Description
Denne oppfinnelse angår en fremgangsmåte for fremstilling av nye 3-substituerte-2-oksindol-derivater som er nemmere av prostaglandin H2 syntase, 5-lipoksygenase og interleukin-1 biosyntese. Forbindelsene fremstilt i henhold til oppfinnelsen er egnet som nemmere av prostaglandin H2 syntase og interleukin-1 biosyntese, som sådan, og som analgetiske, antiinflammatoriske og antiartritiske midler ved behandling av kroniske inflammatoriske sykdommer. Farmasøytiske preparater kan inneholde slike 3-substituerte-2-oksindol-derivater og benyttes til å hemme prostaglandin H2 syntase og biosyntese av interleukin-1 og behandle kroniske inflammatoriske sykdommer i pattedyr.

hvor R<3> og R4 hver er H, fluor, klor, (Cj-CJ alkyl, (Cj-Cj-alkoksy eller trifluormetyl.
Patentet beskriver også at disse 2-oksindol-l-karboks-amider er hemmere av cyklooksygenase og lipoksygenase, har analgetisk aktivitet i pattedyr, og er egnet til behandling av smerte og lindring av symptomer ved kroniske sykdommer, så som inflammasjon og smerte forbundet med reumatoid artritt og osteoartritt.
US-patent 4.556.672 omtaler enkelte 3-acyl-substituerte-2-oksindol-l-karboksamider med formel
hvor X, Y og R<1> er som beskrevet ovenfor for forbindelsene i US-patent 4.569.942. Forbindelsene i US-patent 4.556.672 er
omtalt som forbindelser med samme aktivitet som de i det ovenfor nevnte US-patent 4.569.942.
US-patent 4.861.794 omtaler anvendelsen av forbindelser med formel
og de farmasøytisk akseptable basesalter derav, hvor X er H, Cl eller F, Y er H eller Cl og R er benzyl eller tienyl, som hemmere av biosyntese av interleukin-1 (IL-1) og til å behandle IL-1 medierte forstyrrelser og dysfunksjoner. PCT-søknad serie nr. PCT/US88/03658 av 18. oktober, 1988, beskriver ikke-steroide antiinflammatoriske midler med formel
hvor hver av X og Y er hydrogen, fluor eller klor;
R<1> er 2-tienyl eller benzyl; og R er alkanoyl, cykloalkyl-karbonyl, fenylalkanoyl, benzoyl og visse substituerte benzoylgrupper, tenoyl, omega-alkoksy-karbonylalkanoyl, alkoksykarbonyl, fenoksykarbonyl, 1-alkoksykarbonyloksy, alkylsulfonyl, metylfenylsulfonyl og dialkylfosfonat.
Interleukin-1 (IL-1) er rapportert å stimulere benresorpsjon både in vitro og in vivo. Hayward, M. og Fiedler-Nagy, CH. , Agents and Actions, 22., 251-254 (1987). Det er der også nevnt at IL-1, bl. a. induserer produksjonen av prostaglandin E2 (PGE2) . PGE2 er en stimulator for benresorpsjon og har vært anvendt ved tap av bensubstans. Se Hayward, M. Foreliggende oppfinnelse tilveiebringer en fremgangsmåte for fremstilling av nye 3-substituerte-2-oksindol-forbindelser med formel
og farmasøytisk akseptable salter derav, hvor R<2> er CONH2 n er 0 eller 1; og en av A og B er H og den annen er 3'-F, 5'-Cl, 5'-Br, 5'-I, 5'-OCH3, 5-SCH3, 5-OCH2CH3, 5'-COCH3, 5'-COOCH3, 5'-S02CH3, 5'-CON(C<H>3)2 eller 5'-S02N(C<H>3)2, eller A er 4'-Br og B er 5'-Br; eller X er 5-F og Y er 6-C1 og Q er og en av A og B er H og den annen er 4'-Cl; eller en av X og Y er H og den annen er Cl og Q er 5-metyltio-l,3,4-oksadiazolyl, 6-klorpyridinyl eller 5-brom-3-furyl. Mens de ovennevnte forbindelser med formel I er vist som enoler, er det underforstått at forbindelser med formel I kan anta sin tautomere keto-form, det vil si
Fremstilling av samtlige slike tautomere former ligger innenfor rammen av oppfinnelsen og betraktes som å være gjengitt med formel I. Substituenter på den eksocykliske dobbeltbinding i 3-stillingen i forbindelser med formel I kan dessuten ha syn- eller anti-form eller være en blanding av begge. Forbindelsene med formel I med strukturene
og blandinger av disse, faller således innenfor rammen av formel I, og alle slike isomerer ansees som å være gjengitt med formel I. En foretrukket gruppe av forbindelser er de hvor Q er
Mer foretrukne forbindelser er de hvor en av A og B er 3'-F,
5'-Cl, 5'-Br, 5'-I eller 4'-Cl, og den annen er H, eller A er 4'-Br og B er 5'-Br. Særlig foretrukne forbindelser er de som er beskrevet umiddelbart ovenfor hvor X er 5-F, Y er 6-C1, og en av A og B er H og den annen er 4'-Cl. Spesielt foretrukket er nevnte forbindelser hvor n er null.
Forbindelsene med den ovenfor angitte formel I er virk-somme som hemmere av prostaglandin H2 syntase (cyklooksygenase), som hemmere av 5-lipoksygenase og som hemmere av interleukin-1 (IL-1) biosyntese i pattedyr. Forbindelsene med formel I er således egnet til å hemme prostaglandin H2 syntase og IL-l-biosyntese i pattedyr. Forbindelsene med formel I er i tillegg til deres anvendelighet som hemmere, for seg, også nyttige som analgetiske, antiinflammatoriske og antiartritiske midler ved behandling av kroniske inflammatoriske sykdommer i pattedyr.
Eksempler på slike kroniske inflammatoriske sykdommer kan være psoriasis, reumatoid artritt og osteoartritt.
En fremgangsmåte for fremstilling av forbindelser med formel I er vist i Reaksjonsskjema A ovenfor, og beskrives i det følgende. De substituerte 2-oksindol-forbindelsene med formel IV fremstilles etter fremgangsmåtene beskrevet i US-patent 3.634.453, US-patent 4.556.672, US-patent 4.569.942, US-patent 4.695.571, EP 175551 og referanser angitt her. Disse inkorporeres herved med hele deres innhold. Karboksylsyre-forbindelsene med formel II fremstilles som beskrevet nedenfor og aktiveres ved å omsette forbindelsen med formel II med et molart overskudd av tionylklorid, eventuelt i nærvær av et reaksjons-inert oppløsningsmiddel. Passende reaksjons-inerte oppløsningsmidler er de som i det minste delvis vil oppløse én eller samtlige av reaktantene, og som ikke ugunstig vil påvirke hverken reaktanter eller produktet. Den resulterende karbonylklorid-forbindelse med formel II oppløses i et reaksjons-inert oppløsningsmiddel og tilsettes langsomt til en oppløsning, avkjølt til ca. 0°C, som omfatter en tilnærmet