Description
Combinatie van botmateriaal en collageen voor het repareren van beschadigde gewrichten.
De onderhavige uitvinding heeft betrekking op het terrein van het repareren van beschadigde of defecte gewrichten.
Gelede gewrichten bij zoogdieren zijn bedekt met 5 gewrichtskraakbeen, dat rechtstreeks contact van de tegenover elkaar liggende botoppervlakken belet en een soepele beweging van de gelede botten ten opzichte van elkaar mogelijk maakt.
Er zijn diverse voorstellen gedaan voor het repareren van beschadigingen en defecten van gewrichtskraakbeen. 10 Hiertoe behoren implantatie van gekweekte chondrocyten op de plaats van de kraakbeenbeschadiging en het bedekken van de beschadiging met een collageenplaatje.
Soms strekt de beschadiging of het defect in een geleed gewricht zich dieper dan het gewrichtskraakbeen in het 15 onderliggende bot uit. Er blijft binnen dit vakgebied een behoefte bestaan aan materialen en werkwijzen voor het repareren van gelede gewrichten waarbij een beschadiging of defect zich door het kraakbeen heen in het bot uitstrekt.
In overeenstemming met de onderhavige uitvinding 20 omvat een botmineraalproduct voor gebruik bij het repareren van gecombineerde kraakbeendefecten en botdefecten bij gelede botgewrichten poreuze botmineraaldeeltjes, afkomstig van natuurlijk bot, met een kristalstructuur die in hoofdzaak die van natuurlijk bot is en die in hoofdzaak vrij zijn van elk 25 endogeen organisch materiaal, waarbij de deeltjes ten minste aan een oppervlak daarvan resorbeerbare, fysiologisch verenigbare collageen II-vezels bevatten en waarbij de gewichtsverhouding van collageen II-vezels tot de poreuze botmineraaldeeltjes ten minste ca. 1:40 is.

De onderhavige uitvinding is bruikbaar voor het reconstrueren van weefseldefecten waaronder zowel kraakbeen-defecten als botdefecten, in het bijzonder bij gelede gewrichten zoals de knie en de ruggegraat.
20 Het botmineraalproduct volgens de onderhavige uitvinding, samengesteld uit deeltjes van poreus botmineraal en collageen II-vezels, verschaft een substraat voor het ingroeien van zowel natieve chondrocyten als natieve osteo-cyten in de matrix om kraakbeen- en botregeneratie te beïn-25 vloeden.
De collageen II-matrix van het product volgens de onderhavige uitvinding verleent tevens sterkte aan het broze botmineraal.
Volgens de onderhavige uitvinding wordt een gezui-30 verd deeltjesvormig botmineraalproduct verschaft voor gebruik in de geneeskunde, waarbij de deeltjes van het genoemde mineraal in hoofdzaak vrij zijn van elk endogeen organisch materiaal en ten minste op het oppervlak ervan resorbeerbaar, fysiologisch verenigbaar, collageen II-materiaal hebben.
35 Botten van geslachte dieren zijn een goedkoop uit gangsmateriaal dat in grote hoeveelheden beschikbaar is. Deze bevatten 50 tot 60% zeer fijn gekristalliseerd hydroxyl-apatiet dat is gebonden door collageen weefsel en significante hoeveelheden eiwitachtig en ander materiaal alsmede 1018560* 3 daarmee verbonden vet- en spierweefsels bevat. Met het oog op de biologisch gevormde kristalstructuur ervan kan het ook worden beschouwd als een sterke biologisch verenigbare pro-thesebotvervanging. Ten gevolge van het grote soortelijke 5 oppervlak ervan kan het bijvoorbeeld ook worden gebruikt als een adsorbens of als een drager voor medicatie met langzame afgifte.
