发明概述
本发明涉及表达一种或多种免疫细胞刺激受体激动剂的新的可复制溶瘤病毒、包含可复制溶瘤腺病毒的药物组合物及其在治疗多种癌症中的用途。在优选的实施方案中,可复制溶瘤病毒是腺病毒。可复制溶瘤病毒可提供来自第一复制循环的免疫细胞刺激受体激动剂,通过激活识别肿瘤抗原的淋巴细胞并逆转肿瘤周围的免疫抑制环境,而引发持续的效应器抗肿瘤免疫应答。在某些方面,与当分别给予时的未修饰的病毒(即不表达免疫细胞刺激受体激动剂)和免疫细胞刺激受体激动剂相比,将可复制溶瘤病毒(例如腺病毒)给予患有癌症的受试者提供提高的和甚至协同的抗肿瘤免疫。在相关方面中,可复制溶瘤病毒的抗肿瘤作用甚至在病毒清除后仍持续,甚至延伸到一种或多种未被感染的肿瘤。

CD28是最主要的共刺激受体,在T细胞上组成型表达,并在刺激幼稚T细胞用于增殖、效应子功能和分化中起关键作用。在一个实施方案中,可复制溶瘤病毒(例如腺病毒)表达CD28激动剂例如人CD80 (B7.1)、GenBank检索号NM_005191 (mRNA)和NP_005182 (蛋白质)或CD86 (B7.2)、GenBank检索号NM_175862 (mRNA)和Swiss-Prot数据库检索号P42081的激动剂。
GITR在调节T细胞和激活的CD4+和CD8+ T细胞上以高水平组成型表达。GITR被其受体GITR配体(GITRL)接合(Engagement)表明遏制调节T细胞的抑制作用并共激活效应T细胞。在一个实施方案中,可复制溶瘤病毒(例如腺病毒)表达GITR的激动剂(例如人GITRL,NCBI数据库Entrez Gene ID: 8995)。
4-1BB (CD37)在激活的CD4+和CD8+ T细胞的表面上、在天然杀伤细胞、单核细胞和静息树突细胞上表达。4-1BB与其配体4-1BB配体(4-1BBL)的接合在T细胞存活和长期免疫记忆的建立中起作用,并选择性促进1型细胞因子例如IL-2、IFN-γ和TNF-α。在一个实施方案中,可复制溶瘤病毒(例如腺病毒)表达4-1BB的激动剂(例如人4-1BBL),其全部氨基酸序列可见于Swiss-Prot数据库的检索号P41273。
HVEM在外周血T细胞、B细胞和单核细胞中表达。HVEM与其受体LIGHT的接合共刺激T细胞和B细胞活化,上调凋亡基因,并诱导细胞因子产生,特别是IFN-γ和TNFα的产生。在一个实施方案中,可复制溶瘤病毒(例如腺病毒)表达HVEM的激动剂例如人淋巴毒素样(LIGHT),其完全氨基酸序列可见于Swiss-Prot数据库的检索号043557。

可在例如溶瘤病毒的任何非必需位置插入编码免疫细胞刺激受体激动剂的DNA,只要溶瘤病毒保持可复制性。在一个实施方案中,溶瘤病毒是具有包含编码免疫细胞刺激受体激动剂的序列的异源核酸的腺病毒,所述序列插入在腺病毒尾丝基因的下游,籍以通过腺病毒主要晚期启动子驱动编码蛋白质的表达。在一个优选的实施方案中,将包含编码免疫细胞刺激受体激动剂的序列的异源核酸插入可复制腺病毒骨架的E3区。E3区对于病毒复制不是必需的;然而,E3蛋白在调节宿主免疫应答中起作用。可复制腺病毒可包含完全或部分E3缺失。例如,可复制腺病毒可包含E3区的1、2、3或更多个可读框(ORF)的缺失和插入其位置的异源核酸。在一个实施方案中,gpl9k和6.7K基因缺失,而异源核酸插入gpl9k/6.7K缺失的E3区。在一个相关的实施方案中,如Bett等, J. 某些实施方案涉及治疗癌症的方法,所述方法包括将表达一种或多种上述免疫细胞刺激受体激动剂的可复制溶瘤病毒(例如腺病毒)或包含可复制溶瘤病毒的药物组合物给予肿瘤。在某些方面,该方法包括将含有包含插入Delta-24腺病毒骨架的非必需区域的编码免疫细胞刺激受体激动剂的核酸序列的异源核酸的Delta-24腺病毒给予肿瘤。在一个优选的实施方案中,Delta-24腺病毒基因组的E3区的部分或E3区的全部缺失并被异源核酸置换。在一个特别优选的实施方案中,本发明提供通过将含有包含插入腺病毒骨架的非必需区域(例如缺失的E3区)的编码免疫细胞刺激受体激动剂(例如OX40L)的核酸序列的异源核酸的Delta-24-RGD腺病毒给予受试者来治疗人类受试者的癌症(例如神经胶质瘤)的方法。在一些实施方案中,人类受试者显示Th1白介素模式。在其它实施方案中,人类受试者显示Th2白介素模式。如果受试者的IL-12/IL-4比率小于约20、小于约15或小于约10,则认定受试者显示Th2白介素模式。显示Th1白介素模式的受试者一般将显示大于20和在某些情况下大于50、大于100和甚至大于300的IL-12/IL-4比率。可如下测定受试者的IL-12/IL-4比率:通过从受试者中获得样品(例如血液或血清样品),使样品与针对IL-12和IL-4的抗体接触,并测定样品中随抗体与其各自抗原结合的量而变化的IL-12和IL-4的量(例如通过ELISA)。
在相关的实施方案中,共给予一种或多种Th1刺激剂与表达一种或多种上述免疫细胞刺激受体激动剂的可复制溶瘤病毒以治疗受试者的癌症(例如成胶质细胞瘤)。在一些实施方案中,受试者的IL-12/IL-4比率小于约20 (即显示Th2白介素模式)。在其它实施方案中,受试者的IL-12/IL-4比率大于约20 (即显示Th1白介素模式)。Th1刺激剂包括而不限于(i) Th1细胞因子,例如IL-12p70、IL-2和IFN-γ,(ii)增加Th1细胞因子产生的作用剂,例如REVLIMID (来那度胺),(iii)抑制调节T细胞的作用剂(例如烷化剂,例如替莫唑胺(4-甲基-5-氧代-2,3,4,6,8-五氮杂二环[4.3.0]九-2,7,9-三烯-9-甲酰胺)、环磷酰胺(2-氧化(RS)-N,N-双(2-氯乙基)-1,3,2-氧氮磷杂环己烷-2-胺)、洛莫司汀(CCNU;N-(2-氯乙基)-N'-环己基-N-亚硝基脲)、双-氯乙基亚硝基脲(BCNU)、盐酸美法仑(4 [双(氯乙基)氨基]苯丙氨酸)、白消安(二甲烷磺酸丁烷-1,4-二基酯)、氮芥(mechlorethamine/nitrogenmustard)、苯丁酸氮芥、异环磷酰胺、链佐星、达卡巴嗪(DTIC)、塞替派、六甲蜜胺(六甲基蜜胺)、顺铂、卡铂和奥沙利铂(oxalaplatin))和(iv)刺激细胞介导的免疫应答的作用剂(例如伊匹木单抗、Tremelimumab、MDX-1106、MK-3475、AMP-224、Pidilizumab和MDX-1105)。