ekvimolar mengde av det substituerte 2-oksindol med formel IV og et molart overskudd av et basisk middel i et reaksjons-inert oppløsningsmiddel. Det reaksjons-inerte oppløsnings-middel er som beskrevet ovenfor, men i praksis benyttes et polart aprotisk oppløsningsmiddel, så som N,N-dimetylformamid, N,N-dimetylacetamid, N-metylpyrrolidon eller dimetylsulfoksyd. Et foretrukket oppløsningsmiddel er N,N-dimetylformamid. Et bredt utvalg av basiske midler kan benyttes i omsetningen mellom et karbonylklorid med formel III og en substituert 2-oksindol-forbindelse med formel IV. Foretrukne basiske midler er imidlertid tertiære aminer, så som trimetylamin, trietylamin, tributylamin, N-metylmorfolin, N-metylpiperidin, pyridin og 4-(N,N-dimetylamino)pyridin, med 4-(N,N-dimetylamino)pyridin som et særlig foretrukket basisk middel. Etter tilsetning av karbonylklorid-forbindelsen med formel III til den substituerte 2-oksindol-forbindelse med formel IV, får blandingen oppvarmes til ca. 25°C og reagere videre ved denne temperatur. Reaksjonstider på ca. 3 0 minutter til 2 timer er vanlige. Ved endt reaksjon surgjøres reaksjonsblandingen, hvorpå produktet gjenvinnes ved filtrering. Produktet kan deretter vaskes, tørkes og renses ytterligere etter standard-metoder, så som omkrystallisasjon.
En annen egnet fremgangsmåte for fremstilling av forbindelser med formel I hvor R<1> er H, omfatter tilkobling av
substituenten til 3-stillingen av den nødvendige 2-oksindol-forbindelse med formel ved å omsette en forbindelse med formel VI med et derivat av den passende syre med den ovenfor angitte formel II, ifølge fremgangsmåtene beskrevet i US-patent 4.556.672. De resulterende forbindelser med formel
omdannes deretter ved innføring av R<2> til de korresponderende forbindelser med formel I ovenfor, ifølge fremgangsmåter beskrevet i US-patent 3.634.453; US-patent 4.556.672; US-
patent 4.569.942; US-patent 4.695.571; EP 175551 og referanser angitt i disse.
Forbindelser med formel I kan også fremstilles ved den fremgangsmåte som er gjenstand for avdelt søknad 93.42 68.
Enkelte av karboksylsyre-forbindelsene med formel II er kjente, og karboksylsyre-forbindelsene med formel II frem-
stilles etter kjente fremgangsmåter, eller fremgangsmåter analoge med disse. Forbindelsene med formel I er sure og kan danne basesalter. Samtlige slike basesalter ligger innenfor oppfinnelsens ramme og kan fremstilles etter konvensjonelle metoder. De kan for eksempel ganske enkelt fremstilles ved å bringe de sure og basiske deler i kontakt, vanligvis i et støkiometrisk forhold, i et vandig, ikke-vandig eller delvis vandig medium. Saltene gjenvinnes enten ved filtrering, ved utfelling med et ikke-oppløsende med påfølgende filtrering, fordampning av oppløsningsmidlet eller, når det gjelder vandige oppløsninger, ved lyofilisering. Typiske salter av forbindelsene med formel I som kan fremstilles er primære, sekundære og tertiære aminsalter, alkalimetallsalter og jord-alkalimetallsalter. Særlig verdifulle er etanolamin-, dietanolamin- og trietanolaminsaltene.
Egnede basiske midler for saltdannelse tilhører både organiske og uorganiske typer, og de innbefatter organiske aminer, alkalimetallhydroksyder, alkalimetallkarbonater, alkalimetallbikarbonater, alkalimetallhydrider, alkalimetall-alkoksyder, jordalkalimetallhydroksyder, jordalkalimetall-karbonater, jordalkalimetallhydrider og jordalkalimetallalk-oksyder. Representative eksempler på slike baser er primære aminer, så som n-propylamin, n-butylamin, anilin, cykloheksyl-amin, benzylamin, p-toluidin, etanolamin og glukamin; sekundære aminer, så som dietylamin, dietanolamin, N-metyl-glukamin, N-metylanilin, morfolin, pyrrolidin og piperidin; tertiære aminer, så som trietylamin, trietanolamin, N,N-dimetylanilin, N-etylpiperidin og N-metylmorfolin; hydroksyder, så som natriumhydroksyd; alkoksyder, så som natriumetoksyd og kaliummetoksyd; hydrider, så som kalsium-hydrid og natriumhydrid; og karbonater, så som kaliumkarbonat og natriumkarbonat.
Forbindelsenes evne til å hemme interleukin-1 biosyntese kan demonstreres gjennom den nedenfor beskrevne prosedyre.
C3H/HeN mus (Charles River, Wilmington, Massachusetts, U.S.A.) avlives ved servikal dislokasjon, hvorpå deres abdomen gis en spray med 70% etanol for å hindre bakterievekst ved den etterfølgende fremstilling av cellulære preparater. Hver mus injiseres peritonealt 8 ml RPMI<1> inneholdende 5% FCS<2>, penicillin-streptomycin (100 enhet er/ml-100 /xg/ml) og glutamin (2 mM) . Peritoneum gnis for å frigjøre celler. Deretter gjøres et snitt gjennom abdomen-huden for å blottlegge det under-liggende muskellag. Peritonealvæsken fjernes med en nål (20 gauge) ved å føre nålen med kromningen ned, gjennom det blottlagte muskellag like nedenfor sternum. Peritonealvæsken fra 6 mus slås sammen i et konisk plastrør og undersøkes mikroskopisk på bakteriell forurensning. Uforurenset væske sentrifugeres ved ca. 600 x g i 6 minutter, hvorpå supernatanten avdekanteres. Cellepellets fra 5 til 6 rør kombineres og resuspenderes i totalt 2 0 ml RPMI-FCS<3>. Celleantallet fastslås deretter ved bruk av et hemacytometer og celle-leve-dyktigheten bestemmes ved trypanblått-farving, også ved bruk av et hemacytometer. Cellene fortynnes deretter til 3 x 10<6 >celler/ml ved bruk av RPMI-FCS. Til brønnene i en 35 mm brønn-plate tilsettes 1 ml av ovennevnte cellesuspensjon. Cellene inkuberes i 2 timer ved 37°C i 5% C02-atmosfære for å få makro-fagene til å klebe seg til brønnveggene. Supernatanten fjernes ved kraftig omhvirvling og dekantering av brønnene. Adherende celler (dvs. makrofager) vaskes to ganger med RPMI-SF<4>. Brønner som inneholder adherende celler tilsettes 1 ml av testforbindelsen i konsentrasjoner varierende fra 0,1 til 100 Mg/ml i RPMI-SF eller 1 ml RPMI-SF som kontroll. Deretter