Natuurlijk botmineraal bevat hydroxyapatietachtige kristallieten met een bepaalde mate van kristalliniteit, 10 habitus en afmeting (onregelmatige plaatjesachtige morfologie, een dikte van 5-10 mm, een lengte van 10-50 mm) en oppervlaktechemie ten gevolge van de calcium-tot-fosfaatver-houding (37,5-38,0% calcium en 15,5-19,0% fosfor). In het natuurlijke botmineraal zijn tevens kleine hoeveelheden niet-15 kristallijne stoffen en andere calciumfosfaatkristallijne fasen waaronder de minerale Brishite en Nihitlockite en octa-calciumfosfaat aanwezig. De anorganische fase van het bot bevat poreusheid met inbegrip van ultrastructurele tussenruimten (10-100 mm) tussen de natuurlijk voorkomende kristal-20 lieten en zijn gevormd door verwijdering van de organische fase, en microscopische ruimten (1-20 micron, waaronder osteocytlacunen, kanaaltjes, vasculaire kanalen, kanalen van Volkmann en de kanalen van Havarsiaanse systemen (100-500 mm). Het soortelijk oppervlak, hetgeen een mate is voor de 25 poreusheid, ligt in het traject van 50 tot 100 m2/gm zoals bepaald volgens de kwikporosiemetrie. De kristalliniteit van botmineraal kan worden gekarakteriseerd door middel van rönt-genstralingsdiffractie en de poreusheid en kristallietmorfo-logie en grootte door middel van electronenmicroscopie.
30 Kleine hoeveelheden niet-apatitische kristallieten kunnen worden gedetecteerd met behulp van thermogravimetrische analyse.
De samenstelling en structuur van natuurlijk bot-materiaal kunnen echter niet worden gedupliceerd door in 35 vitro gevormde producten of door natuurlijk voorkomende hydroxyapatieten die eerder zijn bereid. Er zijn twee werkwijzen voor de zuivering van natuurlijk botmineraal voorgesteld, namelijk calcineren en oplosmiddelextractie.

De temperatuur die tijdens het calcineren benodigd is voor het vernietigen van de organische bestanddelen van de botten is tamelijk hoog. Dit leidt tot overmatige herkristal-lisatie van het minerale gedeelte onder vorming van veel gro-5 vere kristallen. Het aldus gevormde materiaal vertoont een relatief klein soortelijk oppervlak. Aldus wordt een dergelijk materiaal niet gemakkelijk geremodelleerd om nieuw bot op implantatie te vormen en kunnen implantaten onbepaald ongeremodelleerd blijven ofschoon dit aanvaardbaar kan zijn 10 voor sommige doeleinden.
Volgens de extractiewerkwijzen worden de eiwitten geëxtraheerd uit ontvet bot met een geschikt oplosmiddel. Het verkregen botmineraal wordt vervolgens gewassen om het oplosmiddel te verwijderen.
15 In beide gevallen wordt, wanneer organische onzui verheden worden verwijderd uit het natuurlijk bot waarbij slechts het botmineraal achterblijft, de sterkte van het materiaal in hoge mate verminderd en zijn de afzonderlijke stukjes gezuiverd botmineraal dientengevolge uitermate broos. 20 Dit maakt de bewerking van het materiaal moeilijk en kan leiden tot ongewenste effecten bij implantatie.
Het Amerikaanse octrooischrift nr. 5.573.771 van aanvraagster (hierin door middel van verwijzing opgenomen) beschrijft een medicinaal botmineraalproduct, waarbij het 25 botmineraal wordt versterkt met een matrix die is samengesteld uit collageen van het type I (collageen I) of een mengsel van collageen van het type I en collageen van het type III (collageen I en collageen III).
Collageen komt in een aantal vormen in het dierlijk 30 lichaam voor en verschillende weefsels bevatten verschillende verhoudingen van de desbetreffende typen. Collageensponsmateriaal dat wordt gebruikt in de geneeskunde en in cosmetica is in het algemeen afkomstig van huid en pezen en is voornamelijk samengesteld uit collageen I en/of collageen III. Bot-35 collageen bevat voornamelijk collageen I en collageen II.
Het collageen II-materiaal volgens de onderhavige uitvinding kan, naast in hoofdzaak zuiver collageen II, diverse verhoudingen van collageen I, collageen III en mengsels ervan, vermengd met het collageen II, bevatten. Zo kan 101 8560·· 5 1 > bijvoorbeeld het collageen II-materiaal daarin zijn gemengd met ca. 10 gew.% (bij voorkeur ca. 0,1-5 gew.%) collageen III en/of ca. 1 tot 50 gew.% collageen I.
