Spaet, publiserad av Grune & Stratton, Inc. Det är således väl- känt att vävnadsproteiner koagulerar vid temperaturer mellan 50 sen 1oo°c.
Koaguleringen av blödande kärl, såsom t.ex. vid blödande ulceration i gastrointestinala delar av kroppen, kräver att ett långt endoskop användes, från vars distala ände det blö- dande området först måste identifieras och därefter behand- las med ett instrument, som är genomfört genom en kanal utbil- dad i endoskopet. Det är ej enkelt att lokalisera det blödan- de stället, eftersom vävnadsväggen vid undersökning ofta rör sig, och sönderfall i form av partiklar lätt uppstår och själ- va blodflödet tenderar att dölja de blödande källorna. Dessa kan vara mycket små, av storleksordningen mindre än 1 mm, var- vid mânga är närvarande i ett speciellt område och varje måste koaguleras.
Endoskopct är därför också utbildat med en spolkanal, genom vilken en fluid, såsom en vätska eller gas, kan ledas, för att spola bort sönderfallet och tillåta visuell granskning av det vävnadsområde som skall behandlas. Vid ovan angivna arti- kelhßndoscopic Laser Treatment, användes ett gasflöde som är koaxiellt med laserfibern för att rensa vävnaden. Elledarna utskjuter i form av åtskilda slingor från en distal ände av katetern. vid behandling av ett vävnadsområde, utsattes varje liten blodkälla för värmebehandling. Detta betyder att vävnaden rensas medelst en fluidspolning, som efterföljes av applika- tion av värme, varefter området åter rensas och värme appli- ceras och så vidare, tills alla de blödande områdena har koa- gulerats. Vid sådan behandling skall de upprepade applikatio- nerna utföras med färdighet och på ett omsorgsfullt sätt och med ett minimum av ovälkomna sidoeffekter, såsom vidhäftning av koaguleríngsanordningen vid vävnadsområdena. Lasertekniken har den fördelen att den ej kräver fyskdfkontakt, och undvi- ker således sådana vidhäftningsproblem, men på grund av det varierande sätt, på vilket olika vävnadstíllstând tillåter absorption av laserenergi, är exakt styrning svår under väv- nadsbehnndling. Den enpoliga elektrokirurgiska anordningen tenderar att skada vävnad som ej är avsedd att behandlas och även att medföra skador i själva målområdet, såsom exempelvis överdrivet djupa effekter i målområdet. Tvåpolig elektrokirur- gisk behandling av vävnad har följaktligen använts och före- slagits för ökad säkerhet, emedan den elektriska strömmen är begränsad till det lilla området mellan elektroderna. Flera tvåpoliga anordningar har föreslagits.
Så t.ex. föreslår ett tidigt US-patent 16U 18U (Kidder) från 1875 en tvåpolig elektrokirurgisk anordning, vid vilken ett par elektriska ledare är skruvlínjeformat lindade på en sond- kropp av gummi, i vilken ledarna är inbäddade. Elledarna upp- hör vid en distal, halvsfäriskt formad ände av sondkroppen.