tilsettes 100 /il LPS<5> i RPMI-SF (1 mg/5 ml) til hver brønn. Platene inkuberes ved 37°C i en 5% C02-atmosfære i 24 timer. Overstående væske fra hver brønn tas ut og undersøkes omgående på IL-1 eller nedkjøles eller dypfryses for senere under-søkelse.
Supernatantene undersøkes kvantitativt for IL-1 etter den nedfor beskrevne metodikk for bestemmelse av reseptorbinding. En standardkurve frembringes på følgende måte. EL4-6,1 murin thymoma-celler [10-15 x 10<6> celler i 0,4 ml bindingsbuffer (RPMI 1640, 5% FCS, 25 mM HEPES, 0,01% NaN3, pH 7,3)] tilsettes til varierende mengder av umerket murin rIL-la [rekombinant IL-la produsert i Escherichia coli fra den publiserte amino-syre-sekvens 115-270 for IL-la, Lomedico, P. M., et al., Nature 312, 458-462 (1984)] (40 pg til 40 ng i 0,5 ml buffer) og inkuberes i 1 time ved 4°C under kontinuerlig risting, hvorpå 0,8 ng (0,1 ml) human <125>I-rIL-l/8 (New England Nuclear, Boston, Massachusettes, U.S.A.) tilsettes og ristingen fortsettes i ytterligere 3 timer. Prøver filtreres med et Yeda-apparat (Linca Co., Tel-Aviv, Israel) gjennom Whatman GF/C2,4 cm glassfiber-filter (blokkert med 0,5% melkepulver i 2 timer ved 37°C) og vaskes én gang med 3 ml iskald buffer. Filterne telles i en Searle gamma-teller, og ikke-spesifikk binding fastsettes som den CPM som bindes i nærvær av 200 ng umerket rIL-la. Det konstrueres en Hill kalibreringskurve ved å avsette log (Y/100-Y) mot log C, hvor Y utgjør prosent-andelen av kontroll <125>I-rIL-l/?-binding og C er konsentrasjonen av umerket rIL-la. En lineær minstekvadraters linje tilpasses gjennom Y-verdier mellom 20 og 80%. For å beregne IL-l-nivåene i supernatantene oppnådd som beskrevet ovenfor, gjentas prosedyren med fortynnede supernatanter i stedet for rIL-la, hvorpå de målte prosentuelle bindingsverdier benyttes for å bestemme IL-1-konsentrasjoner fra et standard Hill-diagram. Den evne som forbindelsene med formel I har til å hemme prostaglandin H2 syntase og 5-lipoksygenase, demonstreres gjennom den nedenfor beskrevne metode. Ved å benytte denne måles nivåene av kjente produkter av prostaglandin H2 syntase og 5-lipoksygenase for celler behandlet med testforbindelsen, hvor hemming av prostaglandin H2 syntase og/eller 5-lipoksygenase gir seg utslag i en reduksjon i mengden av, eller fravær av, de kjente produkter fra disse enzymer.
RBL-1 celler, holdt i et monolag, dyrkes 1 til 2 dager i Spinner-kultur i Minimum Essential Medium (Eagle) med Earle's salter pluss 15% føtalt storfeserum supplert med antibiotisk/- antimykotisk oppløsning (Gibco) etter metodikk av Jakschik, B. A., et al., Nature, 287:51-52 (1980). Cellene vaskes to ganger og resuspenderes i kald RPMI 1640 til en celle-densitet på 4 x 10é celler/ml. Deretter likevektsinnstilles en 0,25 ml alikvot av testforbindelsen i den ønskede konsentrasjon i RPMI 1640 ved 37°C i 5 minutter. Den stabiliserte alikvot tilsettes til en 0,25 ml alikvot forvarmet cellesuspensjon, og blandingen inkuberes ved 37°C i 5 minutter. En 10 /xl oppløsning inneholdende "c-arakidonsyre og A-23187 (calsium-ionophor, Sigma Chemical) tilsettes, hvorpå blandingen inkuberes ved 37°C i ytterligere 5 minutter. Deretter tilsettes 267 /il acetonitril-/0,3% eddiksyre, hvoretter blandingen får stå på is i 30 minutter. Røret som inneholder blandingen vibreres, klarnes ved sentrifugering (3000 rpm., 10 minutter), hvoretter supernatanten avdekanteres og resentrifugeres i 2 minutter i en mikrosentrifuge ved høy hastighet. En 