Het collageen II-materiaal volgens de onderhavige 5 uitvinding kan elk van de afzonderlijke deeltjes impregneren om de bewerkingseigenschappen van het product bij de vervaardiging en het gebruik te verbeteren. In dat geval is de gewichtsverhouding van het collageen II-materiaal tot het gezuiverde botmineraal met voordeel groter dan 1:40, bij 10 voorkeur groter dan 1:8 en kleiner dan 4:1, en bij voorkeur kleiner dan 1:2. Met voordeel bevat het collageen II-mate-riaal ca. 1/3 gew.% van het botmineraalproduct volgens de onderhavige uitvinding, bij voorkeur ca. 5-15% daarvan. Het collageen II-materiaal dringt door de poreuze structuur van 15 het botmineraal en vervangt op effectieve wijze iets van het eerder in het natuurlijke bot aanwezige natuurlijke eiwitachtige materiaal dat, ofschoon het sterkte verschaft, ook immunologische weefselreacties bij implantatie van het botmineraal teweegbrengt.
20 Het collageen II-materiaal kan worden gebruikt om een matrix te verschaffen voor het deeltjesvormige botmineraal waaruit gevormde voorwerpen kunnen worden gemaakt. In dit geval is het mogelijk om collageen II te gebruiken samen met een gelvormende macromoleculaire stof zoals gelatine. De 25 gewichtsverhouding van het vezelige materiaal tot het botmineraal kan bijvoorbeeld liggen in het traject van 1:40 tot 3:20, bijv. ca. 1:10. De gelfase bedraagt met voordeel 2 tot 20 gew.% van het botmineraal, bijv. ca. 5%. In het geval dat gelatine wordt gebruikt als de gelfase kan het enigszins ver-30 knoopt worden, bijv. ca. 0,2% formaldehyde.
Het botmineraal is bij voorkeur van sponsachtig bot en is verbonden met de collageen II-vezels om aan de matrix fysische sterkte toe te voegen. Volgens voorkeursuitvoeringsvormen wordt het botmineraal/collageenproduct volgens de uit-35 vinding gebruikt als een matrix voor om kraakbeendefecten bij gelede gewrichten, in gevallen waarin extra botverlies aanwezig is, te regenereren.

Het botmineraalproduct volgens de uitvinding kan worden gebruikt voor het regenereren van kraakbeen in knieën, voeten, de ruggegraat enz. en als een remodellerende implant of prothesebotvervanging, bijvoorbeeld in de orthopedische 5 chirurgie met inbegrip van heuprevisies, vervanging van bot-verlies, bijv. in de traumatologie, remodellering in de kaak-en gelaatschirurgie of het opvullen van periodontale defecten en tandextractieholten, met inbegrip van het verhogen van opstaande randen. Het geïmpregneerde deeltjesvormige materi-10 aal volgens de uitvinding kan derhalve worden gebruikt voor een pakking in een verscheidenheid aan botholten en de verminderde broosheid ervan is significant voor wat betreft het behulpzaam zijn bij de bewerkings- en pakkingsprocedures.
Volgens één uitvoeringsvorm van de uitvinding wordt 15 het botmineraalproduct/collageen II-materiaal ingebracht in een botdefect, en vervolgens worden het botmineraal/collageen II-product en het botdefect bedekt met een collageenmembraan voor weefselreconstructie. Geschikte collageenmembranen zijn in het vak bekend en kunnen een enkelvoudige laag collageen 20 II-membraan zijn, zoals beschreven in WO 96/25961 (Amerikaanse octrooiaanvrage nr. 08/894.517), een collageen I/III-membraan met een tweeledig oppervlak, zoals beschreven in het Amerikaanse octrooischrift nr. 5.837.278, met een compact, glad, niet-poreus buitengrensoppervlak en daartegenover lig-25 gend zacht vezelig oppervlak, of een uit meer lagen bestaand membraan dat een open, sponsachtige collageen II-matrixlaag en ten minste één grenslaag van collageen I, collageen III of een mengsel ervan omvat, zoals beschreven in de Amerikaanse octrooiaanvrage nr. 09/545.465, ingediend op 7 april 2000, 30 die overeenkomt met PCT/GB98/02976, en dergelijke. De documenten waarnaar hierboven wordt verwezen dienen te worden beschouwd als hierin door verwijzing opgenomen.
