正常及び病気の初代ヒト腱細胞はrhPDGF−BBに応答して増殖する
本研究は、腱障害を有する患者に由来する初代腱細胞の増殖及び/又は走化性をrhPDGF−BBが直接活性化するかどうかを調べた。そのような発見は、腱障害におけるrhPDGF−BBの治療可能性を裏付け得る。
患者
アキレス腱障害患者5名及び後脛骨筋腱(PTT)の腱障害患者5名を含む腱障害患者10名を本研究に用いた。膝の完全な関節置換術を受けた患者5名を更に含めた。
臨床的兆候に対して行われた再腱外科手術中に正常な腱及び損傷した腱から、廃棄されるはずであった腱組織を得た。これらの組織には、アキレス腱又はPTT腱の腱障害部分並びに長指屈筋(FDL)腱組織、アキレス腱組織、及び膝蓋腱組織の健康(腱障害部分でない)部分が含まれていた。これらの組織から初代腱細胞外植片培養を得、3〜5継代目に調べた。腱細胞の特定は、腱細胞特異的な遺伝子スクレラキシスの発現並びにコラーゲンa1(I)、a2(I)、及びa1(III)の遺伝子の発現について特異的プライマーを用いたリアルタイムPCRアッセイで評価することで確認した。
腱細胞単層をトリプシン処理し、0.5%の透析されたウシ胎児血清を含むDMEM/F12培地に再懸濁し、一晩付着させた後、漸増濃度のrhPDGF−BBと一緒に24時間インキュベートした。市販のアッセイ(ロシュ・アプライド・サイエンス社、インディアナ州インディアナポリス)を用いてDNA合成中における細胞中へのBrdU取込みに基づいて、細胞増殖速度の変化を評価した。各用量のrhPDGF−BBについて各培養を3重(triplicate)で調べた。
腱細胞単層をトリプシン処理し、0.5%の透析されたウシ胎児血清を含むDMEM/F12培地に再懸濁し、96ウェルChemoTx(登録商標)ディスポーザブル細胞遊走システム(ニューロ・プローブ社(Neuro Probe)、メリーランド州ゲイサーズバーグ)の上側チャンバーに入れた。下側チャンバーには漸増濃度のrhPDGF−
BBを入れた。チャンバーを隔てる膜を介して腱細胞を48時間遊走させた。次いで、96ウェルプレートをスピンし、3回凍結融解することで遊走した細胞を溶解させた。プロメガ社(ウィスコンシン州マディソン)から市販されているキットを用いて、細胞質の乳酸デヒドロゲナーゼ(LDH)に基づいて、遊走した生細胞の量を測定した。
rhPDGF−BBを用いた刺激が用量依存的に腱細胞の増殖に影響するかどうかについて、一元配置ANOVAを用いて決定した。
腱細胞培養だけがスクレラキシスmRNAを発現し、対照の肺線維芽細胞培養又は対照の一次T細胞培養はこれを発現しなかった。一方、腱細胞及び線維芽細胞はコラーゲン遺伝子mRNAを発現したが、リンパ球はこれを発現しなかった。
これらの実験の結果は、健康組織及び腱障害組織に由来する腱細胞が、増殖速度及び走化性速度を増大することによりrhPDGF−BBに応答することを示唆している。重要なことに、一部の患者に由来する腱細胞はPDGFに対して逆説的な応答を示し、低用量
より高用量の方が影響が小さかった。同様に重要なことに、疾患腱に由来する腱細胞は、いくつかの場合で、PDGFに対して健康な腱に由来する腱細胞と示差的な応答をした。このことは、臨床状況において適切な投薬が最も重要となり得ることを示している。
rhPDGF−BBを用いた安全性の研究
局所注射試験

rhPDGF−BBをラットに単回アキレス腱内注射しても死亡率又は瀕死状態に対する影響はなかった。更に、臨床観察、体重への影響、又は食物消費量に、試験物質に関連する生物学的に有意な差はなかった。
本研究の目的は、イヌのアキレス腱内に送達した後の組換えヒト血小板由来成長因子(rhPDGF−BB)の局所的毒性を決定することである。
本研究の目的は、静脈内注射によって投与されたrhPDGF−BBの全身毒性を決定することである。
コラゲナーゼ誘発ラットアキレス腱損傷モデルにおけるrhPDGF−BBの腱内(IT)適用の用量反応
本研究の目的は、ラット腱コラゲナーゼモデルにおいてrhPDGF−BBの腱内適用の用量反応を決定してアキレス腱の損傷及びリモデリングに対するrhPDGF−BBの修復効果を検証することである。本発明者らは、rhPDGF−BBの腱内送達により、細胞増殖がアップレギュレートされ且つ腱の生体力学的強度が回復することによって、腱が修復されるという仮説を立てた。
