Description
Den foreliggende oppfinnelse vedrører derivater av ecteinascidinene, særlig ecteinascidin 736 (ET-736), farmasøytiske preparater som inneholder dem, og deres anvendelse som antitumorforbindelser.
Oppfinnelsens bakgrunn
Ecteinascidinene er svært sterke antitumormidler isolert fra havkappedyret Ecteinascidia turbinata. Flere ecteinascidiner er blitt rapportert tidligere i patent- og vitenskapslitteraturen. Se f.eks.: US patentskrift nr. 5 256 663 som beskriver farmasøytiske preparater som omfatter stoff ekstrahert fra det tropiske, marine, virvelløse dyr Ecteinascidia turbinata, og der betegnet som ecteinascidiner, og anvendelse av slike preparater som antibakterielle, antivirale og/eller antitumormidler hos pattedyr.
US patentskrift nr. 5 089 273 som beskriver nye preparater av stoff ekstrahert fra det tropiske, marine, virvelløse dyr Ecteinascidia turbinata, og der betegnet som ecteinascidinene 729, 743, 745, 759A, 759B og 770. Disse forbindelsene er anvendbare som antibakterielle og/eller antitumormidler hos pattedyr.
US patentskrift nr. 5 149 804 som beskriver ecteinascidinene 722 og 736 (Et 722 og 736) isolert fra det karibiske kappedyr Ecteinascidia turbinata og deres strukturer. Et 722 og 736 beskytter mus in vivo ved svært lave konsentrasjoner mot P388-lymfom, B16-melanom og Lewis lungekarsinom.

I 2000 ble det rapportert en halvsyntetisk prosess for dannelsen av ecteinascidinforbindelser og beslektede strukturer, slik som ftalascidin, ved å starte fra naturlige bis(tetrahydroisokinolin)alkaloider, slik som saframycin- og safracinantibiotikaene som er tilgjengelige fra forskjellige dyrkningsvæsker. I WO 9209607 kreves ecteinascidin 736 samt ecteinascidin 722 med hydrogen i stedet for metyl på nitrogenatomet som er felles for ringene C og D i ecteinascidin 736, og O-metylecteinascidin 736 med metoksy i stedet for hydroksy på ring C i ecteinascidin 736.
Til tross for de positive resultatene oppnådd ved kliniske applikasjoner i kjemoterapi, er letingen innenfor området ecteinascidinforbindelser fortsatt åpen for identifiseringen av nye forbindelser med optimale cytotoksisitets- og selektivitetsegenskaper mot tumoren, og med en redusert systemisk toksisitet og forbedrede farmakokinetikkegenskaper.

Disse forbindelsene har høyere aktivitet, et bredere terapeutisk vindu og forbedrede farmakokinetikkegenskaper. Foretrukne substituenter er metoksy, metyl, hydroksy, benzyloksy og fluor.
Forbindelser hvor R3er en ester eller en eter, er blant de foretrukne forbindelser. Generelt har de forbedrede toksikologiegenskaper og gir således et bredere terapeutisk vindu. Av disse er forbindelser med en ester eller et karbonat i denne stillingen mest foretrukket, og særlig karbonater. Estere med voluminøse grupper (lange alifatiske eller aromatiske rester) gir bedre resultater. Blant karbonatene er tert.-butyloksykarbonyl (tBOC) og vinyloksykarbonyl (VOC) de mest foretrukne substituenter for disse stillingene.
Forbindelser hvor R5ikke er hydrogen, er en annen klasse av foretrukne forbindelser. Se f.eks. forbindelsene 37 til 44. Disse forbindelsene er tilbøyelige til å være mindre aktive (cytotoksiske), men har lavere toksisitet og forbedrede farmakokinetikkegenskaper. Når R5ikke er hydrogen, genereres et kiralt sentrum, og vi har funnet at det er forskjell i aktivitet mellom diastereoisomerene.
