Description
Stand der Technik Gebiet der Erfindung [0001] Die vorliegende Erfindung betrifft das Gebiet der Reparatur von verletzten oder defekten Gelenken. Beschreibung des Stands der Technik [0002] Artikulierende Gelenke in Säugetieren sind mit artikularem Knorpel überzogen, der den direkten Kontakt der entgegengesetzten Knochenoberflächen verhindert und eine geschmeidige Bewegung der artikulierenden Knochen relativ zueinander ermöglicht. [0003] Es hat mehrere Vorschläge zur Reparatur von Verletzungen und Defekten in artikularem Knorpel gegeben. Diese umfassen Implantation von kultivierten Chondrozyten am Ort der Knorpelverletzung und Abdeckung der Verletzung mit einem Kollagen-Patch. [0004] Manchmal erstreckt sich die Verletzung oder der Defekt in einem artikularen Gelenk tiefer als der artikulare Knorpel in den darunterliegenden Knochen. Es besteht ein Bedarf auf dem Gebiet der Materialien und Methoden zur Reparatur von artikulierenden Gelenken, bei welchen sich die Verletzung oder der Defekt durch das artikulare Gelenk hindurch in den Knochen erstreckt. Zusammenfassung der Erfindung [0005] Ein. Knochenmineralprodukt gemäss der vorliegenden Erfindung zur Verwendung in der Behandlung von kombinierten Knorpel- und Knochenverletzungen oder Defekten in artikularen Gelenken umfasst poröse, von natürlichem Knochen hergeleitete Knochenmineralpartikel, die eine Kristallstruktur haben, die im Wesentlichen der von natürlichem Knochen entspricht, und die im Wesentlichen frei von endogenem organischem Material sind, wobei die Partikel an mindestens einer Oberfläche resorbierbare, physiologisch verträgliche Kollagen-II-Fasern besitzen, wobei das Gewichtsverhältnis von besagten Kollagen-II-Fasern zu besagten porösen Knochenmineralpartikeln mindestens ungefähr 1:40 ist. Beschreibung der bevorzugten Varianten [0007] Die vorliegende Erfindung ist von Nutzen in der Rekonstruktion von Gewebedefekten, die sowohl Knorpeldefekte wie auch Knochendefekte, vor allem in artikulierenden Gelenken wie zum Beispiel dem Knie und der Wirbelsäule, umfassen. [0008] Das Knochenmineralprodukt der vorliegenden Erfindung, das Partikel von porösem Knochenmineral und Kollagen-II-Fasern umfasst, stellt ein Substrat für das Einwachsen von sowohl nativen Chondrozyten wie auch nativen Osteozyten in die Matrix bereit, um eine Knorpel- und Knochenregeneration zu bewirken. [0009] Zudem verleiht die Kollagen-II-Matrix des Produktes der vorliegenden Erfindung dem brüchigen Knochenmineral Festigkeit. [0010] Gemäss der vorliegenden Erfindung wird ein gereinigtes, partikuläres Knochenmineralprodukt zur Verwendung in der Medizin bereitgestellt, wobei die Partikel des besagten Minerals im Wesentlichen frei von allem endogenen, organischen Material sind und an mindestens einer Oberfläche resorbierbare, physiologisch verträgliche Kollagen-II-Fasern besitzen. [0011] Knochen von geschlachteten Tieren sind ein preiswertes, in grossen Mengen erhältliches Rohmaterial. Diese beinhalten 50 bis 60% sehr fein kristallisiertes Hydroxylapatit, gebunden durch Kollagengewebe, und enthalten beträchtliche Quantitäten an proteinhaltigen oder anderen Substanzen wie auch assoziiertes Fett- und Muskelgewebe. Angesichts dessen biologisch gebildeter Kristallstruktur kann es ebenfalls als ein höchst biokompatibler, prothetischer Knochenersatz betrachtet werden. Dank dessen grosser spezifischer Oberfläche kann es zum Beispiel ebenfalls als ein Adsorbens oder als Trägermaterial für ein "slow release"-Medikament verwendet werden. [0012] Natürliches Knochenmineral umfasst Hydroxyapatit-ähnliche Kristalliten mit einem bestimmten Mass an Kristallinitat, Kristallhabitus und Grösse (irreguläre, plattenförmige ("platelike") Morphologie, 5-10 mm in Dicke, 10-50 mm in Länge) und eine Oberflächenchemie, die durch das Kalzium-zu-Phosphat-Verhältnis entsteht (37.5-38% Kalzium und 15.5-51.9% Phosphor). Ebenfalls vorhanden in natürlichem Knochenmineral sind geringe Mengen von nicht-kristallinen