Abstract

发明背景
在世界范围内,心力衰竭是死亡率和发病率的主要原因。在心力衰竭更严重的病例(纽约心脏协会第IV级)中,2年死亡率超过50%(Braunwald,E.B.,Heart Disease,第4版,(费城:W.B.Saunders Co.,1992))。心律失常(心力衰竭的共同特征)导致了与该疾病相关的很多人死亡。尤其是,所有心脏病患者中大约50%死于致死性心律失常。心脏中的某些室性心律失常是迅速致死的——被称作“心源性猝死”(SCD)现象。然而,致死性室性心律失常也可发生在年轻、其他方面都健康的个体当中,他们不知道患有结构性心脏病。事实上,室性心律失常是其他方面都健康的个体猝死的最常见的原因。

由上述内容看来,很明显,RyR2通道泄漏与很多病理状态有关——既在患病心脏中又在结构正常的心脏中。因此,修复RyR2泄漏的方法能够预防数百万患者的致死性心律失常。
JTV-519(4-[3-(4-苄基哌啶-1-基)丙酰基]-7-甲氧基-2,3,4,5-四氢-1,4-苯并噻氮单盐酸盐;也称为k201),1,4-苯并噻氮衍生物,是新的钙离子通道调节剂。除了调节心肌细胞Ca2+水平以外,JTV-519还调节豚鼠心室细胞的Na+电流和内向整流器K+电流,并抑制豚鼠心房细胞的延迟整流器K+电流。研究已经显示,JTV-519对儿茶酚胺诱导的心肌损伤、心肌损伤诱导的肌纤维过度收缩、和缺血/再灌注损伤具有很强的心脏保护作用。在实验性肌纤维过度收缩模型中,JTV-519显示了比普萘洛尔、维拉帕米和地尔硫卓更大的心脏保护作用。实验数据也显示,JTV-519通过减少动物模型的细胞内Ca2+超负荷水平,有效地预防了心室缺血/再灌注。
发明概述
本发明基于以下令人惊奇的发现,即RyR2是预防引起运动诱导的心源性猝死(SCD)的心律失常的靶。正如本文所描述的,发明人制造了具有7个不同CPVT突变的突变型RyR2通道,并研究了它们的功能。所有的7个突变型都具有功能缺陷,它导致该通道当在运动过程中受刺激时,变得易于泄漏(SR钙泄漏)。本发明人的研究首次鉴别了SR钙泄漏引起DAD的机制。值得注意的是,突变型CPVT通道缺陷使得该通道看起来像末期心力衰竭患者心脏中的泄漏通道,末期心力衰竭是以致死性心律失常的高发病率为特征的疾病。因此,本发明人在本文中证明了CPVT中的VT机制与心力衰竭中的VT机制是相同的。
发明人还在本文中公开了药物JTV-519(k201),1,4-苯并噻氮家族化合物的一员,修复了RyR2通道的泄漏。正如发明人在本文中所显示的,JTV-519增强了FKBP 12.6对PKA-磷酸化RyR2和对突变型RyR2的结合,否则它们对FKBP 12.6具有减弱的亲和力或不与之结合。JTV-519的这种作用修复了RyR2的泄漏,后者触发致死性心律失常(心源性死亡),并在心力衰竭中促成心肌功能障碍。另外,发明人还开发了JTV-519的新合成法,以及该药物的放射标记形式。

发明的详细描述
如以上所讨论的,儿茶酚胺能多形性室性心动过速(CPVT)是心脏结构正常个体的遗传性疾病。它以应激引起的室性心动过速,一种可引起心源性猝死(SCD)的致死性心律失常为特征。位于肌质网(SR)上的RyR2通道突变已经与CPVT相关联。为了确定构成CPVT中致死性心律失常基础的分子机制,发明人研究了CPVT-相关的突变型RyR2通道(例如S2246L、R2474S、N4104K、R4497C)。

根据本发明的方法,患者、尤其是患者细胞内RyR2-结合的FKBP12.6水平的降低可被限制或阻止。本发明的患者可为任何动物,包括两栖动物、禽类、鱼、哺乳动物和有袋类动物,但优选是哺乳动物(例如,人;家畜,如猫、狗、猴、小鼠或大鼠;或商业动物,如奶牛或猪)。另外,本发明患者是运动引起的心律失常的候选者。运动引起的心律失常是一种心脏病症(例如心室纤维性颤动或室性心动过速,包括导致心源性猝死的任何心脏病),它在患者进行身体运动过程中/之后发生。运动引起的心律失常“候选者”是已知有、或认为有、或怀疑有在身体运动过程中/之后发生心律失常危险的对象。运动引起的心律失常候选者的例子包括但并不限于,已知患有儿茶酚胺能多形性室性心动过速(CPVT)的动物/人;怀疑患有CPVT的动物/人;以及已知有、或认为有、或怀疑有在身体运动过程中/之后发生心律失常危险的,以及将要去运动、目前正在运动、或刚完成运动的动物/人。正如以上所讨论的,CPVT是心脏结构正常的个体中的遗传性疾病。它以应激-引起的室性心动过速——一种可引起心源性猝死的致死性心律失常为特征。在CPVT患者中,身体劳累和/或应激诱导双向性和/或多形性室性心动过速,后者在没有可检测的结构性心脏病的情况下导致心源性猝死(SCD)。CPVT个体在进行运动时有室性心律失常,但在休息时不发生心律失常。
