Abstract
Description
この外反母趾の治療では、変形した外反母趾角の矯正が重要で、保存療法と手術療法が行われている。大きな変形に対しては、外科手術による矯正が行われるが、どの変形に対しても対応可能な手術方法はなく、変形が高度化する前に治療することが肝要である。
また、外科手術以外の治療としては、矯正を目的とした治具を用いる装具療法がある。この装具療法としては、歩行時に用いられる足底挿板(インソール)や矯正装具がある。
例えば、特許文献2ないし4では、サポーターによる矯正装具が開示されている。また、特許文献5及び6では、管状体と板片または弾性パッドからなる矯正器具が開示されている。また、特許文献7では、形状記憶合金を用いた矯正器具で、Ti-Ni形状記憶合金線からなるメッシュによりアーチ形態を維持することによるものが開示されている。
例えば、図12は、外反母趾の矯正用具の一例を示す図である。図13は、外反母趾の矯正用具の他の一例を示す図である。図12及び図13に示す外反母趾の矯正用具は、夜間就寝用の矯正用具であって歩行用を考慮していない。特に、図12に示す外反母趾の矯正用具では、レザーベルトの固定位置を加減することにより母趾を徐々に適正位置に保持していけるが、熱可塑性のプラスチック板で作られているので、起立・歩行はできないという不具合がある。この他に、昼間歩行用の外反母趾の矯正用具もあるが、母趾を覆う指袋からつながったベルトを加減することにより牽引力を調節でき、使用者自身で調整しなければならないという不具合がある。
さらに、例えば、外反母趾診療ガイドライン(日本整形外科学会診療ガイドライン委員会外反母趾ガイドライン策定委員会)でも「装着中軽度の変形矯正効果を期待出来るが、非装着時に変形が改善されるかは結論出来ない。」としており、現在でも矯正を目的とした有効な装具療法がないのが現状である。
本発明の外反母趾矯正装具は、外反母趾を矯正する外反母趾矯正装具において、形状記憶合金で形成されている矯正板と、これを取り付ける布状体で形成されている取付具とを有し、前記矯正板は、母趾内側方向から足の本体部分に第1取付具と母趾の本体部分に第2取付具で取り付けられていることを特徴とする。
また、本発明の外反母趾矯正装具は、さらに、前記矯正板は、母趾内側方向から足の本体部分と母趾の本体部分とを取付具で取り付けられることを特徴とする。
また、本発明の外反母趾矯正装具は、さらに、前記矯正板は、母趾内側方向から足の本体部分に第1取付具と母趾の本体部分に第2取付具で取り付けられることを特徴とする。
また、本発明の外反母趾矯正装具は、さらに、矯正板の第1中足趾節関節部に接当する部分に開口部が形成されていて、この開口部周囲を支点として母趾を内側に矯正することを特徴とする。

また、本発明の外反母趾矯正装具の製造方法は、さらに、前記Cu-Al-Mn系形状記憶合金は、α+βの2相組織で冷間圧延して板材を形成し、所定の形状にした後に、700~950℃の温度範囲、0.01~1時間の時間範囲における溶体化処理と50~250℃の温度範囲、0.1~1時間の時間範囲の時効処理されていることを特徴とする。
本発明の外反母趾矯正装具によって、形状記憶合金を用いて、簡単に装着できて、大きな矯正力で外反母趾、内旋母趾を矯正することができる。
また、本発明の外反母趾矯正装具の製造方法によって、簡単に装着できて、大きな矯正力をもたらす形状記憶合金を用いる外反母趾矯正装具を容易に製造することができる。
本発明の外反母趾矯正装具1は、図1に示すように、矯正板11と取付具12とを有している。