用于与本发明的可复制溶瘤病毒共同给予的优选的Th1刺激剂包括IFN-γ (优选重组体)和替莫唑胺。本发明的可复制溶瘤病毒和Th1刺激剂可单独、同时或序贯给予患有癌症的受试者以治疗癌症。在一个实施方案中,将Th1刺激剂给予受试者,之后给予可复制溶瘤病毒。在其它相关的实施方案中,提供包含以下的组合物或试剂盒:(i) Th1刺激剂,和(ii)表达一种或多种本文所述免疫细胞刺激受体激动剂的可复制溶瘤腺病毒,各自以有效治疗受试者的癌症的量与另一种组合。在一个优选的实施方案中,组合物或试剂盒包含(i)选自以下的Th1刺激剂:重组IFN-γ、替莫唑胺、CCNU、BCNU、盐酸美法仑和白消安,和(ii)表达OX40激动剂(例如OX40L)的可复制溶瘤腺病毒(例如Delta-24或Delta-24-RGD)。
在某些实施方案中,提供表达PD-L1或PD-1拮抗剂的可复制溶瘤病毒(例如腺病毒)。在一些实施方案中,除表达免疫细胞刺激受体激动剂以外,可复制溶瘤病毒还表达PD-L1或PD-1拮抗剂。在其它实施方案中,可复制溶瘤病毒表达PD-L1或PD-1拮抗剂但不表达免疫细胞刺激受体激动剂。PD-L1已被鉴定为抗肿瘤T细胞的负调节剂,并在高达50%的人类癌症中表达。肿瘤细胞上的PD-L1与激活的效应T细胞上的PD-1结合导致激活PI3激酶信号转导级联,这进而阻断杀死肿瘤细胞所需的细胞毒性介质的产生。如本文所述,PD-L1或PD-1拮抗剂是一种破坏PD-L1和PD-1间的相互作用的分子。一方面,可复制溶瘤病毒是包含插入腺病毒基因组的非必需区域的编码PD-L1或PD-1拮抗剂的异源核酸的腺病毒。在相关的方面中,异源核酸编码抗PD-L1抗体例如MPDL3280A或抗PD-1抗体例如nivolumab或lambrolizumab。在其它实施方案中,异源核酸编码PD-L1或PD-1拮抗剂,例如描述于美国专利申请公布号2009/0217401、20110195068和20120251537和美国专利号8,217,149 (其各自的内容通过引用结合到本文中)中的那些。在某些实施方案中,提供治疗人类癌症(例如神经胶质瘤)的方法,所述方法包括给予有效量的表达PD-L1和/或PD-1拮抗剂的可复制溶瘤病毒。在一个优选的实施方案中,可复制溶瘤病毒是表达PD-L1和/或PD-1拮抗剂的腺病毒。在一个优选的实施方案中,腺病毒是Delta-24或Delta-24-RGD腺病毒。

在一个相关方面,除表达免疫细胞刺激受体激动剂以外,可复制溶瘤病毒还在其表面上表达NY-ESO-1 (GenBank U87459.1)或其免疫原性肽(例如SLLMWITQCFLPVF)。优选可复制溶瘤病毒是腺病毒,且编码NY-ESO-1或其免疫原性肽的核酸被插入编码表面蛋白的基因中,籍此腺病毒表达包含NY-ESO-1或其免疫原性肽的嵌合表面蛋白。一方面,将编码NY-ESO-1或其免疫原性肽的核酸插入编码腺病毒六邻体的基因的超变区5中。
将编码肿瘤抗原的核酸插入腺病毒基因中应“在框内”进行使得病毒在其表面上表达肿瘤抗原。
某些方面不需要完全切除肿瘤,这是在其它方法中在患者征募时的限制因素。此外,本发明方法和组合物的某些方面具有在免疫系统中产生记忆的潜力,并防止或降低肿瘤复发的可能性。
术语“可复制性”是指对于特定细胞或组织中的病毒复制所需要的任何基因功能没有缺陷的任何病毒载体。载体必需能够复制和被包装,但是只会在特定的细胞或组织中有条件地复制。如本文所述对本发明的可复制腺病毒载体进行改造以降低或消除其在正常细胞中复制的能力同时保持其在特定的肿瘤疾病细胞类型中有效复制的能力。通常,可复制腺病毒包含足够的E1、E2和E4区,使得腺病毒能够复制并被包装而不需要外在(intrans)供应的组分。

发明描述
本发明的方法和组合物包括包含编码免疫细胞刺激受体激动剂的异源核酸的溶瘤病毒(例如腺病毒)的构建和证实,与当分别给予时未修饰的溶瘤病毒(即不含编码免疫细胞刺激受体激动剂的异源核酸的遗传上相似或相同的溶瘤病毒)和免疫细胞刺激受体激动剂相比,其显示提高的和甚至协同的抗肿瘤作用。
I. 可复制溶瘤病毒
本发明的表达一种或多种免疫细胞刺激受体激动剂的可复制溶瘤病毒包括任何天然存在的(例如来自“场源”)或修饰的可复制溶瘤病毒。除表达一种或多种免疫细胞刺激受体激动剂以外,可对溶瘤病毒进行例如修饰以提高病毒对癌细胞的选择性。

用于实施本发明的可复制溶瘤病毒的具体实例包括腺病毒、反转录病毒、呼肠孤病毒、棒状病毒、新城疫病毒(NDV)、多瘤病毒、疫苗病毒、单纯疱疹病毒、微小RNA病毒、柯萨奇病毒和细小病毒。
在一个实施方案中,可复制溶瘤病毒是选自任选被修饰以提高癌症选择性的水泡性口炎病毒(VSV)和Maraba毒株的棒状病毒。所述修饰包括但不限于赋予病毒对宿主IFN应答易感的基质(M)基因的突变。
在另一个实施方案中,可复制溶瘤病毒是疫苗病毒,其非限制性实例包括任选被修饰以提高癌症选择性的Western Reserve、Wyeth和Copenhagen毒株。所述修饰包括但不限于:非功能性胸苷激酶基因、非功能性疫苗生长因子基因和非功能性1型干扰素结合基因。
另一方面,可复制溶瘤病毒选自单纯疱疹病毒(HSV)病毒(例如HSV-1或HSV1716)和新城疫病毒(NDV)。
腺病毒是特别优选的可复制溶瘤病毒。
