Description
Foreliggende oppfinnelse angår substituerte 1,3,4-oksadiazolforbindelser, fremgangsmåten for å redusere nivåer av tumornekrosefaktor a og å øke cAMP-nivåene og å behandle inflammatorisk og autoimmune sykdommer og cancer i pattedyr ved administrering av disse forbindelser, samt farmasøyt-iske preparater inneholdende slike derivater.
Tumornekrosefaktor a, TNFa, er et cytokin som frigis primært av celler i immunsystemene som respons til visse immu-nostimulatorer. Administrert til dyr eller mennesker forår-saker den inflammasjon, feber, kardiovaskulære effekter, hemorhage, koagulering, kakheksi samt akuttfaseresponser tilsvarende de som ses under akutte infeksjoner, inflammatoriske sykdommer og sjokktilstander. Eksessiv eller ikke-regulert TNFa-produksjon har vært implisert i et antall sykdomstilstander. Disse inkluderer endotoksemi og/eller toksisk sjokksyndrom (Tracey et al., "Nature" 330, 662-664
(1987) og Hinshaw et al., "Cic. Shock" 30, 279-292 (1990)), reumatoid artritt, inflammatorisk tarmsykdom, kakeksi (De-zube et al., "Lancet", 335 (8690), 662 (1990)), og lupus. TNFa-konsentrasjoner ut over 12.000 pg/ml er påvist i pul-monæraspirater fra Adult Respiratorisk Distress Syndrom (ARDS) pasienter (Millar, et al., "Lancet" 2(8665), 712-714
(1989)). Systemisk infusjon av rekombinant TNFa resulterte i endringer som typisk ses ved ARDS (Ferrai-Baliviera, et al., "Arch. Surg.", 124(12), 1400-1405 (1989)).
TNFa synes å være involvert i et antall benresorpsjonssyk-dommer, inkludert artritt. Aktivert ville leukocyttene gi benresorpsjon. TNFa bidrar tilsynelatende til denne mekanisme (Bertolini et al., "Nature" 319, 516-518 (1986) og Johnson et al., "Endocrinology" 124(3). 1424-1427 (1989)). TNFa er også vist å stimulere benresorpsjon og inhibere bendannelse in vitro og in vivo ved stimulering av osteo-klastdannelse og aktivering kombinert med inhibering av osteoblastfunksjoner. En annen forbindelse med sykdom er assosiasjonen mellom produksjonen av TNFa av tumor- eller vertsvev og malignansitetsassosiert hyperkalcemi ("Calci. Tissue Int." (US) 46(suppl), S3-10 (1990)). I poding-mot-vertsreaksjoner er økede serum-TNFa-nivåer assosiert med en vesentlig komplikasjon etter akutt allogenisk ben-marg transplantasjon (Holler et al., "Blood", 75(4), 1011-1016 (1990)).
Validering av TNFa-inhibering som en klinisk terapi har vært påvist ved den terapeutiske anvendelse av TNFa-antistoffer og oppløselige TNFa-reseptorer. TNFa-blokkering med monoklonale anti-TNFa-antistoffer har vist seg å være fordelaktig ved reumatoid artritt (Elliot et al., "Int. J. Pharmac." 1995 17(2), 141-145). Høye nivåer av TNFa er assosiert med Chrons sykdom (von Dullemen, et al., "Gastroen-terology", 1995, 109(1), 129-135) der behandling med opplø-selig TNFa-reseptor ga kliniske fordeler.
Cerebral malaria er et letalt hyperakutt neurologisk syndrom assosiert med høye blodnivåer av TNFa og den alvor-ligste komplikasjon som opptrer hos rna1ariapasienter. Eleverte nivåer av serum-TNF<X korrelerte direkte med alvoret av sykdommen og prognosene hos pasienter med akutte mala-riaangrep (Grau, et al., "N. Engl. J. Med." 320(24), 1586-1591 (1989)).
TNFa spiller en rolle på området kroniske pulmonare inflammatoriske sykdommer. Avsetning av silikapartikler fører til silikose, en sykdom med progressiv respiratorisk svikt forårsaket av en fibrotisk reaksjon. Antistoffer mot TNFa blokkerte fullstendig den silikainduserte lungefibrose hos mus (Pignet, et al., "Nature", 344, 245-247 (1990)). Høye nivåer av TNFa-produksjon (i serum og i isolerte makrofager) er påvist i dyremodeller av silika- og asbestindusert fibrose (Bissonnette, et al., "Inflammation" 13(3), 329-339
(1989) ). Alveolære makrofager fra pulmonar sarkoidosepa-sienter er også funnet å spontant frigi massive mengder TNFa sammenlignet med makrofager fra normale donorer (Baughman, et al., "J. Lab. Clin. Med." 115(1), 36-42
(1990) ).