100 /xl alikvot av super-natanten analyseres deretter ved HPLC på en Perkin-Elmer-HS (3 mikron) kolonne ved bruk av et gradient-elueringssystem av acetonitril/H20 med 0,1% trifluoreddiksyre ved en strømnings-hastighet på 2 ml/min. Radioaktivitetsregistrering foretas med en Berthold LB504 Radioactivity Monitor forsynt med en 800 /il kyvette under blanding av 2,4 ml/min. av Omnifluor (varemerke tilhørende New England Nuclear, Boston, Massachusetts, U.S.A.) med kolonne-utløpet. Beregning av den eluerte radioaktivitet foretas ved bruk av en Spectra Physics SP4200 integrator. De således oppnådde data benyttes i et data-reduksjonsprogram hvor integrasjonsenhetene for hvert produkt beregnes som prosent av de totale integrasjonsenheter og sammenlignes med gjennomsnittlige kontrollnivåer.
Forbindelsene med formel I har analgetisk aktivitet. Denne aktivitet ble vist på mus ved at de oppviste blokkering av abdominal strekking som induseres ved administrasjon av 2-fenyl-1,4-benzokinon (PBQ). Metoden er basert på arbeidet til Siegmund et al., Proe. Soc. Exp. Biol. Med., 95, 729-731
(1957), tilpasset masseundersøkelse (se dessuten Milne & Twomey, Agents and Actions, 10, 31-37 [1980]). Musene ble fastet over natten før administrasjon og testing.
Forbindelsene med formel I ble oppløst eller suspendert i et bæremiddel bestående av etanol (5%), "emulphor 620" (en blanding av polyoksyetylen-fettsyreestere, 5%) og saltopp-løsning (90%). Dette bæremiddel tjener også som kontroll. Dosen utgjorde en logaritmisk skala (dvs..0,32, 1,0, 3,2, 10, 32....mg/kg). Administrasjonsmåten var per os, med konsentrasjoner som ble variert for å gi et konstant doseringsvolum på 10 ml/kg legemsvekt. Den ovennevnte fremgangsmåte til Milne & Twomey ble benyttet for å bestemme effektivitet og styrke. Musene ble behandlet med forbindelsene peroralt og fikk 1 time senere PBQ, 2 mg/kg, intraperitonealt. Hver enkelt mus ble deretter anbragt i et oppvarmet plastbur (lucite). 5 minutter etter PBQ-administrasjon ble antallet av abdominale sammentrekninger registrert i de påfølgende 5 minutter. Graden av analgetisk beskyttelse (% MPE) ble beregnet på basis av undertrykkelse av abdominale sammentrekninger i forhold til antallet fra kontrolldyr undersøkt på samme dag. Minst 4 slike bestemmelser (N=5) gir dose-respøns-data for beregning av en MPE50, det beste estimat av den dose som reduserer abdominale sammentrekninger til 50% av kontroll-nivåene.
Forbindelsene med formel I har også inflammatorisk aktivitet. Denne aktivitet ble demonstrert på rotter etter en metode basert på "standard carrageenin induced rat foot edema test" (Winter et al., Proe. Soc. Exp. Biol. Med., 111, 544
[1963]).
Uanestetiserte, voksne albino hannrotter med 150-190 g kroppsvekt ble nummerert, veiet og påført en blekkmarkering på den høyre laterale malleol. Hver fot ble dyppet i kvikksølv nøyaktig til blekkmerket. Kvikksølvet befant seg i en glass-sylinder forbundet med en Statham trykk-transduktor. Signalet fra transduktoren ble ført via en kontrollenhet til et mikro voltameter. Volumet av kvikksølv fortrengt av den neddyppede fot ble avlest. Medikamentet ble gitt med sonde. 1 time etter medikamentadministrasjon ble det indusert ødem ved injeksjon av 0,05 ml 1% oppløsning av carrageenin i plantarvevet i den markerte fot. Umiddelbart deretter ble volumet av den injiserte fot målt. Økningen i fotvolum 3 timer etter injeksjonen av carrageenin utgjorde den individuelle inflammatoriske respons.