腺病毒(Ad)是感染人、但显示宽泛宿主范围的大的(~36 kb) DNA病毒。在物理上,腺病毒是含有双链线性DNA基因组的二十面体病毒。存在人腺病毒的约50个血清型,根据分子、免疫学和功能标准被分成6个家族。到成年时,实际上每个人都被较常见的腺病毒血清型感染,主要影响是感冒样症状。
宿主细胞的腺病毒感染导致腺病毒DNA被附加型地维持,这降低与整合性载体有关的潜在基因毒性。此外,腺病毒在结构上是稳定的,在广泛扩增后未检测到基因组重排。不论细胞周期阶段如何,腺病毒实际上都可感染大多数的上皮细胞。迄今为止,腺病毒感染似乎只与人的轻微疾病(例如急性呼吸道疾病)有关。
可将57个人腺病毒血清型(HAdV-1至57)任一个的成员掺入编码本发明的免疫细胞刺激受体激动剂的异源核酸中。人Ad5在遗传学上和生物化学上被充分表征(GenBankM73260;AC_000008)。因此,在一个优选的实施方案中,溶瘤腺病毒是可复制Ad5血清型或包含Ad5组分的杂合血清型。腺病毒可以是野生型毒株,但优选通过例如降低病毒在正常静息细胞内复制的能力但不影响病毒在肿瘤细胞中复制的能力而进行遗传修饰以提高肿瘤选择性。本发明包括的可复制溶瘤腺病毒的非限制性实例包括Delta-24、Delta-24-RGD、ICOVIR-5、ICOVIR-7、ONYX-015、ColoAd1、H101和AD5/3-D24-GMCSF。Onyx-015是在E1B-55K和E3B区中有缺失以提高癌症选择性的病毒血清型Ad2和Ad5的杂合体。H101是Onyx-015的修饰形式。ICOVIR-5和ICOVIR-7包含E1A的Rb结合位点缺失且E1A启动子被E2F启动子置换。ColoAd1是嵌合Add11p/Ad3血清型。AD5/3-D24-GMCSF (CGTG-102)是编码GM-CSF的血清型5/3衣壳修饰的腺病毒(Ad5衣壳蛋白knob被血清型3的knob结构域置换)。
在一个特别优选的实施方案中,可复制溶瘤腺病毒是Delta-24或Delta-24-RGD。Delta-24描述于美国专利申请公布号20030138405和20060147420,其每一个通过引用结合到本文中。Delta-24腺病毒来源于腺病毒5型(Ad-5),并含有E1A基因的CR2部分内的24碱基对缺失,其包括负责相当于所编码的E1A蛋白中的氨基酸122-129的结合Rb蛋白(核苷酸923-946)的区域(Fueyo J等, Oncogene, 19:2-12 (2000))。Delta-24-RGD另包含将RGD-4C序列(其与αvβ3和αvβ5整联蛋白强结合)插入尾丝knob蛋白的H1环中(Pasqualini R.等,Nat Biotechnol, 15:542-546 (1997))。E1A缺失提高病毒对癌细胞的选择性;RGD-4C序列提高病毒在神经胶质瘤中的感染性。
注射入肿瘤的溶瘤腺病毒诱导细胞死亡和新的腺病毒子代的释放,其通过感染相邻细胞,产生治疗波(treatment wave),治疗波如果不终止,则可导致肿瘤被全部破坏。在细胞培养系统中和在恶性神经胶质瘤异种移植物模型中已表明Delta-24的巨大抗肿瘤作用。在1期临床试验中,Delta-24-RGD显示出乎意料的抗肿瘤作用,目前是额外的临床试验的主题。尽管肿瘤细胞的溶解是针对Delta-24-RGD溶瘤腺病毒所提出的主要抗癌机制,但在患有复发性神经胶质瘤的患者中,1期临床试验的数据和其它观察结果表明可通过抗肿瘤免疫应答的腺病毒介导的引发,提高直接的溶瘤作用。因此,约10%的用Delta-24-RGD治疗的患者显示在某些情况下,通过免疫细胞引起的肿瘤的浸润相当可观。在这些情况下,代表被治疗的那些的少部分,观察到Th1占主导的免疫应答似乎与最适抗肿瘤应答相关。本发明的方面旨在在大多数患者中提高这种抗肿瘤功效。本发明的可复制溶瘤腺病毒被设计成如下实现这种功效:(i)提高针对腺病毒和肿瘤抗原两者的Th1免疫应答,和(2)逆转肿瘤的免疫抑制环境。给予本发明的溶瘤腺病毒导致激活识别有或没有病毒感染的癌细胞的淋巴细胞群,因此提供甚至在病毒清除后仍持续的提高的和延长的抗肿瘤作用。此外,免疫细胞刺激受体(例如OX40)的激活导致在维持肿瘤的免疫抑制环境中起作用的T调节细胞的数目和活化状态降低。与分别给予腺病毒和免疫细胞刺激受体激动剂相比,本发明的溶瘤腺病毒提供通过将激动剂定位于肿瘤部位从而降低全身性给予激动剂伴有的不需要的副作用而提供显著优势。
腺病毒的感染周期以2步发生:先于腺病毒基因组复制开始并允许调节蛋白质和参与病毒DNA复制和转录的蛋白质的产生的早期,以及导致结构蛋白质合成的后期。早期基因分布在分散于腺病毒基因组的4个区内,命名为E1-E4 (E表示“早期”)。早期区域包含至少6个转录单位,其每一个具有其自身启动子。早期基因的表达被自我调节,某些基因在其它基因之前表达。3个区,E1、E2和E4是病毒复制所必需的。因此,如果对于这些功能之一腺病毒是有缺陷的,则这种蛋白质将必须外在供应,否则病毒不能复制。
E1早期区域位于腺病毒基因组的5'端,并含有2个病毒转录单位,ElA和E1B。该区编码极早期地参与病毒周期并且对腺病毒几乎所有其它基因的表达都是必需的蛋白质。具体地说,E1A转录单位编码反式激活其它病毒基因的转录的蛋白质,诱导从E1B、E2A、E2B、E3、E4区和晚期基因的启动子开始的转录。通常,外源序列被整合以代替E3区的全部或部分。
腺病毒通过细胞表面受体进入允许的宿主细胞,然后被内化。与复制循环第一步所需的某些病毒蛋白质有关的病毒DNA进入被感染细胞的核中,在其中开始转录。腺病毒DNA的复制发生在被感染细胞的核中,并且不需要细胞复制。新的病毒颗粒或病毒粒被组装,之后它们从被感染细胞中释放,并且可感染其它允许的细胞。
腺病毒是有吸引力的递送系统。本发明的实施方案采用具有每批1×1016个病毒颗粒的平均产量的悬浮细胞方法。该方法不含或基本不含蛋白质、血清和动物来源的组分,使之适于大范围的预防性和治疗性疫苗产品两者。
