Abstract
Description
発明の分野
本発明は、一般に、腫瘍学およびがん治療の分野に関する。より具体的には、本発明は、OX40リガンド(OX40L)などの免疫細胞刺激受容体アゴニストを発現するように遺伝子改変された複製的腫瘍溶解性ウイルスに関する。
がんは、世界中でヒトにおける罹患率および死亡率の主要原因の1つのままである。外科手術、化学療法および放射線は、がんを治癒することに幾分成功して利用されているが、新規戦略が必要とされている。正常細胞中よりも腫瘍細胞においてよく複製するウイルスは、腫瘍溶解剤としての有望さを示している。アデノウイルスを使用する遺伝子移入および腫瘍溶解の実行可能性は、十分に確立されている。
本発明は、1または複数の免疫細胞刺激受容体アゴニストを発現する新規の複製コンピテントな腫瘍溶解性ウイルス、複製コンピテントな腫瘍溶解性アデノウイルスを含む医薬組成物、および種々のがんを処置する際のそれらの使用に関する。好ましい実施形態では、この複製コンピテントな腫瘍溶解性ウイルスは、アデノウイルスである。この複製コンピテントな腫瘍溶解性ウイルスは、最初の複製サイクルから免疫細胞刺激受容体アゴニストを提示し、腫瘍抗原を認識するリンパ球を活性化することによって持続性のエフェクター抗腫瘍免疫応答を引き起こし、腫瘍を取り囲む免疫抑制的環境を逆転させる。ある特定の態様では、がんを有する対象への、アデノウイルスなどの複製コンピテントな腫瘍溶解性ウイルスの投与は、別々に投与した場合の非改変ウイルス(即ち、免疫細胞刺激受容体アゴニストを発現しない)および免疫細胞刺激受容体アゴニストと比較して、増強された、さらには相乗的な抗腫瘍免疫を提供する。関連の態様では、この複製コンピテントな腫瘍溶解性ウイルスの抗腫瘍効果は、ウイルスのクリアランス後であっても持続し、さらには1または複数の非感染腫瘍にさえ及ぶ。
2G2、IF7、ACT35、mAb L106およびmAb OX86が含まれる。他のOX40アゴニストには、その全内容が参照によって本明細書に組み込まれる、米国特許出願公開第US20060281072号に記載されたものなどが含まれる。

前記アデノウイルスが、E3遺伝子領域の一部または全部の欠失を含む、項目2に記載の複製コンピテントな腫瘍溶解性アデノウイルス。
(項目4)
前記異種核酸が、前記アデノウイルスのE3が欠失した前記遺伝子領域に挿入されている、項目3に記載の複製コンピテントな腫瘍溶解性アデノウイルス。
(項目5)
前記OX40アゴニストがOX40リガンド(OX40L)(gp36)である、前記項目のいずれかに記載の複製コンピテントな腫瘍溶解性アデノウイルス。
(項目6)
OX40Lをコードする前記核酸が、GenBank受託番号NP_003317.1に示されるアミノ酸配列またはそれと少なくとも95%同一な配列を有するポリペプチドをコードする、項目5に記載の複製コンピテントな腫瘍溶解性アデノウイルス。