Forbindelser hvor R2er en ester eller en eter, er også foretrukne forbindelser. Generelt har de forbedrede toksikologiegenskaper og gir således et bredere terapeutisk vindu. Av disse er forbindelser med en ester eller et karbonat i denne stillingen mest foretrukket, og særlig karbonater. Estere med voluminøse grupper (lange alifatiske eller aromatiske rester) gir bedre resultater. Blant karbonatene er tert.-butyloksykarbonyl (tBOC) og vinyloksykarbonyl (VOC) de mest foretrukne substituenter for disse stillingene.
Det finnes forbindelser som har gode ADME-egenskaper (absorpsjon-fordeling-metabolisme-utskillelse), noe som er god indikasjon for farmakokinetikk.
Som nevnt ovenfor, har forbindelser ifølge foreliggende oppfinnelse, fortrinnsvis de med voluminøse substituerte grupper, et godt terapeutisk vindu, og forestringen av fenolene med forskjellige syrer og karbonater resulterer i en generell forbedring av de farmasøytiske egenskapene: det er en signifikant reduksjon i hepatocytt-toksisitet, og også en god profil for legemiddel-legemiddel-interaksjoner ettersom disse derivatene ikke oppviser cytokrominhibering og dessuten har høyere metabolsk stabilitet.
Flere aktive antitumorforbindelser er blitt fremstilt, og det antas at mange flere forbindelser kan dannes i overensstemmelse med læren i den foreliggende beskrivelse.
Antitumoraktiviteter av disse forbindelsene omfatter leukemier, lungekreft, tykktarmskreft, nyrekreft, prostatakreft, eggstokkreft, brystkreft, sarkomer og melanomer.
En annen spesielt foretrukket utførelsesform av den foreliggende oppfinnelse er farmasøytiske preparater som kan anvendes som antitumormidler, og som inneholder som aktiv bestanddel en forbindelse eller forbindelser ifølge oppfinnelsen, samt fremgangsmåtene for deres fremstilling.
Eksempler på farmasøytiske preparater omfatter ethvert fast stoff (tabletter, piller, kapsler, granulater, etc.) eller enhver væske (oppløsninger, suspensjoner eller emulsjoner) med egnede sammensetninger for oral, topisk eller parenteral administrering.
Administrering av forbindelsene eller preparatene ifølge foreliggende oppfinnelse kan skje ved hjelp av hvilken som helst egnet metode, slik som intravenøs innsprøytning, oralt preparat, intraperitonealt og intravenøst preparat.
Forbindelsene og preparatene ifølge denne oppfinnelsen kan anvendes sammen med andre legemidler for å tilveiebringe en kombinasjonsterapi. Som illustrert i reaksjonsskjema 1, kan mellomprodukt 36 omdannes til ET-736 (eller et substituert derivat) i to trinn.
Det første trinnet for fremstilling av de foretrukne forbindelsene av type I ifølge foreliggende oppfinnelse fra forbindelse 36 er innføringen av tetrahydro-p-karbolin-enheten ved omsetning med det tilsvarende primære eller sekundære amin.
Det andre trinnet er omdannelsen av CN-gruppen til en OH-gruppe ved omsetning med sølvnitrat i ACN/H20.
Det er også mulig å oppnå nye derivater med forskjellige substituentgrupper (-OR4, 18-stilling og =NR5) gjennom en acylerings- eller alkyleringsreaksjon fra de foretrukne forbindelser I. I alle disse tilfellene er Ri og R2i utgangsmaterialet et hydrogenatom. Fra det samme mellomprodukt og gjennom en alkyleringsreaksjon med allylbromid eller en acyleringsreaksjon med vinylklorformiat kan det oppnås N- og O-allylerte og N- og O-vinylderivater. Alle disse forbindelsene ved omsetning med sølvnitrat fører til sluttproduktene hvor CN-gruppen er omdannet til en OH-gruppe.
Som fagfolk innen teknikken lett vil forstå, kan reaksjonsskjemaet som er beskrevet her, modifiseres ved anvendelse av et bredt spekter av substituerte, primære aminer for å fremstille en serie av substituerte ecteinascidin 736-derivater for å danne forbindelser som er en del av denne oppfinnelsen.