若干因素有利于使用溶瘤腺病毒治疗脑肿瘤。第一,神经胶质瘤通常是局部化的,因此有效的局部方法应足以医治该疾病。第二,神经胶质瘤带有若干表达不同遗传异常的细胞群。因此,可限制对单基因转移至癌细胞敏感的肿瘤谱。第三,可复制腺病毒可感染和破坏在G0停滞的癌细胞。因为神经胶质瘤总是包括非周期中的细胞,这个性质是重要的。最后,p16-Rb途径在大多数神经胶质瘤中异常,因此使Delta-24腺病毒对于治疗这些肿瘤特别有效,尽管成视网膜细胞瘤抑制基因功能的丧失与不同肿瘤类型的起因有关,并且不限于神经胶质瘤的治疗。
如果腺病毒突变使得它有条件地复制(在某些条件下可复制),则对于病毒复制可能需要辅助细胞。当需要时,辅助细胞系可来源于人细胞,例如人胚肾细胞、肌肉细胞、造血细胞或其它人胚间充质或上皮细胞。或者,辅助细胞可来源于对于人腺病毒是允许的其它哺乳动物物种的细胞。所述细胞包括例如Vero细胞或其它猴胚间充质或上皮细胞。在某些方面,辅助细胞系是293。培养宿主和辅助细胞的各种方法可见于本领域中,例如Racher等,1995。
在某些方面,溶瘤腺病毒在具有突变型Rb途径的细胞中是可复制的。在转染后,从覆盖琼脂糖的细胞中分离腺病毒噬斑,使病毒颗粒扩增用于分析。有关详细方案,技术人员参照Graham和Prevac,1991。
用于腺病毒载体产生的备选技术包括利用细菌人工染色体(BAC)系统、使用含有互补腺病毒序列的2种质粒在recA+细菌菌株中的体内细菌重组、和酵母人工染色体(YAC)系统(PCT公开文本95/27071和96/33280,其通过引用结合到本文中)。
腺病毒易于生长和操作,并显示体外和体内宽泛的宿主范围。可以高滴度(例如大于109噬斑形成单位(pfu)/ml)获得该组病毒,并且它们是高度感染的。腺病毒的生命周期不需要整合到宿主细胞基因组中。
可对本文所述溶瘤腺病毒进行修饰以改进溶瘤腺病毒治疗癌症的能力。Jiang等(Curr Gene Ther. 2009 Oct 9(5):422-427)描述了溶瘤腺病毒的所述修饰,另参见美国专利申请号20060147420,其每一个通过引用结合到本文中。
几种肿瘤类型中低水平的柯萨奇病毒和腺病毒受体(CAR)的存在与否可限制溶瘤腺病毒的功效。可将各种肽基序加入尾丝knob中,例如RGD基序(RGD序列模拟细胞表面整联蛋白的正常配体)、Tat基序、聚赖氨酸基序、NGR基序、CTT基序、CNGRL基序、CPRECES基序或strept标签基序(Rouslahti和Rajotte,2000)。可将基序插入腺病毒尾丝蛋白的HI环中。修饰衣壳允许不依赖CAR的靶细胞感染。这允许更高的复制,更有效的感染,并增加肿瘤细胞的溶解(Suzuki等,2001,通过引用结合到本文中)。还可加入结合特异性人神经胶质瘤受体(例如EGFR或uPR)的肽序列。专门或优选存在于癌细胞表面上的特异性受体可用作腺病毒结合和感染的靶标,例如EGFRvIII。
II. 表达盒
在本发明的某些实施方案中,本文给出的方法包括编码免疫细胞刺激受体激动剂的核酸序列,其中核酸包括在“表达盒”中。术语“表达盒”意指包括含有编码其中核酸编码序列的部分或全部能够被转录的基因产物的核酸的任何类型的遗传构建体。
启动子和增强子—为了表达盒实现转录物的表达,核酸编码基因应在启动子的转录控制下。“启动子”是一个控制序列,是转录起始和转录速率受其控制的核酸序列的区域。短语“有效位于”、“有效连接”、“在控制下”和“在转录控制下”意指启动子位于相对于核酸序列是正确的功能位置和/或取向以控制该序列的转录起始和/或表达。启动子可与或不与“增强子”联用,所述增强子是指参与核酸序列的转录激活的顺式作用调节序列。
预期本领域普通技术人员已知的在受试者的细胞中可起作用的任何启动子为可应用于本发明的方法和组合物的启动子。本领域的普通技术人员应熟悉可应用于本发明方法和组合物的许多类型的启动子。在某些实施方案中,例如,启动子是组成型启动子、诱导型启动子或阻抑型启动子。启动子也可为组织选择性启动子。组织选择性启动子在本文定义为是指与其它组织类型相比,在某些组织类型中相对更有活性的任何启动子。启动子的实例包括CMV启动子。
启动子应是在细胞中有活性的启动子,自启动子的表达导致抗原决定簇呈递至受试者的免疫系统。例如,在细胞是上皮细胞时,用于实施方案的启动子应是在该特定细胞类型中具有活性的启动子。
启动子可以是与基因或序列天然缔合的启动子,因为可通过分离位于编码区段和/或外显子上游的5′非编码序列而获得。所述启动子可称为“内源的”。类似地,增强子可以是位于核酸序列下游或上游的与该序列天然缔合的增强子。或者,某些优势可通过将编码核酸区段置于重组或异源启动子控制下而获得,所述启动子是指通常不与其天然环境中的核酸序列缔合的启动子。重组或异源增强子亦指通常不与其天然环境中的核酸序列缔合的增强子。所述启动子或增强子可包括其它基因的启动子或增强子,自任何其它原核、病毒或真核细胞分离的启动子或增强子和非“天然存在的”即含有不同转录调节区的不同元件和/或改变表达的突变的启动子或增强子。除了合成产生启动子和增强子的核酸序列以外,可采用重组克隆和/或核酸扩增技术包括PCR™,产生序列(参见美国专利号4,683,202和5,928,906,各通过引用结合到本文中)。
自然,使用有效指导DNA区段在选用于表达的细胞类型、细胞器和生物体中表达的启动子和/或增强子将是重要的。分子生物学领域技术人员一般了解用于蛋白质表达的启动子、增强子和细胞类型组合的使用,例如参见Sambrook等(2001),通过引用结合到本文中。启动子可以是异源或内源的。
不认为用来控制目标核酸表达的具体启动子是关键性的,只要它能够在靶定细胞中以足够的水平表达多核苷酸。因此,在人细胞被靶定时,优选将多核苷酸编码区置于能够在人细胞中表达的启动子的附近和控制下。一般来讲,所述启动子可包括人或病毒启动子。
