Description
FARMASØYTISK AKTIVE ISOINDOLINDERIVATER
Foreliggende oppfinnelse angår ikke-polypeptidisoindolin-derivater som reduserer nivåene av tumornekrosefaktor a (TNFa), og den angår behandling av sykdomstilstander som me-dierer dermed. For eksempel er forbindelser som selektivt inhiberer TNFa brukbare ved behandling av inflammasjon og bevirker relaksasjon i glatte luftveismuskler med et minimum av uønskede bivirkninger, for eksempel kardiovaskulære eller antiplateeffekter. Foreliggende oppfinnelse angår også metoder for behandling av farmasøytiske preparater som benytter slike forbindelser.
Tumornekrosefaktor a eller TNFa er et cytokin som frigis primært av mononukleære fagocytter i respons på et antall im-munostimulatorer. Administrert til mennesker eller dyr forår-saker den inflammasjon, feber, kardiovaskulære effekter, he-morrhagi, koagulasjon og akuttfaseresponser tilsvarende de man ser ved akutte infeksjoner og sjokktilstander. Eksessiv eller ikke-regulert TNFa-produksjon er således involvert i et antall sykdomstilstander. Disse inkluderer endotoksemi og/eller toksisk sjokksyndrom [Tracey et al., "Nature", 330, 662-664 (1987) og Hinshaw et al., "Circ. Shock", 30, 279-292
(1990)]; rheumatoid artritt, Crohns sykdom, IBD, kakesi [De-zube et al., "Lancet", 335 (8690), 662 (1990)] og adult respiratorisk distress-syndrom der TNFa-konsentrasjoner ut over 12 000 pg/ml er detektert pulmonære aspirater fra ARDS-pasienter [Millar et al., "Lancet", 2 (8665), 712-714
(1989)]. Systemisk infusjon av rekombinant TNFa har også re-sultert i endringer som typisk sees ved ARDS [Ferrai-Baliviera et al., "Arch. Surg.", 124(12), 1400-1405 (1989)].
TNFa synes å være involvert i benresorpsjonssykdommer inkludert artritt. Når aktivert, vil leukocytter produsere benresorpsjon, en aktivitet der data antyder at TNFa bidrar [Ber-tolini et al., "Nature", 319, 516-518 (1986) og Johnson et al., "Endocrinology", 124(3), 1424-1427 (1989)]. TNFa er også vist å stimulere benresorpsjon og inhibere bendannelse in vitro og in vivo ved stimulering av osteoblastdannelse og aktivering kombinert med inhibering av osteoblastfunksjon. Selv om TNFa kan være involvert i mange benresorpsjonssykdommer, inkludert artritt, er den mest fremtredende forbindelse med sykdom assosiasjonen mellom produksjonen av TNFa hos tumor eller vertsvev og malignansassosiert hyper-kalsemi ["Cali. Tussue Int. (US)" 46 (suppl.), S3-10 (1990)]. I podings-mot-vert-reaksjoner er økede serum-TNFa-nivåer assosiert med vesentlige komplikasjoner etter akutt allogeniske benmargtransplanteringer [Holler et al., "Blood", 75(4), 1011-1016 (1990)].
Cerebral malaria er et letalt hyperakutt neurologisk syndrom assosiert med høye blodnivåer av TNFa og den mest alvorlige komplikasjon som opptrer hos malariapasienter. Nivåer for serum-TNFa korrelerer direkte med alvoret av sykdommen og prognosen hos pasienter med akutt malariaangrep [Grau et al., "N. Eng. J. Med.", 320(24), 1586-1591 (1989)].
Ikke-regulert angiogenese er patologisk og understøtter progresjon av mange neoplastiske og ikke-plastiske sykdommer inkludert tumorvekst og metastaser, artritt, visse typer øye-lidelser, og psoriasis, se for eksempel Moses et al., 1991, "Biotech.", 9:630-634; Folkman et al., 1995, "N. Engl. J. Med.", 333: 1757-1763; Auerbach et al., 1985, "J Microvasc. Res.", 29: 401-411; Folkman, 1985, "Advances in Cancer Re-search", utg. Angiogenese må således tas med i betraktning i forskjellige sykdomstilstander, tumormetastaser og unormal vekst av endoteliale celler. Patologiske tilstander som skapes ved ikke-regulert angiogenese, er gruppert sammen som angiogenisk av-hengig eller angiogenisk assosierte sykdommer. Kontroll av de angiogeniske prosesser kan føre til mitigering av disse til-standene .
Komponentene for angiogenese relatert vaskulær endotelial celleproliferering, migrering og invasjon er funnet å være regulert delvis av polypeptidvekstfaktorer. Endotelialceller som eksponeres til et medium inneholdende egnede vekstfaktorer, kan induseres til å fremkalle noen eller alle av de angiogeniske responser. Polypeptider med in vitro endotelial vekstpromoterende aktivitet inkluderer sure og basiske fi-broblastvekstfaktorer, transformerende vekstfaktorer a og P, plateavledet endotelial cellevekstfaktorer, granulycyttkolo-nistimulerende faktor, interleukin-8, heptocyttvekstfaktor, proliferin, vaskulær endotelial vekstfaktor og placental vekstfaktor, se Folkman et al., 1995, "N. Inhibitoriske influenser overveier i den naturlige opptreden-de balanse mellom endogene stimulatorer og inhibitorer for angiogenese, se Rastinejad et al., 1989, "Cell", 56:345-355. I de tilfeller der neovaskularisering inntrer under normale fysiologiske betingelser, som sårheling, organregenerering, embryonisk utvikling og female reproduktive prosesser, er angiogenesen stringent regulert og romlig og tidsmessig av-grenset. Under betingelser med patologisk angiogenese som det som karakteriserer fast tumorvekst, svikter disse regulato-riske kontroller.