リックスを沈着させて損傷組織を再生する。更に、上記実施例1に示されるように、PDGF−BBにさらされた腱細胞はDNA合成及び走化性の増大を示した。
腱障害を評価するための良く確立された1つのモデルはない。しかし、コラゲナーゼ誘発ラットアキレス腱損傷モデルはアキレス腱損傷に広く使用されてきた。このモデルは、腱炎に相当すると考えられる変性的な腱応答を開始し、上り坂トレッドミル過剰使用モデル(4ヶ月)と比べて急速に腱炎損傷を発症する(3日以内)。したがって、これは、腱損傷に対するrhPDGF−BBの効果を短時間でスクリーニングするモデルとなる。したがって、このモデルは、rhPDGF−BBの治療効果の審査に、誘発期間が比較的短く、非常に好適であり、臨床的腱炎に相当する。
研究対照群に、損傷した腱の自然な治癒応答を見積もるための対照として、rhPDGF−BBを用いないコラゲナーゼ処置ラットアキレス腱のラット腱の自然な修復応答を用いた。研究試験物質に、腱の再生を助ける注射用薬物としてrhPDGF−BBを用いた。組換えヒト血小板由来成長因子BB(rhPDGF−BB)は、骨芽細胞、腱細胞、軟骨細胞、間葉系幹細胞等の間葉起源細胞に分裂促進性及び走化性である。したがって、筋骨格系の損傷部位に導入されると、rhPDGF−BBは結合組織細胞及び前駆細胞を処置部位に引きつけ、その増殖を刺激することにより細胞の数を増やし、これがその後マトリックスを沈着させて損傷組織を再生する。更に、上記実施例1に示されるように、PDGF−BBにさらされた腱細胞はDNA合成及び走化性の増大を示した。

165頭の雄のスプラーグドーリーラット(チャールズ・リバー・ラボラトリーズInt’l社、マサチューセッツ州ウィルミントン)を本研究に用いた。研究に選択する前に、健康及び正常な歩行を確認する視覚検査により全動物をスクリーニングした。全ての動物を、体重に基づいて研究に選択した(コラゲナーゼ注射時におよそ315グラム)。各ラットは尾に書かれた固有の番号により識別した。体重に従ってラットをランダムに各群に割り当てた。
ケージ当たり4頭で収容した動物(1群当たり15頭)にイソフルランで麻酔し、踵関節領域を刈りとって注射のために消毒した。28.5Gの針を付けたインスリン注射器を用いて、全ラットの骨−腱接合部近くの右アキレス腱にコラゲナーゼ(50mMのNaH2P04及び150mMのNaClを含むpH7.4のPBSに溶解した10mg/mlのもの50μl)を注射した(図3)。コラゲナーゼ注射の7日後、28.5Gの針を付けたインスリン注射器を用いて、ビヒクル又は1.02μg、10.2μg、若しくは102μgのrhPDGF−BBを総体積30μlで投与した。腱のダメージの組織病理学的評価及び生体力学のために7日目(ベースライン)、14日目、28日目に動物を殺した。15頭の各群について、6頭を組織病理学に用い、9頭の後脚(処置及び非処置後肢の両方)を取り、解剖し、その後の生体力学的評価のために凍結した。生体力学的評価は以下に説明する。
屠殺するまで動物を少なくとも1日1回観察した。動物の処置はUSDAの動物保護法(連邦規則集第9巻の第1部、第2部、及び第3部)に略述されている規則並びに実験動物の管理と使用に関する指針(ILAR publication, 1996, National Academy Press)及びHSS獣医手技に規定されている条件に従った。

剖検の直前に、以下のシステムに従って足首の太さをスコア化した:0=成長なし;1=軽度の成長;2=中程度の成長;3=重度の成長。
剖検では、皮膚を注意深く踵関節領域から外し、足全体を10%中性緩衝ホルマリン(NBF)に浸し、屈曲状態で固定した。10%NBF中で最低12時間及び脱灰のための10%ギ酸中で4〜5日の後、腱−骨接合部を特に強調して足首の内側及び外側で切り整え、約1/4インチ厚の組織ブロックを得(腱が付着している足首の中央部)、ラベルした組織用カセットに入れた。切り整えた足首組織ブロックを矢状方向でパラフィン包埋に処理した。回転式ミクロトームを用いて、骨−腱接合部が好適に視覚化されるまで、200ミクロン毎に代表的な4〜6ミクロン厚の切片を取り、その後、これらの切片をヘマトキシリン・エオジン(H&E)染色、マッソン三色染色し、増殖細胞核抗原(PCNA)の免疫組織化学的検出に用いた。
H&E染色により、炎症細胞の種類(好中球、リンパ球、及びマクロファージ)を決定した。炎症を以下のようにスコア化した:(a)0=炎症なし;(b)1=微小炎症(100%単核;好中球なし);(c)2=中程度の炎症(好中球=19%;細胞の残りは単核);及び(d)3=顕著な炎症(好中球=20%;細胞の残りは単核)。