本発明の外反母趾矯正装具1の矯正板11は、形状記憶合金で形成されている。形状記憶合金は、所定の温度(As温度)以下で変形しても、所定の温度(Af温度)以上に加熱すると、元の形状に回復する性質である形状記憶効果と、所定の温度(Af温度)以上で変形しても、除荷するともとの形状に回復する性質である超弾性効果の一方、または両方を有する合金をいう。形状記憶効果を利用すると加熱することで外反母趾21に矯正力を与えることができる。特に、超弾性変形を用いることで、一定の荷重または小さな荷重変化で変形・回復をすることができるため、超弾性効果を利用することが好ましい。したがって、Af温度が室温以下に設定された矯正板11を人の足2に装着して外反母趾21を矯正する際に、元の形状に回復しようとすることで力が作用して、外反母趾21を矯正する。
形状記憶合金としては、Cu-Al-Mn系、Cu-Zn-A1系、Ti-Mo-Sn系,Ni-Ti系等があり、いずれの系の合金を用いても良い。
また、図1に示すように、取付具12は、母趾(親指)21の内側方向から足2の本体部分に矯正板11を取り付ける第1取付具121と、母趾21の本体部分に矯正板11を取り付ける第2取付具122とを設けて、別体型で用いている。また、ここでは、取付具12として前後に固定するために2体に分けた別体型で説明しているが、母趾21の内側方向から足2の本体部分と母趾21の本体部分とを一体になった取付具で取り付けてもよい。一体型の形態としては、サポーター式や靴下式などがある。本発明の外反母趾矯正装具1としては、足2の本体部分と母趾21の部分を固定できるようになっていれば足りる。取付具12と矯正板11とは、熱又は接着剤等で一体化させている。または、取付具12に空洞のようなポケット123を設けておき、そこに矯正板11を挿入することで一体化しても良い。これによって、装着した後もずれないままで矯正板11を足2に装着させておくことができる。また、伸縮性を持たせることで足2に密着させて装着することができる。布状体にすることで、装着しやすくすることができる。装着は、面ファスナーを設けて留めることで達成する。また、包体止めで引っかけて留めても良いし、布状体自身で結んでも良い。
外反母趾21の場合、履いている靴等による圧迫で、矢印Aで示すように、母趾21が内側に押されてしまい、それにつられてその他の足2の指が同様の圧力を受ける。そして、足2の第1中足趾節関節部(関節部)26が、逆に、足2の外側に押し出されるように変形している。従って、本発明の外反母趾矯正装具1は、この変形を元に戻すようにする。
図3は、本発明の外反母趾矯正装具の一つの実施形態を示す図で、(a)は矯正板、(b)はその矯正板を装着した足の斜視図であり、(c)は外反母趾矯正装具が母趾に作用する力を模式的に示す図である。
(a)に示すような矯正板11を用いる外反母趾矯正装具1を足2に装着すると、(b)示すように装着される。矯正板11は足の形に沿うように、く字形にすることで安定して装着することができる。また、矯正板11と第1中足趾節関節部26の間にスポンジ14等の弾性部材を配置してもよい。また、(c)に示すように、スポンジ14等の弾性部材は筒状にして、矯正板11をこのスポンジ14の中を通すようにしてもよい。これによって、矯正板11を外反母趾21になっている第一中足趾節関節部26に接当しても、足2を傷つけたり、痛み出したりすることがない。このときに、足2に装着すると、形状記憶合金を用いている外反母趾矯正装具1の超弾性効果によって、図中の矢印Pで示すように、母趾21を足2の外側に引っ張る方向に力が作用する。これによって、靴等による圧迫と逆方向に母趾21を引っ張ることで、母趾21の変形を予防し、又、変形を無くし元の形態に戻すことができる。
さらに、図示しないが、実際に靴を履いたときの高さを調整するように、足底の取付具12に弾性部材を配置する。これで、足2の甲を上げることにより矯正中の足2に対する変形が促進されるのを防止することができる。