Den analgetiske virkning av forbindelsene med formel I gjør dem egnet for akutt administrasjon for kontroll av smerte hos pattedyr, f.eks. postoperative smerter eller traumatiske smerter. Forbindelsene med formel I er dessuten egnet for kronisk administrasjon til pattedyr for å lette symptomer på kroniske sykdommer så som inflammasjon av reumatoid artritt, og smerte forbundet med osteoartritt og andre muskel/skjelett-forstyrrelser.
Forbindelsenes evne til å hemme IL-1 biosyntese gjør dem som sådanne, egnet som IL-1 biosyntesehemmere. Det gjør dem også egnet ved behandling av IL-l-medierte forstyrrelser og immune dysfunksjoner i pattedyr. De nevnte IL-l-medierte forstyrrelsene innbefatter, men er ikke begrenset til, metabolismeforstyrrelser i ben og bindevev, så som osteoporose, periodontale sykdommer og "tissue scarring". IL-l-medierte immune dysfunksjoner innbefatter, men er ikke begrenset til, allergi og psoriasis.
Forbindelsenes evne til å hemme prostaglandin H2 syntase, gjør dem som sådanne, egnet som prostaglandin H2 syntase-hemmere i og med at virkningen av dette enzym som kjent inngår i patogenesen av artritiske ledd i pattedyr.
Når en forbindelse med formel I eller et farmasøytisk akseptabelt salt derav, skal benyttes som hemmer av IL-1, kan en hemmer av prostaglandin H2 syntase, et analgetisk middel eller et antiinflammatorisk middel administreres til pattedyr, enten for seg, eller, fortrinnsvis, i kombinasjon med farma-søytisk akseptable bære- eller fortynningsmidler i et farma-søytisk preparat i overensstemmelse med vanlig farmasøytisk praksis. En forbindelse kan gis oralt eller parenteralt. Parenteral administrasjon innbefatter intravenøs, intramuskulær, intraperitoneal, subkutan og lokal administrasjon.
I et farmasøytisk preparat som inneholder en forbindelse med formel I, eller et farmasøytisk akseptabelt salt derav, vil vektforholdet mellom bæremiddel og virkestoff normalt ligge i området fra 1:4 til 4:1, fortrinnsvis 1:2 til 2:1. Forholdet vil imidlertid variere fra situasjon til situasjon med slike faktorer som virkestoffets oppløselighet, den til-tenkte dosering og den aktuelle administrasjonsmåte.
For oral bruk av en forbindelse med formel I kan forbindelsen gis, for eksempel i form av tabletter eller kapsler eller som en vandig oppløsning eller suspensjon. Når det er tale om tabletter for oral bruk, innbefatter vanlig benyttede bæremidler laktose og maisstivelse, samt glattemidler så som magnesiumstearat. For oral administrasjon i kapselform er egnede fortynningsmidler laktose og tørket maisstivelse. Når det er behov for vandige suspensjoner for oral bruk, kombineres virkestoffet med emulgerings- og/eller suspenderingsmidler. Om ønskes kan enkelte søtnings- og/eller smaksforbedrende midler tilsettes. For intramuskulær, intraperitoneal, subkutan og intravenøs bruk fremstilles vanligvis sterile oppløsninger av virkestoffet, hvorunder pH av opp-løsningene justeres og tilsettes buffer. For intravenøs bruk bør den totale konsentrasjon av oppløst stoff kontrolleres for å holde preparatet isotonisk.
Ved human anvendelse av en forbindelse med formel I eller et salt derav, vil døgndosen normalt avgjøres av behandlende lege. Doseringen vil dessuten avhenge av pasientens alder, vekt og respons, samt av symptomenes grad og styrken av den gitte forbindelse. Ved akutt administrasjon for å lindre smerte vil imidlertid en effektiv analgetisk dose i de fleste tilfeller være mellom ca. 5 mg og 500 mg alt etter behov (f.eks. hver 4. til 24. timer).
For kronisk administrasjon for å lette (behandle) inflammasjon og smerte, hemme IL-1 biosyntese og/eller hemme prostaglandin H2 syntase, vil i de fleste tilfeller en virksom dose være fra ca. 5 mg til 1,0 g per dag, fortrinnsvis 50 mg til 500 mg per dag, som enkle eller avdelte doser. Det kan imidlertid i enkelte tilfeller være nødvendig å bruke doseringer utenfor disse grensene.
Fremstilling 1
5- iod- 2- tiofenkarboksylsyre
Tittelforbindelsen er beskrevet av Schick, J. W., et al., J. Am. Chem. Soc. 70:286 (1948), og ble fremstillet etter følgende fremgangsmåte. Et 25 ml (62,5 mmol) volum av 2,5M heksanoppløsning av n-butyllitium ble langsomt tilsatt via en sprøyte til en avkjølt (tørr-is/2-propanol) 100 ml tetrahydro-furanoppløsning av 9,0 ml (64,2 mmol) diisopropylamin. Opp-løsningen ble holdt ved en temperatur lavere enn -60°C i løpet av n-butyllitium-tilsetningen. Etter tilsetning ble kjølebadet fjernet og oppløsningen fikk anta romtemperatur (22°C) og ble deretter avkjølt på nytt til -60°C. Til den avkjølte reaktor ble det langsomt tilsatt 3,2 g (25,0 mmol) 2-tiofenkarboksylsyre oppløst i 100 ml tetrahydrofuran. 30 minutter etter endt tilsetning av 2-tiofénkarboksylsyre, ble ca. 17,2 g
(87,8 mmol) jodtrifluormetan kondensert over i reaksjonsblandingen. Etter 5 minutter ble kjølebadet fjernet og blandingen oppvarmet til 0°C, hvorpå reaksjonen ble avbrutt med 50 ml vann. Den basiske vandige oppløsning ble vasket med
500 ml dietyleter. Eteroppløsningen ble ekstrahert med 50 ml IN natriumhydroksyd og de to vandige oppløsningene kombinert og vasket med eter. Den basiske oppløsningen ble surgjort og ekstrahert tre ganger med 100 ml dietyleter. Tørking av den organiske oppløsning med vannfri magnesiumsulfat og påfølgende filtrering og konsentrering ga et fast råprodukt. Delvis rensing ble oppnådd ved utfelling av det faste produkt på nytt fra varm vandig etanol for å gi 3,79 g av det ønskede produkt i svakt forurenset form som en blanding av mørkerødt faststoff og gule krystaller. Omkrystallisasjon av den faste blanding ga 2,18 g (8,58 mmol, 34% utbytte) ren tittelforbindelse som lysegule nåler, smp. 132-134°C (heksaner).

