Abstract

背景技术
感染仍然是当今医疗行业的一大问题。感染通常由血管内(IV)管线(例如静脉内、动脉内等)的污染、注射部位或(例如静脉、动脉或毛细管的)抽血部位的污染、导尿管、伤口部位或切口部位的污染、以及医疗设施中很多其它感染源导致。例如,仅在美国医院中,中心静脉导管每年导致大约250,000例血流感染,在财政资源消耗和患者发病率方面产生大量费用—医学博士O'Grady等,Guidelines for the Prevention of IntravascularCatheter-Related Infections(血管内导管相关感染的预防指南),2011,疾病控制中心,卫生和人类服务部。这些数字不包括因注射部位、抽血部位、非静脉导管的污染、或医疗设施中任意其它多种污染源导致的感染。感染在发展中国家更是个问题,在发展中国家,针筒、IV管线(line)及其它器械通常会给多个不同的患者使用及再次使用。

具体实施方式
综述
本发明描述了减少和/或防止感染的方法。在一方面中,本发明描述了可单独使用或与一个或多个医疗设备结合使用,以对血管内(IV)管线端口、其它IV部件(例如注射器、腔(lumens)、阀等)、注射部位、(例如静脉、动脉或毛细管的)抽血部位、伤口部位、切口部位、腹膜透析部位、膀胱或肾造口术部位、其它引流部位或任意其它易受感染部位进行清洁和/或消毒的示例性抗菌成分。
示例性端口保护和清洁设备
图1A至图5C示出多个示例性端口(Port)清洁和保护帽的细节。每个帽可以由例如(但不限于)聚乙烯、聚丙烯、共聚物和/或它们的混合物制成。所述帽还可以包括紫外线防护材料或药剂,以在存储、运输等期间保持过氧化氢的完整性。所述帽本身可以另外地或替代地容纳在含有紫外线防护材料的封装中以抑制过氧化氢的分解。
图1A和图1B示出IV端口保护帽100的示例,其设计用以旋接在螺纹端口上,例如凹形的连接件,以提供抵抗再污染的物理屏障。如图1A所示,保护帽100由保护盖102气密密封。通过声波焊接、微波焊接、热熔接或其它结合技术,保护帽100可拆卸地熔接或结合至保护帽100。保护盖102可以由与保护帽100相同或不同的材料制成。为了促使保护盖102密封至保护帽100,如图1B所示,保护帽包括置于围绕保护帽100开口的边缘106或凸缘的顶面上的能量导向器104。能量导向器104包括具有相对于保护帽100的边缘106较小的横截面的凸起的脊或肋条。能量导向器104的小横截面允许能量导向器更迅速地熔化,并且以相较于熔化保护帽100的整个边缘106所需的更小的能量与保护盖102熔接。能量导向器104还允许保护盖102在相对薄的区域上熔接至保护帽100,由此使得保护盖102更容易地从保护帽100上移除(相较于在保护帽100的边缘106的整个面积上熔接)。
边缘106设计成“非接触边缘”,其从保护帽100的主体的周边径向延伸,由此将使用者的手指在使用期间接触保护帽的内表面的可能性降至最低。在所示实施例中,能量导向器104设置在保护帽的开口的径向以外,边缘106的外边沿以内。这就确保了边缘106处于能量导向器104以内的部分在使用前保持无菌。非接触边缘106增大了这种可能性,即边缘106处于能量导向器104以内的部分甚至在使用中保持无菌。在其它实施例中,能量导向器能够设置在边缘106上的任意位置(例如集中如图所示,在边缘的邻近开口的内周边处,或是在边缘的外周边处)。
如图1B所示,保护帽100还包括涂料器材料108(为简明起见在该图中以分解视图示出)。在所示示例中,涂料器材料包括圆柱形泡沫状材料,所述圆柱形泡沫材料具有围绕圆柱体侧面的圆周的开放微孔(cell)区域110以及位于圆柱体的一个或两个轴向端部上的封闭微孔区域112。开放微孔区域110使得涂料器材料108能够吸收并携载抗菌成分,例如下述的那些抗菌成分。封闭微孔区域112用于至少部分地覆盖并密封IV端口的端部以防止IV端口泄露并防止相当数量的抗菌成分进入IV端口。开放微孔区域110和封闭微孔区域112均能够具有一定量的纹理或粗糙度以擦磨IV端口。在一些实施例中,涂料器材料能够包括聚氨酯、硅、硅橡胶、聚乙烯、聚丙烯、热塑性弹性体或类似材料,以及它们的混合物。