Makrofagindusert angiogenese er kjent for å være mediert av TNFa. Leibovich et al. ["Nature", 329, 630-632 (1987)] viste at TNFa induserer in vivo kapillær blodkardannelse i rotte-cornea og utviklingen av kyllingkorioallantoriske membraner ved meget små doser og antyder at TNFa er en kandidat for å indusere angiogenese ved inflammasjon, sårheling og tumorvekst .
TNFa-produksjon er også uavhengig assosiert med krefttil-stander og særlig induserte tumorer [Ching et al., "Brit. J. Cancer", (1955), 72, 339-343 og Koch, "Progress in Medicinal Chemistry", 22, 166-242 (1985]. Enten angiogenese er involvert med TNFa-produksjon eller ikke, er angiogenesen promi-nent i fast turmordannelse, og metastase og angiogeniske fak-torer er funnet assosiert med flere faste tumorer som rhab-domyosarkoma, retinoblastoma, Ewing-sarkoma, neublastoma og osteosarkoma. Tumorer der angiogenese er viktig, inkluderer faste tumorer og benigne tumorer som akustisk neurom, neuro-fibrom, trakoma og pyogenisk granuloma. Uavhengig av virkningen på TNFa-produksjonen, kan prevensjonen av angiogenese stanse veksten av disse tumorer og den resulterende skade på dyret på grunn av nærværet av denne tumor. Angiogenese har vært assosiert med blodbårne tumorer som leukemier og forskjellige akutte eller kroniske neoplastiske sykdommer på benmargen. Under slike betingelser inntrer ikke-begrenset proliferering av hvite blodlegemer, vanligvis ledsaget av anemi, forringet blodkoagulasjon og forstørrelse av lymfeknu-tene, lever og milt.
Angiogenese er også involvert i tumormetaser. Således inntrer angiogenesestimulering i vaskularisering av tumoren og tilla-ter at tumorcellene kommer inn i blodstrømmen og sirkulerer i kroppen. Etter at tumorcellene har forlatt primærsetet og har slått seg ned i det sekundære metastasesetet, må angiogenese inntre før den nye tumor kan vokse og ekspandere.
Alle de forskjellige celletyper i kroppen kan transformeres til benigne eller malignante tumorceller. Det hyppigste tu-morsetet er lungen, fulgt av kolorektal-, bryst-, prostata-, blære-, pankreas- og ovariesetet. Andre prevalente typer av cancer er leukemi, sentralnervesystemcancer inkludert hjerne-cancer, melanoma, lymfoma, erytroleukemi, uterincancer og hode- og halscancer.
TNFa spiller også en rolle på området kroniske, pulmonære inflammatoriske sykdommer. Avsetning av silikapartikler fører til silikose, en sykdom med progressiv respiratorisk svikt forårsaket av en fibrotisk reaksjon. Antistoff mot TNFa blokkerte fullstendig den silikainduserte lungefibrose i mus [Pignet et al., "Nature", 344: 245-247 (1990)]. Høye nivåer av TNFa-produksjon (i serum og i isolerte makrofager) er påvist i dyremodeller med silika- og asbestindusert fibrose
[Bissonnette et al., "Inflammation", 13(3), 329-339 (1989)]. Alveolære makrofager fra pulmonære sarkoidosepasienter er også funnet spontant å frigi massive mengder TNFa sammenlig-net med makrofager fra normale donorer [Baughman et al., "J. Lab. Clin. Med.", 115(1), 36-42 (1990)].
TNFa er også involvert i den inflammatoriske respons som følger reperfusjon, kalt reperfusjonsskade. Det er en hoved-grunn til vevskade etter tap av blodstrøm [Vedder et al., "PNAS", 87, 2643-2646 (1990)]. TNFa endrer også egenskapene for endoteliale celler og har forskjellige pro-koagulante aktiviteter, for eksempel å gi en økning i vevfaktorprokoagu-lantaktivitet og suppresjon av den antikoagulante protein-C-vei så vel som en nedregulering av ekspresjonen av trombomo-dulin [Sherry et al., "J. Cell. Biol.", 107, 1269-1277
(1988)]. TNFa har proinflammatoriske aktiviteter som sammen
med dens tidlige produksjon (under de innledende trinn av et inflammatorisk evenement) gjør den til en sannsynlig formid-ler av vevskade ved flere vesentlige lidelser inkludert, men ikke begrenset til, myokardialt infarkt, slag og sirkulato-risk sjokk. Av spesiell betydning kan TNFa-indusert ekspre-sjon av adhesjonsmolekyler være, for eksempel det intracellu-lære adhesjonsmolekyl (ICAM), eller det endoteliale leukocyttadhesjonsmolekyl (ELAM) på endoteliale celler [Munro et al., "Am. J. Path.", 135(1), 121-132 (1989)].
