Priorität: 28. Juli 1975, V.St.A., Nr. 599 804
In der US-PS 3 339 546 ist ein medizinisches Verbandsmaterial
beschrieben, dessen Oberseite mit einer wasserundurchlässigen Folie beschichtet und deren Unterseite mit einer klebenden
Masse beschichtet ist. Die Klebemasse ist ein Gemisch aus einem wasserlöslichen oder in Wasser" quellbaren Hydrokolloid
und einem wasserunlöslichen, viskosen, gumraenartigen, elastischem
Bindemittel.
In den US-PSen 3 122 140, 3 122 141, 3 122 142 und 3 156 242
sind medizinische Verbandsstoffe aus einer Lage Schaumstoff,
wie einem Polyurethanschaumstoff beschrieben, die Fasergespinste enthalten, die wie Kapillaren wirken. Verbandsstoffe
aus einer Lage zusammengepreßtem offenzelligem Polyurethanschaumstoff
, der auf der einen Seite mit einem Adhäsionskleber

beschichtet ist, werden in den US-PSen 3 649 436 und
3 665 918 sowie in der GB-PS 1 253 845 beschrieben. In der
. US-PS 3 665 918 ist darüberhinaus beschrieben, daß der zusammengepreßte
Schaumstoff eine poröse Kunststoffbeschichtung aufweisen kann. Verbände, bei denen ein offenzelliger Polyurethanschaumstoff
direkt auf die Wunde gelegt werden kann, werden in den US-PSen 3113 568, 3 157 178, 3 491 753 und
3 648 692 beschrieben. Andere bekannte Verbandsstoffe, die eine Schicht eines offenzelligen Schaumstoffes enthalten,
sind das in der US-PS 3 536 072 beschriebene Streckverbandmaterial, die in der US-PS 3 669 106 beschriebene Abdeckung
zur Vorbereitung auf chirurgische Eingriffe,und die in der US-PS 3 301 254 beschriebene chirurgische Kompresse.
Der Erfindung liegt die Aufgabe zugrunde, ein medizinisches Verbandsmaterial zu schaffen, das aus einer Kernschicht aus
einem Weichschaumstoff mit halboffenen Zellen besteht, die auf der einen Seite mit einer Adhäsionskleberschicht und auf
der anderen Seite mit einer wasserundurchlässigen flexiblen Folie versehen ist.
Das Verbandsmaterial der Erfindung ist in den Zeichnungen schematisch erläutert:
Fig. 1 zeigt stark vergrößert das Verbandsmaterial;
Fig. 2 ist ein Schnitt entlang der Linie 2 2 von Fig. 1.
Die Zeichnungen zeigen das Verbandsmaterial 10, das aus einer
Adhäsionskleberschicht 11, einer Schicht eines Schaumstoffes

mit halboffenen Zellen 12 und einer FoIienbeschichtung 13 aufgebaut
ist.
Die Adhäsionskleberschicht 11 enthält ein kautschukartiges
Polymer, in dem ein wasserlösliches oder in Wasser quellbares
Hydrokolloid oder eine Hydrokolloidmischung, ein Klebrigmacher und ein Weichmacher oder ein Lösungsmittel fein dispergiert
sind. Bevorzugte kautschukartige Polymere sind natürliche oder synthetische, viskose, kautschukartige Verbindungen,
wie Naturkautschuk, Siliconkautschuk," Acrylnitrilkautschuk, Polyurethankautschuk und Polyisobutylen. Besonders bevorzugt
ist ein Gemisch von Polyisobutylenen mit einem mittleren Molekulargewicht von 10 000 bis 1·1 700 und 81 000 bis 99 000.
Beispiele für geeignete Hydrokolloide sind Polyvinylalkohol,
pulverförmiges Pektin, Gelatine, Natriumcarboxymethylcellulose,
hochmolekulares Polyäthylenglykol und Carboxypolymethylen.· Bevorzugt ist ein Gemisch von Gelatine und Natriumcarboxymethylcellulose.
Beispiele für geeignete Weichmacher oder Lösungsmittel sind Mineralöl und Petrolatum, wobei Mineralöl
bevorzugt ist. Terpenharz ist der bevorzugte Klebrigmacher. Die viskose, kautschukartige Verbindung wirkt als Bindemittel
für die Hydrokolloidpartikel. Außerdem verleiht sie der fertigen Klebermasse Klebrigkeit, Elastizität und Geschmeidigkeit.
Um die Lagerfähigkeit des Verbandsmaterials zu erhöhen, enthält die Klebermasse vorzugsweise noch ein Antioxidationsmittel,
wie butyliertes Hydroxytoluol oder butyliertes Hydroxyanisol.