类似地,尽管涂料器材料108示出为大体圆柱体,在其它实施例中,涂料器材料还能够具有其它形状和/或尺寸。并且,涂料器材料108能够包括不同的表面处理(例如刀槽花纹、裂纹等)、表面抛光(例如宏观的、微观的或纳米结构等)、和/或轮廓(例如圆化的、有棱纹的、有突起的、指状等)。
图2A至图2C分别示出IV端口保护帽的多个变体200A、200B和200C(总体称为保护帽200),配合多种外直径(OD)的IV端口连接件使用,例如凸形连接件。这些实施例的保护帽200是滑动配合帽,在于它们被设计用以滑盖在并且牢固地配合在不同外直径的IV端口连接件上,因为不是所有的端口连接件都具有标准化的外直径。
图2A示出具有台阶状内表面的保护帽200A,包括第一内表面202和第二内表面204,第二内表面204具有相对于第一内表面较小的平均直径。第一内表面202和第二内表面204的直径选定为匹配市场上普通端口的外直径、或选定为市场上端口的最大和最小外直径、或基于其它标准。并且,第一和第二内表面202和204均能够是锥形的(即具有拔模角度θ),使得第一和第二内表面的直径在最接近保护帽200A的开口的位置最大,并且朝向保护帽的底部闭合端减小。第一内表面202的拔模角度能够等于、大于或小于第二内表面204的拔模角度。当保护帽200A位于IV端口上时,保护帽200A将滑盖在IV端口上,直至IV端口的外直径在第一内直径202(在IV端口具有相对大的外直径的情况)或第二内直径204(在IV端口具有相对小的外直径的情况)处抵触并密封保护帽200A的内部表面。
图2B和图2C分别示出滑动配合保护帽的替代实施例200B和200C,它们具有连续的光滑的内表面。相比于图2A的实施例示出的台阶状,保护帽200B和200C具有连续的光滑的内表面。与台阶状保护帽200的内表面相似,保护帽200B和200C的内表面也是锥形的以适应不同外直径的IV端口。但是,为了适应具有较大范围的外直径的IV端口,保护帽的拔模角度θ在如保护帽200B的例子中需要更大(即更明显的锥度),和/或在如保护帽200C的例子中保护帽需要制造地更深。
图3示出IV端口清洁系统300示例,包括清洁帽302,所述清洁帽具有携带抗菌成分的涂料器材料304。清洁帽302能够用于清洁IV端口306。在示出的示例中,IV端口包括凹形的连接件。但是,在其它实施例中,这种IV端口清洁系统能够用于或适于用来清洁凸形的 连接件,以及其它类型的IV和非IV端口和/或腔。清洁帽302能够用于(例如)在将端口连接至互补(complimentary)端口、注射药物、抽血或以其它方式使用IV端口306之前给IV端口306消毒。在消毒以后,抗菌成分的挥发性组分可能随时间的经过而挥发,留下乙二胺四乙酸(EDTA)的薄膜或隔层308或对污染物提供持续抵御的其它螯合剂。
一旦消毒,如上文所述的一个保护帽能够应用于IV端口306以提供抵抗再污染的物理屏障。保护帽可以包括相同或不同的抗菌成分或其它成分。在一些实施方式中,保护帽能够牢固地联结至IV端口306并且密封IV端口。在保护帽中的抗菌成分随时间变干的情况中,保护帽仍能包括EDTA或其它螯合剂的残留隔层,所述残留隔层将提供对污染物的进一步的抵御。在其它实施例中,保护帽能够在保护帽的内部表面和/或外部表面的整体或部分上简单地涂覆EDTA或其它螯合剂的薄膜或隔层。
图4A至图4C更详细地示出图3所示的IV端口清洁和消毒系统300。具体地,图4A示出使用之前的清洁帽302。如图4A所示,清洁帽302能够与保护盖400一起使用,保持清洁帽302的内部处于无菌状态。另外地或替代地,清洁帽302能够封装在另一无菌封装件(未在此图中示出)中以保持整个清洁帽302在使用前处于无菌环境。为了使用清洁帽302来清洁IV端口306,保护密封层能够被移除,露出帽302的内部,如图4B所示。清洁帽302随后能够放置在IV端口306上并旋拧或者以其它方式用于擦拭并清洁IV端口306。具体地,通过抵靠IV端口306旋拧并按压清洁帽302,携带抗菌成分的涂料器材料304能够用于擦拭IV端口306的外部表面。