コラーゲン原線維の組織化をH&E及びトリクロームによる染色により評価した。コラーゲンの組織化を以下のようにスコア化した:(a)0=コラーゲン原線維が完全に組織崩壊している;(b)1=コラーゲン原線維の一部は整列しているが、束の大部分が高度に組織崩壊している;(c)2=コラーゲン原線維が高度に整列しているが、束はまだ幾分組織崩壊している;及び(d)3=組織中に存在するコラーゲン原線維が完全に整列しており、コラーゲン束の組織崩壊がない。
修復組織中のコラーゲン繊維密度をH&E及びトリクロームによる染色により評価した。コラーゲン繊維密度を以下のようにスコア化した:(a)0=低密度コラーゲン束;(
b)1=中密度コラーゲン束;及び(c)2=高密度コラーゲン束。
修復組織における血管新生をH&E及びトリクロームによる染色で評価した。血管新生を以下のようにスコア化した:(a)0=なし(血管新生なし);(b)1=中程度;及び(c)2=豊富。
PCNA(増殖細胞核抗原)の免疫組織化学(IHC)を用いて細胞増殖を評価した。接眼マイクロメーターを用いて39.4×197μm(7762μm2)の領域又はマイ
クロメーター上の10×50ユニットを描いた。各検体について、この寸法の3つの視野を計数した。これらの3つの等距離の視野から、増殖している細胞を計数した。
顕微鏡下で接眼マイクロメーターを用いて腱の2つの異なる部位を測定した。踵骨付着点(付着部位の厚さを最もよく表すと考えられる非接線領域(non−tangential area))及び腱自体(増殖応答のある付着部位から遠い腱本体の最も厚い非接線領域)の測定値を取った。
本研究のこの部分には99頭の動物を用いた。損傷を誘発した腱を、反対側のコラゲナーゼ処置しなかった腱と共に評価した。合計123個の生体力学的検体を評価した。処置の割当ての概略を上記表1に示す。
荷重が0.05N未満となるまで、サンプルを引っ張って破損するまで試験する。試験前に、検体の横断面積を測定した。特定可能な群の分類を一切示さず、盲検的に全ての検体を試験した。
肉眼的な足首の太さ(Gross Ankle thickness)へのrhPDGF−BBの影響
rhPDGF−BBの単回腱内注射の7日後に肉眼的な足首の太さの用量依存的増加が観察され(図6A及び図7、Tx後7日目)、中用量(10.2μg)及び高用量(102μg)で注射7日後に腱の大きさが有意に増加していた。Tx後21日目では、ビヒクル対照群(酢酸ナトリウムバッファー)以外の全群で肉眼的な足首の太さが減少していた。このことは、処置後21日目における組織のリモデリングを示唆している(図6B及び図7、Tx後21日目)。図6A、6B、及び7のデータは、rhPDGF−BB処置後7日目及び21日目におけるアナログスケールに基づいている(0=成長なしから、3=重度の成長)。記載データは、1群当たりn=6頭の足首の太さの平均(±SEM)である。
処置時の動物の平均体重は314.48グラムであった。処置後7日目及び21日目の平均体重はそれぞれ396.4及び467.0グラムであった。いずれの時点でも処置に関連する体重の変化はなかった。
異常な骨の成長及び再吸収のどちらも確認されなかった。
図8は、処置後7日目及び21日目の踵骨付着点において測定した腱幅(μm±SEM)を示す図である。記載データは、1群当たりn=6頭の踵骨付着点における腱幅の平均(±SEM)である(*ビヒクル対照群に対してp=0.01)。腱幅は接眼マイクロメ
ーターを用いた顕微鏡観察により測定した。
−BB用量群で踵骨付着点における腱幅の有意な増加(〜137%の増加)が観察された(図8、Tx後7日目)。3.24及び324μg/kgのrhPDGF−BBで処置した動物は、腱幅の有意な増加を示さなかったが、ビヒクル対照より大きい傾向があった。処置後21日目では、rhPDGF−BBで処置した動物及び処置しなかった動物の腱幅に有意な差は測定されなかった(図8、Tx後21日目)。処置後21日目(図8、Tx後21日目)までに、32.4μg/kg rhPDGF−BB用量群は、処置後7日目
と比べて40%の腱幅の減少を示した。このことは、腱がリモデリングされたことを示唆している(図8)。