Forhøyede nivåer av TNFa er implisert i reperfusjonsskade, den inflammatoriske respons som følger reperfusjon og er en hovedårsak til vevskade etter blodstrømstap (Vedder, et al., "PNAS" 87, 2643-2646 (1990)). TNFa endrer også egen-skapene til endoteliale celler og har forskjellige prokoa-gulantaktiviteter, for eksempel å produsere en økning i vevfaktor prokoagulantaktiviteten, å dempe antikoagulant protein C-veien og å regulere ned ekspresjonen av trombomo-dulin (Sherry, et al., "J. Cell. Biol." 107, 1269-1277
(1988)). TNFa har proinflammatorisk aktiviteter som sammen med dens tidlige produksjon (under de første trinn av et inflammatorisk evenement), gjør den til en sannsynlig me-diator for en vevskade i flere betydelige lidelser inkludert, men ikke begrenset til myokardialt infarkt, slag og sirkulatorisk sjokk. TNFa-indusert ekspresjon av adhe-sjonsmolekyler (ICAM) eller endoteliale leukocyttad-hesjonsmolekyler (ELAM) på endoteliale celler kan være spesielt viktig (Munro, et al., "Am. J. Path" 135(1), 121-132
(1989)) .
Det er rapportert at TNFa er en potent aktivator for retrovirus replikasjon inkludert aktivering av HIV-1. (Duh, et al., "Proe. Nat. Acad. Sei." 86, 5974-5978 (1989); Poll, et al., "Proe. Nat. Acad. Sei." 87, 782-785 (1990); Monto, et al., "Bood" 79, 2670 (1990); Clouse, et al., "J. Immunol." 142, 431-438 (1989); Poll, et al., "AIDS Res. Hum. Retrovirus", 191-197 (1992)). I det minste tre typer eller stammer av HIV (dvs. HIV-1, HIV-2 og HIV-3) er identifisert. Som en konsekvens av HIV-infeksjon blir T-cellemediert immunitet forringet og infekterte individer mani-festerer alvorlige opportunistiske infeksjoner og/eller uvanlige neoplasmer. HIV-inngang i T-lymfocyttene krever T-lymfocyttaktivering. Andre viruser som HIV-1, HIV-2 infek-terer T-lymfocytter etter T-celleaktivering. Denne virus-proteinekspresjon og/eller replikasjon medieres eller opprettholdes av denne T-celleaktivering. Når først en aktivert T-lymfocytt er infektert med HIV, må T-lymfocytten fortsette å opprettholdes i en aktivert tilstand for å til-late HIV-genekspresjon og/eller HIV-replikasjon. Cytokiner og spesielt THFa er implisert i aktivert T-cellemediert HIV-proteinekspresjon og/eller -virusreplikasjon ved å spille en rolle i vev opprettholdelsen i T-lymfocytt-aktivering. Derfor understøtter interferens cytokinaktivi-tet som prevensjon eller inhibering av cytokinproduksjonen, og særlig TNFa, i et HIV-infektert individ en begrensning av opprettholdelsen av T-lymfocytt forårsaket av HIV-infeksjon.
Monocytter, makrofager og relaterte celler som kupffer- og glialceller, er også implisert i opprettholdelsen av HIV-inf eks j onen. Disse celler er på samme måte som T-celler, mål for viral replikasjon, og viral replikasjon er avhengig av aktiveringstilstanden for cellene. Cytokiner som TNF a er påvist å aktivere HIV-replikasjon i monocytter og/eller makrofager (Poli, et al., "Pre. Nati. Acad. Sei., 87, 782-784 (1990)), derfor understøtter prevensjon eller inhibering av cytokin-produksjon eller -aktivitet en begrensning av HIV-progresjon for T-celler. Ytterligere studier har identifisert TNFa som en felles faktor ved aktivering av HIV in vitro og har gitt en klar virkningsmekanisme via et nukleært regulatorisk protein som finnes i cellecytoplasma (Osborn, et al., "PNAS" 86 2336-2340). Dette funn antyder at en reduksjon av TNFa-syntesen kan ha en antiviral effekt i HIV-inf eks joner ved å redusere transkripsjonen og således vi-rusproduksj onen.
AIDS viralreplikasjon av latent HIV i T-celle og makro-faglinjer kan induseres av TNFa (Folks, et al., "PNAS" 86, 2365-2368 (1989)). En molekylær mekanisme for virusindusert aktivitet antydes av evnen hos TNFa til å aktivere et genregulatorisk protein, NFKB, som finnes i cellecytoplasma, og som fremmer HIV-replikasjon ved å binde til en viral regulatorisk gensekvens (LTR) (Osborn, et al., "PNAS" 86, 2336-2340 (1989)). TNFa i AIDS-assosiert 2336-2340 (1989)). TNFa i AIDS-assosiert kakheksi antydes av eleverte serum TNFa og høye nivåer av spontan TNFa-produksjon i perifere blodmonocytter fra pasienter (Wright, et al., "J. Immunol. 141(1), 99-104 (1988)). TNFa har vært implisert i tallrike roller med andre virale infeksjoner som cytomegaliaviruset (CMV), influensaviruset, adenovirus samt herpesfamilien av virus av samme grunner som angitt ovenfor.
