Abstract

技术领域

背景技术
当前的器官保存技术通常涉及器官在冰上化学灌注液溶液中的低温储存。然而,使用常规方法导致损伤度根据器官被保持离体的时间长度而增加。这些时间约束限制了可以从给定供体部位到达的受体的数量,从而限制了所获取的心脏用的受体库。即使在几小时的时间限制内,心脏仍可能受到严重损伤。
对离体器官的有效保存还将提供许多其他益处。例如,长时间的离体保存将允许对所获取的器官进行更仔细的监测和功能测试。这将继而允许较早地发现所获取的器官中的缺陷并对该缺陷进行可能的修复,从而进一步降低移植失败的可能性。对器官进行简单修复的能力也将允许保存许多具有较小缺陷的器官,而当前的移植技术则要求不再使用具有较小缺陷的器官。另外,可以实现器官与特定受体之间更有效的匹配,从而进一步降低最终器官排斥的可能性。

为了保持生理性或近生理性的灌注液流经心脏,器官必须经由主动脉而与系统接合。这种接合是经由主动脉插管实现的。当前的主动脉插管设计导致了器官滑移、保持液密密封方面的困难以及主动脉损伤。通常,这些设计仅依赖于与主动脉接触的用以将主动脉张紧至主动脉插管的扎带。根据主动脉的尺寸和主动脉插管的尺寸,由于扎带对主动脉组织施加太大的张力,所以存在撕裂的风险,或者在扎带没有施加足够张力的情况下,存在泄漏的风险。因此,存在对于这样的主动脉插管的需求:该主动脉插管便于医护人员进行部署、与主动脉形成紧密的密封、减少主动脉滑动并且对主动脉造成最小的损伤。
鉴于上述情况,需要用于将主动脉附接至系统的改进的装置以及在离体器官护理系统中使用的方法。
发明内容

本发明的一个方面包括使用主动脉插管将心脏安置成与器官护理系统流体连通的方法,该方法包括:选择主动脉插管,该主动脉插管的尺寸设定成适配于心脏的主动脉,该主动脉插管包括插管本体和枢转臂带,该插管本体进一步包括适于连接至器官护理系统的配装部以及与主动脉接触的主动脉接合部,该枢转臂带可操作地连接至枢轴安装件,其中,枢轴安装件允许枢转臂带均匀地接触主动脉以将主动脉保持在主动脉接合部上;按压枢转臂以使得:枢转臂绕销钉旋转并且枢转臂带背离插管本体移动;将插管安置在主动脉中;释放枢转臂;将扎带围绕枢转臂带张紧以将主动脉保持就位;以及将渐缩的配装部插入器官护理系统中。在一个实施方式中,该方法还包括如下步骤:在将主动脉安置在主动脉插管上之前,在主动脉上缝合手术用毡制拭子。

插管本体
图1是描绘一个实施方式中的主动脉插管100的图。主动脉插管装置100包括插管本体114、锁紧螺母102和枢转臂140。插管本体114可以包括三个子部分,所述三个子部分为渐缩的配装部108、渐缩的中间部分130和主动脉接合部132。这些子部分可以在图1中以及在图2a至图2b中描绘的插管本体114的各个侧视图中看出。在一个实施方式中,插管本体114由注塑模制的透明聚碳酸酯制成。然而,本领域技术人员将理解的是,插管本体可以由其他类型的塑料或任何其他合适的材料制成。