在不同的实施方案中,可以使用人巨细胞病毒(CMV)瞬时早期基因启动子、SV40早期启动子和劳斯肉瘤病毒长末端重复序列。还预期使用本领域众所周知的实现多核苷酸表达的其它病毒或哺乳动物细胞或细菌噬菌体启动子,条件是表达水平足以产生免疫应答。
在本发明的情况下,可使用的启动子/元件的其它实例包括下列元件,其无意穷尽所有可能的启动子和增强子元件,而只是其实例性的:免疫球蛋白重链;免疫球蛋白轻链;T细胞受体;HLA DQ α和/或DQ β;β干扰素;白介素-2;白介素-2受体;MHC II类;MHC II类HLA-DRα;β-肌动蛋白;肌肉肌酸激酶(MCK);前白蛋白(运甲状腺素蛋白);弹性蛋白酶I;金属硫蛋白(MTII);胶原酶;白蛋白;甲胎蛋白;t-珠蛋白;β-珠蛋白;c-fos;c-HA-ras;胰岛素;神经细胞粘附分子(NCAM);α1-抗胰蛋白酶;H2B (TH2B)组蛋白;小鼠和/或I型胶原;葡萄糖调节蛋白(GRP94和GRP78);大鼠生长激素;人血清淀粉状蛋白A (SAA);肌钙蛋白I (TNI);血小板衍生生长因子(PDGF);杜兴肌营养不良;SV40;多瘤病毒;反转录病毒;乳头瘤病毒;乙型肝炎病毒;人免疫缺陷病毒;巨细胞病毒(CMV)和长臂猿白血病病毒。
增强子最初被检测为提高自位于同一DNA分子远端位置的启动子转录的遗传元件。增强子和启动子之间的基本差别是操纵上的。增强子区整体来看必须能够刺激在远处的转录;这不必对启动子区或其组分元件适用。另一方面,启动子必须具有指导在特定位点和以特定取向开始RNA合成的一个或多个元件,而增强子缺乏这些特异性。启动子和增强子常常是重叠的和邻接的,常常好像具有非常类似的模块结构。另外,还可使用任何启动子/增强子组合(按照真核生物启动子数据库(Eukaryotic Promoter Data Base),EPDB)以驱动基因表达。在响应特定生理信号时受调节的启动子进一步选择可允许构建体的诱导型表达。例如,有关在人PAI-1启动子控制下的多核苷酸,表达是通过肿瘤坏死因子可诱导的。诱导型元件(其是在响应特定刺激时可被激活的核酸序列的区域)的实例包括(元件/诱导物):MT II/佛波酯(TFA)或重金属;MMTV (小鼠乳腺肿瘤病毒)/糖皮质激素;β-干扰素/聚(rI)x或聚(rc);腺病毒5 E2/E1A;胶原酶/佛波酯(TPA);溶基质蛋白酶/佛波酯(TPA);SV40/佛波酯(TPA);鼠MX基因/干扰素,新城疫病毒;GRP78基因/A23187;α-2-巨球蛋白/IL-6;波形蛋白/血清;MHC I类基因H-2κb/干扰素;HSP70/E1A,SV40大T抗原;增殖蛋白/佛波酯-TPA;肿瘤坏死因子/PMA;和促甲状腺激素a基因/甲状腺激素。
起始信号—对于编码序列的有效翻译还可能需要特定的起始信号。这些信号包括ATG起始密码子或邻近序列。可能需要提供外源翻译控制信号,包括ATG起始密码子。本领域的普通技术人员应容易地能够对此进行测定并提供必需的信号。
IRES—在本发明的某些实施方案中,内部核糖体进入位点(IRES)元件被用来产生多基因或多顺反子信息。IRES元件能够绕过5′甲基化帽(Cap)依赖性翻译的核糖体扫描模型并开始在内部位点翻译。描述了来自微小RNA病毒科的2个成员(脊髓灰质炎和脑心肌炎)的IRES元件,还描述了来自哺乳动物信息的IRES。IRES元件可与异源可读框连接。各被IRES分隔开的多个可读框可在一起转录,产生多顺反子信息(参见美国专利号5,925,565和5,935,819)。
多克隆位点—表达盒可包括多克隆位点(MCS),其是含有多个限制性内切酶位点的核酸区,其任一个可与标准重组技术联用以消化载体。
多腺苷酸化信号—在表达中,通常可包括多腺苷酸化信号以实现转录物适当的多腺苷酸化。不认为多腺苷酸化信号的性质对成功实施本发明是关键性的,和/或可使用任何这类序列。优选的实施方案包括方便和/或已知在各种靶细胞中良好起作用的SV40多腺苷酸化信号和/或牛生长激素多腺苷酸化信号。还预期转录终止位点作为表达盒的元件。这些元件可用于提高信息水平和/或使从盒到其它序列的读取减到最小。
其它表达盒组分—在本发明的某些实施方案中,可通过在表达载体中包括报道基因,来体外鉴定被腺病毒载体感染的细胞。一般来讲,可选择的报道分子是赋予允许选择的性质的报道分子。阳性选择报道分子是其中报道基因的存在允许其选择的报道分子,而阴性选择报道分子是其中其存在防止其选择的报道分子。阳性选择标记的实例是抗药性标记(赋予对新霉素、嘌罗霉素、潮霉素、DHFR、GPT、zeocin和组氨醇的抗性的基因)。报道分子的其它类型包括可筛选报道分子,例如GFP。
在制备包含编码肿瘤相关抗原的异源核酸区段的腺病毒核酸时,本发明的实施方案可采用现有的腺病毒平台技术。腺病毒疫苗构建的方面包括将遗传物质插入腺病毒载体中,并通过核酸、病毒和病毒产物的表征和测序来证实构建体。然后使腺病毒疫苗完成经设计以评价可扩展性的一系列可行性研究。
III.癌症
本发明的方法可用于治疗癌症。癌症类型的具体实例包括但不限于神经胶质瘤、黑素瘤、转移癌、腺癌、胸腺瘤、淋巴瘤、肉瘤、肺癌、肝癌、结肠癌、非霍奇金淋巴瘤、霍奇金淋巴瘤、白血病、子宫癌、乳腺癌、前列腺癌、卵巢癌、宫颈癌、膀胱癌、肾癌、胰腺癌等。
术语“神经胶质瘤”是指起源于脑或脊髓中的神经胶质的肿瘤。神经胶质瘤来源于神经胶质细胞类型,例如星形细胞和少突胶质细胞,因此神经胶质瘤包括星形细胞瘤和少突神经胶质瘤以及退行性神经胶质瘤、成胶质细胞瘤和室管膜瘤。星形细胞瘤和室管膜瘤可发生在儿童和成人两者中的脑和脊髓的所有区域中。少突神经胶质瘤通常发生在成人的脑半球中。神经胶质瘤在儿科中占脑肿瘤的75%,在成人中占脑肿瘤的45%。其它脑肿瘤为脑膜瘤、室管膜瘤、松果体区肿瘤、脉络丛肿瘤、神经上皮肿瘤、胚胎性瘤、外周成神经细胞瘤、脑神经瘤、造血系统肿瘤、生殖细胞瘤和星形区(stellar region)肿瘤。本发明的方法可用于治疗任何脑癌。