Til en oppløsning av 5-formyl-2-tiofenkarboksylsyre (fremstillet ifølge Carpenter, A. J., et al., Tetrahedron 4.1:3808 (1985)) (2,75 g, 17,61 mmol) i 75 ml tetrahydrofuran ble det tilsatt l,l'-karbonyldiimidazol (3,71 g, 22,88 mmol) og oppløsningen omrørt under tørr argon i 2 1/2 time og behandlet med et overskudd av dimetylamin-gass. Oppløsningen ble konsentrert i vakuum til en olje som ble oppløst i etylacetat (100 ml) og ekstrahert med IN saltsyre (2 x 50 ml) og deretter med 5% natriumbikarbonat (2 x 50 ml). Hvert av de vandige lag ble vasket med etylacetat (2 x 50 ml) og de kombinerte organiske lagene tørket (magnesiumsulfat). Konsentrering i vakuum førte til et blekgult faststoff (2,42 g, 75%) .

5- r( N. N- dimetylamino) karbonyl1- 2- tiofenkarboksylsyre
En 2,39 g (13,04 mmol) porsjon av det rå 5-[(N,N-dimetylamino)karbonyl]-2-tiofenkarboksaldehyd ble tilsatt til en omrørt suspensjon av sølvoksyd fremstillet ved tilsetning av 2,29 g (57,13 mmol) natriumhydroksyd og 5,85 g (34,44 mmol) sølvnitrat i 100 ml vann. Etter omrøring ved romtemperatur i 15 minutter og filtrering gjennom et lag kiselgur, ble filtratet surgjort fra pH 12 til 2 med konsentrert saltsyre og ekstrahert med etylacetat. Ekstraktene ble tørket (magnesiumsulfat) og konsentrert i vakuum for å gi et hvitt faststoff (2,01 g, 77%). En analyseprøve ble oppnådd ved utgnidning med varm etylacetat, smp. 158-159°C

Metyl 5- formyl- 2- tiofenkarboksylat Tittelforbindelsen er beskrevet av Gronowitz, S., et al., Arkiv, for Kemi. 21:265 (1963) , og ble fremstillet etter følgende fremgangsmåte. Metyljodid (4,36 g, 30,74 mmol) ble tilsatt til en omrørt suspensjon av 5-formyl-2-tiofenkarboksylsyre (fremstillet ifølge Carpenter, A. J., et al., Tetrahedron 4JL:3808 (1985)) (4,00 g, 25,61 mmol) og natriumkarbonat (9,50 g, 89,65 mmol) i 75 ml N,N-dimetylformamid. Etter omrøring over natten ved romtemperatur ble blandingen helt over i vann (350 ml), mettet med fast natriumklorid og ekstrahert med etylacetat. De kombinerte ekstraktene ble vasket med saltoppløsning, tørket (magnesiumsulfat) og konsentrert i vakuum til et grått faststoff (3,83 g, 88%), smp. 85-87°C.

5- metoksykarbonyl- 2- tiofenkarboksylsyre Tittelforbindelsen er beskrevet av Benkeser, R. A., et al., J.O.C. 38, 3660 (1973) og i Britisk patent 705950, og ble fremstillet etter den følgende fremgangsmåte. En omrørt opp-løsning av metyl 5-formyl-2-tiofenkarboksylat (2,00 g,
11,75 mmol) i 100 ml aceton ble behandlet dråpevis med Jones' reagens (9 ml) . Etter endt tilsetning ble blandingen omrørt ved romtemperatur i 1 time, overskudd av oksydant dekomponert med isopropanol og blandingen filtrert gjennom kiselgur. Aceton ble fjernet i vakuum, residuet oppløst i etylacetat
(75 ml) og oppløsningen tørket over magnesiumsulfat. Filtrering og konsentrering førte til et gult faststoff
(1,60 g, 73%). En analyseprøve ble oppnådd ved utgnidning med varm etylacetat, smp. 186-189°C.

