Description
Die Erfindung betrifft einen chirurgischen Apparat zum Zusammennähen von Organen mittels Klammern,
der gelenkig und trennbar verbunden in gestreckter Anordnung ein Anschlag- und ein Klammergehäuse, die
je eine parallel zueinanderliegende Backe aufweisen, eine als Amboß mit Vertiefungen zum Einbiegen der
Klammern und eine zum Anbringen eines Klammermagazins, einen Stößel für das gleichzeitige Ausstoßen
einer Vielzahl von Klammern und einen Fixator der gegenseitigen Lage der Gehäuse aufweist.
Ein derartiger chirurgischer Apparat ist aus der DE-OS 26 19 681 bekannt Bei diesem Apparat werden
die Klammern auf einem begrenzten Abschnitt nacheinander ausgestoßen, wobei in Abhängigkeit von der
unterschiedlichen Stärke der mit einer Naht zu versehenden Organteile die sie zusammendrückenden
Flächen oftmals nicht parallel verlaufen, so daß qualitativ ungleichmäßige Nähte erhalten werden.
Aus der DE-AS 11 59 127 ist ein chirurgisches Gerät
bekannt, mit dem man gleichzeitig nur eine Klammer ablegen kann. Das Gera» weist einen Spaltregler in
Form eines entlang einer Skala verschiebbaren und fixierbaren Walzenkörpers auf. Dieses System ist
jedoch für Geräte, mit denen eine Naht mit einer Vielzahl von Klammern gelegt werden soll, absolut
ungeeignet, weil durch die unparallele gegenseitige Verschiebung der das zu verbindende Gewebe zusammendrückenden Backen dann die Nahtgüte erheblich
beeinträchtigt würde.
Aus der US-PS 33 14 581 ist ein chirurgisches Gerät bekannt das nach dem gleichen Prinzip arbeitet wie das
aus der DE-AS 1159127 bekannt ist Hierbei wird
jedoch eine Naht mit einer Vielzahl von Klammern gelegt, die jedoch nur senkrecht zur Längserstreckung
der Backen angeordnet werden können, so daß die mit dem Gerät legbaren Nähte stark eingeschränkt sind.
Aufgabe der Erfindung ist es, ein chirurgisches Nähgerät gemäß dem Oberbegriff des Anspruchs 1
ίο dahingehend zu verbessern, daß die Backe des Anschiaggehäuses so ausgeführt und angeordnet ist daß
auf die gesmate Länge einer Naht im weiten Bereich von unterschiedlichen Nähspalten ein gleichmäßiges
Zusammendrücken der Naht gewährleistet wird, wobei die das Gewebe zusammendrückenden längsgestreckten Flächen parallel verlaufen.
Diese Aufgabe wird durch die kennzeichnenden Merkmale des Anspruchs 1 gelöst
Das vorgesehene Amboßteil gewährleistet die Regelungsmöglichkeit des Spaltes zwischen den Backen,
wodurch ein gleichmäßiges Zusammendrücken der Naht über deren gesamte Länge in weitem Bereich von
Nähspaltstärken gewährleistet wird. Dies trägt seinerseits zur Erhöhung der Qualität und Zuverlässigkeit der
Klammemaht bei, verringert die Verletzungsmöglichkeit der Gewebe von Innenorganen erheblich und
begünstigt das komplikationslose Verwachsen der Gewebe in der Nachoperationsperiode.
In einer der vorzugsweisen Varianten der Erfindung
ist im Backenkörper eine Längsnut ausgeführt, in der ein Vierkantstück regel- und verschiebbar angeordnet ist.
Zur Verschiebung des Vierkantstücks ist eine gekröpfte Welle vorgesehen, deren Zapfen am Anschlaggehäuse befestigt sind, wobei die Kröpfung der
Welle in einer im Vierkantstück ausgeführten Nut angeordnet ist
Vorzugsweise besitzt einer der Zapfen einen Griff mit einem Fixator zur Festlegung der Welle nach einer
Drehung.