図9は、処置後7日目及び21日目において測定された腱中央部分の腱幅(μm±SEM)を示す図である。記載データは、1群当たりn=6頭の腱本体の腱幅の平均(±SEM)である(*ビヒクル群に対してp<0.05)。腱幅は、接眼マイクロメーターを用
いた顕微鏡観察により測定した。
対して〜97%の腱幅増加が観察された(図9、Tx後7日目)。3.24及び324μg/kg用量群では、ビヒクル群及び非処置(Tx)群と比べて有意でない用量依存的増加が観察された。21日目には、全群で腱幅の有意な差は観察されなかった(図9、Tx後21日目)。しかし、21日目に、32.4μg/kgのrhPDGF−BB用量群は処置後7日目と比べて116%の腱幅減少を示した。これは腱がリモデリングされたことを示唆している。
細胞増殖の定量化は、免疫染色されたPCNA陽性細胞の細胞計数によりなされた(図10)。記載データは、1群当たりn=6頭の細胞数の平均(±SEM)である(*ビヒ
クルに対してp<0.05)。各検体について、寸法の等しい3つの視野を数えた。
殖レベルまで戻っていた。このことは、rhPDGF−BBの増殖反応が可逆的であることを示唆している(図10、Tx後21日目)。
全群、全時点で、マクロファージ及び単核細胞からなる軽度から中程度の炎症が観察された(表2)。
全群、全時点で、血管新生の有意な変化は観察されなかった(表3)。
全群、全時点で、コラーゲン密度の有意な変化は観察されなかった(表4)。
全群、全時点で、コラーゲン組織化の有意な変化は観察されなかった(表5)。
このモデルにおいて、断裂までの最大荷重値によれば、非処置群は時間と共に自然修復した(図11)。処置後7日目における断裂までの平均最大荷重値は、ビヒクル対照群及び非処置群と比べて32.4μg/kg用量群で有意に上昇していた(それぞれ〜43%及び27%)(図11、Tx後7日目)。処置後21日目でも、断裂までの最大荷重はビヒクル対照と比べて依然として有意に上昇していた(図11、Tx後21日目)。処置後21日目では、32.4μg/kg用量群及び非処置群の平均最大荷重はほぼ同じであり、このことは、rhPDGF−BB処置により、非処置の場合より修復速度が上がったことを示している。しかし、ビヒクル群及び324μg/kg rhPDGF−BB群では
、投薬後21日目に32.4μg/kg用量群及び非処置群よりも断裂までの最大荷重値が有意に下がっていた。記載データは1群当たりn=9頭の断裂までの最大荷重の平均(±SEM)である(+「ビヒクル」群に対してp<0.05;**「非処置」群に対してp
<0.05)。
裂までの平均最大荷重値(26.8N)は、コラゲナーゼ注射後14日目における無傷の腱(19.03N)より41%増加していた。これは、32.4μg/kg rhPDG
F−BB群の生体力学的特性が、通常の発達をしている腱より速く成熟したことを示唆している。更に、コラゲナーゼ注射後14日目における32.4μg/kg rhPDGF
−BB群の断裂までの平均最大荷重(26.8N)は、コラゲナーゼ注射後28日目における無傷の腱のもの(27.98N)に近づいていた。7、14、及び28日目の無傷群
及び非処置(コラゲナーゼ注射)群における断裂までの最大荷重値はほぼ同じであった。14日目(Tx後7日目)、32.4μg/kg rhPDGF−BB群は、ビヒクル群
、無傷群、及び非処置群より断裂までの最大荷重値が高く、それぞれ43%、41%、及び27%増加していた。しかし、28日目(Tx後21日目)では、無傷群、非処置群、及び32.4μg/kg rhPDGF−BB群の断裂までの最大荷重はほぼ同じであっ
た(表6)。
生存中パラメーター
足首の太さの結果から、rhPDGF−BB処置後7日目には腱の大きさが用量依存的に増加するが、21日目までには成長が止まり、組織がリモデリングされて腱の大きさが小さくなることが示された。
rhPDGF−BBは細胞増殖、特に線維芽細胞増殖を増加させた。細胞増殖及び顕微鏡的腱幅の増加は可逆的であり、これは組織のリモデリングの潜在的適応期を示している。
断裂までの生体力学的荷重データから、非rhPDGF−BB処置コホートと比べた時、rhPDGF−BBが腱の力学的強度のより速い修復反応を開始させることが示された。処置後21日目までに細胞増殖及び腱幅はベースラインに戻ったが、これによって生体力学的な腱の強度は消失しなかった。