TNFa-blokkering med monoklonale anti-TNFa-antistoffer har vist seg å være fordelaktige ved rheumatoid artritt [Elliot et al., "Int. J. Pharmac", 1995, 17(2), 141-145] og Crohns sykdom [von Dullemen et al., "Gastroenterology", 1995, 109 (1), 129135] .
Videre er det nå kjent at TNFa er en potent aktivator for retrovirusreplikasjon inkludert aktivering av HIV-1, [Duh et al., "Proe. Nat. Acad. Sei.", 86, 5974-5978 (1989); Poll et al., "Proe. Nat. Acad. Sei.", 87, 782-785 (1990); Monto et al., "Blood", 79, 2670 (1990); Clouse et al., "J. Immunol.", 142, 431-438 (1989); Poll et al., "AIDS Res. Hum. Retrovi-rus", 191-197 (1992)]. AIDS stammer fra infeksjon av T-lymfocytter med human defektvirus (HIV). Minst tre typer eller stammer av HIV er identifisert, nemlig HIV-1, HIV-2 og HIV-3. Som en konsekvens av HIV-infeksjonen blir T-celle-mediert im-munitet forringet, og infiserte individer manifesterer alvorlige opportunistiske infeksjoner og/eller uvanlige neoplas-mer. HIV-inntrenging i T-lymfocytten krever T-lymfocytt-aktivering. Andre viruser som HIV-1, HIV-2 infiserer T-lymfocytter etter T-celle-aktivering, og slik virusproteinin-feksjon og/eller -replikasjon medieres eller opprettholdes av slik T-celle-aktivering. Når først en aktivert T-lymfocytt er infisert med HIV, må T-lymfocytten fortsette å holdes i aktivert tilstand for å tillate HIV-genekspresjon og/eller HIV-replikasjon. Cytokiner, og spesielt TNFa er involvert i aktivert T-celle-mediert HIV-proteinekspresjon og/eller virus-replikasjon ved å spille en rolle ved opprettholdelse av T-lymfocyttaktivering. Interferens med cytokin-aktiviteten slik som ved prevensjon eller inhibering av cytokinproduksjon, særlig TNFa, i et HIV-infektert individ, understøtter derfor begrensning av opprettholdelsen av T-lymfocytter forårsaket av HIV-infeksjon.
Monocytter, makrofager og relaterte celler som kupffer- og glialceller, har også vært involvert ved opprettholdelse av HIV-infeksjonen. Disse celler er, som T-cellene, mål for viral replikasjon, og nivået av viral replikasjon avhenger av aktiveringstilstanden av cellene, [Rosenberg et al., "The Im-munopathogenesis of HIV Infection", "Advances in Immunology", 57 (1989)]. Cytokiner, som TNFa, har vært påvist å aktivere HIV-replikasjon i monocytter og/eller makrofager [Poli et al.
"Proe. Nat. Acad. Sei.", 87, 782-784 (1990)]; derfor under-støtter prevensjon eller inhibering av cytokinproduksjon eller -aktivitet en begrensning av HIV-progresjonen for T-celler. Ytterligere studier har identifisert TNFa som en felles faktor for aktivering av HIV in vitro og har gitt en klar virkningsmekanisme via et nukleært regulatorisk protein funnet i cellenes cytoplasma (Osbom et al., "PNAS" 86, 2335-2340) . Denne påvisning antyder at en reduksjon av TNFa-syntesen kan ha en antiviral effekt på HIV-infeksjoner ved å redusere transkripsjonen og derved virusproduksjonen.
Viral AIDS-replikasjon av latente HIV i T-celle og
-makrofaglinjer kan induseres ved TNFa [Folks et al.,
"PNAS", 86, 2365-2358 (1989)]. En molekylær mekanisme for vi-rusen som induserer aktivitet, antydes av TNFa's evne til å aktivere et genregulatorisk protein (NKKB) som finnes i cellenes cytoplasma og som promoterer HIV-replikasjon via bin-ding til en viral regulatorisk gensekvens (LTR) [Osborn et al., "PNAS", 86, 2336-2340 (1989)]. TNFa i AIDS-assosiert kakeksi antydes ved elevert serum-TNFa og høye nivåer av spontant TNFa-produksjon i perifere blodmonocytter fra pasienter [Wright et al., "J. Immunol.", 141(1), 99-104 (1988)]. TNFa har vært implikert i forskjellige roller med andre virale infeksjoner som cytomegaliavirusen (CMV), influensa-virusen, adenovirusen og herpesfamilien av viruser av tilsvarende grunner som de som er angitt.