如果IV端口306被细菌、孢子、寄生菌、病毒、体液或其它污染物污染,抗菌成分将开始发泡或起泡沫402,提供对污染物的可视化的指示。发泡或起泡沫反应是由于过氧化氢与细菌、孢子、寄生菌以及病毒发生作用导致的。过氧化氢还将产生气泡或泡沫,作为对与体液中的血红蛋白或血小板的芬顿反应(Fenton reaction)的响应。气泡形成的尺寸和速率能够表明污染程度,给予医务人员关于设备被污染并且可能需要进一步清洁和/或更换以避免感染的可视化的指示。
图5A至图5C示出保护帽500的示例,在一些实施例中,保护帽500能够与图3和图4A至图4C中的清洁帽302结合使用。在IV端口306已经被清洁之后,保护盖502能够从保护帽500上移除以露出保护帽的内部,如图5A和图5B共同示出。与清洁帽302一样,保护帽500也能够封装在另一无菌封装件(未在此图中示出)中,以保持整个保护帽500在使用前处于无菌状态。
保护帽500随后能够应用于IV端口306以提供抵抗再污染的物理屏障,如图5C所示。保护帽500可以包括与清洁帽302相同或不同的抗菌成分或其它成分。如果当保护帽500联结至IV端口306时发生发泡或起泡504,这种发泡或起泡反应将提供给医务人员关于IV端口306仍被污染并且需要进一步清洁和/或替换的可视化指示。
在一些实施例中,清洁帽302、保护帽500或这两者中的抗菌成分可以包括染色剂或着色剂(其用以进一步强化污染物的可视化指示)。当被提供时,清洁帽302中的染色剂或着色剂可以具有与保护帽500相同或不同的颜色。例如,染色剂或着色剂的颜色可以匹配相应帽的颜色。在另一示例中,染色剂或着色剂可以具有与相应帽的颜色形成对比的颜色。
示例性的IV端口清洁和保护设备的另外的细节可参见Tennican于2007年5月8日提交的申请号为11/745,843的美国专利申请,该申请以引用的方式合并于此。
示例性的部位准备设备
图6A和图6B示出部位清洁和消毒设备600的示例,其能够用于对血管内(IV)管线端口、其它IV部件(例如注射器、腔、阀等)、注射部位、(例如静脉、动脉或毛细管的)抽血部位、导管和/或导管插入部位、伤口部位、切口部位、腹膜透析部位、引流部位或任意其它易受感染的部位进行清洁和/或消毒。
设备600包括由保护盖604密封的外壳或帽602(在该图中以虚线显示,以示出设备600的内部特征)。帽602可以包括图1A和图1B中的保护帽100并且以与图1A和图1B中保护帽100相似的方式构造。例如,帽602能够构造为包括“非接触边缘”。另外,保护盖604能够采用与上述针对图1A和图1B中的保护帽100描述的技术相同的技术密封至帽602。例如,并且如上文所述,保护盖604能够熔接于能量导向器以帮助移除保护盖。容纳在帽602之内的是涂料器材料608,例如泡沫或海绵材料,其装有例如上述那些抗菌成分。当保护盖604就位后(如图6A所示),涂料器材料606处于压缩状态,这样,当保护盖604移除后,涂料器材料606膨胀并从帽602的内腔中伸出(如图6B所示),用于清洁或消毒所需部位。
在一些实施例中,帽602能够包括柔性的和/或穹状的底部表面608,其能够被按压(如图6B的箭头所示)以帮助涂料器材料606从帽602的内腔中展开。
在该实施例中的涂料器材料606可以包括开放微孔泡沫材料、包括开放微孔区域和封闭微孔区域的泡沫材料、海绵材料、研磨材料、网状材料或任意其它适于对部位进行清洁和消毒的材料。在一些实施例中,涂料器材料可以包括聚氨酯、硅、硅橡胶、聚乙烯、聚丙烯、热塑性弹性体等材料及它们的混合物。而且,尽管涂料器材料606示出为大体圆柱形体,然而在其它实施例中,涂料器材料可以采用其它形状和/或尺寸。并且,涂料器材料606可以包括不同的表面处理(例如切沟、切口等)、表面抛光(例如宏观结构、微观结构或纳米结构等)和/或轮廓(例如圆化的、有棱纹的、突起、指状物等)。