Elementen und andere Kalziumphosphat-kristalline Phasen, einschliesslich der Mineralien Brushit und Nihitlockit, und octa-Kalziumphosphat. Die anorganische Phase des Knochens weist eine Porosität auf einschliesslich ultrastruktureller Interstitien (10-100 mm) zwischen den natürlich vorkommenden und den durch Entfernung der organischen Phase produzierten Kristalliten, und mikroskopische Bereiche (1-20 Mikron) einschliesslich der Osteozyten in Lakunen ("osteocyte lacunae"), Canaliculi, der vaskulären Kanäle, Volkmann-Kanäle und der Kanäle des Havers' Systems (100-500 mm). Der spezifische Oberflächenbereich, der ein Mass für die Porosität darstellt, liegt, wie mittels Quecksilber-Porosimetrie bestimmt, im Bereich von 50 bis 100 m<2>/gm. Die Kristallinität des Knochenminerals kann mittels Röntgenstrahlbeugung und die Porosität und Kristallit-Morphologie und Grösse mittels Elektronmikroskopie charakterisiert werden. Geringe Mengen an nicht-apatitischen Kristalliten können mittels thermogravimetrischer Analyse ermittelt werden. [0013] Jedoch kann die Zusammensetzung und Struktur von natürlichem Knochenmaterial nicht durch in vitro hergestellte Produkte oder durch zuvor hergestellte, natürlich vorkommende Hydroxyapatite dupliziert werden. Zwei Methoden zur Reinigung von natürlichem Knochenmineral wurden vorgeschlagen, nämlich Kalzinierung und Lösungsmittelextrahierung. [0014] Die Temperatur, die während der Kalzinierung zur Verbrennung der organischen Komponenten der Knochen erforderlich ist, ist ziemlich hoch. Dies führt zu umfangreicher Rekristallisation des Mineralteils unter Bildung von viel grobkörnigeren Kristallen. Das so gebildete Material besitzt eine relativ kleine spezifische Oberfläche. Deshalb wird ein solches Material nicht ohne weiteres umgeformt, um nach einer Implantation neue Knochen zu bilden, und solche Implantate können für unbegrenzte Zeit unverändert bleiben, obwohl dies für gewisse Zwecke akzeptabel sein kann. [0015] In den Extrahierungsprozessen werden die Proteine von entfettetem Knochen mittels eines geeigneten Lösungsmittels extrahiert. Das erhaltene Knochenmineral wird dann gewaschen, um das Lösungsmittel zu entfernen. [0016] In beiden Fällen wird, bei Entfernung der organischen Verunreinigungen vom natürlichen Knochen, um nur das Knochenmineral übrig zu lassen, die Festigkeit des Materials enorm reduziert und die einzelnen Stücke des gereinigten Knochenminerals sind folglich äusserst brüchig. Dies erschwert die Handhabung des Materials und kann nach der Implantation zu unerwünschten Auswirkungen führen. [0017] Das gemeinsam besessene U.S.Patent Nr. 5 573 771 (hierin als Referenz eingegliedert) offenbart ein medizinisches Knochenmineralprodukt, bei welchem das Knochenmineral durch eine Matrix aus Typ-I-Kollagen (Kollagen I) oder einer Mischung von Typ-I-Kollagen und Typ-III-Kollagen (Kollagen I und Kollagen III) verstärkt wird. [0018] Kollagen tritt in einer Anzahl Formen im tierischen Körper auf, und verschiedene Gewebe enthalten unterschiedliche Proportionen der jeweiligen Typen. Das in der Medizin und in Kosmetik verwendete Kollagenschwammmaterial ist gewöhnlich von Haut und Sehnen hergeleitet und umfasst vorwiegend Kollagen I und/oder Kollagen III. Knochenkollagen umfasst vorwiegend Kollagen I und Kollagen II. [0019] Das Kollagen-II-Material der vorliegenden Erfindung kann zusätzlich zu im Wesentlichen reinem Kollagen II verschiedene Proportionen von Kollagen I, Kollagen III und Mischungen davon gemischt mit Kollagen II enthalten. Zum Beispiel kann das Kollagen-II-Material ungefähr 0.1-10% nach Gewicht (vorzugsweise 0.1-5% nach Gewicht) Kollagen II und/oder ungefähr 1-50% nach Gewicht Kollagen I darin eingemischt haben. [0020] Das Kollagen-II-Material der vorliegenden Erfindung kann jedes der einzelnen Stücke durchtränken, um die Handhabungseigenschaften des Produktes während der Herstellung und Verwendung zu verbessern. In diesem Fall ist das Gewichtsverhältnis des Kollagen-II-Materials