Særlig kan reaksjonsbetingelsene varieres til å passe andre kombinasjoner av substituentgruppene i tetrahydro-p-karbolin-enheten.
Den foretrukne fremgangsmåte for fremstilling av ecteinascidin 694 og beslektede forbindelser med forskjellige substituenter i 5- og 18-stillingen (-OR6og -OR7) er beskrevet nedenunder i det følgende reaksjonsskjema.
I reaksjonsskjema 2 gjør hydrolysen av acetylgruppen i C-5 under basiske betingelser det mulig å fremstille mellomproduktet med hydroksylgruppen i denne stillingen. Fra denne forbindelsen og ved hjelp av en acyleringsreaksjon med anhydrider, syreklorider eller karboksylsyrer fremstilles nye derivater som er mono-O-substituerte, og mono- og di-O-substituerte (i C-5 og C-18). Reaksjonen for omdannelse av CN-gruppen til OH utføres ved de klassiske betingelsene (sølvnitrat i CH3CN/H20). På den annen side kan Et-694 fås fra Et-736 gjennom hydrolysen av acetylgruppen i C-5 med KOH/MeOH.

FORSØKSDEL
Reaksjonsskjema 1
Metode 1. Til en løsning av 1 ekvivalent utgangsmateriale i eddiksyre (5,33 E-5M) under argon ved romtemperatur tilsettes 3,5 ekvivalenter tryptamin. Reaksjonsblandingen ble omrørt i løpet av 24 timer, og eddiksyren ble avdampet. En vandig, mettet oppløsning av NaHC03ble tilsatt, og blandingen ble ekstrahert med CH2CI2, og de organiske lagene ble tørket over Na2S04. Hurtigkromatografi gir rene forbindelser.
Metode 2. Til en løsning av 1 ekvivalent av forbindelse 1 i CH2CI2(0,032 M) under argon ved romtemperatur ble det tilsatt 2 ekvivalenter Et3N og 2 ekvivalenter av buty ry I kloridet eller Boc-anhydridet (3 ekvivalenter) eller vinylklorformiat. Reaksjonen ble fulgt ved hjelp av TLC og stanset med en vandig, mettet oppløsning av NaHC03, reaksjonsblandingen ble ekstrahert med CH2CI2og de organiske lagene tørket over Na2S04. Metode 3. Til en oppløsning av 1 ekvivalent av forbindelse 1 i DMF (0,032 M) under argon ved romtemperatur ble det tilsatt 3 ekvivalenter Cs2C03og 3 ekvivalenter av allylbromidet. Reaksjonen ble fulgt ved hjelp av TLC og stanset med en vandig, mettet oppløsning av NaHC03, reaksjonsblandingen ble ekstrahert med CH2CI2og de organiske lagene tørket over Na2S04. Hurtigkromatografi giren blanding av to rene forbindelser: forbindelse 24 (ET-736-CN-AII) og forbindelse 25 (ET-736-CN-diAII).
Metode 4. Til en oppløsning av 1 ekvivalent utgangsmateriale i CH3CN/H20 3:2 (0,009 M) ble det tilsatt 30 ekvivalenter AgN03. Etter 24 timer ble reaksjonen stanset med en blanding i forholdet 1:1 av mettede oppløsninger av koksalt og NaHC03, det ble omrørt i 10 minutter og fortynnet og ekstrahert med CH2CI2. Det organiske lag ble tørket med Na2S04. Kromatografi gir rene forbindelser.

BIOANALYSER FOR ANTITUMORSKREENING
Sluttresultatet av disse analysene er å avbryte veksten av en " in vitro" tumorcellekultur ved hjelp av en vedvarende eksponering av cellene for prøven som skal testes.
Cellelinjer
Inhibering av cellevekst ved hjelp av kolorimetrisk analyse
En kolorimetrisk analysetype under anvendelse av reaksjon med svovel-rhodamin B (SRB), er blitt tilpasset for en kvantitativ måling av cellevekst og -levedyktighet [ved å følge teknikken beskrevet av Philip Skehan et al. (1990), New colorimetric cytotoxicity assay for anticancer drug screening, J. Nati. Cancer Inst., 82:1107-1112].