Den nukleære faktor KB, NFKB, er en pleiotropisk transkripsjonen aktivator (Lenardo, et al., "Cell" 1989, 58, 227-29). NFKB har vært implisert som transkripsjonen aktivator i et antall sykdommer og inflammatoriske tilstander og antas å regulere cytokinnivået inkludert, men ikke begrenset til TNFa og aktiv HIV-transkripsjon (Dbaibo, et al., <*>J. Biol. Chem." 1993, 17762-66; Duh, et al., "Proe. Nati. Acad. Sei." 1989, 86, 5974-78; Bachelerrie, et al., "Nature" 1991, 350, 709-12; Boswas, et al., "J. Acquired Immune Deficiency Syndrome" 1993, 6, 778-786; Suzuki, et al., "Biochem. And Biophys. Res. Comm." 1993, 193, 277-83; Suzuki, et al., "Biochem. And Biophys. Res Comm." 1992, 189, 1709-15; Suzuki, et al., "Biochem. Mol. Bio. Int." 1993, 31(4), 693-700; Shakhov, et al., "Proe. Nati. Acad. Sei." USA 1990, 171, 35-47; og Staal, et al., "Proe. Nati. Acad. Sei." USA 1990, 87, 9943-47). Således ville det være nyttig å inhibere NFKB-aktivering, nukleær translasjon eller binding for å regulere transkripsjonen av et eller flere cytokingener og via denne modulering og andre meka-nismer å være nyttig med henblikk å inhibere et antall sykdomstilstander .
Mange cellefunksjoner medieres av nivåer av adenosin 3',5'-cyklisk monofosfat (cAMP). Slike cellulære funksjoner kan bidra til inflammatoriske tilstander og sykdommer inkludert astma, inflammasjon og andre tilstander (Lowe and Cheng, "Drugs of the Future", 17(9), 799-807, 1992). Det er vist at en forhøyning i cAMP i inflammatoriske leukocytter inhiberer deres aktivering og den etterfølgende frigivning av inflammatoriske mediatorer inkludert TNFa og NFKB. Økede nivåer av cAMP fører også til relaksering av den glatte luftveismuskel.
Den primære cellulære muskel for inaktivering av cAMP er nedbrytning av cAMP av en familie av isoenzymer som angis som sykliske nukleotidfosfodiesteraser (PDE) (Beavo and Reitsnyder, "Trends in Pharm.", 11, 150-155, 1990). Det er ti kjente medlemmer av familien av PDE'er. Det er godt do-kumentert at inhiberingen av PDE type IV (PDE 4)-enzymet er særlig effektivt både ved inhibering av inflammatorisk me-dioatorfrigivning og relaksering av den glatte luftvei-muskel (Verghese, et al., "Journal of Pharmacology and Ex-perimental Therapeutics", 272(3), 1313-1320, 1995).
En reduksjon av TNFa-nivåene og/eller økning av cAMP-nivåene utgjør således en verdifull terapeutisk strategi for behandling av mange inflammatoriske, infeksiøse, immu-nologiske og malignante sykdommer. Disse inkluderer, men er ikke begrenset til: septisk sjokk, sepsis, endotoksisk sjokk, hemodynamisk sjokk og sepsis syndrom, postiskemisk reperfusjonsskade, malaria, mycobakteriell infeksjon, me-ningitt, psoriasis og andre dermale sykdommer, kongestive hjertesvikt, fibrotisk sykdom, kakheksi, podingsawising, cancer, tumorvekst, uønsket angiogenese, autoimmunsykdom, opportunistiske infeksjoner i AIDS, reumatoid artritt, reumatoid spondylitt, osteoartritt, andre artrittiske tilstander, inflammatorisk tarmsykdom, Crohns sykdom, ulcerativ kolitt, multippel sklerose, systemisk lupus erytrematose, ENL i leprosi, strålingsskade og hypertoksisk alveolær ska-de. Tidligere forsøk rettet mot suppresjon av virkningene av TNFa har rangert fra anvendelse av steroider som deksa-metason og prednisolon til bruken av både polyklonale og monoklonale antistoffer (Beutler, et al., "Science" 234, 470-474 (1985); WO 92/11383).
Angiogenese, prosessen ved utvikling og dannelse av nye blodkar, spiller en viktig rolle i tallrike normale og patologiske fysiologiske evenementer. Angiogenese inntrer som respons på spesifikke signaler og involverer en kompleks prosess som karakteriseres ved infiltrering av den basale lamina på grunn av vaskulære endoteliale celler i respons på angiogenetiske vekstsignaler, migrering av de' endoteliale celler mot signalkilder og etterfølgende proliferering og dannelse av kapillarrøret. Blodstrøm gjennom den nydan-nede kapillar initieres etter at de endoteliale celler kom-mer i kontakt med bg forbindes med en allerede eksisterende kapillar. Angiogenese kreves for tumorvekst ut over en viss størrelse.
Inhibitoriske innflytelser er fremherskende i den naturlig opptredende balansen mellom endogeniske stimulatorer og inhibitorer av angiogenese (Rastinejad, et al., 1989, "Cell" 56:345-355). I de sjeldne tilfeller der neovaskularisering inntrer under normale fysiologiske tilstander som sårhe-ling, organregenerering, embryonisk utvikling og female re-produktive prosesser, er angiogenese stringent regulert og spatialt og temporært avgrenset. Under tilstander med pato-patologisk angiogenese, så som det som karakteriserer fast tumorvekst, mangler disse regulatoriske kontroller.