zum gereinigten Knochenmineral von Vorteil grösser als 1:40, vorzugsweise grösser als 1:8 und geringer als 4:1, vorzugsweise geringer als 1:2. Das Kollagen-II-Material umfasst von Vorteil ungefähr 1-30% nach Gewicht des erfindungsgemässen Knochenmineralproduktes, vorzugsweise ungefähr 5-15% davon. Das Kollagen-II-Material durchdringt die poröse Struktur des Knochenminerals und ersetzt auf wirkungsvolle Weise einiges des zuvor im natürlichen Knochen vorhandenen, natürlichen, proteinhaltigen Materials, das, obwohl es Festigkeit verleiht, ebenfalls immunologische Gewebsreaktionen nach Implantation des Knochenminerals auslöst. [0021] Das Kollagen-II-Material kann verwendet werden, um eine Matrix für das partikuläre Knochenmineral bereitzustellen, aus welchem geformte Objekte gebildet werden können. In diesem Fall ist es möglich, Kollagen II zusammen mit einer gelbildenden, makromolekularen Substanz, wie um Beispiel Gelatine, zu verwenden. Das Gewichtsverhältnis des faserigen Materials zum Knochenmineral kann zum Beispiel im Bereich von 1:40 bis 3:20, z.B. ungefähr 1:10 sein. Die Gelphase beträgt vorzugsweise 2 bis 20 Gewichtsprozente des Gewichts des Knochenminerals, z.B. ungefähr 5%. Bei Verwendung von Gelatine als Gelphase kann es leicht vernetzt sein, z.B. mit ungefähr 0.28% Formaldehyd. [0022] Das Knochenmineral kommt vorzugsweise von Spongiosa-Knochen und ist mit den Kollagen-II-Fasern verknüpft, um der Matrix physische Festigkeit zu verleihen. In bevorzugten Varianten wird das erfindungsgemässe Knochenmineral/Kollagen-Produkt als Matrix zur Regeneration von Knorpeldefekten in artikulierenden Gelenken, bei welchen zusätzlich Knochenverlust vorhanden ist, verwendet. [0023] Das erfindungsgemässe Knochenmineralprodukt kann zur Knorpelregeneration in Knien, Füssen, Wirbelsäulen, etc. verwendet werden und als Umgestaltungsimplantat oder prothetischer Knochenersatz, zum Beispiel in der orthopädischen Chirurgie einschliesslich Hüftenüberarbeitung, Ersatz von Knochenverlust, z.B. in der Traumatologie, Umgestaltung in maxillofazialer Chirurgie oder Füllen periodontaler Defekte und Zahnextraktions-Alveolen, einschliesslich Kieferkammaufbau. Das durchtränkte, partikuläre Material der Erfindung kann demzufolge zum Verpacken in verschiedene Knochenkavitäten verwendet werden und dessen verminderte Brüchigkeit ist bei der Förderung des Handhabungs- und Verpackungsverfahrens von Bedeutung. [0024] In einer Methode gemäss der Erfindung wird das Knochenmineralprodukt/Kollagen-II-Material in einen Knochendefekt eingeführt und danach werden das Knochenmineral/Kollagen-Il-Produkt und der Knochendefekt mittels einer Kollagenmembran zur Geweberekonstruktion überzogen. Geeignete Kollagenmembranen sind in Fachkreisen bekannt und können eine Kollagenmembran in Form einer einzigen Schicht sein, wie in WO 96/25961 (U.S. Patentanmeldung Nr. 08/894 517) beschrieben, eine Kollagen-I/III-Membran mit doppelter Oberfläche, wie in U.S. Patent Nr. 583 727 beschrieben, sein, mit einer kompakten, glatten, nicht-porösen Aussenbarrierenoberfläche und einer entgegengesetzten, weichen, faserigen Oberfläche, oder eine mehrschichtige Membran, die eine offene, schwammartige Kollagen-II-Matrixschicht und mindestens eine Barrierenschicht aus Kollagen I, Kollagen III oder einer Mischung davon wie in U.S. Patentanmeldung Nr. 09/545 465, angemeldet am 7. April 2000, entsprechend zu PCT/GB98/02 976, beschrieben, sein oder Ähnliches. Die obenerwähnten Dokumente sind hierin als Referenz eingegliedert. [0025] In einer bevorzugten Variante hat die Membran eine äussere, glatte Barrierenoberfläche, die eine Zelladhäsion daran hemmt und als Barriere wirkt, um ein Durchdringen von Zellen hindurch zu verhindern, und eine entgegengesetzte Membranseite, die faserig und weich ist, um Zellwachstum daran zu ermöglichen. Solch ein Produkt ist Bio-Gide<(RTM)>, das von Ed. Geistlich Söhne AG für Chemische Industrie der Schweiz verkauft wird. Fig. 5 illustriert das