在本发明方法中,患者细胞优选是横纹肌细胞。横纹肌是这样的一种肌肉,其中收缩性肌原纤维的重复单位(肌小节)的所有细胞都按顺序(in registry)排列,形成在光学显微镜水平下可观察到的横纹或斜纹。横纹肌细胞的例子包括但并不限于,随意(骨骼)肌细胞和心肌细胞。在优选实施方案中,本发明方法所用的细胞是人心肌细胞。当用于本文中时,术语“心肌细胞”包括心肌纤维,例如在心脏心肌层中发现的那些。心肌纤维由邻接的心肌细胞,或闰盘处首尾相连的心肌细胞的链组成。这些闰盘具有两种类型的细胞连接:扩展的桥粒,其沿着它们的横向部分延伸;以及间隙连接,它们大部分位于它们的纵向部分。
在本发明方法中,患者细胞内RyR2-结合的FKBP 12.6水平的降低受对患者给予JTV-519的限制或阻止;这也可以通过允许患者细胞和JTV-519之间的接触来实现。JTV-519(4-[3-(4-苄基哌啶-1-基)丙酰基]-7-甲氧基-2,3,4,5-四氢-1,4-苯并噻氮单盐酸盐);也称为k201,是1,4-苯并噻氮衍生物和钙离子通道调节剂。除了调节心肌细胞Ca2+水平以外,JTV-519还调节豚鼠心室细胞的Na+电流和内向整流器K+电流,并抑制豚鼠心房细胞的延迟整流器K+电流。FK506和雷帕霉素是可用于设计其它化合物的药物,该其它化合物稳定为运动引起的心律失常的候选者的患者细胞内的RyR2-FKBP 12.6结合。FK506和雷帕霉素都使FKBP 12.6从RyR2解离。设计和/或筛选与这些药物结构相关、但具有相反作用的化合物是可能的。
在本发明方法中,JTV-519可以治疗组合物的方式对患者进行给药,该组合物包含JTV-519和药学可接受的载体。药学可接受的载体必须是“可接受的”,意思是与组合物的其它成分是相容的,并且对其接受者是无害的。本文采用的药学可接受的载体选自用作药物制剂的物质的各种有机或无机物质,并且它可以镇痛药、缓冲剂、粘合剂、崩解剂、稀释剂、乳化剂、赋形剂、填充剂、助流剂、增溶剂、稳定剂、助悬剂、等张剂、载体和增粘剂的形式掺入。如果需要的话,药物添加剂,例如抗氧化剂、芳香剂、着色剂、香味改进剂、防腐剂和增甜剂也可加入。药学可接受载体的例子包括羧甲基纤维素、微晶纤维素、甘油、阿拉伯胶、乳糖、硬脂酸镁、甲基纤维素、粉末、盐水、海藻酸钠、蔗糖、淀粉、滑石粉和水,除了别的以外。
本发明的药物制剂可用药学领域熟知的方法制备。例如,JTV-519可与载体或稀释剂联合制成混悬剂或溶液。任选地,也可加入一种或多种附加成分(例如缓冲液、调味剂和表面活性剂等)。载体的选择将取决于给药途径。
通过使患者体内的靶细胞(例如心肌细胞)与JTV-519接触,可将JTV-519对患者进行给药。使用用于导入和施用蛋白、核酸和其它药物的已知技术,可将JTV-519与患者细胞接触(例如导入)。将细胞与JTV-519接触(即用JTV-519处理细胞)的方法的例子包括但并不限于吸收、电穿孔、浸渍、注射、导入、脂质体传递、转染、传输、载体和其它药物递送载体和方法。当靶细胞位于患者的特定部分时,希望通过注射或通过其它方法(例如通过将JTV-519导入血液或另一种体液)将JTV-519直接导入细胞。靶细胞可被包含于患者的心脏组织中,并可通过已知技术很容易确定的标准检测方法在患者心脏组织中检测到,其实例包括但并不限于免疫学技术(例如免疫组织化学染色)、荧光显像技术和显微镜技术。
另外,可通过已知方法将本发明的JTV-519对人类患者或动物患者给药,包括但并不限于口服给药、胃肠外给药和透皮给药。优选地,通过筋膜上、囊内、颅内、皮内、鞘内、肌内、眼眶内、腹膜内、脊柱内、胸骨内、血管内、静脉内、实质、皮下或舌下注射,或通过导管,胃肠外施用JTV-519。在一个实施方案中,经由插入患者心脏的导管,通过靶向传递给心肌细胞的方式,将该物质对患者进行给药。