Nachfolgend wird die Erfindung durch Beschreibung einer konkreten Ausführungsvariante unter Bezug auf
die Zeichnungen näher erläutert. Es zeigt
Fig. 1 eine Gesamtansicht eines Apparates gemäß der Erfindung mit teilweisem Schnitt;

so Der chirurgische Apparat zum Zusammennähen von Organen mittels Klammern enthält ein Anschlaggehüuse 1 und ein Klammergehäuse 2 (Fig. 1, 2 und 3), die
durch eine Achse 3 (F i g. 1 und 5) miteinander gelenkig und trennbar verbunden sind. Die Gehäuse 1 und 2
besitzen an dem einen Ende jeweils eine Backe 4 bzw. 5 und sind mit Griffringen 6 an dem anderen Ende
versehen. An der Backe 5 des Klammergehäuses 2 sind Klammermagazine 7 und 8 mit Klammern 9 und Stößel
10 für die Klammern 9 abnehmbar angebracht In der Backe 4 des Anschiaggehäuses 2, die als Amboßteil
dient, ist eine Nut A ausgeführt und in ihr ein Vierkantstück 11 mit Vertiefungen 12 zum Einbiegen
der Klammern 9 querverschiebbar, d. h. in Richtung auf das Klammergehäuse 2 zu und weg von ihm
verschiebbar, angeordnet. Im Körper des Vierkantstücks 11 ist eine Längsnut 13 vorgesehen, in der eine
Kröpfung 14' einer gekröpften Welle 14 angeordnet ist, die Zapfen 15 und 16 (Fig. 1, 3 und 4) besitzt. Die
;■ gekröpfte Welle 14 ist am Anschlaggehäuse 1 mittels
j Stiften 17 befestigt. Der Zapfen 16 ist mit einem Griff 18
mit einem Gewindeende versehen, auf das eine Rolle 19 :; aufgeschraubt ist, die mit radialen Vorsprüngen 20
'(« (Fig.3) am Anschlaggehäuse 1 kontaktiert und die
Walle 14 vor einer unkontrollierten Drehung sichert Am Klammergehäuse 2 ist ein Gleitstück 21 angeordnet,
an dem mittels Schrauben 22 ein Plattenmesser 23 und Leisten mit Keilabschrägungen 24 befestigt sind, die in
Schlitznuten 25 und 26 (F i g. 3 und 4) verschiebbar sind. Am Ende der Achse 3 sind Ansätze 27 vorhanden, die in
Nuten 28 (Fig.5) am Anschlaggehäuse 1 hineinragen und zusammen mit dem Anschlaggehäuse 1 und dem
■■; Klammergehäuse 2 ein Einzinkenschloß bilden. Im
zusammengeschlossenen Zustand sind das Anschlaggehäuse 1 und das Klammergehäuse 2 durch eine
Zahnstange 29 (F i g. 6) fixiert Um ein Gewebedurchzie-.,-.:'
hen beim Schneiden mit dem Messer 23 zu vermeiden, sind an den zusammendrückbaren Oberflächen der
Backen 4 und 5 Vorsprünge 30 (Fig. 1 und 4) Ni vorgesehen.