Volgens één voorkeursuitvoeringsvorm heeft het membraan een vlak buitengrensoppervlak, dat celhechting daar-35 aan tegengaat en dient als een barrière om het daardoorheen gaan van cellen te beletten en voorts een daartegenover liggend membraanoppervlak dat vezelig en zacht is teneinde cel-groei daaraan mogelijk te maken. Eén dergelijk product is Bio-Gide®, op de markt gebracht door Ed. Geistlich Söhne AG
1 01 856 0 ’ 7 für Chemische Industrie uit Zwitserland. Fig. 5 toont het Bio-Gide®-product met het gladde buitengrensoppervlak 20 en het zachte vezelige oppervlak 22. Bij gebruik in overeenstemming met de onderhavige uitvinding word het zachte, vezelige 5 oppervlak aangebracht in de richting van het defect met het gladde buitengrensoppervlak naar buiten gericht. Het Bio-Gide®-product is beschreven in het Amerikaanse octrooischrift 5.837.278 (supra) en is samengesteld uit ca. 95-99% collageen I en ca. 1-5% collageen III.
10 Volgens een andere voorkeursuitvoeringsvorm van de uitvinding wordt een uit meer lagen bestaand membraan gebruikt zoals beschreven in de Amerikaanse octrooiaanvrage 09/545.465 waarnaar hierboven wordt verwezen. Een dergelijk uit meer lagen bestaand membraan kan worden gemaakt door het 15 aanbrengen van een collageen II-suspensie op het zachte vezelige oppervlak van het Bio-Gide®-product zoals hierboven beschreven. Een dergelijk product wordt getoond in fig. 6 en omvat een buitengrensoppervlak 20 en een matrixlaag, voornamelijk van collageen II met een open, sponsachtige textuur 20 24, welke is aangebracht als een suspensie op het zachte, vezelige oppervlak 22 en vervolgens wordt gedroogd. Wanneer een product zoals getoond in fig. 6 wordt gebruikt in overeenstemming met de onderhavige uitvinding wordt de open, sponsachtige collageen II-laag 24 aangebracht op het defect, 25 waarbij het gladde buitengrensoppervlak 20 naar buiten is gericht.
Zoals hierboven aangegeven kan de uitvinding in het bij zonder worden toegepast bij het regenereren van defecten in gelede gewrichten, waarbij zowel het kraakbeen als het 30 onderliggende bot beschadigd zijn. Het botmineraal/collageen-product volgens de uitvinding kan worden gebruikt om een gebied op te vullen waar botbeschadiging heeft plaatsgevonden, en het opgevulde gebied van het defect in het bot kan vervolgens worden bedekt met een collageenmembraan.
35 Om de regeneratie te verbeteren kunnen extracellu- laire gekweekte chondrocyten worden toegevoegd aan de bot-mineraal/collageenmatrix volgens de uitvinding voorafgaande aan de implantatie, en de met chondrocyten beladen matrix kan worden geïmplanteerd tijdens open chirurgie of artroscopische 101 8560 " 8 chirurgie. Als alternatief of in aanvulling daarop kan de geïmplanteerde matrix worden bedekt met een collageenmembraan dat is samengesteld uit collageen I, II en/of III of bedekt met een synthetisch membraan. Een dergelijk collageenmembraan 5 of synthetisch membraan kan als alternatief of aanvullend worden beladen met extracellulaire gekweekte chondrocyten, waarbij het membraan is aangebracht als het opgevulde botimplantaat met behulp van open chirurgie of artroscopische chirurgie.
10 Het gezuiverde botmineraal kan bijvoorbeeld een pro duct zijn zoals beschreven in de internationale octrooiaanvrage WO 86/07265 (PCT/GB86/00310). Dergelijke producten kunnen worden bereid door het krachtig ontvetten van deeltjesvormig bot, bijvoorbeeld runderdijbeenderen, en behandeling 15 met ammonia of een organisch amine om een resteiwit af te breken, gevolgd door grondig wassen met water. Een dergelijk materiaal blijft resorbeerbaar na implantatie waarbij het behulpzaam is bij het remodel1ingsproces.