本研究の生物学的及び生体力学的結果により、腱内に注射されたrhPDGF−BBの局所的安全性が確認された。rhPDGF−BB1回注射後、21日間で弱まる腱成長の初期段階があった。本研究では、接眼マイクロメーターを用いて顕微鏡下で腱幅を測定し、PCNA免疫染色を用いて細胞増殖を測定した。それらの測定の結果、32.4μg/kgのrhPDGF−BBで処置された動物で、処置後7日目に顕微鏡的腱幅及び細胞増殖が増加することが示された。しかし、処置後21日目までに、腱幅及び増殖は対照レベルに戻った。腱へのrhPDGF−BB注射後21日間にわたり、異所性の又は異常な骨、及び異常な腱の成長は観察されなかった。
組換えヒト血小板由来成長因子−BB(rhPDGF−BB)静脈内投与後のスプラーグドーリーラットにおけるrhPDGF−BBの薬物動態
本研究の目的は、雄のスプラーグドーリーラットに単回静脈内投与されたrhPDGF−BBの薬物動態を評価することである。本研究は、静脈内投与後の体液(血清)に由来するナイーブrhPDGF−BBのクリアランスを評価するようにデザインした。rhPDGF−BBの薬物動態学的性質及び薬力学的性質をより深く理解するために、静脈内曝露後の全身クリアランスを決定した。本発明者らは、rhPDGF−BBの静脈内投与後のクリアランス速度は速いという仮説を立てた。
ラットモデル
ラットは、種々のクラスの化学物質の薬物動態及び毒性を評価するために一般的に使用されている齧歯類モデルであり、ラットには大きな過去のデータベースが存在する。合計48頭の雄のスプラーグドーリーラットを本研究に用いた。6頭/群/時点で動物を2群に分けた(表7)。本研究は、本研究の目的、本発明者らの科学的ニーズ、及び現代の科学的標準を満たす、可能な限り少ない数の動物を用いるようにデザインされた。このデザインはデータの有意義な分析を可能にするのに十分な大きさの群を提供する。rhPDGF−BBは、インスリン注射器(28.5G)を用いて側尾静脈に静脈内送達により投与した。
試験物質は0.4mg/mlのrhPDGF−BBを含む20mM酢酸ナトリウムバッファー(pH6.0±0.5)である。対照物質は20mM酢酸ナトリウムバッファー(pH6.0±0.5)である。
セタート)バッファーの試験物質の少なくとも2つの未開封、未使用バイアルを、投薬、安定性、及び濃度分析に用いるバイアルと同じ保存条件(4℃)下に保持した。UV/Vis分光測定及び逆相HPLC分析からなる安定性及び用量の検証分析を行った(付属書類II)。
本研究には48頭の雄のスプラーグドーリーラットを用いた。本研究で使用したラットはその重量に基づいて選択した。処置時点における平均重量は約227グラムであった。各ラットは永久マーカー(permanent marker)で書かれた固有の尾番号により識別した。麻酔等の適当な研究関連事象の際には食物及び水を控えたが、それ以外では自由に摂取させた。研究選択前に、視覚的検査によりすべての動物をスクリーニングした。
血清中でrhPDGF−BBの100%のバイオアベイラビリティ及び薬物動態特性が達成されるように静脈内送達を選択した。用量は、ラット腱コラゲナーゼモデルにおけるrhPDGF−BB腱内適用の用量反応の有効性に関する先の研究(実施例3参照)に基づいて選択した。本研究で使用される最大用量濃度は、ラットの重量が0.227kgで
あったことに基づき、100μg、すなわち0.44mg/kgである。
屠殺するまで動物を少なくとも1日1回観察した。rhPDGF−BB注射前及び屠殺前に体重を記録した。食物消費量は定性的であった。
与により動物を安楽死させた。反射(まばたき、引っ込み等)の欠如により死亡を確認した。本研究中、肉眼的又は組織病理学的観察は行わなかった。予定外の動物の死亡は記録されなかった。
約600μlの血液を血清チューブに取り、1,800g、室温で10分間遠心して血清を得た。約300μlの血清を2mlのエッペンドルフチューブに入れた。分析前に血清を−70℃で保存した。
各時点でrhPDGF−BB血清濃度を測定した。R&Dシステムズ社のQuantikine ELISAキットを用いてrhPDGF−BBを定量化した
WinNonlinの非コンパートメントモデルを用いて以下のパラメーターを計算した:最終半減期(t1/2)、Tmax、Cmax、AUC0−last、及びCL。分析は、Quantikine ELISAキットアッセイを用いて血清中で決定される種々の時点で存在するrhPDGF−BBの量を用いて行われた。
統計分析は、必要に応じて、平均、標準偏差、変動係数等の記述的パラメーターに限定した。