Den nukleære faktor kB (NFkB) er en pleiotropisk transkripsjonen aktivator (Lenardo et al., "Cell", 1989, 58, 227-229) . NFKB har vært involvert som en transkripsjonen aktivator i en rekke sykdommer og inflammatoriske tilstander og an-tas å regulere cytokinnivåene inkludert, men ikke begrenset til, TNFa og også å være en aktivator av HIV-transkripsjonen
(Dbaibo et al. "J. Biol. Chem.", 1993, 17762-66; Duh et al., "Proe. Nat. Acad. Sei.", 1989, 86, 5974-78; Bachelerie et al., "Nature", 1991, 350, 709-12; Boswas et al., "J. Acuired Immune Deficiency Syndrome", 1993, 6, 778-786; Suzuki et al., "Biochem. And Biophys. Res. Comm.", 1993, 193, 277-83; Suzuki et al., "Biochem. And Biophys. Res. Comm.", 1992, 189, 1709-15; Suzuki et al., "Biochem. Mol. Bio. Int.", 1993, 31(4), 693-700; Shakhov et al., "Proe. Nat. Acad. Sei. USA", 1990, 171, 35-47; og Staal et al., "Proe. Nat. Acad. Sei. USA", 1990, 87, 9943-47). Således kan inhibering av NFKB-bindingen regulere transkripsjon av ett eller flere cytokingener og via denne moduleringen og andre mekanismer være brukbare ved inhibering av et antall sykdomstilstander. Forbindelsene som her er beskrevet, kan inhibere virkningen av NKKB i nukleus og er derved brukbare ved behandling av et antall sykdommer inkludert, men ikke begrenset til, rheumatoid artritt, rheumatoid spondylitt, osteoartritt, andre artritiske tilstander, cancer, septisk sjokk, sepsis, endotoksisk sjokk, podings-mot-vertssykdom, "wasting", Crohns sykdom, inflammatorisk tarmsykdom, uleerativ colitt, multiplesklerose, systemisk lupus erytrematose, ENL i leprosi, HIV, AIDS og opportuniske infeksjoner i AIDS. TNFa- og NFKB-nivåene påvirkes av en re-siprok feedback-sløyfe. Som antydet ovenfor, påvirker forbindelsene ifølge oppfinnelsen nivåene av både TNFa og NFKB.
Ved å redusere TNFa-nivåene utgjør således verdifulle terapeutiske strategier for behandling av mange inflammatoriske, infektiøse, immunologiske og malignante sykdommer. Disse inkluderer, men er ikke begrenset til septisk sjokk, sepsis, endotoksisk sjokk, hemodynamisk sjokk og sepsissyndrom, post ischemisk reperfusjonsskade, malaria, mycobakteriell infeksjon meningitt, psoriasis, kongestive hjertesvikt, fibrotisk sykdom, kakeksi, podingsawisning, cancer, autoimmun sykdom, opportunisk infeksjon i AIDS, rheumatoid artritt, rheumatoid spondylitt, osteoartritt, andre artriske tilstander, Crohns sykdom, ulcerativ colitt, multippel sklerose, systemisk lupus erytrematose, ENL i leprosi, strålingsskade og hyperoksisk alveolær skade.
Foreliggende oppfinnelse angår en forbindelse med formel I der karbonatomene angitt med <*> utgjør et chiralitetssenter: ;der ;X er -C(0)- eller -CH2-; ;R<1> er Ci-a-alkyl eller -NHR<3>;;R<2> er hydrogen, Ci-8-alkyl eller halogen; og ;R<3> er hydrogen eller Ci_8-alkyl. ;Foreliggende oppfinnelse angår også syreaddisjonssalter av disse isoindolinderivater som er i stand til protonering. Slike salter inkluderer de som stammer fra organiske eller uorganiske syrer, som, uten begrensning, omfatter salt-, hy-drobrom-, fosfor-, svovel-, metansulfon-, eddik-, vin-, mel-ke-, rav-, sitron-, eple-, malein-, sorbin-, akonitin-, sali-cyl-, ftal-, embon-, enantionsyre og lignende. ;Forbindelsene kan fremstilles ved et antall metoder. Forbindelsene kan administreres som i det vesentlige chiralt ren (S,S)-, (S,R)-, (R,R)- eller (R,S)-isomer eller som blandinger av to eller flere av disse isomerer og alle ligger innenfor rammen av oppfinnelsen. Blandinger kan benyttes som sådanne eller kan separeres i de individuelle isomerer på mekanisk måte, for eksempel ved kromato-grafi ved bruk av en chiral absorbent. Alternativt kan de individuelle isomerer fremstilles i chiral form eller separeres kjemisk fra en blanding ved å tildanne salter med en chiral syre, eller ha individuelle enantiomerer av 10-kamfor-sulfonsyre, kamforsyre, bromkamforsyre, metoksyeddiksyre, vinsyre, diacetylvinsyre, eplesyre, pyrrolidon-5-karboksyl-syre og lignende, og så befri en eller begge av de oppløste baser, eventuelt å gjenta prosessen for derved å oppnå en av eller begge i det vesentlige frie for den andre, det vil si ved en optisk renhet >95 %.
En første foretrukket subgruppe er de forbindelser med formel I der R<2> er hydrogen, metyl eller fluor, særlig hydrogen.
En andre foretrukket subgruppe er de forbindelser med formel I der R<1> er amino.
En tredje foretrukket subgruppe er de forbindelser med formel I der R<1> er metyl.

PBMC fra normale donorer oppnås ved Ficoll-Hypaque-densitets-sentrifugering. Celler dyrkes i RPMI supplert med 10 % AB+ serum, 2 mM L-glutamin, 100 U/ml penicillin og 100 mg/ml streptomycin.
Testforbindelsene oppløses i dimetylsulfoksyd (Sigma Chemi-cal) , og ytterligere fortynninger skjer i supplert RPMI. Di-metylsulf oksydsluttkonsentrasj onen i nærvær eller fravær av medikament i PBMC-suspensjonene i 0,25 vekt-%. Testforbindelsene analyseres i halvlogfortynninger ut fra 50 mg/ml. Testforbindelsene settes til PBMC (IO<6> celler/ml) i 96-brønners plater i en time før tilsetning av LPS.