矯正板11は、(a)に示すように、第1中足趾節関節部26に接当する部分に開口部111が形成されている。この開口部111を外反母趾21で足2から外に飛び出している部分に接当させることで、矯正板11を足2に密着させて、取付具12で装着させることができる。さらに、この開口部111には、スポンジ等の発泡体、ゴムのような弾性体の軟質材からなるパッドを設けている。これで、矯正板11を外反母趾21になっている第1中足趾節関節部26に接当しても、足2を傷つけたり、痛み出したりすることがない。
この矯正板11を有する外反母趾矯正装具1を、(b)に示すように、この開口部111周囲を支点として、第2取付具122で母趾(親指)21に矯正板11を装着させる。したがって、(c)に示すように、矢印Pの方向に力が作用し、足2から離れる方向の内側に引き出させる。これによって、外反母趾21になっている足2の矯正、または、押されている母趾21を外反母趾21にならない方向に予防し、又、変形を無くし元の形態に戻すことができる。
また、図5は、非常に悪化した外反母趾21に対する本発明の外反母趾矯正装具を装着した状態を示している図である。
(a)に示しているように、非常に悪化した外反母趾21では、第1中足趾節関節部26が足2から大きく外側に飛び出している。このような状態では、平板の矯正板11では装着することが難しいし、また、装着しても矯正板11が第一中足趾節関節部26に接当して傷つけたり痛みを与えたり、矯正板11が変位して矯正効果を減少させたりすることがある。このような外反母趾21には、開口部111を有する矯正板11を用いる外反母趾矯正装具1を用いることが好ましい。さらに、矯正板11と第1中足趾節関節部26の間にスポンジ14等の弾性部材を配置することが好ましい。これによって、(c)に示すように、矢印Pの方向に力が作用し、足2から離れる方向の内側に引き出させる。これによって、外反母趾21になっている足2の矯正、または、押されている母趾21を外反母趾21にならない方向に予防し、又、変形を無くし元の形態に戻すことができる。
矯正板11は、(a)に示すように、第1中足趾節関節部26に接当する部分に開口部111が形成されていて、かつ、一方は線状で、対向する側は楕円形状をしている。これで、(b-1)に示すように、別体型の外反母趾矯正装具1では、足2の形状に合わせて土踏まずがあって形状に起伏がある足底ではなく、足2の甲に近い部分に第2の取付具121の取付具12を用いて装着させている。
この時に、(b-2)に示すように、取付具12は、足2の本体部分と母趾部分とを同時に装着する一体型であっても良い。これによって、外反母趾矯正装具1の足2への装着が容易であり、または、外反母趾矯正装具1が足2の動きによって変位するのを防止することができる。このような一体型の外反母趾矯正装具1では、運動用具のようなサポーター式になって、歩くことや走ること等の運動するに際しても、更に、母趾21に変形の圧力がかかるのを予防し、または、変形を無くし元の形態に戻すことができる。これによって、(b-1)及び(b-2)に示すように、矢印Pの方向に力が作用し、足2から離れる方向の内側に引き出させる。これによって、外反母趾21になっている足2の矯正、または、押されている母趾21を外反母趾21にならない方向に予防し、又、変形を無くし元の形態に戻すことができる。
また、この取付具12では、(c)に示すように、矯正板11と一体にするのではなく、矯正板11を装着させるポケット123を設ける。このポケット123に、矯正板11を装着することで、靴等による圧迫と逆方向に母趾21を引っ張り、母趾21の変形を予防し、または、変形を無くし元の形態に戻すことができる。
また、ポケット123に装着する矯正板11の枚数、種類を取り替えることで、逆方向に母趾21を引っ張る強度を調整することができる。
上述したように、外反母趾21の場合、履いている靴等による圧迫で、矢印Aで示すように、母趾21が内側に押されてしまい、それにつられてその他の足2の指が同様の圧力を受ける。さらに、外反母趾21の場合、(a)に示すように、矢印Bで示すように、母趾21を内旋させるような圧力を受けて変形していることが多い。この内旋は、(b)に示すように、多くの場合は、足2の上の方から下の足底に向かって内旋している。これも、足2の指、骨に大きな変形を生じさせることから、内旋矯正も必要である。