术语黑素瘤包括但不限于黑素瘤、转移性黑素瘤、来源于黑素细胞或黑素细胞相关痣细胞的黑素瘤、黑素细胞癌、黑素上皮癌、黑素肉瘤、原位黑素瘤、浅表扩展性黑素瘤、结节性黑素瘤、恶性雀斑样痣黑素瘤、肢端雀斑样黑素瘤、侵袭性黑素瘤或家族性非典型痣和黑素瘤(FAM-M)综合征。可通过染色体异常、变性生长和发育障碍、促有丝分裂剂(mitogenic agent)、紫外辐射(UV)、病毒感染、基因的不当组织表达、基因表达的改变和细胞上的呈现或致癌物引起哺乳动物中的这类黑素瘤。前述癌症可通过本发明的方法评价或治疗。在癌症的情况下,可将编码与癌症相关的抗原(例如肿瘤相关抗原(TAA))的基因连同编码一种或多种免疫细胞刺激受体激动剂分子的核酸一起掺入重组病毒基因组或其部分。与癌症相关的抗原可在癌细胞表面上表达、可分泌或可为内部抗原。
IV. 药物组合物
本发明还提供包含本发明的任何组成和药学上可接受的载体的药物组合物。本发明还提供包含本发明的任何组成的疫苗组合物。疫苗组合物可进一步包含至少一种佐剂。
本发明还提供刺激受试者的抗肿瘤免疫应答的方法,所述方法包括给予受试者本发明的组合物。
按照本发明,将表达一种或多种免疫细胞刺激受体激动剂和任选一种或多种肿瘤相关抗原的腺病毒给予受试者以诱导免疫应答用于治疗或预防目的。因此,在某些实施方案中,将表达构建体配制在适于该目的的组合物中。短语“药学上的”或“药理学上可接受的”是指当给予动物或人时不产生有害的、变应性的或其它不良反应的组合物。如本文所述,“药学上可接受的载体”包括任何和全部溶剂、载体、分散介质、包衣材料、抗细菌剂和抗真菌剂、等渗剂和吸收延迟剂等。对于药学活性物质,这类介质和作用剂的使用是本领域众所周知的。除非任何常规介质或作用剂与本发明的表达构建体不相容,否则预期其在治疗组合物中的应用。还可将补充活性成分掺入组合物中。例如,补充活性成分可以是其它的免疫原性物质。
适于注射用的药物形式包括无菌水性溶液剂或分散剂和用于临用前制备无菌注射用溶液剂或分散剂的无菌粉剂。在所有情况下,形式必须是无菌的,且必须流动至存在易注射性的程度。在生产和贮存条件下必须是稳定的,并且必须防止例如细菌和真菌等微生物的污染作用。载体可以是溶剂或分散介质,含有例如水、乙醇、多元醇(例如甘油、丙二醇和液体聚乙二醇等)、其合适的混合物和植物油。如需要,可使用不同的抗细菌剂和抗真菌剂,例如对羟基苯甲酸酯、三氯叔丁醇、苯酚、山梨酸、硫柳汞等。在许多情况下,优选可包括等渗剂,例如糖或氯化钠。可通过在组合物中使用延迟吸收剂(例如单硬脂酸铝和明胶)导致注射用组合物的延迟吸收。
通过将所需量的化合物与各种其它上列成分(需要时)一起掺入合适的溶剂中,接着过滤除菌,来制备无菌注射用溶液剂。一般来讲,通过将各种灭菌活性成分掺入含有基础分散介质和来自上列所需其它成分的无菌溶媒中,来制备分散剂。在用于制备无菌注射用溶液剂的无菌粉剂的情况下,优选的制备方法是真空干燥和冷冻干燥技术,其得到活性成分加上来自其前述过滤除菌溶液的任何其它所需成分的粉末。
在配制时,溶液剂将以与剂型相容的方式和以治疗或预防有效的量给予。对于呈水性溶液剂的胃肠外给药,必要时应使溶液剂适当缓冲,且液体稀释剂首先用足够盐水或葡萄糖提供等渗性。这些特殊的水性溶液剂尤其适于血管内和肿瘤内给药。就此而论,根据本公开内容可以使用的无菌水性介质将是本领域技术人员已知的。
根据受治疗的受试者的状况,必然发生剂量的某些变化。在任何情况下,负责给药的人应确定各个受试者的合适剂量。此外,对于给予人,制品应符合FDA要求的无菌度、热原性、总体安全性和纯度标准。
剂量—根据预定目标,例如刺激针对肿瘤的免疫应答,来确定治疗剂或预防剂的有效量。本领域的技术人员非常了解在体内和离体情况下如何应用基因递送。对于病毒载体,技术人员一般将制备病毒载体原液。根据可得到的病毒的种类和滴度,可向受试者递送至少约、至多约或约1×104、1×105、1×106、1×107、1×108、1×109、1×1010、1×1011或1×1012个感染性颗粒或之间的任何值或范围。在其它方面,可以单次给药或多次给药给予本发明的腺病毒。可以1 x 105噬斑形成单位(PFU)、5 x 105 PFU、至少1 x 106 PFU、5 x 106或约5 x 106 PFU、1 x 107、至少1 x 107 PFU、1 x 108或约1 x 108 PFU、至少1 x 108 PFU、约或至少5 x 108 PFU、1 x 109或至少1 x 109 PFU、5 x 109或至少5 x 109 PFU、1 x 1010PFU或至少1 x 1010 PFU、5 x 1010或至少5 x 1010 PFU、1 x 1011或至少1 x1011、1 x 1012或至少1 x 1012、1 x 1013或至少1 x 1013 PFU的剂量给予病毒。例如,可以介于约107与1013PFU之间、介于约108与1013 PFU之间、介于约109与1012 PFU之间或介于约108与1012 PFU之间的剂量给予病毒。
可局部或全身给予本发明的可复制溶瘤病毒。例如而无限制地,可以血管内(动脉内或静脉内)、肿瘤内、肌内、皮内、腹膜内、皮下、口服、胃肠外、鼻内、气管内、经皮、脊柱内、经眼或颅内给予本发明的溶瘤病毒。在优选的实施方案中,血管内或肿瘤内给予本发明的腺病毒。
还可以细胞载体给予本发明的可复制溶瘤病毒。在这方面,神经元和间充质干细胞具有高移行能力但仍保持被限制在肿瘤组织中。显示了在注射入神经胶质瘤后,成年间充质细胞亚群(骨髓衍生肿瘤浸润细胞或BM-TIC)浸润完整肿瘤。因此,在产病毒神经元或间充质干细胞(例如BM-TIC)载体中可给予本发明的溶瘤病毒(例如通过将载体细胞注射到肿瘤中)。