Denne analyseformen gjør bruk av 96-brønners cellekulturmikroplater med 9 mm i diameter (Faircloth, 1988; Mosmann, 1983). De fleste cellelinjene erholdes fra American Type Culture Collection (ATCC), avledet fra forskjellige humankrefttyper.
Celler opprettholdes i RPMI 1640, 10% FBS, supplert med 0,1 g/l penicillin og 0,1 g/l streptomycinsulfat, og inkuberes så ved 37 °C, 5% C02og 98% fuktighet. For forsøkene ble celler innhøstet fra subsammenflytende kulturer under anvendelse av trypsin og på nytt oppslemmet i friskt medium før utplating.
Celler inokuleres i 96-brønners mikrotiterplater, ved 5 x IO3 celler pr. brønn, i aliquoter å 195 ul medium, og de får feste seg til plateoverflaten ved dyrking i legemiddel-fritt medium i 18 timer. Deretter tilsettes prøver i aliquoter å 5 ul i et område fra 10 til IO"<8>ug/ml, oppløst i DMSO/EtOH/PBS (0,5:0,5:99). Etter 48 timers eksponering måles antitumoreffekten ved hjelp av SRB-metodikken: celler festes ved å tilsette 50 ul kald 50 %
(vekt/volum) trikloreddiksyre (TCA) og inkubasjon i 60 minutter ved 4 °C. Plater vaskes med avionisert vann og tørkes. 100 ul SRB-oppløsning (0,4 % (vekt/volum) i 1% eddiksyre) tilsettes til hver mikrotiterbrønn, og det inkuberes i 10 minutter ved romtemperatur. Ubundet SRB fjernes ved vasking med 1 % eddiksyre. Plater lufttørkes, og bundet farge
oppløseliggjøres med Tris-buffer. Optiske tettheter avleses på en automatisert spektrofotometrisk plateavleser ved en eneste bølgelengde på 490 nm.
Verdiene for gjennomsnitt +/- SD av data fra brønner in triplo beregnes. Noen parametre for cellulære responser kan beregnes: GI = vekstinhibering, TGI = total vekstinhibering (cytostatisk effekt) og LC = celledreping (cytotoksisk effekt).
Erholdte resultater kan forutsi nyttigheten av et bestemt legemiddel som en potensiell kreftbehandling. For denne teknikken velges forbindelser som oppviser GI50-verdier som er lavere enn 10 ug/ml, for å fortsette med videre studier. GI50-data gjør det mulig å forutsi at et legemiddel ikke bare kan være cytostatisk, men at det også kan ha et potensial når det gjelder tumorreduksjon.

Toksisitet ble fastlagt ved hjelp av metodene rapportert i Toxicology in Vitro, 15 (2001) 571-577, J. Luber Narod et al.: "Evaluation of the use of in vitro methodologies as tools for screening new compounds for potential in vivo toxicity".
Metoder
For å fastlegge cytotoksisiteten til legemidlene overfor normale celler, brukte vi 96-brønners plater utplatet ved en tetthet på 5 000 celler pr. brønn (bortsett fra FDC-P1 som ble platet ut ved 12 000 celler pr. brønn) med normale cellelinjer (ATCC, tabell 1) opprettholdt ifølge retningslinjene til ATCC: AML-12, normale museleverceller; NRK-52E, normale rottenyreceller; L8, normale rotteskjelettmuskelceller; FDC-P1, normale musemyelostamceller; og H9c2 (2-1), normale rottehjertemuskelceller. Cellene i hver plate fikk bunnfelles over natten før tilsetning av testlegemidlet. I tillegg ble primære, neuronale kulturer fremstilt fra embryonal (dag e-17) helhjerne (forhjerne og hjernestamme) og ryggmarg ved å anvende etablerte metoder (Federoff og Richardson, 1997).