PBMC (IO<6> celler/ml) i nærvær eller fravær av testforbindel-sen stimuleres ved behandling med 1 mg/ml LPS fra Salmonella minnesota R595 (List Biological Labs, Campbell, CA). Cellene inkuberes så ved 37 °C i 18-20 timer. Supernatantene høstes og analyseres umiddelbart på TNFa-nivåer eller holdes ned-frosset ved -70 °C (i ikke mer enn 4 dager) inntil analyse.
Konsentrasjonen av TNFa i supernatanten bestemmes ved human TNFa ELISA-sett (Endogen, Boston, MA) i henhold til produ-sentens retningslinjer.
Forbindelsene kan benyttes, under overvåkning av kvalifisert personell, for å inhibere de uønskede effekter av TNFa og NFKB. Forbindelsene kan administreres oralt, rektalt eller parenteralt, alene eller i kombinasjon med andre terapeutiske midler inkludert antibiotika, steroider og så videre til et pattedyr som trenger slik behandling. Orale doseformer inkluderer tabletter, kapsler, dragéer og tilsvarende formgitte, komprimerte farmasøytiske former. Isotoniske saltoppløsninger inneholdende 20-100 mg/ml kan benyttes for parenteral administrering som omfatter intramuskulær, intratekal, intravenøs eller intraarteriell administrering. Rektal administrering kan gjennomføres ved bruk av suppositorier tildannet fra konvensjonelle bærere som kakaosmør.
Doseregimet må bestemmes mot den spesielle indikasjon, alder, vekt og den generelle fysiske tilstand hos pasienten samt den respons som ønskes, men generelt vil dosene ligge fra rundt 1 til rundt 1000 mg pr. dag etter behov i enkle eller flere
daglig administreringer. Generelt kan et første behandlings-regime kopieres fra det som er kjent for å være effektivt med
henblikk på å interferere med TNFa-aktiviteten på andre TNFOc-medierte sykdomstilstander ved hjelp av forbindelsene ifølge oppfinnelsen. Behandlede individer vil undersøkes re-gelmessig for T-celletall og T4:T8-forhold og/eller mål på
viremi som nivåene for revers transkriptase eller virale pro-teiner, og/eller for progresjon av cytokinmedierte sykdomsas-sosierte problemer som kakeksi eller muskeldegenerering. Hvis ingen effekt observeres etter det normale behandlingsregimet, blir mengden av cytokinaktivitetsinterfererende middel som
administreres, øket, for eksempel med 50 % i løpet av en uke.
Forbindelsene ifølge oppfinnelsen kan også benyttes topisk ved terapi eller profylakse av topiske sykdomstilstander som er mediert eller forverret av eksessiv TNFa-produksjon som virale infeksjoner, for eksempel de som er forårsaket av her-pesviruser eller viral konjunktivitt, psoriasis, andre hudli-delser og sykdommer, og så videre.
Forbindelsene kan også benyttes ved veterinærbehandling av pattedyr andre enn mennesker som trenger prevensjon eller inhibering av TNFa-produksjonen. TNFa-medierte sykdommer for behandling, terapeutisk eller profylaktisk, i dyr inkluderer sykdomstilstander som de som er angitt ovenfor, men særlig virale infeksjoner. Eksempler inkluderer felin immunode-fektvirus, equininfektiøs anemivirus, kaprinartrittvirus, visnavirus og maedivirus, så vel som andre kjente lentiviru-ser.
Oppfinnelsen inkluderer således anvendelse av en effektiv mengde av en chiral, i det vesentlige ren (R)- eller (S)-isomer av en forbindelse med formel I eller en blanding av disse isomerer til fremstilling av medikamenter for å redusere eller inhibere uønskede nivåer av TNFa, redusere eller inhibere uønskede nivåer av matriksmetalloproteinaser, behandling av uønsket angiogenese, cancer, inflammatorisk sykdom, autoimmun sykdom, artritt, rheumatoid artritt, inflammatorisk tarmsykdom, Crohns sykdom, aftøse ulcere, kakeksi, po-ding -mot -vert -sykdom, astma, adult respiratorisk distress-syndrom og ervervet immundefektsyndrom hos et pattedyr.
Oppfinnelsen omfatter også farmasøytiske preparater der
(i) en mengde av en chiral, i det vesentlige ren (R)- eller (S)-isomer av en forbindelse med formel I eller en blanding av disse isomerer, som ved administrering i en enkelt eller flere doseregimer er farmasøytisk ef-fektive, kombineres med
(ii) en farmasøytisk akseptabel bærer.
Farmasøytiske preparater kan types ved orale doseringsformer som inkluderer tabletter, kapsler, dragéer og tilsvarende formgitte, komprimerte farmasøytiske former inneholdende fra 1 til 100 mg medikament pr. enhetsdose. Blandinger inneholdende fra 20 til 100 mg/ml kan formuleres for parenteral administrering som inkluderer intramuskulær, intratekal, intra-venøs eller intra-arteriell administreringsvei. Rektal administrering kan gjennomføres ved bruk av suppositorier som er formulert fra konvensjonelle bærere som kakaosmør.