Das Hydrokolloid oder die Hydrokolloidmischung sollen vor-■
zugsweise mindestens 30, insbesondere 40 bis 50 Gewichtsprozent der Klebermasse betragen. Die Dicke der Adhäsionskleberschicht
kann etwa 127 bis etwa 760 μ, vorzugsweise 127 bis
381 u betragen.
Der Weichschaumstoff mit halboffenen Zellen 12 kann aus den verschiedensten Kunststoffen, wie Polyester- oder Polyäther-Polyurethanschaumstoffen,
Styrol-Butadien-Copolymerisat-Schaumstoffen
und Schaumstoffen auf Kautschulibasis ,bestehen. Das verwendete Material muß ungiftig und beständig sein. Der
bevorzugte Schaumstoff ist ein Polyurethanweichschaumstoff mit etwa 20 bis etwa 40, vorzugsweise etwa 30 Zellen pro cm.
Der Ausdruck "halboffen" bedeutet, daß der Anteil an offenen oder geborstenen Zellen etwa 30 bis 70 % beträgt. Die Dicke
der Schaumstoffschicht mit halboffenen Zellen kann etwa 500 bis etwa 1270 u betragen.
Die äußere Schicht aus einer elastischen oder flexiblen Folie 13 kann aus einem wasserundurchlässigen, geschmeidigen elastomeren
Kunststoff bestehen, beispielsweise aus einem flexiblen Polyurethan, Polyacrylsäureester oder Naturkautschuk. Das verwendete
Material kann etwas gasdurchlässig sein, um den Durchtritt der Luft bis zur Hautoberfläche zu erleichtern. Es soll
vorzugsweise durchsichtig oder ausreichend durchscheinend sein, damit die versorgte Hautpartie beobachtet v/erden kann, ohne
daß der Verband abgenommen werden muß. Das bevorzugt eingesetzte Material ist eine flexible Polyurethanfolie. Die Be-

ren Schichten bestehen. Die Gesamtdicke der Beschichtung 13
beträgt etwa 25 bis etwa 76 u, vorzugsweise etwa 38 bis etwa
Während der Abpackung des Verbandsmaterials wird die freiliegende Seite der Adhäsionskleberschicht 11 mit abziehbarem
siliconbeschichteten Schutzpapier (in den Zeichnungen nicht dargestellt) versehen.
Zur Herstellung des Verbandsmaterials 10 werden das Hydrokolloid,
das kautschukartige Polymer, der Klebrigmacher und
Weichmacher sowie gegebenenfalls andere Bestandteile, wie beispielsweise ein Antioxidationsmittel, in einem organischen
Lösungsmittel, wie Heptan oder Hexan, gemischt. Die erhaltene Aufschlämmung wird auf einen Teil des Schutzpapiers aufgetragen.
Das Lösungsmittel wird verdampft. Nach dem Trocknen ist das Hydrokolloid in der gesamten Adhäsionskleberschicht dispergiert.
Dieser Aufbau wird sodann mit einem Schichtstoff zusammengepreßt. Der Schichtstoff besteht aus einem Weichschaumstoff
mit halboffenen Zellen und einer wasserundurchlässigen flexiblen Folie aus einem elastomeren Kunststoff,
die auf der einen Seite der Weichschaumstoffschicht aufgebracht
ist. Ein Verfahren zur Herstellung eines derartigen Schichtstoffes ist in der US-PS 3 547 753 beschrieben. Selbstverständlich
können auch andere Verfahren zur Herstellung dieses Schichtstoffes angewandt werden. Das erhaltene Verbandsmaterial
mit Schutzpapier kann zu einer Rolle aufgewickelt oder in verschiedene Größen geschnitten und abgepackt werden.