清洁设备的示例性封装件
上述多种端口和/或部位清洁设备中的任意端口和/或部位清洁设备能够独立封装或与多个设备配套封装在多个封装件中。
图7A和图7B示出封装这里所述的端口和/或部位清洁设备的两个示例方式。如图7A所示,通过将所述设备夹在多层热塑材料之间并且围绕设备的外周缘将材料片彼此密封(例如通过声波焊接、微波焊接、热粘合或类似方式),每个设备能够单独地密封在小袋或小包700中。
通过将多个设备放置在热塑材料片之间并且随后将所述材料片相对于彼此围绕每个设备的外周缘密封(使用上述任意密封方法),图7A中所描述的方法能够扩展为同时将多个设备封装成条形件。结果是包含多个独立密封的设备的条形件702。独立的设备可以接着通过在条形件702中的设备之间进行切割而分配。替代地,条形件702可以包括位于条形件702中的独立设备之间的穿孔或划线。
图8A和图8B示出端口和/或部位清洁设备的两个示例封装件。如图8A所示,封装件800A包括多个不同的、独立封装的部位清洁设备,它们联结在一起形成条形件802A或套件。条形件802A能够根据参照图7A和图7B描述的方法或其它方法进行封装。在所示示例中,封装件800A包括肥皂包(soap package)804、第一帽设备806A、第二帽设备806B、以及第三帽设备806C(总体称为帽设备806)。肥皂袋804能够用于提供部位的初始清洁,并且包括医疗工业中任意已知的清洁剂。不同的帽设备能够包括端口清洁帽、保护帽、部位清洁帽、或这些帽的组合。在一些示例中,帽设备806能够对应于具有不同抗菌成分和/或不同浓度的抗菌成分的帽。
在一个特定示例中,第一帽设备806A(例如黄帽)能够具有相对高浓度的过氧化氢、酒精、和/或螯合剂,第二帽设备806B(例如蓝帽)能够具有比第一帽设备浓度要低的过氧化氢、酒精和/或螯合剂,第三帽设备806C(例如绿帽)能够具有比第二帽设备更低浓度的过氧化氢、酒精和/或螯合剂。在这种情况中,第一帽设备806A能够用于首先清洁部位,然后是第二帽设备806B,最后是第三帽设备806C。较高浓度的抗菌成分能够提供较高程度的消毒,但是可能会导致对患者组织的刺激(特别是如果允许保持与组织接触很久一段时间)。上述方法能够用于通过使用第一帽提供高程度的消毒,然后由第二帽和第三帽除去较高浓度的抗菌成分,从而使刺激患者组织的风险降至最低。
图8B示出封装件800B,其与封装件800A相同,除了封装件800B包括第四帽设备806D而不是肥皂袋804。第四帽设备806D可以包括类似于肥皂袋804的肥皂成分,或者可以包括具有比第一帽806A更高浓度的过氧化氢、酒精和/或螯合剂的抗菌成分。
示例性的抗菌成分
在一个示例实施方式中,能够与这里所述的方法结合使用的抗菌成分可以包括例如Tennican等于2010年9月1日提交的申请号为12/874,188的美国专利申请中描述的那些,其以引用的方式合并于此。在这种情况中,抗菌成分能够包括水(H2O)、浓的且无毒的螯合剂例如乙二胺四乙酸(EDTA)(例如二钠EDTA、二钠钙EDTA、镁EDTA、镓EDTA)或柠檬酸钠(或酸、盐、派生物或其它形式的EDTA或柠檬酸钠)、短链一元醇(例如具有分子式C2H5OH和实验式C2H6O的乙醇),以及浓的、小分子氧化剂(例如过氧化氢(H2O2))。在一个特定示例中,所述成分能够本质上由水、EDTA、乙醇和过氧化氢构成。但是,在其它示例中,其它抗菌成分能够结合本申请所述设备使用。
所述抗菌成分可以是例如液体形式、胶凝形式或泡沫形式,并且能够基于特定应用的需求结合一种或多种载体或稀释剂。例如,在抗菌成分用作洗手液的应用中,抗菌成分能够采用胶凝形式。在其它示例中,如果抗菌成分用作清洁剂、冲洗液、或刺激剂,抗菌成分能采用液体形式。在这种情况中,多种成分的浓度能够基于例如所需的消毒程度,所述成分是否直接用于活的组织或医疗设备,和/或以避免对所述成分将直接或间接(例如经由所述成分应用的医疗设备)用在的组织的刺激。在其它示例中,液体形式的抗菌成分能够汽化或雾化以用于患者的鼻腔通道或其它呼吸道。在其它示例中,抗菌成分能够包括或结合润滑剂(例如甘油)、表面活性剂或乳化剂(例如甘油单月桂酸酯(GML))或类似,并且能够用于导管、气管导管、视镜(scope)、仪器或其它用于插入患者体内的设备。