Farmasøytiske preparater vil omfatte en eller flere forbindelser ifølge oppfinnelsen assosiert med minst en farmasøy-tisk akseptabel bærer, diluent eller eksipient. Ved fremstilling av slike preparater blir de aktive bestanddeler vanligvis blandet med eller fortynnet med en eksipient eller innelukket i en slik bærer som kan være i form av en kapsel eller en pose. Når eksipienten tjener som diluent, kan den være et fast, halvfast eller flytende materiale som virker som bærer eller som medium for den aktive bestanddel. Således kan preparatene foreligge i form av tabletter, piller, pulvere, eliksirer, suspensjoner, emulsjoner, oppløsninger, siru-per, myke og harde gelatinkapsler, suppositorier, sterile, injiserbare oppløsninger og sterile, pakkede pulvere. Eksempler på egnede eksipienter er laktose, dekstrose, sukrose, sorbitol, mannitol, stivelse, akasiegummi, kalsiumsilikat, mikrokrystallinsk cellulose, polyvinylpyrrolidinon, polyvi-nylpyrrolidon, cellulose, vann, sirup og metylcellulose, idet formuleringen i tillegg kan inneholde smøremidler som talkum, magnesiumstearat og mineralolje, fuktemidler, emulgerings- og suspenderingsmidler, preserveringsmidler som metyl- og pro-pylhydroksybenzoater, søtningsmidler og smaksstoffer.
Preparatene formuleres fortrinnsvis i enhetsdoseform, noe som betyr fysisk diskrete enheter egnet som en enhetsdose, eller en forhåndsbestemt fraksjon av en enhetsdose for administrering i et enkelt eller flere doseregimer til mennesker og andre pattedyr, idet hver enhet inneholder en på forhånd be-stemt mengde aktivt materiale som er beregnet til å gi den ønskede terapeutiske effekt i assosiasjon med en egnet farma-søytisk eksipient. Preparatene kan formuleres for å gi en umiddelbar, en opprettholdt eller en forsinket frigivning av aktiv bestanddel etter administrering til pasienten ved å benytte i og for seg kjente prosedyrer.
De følgende eksempler skal beskrive oppfinnelsens art, men skal ikke anses som begrensende når det gjelder dens omfang, idet oppfinnelsens omfang kun er definert av de vedlagte krav.
Eksempel 1
2-(5-hydroksy-2,6-dioksopiperid-3-yl)-4-metylisoindolin-l,3-dion
A. 3-( 4- metyl- l, 3- dioksoisoindolin- 2- yl)- 2, 6- diokso- 5-acetoksypiperidin
En blanding av 2,6-diokso-3-benzyloksykarbonylamino-5-acetoksypiperidin (9 g, 28,2 mmol) (U. Teubert et al., "Aren. Pharm. Pharm. Med. Chem.", (1998) 7-12) og 0,9 g 10 % Pd/C i 90 ml eddiksyre omrøres under et hydrogentrykk på 50-60 psi i 3 timer. Suspensjonen filtreres gjennom en Celite-pute og vaskes med eddiksyre. Til filtratet settes 3-metylfltalsyre-anhydrid (4,56 g, 28,2 mmol) og blandingen oppvarmes til til-bakeløp i 18 timer. Oppløsningsmidlet fjernes under vakuum, og man oppnår 3-(4-metyl-l,3-dioksoisoindolin-2-yl)-2,6-diokso-5-acetoksypiperidin som kan renses ytterligere ved kolonnekromatografi.
B. 2-( 5- hydroksy- 2, 6- dioksopiperid- 3- yl)- 4- metylisoindolin-1, 3- dion
En oppløsning av 3 -(4-metyl-l,3-dioksoisoindolin-2-yl)-2,6-diokso-5-acetoksypiperidin (lg, 3,5 mmol) og p-toluensulfonsyre (0,33 g, 1,8 mmol) i 10 ml metanol oppvarmes til tilba-keløp i 5 timer. Oppløsningsmidlet fjernes under vakuum, og man oppnår 2-(5-hydroksy-2,6-dioksopiperid-3-yl)-4-metyliso-indolin-1,3-dion. Produktet kan renses ytterligere ved kolonnekromatografi.
Eksempel 2
4-amino-2-(5-hydroksy-2,6-dioksopiperid-3-yl)isoindolin-1,3-dion
A. 5-( 1, 3- diokso- 4- nitro- l, 3- dihydro- isoindol- 2- yl)- 2, 6-diokso- piperidin- 3- yl- acetat
Til en blanding av 2-(2,6-diokso-piperidin-3-yl)-4-nitro-isoindol-1,3-dion (10,0 g, 33 mmol) i 200 ml eddiksyre ble det satt brom (3 ml, 59 mmol) ved romtemperatur. Blandingen ble oppvarmet til tilbakeløp i 2 dager. Reaksjonsblandingen ble avkjølt til romtemperatur, og det resulterende gule faststoff ble filtrert. Faststoffet ble omrørt i 500 ml aceton:heksan 1:1 i 30 minutter. Suspensjonen ble filtrert, og man oppnådde 2-(5-brom-2,6-diokso-piperidin-3-yl)-4-nitro-isoindol-l,3-dion som et hvitaktig faststoff (6,8 g, 54 % utbytte). En blanding av 2-(5-brom-2,6-diokso-piperidin-3-yl)-4-nitro-isoindol-l, 3-dion (6,0 g, 16 mmol) og natriumacetat (2,1 g, 26 mmol) i 60 ml DMF ble omrørt ved romtemperatur i 2 dager. Oppløsningsmidlet ble fjernet under vakuum for å gi et faststoff. Dette ble oppslemmet i 60 ml buffer pH = 6 og 1,5 g natriumbisulfitt i 6 timer, filtrert og vasket med 100 ml vann og så slemmet opp i 100 ml aceton:heksan 1:1. Suspensjonen ble filtrert og vasket med 50 ml heksan, og man oppnådde 5-(1,3-diokso-4-nitro-l,3-dihydro-isoindol-2-yl)-2,6-diokso-piperidin-3-yl-acetat som et hvitaktig faststoff (3,7 g, 65 % utbytte).