Ikke-regulert angiogenese blir patologisk og understøtter progresjon av mange neoplastiske og ikke-neoplastiske sykdommer. Et antall alvorlige sykdommer domineres av anormal neovaskularisering, inkludert fast tumorvekst og metastaser, artritt, visse typer øyelidelser og psoriasis (Moses, et al., 1991, "Biotech." 9:630-634; Folkman et al., 1995, "N. Engl. J. Med.", 333-1757-1763; Auerbach, et al., 1985, "J. Microvasc. Res." 29:401-411; Folkman, 1985, "Advances in Cancer Research", red. Klein og Weinhouse, Academic Press, New York, s. 175-203; Patz, 1982, "Am. J. Opthalmol." 94:715-743; og Folkman, et al., 1983, "Science" 221:719-725). I et antall patologiske tilstander bidrar angiogeneseprosessen til sykdomstilstanden. For eksempel antyder signifikante data at veksten av faste tumorer er avhengig av angiogenese (Folkman og Kagsbrun, 1987, "Science" 235:442-447).
Opprettholdelsen av avaskulariteten i cornea, linse og tra-bekulærnettverket er avgjørende for synet så vel som for okkulær fysiologi, se for eksempel oversikter av Waltman, et al., 1978, "Am. J. Opthal." 85:704-710 og Gartner, et al., 1978, "Surv. Ophthal." 22:291,312. Vanligvis er be-handlingen av disse sykdommer, og særlig først når neovaskularisering har oppstått, uttilstrekkelig, og blindhet er ofte resultatet.
En inhibitor for angiogenese kunne ha en betydelig terapeutisk rolle ved å begrense bidragene ved denne prosess til patologisk progresjon av de underliggende sykdomstilstander så vel som å tilveiebringe verdifulle midler for å studere deres etiologi. For eksempel kunne midler som inhiberer tu-morneovaskularisering, spille en betydelig rolle ved å inhibere metastatisk og fast tumorvekst.
Flere typer forbindelser har vært benyttet for å forhindre angiogenese. Taylor, et al., benyttet protamin for å inhibere angiogenese, (Taylor, et al, "Nature" 297:307 (1982)). Toksisiteten hos protamin begrenser den praktiske anvendelse som terapeutikum. Folkman, et al. benyttet heparin og steroider for å kontrollere angiogenese (Folkman, et al., "Science" 221:719 (1983) og US-patent nr. 5,001,116 og 4,994,443). Steroider som tetraahydrokortisol og som mangler gluko- og mineralkortikoid aktivitet, er angioge-niske inhibitorer. Interferon-|3 er også en potent inhibitor for angiogenese indusert av allogeniske miltceller (Sidky, et al., "Cancer Research" 47:5155-5161 (1987)). Human rekombinant interferon-a ble rapportert å være vellykket benyttet ved behandling av pulmonær hemangiomatose, en angio-geneseindusert sykdom (White, et al., "New England J. Med." 320:1197-1200 (1989)).
Andre midler som har vært benyttet for inhibere angiogenese, inkluderer askorbinsyreeter og relaterte forbindelser (Japansk Kokai Tokkyo Koho nr. 58-131978). Sulfatert poly-sakkarid DS 4152 viser også angiogenisk inhibering (Japansk Kokai Tokkyo Koho nr. 63-119500). Et fungalprodukt, fumagillin, er et potent angiostatisk middel in vitro. Forbindelsen er toksisk in vivo, men et syntetisk derivat, AGM 12470, har vært benyttet in vivo for å behandle collagen II artritt. Fumagillin og o-substituerte fumagillinderivater er beskrevet i EP-publikasjoner nr. 0325199A2 og 0357061A1.
I US 5,874,081 beskriver Parish bruken av monoklonale antistoffer for å inhibere angiogenese. I WO 92/12717 beskriver Brem, et al. at visse tetrasykliner og særlig minosyklin, klortetrasyklin, demeklosyklin og lymesyklin er brukbare som inhibitorer for angiogenese. Brem et al. beskriver at minosyklin inhiberer angiogenese i en grad sam-menlignbar med den kombinasjonsterapien av heparin og kor-tison ("Cancer Research", 51, 672-675, 15. januar 1991). Teicher, et al. beskriver at tumorvekst reduseres og antal-let metastaser reduseres når det antiangiogeniske middel metastaser reduseres når det antiangiogeniske middel minosyklin benyttes i forbindelse med cancerkjemoterapi eller - stråleterapi ("Cancer Research", 52, 6702-6704, 1. desember 1992).
Makrofagindusert angiogenese er kjent for å bli stimulert av TNFa. Leibovich et al. rapporterte at TNFa induserer in vivo kapillar bloddannelse i rottekornea og de utviklende kyllingchorioallantoriske membraner ved meget lave doser og antyder at TNFa er en kandidat for å indusere angiogenese i inflammasjon, sårne1ing og tumorvekst ("Nature", 329, 630-632 (1987)).
Alle de forskjellige celletyper i kroppen kan transformeres til benigne eller malignante tumorceller. Det hyppigste tu-morsetet er lungen fulgt av kolorektal, bryst, prostata, blære, bukspyttkjertel og så ovarie. Andre overveiende typer av cancer inkluderer leukemi, sentralnervesystemcance-re, hjernecancer, melanoma, lymfoma, erytroleukemi, uterin-cancer, bencancer og hode- og halscancer.