结论
尽管本发明描述了具有具体结构特征和/或方法步骤的实施例,可以理解,权利要求并不必须限定为所述的特定特征或步骤。反而,所述特定特征和步骤仅是落入本申请的权利要求范围内的解释性实施例。
Claims (22)
1.一种医疗设备,包括:
帽,包括:
圆柱形腔体,所述圆柱形腔体具有开口和在开口处的凸缘;
构造为与使用者配合的外表面;
处于所述圆柱形腔体中的内表面;以及
能量导向器,所述能量导向器位于所述凸缘上并且垂直于所述圆柱形腔体的开口延伸;
能量导向器包括具有相对于帽的凸缘较小的横截面的凸起的脊或肋条,能量导向器的小横截面允许能量导向器更迅速地熔化,并且以相较于熔化帽的整个凸缘所需的更小的能量与保护盖熔接,能量导向器还允许保护盖在相对薄的区域上熔接至帽,由此使得相较于在帽的凸缘的整个面积上熔接,保护盖更容易地从帽上移除。
2.根据权利要求1所述的医疗设备,其中,所述帽包括聚丙烯、聚乙烯、共聚物材料或它们的混合物。

10.一种医疗设备,包括:
帽,该帽具有内表面和凸缘,所述内表面具有圆柱形腔体,所述凸缘位于所述圆柱形腔体的开口处,用以附接保护盖,其中所述圆柱形腔体的开口处的凸缘还包括从圆柱形腔体向远端地延伸的凸起脊部,所述凸起脊部构造为与所述可移除的保护盖熔接,所述脊部具有相对于帽的凸缘较小的横截面,所述小横截面允许所述脊部更迅速地熔化,并且以相较于熔化帽的整个凸缘所需的更小的能量与保护盖熔接,所述脊部还允许保护盖在相对薄的区域上熔接至帽,由此使得相较于在帽的凸缘的整个面积上熔接,保护盖更容易地从帽上移除;以及
泡沫插入件,所述泡沫插入件位于所述帽的圆柱形腔体内,所述泡沫插入件由保护盖的附接而被压缩,并且构造为当所述保护盖移除时延伸超出所述圆柱形腔体的开口。
11.根据权利要求10所述的医疗设备,其中所述泡沫插入件为圆柱形,包括内部开放微孔层,所述内部开放微孔层在所述圆柱形的顶部和底部连接两个外部封闭微孔层。
12.根据权利要求11所述的医疗设备,其中所述泡沫插入件的内部开放微孔层包括抗菌成分。
13.根据权利要求12所述的医疗设备,其中所述抗菌成分包括表面活性剂、水、低分子量酒精、过氧化物、过氧化物生成剂或螯合剂中的一个或多个。
14.根据权利要求11所述的医疗设备,其中所述泡沫插入件的外部封闭微孔层包括行道纹理。

17.根据权利要求16所述的设备,其中所述帽的圆柱形腔体的开口处的凸缘构造为从所述圆柱形腔体的外周缘径向延伸。
18.根据权利要求16所述的设备,其中所述泡沫插入件包括抗菌成分,所述抗菌成分包括表面活性剂、水、低分子量酒精、过氧化物、过氧化物生成剂或螯合剂中的一个或多个。
19.根据权利要求16所述的设备,其中所述泡沫插入件包括行道纹理。
20.根据权利要求16所述的设备,其中所述帽包括聚丙烯、聚乙烯、共聚物材料或它们的混合物。
21.一种防止感染源传播的方法,包括:
识别待清洁、净化或消毒的表面;以及
将医疗涂料器应用于所述表面,由此清洁、净化或消毒所述表面,所述医疗涂料器包括:
具有内表面和凸缘的帽,所述内表面具有圆柱形腔体,所述凸缘处于所述圆柱形腔体的开口处,其中所述帽包括能量导向器,所述能量导向器位于所述凸缘上并且垂直于所述圆柱形腔体的开口延伸,能量导向器包括具有相对于帽的凸缘较小的横截面的凸起的脊或肋条,能量导向器的小横截面允许能量导向器更迅速地熔化,并且以相较于熔化帽的整个凸缘所需的更小的能量与保护盖熔接,能量导向器还允许保护盖在相对薄的区域上熔接至帽,由此使得相较于在帽的凸缘的整个面积上熔接,保护盖更容易地从帽上移除;

所述待清洁、净化或消毒的表面为位于医疗设备上的区域。
22.根据权利要求21所述的方法,其中所述清洁、抗菌或消毒剂包括表面活性剂、水、低分子量酒精、过氧化物、过氧化物生成剂或螯合剂中的至少一个。