对于口服给药来说,JTV-519制剂可以胶囊、片剂、散剂、颗粒剂或混悬剂的形式呈现。制剂可具有常规添加剂,例如乳糖、甘露醇、玉米淀粉或马铃薯淀粉。制剂也可与粘合剂,例如微晶纤维素、纤维素衍生物、阿拉伯胶、玉米淀粉或明胶一起存在。另外,制剂可与崩解剂,例如玉米淀粉、马铃薯淀粉或羧甲基纤维素钠一起存在。制剂也可与无水磷酸氢钙或淀粉羟乙酸钠一起存在。最后,制剂可与润滑剂,例如滑石粉或硬脂酸镁一起存在。
对于胃肠外给药(即通过消化道以外的途径注射给药)来说,JTV-519可与无菌水溶液联合,优选该水溶液与患者血液等渗。该制剂可如下制备:将固体活性成分溶于包含生理学相容物质,例如氯化钠和甘氨酸等,并且具有与生理条件相容的缓冲pH的水中,以便制备水溶液,然后将所述溶液灭菌。制剂可存在于单位容器或多剂量容器,例如密封安瓿或小瓶中。制剂可通过任何注射方式传递,包括但并不限于,筋膜上、囊内、颅内、皮内、鞘内、肌内、眼眶内、腹膜内、脊柱内、胸骨内、血管内、静脉内、实质、皮下或舌下,或通过插入患者心脏的导管。
对于透皮给药来说,JTV-519可与皮肤吸收促进剂,例如丙二醇、聚乙二醇、异丙醇、乙醇、油酸和N-甲基吡咯烷酮等联合,它们增加皮肤对JTV-519的渗透性,允许JTV-519渗透通过皮肤进入血流。JTV-519/促进剂组合物也还可与聚合物质,例如乙基纤维素、羟丙基纤维素、乙烯/醋酸乙烯酯和聚乙烯吡咯烷酮等联合,以提供凝胶形式的组合物,其可溶于溶剂,例如二氯甲烷,蒸发至所需的粘度,然后应用到背衬材料以提供贴剂。
根据本发明的方法,JTV-519可以有效限制或预防患者,优选患者细胞中RyR2-结合的FKBP 12.6水平降低的量对患者进行给药(JTV-519可与患者细胞接触)。本领域技术人员基于已知方法,包括在体内建立的滴定曲线分析和本文公开的方法和测定法,可很容易地确定该量。JTV-519有效限制或预防患者中RyR2-结合的FKBP 12.6水平降低的合适的量可为约5mg/kg/天到约20mg/kg/天,和/或可为足以达到约300ng/ml到约1000ng/ml的血浆水平的量。优选地,JTV-519的量从约10mg/kg/天到约20mg/kg/天。

在上述方法的各种实施方案中,患者中运动引起的心律失常与VT有关。在优选实施方案中,VT是CPVT。在这些方法的其它实施方案中,患者是引起的心律失常的候选者,包括运动引起的心源性猝死的候选者。
由上述方法看来,本发明还提供了JTV-519在限制或预防为运动引起的心律失常的候选者的患者中RyR2-结合的FKBP 12.6的水平降低的方法中的应用。本发明还提供了JTV-519在治疗或预防患者中运动引起的心律失常的方法中的应用。此外,本发明提供了JTV-519在预防患者中运动引起的心源性猝死的方法中的应用。
正如以上所讨论的和本文所显示的,发明人的数据显示,心脏里安娜碱受体RyR2在丝氨酸2809上的蛋白激酶A(PKA)磷酸化,通过释放FK506结合蛋白FKBP 12.6,激活了通道。在衰竭的心脏中(包括心力衰竭的人心脏和动物模型),RyR2是PKA-高度磷酸化的,形成结合FKBP 12.6的量减少和对钙引起的激活敏感性提高的有缺陷的通道。这种改变的纯粹结果就是RyR2通道是“泄漏的”。这些通道泄漏可以导致细胞内钙储备的耗竭,以达到肌质网(SR)内不再有足够的钙来给肌肉收缩提供强刺激的程度。这导致心肌虚弱收缩。作为通道泄漏的又一个后果,RyR2通道在被称为“舒张期”的心脏周期静息相期间释放钙。舒张期期间钙的释放可以触发引起心源性猝死(SCD)的致死性心律失常(例如室性心动过速和心室纤维性颤动)。
发明人还已经显示,用机械泵装置(被称为左心室辅助装置(LVAD),它将心脏置于休息状态并恢复正常功能)治疗心力衰竭,与RyR2的PKA高度磷酸化减少和通道功能正常化有关。此外,发明人还已经显示,用β-肾上腺素能阻断剂(β阻断剂)治疗狗(它具有起搏引起的心力衰竭)逆转了RyR2的PKA高度磷酸化。β阻断剂抑制激活PKA的通路。从发明人的工作结果可得出的结论是,RyR2的PKA高度磷酸化增加了通道活性,导致由于通道的特定触发器(激活剂)而有更多钙释放到细胞内。
如本文进一步公开的,发明人已经确定,运动引起的心源性猝死与RyR2蛋白(尤其是CPVT-相关的RyR2突变体蛋白)的磷酸化增加,以及RyR2-结合的FKBP 12.6的水平降低有关。利用该机制来设计预防运动引起的心源性猝死的有效药物是有可能的。具有限制或预防RyR2-结合的FKBP 12.6水平降低能力的候选物质可能,作为该限制或预防活性的结果,对RyR2-相关的生物学事件有效果,从而预防运动引起的心源性猝死。