En termiskt uppvärmd kniv beskrives och visas i US-patent 1 366 756 (R.H. Wappler), som använder ett par i tvärsnitt halvcirkelformade elledande stänger, som är líndade omkring en isolator och kopplade till en glödkniv. 193N föreslår Kimble en tvåpolig elektrokirurgisk anordning i US-patent 1 985 669, vid vilken ett par elledare är lindade omkring en gemensam isolator och utskjuter från ett kvarhållningsor- gan på ett_sätt som är användbart vid applikation på tvären eller med spetsen före på ett vävnadsområde. 8200126-4 US«patent N 011 872 (Komiya) föreslår en elektrokirurgisk anordning, vid vilken, såsom exempelvis visas i fig. 5, 9 och 11, en elledare är kopplad till en högfrekvensenergíkäl- la och utbildad av tre eller fyra elektroder. Elektroderna 5 sträcker sig individuellt från en distal ände, varvid mellan- rummen mellan elektroderna är varierbara för att betjäna el- ler gripa över vävnadsområden med olika storlek. I US-patent 3 987 795 (Morrison) beskrives en elektrokirurgisk anordning som arbetar med en metod belägen mellan de en- och tvâpoliga 10 metoderna för elektrokirurgí. Detta uppnås genom att på en kropp, exempelvis av keramik eller glas; monteras en aktiv elektrod och en returelektrod, vars ytområde är betydligt större än den aktiva elektrodens. Olika sondutformningar är illustrerade på ritningen. 15 Ehuru dessa tidigare kända elektrokirurgiska anordningar är användbara, medger de av flera skäl ofta ej tillfredsställande ingrepp. Som anmärkts ovan, är det exempelvis viktigt att sond- kroppen, medelst vilken en brännande högfrekvent ström till- 30 föres, upprepade gånger och exakt kan bringas att stöta emot de tunna blodkärlens öppningar i det vävnadsområde som skall t behandlas oberoende av sondens orientering. Detta kräver att sonden är manuellt styrbar vid den proximala änden av ett endoskop, att tillfredsställande elektrisk kontakt uppnås för 25 koagulering av ett blodkärl eller annat vävnadsmålområde, an- tingen sondkroppen appliceras med spetsen före, med vinkel eller på tvären på vävnadsområdet.
Användning av elektrodutformningar såsom visade vid ovan be- 50 skriven tidigare känd teknik, är således ofta otillfredsstäl- lande, på grund av det stora antalet sondapplikatíoner som er- fordras för behandling av ett vävnadsmål eller för att uppnå koagulering av ett blödande vävnadsområde. 55 Qversiktwöver uppfinningen Medelst en elektrokirurgisk anordning enligt föreliggande upp- finning uppnås en exaktare och noggrannare vävnadsbehandling I ~ medelst en flerpolig sondkropp, på vilken flera elektroder är fördelade på ett förutbestämt sätt. Sondkroppen är försedd med elledare, som är utbil~ dade av flera elektroder. De olika elledarnas elektroder är selektivt dímensionerade och på det hela taget likformigt för- delade i åtskilda par över den distala änden och sidan på sondkroppens periferiyta och med ett förutbestämt minimiantal av åtskilda par för att möjliggöra godtyckligt inriktad fler- polig behandling av vävnad då sondkroppen under arbete utskju- ter från endoskopets distala ände. Termen flerpolig, som an- vändes härí, betyder den elektrokirurgiska användningen av flera elektroder, som är anordnade i fixerat förhållande till varandra på en sondkropp för åtminstone tvåpolig kontakt och en exakt behandling av små vävnadsmål inom ett brett oriente- ringsområde för den elektrokirurgiska anordningen relativt vävnadsmålet. ' ' Som beskrivits för den ena utföringsformen av den elektro- kirurgiska anordningen enligt uppfinningen, är sondkroppen försedd med en central borrning, som sträcker sig från sond- kroppens proximala ände till dess distala och har en storlek som är så avpassad att den tillåter passage av tillräckligt mycket vätska för att rensa det vävnadsområde som skall be- handlas. Sondkroppsborrningen är utbildbar med en ledande fodring såsom del av en elledare, längs vilken högfrekvent ström tillföres ti11'elektroderna som konvergerar vid och är kopplade till den ledande fodringen vid sondkroppens distala ände.
Medelst den elektrokirurgiska anordningen enligt uppfinningen kan tillträde till ett blödande vävnadsområde ske inom ett brett oríenteríngsområde, men likväl kan detta behandlas med större effektivitet och medelst färre sondapplikationer. En mera likformig koaguleríng uppnås med begränsat skadedjup och en bättre förutsägbar koaguleringszon. En mekaniskt mild kon- takt med vävnaden som skall behandlas är möjlig.