Het is ook mogelijk om gezuiverd botmineraal te 20 bereiden door middel van calcineren van deeltjesvormig poreus of corticabot, bijvoorbeeld bij 900°C gedurende 24 uur. Dergelijk gecalcineerd botmineraal kan worden gebruikt in het geval waarin permanente niet-resorbeerbare implantaten nodig zijn, bijvoorbeeld bij de verhoging van opstaande randen. Hoe 25 dan ook is het bot na verwijdering van organisch materiaal uitermate broos en wordt de sterkte ervan in hoge mate verbeterd door behandeling volgens de onderhavige uitvinding.
In het geval waarin het bot dient te worden gebruikt als geneesmiddeldrager, zoals aangegeven in de bovengenoemde 30 internationale octrooiaanvrage, kan het botmineraal op bruikbare wijze één of meer geadsorbeerde geneesmiddelen of andere fysiologisch actieve stoffen dragen. In overeenstemming met één uitvoeringsvorm omvat het product volgens de onderhavige uitvinding ten minste één geabsorbeerde farmaceutisch of bio-35 logisch actieve stof of mesenchymale stamcellen met een vermogen om te differentiëren in cellen om kraakbeen of bot te regenereren.

Fysiologisch actieve stoffen welke kunnen worden geabsorbeerd op het botmineraal zijn bij voorkeur ten minste ten dele in water oplosbaar en bevatten antibacteriële stoffen zoals antibiotica, bijvoorbeeld penicilline, cephalo-5 sporine, aminoglycosiden, enz., sulfonamiden en, in het bijzonder, condensatieproducten van formaldehyde met taurinamide of N-gesubstitueerde taurinamide. De laatstgenoemde verbindingen kunnen worden voorgesteld door de formule

De verbinding met de formule (I) waarbij R1 en R2 beide waterstof zijn is taurultam, terwijl de verbinding waarbij R1 waterstof is en R2 de formule (II) heeft tauro-lidine is. Deze verbindingen dienen als methylolomzettings-30 agentia en zijn niet alleen effectief bij het vernietigen van zowel gram-negatieve als gram-positieve bacteriën, maar ook bij het inactiveren van zowel endotoxinen als exotoxinen die worden geproduceerd door de bacteriën.
Andere geschikte fysiologisch actieve stoffen omvat-35 ten eiwitten en polypeptiden, die in staat zijn om behulpzaam te zijn bij botregeneratie, met name niet-collageen eiwit, afkomstig van botmatrix en botcellen. Deze omvatten mitogene factoren zoals skeletgroeifactor en morfogene en angiogene 101 856 0 ^ 10 factoren alsmede transformerende botgroeifactor. Groeifactoren uit de natuurlijke botmatrix, zoals osseine of met meer voorkeur osteopoitinetine zijn bijzonder gunstig.
Volgens een uitvoeringsvorm wordt een farmaceutisch 5 actieve stof gekozen uit de groep bestaande uit morfogene boteiwitten (BMPs) zoals BMP-2-8, of andere skeletmatrix-moleculen alsmede signaalpeptiden zoals transformerende groeifactor-jS TGF-/3, TGF/31, vasculaire endotheliale groeifactor (VEGF), insuline-achtige groeifactor (IGF), met para-10 thyroidehormoon verwant eiwit (PTHrP) en van bloedplaatjes afgeleide groeifactor (PDGF).
Het product volgens de uitvinding kan ook dienen als een drager voor stamcellen die zijn toegewezen aan een bepaalde differentiatielijn zoals gewrichtskraakbeen of -bot. 15 Dergelijke stamcellen kunnen in vitro worden gekweekt om het aantal ervan te doen toenemen en worden aangebracht op de te repareren plaatsen in de dragermatrices met of zonder groeifactoren.
Het zal duidelijk zijn dat fysiologisch actieve 20 stoffen als alternatief of aanvullend kunnen worden opgenomen in de macromoleculaire stof, bijv. geïmpregneerde gelatine. Dit is in het bij zonder geschikt voor eiwitten zoals de hierboven uiteengezette botgroeifactoren.
