Description
Foreliggende oppfinnelse vedrører munnhygieniske preparater. Mer spesielt angår oppfinnelsen munnhygieniske preparater som hindrer og bekjemper munnlukt, tannsten, plaque og karies, og inneholder aktive zinkioner og fluoridioner. Disse preparater er også effektive for hindring av gingivitt og periodontal sykdom. Bruken av oppløselige fluoridsalter slik som stannofluorid, natriummonofluorfosfat og natriumfluorid, for å redusere hyppigheten av tannkaries hos den alminnelige befolkning er en velkjent og pågående bestre-belse. Administrasjonen av disse fluoridforbindelser har mange former, inkludert fluortilsetning av drikkevann, profesjonell behandling av tannleger og inkorporering i munnhygieniske preparater slik som tannpasta og munnvask.
Det er også kjent at sink spiller en rolle innen tannpleien. I US patent 4.160.821 beskrives det f.eks. et preparat for behandling av gingivitt omfattende en bærer inneholdende en høy konsentrasjon av glycerol og et sinksalt, slik som sinkklorid som er oppløselig i glycerolen.
I en artikkel betegnet "Inhibition of Plaque Growth by Zinc Salts", Journal of Periodontal Research", vol. 18, side 634-642, 1983, rapporterte Harrap et al. at oppløsninger av zinksalter reduserte plaque med 30% langs tannkjøttkanten. I "Effect of some Polyvalent Cations of Plaque Formation in vivo", Scandinavian Journal of Dental Research, Vol. 186, side 103-107, 1978, angis det at munnskyllinger inneholdende enten ZnCl„ „,
2, AICI3, MgCl2, SnF2 eller SnCl2 viste signifi-kant plaque-reduserende aktivitet. En oral skylling inneholdende 1,0% sinksalter (som fenolsulfonatet) eller 0,125% sinkkompleks med tribromsalan ble funnet klinisk å redusere den midlere tannstenhyppighet med ca. 60% og 5 3% respektivt, i to etter hverandre følgende forsøk av tre måneders varig-het som rapportert av Picozzi et al. i en artikkel betegnet "Calculus Inhibition in Humans", Journal of Periodontology, vol. 43, side 692-695, 1972. Sinkforbindelser (som 1%
zinkfenolsulfonat og 0,125% sinktribromsalan (formulert i en vandig munnvask forsinket betydelig utviklingen av gingivitt i en testgruppe sammenlignet med en placebogruppe av Fishman et al., Journal of Periodontology, vol. 44, side 535-539, 1975.
I britisk patent 1.373.003 beskrives et tannpastapreparat som har aktivitet mot plaque og tannsten på en tannover-flate, omfattende et tungt oppløselig sinksalt som er definert som et sinksalt av en syre, forskjellig fra zinkfluorid eller dets hydrater, med en vannoppløselighet større enn den til sinkfosfat og mindre enn 1 g zink pr. 100 ml vann ved 20°c og en blanding av detergenter. Tannpastaen kan også innheholde et forenlig slipemiddel slik som aluminiumoksyd og andre konvensjonelle tannpastabestanddeler. US patent 4.146.606 angår et farmasøytisk preparat for anvendelse på tenner, som blant annet kan undertrykke tannkaries, og dette preparat omfatter en strontium-forbindelse, en sinkforbindelse, tannin og eventuelt en fluorforbindelse.
US patent 4.138.477 beskriver et preparat for hindring og bekjempelse av munnlukt, som også angis å være effektiv for hindring av tannsten, plaque, karies og periodontalsykdom-mer, inneholdende som vesentlig bestanddel et sink-polymer-kompleks dannet ved omsetning eller gelatering av en sinkforbindelse med en anionisk polymer inneholdende karboksyl-, sulfon- og/eller fosforsyreradikaler. Preparatet kan også blant annet innbefatte en fluorholdig forbin-delse som beskytter tennene mot tannråte. US patent 4.396.599 beskriver antikariespreparater inkludert tannpastaer inneholdende en fluoridforbindelse og en sinkforbindelse med et forhold for sinkion til fluoridion på minst ca. 7:1, beregnet på vekt.
Det er et formål med oppfinnelsen å tilveiebringe forbedrede munnhygieniske preparater.
Et annet formål med oppfinnelsen er å tilveiebringe munnhygieniske preprarater som hindrer og bekjemper munnlukt, tannsten, plaque, karies og gingivitt.