Användning av flera elektrodpar av olika elledare tillförsäk- rar åtminstone tvåpolig eller mângtvåpolig vävnadskontakt vid applícering av sondkroppen, medan sondkroppen är tillräckligt liten för att eiektriskt koagulera de indiviaue11a biøakärien FJ \.71 f) 0 55 IH) 8200126-4 6 från den distala änden av ett endoskop. En Särskilt effektiv sondkropp enligt uppfinningen använder sig av minst sex elektroder, som bildar motsvarigheten till sex tvåpoliga koaguleringsanordningar, omkring den endoskopiskt förflytt- hara sondkroppens periferiyta. Medelst en sådan elektroki- rurgisk anordning kan tvåpolig eller trepolig vävnadskontakt, eller vävnadskontakt med högre polantal, utföras oberoende av sondkroppens orientering, för effektiv behandling av vävnad, såsom t.ex. gastrisk blödande ulceration.
Ett ändamål med föreliggande uppfinning är därför att åstad- komma en elektrokirurgisk anordning, som är pålitligt appli- cerbar på ett noggrant sätt vid elektrokirurgisk behandling av små vävnadsmål.
Ytterligare ett ändamål med uppfinningen är att åstadkomma en i elektrokírurgisk anordning, som är förflyttbar genom ett endo- skop på ett pålitligt och konsekvent sätt för koagulering av blödande kärl från endoskopets distala ände. Ytterligare ett ändamål med uppfinningen är att åstadkomma en godtyckligt in- riktbar, effektiv elektrokirurgisk anordning, medelst vilken gastrointestinal blödande ulceration är behandlingsbar på ett effektivt sätt genom ett endoskop.

Endoskopet 10 uppvisar en lång böjlig axel 12, ehuru uppfinningen är användbar med olika endoskop uppvi- sande stel axel. Vid sin proximala ände är endoskopet 10 för- sett med ett styrhuvud 1U, en betraktningsanordning 16 och en spak 18 för styrning av deflexionen hos den böjliga axelns 12 böjliga distala ände 20. Den böjlíga axeln 12 har ett antal kanaler som möjliggör betraktande med hjälp av flexibel fiber- optik, en kanal för tillförsel av en rengöringsfluid, såsom gas eller vatten, och en kanal genom vilken speciella anord- ningar är genomförbara, såsom en pincett, borste, eller kniv, för utförande av en kirurgisk procedur.
Endoskopet 10 i fig. 1 är utbildat med en kulformad elektro- kírurgisk anordning 22, medelst vilken vävnad är behandlings- bar från axelns 12 distala ände 20. Den elektrokirurgiska an- ordningen 22 är presspassat förbunden med distala änden på en lång kateter 2D, som är genomförd genom en endoskopkanal för förbindning genom en fluidtät anslutning 26 med en tryèkfluid- källa 28 vid endoskopets 10 proximala ände. Isolerade elektris- ka kablar 30, 52, som är förbundna medcbn elektrokirurgiska an- ordningen 22, är genomförda genom kateterns ZU lumen BH och anslutningen 26 för koppling till en högfrekvenskälla 36.
Existerande elektrokirurgiska generatorer är användbara och om så är nödvändigt används ett enkelt anpassningsnät, såsom en resistor över kablarna 30, 32, i några fall är en isoler- transformator anbringad i kretsen. ' Den elektrokirurgiska anordningen 22 är utformad av en elektriskt isolerande sondkropp H0, som är avpassad för genom- föring genom en endoskopkanal från dess proximala ände till dess distala. Sondkroppen H0 är såsom den är illustrerad i fig. 2, 5 och H mycket förstorad, eftersom vid en praktisk storlek dess maximala tvärsnittsmått är av storleksordningen 10 30 NO 8200126-4 2,U mm. Sondkroppen H0 uppvisar en väsentligen cylindrisk form, varvid dess distala ände H2 är mjukt trubbigt avrundad till väsentligen halvsfärisk form.