図8は、本発明の外反母趾矯正装具の一つの実施形態を示す図で、(a)は矯正板、(b)はその矯正板を装着した足の斜視図であり、(c)は外反母趾矯正装具が母趾に作用する力を模式的に示す図である。
(a)に示すように、本発明の外反母趾矯正装具1は、矯正板11の他に、固定板13を有する。
また、このときに固定板13が及ぼす状態を、(b)はその固定板13を装着した足2の斜視図であり、(c)はその固定板13を装着した足2の正面図で示している。
矯正板11により、(c)に示すように、矢印P1の方向に力が作用し、足2から離れる方向の内側に引き出させて、外反母趾21になっている足2の矯正することができる。さらに、母趾21の爪の部分又は母趾21の全体に平板の固定板13を装着して、矢印P2で示す方向の力が作用し、母趾21が内旋で受けているのと逆方向に回転するような圧力を加えて、内旋矯正を行っている。
固定板13は、プラスチック、樹脂、金属等の硬質材であればいずれでもよい。固定板13を装着する代わりに、取付具12の一部に硬い布などを取り付けて硬くすることで代用することもできる。
ここでは、固定板13は、取付具12で取り付けているが、矯正板11と一体化して、かつ、超弾性状態の形状記憶合金にしても良い。超弾性状態の形状記憶合金にすることで、取付具12からずれても、押さえられていれば母趾21に対して回転する力を及ぼすことができる。さらに、超弾性の形状復元力を矯正力とすることができる。
固定板13は、マルテンサイト相状態の形状記憶合金にしても良い。マルテンサイト状の形状記憶合金は柔軟であるため、足の形に応じた適切な形に形状付与することができる。
本発明の外反母趾矯正装具1は、取付具12として弾性体のゴムを用いる。このゴムによって、簡単に、且つ、確実に足2に装着することができる。さらに、ゴムの一部に切り込み124を設けて、矯正板11の端部を挿入することで、矯正板11の取付も容易に行うことができる。
また、ゴムは、一体型の取付具12でも良いが、第1取付具121、第2取付具122の双方又はいずれか一方であっても良い。特に、第2取付具122に用いることで、母趾21の先端から挿入することで、容易に装着することができる。
図10は、本発明の外反母趾矯正装具の一つの実施形態を示す図である。
本発明の外反母趾矯正装具1は、取付具12として、第1の取付具121,第2の取付具122のゴム等の弾性体に面ファスナー125を追加して用いている。この面ファスナー125によって、簡単に、且つ、確実に足2に装着することができる。
また、面ファスナー125は、布状体が一体型の取付具12に追加しても良いし、布状体が別体型の第1取付具121、第2取付具122の双方又はいずれか一方に追加しても良い。特に、第2取付具122に用いることで、母趾21の先端の細い部分でも、容易に且つ短時間で装着することができる。これで、矢印Pの方向に力が作用し、足2から離れる方向の内側に引き出させて、外反母趾21になっている足2の矯正することができる。
通常の金属であれば、曲げ変形量に応じて、最初は弾性変形し、その後塑性変形領域に入って、矢印M1に示すように、塑性変形する。そして、破壊する前に荷重を付加するのをやめると、矢印M2のように、荷重が低下して行くが、変形量は元には戻らず、塑性変形分が残ってしまう。