Het botmineraal zal normaliter de vorm hebben van 25 deeltjes met een gemiddelde diameter van 0,1-10 mm. Deeltjes voor opname in collageen II-weefsel zullen bij voorkeur sponsachtig bot zijn en zullen in het algemeen een afmeting hebben in het traject van 0,1 tot 5 mm, bij voorkeur 0,5 tot 2 mm. Het kan gunstig zijn voor een dichte pakking van de 30 botmineraaldeeltjes om gebruik te maken van een mengsel van twee of meer deeltjesgrootten, bijv. 0,25 tot 1 mm en 1 tot 2 mm of een breed traject, bijv. 0,25 tot 2 mm.
Het gezuiverde botmineraal kan bijvoorbeeld worden verkregen volgens de werkwij ze zoals beschreven in de boven-35 genoemde internationale octrooiaanvrage. Zo kunnen bijvoorbeeld vetten worden verwijderd onder gebruikmaking van één of meer conventionele vetoplosmiddelen zoals ethers, bijv.
1 01 856 O'* 11 dimethylether; ketonen, bijv. aceton; of koolwaterstoffen of gehalogeneerde koolwaterstoffen bijv. heptaan of methylcyclo-hexaan of tolueen.
Het kan van voordeel zijn om een extractiemiddel 5 zoals tolueen te verwijderen met behulp van een tussentijdse extractie met een in water mengbaar oplosmiddel, zoals ethanol, voorafgaande aan verdere bewerking. Collageenmateriaal kan worden opgelost onder gebruikmaking van proteo-lytische agentia zoals basen, bijv. kaliumhydroxyde in 10 glycerol of organische basen zoals aminen, bijv. ethyleen-diamine, of amiden zoals formamide, bij voorkeur bij verhoogde temperaturen. Dergelijke middelen zijn bij voorkeur in water mengbaar om het verwijderen door wassen met water te vergemakkelijken. Met name zijn goede resultaten verkregen 15 onder gebruikmaking van met ethyleendiamine onder terug-vloeiing geëxtraheerd bot.
Extractie kan met voordeel worden voortgezet in elk stadium, indien nodig met veranderingen van oplosmiddel, totdat geen verder materiaal wordt geëxtraheerd, bijv. gedu-20 rende perioden tot één of twee weken. Het kan van voordeel zijn om verder te vermalen na aanvankelijke verwijdering van eiwit aangezien het bot gemakkelijker wordt gebroken in dat stadium dan vóór extractie. Na behandeling met base wordt de overmaat oplosmiddelen krachtig verwijderd bijv. door middel 25 van verdamping en/of, in het geval waarin dit geschikt is, wassen met water.
Het materiaal wordt normaliter onderworpen aan een droogstap. Het kan gemakkelijk zijn om het materiaal in dit stadium te steriliseren, bijv. door warmtebehandeling, het-30 geen verdere zuivering teweeg kan brengen. Absorptie en/of adsorptie van de.fysiologisch actieve stof wordt bij voorkeur teweeggebracht door onderdompelen van het behandelde botmate-riaal in een waterige oplossing daarvan, bij voorkeur onder steriele omstandigheden. De concentratie van actieve stof is 35 bij voorkeur relatief hoog om adsorptie en/of absorptie te vergemakkelijken en zal ten dele afhankelijk zijn van de oplosbaarheid van het actieve materiaal.
De uitvinding zal thans nader worden toegelicht aan de hand van de volgende tekeningen.

Fig. 1 toont een uiteinde van een geleed bot 10 met kraakbeen 12, dat gezond en zonder defect is.
Fig. 2 toont een geleed bot 10 met kraakbeen 12, waarbij een defect 14 zich door het kraakbeen 12 heen in het 5 bot 10 uitstrekt.
Fig. 3 toont de botmineraal/collageenmatrix 16 volgens de uitvinding, welke al dan niet kan zijn beladen met chondrocyten, en is opgevuld in het defect 14 in het kraakbeen 12 en het bot 10.