Bio-Gide<(RTM)>-Produkt mit der glatten, äussere Barrierenoberfläche 20 und der weichen, faserigen Oberfläche 22. Bei Verwendung gemäss der vorliegenden Erfindung wird die weiche, faserige Oberfläche auf den Defekt aufgetragen, mit der glatten, äusseren Barrierenoberfläche nach auswärts gerichtet. Das Bio-Gide<(RTM)>-Produkt ist in U.S. Patent Nr. 5 837 278 (siehe oben) beschrieben und umfasst ungefähr 95-99% Kollagen I und ungefähr 1-5% Kollagen III. [0026] In einer weiteren bevorzugten Variante wird eine mehrschichtige Membran gemäss der vorliegenden Erfindung verwendet, wie beschrieben in der obenerwähnten U.S. Patentanmeldung Nr. 09/545 465. Solch eine mehrschichtige Membran kann durch Auftragen einer Kollagen-II-Aufschlämmung auf die weiche, faserige Oberfläche des zuvor beschriebenen Bio-Gide<(RTM)>-Produktes hergestellt werden. Solch ein Produkt ist in Fig. 6 dargestellt und umfasst eine äussere Barrierenoberfläche 20 und eine vorwiegend aus Kollagen II bestehende Matrixschicht mit einer offenen, schwammartigen Struktur 24, die als Aufschlämmung auf die weiche, faserige Oberfläche 22 aufgetragen und danach getrocknet wird. Bei Verwendung eines wie in Fig. 6 dargestellten Produktes gemäss der vorliegenden Erfindung wird die offene, schwammartige Kollagen-II-Schicht 24 auf den Defekt aufgetragen, mit der glatten, äusseren Barrierenoberfläche 20 nach auswärts gerichtet. [0027] Wie zuvor erwähnt ist die Erfindung besonders für die Regeneration von artikulären Gelenkdefekten, in welchen sowohl Knorpel wie auch der darunterliegende Knochen beschädigt ist, anwendbar. Das erfindungsgemässe Knochenmineral/Kollagen-Produkt kann dazu verwendet werden, um einen Knochenschadenbereich aufzufüllen und der aufgefüllte Knochenschadenbereich kann anschliessend mit einer Kollagenmembran überzogen werden. [0028] Um eine Regeneration zu fördern, können extrazelluläre, kultivierte Chondrozyten vor der Implantation zu der erfindungsgemässen Knochenmineral/Kollagen-Matrix gegeben werden, und die Chondrozyten-beladene Matrix kann dann während "offener" Chirurgie oder arthroskopischer Chirurgie implantiert werden. Andrerseits oder zusätzlich dazu kann die implantierte Matrix mit einer Kollagen I, II und/oder III umfassenden Kollagenmembran oder mit einer synthetischen Membran überzogen werden. Solch eine Kollagenmembran oder synthetische Membran kann als Alternative oder zusätzlich mit extrazellulären, kultivierten Chondrozyten beladen werden, wobei die Membran während "offener" Chirurgie oder arthroskopischer Chirurgie über das aufgefüllte Knochenimplantat aufgetragen wird. [0029] Das gereinigte Knochenmineral kann zum Beispiel ein Produkt wie beschrieben in der Internationalen Patentanmeldung WO 86/07 265 (PCT/GB86/00 310) sein. Solche Produkte können durch rigoroses Entfetten von partikulären Knochen, z.B. von Rinder-Oberschenkelknochen, und Behandeln mit Ammoniak oder einem organischen Amin, um restliches Protein abzubauen, gefolgt von ausgiebigen Waschungen mit Wasser, hergestellt werden. Solch ein Material bleibt nach einer Implantation resorbierbar, in Unterstützung des Umformungsprozesses. [0030] Es ist ebenfalls möglich, gereinigtes Knochenmineral durch Kalzinieren des partikulären Spongiosa oder kortikalen Knochens zum Beispiel bei 900 deg. C während 24 Stunden. Solch kalziniertes Knochenmineral ist bei Bedarf von permanenten, nicht-resorbierbaren Implantaten von Nutzen, zum Beispiel beim Kieferkammaufbau. [0031] In jedem Fall ist der Knochen nach Entfernung des organischen Materials äusserst brüchig und seine Festigkeit wird durch eine Behandlung gemäss der Erfindung enorm verbessert. [0032] Bei Verwendung des Knochens als Wirkstoffträger, wie in der obigen Internationalen Patentanmeldung angedeutet, kann das Knochenmineral auf nützliche Art ein oder mehrere absorbierte Wirkstoffe oder physiologisch aktive Substanzen tragen. Gemäss einer Variante umfasst das erfindungsgemässe Produkt mindestens eine absorbierte, pharmazeutisch