Das erfindungsgemäße Verbandsmaterial weist gegenüber bekannten
Verbandsmaterialien zahlreiche Vorteile auf. Das in der Klebstoffschicht vorliegende Hydrokolloid absorbiert Feuchtigkeit,
wie Schweiß und Wundexsudate und transportiert diese Feuchtigkeit von der Hautoberfläche zur Schaumstoffschicht,
von wo sie durch die Seiten des Verbandes entweichen kann. Bei den üblichen Adhäsionsklebstoffen kann die Feuchtigkeit
dagegen nicht von der Hautoberfläche verdunsten. Die hierdurch entstehende Feuchtigkeitsansammlung verursacht Hautreizungen
und Hautausschlag, und beeinträchtigt schließlich auch die Haftung des Verbandes, so daß er sich ablöst. Die Adhäsionsklebers
chi cht dos Verbandsmaterials der Erfindung reguliert den Feuchtigkeitsspiegel der Hautoberfläche. Dadurch
hält der Verband lange Zeit gut und muß weniger oft oder gar nicht gewechselt werden. Der Schichtstoff aus der Schaumstoffschicht
und der Folie gibt dem Verbcoid zusätzliche Flexibilität,
wodurch der Verband völlige Bewegungsfreiheit gewährt, sich angenehm trägt und einen besseren Sitz auf der Haut hat,
insbesondere an solchen Körperstellen, wie Knie oder Ellbogen, an denen der Verband gedehnt wird. Die Folienkaschierung gibt
dem Verband eine glatte Oberfläche, wodurch die Reibung zwischen Verband und Kleidung oder Bettüchern vermindert wird,
so daß ein Verrutschen des Verbandes vermieden wird. Die Folien kaschierung verstärkt auch die Schaumstoffschicht. Dadurch kann
der Verband entfernt und abgelöst werden, ohne dass er reißt.
Durch die Seiten der Schaumstoffschicht kann Luft eintreten, die durch die Adhäsionskleberschicht zur behandelten Hautpartie

vordringen kann. Im Gegensatz zu bekannten Verbänden, bei denen
ein direkter Kontakt zwischen Haut und Schaumstoff be-• steht, wird bei dem erfindungsgemäßen Verbandsmaterial die
Wundfläche durch die Adhäsionskleberschicht so abgeschlossen, • daß eine Verschmutzung und bakterielle Infektion auf ein Min- ·
destmaß beschränkt wird. Im Gegensatz zu den üblichen Adhäsionsklebermassen, die feuchtigkeitsundurchlässig sind, verursacht
die erfindungsgemäß verwendete Adhäsionsklebermasse weder
Hautreizungen noch Hautausschlag oder Exkoriation. Der Verband kann leicht abgenommen werden,, nachdem die Wunde verheilt
ist.
Die in den drei Schichten des Verbandsmaterials der Erfindung verwendeten Werkstoffe gestatten es, das Verbandsmaterial auf
verschiedene Weise zu sterilisieren, beispielsweise im Autoklaven oder durch Gammastrahlung. Das Verbandsmaterial ist
fast durchsichtig oder durchscheinend, so daß die Wunde beobachtet werden kann, ohne daß dazu der Verband entfernt werden
muß.
Das erfindungsgemäße Verbandsmaterial eignet sich auch für die
Versorgung von Brandwunden. Wie vorstehend ausgeführt, schließt die Adhäsionskleberschicht die Wunde ab und vermindert
die Gefahr von Sekundärinfektionen, der offenzellige
Schaumstoff gestattet eine angemessene Luft- und Feuchtigkeitsdurchlässigkeit und gibt dem Verband zusätzliche Flexibilität.
Der Verband gestattet es insgesamt, die behandelte Partie gut zu beobachten. Die Adhäsionskleberschicht ist auf-