B. cis- 5-( 4- amino- 1, 3- diokso- l, 3- dihydro- isoindol- 2- yl)- 2, 6-diokso- piperidin- 3- yl- acetat
En blanding av eddiksyre-5-(4-amino-l,3-diokso-l,3-dihydro-isoindol-2-yl)-2,6-diokso-piperidin-3-yl-ester (3,4 g, 9,4 mmol) og 340 mg 10 % Pd/C i 340 ml metanol ble rystet under hydrogen i en Parr Shaker i 21 timer. Suspensjonen ble filtrert gjennom en pute av Celite<®> og vasket med 100 ml metanol og 250 ml aceton. De kombinerte filtrater ble fordampet under vakuum til et faststoff. Dette ble oppslemmet i 30 ml metanol i 2 timer, filtrert og faststoffet vasket med metanol, og man oppnådde cis-5-(4-amino-l,3-diokso-l,3-dihydro-isoindol-2-yl)-2,6-diokso-piperidin-3-yl-acetat som et gult faststoff (2,0 g, 64 % utbytte) med smeltepunkt 220-223 °C.
C. 4- amino- 2-( 5- hydroksy- 2, 6- diokso- piperidin- 3- yl)-isoindol- 1, 3- dion
En blanding av eddiksyre-5-(4-amino-l,3-diokso-l,3-dihydro-isoindol-2-yl)-2,6-diokso-piperidin-3-yl-ester (1,5 g, 4,5 mmol) og p-toluensulfonsyre (0,46 g, 2,4 mmol) i 50 ml meta-noi ble oppvarmet til tilbakeløp i 22 timer. Suspensjonen ble filtrert og vasket med 2 x 30 ml metanol, og man oppnådde 4-amino-2-(5-hydroksy-2,6-diokso-piperidin-3-yl)-isoindol-1,3-dion som et gult faststoff (1,0 g, 79 %) med smeltepunkt 285-287 °C.
Eksempel 4
4-amino-2-(5-hydroksy-2,6-dioksopiperid-3-yl)isoindolin-l-on A. 3-( 4- amino- l- oksoisoindolin- 2- yl)- 2, 6- diokso- 5-acetoksypiperidin
En oppløsning av 3-(4-nitro-l-oksoisoindolin-2-yl)-2,6-diokso-5-acetoksypiperidin (0,9 g, 3,1 mmol) og 0,1 g 10 % Pd/C i 100 ml metanol rystes under 50-60 psi hydrogen i 3 timer. Suspensjonen filtreres gjennom en pute av Celite og vaskes med metanol for å gi 3-(4-amino-l-oksoisoindolin-2-yl)-2,6-diokso-5-acetoksypiperidin som renses ytterligere ved kolonnekromatografi.

En oppløsning av 3-(4-amino-l-oksoisoindolin-2-yl)-2,6-diokso-5-acetoksypiperidin (0,96 g, 3,5 mmol) og p-toluensulfonsyre (0,33 g, 1,8 mmol) i 10 ml metanol oppvarmes til tilbakeløp i 5 timer. Oppløsningsmidlet fjernes under vakuum, og man oppnår 4-amino-2-(5-hydroksy-2,6-dioksopiperid-3-yl)isoindolin-l-on som renses ytterligere ved kolonnekromatografi.

4-amino-2 -(3 - fluor-5-hydroksy-2,6-dioksopiperid-3-yl)isoindolin-1,3-dion

En oppløsning av 3-fluor-4-(4-nitro-l,3-dioksoisoindolin-2-yl)-2,6-diokso-5-acetoksypiperidin (1,0 g, 2,6 mmol) og 0,1 g 10 % Pd/C i 100 ml metanol rystes under et hydrogentrykk på 50-60 psi i 3 timer. Oppløsningsmidlet fjernes under vakuum for å gi 4-amino-2-(3-fluor-5-hydroksy-2,6-dioksopiperid-3-yl)isoindolin-1,3-dion som renses ytterligere ved kolonnekromatografi.

Tabletter som hver inneholder 50 mg 2-(2,6-diokso-3-hydroksypiperidin-5-yl)-4-aminoisoindolin-l,3-dion kan fremstilles på følgende måte:
De faste bestanddeler tvinges først gjennom en sikt med maskevidde 0,6 mm. Den aktive bestanddel, laktose, talkum, magnesiumstearat og halvparten av stivelsen blandes deretter. Den andre halvpart av stivelsen suspenderes i 40 ml vann, og denne suspensjon settes til en kokende oppløsning av polyetylenglykolen i 100 ml vann. Den resulterende pasta settes til de pulverformige stoffer, og blandingen granuleres, hvis nød-vendig ved tilsetning av vann. Granulatet tørkes over natten ved 35° C og tvinges gjennom en sikt med maskevidde 1,2 mm og presses til tabletter med diameter rundt 6 mm og som er konkave på begge sider.
