本書に記載する神経位置判定リボン装置は、一般に神経が装置の少なくとも1つの面または領域の近くか否かを判定するのに用いられる。例えば、NLR装置を用いて組織を切断もしくは改善する前に神経がNLR装置の一方の面にあるか否かを判定することができ、これを用いて近くの組織、特に神経組織を切断もしくは損傷するのを防止することができる。このように、組織を改善する前に本書に記載するNLR装置の何れかを用いて、組織を通る特定の経路が組織改善装置を用いるのに安全か否かを判定することができる。
上述したように、本書に記載するNLR装置は1以上の組織改善装置に連結することができる。図20Aおよび図20Bは、組織改善装置2001に連結するよう構成されたNLR装置2000の2つの実施例を示している。図20Aでは、組織改善装置2001は、一方の面に組織改善要素2003(例えば、「歯」)を具え、遠位端2005に連結要素を有する細長いフレキシブルな組織改善装置である。連結要素はガイドワイヤカプラとしてもよく、またはそれはNLR装置2000に直接連結するよう構成してもよい。NLR装置2000(図20Aの組織改善装置の遠位端に隣接して示される)は、組織改善装置が少なくとも部分的に入り、連結する開口部またはキャビティを具える。他の変形例では、NLR装置の近位端がNLR装置に入らずに組織改善装置の遠位端に連結するよう構成されている。
本書に記載する装置の全ては、システムの一部として用いることができ、これを神経位置判定システムまたはNLRシステムということができる。システムは、本書に記載する方法を実行する構成要素(例えば、ハードウェア、ソフトウェア等)を具えることができる。
一般に、本書に記載する神経位置判定装置の変形例の何れかを上述したように用いて神経が一方の面またはリボン型の装置の他方の面にあるかどうかを判定(またはチェック)することができる。神経位置判定装置の変形例と実施形態と共に、操作方法の実施例を以下に記載する。本書に記載する方法と特徴は、文脈が特徴または組み合わせを組み合わせることができないことを明示する場合を除いて、記載された他の方法と特徴の何れかと組み合わせて用いることができる。
Claims (16)
- 神経が装置の領域の近くにあるかどうかを判定することができる両手操作型の神経位置判定装置であって、当該装置が、
フレキシブルな細長い本体と、
前記細長い本体上の刺激領域と、
前記細長い本体の遠位端領域にガイドワイヤカプラとを具え、
前記ガイドワイヤカプラがガイドワイヤの近位端領域に前記細長い本体を取り外し可能に取り付けることができるよう構成されており、これにより前記ガイドワイヤの近位端領域が前記細長い本体の遠位端領域に対して前記ガイドワイヤカプラによって固定して保持される間、前記ガイドワイヤを引っ張ることによって所定の位置に前記刺激領域を引っ張ることができ、
前記フレキシブルな細長い本体が第1の面と第2の面を有するリボン型であり、前記第1の面と第2の面が実質的に平行であり、
前記刺激領域が前記細長い本体の第1の面及び第2の面にあり、前記細長い本体の第1の面及び第2の面の長手方向の少なくとも一部に沿って刺激場を放出するよう構成されていることを特徴とする装置。 - 請求項1に記載の装置において、前記刺激領域が刺激場を放出するよう構成された刺激電極を具えることを特徴とする装置。

- 請求項5に記載の装置において、複数の陰極が第1の陰極の導体と電気通信することを特徴とする装置。
- 請求項1に記載の装置において、前記刺激領域が前記細長い本体から第1の方向に刺激場を放出し、かつ前記細長い本体から第2の方向に刺激場を放出するよう構成されており、前記第2の方向が前記第1の方向と異なることを特徴とする装置。
- 請求項1に記載の装置がさらに、前記細長い本体に連結された近位ハンドルを具え、当該ハンドルが前記細長い本体から第1の方向の刺激場または前記細長い本体から第2の方向の刺激場の何れかの起動を選択する操作部を有することを特徴とする装置。
- 請求項1に記載の装置がさらに、前記細長い本体の長手方向に沿って分布する放射線不透過性マーカを具えることを特徴とする装置。
- 請求項1に記載の装置がさらに、前記細長い本体上に前記刺激領域に対して遠位の放射線不透過性マーカと、前記刺激領域に対して近位の放射線不透過性マーカとを具え、これにより近位および遠位の放射線不透過性マーカが前記刺激領域を描くことを特徴とする装置。

フレキシブルな細長い本体と、
前記細長い本体から第1の方向に刺激場を放出するよう構成された前記細長い本体上の第1の刺激領域と、

前記ガイドが組織を貫通する鋭い遠位端を有し、前記ガイドを引っ張ることによって所定の位置に前記刺激領域を引っ張ることができるよう構成されており、

前記近位端と遠位のフレキシブルなガイド領域との間に刺激領域とを具え、
前記遠位のフレキシブルなガイドが組織を貫通する鋭い遠位端を有し、前記遠位のフレキシブルなガイド領域を引っ張ることによって所定の位置に前記刺激領域を引っ張ることができるよう構成されており、
前記フレキシブルな細長い本体が第1の面と第2の面を有するリボン型であり、前記第1の面と第2の面が実質的に平行であり、
前記刺激領域が前記細長い本体の第1の面及び第2の面にあり、前記細長い本体の第1の面及び第2の面の長手方向の少なくとも一部に沿って刺激場を放出するよう構成されていることを特徴とする装置。 - 請求項13に記載の装置において、前記遠位のフレキシブルなガイド領域が少なくとも3インチの自由長を有することを特徴とする装置。
- 請求項13に記載の装置において、前記遠位のフレキシブルなガイド領域が少なくとも10インチの自由長を有することを特徴とする装置。
- 神経が装置の領域の近くにあるかどうかを判定することができるシステムであって、当該システムが、
両手操作型の神経位置判定装置であって、フレキシブルな細長い本体と、前記細長い本体の刺激領域と、前記細長い本体の遠位端領域にガイドワイヤカプラとを具える装置と、
前記神経位置判定装置の前記ガイドワイヤカプラに取り外し可能に連結するよう構成されたガイドワイヤとを具え、
これにより当該ガイドワイヤの近位端領域が前記細長い本体の遠位端領域に対して前記ガイドワイヤカプラによって固定して保持される間、前記ガイドワイヤを引っ張ることによって所定の位置に前記刺激領域を引っ張ることができ、
前記フレキシブルな細長い本体が第1の面と第2の面を有するリボン型であり、前記第1の面と第2の面が実質的に平行であり、
前記刺激領域が前記細長い本体の第1の面及び第2の面にあり、前記細長い本体の第1の面及び第2の面の長手方向の少なくとも一部に沿って刺激場を放出するよう構成されていることを特徴とするシステム。