Et ytterligere formål med oppfinnelsen er å tilveiebringe munnhygieniske preparater som minimaliserer snerpende smak og viser god fluoridion- og sinkion-biotilgjengelighet.
Andre formål ved oppfinnelsen vil fremgå fra den følgende, detaljerte beskrivelse derav.
De ovenstående formål og andre trekk og fordeler ved foreliggende oppfinnelse, oppnås ved munnhygieniske preparater formulert for tilveiebringelse av tilgjengelige fluorid- og sinkioner fra farmasøytisk akseptable salter i en formulering inneholdende et spesielt buffersystem, og som har en pH-verdi fra ca. 3,5 til 6,0.
Foreliggende oppfinnelse angår munnhygieniske preparater inneholdende en effektiv konsentrasjon av et farmasøytisk akseptabelt fluoridsalt og et farmasøytisk akseptabelt sinksalt, et spesielt buffermiddel og har en pH-verdi fra ca. 3.5 til 6,0.
Typiske farmasøytisk akseptable fluoridforbindelser som er egnet for bruk i foreliggende preparater, innbefatter natriumfluorid, kaliumfluorid, lithiumfluorid, aluminiumfluorid, sinkfluorid, stannofluorid, natriummonofluorfosfat, surgjort fosfatfluorid, ammoniumfluorid, ammoniumbifluorid, titantetrafluorid og aminfluorid.
Sinksaltet som er egnet for bruk i foreliggende preparater, innbefatter sinkklorid, sinksulfat, sinkcitrat, sinkacetat, sinklaktat, sinksalicylat, sinktiocyanat, og mer generelt et hvilket som helst farmasøytisk akseptabelt sinksalt.
Fluoridionkonsentrasjonen kan være fra så lav som 0,0025% til ca. 4,0 vekt-%. Det foretrukne området for fluoridion-konsentras jonen er fra ca. 0,005 til ca. 1,0 vekt-%, mer spesielt ca. 0,01-0,5%. Sinkionkonsentrasjonen bør være fra ca. 0,02 til 30,0 vekt-%, fortrinnsvis fra ca. 0,05 til 10,% og mer spesielt fra ca. 0,1 til 5,0%. Mens høyere konsentrasjoner av både sink- og fluoridioner kan anvendes, ville ingen spesiell fordel bli oppnådd ved dette, og det er noen motsigelser i litteraturen vedrørende sikkerheten av høyere konsentrasjoner av fluorid- og sinkioner.
Forholdet for sinkioner til fluoridioner kan variere fra 0,05:1 til 25:1, fortrinnsvis fra ca. 5:1 til 15:1. Fluoridionene opptas av tannemaljen og gjør tennene mindre mot-tagelige for kariesangrep og tannråte ved å øke emaljens hardhet eller redusere emaljeoppløseligheten. Sinkionene hjelper til å hindre dannelsen av tannsten, dårlig munnlukt og gingivitt.
Valget av spesielle buffermidler er vesentlige for foreliggende preparaters ytelsesevne, og er nødvendige for å opprettholde den sure pH-verdi og hindre utfelling av sinkionene. Natriumglukonat i en mengde fra ca. 0,01 til 10,0%, fortrinnsvis fra ca. 0,05 til 5,0%, er et foretrukket buffermiddel. Andre organiske syrer/salter som kan anvendes, innbefatter maleinsyre, aspartinsyre, glykonsyre, ravsyre, glukoronsyre, glukoronsyrelakton, natriumglutamat og fumarsyre. Andre organiske syrer og salter slik som melke-syre, adipinsyre, og vinsyre som vil forventes å oppføre seg på samme måte, har vist seg å være utilfredsstillende.
For å oppnå de ønskede resultater fra foreliggende preparater, bør pH-verdien opprettholdes mellom 3,5 og 6,0. Dette tillater fluoridionene og sinkionene å forbli i oppløsning og ikke utfelles, og dermed gi dem anledning til å få deres ønskede effekt. Dette kan best oppnås ved å regulere pH-verdien til det ønskede område ved tilsetning av en kombinasjon av surt sakkarin og natriumsakkarin i en mengde fra ca. 0,01 til 5,0%, fortrinnsvis 0,1-1,0%. Dersom natriumsaltet eller syren anvendes alene, kan pH-verdien justeres ved tilsetning av en passende syre eller base. Natriumsakkarinet og sakkarin bør foreligge i et forhold fra ca. 20:1 til 2:1, fortrinnsvis ca. 7:1.