因此,本发明还提供了鉴定用于预防运动引起的心源性猝死物质的方法。该方法包括以下步骤:(a)获得或产生包含RyR2的细胞的培养物;(b)使细胞与候选物质接触;(c)将细胞暴露于一种或多种已知增加细胞内RyR2磷酸化的条件下;并且(d)测定该物质是否限制或预防了细胞内RyR2-结合的FKBP 12.6水平的降低。当用于本文中时,“物质”应当包括蛋白质、多肽、肽、核酸(包括DNA或RNA)、抗体、Fab片段、F(ab′)2片段、分子、化合物、抗生素、药物及其任何组合。限制或预防RyR2-结合的FKBP 12.6水平降低的物质可为天然的或合成的,并且可为与RyR2和/或FKBP 12.6有反应性的物质(即对它们有亲和力、与之结合或直接对抗的物质)。如进一步用于本文中的,“包含RyR2”的细胞是其中RyR2、或其衍生物或同系物是天然表达的或天然产生的细胞(优选心肌细胞)。已知增加细胞内RyR2磷酸化的条件包括但并不限于PKA。
在本发明方法中,细胞可与候选物质通过实现药物/物质和细胞之间接触的任何标准方法接触,包括本文所述的任何导入和给药方式。细胞内RyR2-结合的FKBP 12.6水平可通过已知方法测定或检测,包括本领域技术人员已知或本文描述的任何方法、分子操作和测定法。在本发明的一个实施方案中,该物质限制或阻止了细胞内RyR2-结合的FKBP 12.6水平的降低。
如本文公开的,RyR2已经参与了横纹肌细胞内的很多生物学事件。例如,已经显示RyR2通道在心肌细胞内的EC偶联和收缩中挥发着重要作用。因此,设计用于限制或预防细胞尤其是心肌细胞内RyR2-结合的FKBP 12.6水平降低的预防性药物,显然适用于调节很多RyR2-相关的生物学事件,包括EC偶联和收缩。因此,一旦本发明的候选物质已经被筛选,并且已经确定对RyR2-结合的FKBP 12.6的降低水平具有适当的限制或阻止作用,可评价它对细胞尤其是心肌细胞内EC偶联和收缩的影响。预期本发明的预防剂将对预防运动引起的心源性猝死有用。
因此,本发明方法还可包括以下步骤:(e)使候选物质与包含RyR2的细胞的培养物接触;以及(f)测定该物质是否对细胞内RyR2相关的生物学事件有影响。当用于本文中时,“RyR2相关的生物学事件”包括其中涉及RyR2水平或活性的生化或生理过程。如本文公开的,RyR2相关的生物学事件的例子包括但并不限于心肌细胞内EC偶联和收缩。根据本发明的方法,候选物质可在体内与一种或多种细胞(优选心肌细胞)接触。例如,细胞培养物可与包含候选物质的制备物一起培养。然后,候选物质对RyR2相关的生物学事件的作用可通过本领域中已知的任何生物学试验或方法来评价,包括免疫印迹法、单通道记录和本文公开的任何其它方法。
本发明还涉及经上述鉴定方法鉴定的物质,以及包含该物质和药学可接受载体的药物组合物。该物质适用于预防患者中运动引起的心源性猝死,和用于治疗或预防其它RyR2-相关的病况。当用于本文中时,“RyR2-相关的病症”是其中涉及RyR2水平或活性的病况、疾病或障碍,并且包括RyR2-相关的生物学事件。可通过对患者施用有效治疗或预防患者中RyR2-相关的病况的量的物质,来治疗或预防患者中RyR2-相关的病况。本领域技术人员可很容易地确定该量。在一个实施方案中,本发明通过以有效预防患者中运动引起的心源性猝死的量对患者施用该物质,提供了预防患者中运动引起的心源性猝死的方法。
本发明还提供了鉴定用于预防运动引起的心源性猝死的物质的体内方法。该方法可包括以下步骤:(a)获得或产生包含RyR2的动物;(b)对所述动物施用候选物质;(c)将动物暴露于一种或多种已知增加细胞内RyR2磷酸化的条件下;并且(d)测定该物质是否限制或预防动物中RyR2结合的FKBP 12.6水平的降低。该方法还可包括以下步骤:(e)将该物质对包含RyR2的动物施用;以及(f)测定该物质是否对动物中RyR2相关的生物学事件有影响。还提供的是用该方法鉴定的物质;包含该物质的药物组合物;以及通过对患者施用有效预防患者中运动引起的心源性猝死的量的该物质,预防患者中运动引起的心源性猝死的方法。
发明人的工作已经证明,阻断PKA活化的化合物将预期会降低RyR2通道的活化,引起钙较少释放到细胞内。在FKBP 12.6结合位点与RyR2通道结合、但当通道被PKA磷酸化时并不阻断通道的化合物,将也预期会降低通道响应PKA激活或激活RyR2通道的其它触发器的活性。这样的化合物也将导致较少的钙释放到细胞内。由这些发现看来,本发明还提供了鉴定由于其阻断或抑制了RyR2活化而适用于预防运动引起的心源性猝死的物质的其它测定法。