Litiumdiisopropylamid ble fremstillet ved langsom tilsetning av 10,5 ml (26,3 mmol) 2,5M n-butyllitium i heksaner til en avkjølt (2-propanol/tørr-is) tetrahydrofuran-oppløsning (200 ml) av diisopropylamin (5,0 ml, 35,7 mmol) mens reaksjonstemperaturen ble holdt lavere enn -60°C. Etter 5 minutter ble reaksjonsblandingen oppvarmet til romtemperatur i 30 minutter og deretter på nytt avkjølt til under -70°C. En 100 ml tetrahydrofuranoppløsning av 3,4 g (17,8 mmol) 2-(N,N-dimetylaminosulfonyl)tiofen (fremstillet ifølge Slocum, D. W., et al., JOC 38, 4189 (1973)) ble langsomt tilsatt mens reaksjonstemperaturen ble holdt lavere enn -70°C. Etter full-ført tilsetning ble reaksjonsblandingen omrørt i 30 minutter, hvorpå et overskudd av karbondioksyd ble boblet gjennom opp-løsningen. Oppløsningen ble deretter oppvarmet til 0°C og reaksjonen avbrutt med 50 ml IN natriumhydroksyd. En 300 ml porsjon dietyleter ble tilsatt til den vandige tetrahydro-furanoppløsningen, hvorpå fasene ble separert i en skille-trakt. Det organiske lag ble ekstrahert med 50 ml IN natriumhydroksyd. Begge de basiske vandige oppløsningene ble kombinert, vasket med 50 ml dietyleter og surgjort med konsentrert saltsyre. Den sure vandige blandingen ble ekstrahert med dietyleter (2 x 100 ml). Eteroppløsningen ble vasket med saltoppløsning, tørket over magnesiumsulfat, filtrert og konsentrert i vakuum til 3,66 g (15,6 mmol) ønsket tiofenkarboksylsyre som et farveløst faststoff, smp. 184-186°C (litt. smp. 170-172°C) . Totalutbyttet var 87%.

Etter tilbakeløpsbehandling i 1,5 timer ble overskudd av tionylklorid inndampet, hvilket etterlot 1,88 g rå 4-klor-2-tiofenkarbonylklorid som en mørkebrun olje. Syrekloridet ble oppløst i 10 ml N,N-dimetylformamid og langsomt tilsatt til en avkjølt (is-vann) 40 ml N,N-dimetyl-formamidoppløsning av 1,75 g (8,33 mmol) 5-klor-2-oksindol-l-karboksamid og 3,05 g (25,0 mmol) 4-(N,N-dimetylamino)pyridin. Reaksjonen var fullstendig i løpet av 1 time. Blandingen ble helt over i 100 ml IN saltsyre, hvilket førte til dannelse av et bunnfall. Det rå faststoffet ble frafiltrert, tørket og omkrystallisert for å gi 1,89 g (5,3 mmol, 64% utbytte) av tittelforbindelsen som gule nåler, smp. 212-214°C (2-butanon).

En 2,44 g (15,0 mmol) kommersiell prøve av 5-klor-2-tiofenkarboksylsyre og 10 ml tionylklorid ble omsatt ifølge fremgangsmåten i referanse-eksemplet. Råutbyttet av 5-klor-2-tiofenkarbonylklorid var 2,64 g som et oljeaktig faststoff. Dette materiale ble deretter koblet til 2,42 g (11,5 mmol) 5-klor-2-oksindol-l-karboksamid i nærvær av 3,52 g (28,8 mmol) 4-(N,N-dimetylamino)pyridin som i referanse-eksemplet. Opparbeidning ga 4,3 3 g fuktig råprodukt. Tørking og omkrystallisasjon ga 2,99 g (8,42 mmol, 73% utbytte) av tittelforbindelsen som et gult krystallinsk materiale, smp. 220-222°C (2-butanon).

Ved å følge fremgangsmåten i referanse-eksemplet, ble 2,07 g (10,0 mmol) kommersielt tilgjengelig 5-brom-2-tiofenkarboksylsyre omsatt med 10 ml tionylklorid for å gi 2,35 g rå 5-brom-2-tiofenkarbonylklorid som en rød olje. Ved å følge fremgangsmåten i referanse-eksemplet, ble 1,96 g (7,72 mmol) 5-jod-2-tiofenkarboksylsyre (fremstillet som beskrevet i fremstilling 1) blandet med 10 ml tionylklorid og oppvarmet under tilbakeløpskjøling. Reaksjonen førte til 2,10 g rå 5-jod-2-tiofenkarbonylklorid som et gult faststoff. Dette gule faststoffet ble oppløst i 10 ml N,N-dimetyl-formamidoppløsning og langsomt tilsatt til en 40 ml N,N-dimetylformamidoppløsning av 1,75 g (8,33 mmol) 5-klor-2-oksindol-l-karboksamid og 3,05 g (25 mmol) 4-(N,N-dimetylamino)pyridin ifølge referanse-eksemplet. Opparbeidning ga 3,18 g urent produkt som et orange faststoff. Omkrystallisasjon fra tetrahydrofuran ga 1,47 g (3,29 mmol, 40% utbytte) av ren tittelforbindelse som fine orange krystaller, smp. 230-232°C.