Egnede farmasøytisk akseptable munnhygieniske bærere som kan benyttes alene eller i kombinasjon med foreliggende preparater, innbefatter glycerol, vann, etanol, polyetylenglykol, propylenglykol, sorbitol o.l. Andre bærere kan benyttes dersom de er forenlige med de andre bestanddelene i preparatene .
Dersom preparatene ifølge oppfinnelsen er i form av tannpasta, bør de også inneholde et egnet slipemiddel. Slipe-middelet bør være slik at det ikke skader emaljen eller tannbenet, mens de er i stand til å rense og polere tennene samt at det er forenlig med fluoridionene og sinkionene. Foretrukne slipemidler innbefatter silisiuradioksydslipe-midler. Silisiumdioksydslipemidler som er nyttige, er de som har en gjennomsnittlig partikkelstørrelse fra 0,1 til 45 pm, fortrinnsvis fra ca. 45 pm, fortrinnsvis fra ca. 4 til 20 pm; med en pH-verdi fra ca. 3,5 til 9,0; og vanligvis er i form av utfelt silisiumdioksyd, silisiumdioksydxerogeler eller silisiumdioksydhydrogeler. De foretrukne silisium-dioksydslipemidlene er de silisiumdioksydhydrogeler som er tilgjengelig under varebetegnelser "Hydrous Silica Gel". De foretrukne silisiumdioksydhydrogelene har et vanninnhold på 15-60% og en pH-verdi på ca. 4,5. Den lave pH-verdien til silisiumdioksydmaterialene hjelper til å holde fluoridionene og sinkionene oppløselige, og derfor tilgjengelige og hjelper også til å holde formuleringenes pH-verdier stabile. Andre egnede silisiumdioksydslipemidler innbefatter silisiumdioksydxerogelene som beskrevet i US patent 4.100.269, og de utfelte silisiumdioksydmaterialene som er beskrevet i US patent 3.862.307 tilgjengelig under varebetegnelsen "Zeodent".
Silisiumdioksydslipemidlerie bør være til stede i tannpastapreparatene i en mengde fra ca. 5,0 til 70,0%, fortrinnsvis fra ca. 10,0 til 40,0%. Disse materialer er forenlige med sink- og fluoridionene i slike formuleringer og tjener som rense- og poleringsmidler.
Tannpastapreparatene bør også inneholde spesifikke polymerer som er nyttige som bindemidler og fortykningsmidler i preparatene. Disse innbefatter ikke-ioniske vannoppløselige polymerer som er forenlige med de andre bestanddelene i preparatet. Hydroksyetylencellulosepolymerer er tilgjengelige under varebetegnelsen "Natrosol" og er funnet å være foretrukne i preparater for opprettholdelse av viskositeten over et bredt pH-område. Disse ikke-ioniske polymerene bør være til stede i preparatene i en mengde fra ca. 0,03 til 10,0%, fortrinnsvis fra ca. 0,1 til 3,0%.
Det er ofte ønskelig for formuleringsformål å benytte mer enn ett bindemiddel for oppnåelse av de ønskede resultatene. I tillegg til hydroksyalkylcelluloseholdige bindemidler kan andre bindemidler velges fra xantangummi, carragenan, karayagummi, gummi arabicum og tragantgummi. Xantangummi er foretrukket og kan benyttes i en mengde fra ca. 0,02 til 5,0%, fortrinnsvis 0,1-3,0%.
En annen komponent i tannpastapreparater er et fuktemiddel. Fuktemiddelet tjener til å hindre tannpastapreparatene i å stivne ved eksponering til luft, og gir også en ønsket søthet til formuleringene for å minimalisere den snerpethet som tilskrives sinkkloridet. Fuktemiddelet omfatter, på basis av rent fuktemiddel, vanligvis fra ca. 5,0 til 80,0%, fortrinnsvis fra ca. 8,0 til 50,0 vekt-% av de totale preparater. Egnede fuktemidler innbefatter spiselige fler-verdige alkoholer slik som glycerin, sorbitol, xylitol og propylenglykol. Blandinger av glycerin og sorbitol er særlig foretrukne.
Vann er en annen vesentlig komponent i tannpastapreparater. Vann benyttet ved fremstilling av kommersielt egnede tannpastaer bør fortrinnsvis være deionisert og fri for urenheter. Vann omfatter fra ca. 0,05 til 70,0%, fortrinnsvis fra ca. 15,0 til 50,0 vekt-% av formuleringene. Disse vannmengder innbefatter det frie vannet som tilsettes pluss det som innføres med andre materialer.