也可测定与心力衰竭程度相关的RyR2的PKA磷酸化水平,然后将其用于确定设计用于阻断RyR2通道PKA磷酸化的化合物的效能。该测定可基于对RyR2蛋白特异的抗体的使用。例如,RyR2-通道蛋白可被免疫沉淀,然后用PKA和[γ32P]-ATP反向磷酸化(back-phosphorylated)。然后使用磷光计测定转移到RyR2蛋白上的放射性[32P]标记的量(Marx等人,PKA phosphorylation dissociatesFKBP 12.6 from the calcium release channel(ryanodine receptor):defective regulation in failing hearts.Cell,101:365-76,2000)。

化学合成的新途径
在生物活性分子包括JTV-519的制备中,1,4-苯并噻氮衍生物是重要的构建块。发明人已经开发了制备1,4-苯并噻氮中间体化合物,例如7-甲氧基-2,3,4,5-四氢-1,4-苯并噻氮的新方法。发明人的方法利用了很容易获得且便宜的原料,提供了关键1,4-苯并噻氮中间体的高产率。

制备7-甲氧基-2,3,4,5-四氢-14-苯并噻氮和类似化合物的两种方法先前已经在文献中报道过了。由Kaneko等人(美国专利第5,416,066号)公开的第一种方法包括由2,5-二羟苯甲酸开始的六步合成路径。在该方法中,2,5-二羟苯甲酸选择性地与硫酸二甲酯甲基化。然后所得化合物与二甲基硫代氨基甲酰氯反应20h,然后经受高温(270℃)9h。该步的产物与甲醇钠在甲醇中回流20h。然后将回流步骤的产物与2-氯乙胺在碱性条件和高温下反应,生成环化酰胺。用LiAlH4还原环化酰胺生成7-甲氧基-2,3,4,5-四氢-1,4-苯并噻氮(1,4-苯并噻氮中间体)。
制备7-甲氧基-2,3,4,5-四氢-1,4-苯并噻氮的第二种方法由Hitoshi公开在日本专利(JP 10045706)中。该方法由2-溴-5-甲氧基苯甲醛开始。溴化物被NaSMe取代,所得产物用氯氧化,接着在水中回流,生成二硫化物二醛。用2-氯乙胺处理该二醛,所得产物用还原剂例如NaBH4还原。将所得化合物环化生成7-甲氧基-2,3,4,5-四氢-1,4-苯并噻氮。
刚开始,发明人试图用上述方法制备1,4-苯并噻氮中间体,7-甲氧基-2,3,4,5-四氢-1,4-苯并噻氮。然而,他们发现由Kaneko等人(美国专利第5,416,066号)描述的第一种方法包括了高温且反应时间长的合成步骤。另外,发明人发现第三个硫醇化中间体中的硫代基很容易被空气氧化成二硫化物,使得它不可能合成后来的环化产物。发明人还确定Hitoshi(JP 10045706)描述的方法使用了Cl2,因此,不得不使用制备第一个中间体的另一种专利方法,它不需要用NaSMe取代溴化物。
为了克服上述问题,发明人开发了从容易获得且便宜的原料制备7-甲氧基-2,3,4,5-四氢-1,4-苯并噻氮的新方法。发明人的方法简化了分离和纯化步骤,可以用于制备不同的1,4-苯并噻氮中间体,包括7-甲氧基-2,3,4,5-四氢-1,4-苯并噻氮和具有下式所示通式结构的其它化合物:

该方法也可用于制备JTV-519。

根据本发明方法,步骤(a)中的还原剂可为H2。另外,步骤(b)中的重氮化试剂可为NaNO2,步骤(b)中的二硫化物可为Na2S2。此外,步骤(c)中的氯化物可为SOCl2。步骤(d)中的还原剂可为三甲基膦(PMe3),而步骤(d)中的碱是三乙胺。在另一个实施方案中,步骤(e)中的还原剂是LiAIH4。

举例来说,并如实施例7和如下方案1中所示,可从2-硝基-5-甲氧基苯甲酸如下制备7-甲氧基-2,3,4,5-四氢-1,4-苯并噻氮。使用H2连同Pd/C作为催化剂,还原2-硝基-5-甲氧基苯甲酸的硝基,生成2-氨基-5-甲氧基苯甲酸。可用NaNO2使2-氨基-5-甲氧基苯甲酸重氮化,然后用Na2S2处理,生成稳定的二硫化物。不经进一步纯化,可用SOCl2处理稳定的二硫化物,然后与2-氯乙胺在Et3N的存在下反应,生成酰胺。然后,如下酰胺化合物可经由单-罐操作转化为环化化合物。还原试剂(例如三甲基膦或三苯基膦)和碱(例如三乙胺)可加到酰胺化合物的THF(四氢叶酸)溶液中。然后所得的反应混合物回流3h。还原剂(三甲基膦或三苯基膦)将二硫化物(S-S)裂解为它的一硫化物(-S),其就地与氯化物进行分子内环化,生成环化酰胺。然后可用LiAlH4还原环化酰胺生成1,4-苯并噻氮中间体,7-甲氧基-2,3,4,5-四氢-1,4-苯并噻氮。通过将7-甲氧基-2,3,4,5-四氢-1,4-苯并噻氮与3-溴丙酰氯反应,然后将所得化合物与4-苄基哌啶反应,可从7-甲氧基-2,3,4,5-四氢-1,4-苯并噻氮制备JTV-519。