処置時の全動物の平均体重は227グラムであった。動物は平均用量440.53μg/kgのrhPDGF−BBを受けた。
rhPDGF−BBの静脈内(IV)投与では、初期に全身曝露量が高くなった。これは、投薬後の最初の10分間に血液から迅速に排除される中間的クリアランス分子である。
rhPDGF−BBの腱内投与後のスプラーグドーリーラットにおけるrhPDGF−BBの薬物動態

32頭の雄のスプラーグドーリーラットを本研究に用い、重量に基づいて選択した(注射時に約294グラム)。ラットを体重に基づいてランダムに各群に割り当てた。
アキレス腱内投与は、診療所におけるrhPDGF−BBの投与を真似たものである。腱−踵骨接合部の注射部位は、腱及び骨の付着点部位を真似たものである。
腱に1頭当たり30μl(0.102mL/kg)の標的送達体積で、平均用量347μg/kg、34.7μg/kg、及び3.47μg/kgのrhPDGF−BBを単回腱内ボーラス投与された。第4群は、1頭当たりの標的送達体積30μlで、20mM酢酸ナトリウムバッファーの単回腱内注射を受けた。処置は、インスリン注射器(28.5G)を用いて骨腱接合部近くの右アキレス腱中に注射された。血清用の空の1cc注射器(26G)を用いて、以下の出血時間で動物の尾静脈から出血させた(全血〜400μl、血清〜200μl)。Quantikine ELISAを用いて血清中のrhPDGF−BBを測定した。

WinNonlinの非コンパートメントモデルを用いて以下のパラメーターを計算した:最終半減期(t1/2)、最高濃度到達時間(Tmax)、Tmaxで起こる最高濃度(Cmax)、t=0時間からt=最終観察時点までの薬物濃度−時間曲線下面積(AUC0-last)、及びバイオアベイラビリティ(%F)。分析は、Quantikine ELISAキットアッセイを用いて血清中で決定された種々の時点で存在するrhPDGF−BBの量を用いて行った。
身クリアランス(CL/F)に基づいて、時間=0から無限大まで外挿した薬物濃度−時間曲線下面積等の他のパラメーターを計算することができる。
統計分析は、必要に応じて平均値、標準偏差、変動係数等の記述的パラメーターに限定した。
処置時点の全動物の平均体重は294グラムであった。平均用量347μg/kg(102μg)、34.7μg/kg(10.2μg)又は3.47μg/kg(1.02μg)のrhPDGF−BBを動物に投与した。
7μg/kg及び3.47μg/kg用量群のrhPDGF−BB血清濃度は全ての時点で定量レベルより低かった(<0.156ng/mL)ので、薬物動態パラメーターは計算されなかった。
rhPDGF−BBの腱内(IT)投与では、初期に速くて低い全身曝露量が見られた。347μg/kgのrhPDGF−BBより低い用量では、検出可能な血清濃度のrhPDGF−BBは観察されなかった。
外側上顆炎へのrhPDGF−BB注射の影響に関する第II相ランダム化単回投与用量漸増二重盲検プラセボ対照多施設研究
本研究の目的は、「テニス肘」としても知られる外側上顆炎の処置としてのrhPDGF−BB注射の有効性を評価することである。
研究は最大6ヶ所の臨床現場で行う。研究集団は、1群当たり対象25名で4群にラン
ダム化された対象100名である。各コホートにはプラセボ患者5名及び実薬処置患者20名が含まれる。
本研究は、第II相ランダム化単回投与漸増用量二重盲検プラセボ対照多施設研究である。研究登録の選択基準を満たす適格な対象者を、研究参加のためのインフォームド・コンセントを得た後に登録する。対象は処置後24週間フォローされる。研究登録後のランダム化により、1:1:1:1:1の計画で以下の処置群の1つに対象を割り当てる:(1)用量A:バッファーのみ−対照群;(2)用量B:バッファー+0.45mg rh
PDGF−BB;(3)用量C:バッファー+0.75mg rhPDGF−BB;(4
)用量D:バッファー+1.5mg rhPDGF−BB;及び(5)用量E:バッファ
ー+3.0mg rhPDGF−BB。全ての用量で、総体積は、1用量当たりrhPD
GF−BB溶液1.5mLである(それぞれ、0.3mg/ml、0.5mg/ml、1.0mg/ml、及び2.0mg/mlのrhPDGF−BB処置群に相当し、それぞれ70kgのヒトで6.4、10.7、21.4、及び42.9μgのrhPDGF−BBに相当する)。バッファーは20mM酢酸ナトリウム(pH6.0)である。