Sondkroppen HO uppvisar en ytterperiferiyta HU, på vilken ett par elledare H6, H8 är anbringade och elektriskt förbundna med kablar 30 resp. 32. Varje ledare H6, H8 är utbildad av tre tunna längsgående bandelektroder ü6.1, U6.2, U6.3 och U8.1, U8.2, U8.3. Elektroderna är anordnade på periferiytan UH och inriktade väsentligen parallellt med sondkroppens H0 centrumaxel 50 och är likformigt fördelade i periferiell led med vinkelintervall på 600. De olika elledarnas H6, M8 elektro- der är omväxlande åtskilt anordnade längs varandra med avstånd S. Spalten S har i huvudsak omkring samma storlek som elektro- dernas bredd W vid sondkroppens NO cylindriska del, där elektroderna också uppvisar väsentligen lika storlek. För en sondkropp HO med en diameter på 2,H mm är spalterna S och bred- den W utförbara med en storlekmndninp på omkring 0,6 mm.
Elledarens 46 elektroder ü6.1, H6.2 och H6.3 är elektriskt förbundna med en ledande ring 52, som är uanbringad på en radiellt försänkt.ringavsats 5U på sondkroppen HO vid dess proximala ände 56. Elektroderna Ä8.1, U8.2 och H8.3 är vid den distala änden H2 elektriskt förbundna med en ledande fod- ring 58 anbringad i ett centralt genomgående hål 59 i sond- kroppen H0. Fodringen 58 har formen av ett rör, som sträcker sig från den proximala änden 56 in i kateterns 2U lumen BU och uppvisar en central spolkanal 57.
De tunna elektroderna uppvisar en gradvis avtagande bredd vid den distala änden H2 för att ge ett väsentligen fast mellan- rum mellan de omväxlande åtskilt anordnade elektroderna, sam- tidigt som de bildar flera likformigt fördelade motsatta elektrodpar eller -poler omkring centrumaxeln 50 för en eller flera tvåpoliga kontakter med vävnad. Medelst minst sex elektrodpoler (sexpolig) fast åtskilt anordnade omkring den distala änden H2 och sidan av sondkroppens UO periferiyta BU,- är åtminstone tvåpolig vävnadskontakt och ofta kontakt med högre poltal uppnåelig i huvudsak oberoende av sondkroppens H0 orientering relativt vävnaden, varvid lämplig termisk upp- us RU 50 Hu _. ...___.~__.- ...___-Wu “_ ~ ..._ .. . _. 8200126-4 värmning av små vävnadsmål erhålles.
Elektrisk kontakt mellan elektroderna H6.1, H6.2, U6.3 och den ledande ringen 52 uppnås som visas i fig. H medelst de radíella ledande segmenten 60.1, 60.2 resp. 60.3. Kabeln 50 är förbunden med ringen 52 vid ett radiellt urtag 62 i den försänkta ringavsatsen 5U, där urtaget 62 är avpassat för att mottaga den isolerade kabelns 30 ledare 6H. Urtaget 62 uppvi- sar en ledande beläggning 66 som är i elektrisk kontakt med ringen 52 och sammanlödd med elledaren 6U.
Elektrisk kontakt mellan elektroderna H8.1, H8.2, U8.3 och kabeln 52 är utförd vid den proximala änden 56, där elleda- ren 68 i kabeln 32 är lindad omkring och hoplödd med det le- dande röret 58. Rörets 58 förbindning med elektroderna U8.1, H8.2 och H8.5 är utförbar genom lödning.
Vid en bruklig teknik för tillverkning av den elektrokirur- giska anordningen 22 enligt uppfinningen, är sondkroppen HO utformad av ett bearbetbart keramiskt substrat, såsom t.ex. det under handelsnamnet MACOR. Keramiken skäres till önskad form, dvs. med den halvsfäriskt formade distala änden H2, centrumborrningen 59, den försänkta ringavsatsen 5H och ur- taget 62. En ledande metallísk massapåmflassedan under mikro- skop eller genom att sondkroppen förflyttas relativt en tjock filmtryckscreen på det tillskurna keramiska substratet, för bildning av de ledande elektroderna H6, H8 och ringen 52.
