本發明廣泛涉及眼科手術。更特別的是,但非為限制,本發明涉及眼用穿刺套管及排出器。
可由外科醫生使用顯微手術工具從在人體中需小心處理及有限的空間移除組織,例如,在眼睛上手術(諸如用來移除玻璃體、血液、瘢痕組織或晶狀體的程序)。此工具可包括一控制操縱臺及一外科醫生使用來解剖及移除組織之外科手持件。關於後房手術,該手持件可為玻璃刀具探針、雷射探針或用來切割或破碎組織之超音波破碎機(fragmenter),及可藉由長的空氣壓力(氣流式)線及/或電纜、光纜或用來將輸液供應至手術地點及用來從該地點抽回或吸出流體及被切割/破碎的組織之撓性管連接至該控制操縱臺。手持件的切割、輸液及吸出功能可由遠端控制操縱臺來控制,其中該操縱臺不僅提供該外科手持件(例如,往復運動或旋轉切刀或超音波振動針)電力,而且亦可控制輸液的流動及提供用來吸出流體及被切割/破碎的組織之真空(相對於環境)來源。操縱臺的功能可由外科醫生手動地控制(例如,透過使用腳操作的開關或比例控制)。
在後房手術期間,外科醫生可在程序期間使用數個手持件或工具。此程序會需要這些工具插入及移出切口。此重覆移出及插入可造成眼睛在切口位置處損傷。為了解決此關心的事,至少在1980年代中期發展出襯套式套管(hubbed cannulae)。這些裝置可包括一附加有襯套的窄管。可將該管插入在眼睛中的切口至最多該襯套處,其中該襯套可作用為制子以防止管子完全進入眼睛中。該襯套可被縫至眼睛以防止不慎移除。可經由該管將手術工具插入眼睛中,及該管可保護切口側壁不由該工具重覆接觸。此外,外科醫生可使用該工具(藉由操縱該工具,當該工具經由該管插入眼睛中時)來幫助在手術期間定位眼睛。先述技藝套管的缺點可包括在眼睛表面上凸出的高度,和缺乏任何在工具交換或移除期間控制眼壓喪失的設備。當手術裝置不存在時,眼睛(其為一有壓力的眼球)會將水或玻璃體排出該開放式套管。在先述技藝套管中,藉由將一栓塞或罩套塞入管中來密封該套管及防止流體與組織釋出來防止眼壓喪失。此可為一耗時過程,其會需要額外的測試設備和其它操作室人員的輔助及會增加手術後感染的風險。

在某些具體實例中,排出器601可為一與套管分離的裝置,以允許排出器601插入及移出而沒有加入或移除套管101的零件(例如,不必需移除套管101的頂蓋103)。排出器601之尺寸亦可允許使用者(例如,外科醫生)在排出器601之插入及移出期間以手指(或例如,鑷子)處理排出器601。

Claims (11)
- 一種眼用裝置,其包含:一套管,其被構形成用來插入眼睛中;一附加至該套管的頂蓋,其中該頂蓋被構形成不會相對於該套管轉動;及一在該套管與該頂蓋間之密封物,其中該密封物被構形成允許經由在該密封物中的隙縫讓手術工具通過進入該套管中,同時當手術工具不存在於該密封物中時抑制流體流過該密封物;其特徵在於該密封物係由一經超模壓進入在該頂蓋中的凹洞中的彈性體所製得;以及其中該密封物係經模塑進入在該頂蓋中的至少一孔洞。