Eksempel 13
Tabletter som hver inneholder 100 mg 2-(2,6-diokso-3-hydroksypiperidin-5-yl)-4-metylaminoisoindolin-l-on kan fremstilles på følgende måte:
Alle de faste bestanddeler tvinges først gjennom en sikt på 0,6 mm maskevidde. Den aktive bestanddel, laktose, magnesiumstearat og halvparten av stivelsen blandes deretter. Den andre halvpart av stivelsen suspenderes i 40 ml vann, og denne suspensjon settes til 100 ml kokende vann. Den resulterende pasta settes til de pulverformige stoffer, og blandingen granuleres, hvis nødvendig under tilsetning av vann. Granulatet tørkes over natten og tvinges gjennom en sikt med maskevidde 1,2 mm og presses til tabletter med diameter rundt 6 mm og som er konkave på begge sider.
Eksempel 14
Tabletter for tygging, hver inneholdende 75 mg 2-(2,6-diokso-3-hydroksy-5-metylpiperidin-5-yl)-4-metylisoindolin-l,3-dion kan fremstilles på følgende måte:
Alle de faste bestanddeler tvinges først gjennom en sikt med maskevidde 0,25 mm. Mannitolen og laktosen blandes og granuleres under tilsetning av en gelatinoppløsning, tvinges gjennom en sikt med maskevidde 2 mm, tørkes ved 50 °C og tvinges nok en gang gjennom en sikt med maskevidde 1,7 mm. 3-(3-et-oksy-4-metoksyfenyl)-N-hydroksy-3-ftalimidopropionamid, gly-sinet og sakkarinet blandes omhyggelig, mannitolen, laktose-granulatet, stearinsyren og talkum tilsettes så og det hele blandes grundig og presses til tabletter med diameter rundt 10 mm og som er konkave på begge sider og som har en bruddrille på den øvre side.
Eksempel 15
Tabletter som hver inneholder 10 mg 2-(2,6-diokso-3-hydroksy-piperidin-5-yl)-4-aminoisoindolin-l-on, kan fremstilles på følgende måte:
De faste bestanddeler tvinges først gjennom en sikt med maskevidde 0,6 mm. Deretter blir den aktive bestanddel, laktose, talkum, magnesiumstearat og halvparten av stivelsen blandet grundig. Den andre halvpart av stivelsen suspenderes i 65 ml vann, og denne suspensjon settes til en kokende oppløsning av polyetylenglykolen i 260 ml vann. Den resulterende pasta settes til de pulverformige stoffer, og det hele blandes og granuleres, hvis nødvendig under tilsetning av vann. Granulatet tørkes over natten ved 35 °C, tvinges gjennom en sikt med maskevidde 1,2 mm og presses til tabletter med diameter rundt 10 mm, konkave på begge sider og med en bruddrille på den øvre side.

En 0,2 % injeksjons- eller infusjonsoppløsning kan fremstilles, for eksempel på følgende måte:
2-(2,6-diokso-3-hydroksypiperidin-5-yl)-4-aminoisoindolin-l-on-hydroklorid oppløses i 1000 ml vann og filtreres gjennom et mikrofilter. Bufferoppløsningen tilsettes, og det hele gjøres opp til 2500 ml med vann. For å fremstille enhetsdose-former, blir porsjoner på 1,0 eller 2,5 ml hver innført i glassampuller som hver inneholder henholdsvis 2,0 eller 5,0 mg imid.

1. Forbindelse, karakterisert ved at den er valgt fra gruppen bestående av (a) et isoindolin med formelen:
der
karbonatomet angitt med <*> utgjør et chiralitetssenter;
X er -C(0)- eller -CH2-;
R<1> er Ci-8-alkyl eller -NHR3 ;
R<2> er hydrogen, Ci-8-alkyl eller halogen; og R<3> er hydrogen eller Ci_8-alkyl;
og (b) de syreaddisjonssalter av nevnte isoindolin som er i stand til protonering.
2. Forbindelse ifølge krav 1, karakterisert ved at R<2> er hydrogen, metyl eller fluor .
3. Forbindelse ifølge krav 2, karakterisert ved at R<2> er hydrogen.
4. Forbindelse ifølge krav 3, karakterisert ved at R<1> er amino.

14. Farmasøytisk preparat, karakterisert ved at det omfatter (i) en mengde av forbindelse ifølge krav 1 som ved administrering i et enkelt eller multippel doseregime er farmasøytisk effektivt; og (ii) en farmasøytisk akseptabel bærer.
15. Anvendelse av en forbindelse ifølge krav 1 til fremstilling av medikamenter for behandling av en sykdom valgt blant inflammatoriske sykdommer, autoimmune sykdommer, artritt, rheumatoid artritt, inflammatorisk tarmsykdom, Krohns sykdom, aftøse ulcere, kakeksi, po-ding-mot-vertsykdom, astma, adult respiratorisk distress syndrom og ervervet immun defekt syndrom.
16. Anvendelse av en forbindelse ifølge krav 1 for fremstilling av medikamenter for behandling av cancer.
17. Anvendelse av en forbindelse ifølge krav 1 for fremstilling av medikamenter for behandling av uønsket angiogenese.