€;' verläuft wie folgt:
* i Nach leichtem Drehen der Rolle 19 schiebt man durch

1 und in die Dünndarmlichtung die Backe 5 des Klammergehäuses 2 eingeführt. Man verbindet das
; Anschlaggehäuse 1 und das Klammergehäuse 2 und :■' schließt sie, indem man die Griffringe 6 bis zur Fixierung
derselben durch die Zahnstange 29 zusammenführt. Man dreht mittels des Griffes 18 die gekröpfte Welle 14
nach links und führt hierdurch ein gleichmäßiges Zusammendrücken der Wände der zusammenzunähenden
Organe bis zu einem Spalt herbei, der die Anlegung einer hermetischen Naht gewährleistet. Durch Anschrauben
der Rolle 19 bis zu einem dichten Kontakt ihrer Oberfläche mit den Oberflächen der radialen
Vorsprünge 20 gewährleistet man die Fixierung der gekröpften Welle 14 gegen unkontrollierte Drehungen,
s Man verschiebt das Gleitstück 21 mit dem Plattenmesser 23 und den Leisten mit den Keilabschrägungen 24
längs dem Klammergehäuse 2 bis zum Anschlag. Hierbei wirken die Leisten 24 mit ihren Keilabschrägungen
auf die Stößel 10 ein, die die Klammern 9 aus ihren Nuten herausstoßen. Die Klammern 9 durchstechen,
indem sie die Nuten der Magazine 7 und 8 verlassen, mit ihren Füßen die Wände der Organe und kommen zur
Anlagean den Wänden der Vertiefungen 12 am Vierkantstück 11, wobei sie eingebogen werden und
eine Naht bildea Gleichzeitig mit dem Vorschieben der Leisten mit den Keilabschrägungen 24 zerschneidet das
Messer 23 das Gewebe der Organe zwischen den anzulegenden Nähten, wodurch eine Verbindung
zwischen dem Magenstumpf und der Dünndarmschlinge entsteht Danach bringt man das Gleitstück 21 in die
Ausgangsstellung zurück, führt die Backen 4 und 5 auseinander und zieht sie aus den Lichtungen der
zusammengenähten Organe heraus. Die weiteren Manipulationen zum Zusammennähen der Durchstiche
und Ausbilden einer Beutelnaht erfolgen von Hand nach der festgelegten Methodik.
Für den weiteren Einsatz des Apparates zum Anlegen von Nähten beim Zusammennähen oder Vernähen
müssen die leeren Magazine gegen gefüllte ausgewechseit werden.
Beim Einsatz dieses Apparates für andere Spaltgrößen muß die gekröpfte Welle erneut gedreht werden,
um den Abstand zwischen dem Vierkantstück und der Backe des Klammergehäuses zu verändern.
Der erfindungsgemäße Apparat weist hinsichtlich seiner Konstruktion und Einsatzmethodik Vorteile auf,
läßt sich vom Chirurgen mühelos handhaben, gewährleistet einen weiten Bereich von Nähspaltstärken, der
seine Verwendung für beliebige weiche Gewebe des Menschen erlaubt und kann zum Vernähen und
Zusammennähen von Organen des Verdauungstraktes, zum Vernähen von Blutgefäßen und bei unterschiedlichen
plastischen und rekonstruktiven Operationen angewendet werden.

Claims (4)
1. Chirurgischer Apparat zum Zusammennähen von Organen mittels Klammern, der gelenkig und
trennbar verbunden in gestreckter Anordnung ein Anschlag- und ein Klammergehäuse, die je eine
parallel zueinander liegende Backe aufweisen, eine als Amboß mit Vertiefungen zum Einbiegen der
Klammern und eine zum Anbringen eines Klammermagazins, einen Stößel für das gleichzeitige Ausstoßen einer Vielzahl von Klammern und einen Fixator
der gegenseitigen Lage der Gehäuse aufweist, dadurch gekennzeichnet, daß die Backe
(4) des Anschlaggehäuses (1) mit einem getrennten, parallel zu sich selbst verschiebbaren Amboßteil
versehen ist, und daß eine Vorrichtung zu einer regelbaren und in vorgegebener Stellung fixierbaren
Verschiebung des verschiebbaren Amboßteils in Richtung des Klammergehäuses (2) vorgesehen ist
2. Chirurgischer Apparat nach Anspruch 1, dadurch gekennzeichnet, daß im Körper der Backe (4)
eine Längsnut (A) ausgeführt ist, in der ein Vierkantstück (11) mittels einer Vorrichtung regel-
und verschiebbar angeordnet ist.
3. Chirurgischer Apparat nach Anspruch 2, dadurch gekennzeichnet, daß als Vorrichtung zur
regelbaren Verschiebung des Vierkantstücks (11) eine gekröpfte Welle (14) dient, deren Zapfen (15,
16) am Anschlaggehäuse (1) befestigt sind, und daß im Vierkantstück (11) eine Nut (13) ausgeführt ist, in
der sich die Kröpfung (14') der Welle 14 befindet
4. Chirurgischer Apparat nach Anspruch 3, dadurch gekennzeichnet, daß einer der Zapfen (16)
einen Griff (18) mit einem Fixator zur Festlegung der Welle (14) nach einer Drehung besitzt