oder biologisch aktive Substanz oder mesenchymale Stammzellen, die die Fähigkeit besitzen, sich zu Zellen zur Regeneration von Knorpel oder Knochen zu differenzieren. [0033] Physiologisch aktive Substanzen, die an das Knochenmineral absorbiert werden können, sind vorzugsweise wenigstens teilweise wasserlöslich und beinhalten antibakterielle Substanzen wie zum Beispiel Antibiotika, z.B. Penizilline, Cephalosporin, Aminoglykoside etc., Sulfonamide und, vor allem, Kondensationsprodukte von Formaldehyd mit Taurinamid oder N-substituiertem Taurinamid. Die letzteren Verbindungen können durch folgende Formel wiedergegeben werden, worin R<1> Wasserstoff oder eine C1-4-Alkylgruppe bedeutet und R<2> Wasserstoff oder eine Gruppe der Formel bedeutet, worin R<1> die oben angegebene Bedeutung hat. [0034] Die Verbindung der Formel (I), worin sowohl R<1> wie auch R<2 >Wasserstoff bedeuten, ist Taurultam, während die Verbindung, worin R<1> Wasserstoff bedeutet und R<2> die Formel (II) besitzt, Taurolidin ist. Diese Verbindungen wirken als Hilfsstoffe in der Methylol-Übertragung und sind nicht nur in der Zerstörung von sowohl gram-negativen wie auch gram-positiven Bakterien wirksam, sondern auch in der Inaktivierung von sowohl Endotoxinen wie auch Exotoxinen, die von den Bakterien produziert wurden. [0035] Andere nützliche, physiologisch aktive Substanzen umfassen Proteine und Polypeptide, die fähig sind, bei der Knochenregeneration mitzuwirken, vor allem von Knochenmatrix und Knochenzellen hergeleitete, nicht-kollagene Proteine. Diese umfassen mitogene Faktoren, wie zum Beispiel den Skelettwachstumsfaktor und morphogene und angiogene Faktoren, wie auch transformierende Knochenwachstumsfaktoren. Wachstumsfaktoren der natürlichen Knochenmatrix, wie zum Beispiel Ossein oder vorzugsweise Osteopoietin, sind besonders vorteilhaft. [0036] Gemäss einer Variante wird eine pharmazeutisch aktive Substanz aus der Gruppe bestehend aus "Bone Morphogenic Proteins" (BMP's), wie zum Beispiel BMP-2-8, oder anderen Skelettmatrixmolekülen, wie auch Signalpeptiden, wie zum Beispiel des "Transforming Growth Factor-beta ", TGF-beta , TGF-beta 1, des "Vascular, Endothelial Growth Factor" (VEGF), des insulin-ähnlichen Wachstumsfaktors (insulin-like growth factor, IGF), des Parathormon-ähnlichen Peptids (parathyroid hormone related protein, PTHrP) und des von Thrombozyten gebildeten Wachstumsfaktors (platelet derived growth factor, (PDGF) ausgewählt. [0037] Das Produkt der Erfindung kann auch als Träger für Stammzellen, die an eine gewisse Differenzierungslinie, wie zum Beispiel artikularem Knorpel oder Knochen, gebunden sind, wirken. Solche Stammzellen können in vitro kultiviert werden, um deren Anzahl zu vermehren und dann in den Trägermatrizen mit oder ohne Wachstumsfaktoren auf die Reparaturstellen aufgetragen werden. [0038] Es versteht sich, dass physiologisch aktive Substanzen als Alternative oder als Zusatz in die makromolekulare Substanz, z.B. durchtränkte Gelatine, eingebaut werden können. Dies ist besonders für Proteine, wie zum Beispiel den oben beschriebenen Knochenwachstumsfaktoren, geeignet. [0039] Das Knochenmineral wird normalerweise in Form von Partikeln mit einem durchschnittlichen Durchmesser im Bereich von 0.1 bis 10 mm sein. Partikel zum Einbau in Kollagen-II-Fasern werden vorzugsweise von Spongiosa-Knochen sein und werden im Allgemeinen im Grössenbereich von 0.1 bis 5 mm, vorzugsweise 0.5 bis 2 mm, sein. Für das enge Verpacken der Knochenmineralpartikel kann es von Vorteil sein, eine Mischung von zwei oder mehreren Partikelgrössen, z.B. 0.25 bis 1 mm und 1 bis 2 mm oder einen breiten Bereich, z.B. 0.25 bis 2 mm, zu verwenden. [0040] Das gereinigte Knochenmineral kann zum Beispiel mittels der in der obengenannten Internationalen Patentanmeldung beschriebenen Methode hergestellt werden. Demnach können zum Beispiel Fette unter Verwendung eines oder mehrerer Fettlösungsmittel, wie zum Beispiel Äther, z.B. Dimethyläther; Ketone, z.B. Azeton; oder