grund des Gehalts an Hydrokolloid in der Lage, Exsudate von
der Oberfläche der Brandwunde zu absorbieren und diese zur Schaumstoffschicht zu befördern. Dort wird das Exsudat von ,
der durch die Seiten strömende Luft erfaßt und verdampft. Gleichzeitig verhindert die.Adhäsionskleberschieht und die
äußere Folienkaschierung einen übermäßigen Flüssigkeitsverlust an der Brandwundenoberfläche. Da das Hydrckolloid in der
Adhäsionskleberschieht schleimig wird, wenn es mit dem Brandwundenexsudat in Berührung kommt, kann.der Verband fast
schmerzlos entfernt v/erden und ohne daß die Oberfläche der verletzten Haut angegriffen wird.
Das Beispiel erläutert die Erfindung.
60 kg Polyisobutylenkautschuk mit einem mittleren Molekularge- ■
wicht von 81 000 bis 99 000, 54 kg Natriumcarboxymethylcellulose
und 45 kg Gelatine (Teilchengröße etwa 150 μ) werden verknetet.
In dem Gemisch sind die feinverteilten Kautschukpartikel durch die Hydrokolloide.getrennt gehalten und daran gehindert,
aneinander zu kleben. Das erhaltene Gemisch wird mit einer Lösung von 54 kg Polyisobutylen mit einem mittleren Molekulargewicht
von 10 000 bis 11 700, 60 kg Terpenharz als Klebrigmacher, 1,5 kg butyliertes Hydrox3rtoluol als Antioxidationsmittel
und 25,5 kg Mineralöl in Heptan versetzt. Es wird eine Aufschlämmung erhalten. Ein Teil dieser Aufschlämmung wird
auf ein siliconbeschichtetes abziehbares Schutzpapier (Trennpapier) aufgetragen. Das Lösungsmittel wird verdampft.

Ein zweiter Schichtstoff wird aus einem Polyurethanweichschaurristoff
mit teilweise geschlossenen und teilweise offenen Zellen
(der Schaumstoff hat etwa 50 Zellen pro cm, wovon etwa-" 30 bis 70 0A geborsten sind), eine Dicke von 760 y. und einer
Beschichtung aus flexiblem Pol3mrethan mit einer Dicke von 50 u hergestellt.
Der aus dem Adhäsionskleber und Schutzpapier bestehende Schichtstoff
und der aus dem Polyurethanweichschaumstoff und flexiblem Polyurethan bestehende Schichtstoff werden unter sanftem
Druck zusammengepreßt, während sie zwischen Druckwalzen hindurchlaufen. Man erhält ein Verbandsmaterial für medizinische
Zwecke.