それに対して、形状記憶合金の超弾性効果では、曲げ変形量に応じて、最初は弾性変形し、その後も塑性変形領域に入ることなく、矢印M3に示すように、約一定の荷重のままで変形量が大きくなって行く。そして、破壊する前に荷重を付加するのをやめると、矢印M4のように、一定の荷重で変形量が小さくなって、変形量は元の状態にもどり、塑性変形分が残らない。この超弾性現象が、形状復元力としての荷重として作用する。さらに、この荷重を、外反母趾21、内旋母趾21の矯正力に利用することができる。図10の超弾性挙動は単結晶に多く発現し、多結晶材料では変形に対する多少の荷重上昇が生じることがあるが、通常の金属に比べると荷重上昇は非常に小さく、矯正力が外反母趾21の変形度合いにあまり依存しないため、適切な矯正効果を発揮することができる。したがって、本発明の外反母趾矯正装具1の矯正板11、固定板13に形状記憶合金を用いることで、外反母趾21、内旋母趾21の矯正のために超弾性の矯正力を作用させて、逆方向に母趾21を引っ張りかつ回転させることで、母趾21の変形を予防し、または、変形を無くし元の形態に戻すことができる。
形状記憶効果及び超弾性効果を有するCu-Al-Mn系合金は、高温でβ相(b.c.c.)単相になり、低温でβ+α(f.c.c.)の二相組織になる。β単相の生成には3%以上のAlが必要であるが、10%を超える過剰量のAlはCu-Al-Mn系合金を脆化させやすい。好ましくは、6~10%の範囲にAl含有量を設定する。
Mn添加は、低Al側でのβ相の存在を可能とし、Cu-Al-Mn系合金の冷間加工性を向上させる。このような効果は5%以上のMnで顕著になるが、20%を超える過剰添加はマルテンサイト変態温度を低下させて形状記憶効果又は超弾性効果に悪影響を及ぼす。好ましくは、8~12%の範囲でMn含有量を設定する。
他の添加成分として、Ni、Co、Fe、Ti、V、Cr、Si、Nb、Mo、W、Sn、Sb、Mg、P、Be、Zr、Zn、B、C、Ag、ミッシュメタルの一種又は二種以上を合計で0.001~10%添加することもできる。
Ni、Co、Fe、Sn、Sb、Beはマトリックスを強化する作用を呈し、TiはN、Oを固定して無害化する。W、V、Nb、Mo、Zrは、硬さ、耐磨耗性の改善に有効な成分である。Crは耐磨耗性、耐食性を改善し、Siは耐食性を改善し、Mgは熱間加工性、靭性を向上させ、P、ミッシュメタルは脱酸剤として添加され靭性向上にも寄与する。また、Znは形状記憶温度を上昇させ、B、Cは粒界を強化して加工性、靭性を改善し、Agは冷間加工性の向上に寄与する。したがって、500~700℃の温度範囲における中間焼鈍を行いながら、α+βの2相組織で冷間圧延により板材を形成し、所定の形状にした後に、700~950℃、0.01~1時間の時間範囲における溶体化処理、さらに、50~250℃、0.1~1時間の時間範囲における時効処理がされている。
形状記憶合金には加熱により形状が元に戻る形状記憶効果と、応力の除荷により形状が元に戻る超弾性があり、Cu-Al-Mn系合金は室温で超弾性を示すことが望ましい。
通常の金属では変形が大きくなると応力が高くなることが一般的であるが、超弾性では変形による応力の上昇が少ないため、変形力が外反母趾21の変形の程度にあまり依存せず、無理なく矯正力を加えることができる。
Cu-Al-Mn系合金を矯正板11の素材に使用すると、環境変化に対応した矯正力の変化が小さく、安定した矯正力が長期にわたり加えられる。
Al:8.1質量%、Mn:11.2質量%、残部Cuの組成の合金を高周波誘導溶解により作製し、鋳型に鋳造、600℃で熱間圧延、600℃で中間焼鈍をしながら冷間圧延により板材を作製する。得られた板を600℃10分間の焼鈍を行い、所定形状に打ち抜く。さらに、900℃5分間の溶体化処理、水冷、200℃15分間の時効処理を行うことにより、超弾性特性が発現する。

- 外反母趾を矯正する外反母趾矯正装具において、
前記外反母趾矯正装具は、
形状記憶合金で形成されている矯正板と、
これを取り付ける布状体で形成されている取付具とを有する
ことを特徴とする外反母趾矯正装具。 - 請求項1に記載の外反母趾矯正装具において、
前記矯正板は、
母趾内側方向から足の本体部分と母趾の本体部分とを取付具で取り付けられる