Til hver brønn (100 pl medium) ble det tilsatt 10 pl legemiddel i medium ved varierende konsentrasjoner (1x10-10-0,01 mg/ml sluttkonsentrasjon), og det ble inkubert videre over natten ved 37 °C med 5% C02. Etter 24 timer ble de følgende analyser utført. Alle forsøk ble gjentatt minst tre ganger og ble analysert in duplo.
1. MTS-analyse (CellTiter 96 vandig) ble utført i henhold til produsentens (Promega) retningslinjer (for alle celletyper). Cellelevedyktighet (mitokondrien aktivitet) bestemmes via enzymatisk omdannelse av formazan-substratet.
In vitro evaluering av forbindelsene med hensyn på ADME-TOX-profil Fordelinaskoeffisient (log D)
Fordelingskoeffisienten til en kjemisk forbindelse gir et termodynamisk mål for dens hydrofilisitets-lipofilisitets-balanse. Lipofilisitet er en viktig strukturell faktor som påvirker den farmakokinetiske og farmakodynamiske oppførsel til forbindelser. Fordelingskoeffisienten mellom vann eller buffer og 1-oktanol er det mest utbredt brukte mål for lipofilisitet hos kjemiske forbindelser.
Målingen av fordelingskoeffisient ble evaluert basert på en miniatyrisert risteflaskeprosedyre. Buffer (Dulbeccos PBS, pH 7,40) ble brukt som vannfasen. Den testede forbindelse ble oppløst i DMSO ved en konsentrasjon på 100 uM. Slutt-DMSO-konsentrasjonen (1%) under oktanol-buffer-fordelingen er svært lav for å unngå systemavhengig feil ved fordelingen. Mengden av forbindelse i bufferfasen ble bestemt ved hjelp av HPLC med fotodiode-grupperingspåvisning etter en ekvilibreringsfase på 60 minutter. Mengden av forbindelse i oktanolfasen beregnes ved å trekke mengden av forbindelse i buffer fra den totale mengde av forbindelse som bestemmes ut fra en kalibreringsprøve.
Log D beregnes som Logi0av mengden av forbindelse i oktanolfasen dividert med mengden av forbindelse i bufferfasen. Det effektive område for log D-mikroanalysen er omtrent -0,5 til +4,5.
In vitro tarmabsorpsionsanalvser
Tarmepitelpermeabiliteten er et kritisk kjennetegn som bestemmer hastigheten og omfanget av humanabsorpsjon og til sist biotilgjengeligheten av en legemiddelkandidat. Caco-2-permeabilitetsanalyse muliggjør en hurtig fastleggelse av membranpermeabilitet og hjelper således til å rangere forbindelser etter deres absorpsjonspotensial.
Caco-2-cellelinjen er en human tykktarmsadenokarsinomcellelinje som differensierer i kultur og setter sammen epitelforet i tynntarmen hos mennesket. Den er blitt mye brukt som en in vitro tarmepitelmodell for legemiddeltransport- og permeabilitets-skreening av oppdagede forbindelser.
De tilsynelatende permeabilitetskoeffisienter (Papp) ble bestemt i den spiss-til-basolaterale (A-til-B) retning over cellemonolagene (TC-7-underklon av Caco-2) dyrket på polykarbonatmembranfiltere. Forbindelser ble testet ved 50 uM med en slutt-DMSO-konsentrasjon på 1%. Prøver ble analysert ved hjelp av HPLC-MS eller HPLC-MS/MS.

To referanseforbindelser, propranolol (svært permeabel) og ranitidin (lite permeabel) testes i hver analyse som kontroller. Forbindelser med Papp som er lik eller større enn 20 x IO"<6>cm/sek, vil kunne anses som svært permeable og vil sannsynligvis være "ikke permeabilitetsbegrenset". Forbindelser med Papp som er mindre enn 5 x IO"<6>cm/sek, anses som lite permeable og vil sannsynligvis være "permeabilitetsbegrenset". Forbindelser med Papp som er større enn 5 x IO"<6>, men mindre enn 20 x IO"<6>cm/sek, anses for å ha middels permeabilitet.