根据治疗次数和剂量两者待给予的量取决于待治疗的受试者、受试者的状态和所需保护。治疗组合物的精确量还取决于从业人员的判断,并且对于每个个体是独特的。
实施例
包括了下列实施例以及附图以表明本发明的优选实施方案。本领域的技术人员应认识到,实施例或附图中公开的技术代表本发明人发现的在实施本发明时良好起作用的技术,因此可被视为构成其实施的优选模式。然而,根据本公开内容,本领域的技术人员应认识到可在公开的特定实施方案中进行许多改变,并且仍获得相同或类似的结果而不偏离本发明的精神和范围。
实施例1—Delta-24-RGDOX的构建和表征
具有CMV启动子的小鼠OX40L表达盒置换人腺病毒5型基因组的E3区。使负责结合Rb蛋白的E1A基因的CR2部分内的24-bp序列(相当于所编码的E1A蛋白中的氨基酸122-129)缺失。将RGD-4C基序编码序列插入尾丝蛋白的HI环中。参见图1。
评价了在GL261 (小鼠神经胶质瘤)和小鼠黑素瘤B16细胞上通过D24-RGDOX引起的小鼠OX40L (mOX40L)的表达。将GL261或B16细胞用D24-RGDOX以50 pfu/细胞感染。48小时后,将细胞用α-mOX40L抗体(1:100稀释度) (eBioscience,San Diego,CA)染色,然后用FITC标记的第二抗体山羊抗大鼠IG (1:100稀释度) (Santa Cruz Biotechnology)染色。用乙锭同二聚体-1染色(8 μM) (Sigma-Aldrich, St. Louis, MO),监测细胞膜完整性。用流式细胞术分析染色细胞。附图2和3右下角的数字表示表达mOX40L的GL261和黑素瘤B16细胞的百分比。D24-RGDOX在GL261细胞和黑素瘤B16细胞两者上有效地表达OX40L。
评价了mOX40L在GL261-EGFP (表达增强绿色荧光蛋白的GL261)肿瘤细胞中的表达。在C57BL/6小鼠中,颅内注射GL261-EGFP细胞(5 x 104个细胞)。12天后,肿瘤内注射D24-RGDOX (5 x 107 pfu)。在注射后3天,收获肿瘤,用ACCUMAX细胞脱附溶液(EMDMillipore,Billerica,MA)解离。然后将细胞用大鼠单克隆α-mOX40L APC抗体(1:40)(eBioscience)染色。用流式细胞术分析染色细胞。通过门控的肿瘤细胞为EGFP阳性。图4右上角的数字表示表达mOX40L的肿瘤细胞的百分比。这些体内数据表明,在用D24-RGDOX注射后72小时,OX40L在约9%的异种移植物细胞中表达。
测试了D24-RGD和D24-RGDOX在U87 MG (具有上皮形态的人原生成胶质细胞瘤细胞系;American Type Culture Collection,Manassas,VA)或GL261细胞中的复制。将细胞以5 x 104个细胞/孔的密度接种在12孔板中,用病毒以10 pfu/细胞感染。按照生产商说明书,在感染后48小时,使用ADENO-X Rapid Titer试剂盒(Clontech,Mountain View,CA),对感染性病毒子代进行滴度测定。最终的病毒滴度测定为pfu/ml,在图5中表示为3个独立测量的均值+ SD。应用斯氏t检验(两侧),比较2种病毒的复制。显示了D24-RGDOX与其亲代病毒D24-RGD一样在人神经胶质瘤U-87 mg细胞中有效地复制,而两种病毒在GL261细胞中极差地复制。因此,本文所述用小鼠神经胶质瘤模型的抗肿瘤作用显然不足以代表病毒(表达OX40L)在人中的预期的抗肿瘤作用。
评价了D-24-RGD和D24-RGDOX诱导HSP90和HMGB1分泌的能力。将GL261细胞用病毒以200 pfu/细胞感染。24小时后,FBS的浓度从10%变为2%。在图6中规定的时间点上收集培养基(M)和全细胞裂解物(W)。用蛋白质浓缩器(9K MWCO,Thermo Scientific)将培养基浓缩10倍。然后用免疫印迹法分析HSP90和HMGB1表达水平。简单地说,将来自全细胞裂解物或40 μl/泳道浓缩培养基的等量的蛋白质用4-20%梯度十二烷基硫酸钠-聚丙烯酰胺凝胶电泳分离,用电泳法转移到硝化纤维素膜上,将膜用兔多克隆抗HSP90和抗HMGB1 (1:1000稀释度) (Cell Signaling Technology, Beverly, MA)、山羊多克隆抗肌动蛋白(1:1000稀释度) (Santa Cruz Biotechnology,Santa Cruz,CA)探测。采用增强化学发光蛋白质印迹检测系统(Amersham Pharmacia Biotech,Piscataway,NJ),观察蛋白质-抗体复合物。肌动蛋白用作全细胞裂解物的加样对照。图6底部的数字表示分泌到培养基的相对HMGB1水平。尽管在GL261细胞中病毒的复制效率低,但两种病毒诱导ATP和HMGB1释放,其是在免疫原性细胞死亡期间引发炎症和免疫的内源损伤相关分子模式(DAMP)分子的原型。
实施例2—由D24-RGDOX诱导的增强的治疗作用
评价了D24-RGDOX对神经胶质瘤癌症模型存活的作用,并且与分别或一起给予D24-RGD和OX86 (OX40激动剂)的作用进行了比较。在C57BL/6小鼠和无胸腺小鼠中,颅内注射GL261细胞(5 x 104个细胞)。在肿瘤移植后的第3、6和8天,肿瘤内注射D24-RGDOX或D24-RGD (5 x 107 pfu)和/或α-小鼠OX40抗体OX86 (25 μg,由MDACC的单克隆抗体核心设施提供(Monoclonal Antibody Core Facility)) (将病毒注射3次以部分补偿GL261细胞中病毒的低复制)。PBS用作阴性对照。应用log-rank检验(两侧),对治疗组(PBS;D24-RGD;OX86;OX86+D24RGD;D24-RGDOX;各组n=10)间的存活率进行了比较。附图7A和7B分别表明C57BL/6或无胸腺小鼠中规定治疗组(各组n = 10)的总体生存的Kaplan-Meier曲线。该动物存活研究表明,虽然D24-RGD本身显示对于各种注射5 x 107 pfu/小鼠的病毒剂量无作用(p =0.08),但是D24-RGD与OX86的组合显著延长小鼠的存活(中位生存时间24天对比17天,p =0.0002)。重要的是,与D24-RGD相比,D24-RGDOX进一步延长中位生存时间到28.5天(p<0.0001)。小鼠存活期延长主要由病毒和抗体引发的抗神经胶质瘤免疫所致,因为免疫缺陷性GL261-无胸腺小鼠神经胶质瘤模型中未观察到治疗益处(p>0.3) (图7B)。