前記アデノウイルスが、前記アデノウイルスの前記ファイバー遺伝子の前記HIループ領域に挿入されたEGFRvIIIもしくはその免疫原性ペプチドをコードする異種核酸および/または前記アデノウイルスの前記ヘキソン遺伝子の前記超可変領域5に挿入されたNY−ESO−1もしくはその免疫原性ペプチドをコードする異種核酸を含む、項目12に記載の複製コンピテントな腫瘍溶解性アデノウイルス。
(項目15)
前記項目のいずれかに記載の複製コンピテントな腫瘍溶解性アデノウイルスおよび薬学的に許容される担体を含む、医薬組成物。
(項目16)
IL−12p70、IL−2、IFN−γ、レナリドミド、テモゾロミド(4−メチル−5−オキソ−2,3,4,6,8−ペンタアザビシクロ[4.3.0]ノナ−2,7,9−トリエン−9−カルボキサミド)、シクロホスファミド((RS)−N,N−ビス(2−クロロエチル)−1,3,2−オキサアザホスフィナン2−アミン2−オキシド)、ロムスチン(CCNU;N−(2−クロロエチル)−N’−シクロヘキシル−N−ニトロソウレア)、ビス−クロロエチルニトロソウレア(BCNU)、塩酸メルファラン(4[ビス(クロロエチル)アミノ]フェニルアラニン)、ブスルファン(ブタン−1,4−ジイルジメタンスルホネート)、メクロレタミン(ナイトロジェンマスタード)、クロラムブシル、イホスファミド、ストレプトゾシン、ダカルバジン(DTIC)、チオテパ、アルトレタミン(ヘキサメチルメラミン)、シスプラチン、カルボプラチン、オキサリプラチン、イピリムマブ、トレメリムマブ、MDX−1106、MK−3475、AMP−224、ピディリズマブおよびMDX−1105からなる群から選択される1または複数のTh1刺激剤をさらに含む、項目15に記載の医薬組成物。
(項目17)
前記Th1刺激剤が、IFN−γまたはテモゾロミドである、項目16に記載の医薬組成物。
(項目18)
がんの処置を必要とする患者においてがんを処置する方法であって、項目1から14のいずれかに記載の複製コンピテントな腫瘍溶解性アデノウイルスまたは項目15から17のいずれか一項に記載の組成物を、前記患者に投与するステップを含む、方法。
(項目19)
前記患者が、原発性または転移性の脳がん、黒色腫、腺癌、胸腺腫、リンパ腫、肉腫、肺がん、肝臓がん、結腸がん、非ホジキンリンパ腫、ホジキンリンパ腫、白血病、子宮がん、乳がん、前立腺がん、卵巣がん、子宮頸がん、膀胱がん、腎臓がんおよび膵臓がんから選択されるがんを有する、項目18に記載の方法。
(項目20)
前記患者が、低レベルまたは高レベルの神経膠腫を有する、項目19に記載の方法。
(項目21)
前記アデノウイルスが、腫瘍内、血管内に投与されるか、または神経幹細胞担体もしくは間葉系幹細胞担体において投与される、項目18から20のいずれかに記載の方法。
(項目22)
前記アデノウイルスが、腫瘍内投与される、項目21に記載の方法。
(項目23)
前記アデノウイルスが、108〜1013プラーク形成単位(pfu)の用量で、1回または複数回投与される、項目18から22のいずれかに記載の方法。

腫瘍塊中または脈管構造中への有効量の前記アデノウイルスの注射を含む、項目22または23に記載の方法。
(項目25)
腫瘍成長が、注射された腫瘍および少なくとも1つの注射されていない腫瘍の両方において低減される、項目24に記載の方法。

デルタ−24−RGDOXの構築および特徴付け
Type Culture Collection、Manassas、VA)またはGL261細胞におけるD24−RGDおよびD24−RGDOXの複製を試験した。細胞を、12ウェルプレート中に5×104細胞/ウェルの密度で播種し、10pfu/細胞でウイルスに感染させた。感染の48時間後、感染性ウイルス後代を、製造業者の指示に従ってADENO−X Rapid Titer Kit(Clontech、Mountain View、CA)を使用して力価決定した。最終ウイルス力価を、pfu/mlとして決定した。3つの独立した測定の平均±SDとして図5に示す。2つのウイルスの複製を、スチューデントt検定(両側)を使用して比較した。D24−RGDOXは、ヒト神経膠腫U−87mg細胞において、その親ウイルスD24−RGDと同程度に効率的に複製することが示されたが、両方のウイルスは、GL261細胞において非常に不十分に複製する。従って、マウス神経膠腫モデルを用いた、本明細書に記載される抗腫瘍効果は、ヒトにおけるウイルス(OX40Lを発現する)の予測される抗腫瘍効果より顕著に低く現れる。