ことを特徴とする外反母趾矯正装具。 - 請求項1又は2に記載の外反母趾矯正装具において、
前記矯正板は、
母趾内側方向から足の本体部分に第1取付具と母趾の本体部分に第2取付具で取り付けられる
ことを特徴とする外反母趾矯正装具。 - 請求項1ないし3に記載の外反母趾矯正装具において、
前記矯正板は、
矯正板の第1中足趾節関節部に接当する部分に開口部が形成されていて、この開口部周囲を支点として母趾を内側に矯正する
ことを特徴とする外反母趾矯正装具。 - 請求項1ないし4のいずれかに記載の外反母趾矯正装具において、
前記矯正板は、開口部に軟質材からなるパッドが置かれている
ことを特徴とする外反母趾矯正装具。 - 請求項1ないし5のいずれかに記載の外反母趾矯正装具において、
前記取付具は、伸縮性のある弾性体で形成されている
こと特徴とする外反母趾矯正装具。 - 請求項1から6のいずれかに記載の外反母趾矯正装具において、
前記外反母趾矯正装具は、母趾の爪のある部分に平坦な固定板を有する
ことを特徴とする外反母趾矯正装具。 - 請求項1から7のいずれかに記載の外反母趾矯正装具において、
前記外反母趾矯正装具は、矯正板と固定板とが一体になっている
ことを特徴とする外反母趾矯正装具。 - 請求項1ないし8のいずれかに記載の外反母趾矯正装具において、
前記外反母趾矯正装具に用いる矯正板が、Cu-Al-Mn系形状記憶合金である
ことを特徴とする外反母趾矯正装具。 - 請求項7ないし9のいずれかに記載の外反母趾矯正装具において、
前記外反母趾矯正装具に用いる固定板が、Cu-Al-Mn系形状記憶合金である
ことを特徴とする外反母趾矯正装具。 - 請求項9又は10に記載の外反母趾矯正装具において、
前記Cu-Al-Mn系形状記憶合金は、質量%でAl:3~10%、Mn:5~20%、残部:銅及び不可避的不純物を含み、かつ、Ni、Co、Fe、Ti、V、Cr、Si、Nb、Mo、W、Sn、Sb、Mg、P、Be、Zr、Zn、B、C、Ag、ミッシュメタルから選ばれた一種又は二種以上を合計で質量%で0.001~10%添加した合金で形成されている
ことを特徴とする外反母趾矯正装具。 - 形状記憶合金で形成されている矯正板と、これを取り付ける布状体で形成されている取付具とを有する外反母趾矯正装具を製造する外反母趾矯正装具の製造方法において、
前記外反母趾矯正装具の製造方法は、
前記矯正板が、Cu-Al-Mn系形状記憶合金であって、質量%でAl:3~10%、Mn:5~20%、残部:銅及び不可避的不純物を含み、かつ、Ni、Co、Fe、Ti、V、Cr、Si、Nb、Mo、W、Sn、Sb、Mg、P、Be、Zr、Zn、B、C、Ag、ミッシュメタルから選ばれた一種又は二種以上を合計で質量%で0.001~10%添加した合金で形成されている
ことを特徴とする外反母趾矯正装具の製造方法。 - 請求項12に記載の外反母趾矯正装具の製造方法において、
前記外反母趾矯正装具は、母趾の爪のある部分に平坦な固定板を有し、
該固定板が、Cu-Al-Mn系形状記憶合金であって、質量%でAl:3~10%、Mn:5~20%、残部:銅及び不可避的不純物を含み、かつ、Ni、Co、Fe、Ti、V、Cr、Si、Nb、Mo、W、Sn、Sb、Mg、P、Be、Zr、Zn、B、C、Ag、ミッシュメタルから選ばれた一種又は二種以上を合計で質量%で0.001~10%添加した合金で形成されている
ことを特徴とする外反母趾矯正装具の製造方法。 - 請求項12又は13に記載の外反母趾矯正装具の製造方法において、
前記Cu-Al-Mn系形状記憶合金は、α+βの2相組織で冷間圧延して板材を形成し、所定の形状にした後に、700~950℃の温度範囲、0.01~1時間の時間範囲における溶体化処理と50~250℃の温度範囲、0.1~1時間の時間範囲の時効処理されている
ことを特徴とする外反母趾矯正装具の製造方法。