Cancer blir i dag hovedsakelig behandlet med en eller en kombinasjon av tre typer av terapier: kirurgi, stråling og kjemoterapi. Kirurgi involverer massefjerning av sykt vev. Mens kirurgi enkelte ganger er effektiv med henblikk på å fjerne tumorer som er lokalisert på visse seter (f.eks. bryst, kolon og hud), kan kirurgi ikke benyttes ved behandling av tumorer som er lokalisert i andre områder (f.eks. ryggraden) eller ved behandling av disseminerte neoplastiske tilstander (f.eks. leukemi). Kjemoterapi involverer opp-bryting av cellereplikasjonen eller cellemetabolismen. Kjemoterapi benyttes som oftest ved behandling av leukemi så
vel som bryst-, lunge- og testikkelcancer.
Kjemoterapeutiske midler kalles ofte antineoplastiske midler. Alkyleringsmidlene antas å virke ved å alkylere og fornekte guanin og eventuelt andre baser i DNA og derved stanse celledeling. Typiske alkyleringsmidler inkluderer nitrogensenneper, etyleniminforbindelser, alkylsulfater, cisplatin og tallrike nitroureaer. En mangel ved disse forbindelser er at de ikke bare angriper malignante celler, men også andre celler som deler seg naturlig, som de i ben-margen, huden, gastrointestinal mukose og fetalvev. Antime-tabolitter er typisk reversible eller irreversible enzymin-hibitorer eller forbindelser som ellers interfererer med replikasjon, translasjon eller transkripsjon av nukleinsy-re. Således ville det være å foretrekke å finne mindre toksiske forbindelser for cancerbehandling.
Matriksmetalloproteinase (MMP) inhibering har vært assosiert med diverse aktiviteter inkludert inhibering av TNFa (Mohler et al., "Nature", 370, 218-220 (1994)) og inhiberingen av angiogenese. MMP'er er en familie av utskilte og membranbundne sinkendopeptidaser som spiller en nøkkelrolle i både fysiologisk og patologisk vevnedbrytning (Yu, et al., "Drugs & Aging, 1997, (3): 229-244; Wojtowicz-Praga, et al., "Int. New Drugs", 16:61-75 (1997)). Disse enzymer er i stand til å bryte ned komponenter i den ekstracellulæ-re matriks inkludert fibrillære og ikke-fibrillære kollage-ner, fibronektin, laminin og membranglykoproteiner. Vanligvis er det en delikat balanse mellom celledeling, matriks-syntese, matriksnedbrytning (under kontroll av cytokiner), vekstfaktorer og cellematriksinteraksjoner. Under patologiske tilstander kan imidlertid denne balanse forrykkes. Tilstander og sykdommer assosiert ved uønskede MMP-nivåer inkludere, men er ikke begrenset til, tumormetastaseinva-sjon- og vekst, angiogenese, reumatoid artritt, osteoartritt, osteopenia som osteoporose, periodontitt, gingivitt, Chrons sykdom, inflammatorisk tarmsykdom og korneal epider-mal eller gastrisk ulcerering.
Øket MMP-aktivitet er detektert i et vidt spekter av cance-re (Denis, et al., "Invest. New Drugs", 15: 175-185
(1987)). På samme måte som ved TNFa antas MMP'er å være involvert i de invasive prosesser av angiogenese og tumor-metastase.
Foreliggende oppfinnelse er basert på den oppdagelse at visse klasser av ikke-polypeptidforbindelser som er mer de-taljert beskrevet her, reduserer nivåene av TNFa og/eller inhiberer PDE'er, særlig PDE 4 og/eller inhiberer angiogenese og/eller er nyttige ved behandling av cancer, inflammatoriske og autoimmune sykdommer. For eksempel vil forbindelser som selektivt inhiberer PDE 4 spesifikt, i det minste delvis, inhibere inflammasjon og relaksasjon av den glatte luftveismuskel med et minimum av uønskede bivirknin-ger som kardiovaskulære eller antiplate effekter. Forbindelsene ifølge oppfinnelsen er brukbare ved inhibering av fosfodiesteraser, og særlig PDE 4, og ved behandling av sykdomstilstander som medieres derav.
Forbindelsene som her beskrives, kan inhibere virkningen av NFKB i nukleus og er således brukbar ved behandling av et antall sykdommer inkludert, men ikke begrenset til, reumatoid artritt, reumatoid spondylitt, osteoartritt, andre artrittiske tilstander, septisk sjokk, sepsis, endotoksisk sjokk, poding-mot-vertsykdom, utstøting, inflammatorisk tarmsykdom, Chrons sykdom, ulcerativ kolitt, multiple sklerose, systemisk lupus erytrematose, ENL i leprosi, HIV, AIDS og opportunistiske infeksjoner i AIDS. TNFa- og NFKB-nivåene påvirkes av en resiprok feedbacksløyfe. Som angitt ovenfor, påvirker forbindelsene ifølge oppfinnelsen nivåene både av TNFa og NFKB.