10 Fig. 4 toont een collageen- of synthetisch plaatje 18, dat al dan niet is beladen met chondrocyten, dat het defect 14 dat opgevuld is met de nieuwe botmineraal/collageenmatrix 16 volgens de uitvinding bedekt voor het regenereren van de beschadiging zowel aan het kraakbeen 12 als het 15 bot 10. Het plaatje 18 kan worden vastgemaakt met behulp van elk geschikt middel 19, zoals hechtingen, chirurgische naalden of hechtmiddel.
De volgende voorbeelden worden slechts bij wijze van illustratie gegeven: 20
Voorbeeld 1:
Runderdijbeenbotten werden gekookt in heet water totdat zij schoon waren, vermalen tot een deeltjesgrootte van 5 tot 10 mm en onder terugvloeiing geëxtraheerd met tolueen 25 gedurende 24 uur in een Sohxlet-inrichting. Het materiaal werd verder geëxtraheerd met ethanol om tolueen te verwijderen en vervolgens geëxtraheerd bij verhoogde temperatuur met een azeotropisch mengsel van ethyleendiamine en water (85:15) gedurende 8 dagen, waarbij verscheidene keren van 30 oplosmiddel werd verwisseld totdat vrijwel geen verder organisch materiaal meer werd geëxtraheerd. Het product werd vervolgens aan de lucht gedroogd bij 100°C.
Het gedroogde product werd verder vermalen tot een gemiddelde deeltjesgrootte van 0,2 tot 2 mm en in de auto-35 claaf gesteriliseerd. De stukjes sponsachtig runderdijbeen- bot, met een kenmerkende diameter van 10 mm, werden gezuiverd met behulp van dezelfde techniek, waarbij de eindgranulatie achterwege werd gelaten.

Voorbeeld 2
Bevroren kraakbeen afkomstig van vers geslachte varkens werd ondergedompeld in koud water, grondig doorgewassen en mechanisch gezuiverd van vleesresiduen, botten en harde 5 stukjes. Vervolgens werd het materiaal gedurende 30 minuten onder stromend water gewassen.
Vervolgens werd het materiaal drie keer gemalen in een homogeniseerinrichting. De optimale deeltjesgrootte was aan het einde van de deeltjesgroottevermindering ca. 8 mm.
10 De kraakbeenstukjes werden ontwaterd door 4 keer wassen met aceton, elke keer gedurende 8 uur. Het kraakbeen werd vervolgens ontvet door 4 keer extraheren met n-hexaan. Elke behandeling duurde ten minste 8 uur. De verhouding van hexaan tot kraakbeen was 1:10.
15 Na ontvetting werd het kraakbeen opgezwollen in drinkwater. De verhouding water:materiaal was 10:1. De behan-delingsduur was 24 uur.
Het materiaal werd vervolgens behandeld met NaOH (5 gew.%) waardoor de verhouding van kraakbeen tot vloeistof 1:4 20 en de behandelingsduur 32 uur was. Tijdens de behandeling werden de stukjes kraakbeen goed geroerd. Vervolgens werd de alkali uit het kraakbeen gewassen. De oorspronkelijke pH van 14 werd daardoor verlaagd tot 9-11. De opgeloste onzuiverheden werden uitgewassen en van het kraakbeen gescheiden. De 25 uit de alkalische behandeling verkregen vloeistof werd verzameld voor het terugwinnen van glucosaminoglycan.
Het collageenmateriaal werd vervolgens behandeld met sterk HCL (ca. 3 gew.%), aanvankelijk bij een pH-waarde beneden 1,0. De behandelingstijd was 4-6 uur.
30 Het collageen materiaal werd vervolgens met koud water lang genoeg gewassen om de pH-waarde te doen stijgen tot 3-3,5. Alle onzuiverheden werden verwijderd en het product was een zoutvrije collageenmassa, geschikt voor de vervaardiging van een spons of ander collageenmateriaal. Voor 35 dat doel kan de collageenmassa, overeenkomstig het beoogde resultaat, worden ontgast, bevroren en gevriesdroogd.

