, bis bei
410 ein Amplituden-Plateau erkannt wird und bei412 eine ausreichende Zeit verstrichen ist. Wenn diese beiden Kriterien erfüllt sind, erfolgt dann das Beenden der Aufbringung von Ablationsenergie auf das Gewebe, das überwacht wird,414 . Das Beenden der Ablation kann gleichzeitig mit der Erfassung des gesuchten Abfalls oder nach einer vorgegebenen Zeitverzögerung, um eine vollständige Transmuralität sicherzustellen, erfolgen. -
9A veranschaulicht eine zusätzliche Menge von Operationen zum Ausführen eines Transmuralitätsmessverfahrens, das im Großen und Ganzen wie in5 ,6 oder8 ist. Die Operationen von8 können im Anschluss an die Messung der Elektrogrammamplitude bei206 ,306 oder408 (5 ,6 und8 ) ausgeführt werden. Bei den in9A veranschaulichten zusätzlichen Operationen prüft der Mikroprozessor bei500 , ob die Elektrogrammamplitude zu schnell abnimmt, beispielsweise in Reaktion auf ein dA/dt mit einem Wert, der kleiner als ein festgelegter negativer Schwellenwert "d", z. B. –10 mV/s, ist. In Reaktion auf eine erfasste übermäßige Abnahme bei500 setzt der Prozessor den Leistungspegel der Ablationsenergie, die auf das überwachte Gewebe aufgebracht wird, bei502 herab, um den Ablationsvorgang zu verlangsamen. Der Betrieb der Vorrichtung wird dann wie in5 ,6 oder8 fortgesetzt. -
9B veranschaulicht ebenfalls eine zusätzliche Menge von Operationen zum Ausführen eines Transmuralitätsmessverfahrens, das im Großen und Ganzen wie in5 ,6 oder8 ist. Die Operationen von8 können im Anschluss an die Messung der Elektrogrammamplitude bei206 ,306 oder408 (5 ,6 und8 ) ausgeführt werden und können zusammen mit den Operationen von9A oder anstelle dieser ausgeführt werden. Bei den in9B veranschaulichten zusätzlichen Operationen prüft der Mikroprozessor bei510 , ob die Elektrogrammamplitude zu schnell abnimmt, beispielsweise in Reaktion auf ein dA/dt mit einem Wert, der größer als ein festgelegter negativer Schwellenwert "e", z. B. –1 mV/s, ist. In Reaktion auf eine erfasste unzureichende Abnahme bei510 erhöht der Prozessor den Leistungspegel der Ablationsenergie, die auf das überwachte Gewebe aufgebracht wird, bei512 , um den Ablationsvorgang zu beschleunigen. Der Betrieb der Vorrichtung wird dann wie in5 ,6 oder8 fortgesetzt. -
10 ist ein Funktionsablaufplan, der die Gesamtfunktionsweise der Vorrichtung in Verbindung mit einer mehrere Elektroden oder mehrere Elektrodenpaare aufweisenden Ablationsvorrichtung veranschaulicht. Bei dem Ablaufplan von10 werden alle Ablationselektroden oder -elektrodenpaare gleichzeitig aktiviert, und einzelne Ablationselektroden oder -elektrodenpaare werden in Reaktion auf Elektrogrammamplitudenmessungen, die dem Elektrodenpaar zugeordnet sind und anzeigen, dass die Läsion, die zwischen diesem Elektrodenpaar ausgebildet wird, vollständig transmural ist, deaktiviert. Unter diesen Umständen arbeitet das Ablationssystem wie folgt:
Nach der Initialisierung bei600 werden bei602 alle Elektroden 1 bis X aktiviert, was bedeutet, dass alle Elektroden und Elektrodenpaare mit Ablationsenergie versorgt werden. Der Mikroprozessor misst bei604 die Elekt rogrammamplitude, die einer ersten Elektrode oder einem ersten Elektrodenpaar zugeordnet ist, und prüft dann,608 , ob bei604 die Transmuralitätskriterien für eine erste Ablationselektrode oder ein erstes Ablationselektrodenpaar erfüllt sind, wobei die schon in Verbindung mit5 bis8 erörterten Kriterien verwendet werden. Wenn sie erfüllt sind, wird bei610 die Ablationselektrode oder das Ablationselektrodenpaar deaktiviert, indem die Abgabe von Ablationsenergie an die Elektrode oder das Elektrodenpaar eingestellt wird. Wenn nicht, misst der Mikroprozessor bei606 die Elektrogrammamplitude, die der nächsten Ablationselektrode oder dem nächsten Ablationselektrodenpaar zugeordnet ist, und prüft bei608 die Transmuralitätskriterien für die nächste Elektrode. Dieser Prozess wird fortgeführt, bis bei612 alle Elektroden deaktiviert sind, wonach das Verfahren als vollendet angesehen wird,614 , und der Ablationsvorgang wird bei616 beendet. -