Kohlenwasserstoffe oder halogenierte Kohlenwasserstoffe, z.B. Heptan oder Methylcyclohexan oder Toluol, entfernt werden. [0041] Es kann von Vorteil sein, ein Extrahierungsmittel wie zum Beispiel Toluol mittels einer Zwischenextrahierung mit einem wasserlöslichen Lösungsmittel, wie zum Beispiel Ethanol, vor der Fortführung zu entfernen. Kollagenmaterial kann mittels proteolytischer Hilfsmittel wie zum Beispiel Basen, z.B. Kaliumhydroxid in Glyzerol, oder organischen Basen wie zum Beispiel Aminen, z.B. Ethylendiamin, oder Aminen wie zum Beispiel Formamid, vorzugsweise bei erhöhten Temperaturen, gelöst werden. Solche Hilfsmittel sind vorzugsweise wasserlöslich, um das Entfernen mittels Waschungen mit Wasser zu erleichtern. Besonders gute Resultate wurden bei Verwendung von Knochen, die mit rückflussierendem Ethylendiamin extrahiert wurden, erzielt. [0042] Eine Extrahierung kann von Vorteil in jeder Stufe fortgesetzt werden, falls notwendig unter Auswechslung des Lösungsmittels, bis kein zusätzliches Material mehr extrahiert wird, z.B. über Zeiträume von bis zu ein oder zwei Wochen. Es kann von Vorteil sein, den Knochen nach der anfänglichen Proteinentfernung weiter zu verkleinern, da der Knochen auf dieser Stufe einfacher zerbrochen wird als vor der Extraktion. Nach Behandlung mit Base werden die überschüssigen Lösungsmittel rigoros entfernt, z.B. mittels Verdunstung und/oder, wenn geeignet, mittels Waschungen mit Wasser. [0043] Das Material wird normalerweise einem Trocknungsschritt unterworfen. Es kann dienlich sein, das Material auf dieser Stufe zu sterilisieren, z.B. mittels Hitzebehandlung, was eine weitere Reinigung bewirken kann. Absorption und/oder Adsorption der physiologisch aktiven Substanz wird vorzugsweise durch Eintauchen des behandelten Knochenminerals in eine wässrige Lösung der physiologisch aktiven Substanz unter sterilen Bedingungen ausgeführt. Die Konzentration der aktiven Substanz ist vorzugsweise relativ hoch, um eine Adsorption und/oder Absorption zu erleichtern und wird zum Teil von der Löslichkeit des aktiven Materials abhängen. [0044] Die Erfindung wird jetzt im Weiteren mit Bezug auf die Zeichnungen beschrieben. <tb>Fig. 1<sep> illustriert das Ende eines artikulierenden Knochens 10 mit Knorpel 12, der gesund und ohne Defekt ist. <tb>Fig. 2<sep>illustriert einen artikulierenden Knochen 10 mit Knorpel 12, in welchem sich ein Defekt 14 durch den Knorpel 12 in den Knochen 10 erstreckt. <tb>Fig. 3<sep>illustriert die erfindungsgemässe Knochenmineral/Kollagen-Matrix 16, die unbeladen oder mit Chondrozyten beladen sein kann, und in den Defekt 14 im Knorpel 12 und Knochen 10 gefüllt werden kann. <tb>Fig. 4<sep>illustriert einen Kollagen- oder synthetischen Patch 18, der unbeladen oder mit Chondrozyten beladen sein kann, der den mit der erfindungsgemässen Knochenmineral/Kollagen-Matrix 16 aufgefüllten Defekt 14 überzieht, zur Regeneration sowohl der Verletzung des Knorpels 12 wie auch des Knochens 10. Der Patch 18 kann mittels jeglicher geeigneter Methode 19, wie zum Beispiel mittels Nahtmaterial, chirurgische Nägel oder Klebstoff, befestigt werden. [0045] Die folgenden Beispielen werden nur zur Illustration aufgeführt: Beispiel 1 [0046] Rinder-Oberschenkelknochen wurden, bis sie rein waren, in heissem Wasser gekocht, zu einer Partikelgrösse von 5 bis 10 mm zerkleinert und mit Toluol unter Rückfluss während 24 Stunden in einem Soxhlet Apparat extrahiert. Das Material wurde mit Ethanol weiter extrahiert, um das Toluol zu entfernen und dann bei erhöhter Temperatur mit einem azeotropen Gemisch von Diethylendiamin und Wasser (85:15) während 8 Tagen unter mehrmaligem Austausch des Lösungsmittels extrahiert, bis im Wesentlichen kein weiteres organisches Material extrahiert wurde. Das Produkt wurde dann bei 100 deg. C luftgetrocknet. [0047] Das getrocknete Produkt wurde weiter zu einer durchschnittlichen Partikelgrösse von 0.2 bis 2 mm zerkleinert und im Autoklaven sterilisiert. Stücke