Voorbeeld 3
Het uit de alkalische behandeling in voorbeeld 2 verkregen bevatte glycosaminoglycan, alkali, gedenatureerde eiwitten en zouten. Het extract werd eerst geneutraliseerd 5 met HC1, waarbij de pH-waarde na neutralisatie 6 was. Het extract werd vervolgens behandeld met een filterhulpmiddel, voornamelijk Kieselguhr, dat het effect had van het verwijderen van de gedenatureerde eiwitten. Er werd 0,5 gew.% Kieselguhr in het extract opgenomen en door middel van 10 filtratie verwijderd samen met het gedenatureerde eiwit.
De bovenvloeistof werd vervolgens onderworpen aan ultrafiltratie onder gebruikmaking van een membraan met een molecuulgewicht dat werd onderbroken bij ca. 10.000 Dalton.
Op deze wijze werden de zouten verwijderd onder achterlating 15 van gezuiverd glycosaminoglycan.
De aldus verkregen glycosaminoglycanoplossing werd van bovenaf vermengd met collageenmateriaal onder oplevering van een collageen II-matrix met daarin glucosaminoglycan.
20 Voorbeeld 4 2,0 g collageen II-materiaal uit voorbeeld 3 werd vermalen met 500 g gedestilleerd water in een menginrichting. Deze dispersie werd gecentrifugeerd, waarna het bovenstaande water werd verwijderd. Aan de verkregen collageenvezelsuspen-25 sie werd 17,5 g gegranuleerd corticaal runderbot, gezuiverd volgens de hierboven genoemde procedure van voorbeeld 1, toe-gevoegd, gevolgd door grondig mengen en verwijdering van het water door afzuigen (70 mm). Het gegranuleerde bot heeft een deeltjesgrootte van 0,5 tot 1 mm. Na verwijdering van het 30 water werd 5 ml van een 9% gew./gew. waterige gelatine- oplossing toegevoegd (verknoopt met 0,6% van 35% waterige formaldehyde) en het mengsel werd opnieuw door afzuigen gedroogd.
De sponsmassa wordt in stukjes gesneden en onder 35 vacuüm bij 60°C gedroogd. De stukjes spons werden verpakt in houders van polyethyleen en gesteriliseerd met behulp van gammastraling.

Claims (9)
1. Deeltjesvormig botmineraalproduct voor het repareren van gecombineerde kraakbeendefecten en botdefecten, met het kenmerk, dat het product poreuze botmineraaldeeltjes omvat, afkomstig van natuurlijk bot, met een kristalstructuur die in hoofdzaak die van natuurlijk bot is en die in hoofdzaak vrij zijn van elk endogeen organisch materiaal, waarbij de deeltjes ten minste aan een oppervlak daarvan resorbeer-bare, fysiologisch verenigbare collageen II-vezels bevatten en waarbij de gewichtsverhouding van collageen II-vezels tot de poreuze botmineraaldeeltjes ten minste ca. 1:40 is.
2. Product volgens conclusie 1, met het kenmerk, dat de deeltjes een gemiddelde diameter hebben in het traject van ca. 0,1 tot ca. 5 mm.
3. Product volgens conclusie 1, met het kenmerk, dat het product ten minste één geadsorbeerde farmaceutische of biologisch actieve stof of mesenchymale stamcellen bevat met het vermogen om te differentiëren in cellen om kraakbeen of bot te regenereren.
4. Product volgens conclusie 3, met het kenmerk, dat de farmaceutisch actieve stof is gekozen uit de groep bestaande uit taurolidine, taurultam en een mengsel daarvan.
5. Product volgens conclusie 1, met het kenmerk, dat het product gelatine bevat in een gelfase, waarbij de collageen II-vezels in de gelfase aanwezig zijn en waarbij de gelfase ca. 2-20 gew.% van het botmateriaal uitmaakt.
6. Product volgens conclusie 3, met het kenmerk, dat de farmaceutisch actieve stof is gekozen uit de groep bestaande uit morfogentische boteiwitten (BMPs), andere skeletmatrixmoleculen en signaalpeptiden.
7. Product volgens conclusie 6, met het kenmerk, dat de farmaceutisch actieve stof is gekozen uit de groep bestaande uit BMP-2-8, TGF-β, TGF-01, VEGF, IGF, PTHrP en PDGF.