En annen bestanddel i tannpastapreparater er et egnet over-flateaktivt middel eller detergent. Egnede overflateaktive midler er de som er rimelig stabile skum gjennom pH-området og er forenlig med sink- og fluoridforbindelsene samt de andre komponentene. Disse midler er vanligvis vannoppløse-lige, organiske forbindelser og kan være anioniske, ikke-ioniske eller kationiske i natur. Slike materialer er velkjente og innbefatter f.eks. de vannoppløselige saltene av høyere fettsyre-monoglyceridmonosulfater slik som natriumkokosmonoglyceridmonosulfat; høyere alkylsulfater slik som natriuralaurylsulfat, alkylarylsulfonat slik som natriumdodecylbenzensulfonat, høyere fettsyreestere av 1,2-dihydroksypropansulfonat og natriumsalter av kokos-fettsyreamidet av N-metyltaurin. Sistnevnte er særlig foretrukket fordi det er funnet å minimalisere snerpetheten til sinkklorid. Særlig nyttig er de ikke-ioniske blokk-kopolymerene avledet fra kondensasjonen av polyetylenglykol og polypropylenglykol. Disse blokk-kopolymerene er tilgjengelige under varebetegnelsen "Pluronic". Disse blokk-kopolymerene er tilgjengelige i flytende, pasta eller fast form. De foretrukne ikke-ioniske blokk-kopolymerene er faste materialer slik som "Pluronic F-85" og "Pluronic F-127" .
En annen foretrukken ikke-ionisk detergent er den ko-generiske blanding av konjugert polyoksybutylen-polyoksy-etylen beskrevet i US patent 4.323.552 og forhandlet under varebetegnelsen "Butronic". "Butronic Polyol L-1" og "Butronic Polyol R-1" er særlig foretrukne.
De ikke-ioniske overflateaktive midlene av "Pluronic"- og "Butronic"-typen er funnet å minimalisere snerpetheten til sinkkloridet og kan være til stede i en mengde fra ca. 0,5 til 10,0, fortrinnsvis fra ca. 1,0 til 5,0%.
Andre ikke-ioniske overflateaktive midler som kan benyttes, er kondensatene av sorbitanmonostearat med omkring 20 mol etylenoksyd. Amfotere midler inkluderer de kvaterniserte imidazolderivatene som er tilgjengelige under varebetegnelsen "Miranol" slik som "Miranol C^ jfl" .
Kationiske overflateaktive midler kan også benyttes. Disse forbindelser har detergentegenskaper samt germicide og anti-bakterielle egenskaper. Eksempler på slike kationiske detergenter er benzylammoniumklorid, benzyldimetylstearyl-ammoniumklorid, tertiære aminer med en fettalkylgruppe med 1-18 karbonatomer og to (poly)oksyetylengrupper festet til nitrogenet og salter derav med syrer, og forbindelser med strukturen:
hvor R er en fettalkylgruppe og kan ha fra ca. 12 til 18 karbonatomer, x, y og z er totalt 3 eller høyere, samt
salter derav med mineralsyrer eller orgainiske syrer, kan også anvendes. Fra ca. 0,5 til 15 vekt-% av disse kationiske overflateaktive midlene kan benyttes i tannpastapreparater.
I tillegg til de ovenfor beskrevne komponenter kan tannpastaene inneholde en rekke forskjellige eventuelle konvensjonelle tannpastabestanddeler. Slike eventuelle bestanddeler innbefatter preserveringsmidler, smakstoffer, søtningsstoffer, fargestoffer og pigmenter.
Egnede smaksstoffer innbefatter vintergrønnolje, pepper-mynteolje, olje av grønn minte, sassafrasolje og kløverolje. Søtningsstoffer som kan benyttes, innbefatter sakkarin, dekstrose, levulose, aspartame, D-tryptofan, dihydrochalko-ner og natriumcyklamat. Smaksstoffer anvendes vanligvis i tannpastaer i mengder fra ca. 0,01 til 2 vekt-% og søtnings-stoffer i mengder fra ca. 0,05 til ca. 2 vekt-%.