Metabolisme
Hepatisk metabolisme er en primærdeterminant for farmakokinetisk oppførsel, og hurtig førstepasseringsmetabolisme er en hovedårsak til lav biotilgjengelighet. Den metabolske stabilitet ble bestemt ved å anvende human lever S9-homogenat som inkluderer både mikrosom- og cytosolenzymaktiviteter. Testforbindelsen ble fortynnet i metanol (0,625 %) og acetonitril (0,625 %) ved konsentrasjonen 1 uM, og det ble inkubert i den humane leverblanding (protein = 1 mg/ml) i 60 minutter ved 37 °C. Topparealer som tilsvarer alle analytter (metabolske produkter), ble bestemt ved hjelp av HPLC-MS/MS. Arealer ble målt, og forhold mellom topparealer for analytter og de til intern standard for hver analytt ble bestemt. Forholdet mellom forløperforbindelse som er tilbake etter 60 minutter, og mengden som er tilbake på tid null, uttrykt som prosent, er rapportert som metabolsk stabilitet. Inhibering av cvtokromer P450 (CYP450)
Cytokromene P450 er en gruppe av beslektede enzymer som primært befinner seg i leveren og som er ansvarlig for metabolismen av legemidler. Inhiberingen av disse CYP ved hjelp av legemidler står i forhold til legemiddel-legemiddel-interaksjoner og -toksisiteter.
CYP3A4 er den vanligste formen av CYP3A-enzymene som finnes hos voksne, og er den formen som er implisert i de fleste legemiddelinteraksjoner. CYP2D6 metaboliserer mer enn 25% av de klinisk nyttige medisineringer.
For CYP2D6-inhiberingsanalyse testes forbindelsen ved 10 uM in duplo med en 0,25% sluttkonsentrasjon av både metanol og acetonitril i nærvær av det fluorescerende substrat AMMC (3-[2-(N,N-dietyl-N-metylammonium)etyl]-7-metoksy-4-metylkumarin) ved konsentrasjonen på 1,5 uM). Omdannelsen av AMMC til AHMC (3-[2-(N,N-dietyl-N-metylammonium)etyl]-7-hydroksy-4-metylkumarin) bestemmes spektrofluorimetrisk etter inkubasjon med enzymet i 450 minutter ved 37 °C.
For CYP3A4-inhiberingsanalyse ble forbindelsen testet ved 10 uM in duplo med en 0,25% sluttkonsentrasjon av både metanol og acetonitril i nærvær av det fluorescerende substrat BFC (50 uM). Omdannelsen av BFC (7-benzyloksy-4-trifluormetylkumarin) til HFC (7-hydroksy-4-trifluormetylkumarin) ble bestemt ved spektrofluorimetri etter inkubasjon med enzymet i 30 minutter ved 37 °C.
For begge analysene trekkes fluorescensstyrken målt ved t = 0, fra den som er målt etter den passende inkubasjonstid. Forholdet mellom signal og støy beregnes ved å sammenligne fluorescensen i inkubasjoner som inneholder testforbindelsen, med kontrollprøvene som inneholder den samme oppløsningsmiddelbærer. Prosenten av kontrollaktivitet beregnes og rapporteres som prosentvis inhibering.
In vitro sikkerhetsfastleqqelse. Cellelevedyktiqhet
Det toksiske potensial til forbindelser ble undersøkt in vitro ved å anvende primære humanhepatocytter (HEPG2). Forbindelsene ble testet ved 30 uM in duplo med en slutt-DMSO-konsentrasjon på 1%. Etter inkubasjon i 24 timer ved 37 °C ble cellelevedyktigheten bestemt ved hjelp av omdannelsen av oksidert alarmarBlue (resazurin) til redusert alarmarBlue (resorufin). Klorpromazin ble brukt som referanseforbindelse. Resultatene er uttrykt som en prosent av inhibering av kontrollverdier.