枢转臂带124联接至枢转臂140。枢转臂带在图1和图3中是最佳可见的。如图1中所示,在一个实施方式中,插管本体114包括联接至两个枢转臂140的两个枢转臂带124。本领域技术人员将理解的是,枢转带124的数目对应于枢转臂140的数目。枢转臂带124和滑动枢轴窗口118允许插管本体114均匀地夹持主动脉。枢转臂带124设计成在刚硬得足以保持主动脉的同时保持足以符合主动脉并使组织损伤最小化的柔度。枢转臂带124是弯曲的。枢转臂带124可选地具有环部136和引导部142以保持扎带(未示出)围绕枢转臂带124。扎带由柔性尼龙材料或具有类似特性的材料制成。一旦扎带已经穿过环部136并插入引导部142中,扎带就被张紧至所需的张力。对于所有尺寸的插管而言,扎带被张紧的量是相同的。枢转臂带124中的窗口126通过改变带的与主动脉接触的表面积而使施加在主动脉上的压力标准化。因此,窗口126的尺寸根据主动脉的尺寸而变化。窗口126的尺寸计算成使得:当扎带被张紧时,对于每个尺寸的装置100而言,扎带均在主动脉上施加相同的压缩。因此,施加在主动脉上的压缩将主动脉保持就位而不会损伤组织。本领域普通技术人员将理解的是,替代性地,对于每个尺寸的装置100而言,扎带均可以被张紧至特定的张力。另外,可以使用进行夹紧以将主动脉保持就位的其他机构来替代扎带,例如软管夹紧件或张力带。此外,枢转臂带124和窗口126可以具有不同的形状和尺寸。替代性地,窗口可以省略掉。本领域的技术人员还将理解的是,枢转臂140和枢转臂带124可以分别是一体件的一部分。另外,本领域的技术人员将理解的是,枢转臂带124的内表面可以是平滑的,或者可以是带有纹理的以用于额外的牵引。
在一个实施方式中,主动脉被固定至插管本体。抓握垫116被使用者按压,从而使枢转臂140绕滑动枢轴窗口118移动并对扭转弹簧进行压缩。枢转臂140绕位于滑动枢轴窗口118中的销钉120旋转,并且枢转臂带124背离插管本体114移动,与在枢转点是固定的情况下相比,这以优选的方式让出空间以将插管安置在主动脉中。当抓握垫116被释放时,扭转弹簧(未示出)在枢转臂带124上施加压力并将主动脉暂时保持就位。带靠近主动脉并且滑动枢轴窗口118允许枢转点改变,以补偿组织厚度的变化并且以在整个旋转范围内保持枢转臂140与插管本体114的对齐和同心性。这允许带124均匀地坐置在主动脉上。然后,扎带穿过环部136并且位于引导部142之间。扎带被张紧至预定的张力。本领域的技术人员将理解的是,扎带可以用用于固定枢转臂带124的其他机构来替代。在一些实施方式中,扎带可以预先组装在环部136中。
拭子
在一些实施方式中,使用者可以将手术用毡制拭子缝合在主动脉上。拭子用作将主动脉保持在插管本体114上的附加措施,原因在于拭子提供了在枢转臂带124与插管本体114之间不滑动的隔挡件。四组两个拭子(一个内拭子,一个外拭子)围绕主动脉均匀间隔开的并且被缝合。本领域技术人员将认识到的是,可以使用更多或更少的拭子。在一个实施方式中,主动脉在插管本体114上定位成使得拭子不位于枢转臂140之间的空间正上方,以防止拭子滑动穿过枢转臂带124的两个侧部之间的空间。本领域技术人员将认识到的是,拭子可以安置在主动脉上的任何位置并且最终安置成相对于枢转臂带处于任何取向。拭子可以是标准的手术用毡制拭子。替代性地,拭子可以是由高硬度材料比如硅树脂或类似材料制成的注塑模制的、刚性的、弹性体的拭子。本领域技术人员将理解的是,拭子可以用附接至组织的其他材料来替代,并且提供锚固件以防止装置在带与插管本体之间滑动或防止装置损坏组织。这些材料件的示例包括但不限于附接至组织的连续材料环或钉装件(staple)。
梢端保持件
图6描绘了梢端保持件601。梢端保持件601通常是筒形的,但梢端保持件601可以具有其他形状。梢端保持件具有手柄603。手柄可以采取允许使用者握持梢端保持件601的任何形状。梢端保持件601还可以具有螺纹602。锁紧螺母102可以旋拧到螺纹602上。梢端保持件601还可以具有止挡部604,止挡部604从梢端保持件601突出并且用作对锁紧螺母102的止挡点。本领域技术人员将理解的是,可以使用其他设计来将锁紧螺母附接至梢端保持件。替代性地,梢端保持件可以使用本领域技术人员已知的其他机构而被固定至主动脉插管100。一旦被固定,梢端保持件就可以在心脏定位或未定位在主动脉插管100上的情况下被用来保持主动脉插管100。
示例1
主动脉插管100可以用于将心脏连接至器官腔室(未示出)。主动脉插管100保持主动脉打开并将主动脉保持就位,并且允许灌注液被灌注通过心脏,因此心脏可以保持处于近生理条件下。在一个实施方式中,为了部署主动脉插管,使用者首先选择其尺寸设定成适配于心脏的主动脉插管100。在一个实施方式中,可以通过测量主动脉来选择主动脉插管100。使用者按压位于弹簧加载的枢转臂上的指垫116。当使用者按压抓握垫116时,枢转臂140绕位于滑动枢轴窗口118内的销钉120旋转,并且枢转臂带124背离插管本体114移动,从而让出空间以将插管安置在主动脉中。使用者可以将插管安置在主动脉中。然后使用者释放指垫,从而允许枢转臂140靠近主动脉。枢转臂140可以同时或单独地操作。由扭转弹簧产生的压力暂时将主动脉保持就位。在需要的情况下,使用者可以调节主动脉位置,使得主动脉完全接合在插管本体114上。接下来,使用者将扎带经由环部136和引导部142安置在枢转臂带124中。然后,使用者张紧扎带以将主动脉保持就位。在一些实施方式中,扎带可以使用将扎带张紧至预定力的工具而被张紧。使用者将渐缩的配装部108插入器官腔室(未示出)。然后,使用者将锁紧螺母102拧紧。本领域的技术人员将认识到的是,在一些实施方式中,主动脉插管100可以首先坐置在器官腔室内,并且然后主动脉可以固定至主动脉插管100。
Claims (23)
1.一种用于在离体器官护理系统中使用的主动脉插管,所述主动脉插管包括:
插管本体,所述插管本体包括:
适于连接至所述离体器官护理系统的配装部,以及