Disse tannpastapreparatene fremstilles ved sammenblanding av komponentene ved hjelp av konvensjonelle anordninger. Når de først er fremstilt, har preparatene en pH-verdi fra ca. 3,5 til 6,0, når preparatene oppslemmes med vann i et vekt-forhold for vann til preparat på 3:1. Disse tannpastapreparater anvendes på konvensjonell måte, dvs. preparatene eller oppslemmingene børstes på tannoverflåtene og skyldes deretter bort. Ved bruk av tannpastaene på denne måten kommer pastaer eller oppslemminger vanligvis i kontakt med tannoverflåtene i minst ca. 30 sek. Mer foretrukket kommer slike pastaer eller oppslemminger i kontakt med tann-overf låtene i minst ca. 60 sek.
Mens den ovenfor angitte omtale har vært rettet mot tannpastapreparater, kan foreliggende oppfinnelse også omfatte preparater i form av munnvask, gel, pulver, oppløsning, lakk, pastiller, tyggegummi, middel med langsom frigjøring eller andre former for oral anvendelse. Et hvilket som helst farmasøytisk akseptabelt materiale slik som de som vanligvis benyttes i slike orale preparater, som er forenlige med sink- og fluoridionene, kan benyttes i foreliggende preparater .
Spesifikke utørelser av oppfinnelsen er illustrert i de følgende representative eksempler.
Alle benyttede prosentandeler er beregnet på vekt med mindre annet er angitt.

Sammenligningsforsøk ble utført på rotte ved anvendelse av preparatet beskrevet i eksempel 3, en kommersiell tannpasta (tannpasta A) inneholdende 0,5% sinkcitrat, 0,8% natriummonofluorfosfat, med en pH-verdi på 7,85 og intet buffersystem, samt en annen kommersiell tannpasta (tannpasta B) som ikke inneholder noe sinkklorid,0,22% natriumfluorid, en fosfatbuffer, og har en pH-verdi på 7,05. I disse studiene besto hver gruppe av 30 Wistar-hannrotter, omkring 50 g hver og inneburet i par i trådnettingbur. Ved 19 dagers alder ble dyrene veiet og vilkårlig fordelt i grupper. Dyrene ble foret med NIDR diett nr. 5000 og ble gitt deionisert vann og drikke ad libitum. Omkring 0,15 av hvert preparat ble påført ved hjelp av en bomullsapplikator pr. kjeve. Behandlinger ble foretatt to ganger pr. dag, 7 dager pr. uke i 3 uker. Umiddelbart før avlivning ble alle dyrene observert med henblikk på visuelle tegn på dårlig helse og individuelt veiet. Ved slutten av de 3 ukene ble dyrene avlivet; hodene vasket, kjevene og tennene seksjonert, merket og bedømt med henblikk på tannkaries ved hjelp av Keyes-metoden. Dataene ble deretter analysert for signifikante forskjeller mellom gjennomsnitt for både antall og grad av karieslesjoner. Resultatene er vist i nedenstående tabell I.
Eksempel VI
Oppløsninger hver inneholdende 35,00 g sorbitol, 10,00 g glycerol, 2,00 g sinkklorid, 0,22 g natriumfluorid og 16,00 g deionisert vann, fremstilles. Til hver av disse oppløsninger tilsettes forskjellige organiske syrer og salter i varierende mengder og blandes, og dette resulterer i klare oppløsninger med det ønskede pH-området. Oppløsnin-gene hensettes i en uke ved 60°c og blir deretter studert på nytt og registrert som enten klare, uklare eller med et bunnfall. Resultatene er vist nedenfor i tabell II:
De ovenfor angitte resultater demonstrerer at uten noe organisk syre/salt dannes et bunnfall. Ikke alle organiske syrer er imidlertid nyttige i foreliggende preparater. De syrer som resulterer i dannelsen av et bunnfall eller uklarhet (begynnelse på utfelling) ville ikke være egnet fordi de ville resultere i en reduksjon av de tilgjengelige fluoridionene og sinkionene.

1 . Munnhygienisk preparat,
karakterisert ved at det i det vesentlige består av en effektiv konsentrasjon av et farmasøytisk akseptabelt fluoridsalt, et farmasøytisk akseptabelt sinksalt, et buffermiddel og en egnet munnhygienisk bærer, og ved at det har en pH-verdi fra ca. 3,5 til 6,0.
2. Preparat ifølge krav 1,
karakterisert ved at kloridionet er til stede i en konsentrasjon fra ca. 0,0025 til ca. 4,0 vekt-%.