举例来说,如实施例8和如下方案2中所示,可如下制备放射标记的JTV-519。可使用BBr3在苯环处使JTV-519脱甲基化。然后在碱(例如NaH)的存在下,用放射标记的甲基化剂(例如3H-硫酸二甲酯)使所得酚化合物再次甲基化,可生成3H-标记的JTV-519。
本发明还提供了包含放射标记的JTV-519的组合物。可使用本领域中已知的各种不同放射性标记物中的一种,完成JTV-519的标记。本发明的放射性标记物可为,例如,放射性同位素。放射性同位素可为放射可检测辐射的任意同位素,包括但并不限于35S、32P、125I、3H或14C。放射性同位素放射的放射性可以通过本领域中熟知的技术检测。例如,来自放射性同位素的γ射线可使用γ显像技术,尤其是闪烁照相技术检测。

实施例
实施例1——FKBP 12.6-缺陷小鼠
如之前所述,生产FKBP 12.6-缺陷小鼠(Wehrens等人,FKBP12.6 deficiency and defective calcium release channel(ryanodinereceptor)function linked to exercise-induced sudden cardiac death.Cell,113:829-40,2003)。简单地说,使用全长鼠cDNA探针,从DBA/1lacJ文库中分离对于人FK506结合蛋白12.6(FKBP 12.6)的鼠直源物(orthologue)的小鼠基因组λ-噬菌体克隆。通过用PGK-新可选标记替代3.5kb的鼠基因组DNA,设计靶向载体,以删除外显子3和4,它包含鼠FKBP 12.6的全部编码序列(Bennett等人,Identification and characterization of the murine FK506 bindingprotein(FKBP)12.6 gene.Mamm.Genome,9:1069-71,1998)。将5.0-kb的5′片段和1.9-kb的3′片段克隆到pJNS2——具有PGK-新和PGK-TK盒的主链载体中。使用已建立的方案生长和转染DBA/lacJ胚胎干(ES)细胞。首先用Southern分析筛选靶ES细胞,用PCR分析5个阳性ES细胞系,以证实同源重组。将雄性嵌合体培育到DBA/1lacJ雌性中,并通过褐色包被颜色鉴定种系子代。使用5′Southern分析对种系子代进行基因型分析。杂交阳性FKBP 12.6+/-雄性和雌性,子代以大约25%的频率形成FKBP 12.6-/-小鼠。FKBP 12.6-/-小鼠是多产的。
用FKBP 12.6-/-小鼠进行的所有研究都使用年龄和性别相配的FKBP 12.6+/+小鼠作为对照。在以下背景下饲养的FKBP 12.6-/-小鼠之间没有观察到区别:DBA/C57BL6混合的、纯DBA和纯C57BL6。
实施例2——小鼠的遥测记录和运动试验
根据哥伦比亚大学公共机构动物管理和使用委员会(InstitutionalAnimal Care and Use Committee of Columbia University)批准的实验设计,饲养和研究FKBP 12.6+/+和FKBP 12.6-/-小鼠。使用2.5%异氟烷吸入麻醉使小鼠麻醉。在腹膜内植入(Data Sciences International,St.Paul,MN)>7日后获得能走动动物的ECG无线电遥测记录(Wehrens等人,FKBP 12.6 deficiency and defective calcium release channel(ryanodinereceptor)function linked to exercise-induced suddencardiac death.Cell,113:829-40,2003)。为了进行应激试验,使小鼠在倾斜跑台上运动,直至筋疲力尽,然后向腹膜内注射肾上腺素(0.5-2.0mg/kg)(Wehrens等人,FKBP 12.6 deficiency and defective calciumrelease channel(ryanodine receptor)function linked toexercise-induced sudden cardiac death.Cell,113:829-40,2003)。经过4h,平均能走动动物的静息心率。