11 veranschaulicht einen Funktionsablaufplan der Gesamtfunktionsweise einer Vorrichtung, bei der eine mehrere Elektroden oder mehrere Elektrodenpaare aufweisende Ablationsvorrichtung, wie in10 , verwendet wird. Bei dieser Ausführungsform werden die Ablationselektroden oder -elektrodenpaare jedoch nacheinander aktiviert. Nach der Initialisierung bei700 aktiviert der Mikroprozessor bei502 die Abgabe von Ablationsenergie an die erste Ablationselektrode oder das erste Ablationselektrodenpaar und misst bei604 die Elektrogrammamplitude. Bei608 prüft der Prozessor auf die gleiche Weise, wie für9 schon beschrieben, ob die Transmuralitätskriterien erfüllt ist. Wenn sie erfüllt sind, wird die Ablationselektrode oder das Ablationselektrodenpaar bei610 deaktiviert. Wenn nicht, wird die Aufbringung von Ablationsenergie fortgesetzt, wie schon beschrieben, bis die Transmuralitätskriterien erfüllt sind. Nach dem Deakti vieren einer Elektrode oder eines Elektrodenpaares bei610 prüft der Prozessor, ob alle Elektroden aktiviert und bei612 deaktiviert worden sind. Falls nicht, dann aktiviert der Mikroprozessor bei606 die nächste Elektrode oder das nächste Elektrodenpaar und startet die Abgabe von Ablationsenergie zu dieser Elektrode oder zu diesem Elektrodenpaar. Dieser Prozess wird fortgeführt, bis bei612 die letzte Elektrode deaktiviert worden ist, woraufhin der Mikroprozessor bestimmt, dass das Ablationsverfahren vollendet ist,614 , und bei616 wird das Ablationsverfahren angehalten. - Es wird die Meinung vertreten, dass die Methodik der Gesamtfunktionsweise von
10 insofern wünschenswert ist, als sie ein schnelleres Vollenden eines Ablationsverfahrens ermöglicht. Jedoch hat das Verfahren der Gesamtbetriebsweise wie in11 dargestellt den Vorteil, dass es die Verwendung eines etwas vereinfachten Generators ermöglichen kann, da nicht mehrere, separate Elektrogrammmessschaltungen, Leistungssteuerschaltungen und dergleichen, nämlich für jede Elektrode oder jedes Elektrodenpaar, bereitgestellt werden müssen. Es kann ein einfaches Schaltwerk zusammen mit nur einem einzigen HF-Generator und einer Elektrogrammmessschaltung verwendet werden, um Energie nacheinander auf jede Elektrode aufzubringen und die Elektrogrammamplitude gemäß der Erfindung zu überwachen.
Claims (3)
- Ablationssystem mit: Erzeugungsmitteln zum Erzeugen von Ablationsenergie; einer Ablationsvorrichtung mit Ablationsmitteln (
24 ,25 ), die mit den Erzeugungsmitteln verbindbar sind und benachbart zu bzw. neben einem Ort zu abladierenden Gewebes anordnenbar sind, zur Aufbringung der erzeugten Ablationsenergie auf den Gewebeort; einer Elektrogrammmesselektrode, die auf der Ablationsvorrichtung angebracht ist, so dass die Elektrogrammmesselektrode benachbart ist zu dem Gewebeort, wenn die Ablationsmittel benachbart zu dem Gewebeort sind; einer Elektrogrammamplitudenmessschaltung (843 ,845 ,847 ), welche mit der Elektrogrammmesselektrode verbindbar ist, zur Messung der Elektrogramm-Amplitude an dem Gewebeort unter Verwendung der Elektrogrammmesselektrode; und einer Steuerschaltung (802 ), die funktional gekoppelt ist mit den Erzeugungsmitteln zum Initiieren und Beenden der Aufbringung von Ablationsenergie auf die Ablationsmittel, wobei die Steuerschaltung gekoppelt ist mit der Elektrogrammamplitudenmessschaltung, und die Aufbringung von Ablationsenergie auf die Ablationsmittel in Reaktion auf das Auftreten eines Abfalls der Elektrogramm-Amplitude, die durch die Elektrogramm-Amplitudenmessschaltung gemessen wird im Anschluss an die Initiierung der Aufbringung von Ablationsenergie auf die Ablationsmittel, beendet. - System nach Anspruch 1, bei dem die Ablationsmittel eine Ablationselektrode sind, und bei dem die Erzeugungsmittel einen H-F- bzw. R-F-Generator aufweisen.
- System nach einem der vorstehenden Ansprüche, bei dem die Ablationselektrode eine Irrigationsablationselektrode ist.