Den metalliska massan utformas företrädesvis av ett material, som vid användning av värme (bränning) bildar en stark smält- bindning med det keramiska substratet. Massor för detta ända- mål är välkända inom halvledare- och elektroniska tillverk- ningsindustrín. Den metalliska massan är företrädesvis också vidareförlängd in i borrningen 59, och är efterföljd av rörets 58 införing, så att vid bränning en elektrisk förbindning automatiskt uppnås vid den distala änden mellan röret 58 och H8.2 och U8.3. UT HU '"7 8200126-4 10 är elektrokoagulering utförbar vid olika orienteringar hos sondkroppen relativt vävnaden och uten att svängning av sund- kroppen erfordras. Detta är särskilt fördelaktigt då anord- ningen användes genom ett endoskop, så att applikationer me- delst sonden med spetsen före, på snedden eller på tvären resulterar i åtminstone tvåpolíg kontakt.
Medelst en elektrokírurgisk anordning 22 enligt uppfinningen kan det elektriska fältmönstret omkring sondkroppen väljas så, att en homogen termisk uppvärmning åstadkommes nära vävnads- ytan som kontaktas av sondkroppen. Vid den ovan lämnade be- skrivningen av den elektrokirurgiska anordningen 22, kan fält- linjerna 72 som visas i fig. 5 för en given fältstyrka mellan närbelägna elektroder vara såsom allmänt illustreras i fis. 5.
Fältlinjernas 72 radiella utsträckning är en funktion av stor- leken på spalten S mellan elektroderna. För några tillämp- ningar, där ett mindre radiellt fältdjup är önskvärt för reduk- tion av koaguleringsdjupet, är spalten mellan de tunna elektro- derna således reducerbar. I detta fall är flera elektroder an- vändbara, vilket resulterar i ett större antal tvåpoliga kon- takter. Om en djupare vävnadsbehandling erfordras, kan spal- ten S mellan elektroderna ökas. Antalet elektroder och spalt- storlekarna kan således väljas i avhängighet av den speciella fysiologiska vävnad som skall behandlas.
I fig. 6 visas elektroderna matade från en flerfas högfrekvens- källa 76. Källan 76 är en trefaskälla, som är stjårnkopplad med elektroderna ü6.1, ü6.2 och H6.) med en återgång 78 för- bunden med elektroderna ü8.1, ü8.2 och H8.3. Med den flerfa- siga högfrekvenskållan 76 blir spänningen mellan elektroderna, såsom t.ex. U8.1 och U8.2, större än mellan elektroderna 36.1 och H8.1, varigenom ett starkare fält erhålles för djupare koagulering. Förbindningen mellan källan 78 och sondkroppen H0 innefattar fyra kablar i stället för två, såsom vid utfö- ringsformen enligt fig. 2 till 5.
I fig. 7 visas en elektrokirurgisk anordning 80, som använder sig av en liknande utformad sondkropp H0, men där elektroder- na är fördelade i periferiella ändlösa band 82.1 till 82.8.
Detta arrangemang är avsett för vävnadsbehandling av inner- f f) 50 M0 8200126-4 11 vännen i ett anatomiskt rör. Hlektroderna 82.1 till 82.8 är ~rienterade i plan, som är vinkelräta mot centrumaxeln 50.
Elektrisk förbindníng mellan kablarna 30, 52 och elektroder- na 82 utföres medelst ett par elektriska elledare 8U, 86, som är anordnade i borrningar borrade parallellt med anordningens 80 centrumaxel 50. En central borrning 59 är utbildad för att inrymma en ledande fodring eller rör 58. Kontakt mellan le- darna 8U, 86 och elektroderna 82 utföres genom ledande fodra- de eller fyllda hål 88, som är så anordnade, att de skär av- sedda elektroder och ledare 8ü, 86, såsom illustreras i fig. 7.
Ledaren RM är liknande förbunden med ringelektroden 52, på vilken kabeln 30 är fastlödd. Ledaren 86 är förbunden med det ledande röret 58 tillsammans med kabelns 32 ledare. Tillverk- ning av den elektrokirurgíska anordningen 80 är utförbar i likhet med vad som föreslagits för den i fig. 2 visade anord- ningen.