des Rinder-Oberschenkel-Spongiosa-Knochens ("bovine femur spongiosa bone"), mit einem typischen Durchmesser von 10 mm, wurden mit derselben Methode unter Auslassung der abschliessenden Granulation gereinigt. Beispiel 2 [0048] Gefrorener Knorpel von frisch geschlachteten Schweinen wurde in kaltem Wasser eingeweicht, gründlich gewaschen und mechanisch von Fleischresten, Knochen und harten Stücken gereinigt. Anschliessend wurde das Material dreimal in einem Homogenisierapparat gemahlen. Die optimale Partikelgrösse am Ende der Grössenreduktion betrug ungefähr 8 mm. [0049] Die Knorpelstücke wurde durch viermaliges Waschen mit Azeton für je 8 Stunden entwässert. Der Knorpel wurde dann durch viermalige Extrahierung mit n-Hexan entfettet. Jede Behandlung betrug 8 Stunden. Das Verhältnis von Hexan zu Knorpel betrug 1:10. [0050] Nach der Entfettung liess man den Knorpel in Trinkwasser quellen. Das Verhältnis von Wasser:Material betrug 10:1. Die Behandlungsdauer betrug 24 Stunden. [0051] Das Material wurde dann mit NaOH (5% nach Gewicht) behandelt, wobei das Verhältnis von Knorpel zu Flüssigkeit 1:4 und die Behandlungsdauer 32 Stunden betrug. Während der Behandlung wurden die Knorpelstücke gut gerührt. Anschliessend wurde die Lauge vom Knorpel gewaschen. Der ursprüngliche pH wurde dadurch auf 9-11 reduziert. Die gelösten Verunreinigungen wurden ausgewaschen und vom Knorpel getrennt. Die durch die Laugenbehandlung entstandene Flüssigkeit wurde zur Rückgewinnung von Glycosaminglycan gesammelt. [0052] Das Kollagenmaterial wurde dann mit starker HCl (ungefähr 3% nach Gewicht) mit einem anfänglichen pH-Wert unter 1 behandelt. Die Behandlungsdauer betrug 4-6 Stunden. [0053] Anschliessend wurde das Material mit kaltem Wasser lange genug gewaschen, um den pH-Wert auf 3-3.5 ansteigen zu lassen. Alle Verunreinigungen wurden entfernt und das Produkt bestand aus einer salzfreien Kollagenmasse, geeignet zur Herstellung eines Schwammes oder anderen Kollagenmaterials. Zu diesem Zweck konnte die Knorpelmasse, entsprechend dem vorgesehenen Resultat, entgast, gefroren und gefriergetrocknet werden. Beispiel 3 [0054] Der von der Laugenbehandlung in Beispiel 2 entstandene Extrakt enthielt Glycosaminglycan, Lauge, denaturierte Proteine und Salze. Der Extrakt wurde zuerst mit HCl neutralisiert, wobei der pH-Wert nach der Neutralisierung 6 betrug. Der Extrakt wurde dann mit einem Filterhilfsmittel, nämlich Kieselgur, behandelt, mit der Auswirkung, dass die denaturierten Proteine entfernt wurden. 0.5 Gewichtsprozent Kieselgur wurden dem Extrakt zugegeben und mittels Filtration zusammen mit dem denaturierten Protein entfernt. [0055] Der Überstand wurde dann einer Ultrafiltration unterzogen unter Verwendung einer Membran mit einem Molekulargewicht "Cut-Off"-Wert von ungefähr 10 000 Daltons. Auf diese Weise wurden die Salze entfernt, um gereinigtes Glycosaminglycan übrig zu lassen. [0056] Die so erhaltene Glycosaminglycanlösung wurde mit dem obigen Kollagenmaterial. gemischt, um eine Glycosaminglycan-enthaltende Kollagen-Il-Matrix bereitzustellen. Beispiel 4 [0057] 2.0 g Kollagenmaterial von Beispiel 3 wurde mit 500 g destilliertem Wasser in einem Mixgerät zerkleinert. Diese Dispersion wurde zentrifugiert und der Überstand wurde entfernt. Zu der erhaltenen Kollagenfaseraufschlämmung wurden 17.5 g granulierter, kortikaler Rinderknochen, der gemäss der obigen Methode von Beispiel 1 gereinigt wurde, zugegeben, gefolgt von gründlichem Mischen und Entfernen von Wasser mittels Absaugen (70 mm). Der granulierte Knochen hatte eine Partikelgrösse von 0.5 bis 1.0 mm. Nach der Entfernung von Wasser wurden 5 ml einer 9% w/w wässrigen Gelatinelösung zugegeben (vernetzt mit 0.6% von 35% wässrigem Formaldehyd) und das Gemisch wurde wiederum durch Absaugen getrocknet. [0058] Die Schwammmasse wurde in Stücke geschnitten und in vacuo bei 60 deg. C getrocknet. Die Schwammstücke wurden in Polyethylenbehälter verpackt und mittels Gamma-Bestrahlung sterilisiert.