込まれて研究登録目標に達するまで繰り返される。
本研究に含まれる対象は、抵抗を加えた回外運動又は手首の背屈により症状が再現可能
であり、外側上顆炎の臨床診断を受けている、21歳以上の対象である。
本研究から除外される対象は、過去3ヶ月以内にコルチコステロイド注射療法を受けたことがある又は外側上顆炎を処置するための外科的処置を受けた対象;酵母由来産物に対するアレルギーを有する対象;手根管症候群の病歴がある対象;頸部神経根症の病歴がある対象;及び処置の6ヶ月以内に患部の肘に外傷を有した対象である。
登録は約9ヶ月である。追跡調査のための来診には、手順後24週間までの握力検査及び四肢の身体検査が含まれる。安全性のエンドポイントは24週間目の最後の来診まで監視された。
(1)安全性。有害事象の発生を評価することによりrhPDGF−BBの安全性及び忍容性を評価した。rhPDGF−BBは安全であり、患者に耐えられた。
研究手順前の2回目の来診及び研究手順後の4、8、12、及び24週目の来診で以下の評価を完了する:(1)上肢障害評価表のスコア(DASH)、(2)視覚的アナログスコア(VAS)、(3)握力検査により測定される努力の誠実さ。rhPDGF−BBを用いた処置は、処置前のスコアからの向上(DASH及びVAS)、圧力及び/若しくは手首の屈曲よる疼痛の軽減、患部四肢の可動性の向上、並びに/又は握力の上昇等の腱の臨床的転帰の1又は複数の有益な変化をもたらす。
ラットモデルにおける腱治癒に対するrhPDGF−BBコーティング縫合糸の影響:組織学的及び生体力学的研究
要約
序論
アキレス腱の裂傷は、外科的修復が必要となることが多い一般的な損傷である。本研究の目的は2つあり、(1)rhPDGF−BBでコーティングされた縫合糸を用いて修復部位に適切な量の因子を送達することができるかどうかを決定すること及び(2)生体力学及び組織学に基づいてrhPDGF−BBでコーティングされた縫合糸がラットアキレス腱モデルにおける治癒を改善するかどうかを決定することである。
以前に記載されているディップコーティングプロセスを用いて種々の濃度のrhPDGF(0、0.3、1.0、及び10.0mg/ml)で4−0バイクリル縫合糸をコーティングした。0 rhPDGF群を対照とした。ELISAを用いて、種々のディップコ
ーティング溶液中でコーティングされた後の縫合糸上のrhPDGFの濃度を決定した。
rhPDGFで縫合糸をコーティングすることに成功した。ディップコート濃度が高い
ほど、縫合糸上のrhPDGFの量が多くなった。組織学的分析により、対照群とPDGF群の間でコラーゲンスコア又は血管形成に有意な差がないことが示された。生体力学的結果から、対照(1.0±0.2MPa)と高用量PDGF群(1.9±0.5MPa及び2.1±0.5MPa)との間に極限引張応力の有意な差が示された。引張ヤング率は、PDGF 10mg/ml群(7.22、SD 3.79)で他の全ての群より有意に高かった。これにより、PDGFコーティング縫合糸の正の用量反応及び強度改善が示された。
本研究は、修復された腱の材料特性がrhPDGFコーティング縫合糸によって正の用量依存的に改善され得るという本発明者らの仮説を証明した。
本研究で、本発明者らは、再現可能な量のrhPDGF−BBで4−0バイクリル縫合糸をコーティングでき、rhPDGF−BBコーティング縫合糸を用いたラットアキレス腱修復の増強により、生体力学的及び組織学的な評価で修復が用量依存的に改善されると仮説を立てた。
本研究は2つのパートに分けて行った。1)インビトロでの縫合糸のコーティング及び分析並びに2)ラットモデルにおけるインビボでのアキレス腱修復。手順はIUCACによる承認を受けた。
縫合糸コーティング:
4群の4−0バイクリル縫合糸(エチコン社(Ethicon)、ニュージャージー州サマービル)を、以前に記載されているようにディップコーティングプロセスを用いて(Dines J, Weber L, Razzano P, et al. The Effect of Growth Differentiation Factor-5-Coated Sutures on Tendon Repair in a Rat Model. J Shoulder Elbow Surg 2007;16:215S-221S)、(1)20mM酢酸ナトリウムバッファー(キャリア対照)、(2)0.