Uttrykket alkyl angir en enverdig mettet eller umettet rett eller forgrenet, cyklisk eller en blanding derav, hydrokarbonkjede inneholdende 1-8 karbonatomer. Representative for slike alkylgrupper er metyl, etyl, propyl, isopropyl, butyl, isobutyl, sec-butyl, tert-butyl, cyklopentyl og cyklopropylmetyl. Alkoksy angir en alkylgruppe som er bundet til resten av molekylet via et eteroksygenatom. Representative for slike alkoksygrupper er metoksy, etoksy, propoksy, isopropoksy, butoksy, isobutoksy, sek-butoksy, tert-butoksy, cykloheksylmetoksy og cyklopentylme-toksy.
Uttrykket cykloalkyl slik det her benyttes, angir en enverdig, cyklisk hydrokarbonkjede som kan være mettet eller umettet. Hvis ikke annet er sagt, kan en slik kjede inne-holde opptil 18 karbonatomer og inkludere mono-, di-, poly-og benzocykloalkylstrukturer. Monocykloalkyl henviser til grupper som har en enkelt ringgruppe. Polycykloalkyl angir hydrokarbonsysterner som inneholder 2 eller flere ringsyste-rner der en eller flere ringkarbonatomer er felles, dvs. en spirostruktur, fusert eller broslått struktur. Benzocykloalkyl angir en monocyklisk alkylgruppe som er fusert til en benzogruppe. Representative for monocykloalkylgrupper er cyklopropyl, cyklobutyl, cyklopentyl, cykloheksyl, cyklo-heptyl, cyklooktyl, cyklononyl, cyklodecyl, cykloundecyl, cyklododecyl, cyklotridecyl, cyklotetradecyl, cyklopentade-cyl, cykloheksadecyl, cykloheptadecyl og cyklooktadecyl. Representative for polycykloalkyl er dekahydronaftalen, spiro[4,5]decyl, bicyklo[2,2,1]heptyl, bicyklo[3,2,1]oktyl, pinanyl, norbornyl og bicyklo[2,2,2]oktyl. Benzocykloalkyl er spesifisert ved tetrahydronaftyl, indanyl og 1,2-benzo-cykloheptanyl. Cykloalkoksy henviser til en cykloalkyl-gruppe som nettopp beskrevet som er en mono-, poly- eller benzocykloalkylstruktur bundet til resten av molekylet via en eteroksygenatom.
En første foretrukken gruppe forbindelser er de med formel I der Y er C=0.
En ytterligere foretrukken gruppe forbindelser er de med formel I der Y er CH2.
Nok en foretrukken gruppe forbindelser er de med formel I der hver av R<1>, R<2>, R3 og R4 uavhengig av de andre er hydrogen, eller metyl.
Nok en foretrukken gruppe forbindelser er de med formel I der en av R<1>, R2, R3 og R<4> er -NHCOCH3, og de gjenværende av R1, R2, R<3> og R<4> er hydrogen.
Nok en foretrukken gruppe forbindelser omfatter de med formel I der en av R1, R<2>, R<3> og R<4> er metyl og de gjenværende av R1, R<2>, R3 og R<4> er hydrogen.
Nok en foretrukken gruppe forbindelser er de med formel I der hver av R<5> og R6 uavhengig av den andre er metoksy, etoksy eller cyklopentoksy.
Nok en foretrukken gruppe forbindelser er de med formel I der R5 er metoksy og R<6> er etoksy.
Forbindelsene med formel I benyttes, under overvåkning av kvalifiserte profesjonelle, for å inhibere de uønskede virkninger av TNFa og PDE 4. Forbindelsene kan også administreres for å behandle cancertilstander, uønsket angiogenese, inflammasjon, hudtilstander osv. Forbindelsene kan administreres oralt, rektalt eller parenteralt, alene eller i kombinasjon med andre terapeutiske midler inkludert anti-biotika, steroider osv. til et pattedyr som trenger slik behandling. Bruken av uttrykkene BDE IV og PDE 4 anses som ekvivalente.
Forbindelsene kan også benyttes topisk ved terapi eller profylakse av topiske sykdomstilstander inkludert, men ikke begrenset til atopisk dermatitt, psoriasis, lupus, virale infeksjoner som de som forårsakes av herpesviruser, eller viral konjunktivitt, psoriasis, cancer osv. PDE 4-inhibering er en foretrukken utførelsesform selv om inhibering av andre fosfodiesteraser også er omfattet.
Forbindelsene kan også benyttes i veterinærterapi på pattedyr andre enn mennesker som trenger prevensjon eller inhibering av TNFa-produksjon eller PDE 4-inhibering. Videre gjelder TNFa-medierte sykdommer for behandling, terapeutisk eller profylaktisk, i dyr som inkluderer sykdomstilstander som angitt ovenfor. Virale infeksjonsek-sempler inkluderer felinimmundefekt virus, infeksiøst anemivirus hos hest, artrittvirus hos geit, visnavirus og mædivirus så vel som andre lentiviruser.