构造成与主动脉的内表面接触的主动脉接合部;
两个或更多个枢轴安装件;
两个或更多个枢转臂带,所述枢转臂带可操作地连接至所述两个或更多个枢轴安装件,其中,所述两个或更多个枢轴安装件构造成在所述主动脉的所述内表面与所述主动脉接合部接触之后允许所述两个或更多个枢转臂带移动以与所述主动脉的外表面接触从而将所述主动脉保持在所述主动脉接合部上;以及
枢转臂,所述枢转臂连接至所述两个或更多个枢转臂带的第一枢转臂带并且连接至所述两个或更多个枢轴安装件的第一枢轴安装件,
其中,所述枢转臂构造成使得所述枢转臂的抓握垫朝向所述插管本体的移动使得所述第一枢转臂带背离所述插管本体移动。
2.根据权利要求1所述的主动脉插管,其中,所述枢转臂的所述抓握垫是构造成按压所述枢转臂的顶部表面的指垫。
3.一种用于在离体器官护理系统中使用的主动脉插管,所述主动脉插管包括:
插管本体,所述插管本体包括:
适于连接至所述离体器官护理系统的配装部,以及
构造成与主动脉的内表面接触的主动脉接合部;
两个或更多个枢轴安装件;
两个或更多个枢转臂带,所述枢转臂带可操作地连接至所述两个或更多个枢轴安装件,其中,所述两个或更多个枢轴安装件构造成在所述主动脉的所述内表面与所述主动脉接合部接触之后允许所述两个或更多个枢转臂带移动以与所述主动脉的外表面接触从而将所述主动脉保持在所述主动脉接合部上;以及
枢转臂,所述枢转臂连接至所述两个或更多个枢转臂带的第一枢转臂带并且连接至所述两个或更多个枢轴安装件的第一枢轴安装件,其中,所述枢转臂和所述第一枢转臂带形成单一塑料构件。
4.一种用于在离体器官护理系统中使用的主动脉插管,所述主动脉插管包括:
插管本体,所述插管本体包括:
适于连接至所述离体器官护理系统的配装部,以及
构造成与主动脉的内表面接触的主动脉接合部;
两个或更多个枢轴安装件;
两个或更多个枢转臂带,所述枢转臂带可操作地连接至所述两个或更多个枢轴安装件,其中,所述两个或更多个枢轴安装件构造成在所述主动脉的所述内表面与所述主动脉接合部接触之后允许所述两个或更多个枢转臂带移动以与所述主动脉的外表面接触从而将所述主动脉保持在所述主动脉接合部上;
枢转臂,所述枢转臂连接至所述两个或更多个枢转臂带的第一枢转臂带并且连接至所述两个或更多个枢轴安装件的第一枢轴安装件;以及
弹簧,所述弹簧构造成向所述第一枢转臂带施加压力以将所述主动脉保持在所述主动脉接合部上。
5.根据权利要求4所述的主动脉插管,包括销钉,所述销钉构造成与所述第一枢轴安装件以及与所述枢转臂相联以允许所述枢转臂关于所述第一枢轴安装件旋转。
6.一种用于在离体器官护理系统中使用的主动脉插管,所述主动脉插管包括:
插管本体,所述插管本体包括:
适于连接至所述离体器官护理系统的配装部,以及
构造成与主动脉的内表面接触的主动脉接合部;
两个或更多个枢轴安装件;
两个或更多个枢转臂带,所述枢转臂带可操作地连接至所述两个或更多个枢轴安装件,其中,所述两个或更多个枢轴安装件构造成在所述主动脉的所述内表面与所述主动脉接合部接触之后允许所述两个或更多个枢转臂带移动以与所述主动脉的外表面接触从而将所述主动脉保持在所述主动脉接合部上;
枢转臂,所述枢转臂连接至所述两个或更多个枢转臂带的第一枢转臂带并且连接至所述两个或更多个枢轴安装件的第一枢轴安装件;其中,所述两个或更多个枢转臂带中的所述第一枢转臂带包括环部和引导部,所述环部和所述引导部构造成保持扎带围绕所述第一枢转臂带。
7.根据权利要求6所述的主动脉插管,其中,所述两个或更多个枢转臂带中的所述第一枢转臂带包括窗口,所述窗口的尺寸设定成将由所述扎带施加在所述主动脉上的压缩标准化成使得:对于给定的扎带张力而言,不论所述第一枢转臂带的尺寸如何,都将在所述主动脉上施加相同量的压力。
8.根据权利要求1、3、4或6所述的主动脉插管,包括能够构造成将所述主动脉插管可逆地固定至器官腔室的连接器。
9.根据权利要求8所述的主动脉插管,其中,所述连接器是带有螺纹的锁紧螺母。
10.根据权利要求1、3、4或6所述的主动脉插管,其中,所述两个或更多个枢转臂带成形为当所述主动脉被置于所述主动脉接合部上时均匀地接触所述主动脉。
11.一种用于在离体器官护理系统中使用的主动脉插管,所述主动脉插管包括:
插管本体,所述插管本体包括:
配装部,所述配装部成形为连接至器官腔室组件;
主动脉接合部,所述主动脉接合部构造成适配在主动脉内;和
枢轴安装件;
枢转臂,所述枢转臂可操作地连接至所述枢轴安装件,所述枢转臂包括:
枢转臂带,所述枢转臂带构造成将所述主动脉保持抵靠所述主动脉接合部;以及
滑动枢轴窗口,所述滑动枢轴窗口构造成允许所述枢转臂带通过一定范围的运动而保持与所述主动脉接触;以及
销钉,所述销钉可操作地连接至所述枢轴安装件并且布置在所述枢转臂的所述滑动枢轴窗口中,其中,所述枢转臂构造成绕所述销钉旋转,并且其中,所述枢转臂构造成允许所述销钉沿着所述滑动枢轴窗口的长度平移。
12.根据权利要求11所述的主动脉插管,其中,所述插管本体包括渐缩的中间部分,所述渐缩的中间部分从所述配装部延伸至所述主动脉接合部,并且其中,所述枢转臂包括能够构造用于按压所述枢转臂的顶部的抓握垫。
13.根据权利要求11所述的主动脉插管,包括锁紧螺母,所述锁紧螺母能够构造成将所述主动脉插管可逆地固定至所述器官腔室组件。
14.根据权利要求11所述的主动脉插管,包括弹簧,所述弹簧构造成在所述枢转臂和所述插管本体之间施加力,其中,所述力将所述枢转臂朝向所述主动脉接合部引导,以在所述主动脉被置于所述主动脉接合部上时将所述主动脉固定就位。
15.根据权利要求11所述的主动脉插管,其中,所述枢转臂带包括环部和引导部,所述环部和所述引导部构造成保持扎带围绕所述枢转臂带。
16.根据权利要求15所述的主动脉插管,包括窗口,所述窗口的尺寸设定成将由所述扎带施加在所述主动脉上的压缩标准化成使得:对于给定的扎带张力而言,不论所述枢转臂带的尺寸如何,都将在所述主动脉上施加相同量的压力。
17.一种用于在离体器官护理系统中使用的主动脉插管,所述主动脉插管包括:

构造成与主动脉接触的主动脉接合部;

两个或更多个枢转臂带,所述枢转臂带可操作地连接至所述两个或更多个枢轴安装件,其中,所述两个或更多个枢转臂带成形为当所述主动脉被置于所述主动脉接合部上时均匀地接触所述主动脉,并且其中,所述两个或更多个枢轴安装件构造成允许所述两个或更多个枢转臂带移动以与所述主动脉接触从而将所述主动脉保持在所述主动脉接合部上,以及
枢转臂,所述枢转臂连接至所述两个或更多个枢转臂带的第一枢转臂带并且连接至所述两个或更多个枢轴安装件的第一枢轴安装件,其中,所述枢转臂构造成使得所述枢转臂的抓握垫朝向所述插管本体的移动使得所述第一枢转臂带背离所述插管本体移动。
18.根据权利要求17所述的主动脉插管,其中,所述枢转臂和所述第一枢转臂带形成单一塑料构件。
19.根据权利要求17所述的主动脉插管,包括弹簧,所述弹簧构造成向所述第一枢转臂带施加压力以将所述主动脉保持在所述主动脉接合部上。
20.根据权利要求19所述的主动脉插管,包括销钉,所述销钉构造成与所述第一枢轴安装件以及与所述枢转臂相联以允许所述枢转臂关于所述第一枢轴安装件旋转。
21.根据权利要求17所述的主动脉插管,其中,所述枢转臂的所述抓握垫是构造成按压所述枢转臂的顶部表面的指垫。
22.根据权利要求17所述的主动脉插管,其中,所述两个或更多个枢转臂带中的第一枢转臂带包括环部和引导部,所述环部和所述引导部构造成保持扎带围绕所述第一枢转臂带。