实施例3——野生型和RyR2-S2809D突变型的表达
如之前所述,进行RyR2上的PKA靶位(RyR2-S2809D)的诱变(Wehrens等人,FKBP 12.6 deficiency and defective calcium releasechannel(ryanodinereceptor)function linked to exercise-inducedsudden cardiac death.Cell,113:829-40,2003)。使用Ca2+磷酸盐沉淀,用20μg RyR2野生型(WT)或突变型cDNA,并用5μg FKBP 12.6cDNA共转染HEK293细胞。如之前所述制备包含RyR2通道的小囊泡(Wehrens等人,FKBP 12.6 deficiency and defective calcium releasechannel(ryanodine receptor)function linked to exercise-inducedsudden cardiac death.Cell,113:829-40,2003)。
实施例4——RyR2 PKA磷酸化和FKBP 12.6结合
如之前所述,制备心脏SR膜(Marx等人,PKA phosphorylationdissociates FKBP 12.6 from the calcium release channel(ryanodinereceptor):defective regulation in failing hearts.Cell,101:365-76,2000;Kaftan等人,Effects of rapamycin on ryanodinereceptor/Ca(2+)-release channels from cardiac muscle.Circ.Res.,78:990-97,1996)。使用来自Promega(Madison,WI)的TNTTM快速配对转录/转译系统产生35S-标记的FKBP12.6。使用[3H]里安娜碱结合来定量RyR2水平。将100μg微粒体稀释在100μl 10-mM的咪唑缓冲液(pH6.8)中,在37℃下用250-nM(终浓度)[35S]-FKBP 12.6培养60分钟,然后500μl冰冷的咪唑缓冲液猝灭。样品在100,000g下离心10分钟,在咪唑缓冲液中洗涤3次。用沉淀粒的液体闪烁计数来测定结合[35S]-FKBP 12.6的量。
实施例5——免疫印迹
如上所述,在室温下进行微粒体(50μg)的免疫印迹法1h,用抗-FKBP12/12.6(1:1,000)、抗-RyR(5029;1:3,000)(Jayaraman等人,FK506 binding protein associated with the calcium release channel(ryanodine receptor).J.Biol.Chem.,267:9474-77,1992),或抗-磷RyR2(P2809;1:5,000)(Reiken等人,Beta-blockers restore calciumrelease channel function and improve cardiac muscle performance inhuman heart failure.Circulation,107:2459-66,2003)。P2809-磷表位特异性抗-RyR2抗体是亲和力-纯的多克隆兔抗体,是使用肽CRTRRI-(pS)-QTSQ由Zymed实验室(旧金山,CA)定做的,它响应Ser2809处的RyR2 PKA-磷酸化。用HRP-标记的抗-兔IgG(1:5,000稀释度;Transduction Laboratories,Lexington,KY)培养以后,使用ECL(Amersham Pharmacia,Piscataway,NJ)使印迹显像。
实施例6-单通道记录