Ved bruk av fremgangsmåten i referanse-eksemplet, ble 2,86 g (10,0 mmol) kommersielt tilgjengelig 4,5-dibrom-2-tiofenkarboksylsyre tilsatt til 10 ml tionylklorid for å gi en heterogen blanding. Oppvarming av blandingen førte til at oppløsningen ble homogen. Konsentrering av reaksjons-oppløsningen ga 3,15 g rå 4,5-dibrom-2-tiofenkarbonylklorid som en brun olje. Soc, P.T.I., 791 (1983)) ble omdannet til 1,93 g av det korresponderende-syreklorid ved omsetning med 10 ml tionylklorid. Syrekloridet ble direkte omsatt med 1,75 g (8,33 mmol) 5-klor-2-oksindol-l-karboksamid i nærvær av 3,05 g (25 mmol) 4-(N,N-dimetylamino)pyridin som beskrevet i Eksempel 32. Vandig opparbeidning ga 3,02 g av et orange faststoff. Omkrystallisasjon av det urene orange faststoff fra tetrahydrofuran førte til 1,31 g (3,57 mmol, 43% utbytte) rent 5-klor-3-(5-metyltio-2-tenoyl)-2-oksindol-l-karboksamid som et orange faststoff. Smeltepunktsbestemmelsen viste at materialet først smeltet ved 180°C, hvorpå det størknet og igjen smeltet ved 247-250°C (dekomp.).

Acylering av 0,94 g (4,46 mmol) 5-klor-2-oksindol-l-karboksamid med 1,02 g (5,35 mmol) 5-etoksy-2-tiofenkarbonyl-klorid i nærvær av 1,37 g (11,23 mmol) 4-(N,N-dimetylamino)-pyridin førte til 1,50 g rått, gult faststoff. Omkrystallisasjon av det rå faststoff i eddiksyre førte til 0,20 g (0,55 mmol, 12% utbytte) ren tittelforbindelse som et gult faststoff, smp. 183-185°C.

Tittelforbindelsen ble fremstillet ifølge fremgangsmåten i referanse-eksemplet. I det første trinn ble 2,0 g (11,75 mmol) 5-acetyl-2-tiofenkarboksylsyre (fremstillet ifølge Thames, S. F., et al., J. Het. Chem. 2:104 (1966)) behandlet med 15 ml tionylklorid. Fordampning av overskudd av tionylklorid ga et gummiaktig residuum som ble utgnidd med karbontetraklorid for å gi 0,92 g (4,88 mmol, 42% utbytte) 5-acetyl-2-tifenkarbonylklorid som et lyst orange faststoff, smp. 78-80°C. Deretter ble 0,90 g (4,77 mmol) syreklorid omsatt med 0,83 g (3,95 mmol) 5-klor-2-oksindol-l-karboksamid i nærvær av 1,22 g (9,97 mmol) 4-(N,N-dimetylamino)pyridin for å gi 1,42 g (3,91 mmol, 99%) ren tittelforbindelse, etter vandig sur opparbeidning og tørking, som et orange-gult faststoff, smp. 218-221°C.

En enkelt omkrystallisasjon fra eddiksyre ga 2,31 g (5,80 mmol, 70% utbytte) rent 5-klor-3-(5-metylsulfonyl-2-tenoyl)-2-oksindol-l-karboksamid som et dyp-orange faststoff, smp. 212-214°C.

Ved å følge fremgangsmåten i referanse-eksemplet, ble 1,25 g (6,27 mmol) 5-[(N,N-dimetylamino)karbonyl]-2-tiofenkarboksylsyre (fremstillet som beskrevet i fremstilling 3) omdannet til 1,32 g (6,08 mmol 97% utbytte) av det korresponderende syreklorid, smp. 109-111°C ved omsetning med overskudd av tionylklorid. Kobling av 5-[(N,N-dimetylamino)-karbonyl]-2-tiofenkarbonylklorid-til 1,06 g (5,05 mmol) 5-klor-2-oksindol-l-karboksamid i nærvær av 1,56 g (12,73 mmol) 4-(N,N-dimetylamino)pyridin ga et orange faststoff etter opparbeidning. Omkrystallisasjon av faststoffet førte til 0,80 g (2,05 mmol, 40% utbytte) ren tittelforbindelse som et orange faststoff, smp. 219-220°C (eddiksyre).

Overskudd av tionylklorid (3,0 ml, 41,1 mmol) og 0,89 g (6,10 mmol) 3-fluor-2-tiofenkarboksylsyre (fremstillet ifølge Corral', C. , et al., Heterocycles 22:1431 (1985)) ble blandet i 10 ml toluen og omsatt under tilbakeløp i tre timer. Dette ga det korresponderende syreklorid som en gul olje etter opparbeidning. Det gule syreklorid ble oppløst i 3 ml N,N-dimetylformamid og omsatt med 1,28 g (6,10 mmol) 5-klor-2-oksindol-l-karboksamid i nærvær av 1,64 g (13,42 mmol) 4-(N,N-dimetylamino)pyridin i 5 ml N,N-dimetylformamid. Opparbeidning ga 1,7 g gult faststoff. Omkrystallisasjon av dette materialet ga 0,65 g (31% utbytte) av tittelforbindelsen som gule nåler,
smp. 235-240°C (eddiksyre).

Ved bruk av fremgangsmåten i Referanseeksempel, ble en
30 ml N,N-dimetylformamidoppløsning av 958 mg (4,55 mmol) 5-klor-2-oksindol-l-karboksamid og 1,50 g (12,28 mmol) 4-(N,N-dimetylamino)pyridin omsatt med 975 mg (5,46 mmol) 5-metyltio-1,3,4-oksadiazol-2-karbonylklorid (US-patent 4.001.238). Etter sur opparbeidning ble det oppnådd et rått orange faststoff (1,25 g, 78% utbytte). Suspensjon i varm iseddik og deretter filtrering førte til den rene tittelforbindelse (710 mg,
2,01 mmol, 44%) som et lysegult faststoff, smp. 297-299°C.