采用流式细胞术分析,检查由D24-RGDOX诱导的免疫应答,并与D24-RGD的进行比较。在C57BL/6小鼠中,颅内注射GL261细胞(5 x 104个细胞)。在肿瘤移植后第6、8和10天,肿瘤内注射病毒(5 x 107 pfu)。在第14天,首先用Percoll (GE Healthcare Bio-Sciences,Pittsburgh,PA)梯度离心机从髓鞘碎片中分离浸润脑的白细胞(9只小鼠的组),并直接用于流式细胞术分析。所用抗体如下:抗小鼠CD45 APC-EFLUOR 780 (1:200稀释度)、抗小鼠CD3 FITC (1:200稀释度)、抗小鼠CD8a PerCP-Cyanine5.5 (1:80稀释度)(eBioscience)、BRILLIANT VIOLET 650抗小鼠CD4抗体(1:100稀释度) (BioLegend,SanDiego,CA)。在图8中数据显示为一式三份测量的均值+ SD。应用斯氏t检验(两侧),比较治疗组间的细胞数。数据表明,D24-RGDOX比D24-RGD更有效地诱导浸润到肿瘤部位的T淋巴细胞(CD45+ CD3+)、T辅助细胞(CD45+ CD3+ CD4+)、细胞毒性T细胞(CD45+ CD3+ CD8+) (p <0.001)。
评价了D24-RGDOX对抗肿瘤免疫应答的作用,并与D24-RGD的进行了比较。在C57BL/6小鼠中,颅内注射GL261细胞(5 x 104个细胞)。在肿瘤移植后第6、8和10天肿瘤内注射病毒(5 x 107 pfu)。在肿瘤移植后第14天,分离各治疗的小鼠脾的脾细胞(5只小鼠的组)。对于脑淋巴细胞分离(来自具有肿瘤的5个半球的组),首先如上所述从髓鞘碎片中分离浸润脑的白细胞。然后,用梯度离心机在LYMPHOLYTE-M (Cedarlane,Burlington,NC)中分离脑淋巴细胞。为了激活脾细胞,使用1%低聚甲醛(PFA)预先固定的2 x 104个靶细胞与5x 105个脑浸润淋巴细胞或脾细胞/孔在圆底96孔板中一起孵育40小时。用标准ELISA测定法,评价上清液中IFNγ的浓度(小鼠IFN-γ DuoSet,R&D systems)。在图9中数据显示为一式三份测量的均值+ SD。附图10A和10B说明其中如上所述在肿瘤移植后第21天从各治疗组小鼠中分离脑浸润淋巴细胞并与MBC共培养(图10A)且其中如上所述在肿瘤移植后第21天从各治疗组小鼠中分离脾细胞并与规定的靶细胞共培养(图10B)的各实验。在每种情况下,40小时后用标准ELISA测定法测量上清液中IFNγ的浓度(小鼠IFN-γ DuoSet,R&Dsystems)。在图10A和10B中数据显示为一式三份测量的均值+ SD。应用斯氏t检验(两侧),比较治疗组间的活性。这些数据表明,与D24-RGD或D24-RGD-EGFP相比,D24-RGDOX在免疫细胞(脾细胞和脑浸润淋巴细胞(BIL))中诱导针对未被感染或病毒感染的肿瘤细胞的显著更大的活性(p,0.05)。与被D24-RGD感染的肿瘤细胞相比,被D24-RGDOX感染的肿瘤细胞在BIL中引发更强的活性(p < 0.002),表明了由D24-RGDOX引起的OX40L表达提高肿瘤细胞刺激免疫细胞的能力。尽管与BIL中的其它组相比,D24-RGDOX引起针对小鼠脑细胞(MBC)原始培养物的更强的免疫反应(p = 0.01),但仍诱导针对肿瘤细胞比针对MBC显著更高的活性(p> 0.005)。然而,由D24-RGDOX诱导的针对MBC的BIL的这种反应增加(D24-RGD的15.6倍)是急性的,因为在再过7天后下降(D24-RGD的1.6倍)。在7天后,由D24-RGDOX诱导的针对MBC的BIL的急性活性水平下降约4倍。另外,在脾细胞中未观察到针对由D24-RGDOX诱导的MBC的反应提高(p = 0.2),而在脾细胞中在再过7天后,针对肿瘤细胞的提高的反应保持。脾细胞中D24-RGDOX治疗组和其它组之间的活性差异甚至大于前7天。
本发明人首次将溶瘤腺病毒D24-RGD与靶向晚期共刺激OX40L/OX40途径结合以治疗免疫活性小鼠模型的神经胶质瘤。D24-RGDOX显示诱导比其亲代病毒D24-RGD更好的抗神经胶质瘤免疫能力。由于D24-RGD的癌症选择性质所致,OX40L可优先在癌细胞上表达。此外,与同样结合CTLA4的CD28的配体不一样,OX40配体选择性地结合OX40。因此,OX40L在具有识别肿瘤相关病毒抗原的TCR的T淋巴细胞上刺激OX40,导致肿瘤特异性T细胞群的扩增。因此,与OX40激动剂抗体、CTLA-4和PD-1的拮抗剂抗体或使用溶瘤病毒表达免疫调节剂以全面激活免疫细胞不同,通过由D24-RGDOX表达的OX40L调节T细胞更局限于肿瘤特异性T细胞。因此,相对于其它疗法,D24-RGDOX不太可能引起全身毒性。根据本范例,预期用D24-RGDOX将显著增加具有完全应答的人类癌症患者的百分比。还预期由于由OX40L/OX40途径刺激的免疫记忆提高所致,用D24-RGDOX延长临床反应的持续时间。

Claims (32)