如之前所述,在0mV的电压钳条件下,获得来自小鼠心脏的天然RyR2或重组RyR2的单通道记录(Marx等人,PKA phosphorylationdissociates FKBP 12.6 from the calcium release channel(ryanodinereceptor):defective regulation in failing hearts.Cell,101:365-76,2000)。用于通道记录的对称溶液是:反隔室-HEPES,250mmol/L;Ba(OH)2,53mmol/L(在某些实验中,用Ca(OH)2代替Ba(OH)2);pH7.35;以及顺隔室-HEPES,250mmol/L;Tris-碱,125mmol/L;EGTA,1.0mmol/L;以及CaCl2,0.5mmol/L;pH7.35。除非另外说明,在顺隔室中存在150-nM[Ca2+]和1.0-mM[Mg2+]下进行单通道记录。将里安娜碱(5mM)应用于顺隔室以证实所有通道的同一性。使用Fetchan软件(Axon Instruments,Union City,CA),从数字化电流记录中分析数据。所有数据都表示成平均数±SE。使用非配对Student′s t检验进行实验之间平均值的统计对比。认为p<0.05的值是统计学显著的。
JTV-519对RyR2通道的影响列在图1-3和表1(如下)中。如图3中所证明,单通道研究显示,PKA磷酸化(D)以后,RyR2的开放概率增加了,与在特异PKA抑制剂PKI5-24(C)存在下的PKA磷酸化相比。在JTV-519(E)的存在下,在用FKBP12.6培养的PKA-磷酸化RyR2中,单通道功能被正常化了。幅度组织图(右边)显示在PKA-磷酸化RyR2中增加的活性和亚传导孔,但用JTV-519和FKBP 12.6处理以后就没有了。图3F显示在JTV-519的存在下,用FKBP 12.6培养PKA-磷酸化RyR2将RyR2激活的Ca2+-依赖性向右位移,使它类似于非磷酸化通道的Ca2+-依赖性。

用JTV-519处理的(n=8)或对照(n=6)FKBP 12.6+/-小鼠,和用JTV-519处理的(n=5)FKBP 12.6-/-小鼠的动态ECG数据总结。SCL=窦性周期长度;HR=心率;ms=毫秒;bpm=每分钟心搏次数;FKBP 12.6+/-=FKBP 12.6杂合小鼠;FKBP 12.6-/-=FKBP 12.6缺陷小鼠。
实施例7——1,4-苯并噻氮
中间体和JTV-519的合成
对于体内实验来说,发明人需要克数量的JTV-519。然而,经由报道的1,4-苯并噻氮中间体,7-甲氧基-2,3,4,5-四氢-1,4-苯并噻氮(以下方案1中的化合物6)制备该化合物的最初尝试没有成功。该中间体的硫代基团很容易被空气氧化成二硫化物,这使得环化产物(5)的合成不可能。为了克服这一问题,发明人开发了从容易获得且便宜的2-硝基-5-甲氧基苯甲酸(1)开始的新方法。该方法描述于以下方案1中。
使用H2连同Pd/C作为催化剂,还原化合物(1)的硝基,以定量产率生成2-氨基-5-甲氧基苯甲酸(2)。用NaNO2使化合物(2)重氮化,然后用Na2S2处理,得到80%产率的稳定的二硫化物(3)。不经进一步纯化,用SOCl2处理稳定的二硫化物(3),然后与2-氯乙胺在Et3N的存在下反应,生成90%产率的酰胺(4)。用三甲基膦和Et3N的THF溶液回流,化合物(4)经由单-罐操作转化为环化化合物(5)。然后用LiAlH4还原环化酰胺(5)生成7-甲氧基-2,3,4,5-四氢-1,4-苯并噻氮(6)。
方案1
将化合物(6)与3-溴丙酰氯反应,然后使4-苄基哌啶与所得产物反应,制备JTV-519。用1H NMR确立JTV-519的结构。
实施例8——放射标记的JTV-519的合成
发明人合成放射标记的JTV-519的新方法描述在以下方案2中。为了制备放射标记的JTV-519,使用BBr3使JTV-519在苯环上脱甲基化,生成酚化合物(21)。在碱(NaH)的存在下,用放射标记的甲基化剂(3H-硫酸二甲酯)使酚化合物(21)再次甲基化,生成3H-标记的JTV-519(方案2)。
虽然出于清楚和理解的目的,已经相当详细地描述了上述发明,然而本领域技术人员通过阅读公开内容将可以理解,在不背离本发明在所附权利要求书中的准确范围的情况下,可以作出形式和细节的各种改变。