Claims (15)
1. Ein partikuläres Knochenmineralprodukt zur Reparatur von kombinierten Knorpeldefekten und Knochendefekten, wobei das Produkt poröse, von natürlichem Knochen hergeleitete Mineralpartikel, die eine Kristallstruktur haben, die im Wesentlichen der von natürlichem Knochen entspricht, und die im Wesentlichen frei von allem endogenen organischen Material sind, umfasst, wobei die Partikel an mindestens einer Oberfläche resorbierbare, physiologisch verträgliche Kollagen-II-Fasern besitzen, wobei das Gewichtsverhältnis der besagten Kollagen-II-Fasern zu besagten porösen Knochenmineralpartikeln mindestens 1:20 ist.
2. Das Produkt gemäss Anspruch 1, worin die besagten Partikel einen durchschnittlichen Durchmesser im Bereich von 0.1 mm bis 5 mm besitzen.
3. Das Produkt gemäss Anspruch 1, ferner umfassend mindestens eine absorbierte, pharmazeutisch oder biologisch aktive Substanz oder mesenchymale Stammzellen, die die Fähigkeit besitzen, sich zu Zellen zur Regeneration von Knorpel oder Knochen zu differenzieren.
4. Das Produkt gemäss Anspruch 3, worin die besagte, pharmazeutisch aktive Substanz ausgewählt ist aus der Gruppe bestehend aus Taurolidin, Taurultam und einer Mischung davon.
5. Das Produkt gemäss Anspruch 1, ferner umfassend Gelatine in einer Gelphase, worin die besagten Kollagen-II-Fasern in besagter Gelphase vorhanden sind und worin die besagte Gelphase 2-20 Gewichtsprozente des Gewichts des Knochenmaterial umfasst.
6. Verwendung des Produktes gemäss Anspruch 1 zur Herstellung eines Arzneimittels zur Reparatur von kombinierten Knorpeldefekten und Knochendefekten in einem menschlichen oder tierischen Subjekt, worin das Produkt in einen kombinierten Knorpeldefekt und Knochendefekt in einem Patienten implantierbar ist.
7. Verwendung gemäss Anspruch 6, worin der kombinierte Knorpeldefekt und Knochendefekt in einem Gelenkende eines artikulierenden Knochens lokalisiert ist.
8. Verwendung gemäss Anspruch 6, worin das Produkt in einen Knochendefekt einführbar ist und anschliessend das besagte Knochenmineralprodukt und der besagte Knochendefekt mit einer Kollagenmembran überziehbar ist.
9. Verwendung gemäss Anspruch 8, worin die besagte Kollagenmembran eine Schicht aus Kollagen II umfasst.
10. Verwendung gemäss Anspruch 8, worin die besagte Membran eine Matrixschicht, die vorwiegend aus Kollagen II besteht und eine offene, schwammartige Struktur besitzt, und mindestens eine Kollagenbarrierenschicht mit einer kompakten, glatten Barrierenoberfläche umfasst.
11. Verwendung gemäss Anspruch 8, worin die besagte Barrierenschicht vorwiegend aus Kollagen I, Kollagen II oder einer Mischung davon besteht.
12. Verwendung gemäss Anspruch 8, worin die besagte Membran eine Matrixschicht umfasst, die vorwiegend aus Kollagen II besteht und eine offene, schwammartige Struktur besitzt, aufgetragen auf eine weiche, faserige Schicht einer Kollagen-I/III-Membran, die eine kompakte, glatte Barrierenoberfläche entgegengesetzt der besagten Matrixschicht besitzt.
13. Das Produkt gemäss Anspruch 3, worin die besagte, pharmazeutisch aktive Substanz ausgewählt ist aus der Gruppe bestehend aus Bone Morphogenic Proteins, anderen Skelettmatrixmolekülen und Signalpeptiden.
14. Das Produkt gemäss Anspruch 13, worin die besagte, pharmazeutisch aktive Substanz ausgewählt ist aus der Gruppe bestehend aus BMP-2-8, TGF-beta , TGF-beta 1, VEGF, IGF, PTHrP und PDGF.
15. Das Produkt gemäss Anspruch 3 ferner umfassend artikulare Knorpelstammzellen oder Knochenstammzellen.