Efter att härmed ha beskrivit den elektrokirurgiska anord- ningen enligt uppfinningen, kan dess fördelar värderas. Den neutrala spolkanalen genom röret 58 är särskilt användbar vid rensning av vävnadsområden belägna precis framför den elektro- kirurgiska anordningen. Kanalen eller röret är tillräckligt brott för att inrymma önskad flödesmängd av vätska eller gas.
Fluiden är såsom íllustrerats ínledbar genom kateterns 2H lumen BU mellan kablarna 30, 32, eller om utrymmet tillåter är en särskild ledning användbar som passar på insidan av lumen šü och över rörets 58 proximala ände. Den ledande fod- ringen 58 för borrningen 59 i sondkroppen HO är vid vissa tillämpningar utelämnbar. I detta fall är elektrisk kontakt med elektroderna H8.1, ü8.2 och U8.3 upprättbar från en delad ledande ring 52, med vilken kablarna 30, 52 sedan förbindes.
Medelst det geometriska arrangemanget och distributionen av tunna elektroder på den elektrokirurgíska anordningen, såsom visas i fig. 2 till 5, erhålles fördelarna med tvåpolig väv- nadsbehandling, och särskilt förmågan att närma sig ett väv- nadsmålomrñde på måfâ, antingen på tvären, med spetsen före ..._ .. _. ,_._........ - - - - 8200126-4 12 eller på snedden, utan att förmågan till behandling av mål- området förloras. Inkluderandet av en central spolkanal ökar vidare användbarheten för den elektrokirurgiska anordningen.
Claims (12)
1. Elektrokirurgisk anordning för behandling av vävnad inne- fattande en flerpolig, långsträckt sond med en länqdaxel (50) från en proximalände (56) till en distal ände (42) av sond- kroppen (40), k ä n n e t e c k n a d av att en kanal (57) sträcker sig från proximaländen till ett utlopp vid proximal- änden (56) för ett genomflöde för bortförsel av vävnad varvid sonden har endoskopiskt anpassade mått, uppvisar elektriskt isolerade ledare (30,32) med på sondens utsida anordnade elektroder (46,48) så att en le- dares elektroder (46) är belägna emellan en annan ledares elektroder (48), varvid de skilda ledarnas elektroder har sådan bredd, längd och fördelning att de i stort sett är anord- nade i jämnt fördelade, åtskilda par kring distaländen (42) vid utloppet samt över utsidan (44) kring axeln (50) för att ävnad effektivt i alla att sonden medge åtminstone bipolär behandling av v riktningar mot kringliggande vävnad.
2. Elektrokirurgisk anordning enligt krav 1, k ä n n e t e c k» n a d av att elektroderna (46,48) är ungefär lika breda som avståndet mellan dem.
3. Elektrokirurgisk anordning enligt krav 1 eller 2, k ä n n e~ t e c k n a d av att varje ledare är ansluten till minst tre elektroder (46.1, 46.2, 46.3; 48.1, 48.2, 48.3) vid sondens distalände och pâ dess utsida. '
4. Elektrokirurgisk anordning enligt något av krav 1-3, k ä n~ n e t e c k n a d av att sondkroppen (40) är av ett elektriskt isolerande, stelt material.
5. Elektrokirurgisk anordning enligt krav 4, k ä n n e t e c k- n a d av att ett elektriskt ledande foder (58) är anordnat i kanalen (57) i elektrisk förbindelse med en av ledarna (32) och med en grupp av elektroderna (48).
6. Elektrokirurgisk anordning enligt krav 5, k ä n n e t etc k~ n a d av att distaländen (42) har avtrubbad, ogival form. V---~ --_- ~------- I $š200126~4

10. 19. ßlektrokirurgisk anordning enligt något av krav 1-9, k ä n- n e t e c k n a d av att antalet ledare (30,32) och elektroder (46,48) är anpassat att samverka med en flerfasig växelspännings- källa (76).
11. Elektrokirurgisk anordning enligt något av föregående krav, k ä n n e t e c k n a d av att sondkroppen (40) är krökt vid dístaländen (42).