Claims (15)
- - -10-(iy Medizinisches Verbandsmaterial, bestehend aus einer Kernschicht aus einem Weichschaumstoff mit halboffenen Zellen, die auf ihrer einen Seite mit einer Adhäsionskleberschicht und auf ihrer anderen Seite mit einer wasserundurchlässigen, flexiblen Folie versehen ist, dadurch gekennzeichnet, daß die Adhäsionskleberschicht aus einem kautschukartigen Polymer besteht, in welchem ein wasserlösliches oder in Wasser quellbares Hydrokolloid oder eine Hydrokolloidmischung, ein Klebrigmacher und ein Weichmacher dispergiert sind.
- 2. Verbandsmaterial nach Anspruch 1, dadurch gekennzeichnet, daß das Hydrokolloid Polyvinylalkohol, pulverförmiges Pektin, Gelatine, Natriumcarboxymethylcellulose, hochmolekulares Polyäthylenglykol, Carboxypolymethylen oder deren Gemisch ist.
- 3. Verbandsmaterial nach Anspruch 2, dadurch gekennzeichnet, daß das Hydrokolloid oder die- Hydrokolloidmischung mindestens 30 Gewichtsprozent der Adhäsionskleberschicht darstellen.
- 4. Verbandsmaterial nach Anspruch 3, dadurch gekennzeichnet, daß das Hydrokolloid oder die Hydrokolloidmischung etwa 40 bis 50 Gewichtsprozent der Adhäsionskleberschicht darstellt.
- 5. Verbandsmaterial nach Anspruch 1, dadurch gekennzeichnet, daß das kautschukartige Polymer Naturkautschuk, Silikonkaut-709807/0682·'schuk, Acrylnitrilkautschuk, Polyurethankautschuk oder Polyisobutylen ist.
- 6. Verbandsmaterial nach Anspruch 1, dadurch gekennzeich-. net, daß der Weichschaumstoff etwa 20 bis 40 Zellen pro cm
aufweist, wobei etwa 30 bis 70 % der Zellen geborsten sind. - .7·. Verbandsmaterial nach Anspruch 1, dadurch gekennzeichnet, daß die Adhäsionskleberschicht etwa 127 bis 760 μ, die
Schaumstoffschicht etwa 500 bis 1270 μ und die wasserundurchlässige Folie etwa 25 bis 76 u dick ist. - 8. Verbandsmaterial nach Anspruch 1, dadurch gekennzeichnet, daß die Unterseite der Adhäsionskleberschicht mit abziehbarem Schutzpapier versehen ist.
- 9. Verbandsmaterial gemäß Anspruch 1 bis 4, dadurch gekennzeichnet, daß die Adhäsionskleberschicht zusätzlich ein Antioxidationsmittel enthält.
- 10. Verbandsmaterial nach Anspruch 1 bis 9, dadurch gekennzeichnet, daß das kautschukartige Polymer Polyisobutylen und das Hydrokolloid eine Mischung aus Natriumcarboxymethylcellulose und Gelatine ist.
- 11. Verbandsmaterial nach Anspruch 1 bis 10, dadurch gekennzeichnet, daß der Weichschaumstoff ein Polyurethanweichschaumstoff und die Folie eine flexible Polyurethanfolie ist.L709807/0682 '

Verbandsmaterial nach Anspruch 1 bis 11, dadurch gekennzeichnet, daß die Adhäsionskleberschicht aus Polyisobutylen mit einem mittleren Molekulargewicht von 81 000 bis 99 000, Polyisobutylen mit einem mittleren Molekulargewicht von 10 bis 11 700, Natriumcarboxymethylcellulose, Gelatine, Terpenharz, Mineralöl und butyliertem Hydroxytoluol besteht.
- 13· Verbandsmaterial nach Anspruch 11 und 12, dadurch gekennzeichnet, daß die Adhäsionskleberschicht etwa 127 bis 760 u, die flexible Polyurethanfolie etwa 38 bis 50 μ dick ,ist und der Polyurethanweichschaumstoff etwa 30 Zellen/cm aufweist.
- 14. Verbandsmaterial nach Anspruch 13, dadurch gekennzeichnet, daß die Unterseite der Adhäsionskleberschicht mit einem abziehbaren siliconbeschichteten Schutzpapier versehen ist.
- 15. Verfahren zur Herstellung des Verbandsmaterials nach Anspruch 10 bis 14, gekennzeichnet durch folgende Verfahrensschritte:Polyisobutylenkautsch.uk mit einem mittleren Molekulargewicht von 81 000 bis 99 000, Natriumcarboxymethylcellulose und Gelatine v/erden miteinander verknetet; die verknetete Mischung wird unter kräftigem Rühren mit einer Lösung eines Polyisobutylens mit einem mittleren Molekulargewicht von 10 000 bis 11 700, Terpenharz, Mineralöl und butyliertem Hydroxytoluol in Heptan versetzt; die erhaltene Klebstoffaufschlämmung wird auf einen Bogen von siliconbeschichteten Schutzpapier aufgetragen und getrocknet; ein Polyurethanweichschaumstoff mitL ' -1709807/0682halboffenen Zellen, der 30 Zellen pro cm aufweist, wovon etwa 30 Ms 70 % geborsten sind, wird einseitig mit einer flexiblen Polyurethanfolie kaschiert; das mit Klebstoff beschichtete Schtitzpapier wird unter leichtem Druck mit der freien Seite des Polyurethanweichschaumstoffs verbunden.70 9807/0682 'Leerseite