Fremgangsmåter for fremstilling av syrer (I) er beskrevet i US 5,605,914 hvortil det henvises når det gjelder detaljer. Fremstillingen av oksadiazolene (III) kan skje i to trinn eller i en enkolbereåksjon. Omsetning av syren (I) med karbonyldiimidazol (CDI) eller et annet aktiveringsmidde1, fulgt av tilsetning av et acylhydrazid (NHjNHCXO, der X er hydrogen eller alkyl), gir en forbindelse med formel (II). Foretrukne oppløsningsmidler for denne reaksjon ("a") er aprotiske polare oppløsningsmidler som inkluderer acetonitril (CH3CN), tetrahydrofuran (THF) og etylacetat (EtOAc). Forbindelser med formel II kan isoleres på dette punkt. Alternativt kan en forbindelse med formel (II) benyttes i den neste reaksjon "b" uten isolering (et fore-trukket oppløsningsmiddel er i dette tilfellet acetonitril). I reaksjon "b" gir dehydratisering av en forbindelse med formel (II) med dehydratiseringsmiddel som fosforoksyklorid (POCI3) eller fosforpentoksid (P2O5) en forbindelse med formel (III). Man kan benytte varme i forbindelse med reaksjon "b".
Når en av R<1>, R<2>, R<3> og R4 skal være amino i det endelige 1,3,4-oksodiazol, er det ofte ønskelig å benytte den tilsvarende nitroforbindelse (I) og så redusere det resulterende nitroisoindolinon til et aminoisoindolinon etter dannelse. Alternativt kan aminogruppene og andre grupper som kan reagere, omdannes til en egnet beskyttet gruppe.
Beskyttende grupper som her benyttes, angir grupper som ge-nerelt ikke finnes i de endelige terapeutiske forbindelser, men som innføres på et visst trinn i syntesen for å beskyt-te grupper som ellers vil kunne endres under kjemiske mani-puleringer. Slike beskyttende grupper fjernes på et senere trinn i syntesen, og forbindelser som bærer slike beskyttende grupper, er av betydning primært som kjemiske mel-lomprodukter (selv om enkelte derivater også viser biolo-gisk aktivitet). I henhold til dette er den nøyaktige kjemiske struktur for disse beskyttende grupper ikke av vesentlig betydning. Forbindelsene med Formel I har et kiralitetssenter og kan således foreligge som optiske isomerer. Både racematene av disse isomerer og de individuelle isomerer per se så vel som diastereomerer når det er to kirale senter, ligger in-nenfor oppfinnelsens ramme. Racematene kan benyttes som så-danne eller kan separeres i de individuelle isomerer på me-kanisk måte som ved kromatografi eller ved bruk av en kiral absorbent. Alternativt kan de individuelle isomerer fremstilles i kiral form eller separeres kjemisk fra en blanding ved å danne salter med en kiral syre eller base, eller som de individuelle enantiomerer av 10-kamforsulfonsyre, kamforsyre, a-bromkamforsyre, metoksyed-diksyre, vinsyre, deacetylvinsyre, eplesyre, pyrrolidion-5-karboksylsyre og lignende, og så å sette fri en eller begge av de oppløste baser, eventuelt ved å gjenta prosessen, for derved å oppnå enten den ene eller begge, i det vesentlige frie for den andre, dvs. i en form med optisk renhet >95 %.

Foreliggende oppfinnelse angår også fysiologisk akseptable ikke-toksiske syreaddisjonssalter av forbindelsene med formel (I). Slike salter inkluderer de som er avledet fra organiske og uorganiske syrer som, uten begrensning, salt-, hydrobrom-, fosfor-, svovel-, metansulfon-, eddik-, vin-, melke-, rav-, sitron-, eple-, malein-, sorbin-, akonitin-, salisyl-, ftal-, embon- eller enantinsyre og lignende.
Orale doseringsformer inkluderer tabletter, kapsler, dra-géer og tilsvarende formede, komprimerte farmasøytiske for-mer inneholdende 1-100 mg medikament per enhetsdose. Blan-dinger inneholdende fra 20-100 mg/ml kan formuleres for parenteral administrering som inkluderer intramuskulær, intratekal, intravenøs eller intraarteriell administre-ringsvei. Rektal administrering kan gjennomføres ved bruk av suppositorier som er formulert fra konvensjonelle bærere som kakaosmør.
Farmasøytiske preparater omfatter således en eller flere forbindelser ifølge oppfinnelsen assosiert med minst en farmasøytisk akseptabel bærer, diluent eller eksipient. Ved fremstilling av slike preparater blir de aktive bestanddeler vanligvis blandet med eller fortynnet med en eksipiens eller innelukket i en slik bærer som kan være i form av en kapsel eller omhylling. Når eksipienten tjener som en diluent, kan den være et fast, halvfast eller flytende materiale som virker som en bærer eller et medium for den aktive bestanddel. Således kan preparatene foreligge i form av tabletter, piller, pulvere, eliksirer, suspensjoner, emulsjoner, oppløsninger, siruper, myke og harde gelatinkapsler, suppositorier, sterile injiserbare oppløsninger og sterilt pakkede pulvere. Eksempler på egnede eksipienter inkluderer laktose, dekstrose, sukkrose, sorbitol, mannitol, strivelse, akasia, kalsiumsilikat, mikrokrystallinsk cellulose, polyvinylpyrrolidin polyvinylpyrrolidon, cellulose, vann, sirup og metylcellulose, og formuleringene kan i tillegg inkludere smøremidler som talkum, magnesiumstearat og mineralolje, fuktemidler, emulgerings- og sus-pens jonsmidler, preserveringsmidler som metyl- og propyl-hydroksybenzoater samt søtningsmidler eller smaksmidler.
Preparatene formuleres fortrinnsvis i enhetsdoseform, noe som betyr fysikalsk diskrete enheter egnet for en enhetsdose eller en på forhånd bestemt brøkdel av en enhetsdose som skal administreres i en eller flere doseringsregimer til mennesker eller andre pattedyr, idet hver enhet inneholder en på forhånd bestemt mengde aktivt materiale beregnet for å gi den ønskede terapeutiske effekt i forbindelse med en egnet farmasøytisk eksipient. Preparatene kan formuleres for å gi en umiddelbar, opprettholdt eller forsinket frigivning av den aktive bestanddel efter administrering til pasienten ved å benytte i og for seg kjente prosedyrer.
De følgende eksempler skal ytterligere illustrere oppfinnelsen nærmere uten å anses som en begrensning av denne, idet denne bestemmes av de vedlagte krav.
EKSEMPEL 1
211-( 3- etoksy- 4- metoksyfenyl)- 2-( 1, 3, 4- oksadiazol- 2-yl) etyl] isoindolin- 1, 3- dion
En blanding av (3-(1,3-dioksoisoindolin-2-yl)-3-(3-etoksy-4-metoksyfenyl)propansyre (3,0 g, 8,1 mmol) og karbonyldiimidazol (1,45 g, 8,94 mmol) i 15 ml tetrahydrofuran ble omrørt ved romtemperatur i 2 timer. Til oppløsningen ble det satt maursyrehydrazid (644 mg, 10,7 mmol). Blandingen ble omrørt i 18 timer. Den resulterende suspensjon ble filtrert og vasket med eter. Det isolerte faststoff ble om-rørt i en blanding av 40 ml etylacetat og 10 ml vann i 1 time. Suspensjonen ble filtrert og vasket med vann og eter for å gi urent 3-(1,3-dioksoisoindolin-2-yl)-N-karbonyl-amino-3-(3-etoksy-4-metoksyfenyl)propanamid (1,3 g, 39 %) utbytte). En oppløsning av karbonylamino-3-(1,3-dioksoiso-indolin-2-yl)-N-karbonylamino-3-(3-etoksy-4-metoksyfenyl)-propanamid (600 mg, 1,46 mmol) og fosforoksyklorid (POCI3, 0,54 ml, 5,8 mmol) i 20 ml acetonitril ble oppvarmet til tilbakeløp i 2 timer. Denne oppløsningen ble helt i 10 ml vann. Det vandige sjikt ble ekstrahert med 2 x 50 ml etylacetat. De kombinerte organiske sjikt ble vasket med 50 ml mettet natriunhydrogenkarbonat, 50 ml saltoppløsning og tørket over magnesiumsulfat. Fjerning av oppløsningsmidde-let og kromatografi ga en olje. Oljen ble oppslammet i 10 ml eter. Den resulterende suspensjon ble filtrert for å gi 2-[1-(3-etoksy-4-metoksyfenyl)-2-(1,3,4-oksadiazol-2-yl)etyl]isoindolin-1,3-dion som et hvitt fast stoff (250 mg, 43 %).

Således av omsetning av (3-(1,3-dioksobenzo[e]isoindolin-2-yl)-3-(3-etoksy-4-metok-syfenyl)propansyre (1,50 g, 3,58 mmol), karbonyldiimidazol (0,70 g, 4,3 mol) og maursyrehydrazid (310 mg, 5,16 mmol) i 20 ml tetrahydrofuran ga urent 3-(1,3-dioksobenzo[e]isoin-dolin-2-yl)-N-karbonylamino-3-(3-etoksy-4-metoksyfenyl)propanamid (1,0 g, 2,2 mmol), som så ble behandlet med fosforoksyklorid (POCl3, 0,4 ml, 4,3 mmol) i 10 ml acetonitril. N-[2-[ 1-( 3- etoksy- 4- metoksyfenyl)- 2-( 1, 3, 4- oksadiazol- 2-yl) etyl]- 1, 3- dioksoisoindolin- 4- yl] acetamid En blanding av 3-[4-(acetylamino)-1,3-dioksoisoindolin-2-yl]-3-(3-etoksy-4-metoksyfenyl)propansyre (1,69 g, 3,69 mmol) og karbonyldiimidazol (0,71 g, 4,4 mmol) i 20 ml acetonitril ble rørt ved romtemperatur i 2 timer. Til oppløs-ningen ble det satt maursyrehydrazid (289 mg, 4,81 mmol). Blandingen ble så omrørt i 18 timer. Til den resulterende oppløsning ble det satt fosforoksyklorid (1,0 ml, 10,7 mmol), og denne blanding ble brakt til tilbakeløp i 2 timer. Oppløsningen ble helt i 10 ml vann. Det vandige sjikt ble ekstrahert med 2 x 50 ml etylacetat.