3mg/ml rhPDGF−BBを含むバッファー、(3)1.0mg/ml rhPDGF−BBを含むバッファー、及び(4)10.0mg/ml rhPDGF−BBを含
むバッファーでコーティングした。簡潔に述べると、70%エタノールで処置した後、(針の付いた)縫合糸を、rhPDGF−BB(0、0.3、1.0、及び10.0mg/ml)を含む又は含まない酢酸ナトリウムバッファー中に30分間浸漬し、その後、風乾した。Dines et al.に記載されているプロセスと異なり、コーティング溶液中にゼラチ
ンは使用しなかった。生体内での研究に使用するために縫合糸を長さ15cmに切り揃えた。切り揃えた縫合糸の残った長さの部分をインビトロ分析に用いた。
酢酸バッファー(第1群)又はrhPDGF−BB(第2〜4群)の溶液でコーティングされた縫合糸(n=5/群)を溶出バッファー(2%のウシ胎児血清、1%のペニシリン−ストレプトマイシン、1%のL−グルタミン、及び1%のHEPESバッファーを含む最小必須培地)中に入れ、振盪機(rocking platform)の上で37℃でインキュベートした。溶出バッファーは1、6、24、及び48時間の時点で完全に交換した。PDGF−BB ELISA(human PDGF−BB DuoSet、R&Dシステムズ社、ミネソタ州ミネアポリス)を用いて、各時点で放出された全rhPDGF−BBを決定した。
研究デザイン
48頭のスプラーグドーリーラット(350〜400グラム)を、盲検的に4つの処置群のいずれかにランダム化した(各群につきn=12)。群は4つの異なる濃度のrhPDGF−BBを含む上記ディップコーティング溶液、すなわち対照群(0mg/ml r
hPDGF−BB)及び3つの実験群(0.3、1.0、及び10.0mg/ml rh
PDGF−BB初期コーティング濃度)からなる。
全ての手術は無菌状態で行われた。アキレス腱の上の皮膚を1.5cm切開した。ブラントジセクションを用いてアキレス腱を露出させた。この時点で、メスを用いて踵骨の付着点近くで腱を横切した。すぐに、4つの群のいずれかの縫合糸を用いた1種類のmodified Mason-Allen縫合及び1種類の単純結節縫合を用いて腱を修復した。いずれの縫合の実施においてもラットアキレス腱が小さいことによる困難はなかった。コーティングされていない結節バイクリル縫合糸を用いて皮膚を閉じた。外科的修復後、動物は普通に歩き回れるようにした。4週間後、ラットを屠殺し、踵骨の骨片及び近位の腓腹筋−ヒラメ筋複合体を含む腱を回収した。検体を生体力学的分析(n=8/群、新鮮凍結)又は組織学的分析(n=4/群、ホルマリン固定)にランダムに割り当てた。
荷重精度±0.5%の100Nのロードセルを取り付けたインストロン社製のシステム(モデル番号5566)を用いて各検体に生体力学的一軸引張分析を行った。サンプルを、プロテアーゼ阻害剤を含むリン酸緩衝食塩水(PBS)中で4℃で最大4時間解凍した後、解剖して余分な筋肉を除去し、踵骨及び腓腹筋−ヒラメ筋末端を220グリットのサンドペーパーで包んだ。検体を空気圧グリップの間に固定し、PBS浴に浸漬した。サンプルを引っ張って予荷重(1N)を負荷し、サンプルの寸法を測定した。次いで、腱を歪み速度0.25%/秒で引張伸張にさらした。検体が破損するまで引っ張り、パソコンで荷重及び伸張データを収集した。データは10Hzで取得した。試験後、収集したデータを分析して、それぞれ荷重−変位又は応力−歪み曲線の線形部分から(1)線形剛性及び(2)弾性係数を決定した。荷重曲線から最大荷重及び極限引張強さも計算された。
腱検体をアキレス腱付着部位で踵骨から分離し、修復されたアキレス腱を処理してパラフィン中に包埋した。各検体の矢状切片をマロリー3色染色又はシリウスレッド染色により染色した。スライドは画像化され、以前に記載されているように(Dines J, Weber L, Razzano P, et al. The Effect of Growth Differentiation Factor-5-Coated Sutures on Tendon Repair in a Rat Model. J Shoulder Elbow Surg 2007;16:215S-221S)コラー
ゲン組織化及び血管形成の程度についてのスコア化シートを用いて3人の盲検観察者によりスコア化された。処置群について知らされていない3人の独立した観察者が各スライドを評価した。各処置群の合計スコアを計算し、統計的有意性があるかどうか比較した。
時間を反復測定として反復測定ANOVAを用いて、インビトロで放出されたrhPDGF−BBの量を分析した。初期コーティング濃度に対する放出されたrhPDGF−BBの累積量及び送達された総用量は非線形(log−logスケール)最小二乗法に一致していた。一元配置ANOVAをボンフェローニの多重比較検定と共に用いて、群間における生体力学的パラメーターの差を決定した。クラスカル・ワリス検定をダンの多重比較事後検定と共に用いて組織学的分類スコアの統計解析を行った。生体力学データを平均値±SEMで表し、組織学スコアを中央値(範囲)で表す。有意性はp<0.05で決定した。
インビトロrhPDGF−BB放出
インビトロ放出データは、放出されたrhPDGF−BBの量として表されており、縫合糸の長さで正規化されている(ng/cm)(図14A及び14B)。インビトロ放出は、他の群にとってのベースライン測定値と見なされる第1群(0mg/ml)(累積放出量:1.3±0.1ng/cm)を含む全ての群で測定された。インキュベーションの最初の1時間後に、縫合糸からのrhPDGF−BBのボーラス放出が観察された(第2群:6.29±3.0ng/cm;第3群:68.81±9.3ng/cm;及び第4群:5809.42±541.6ng/cm)(図14A)。最初のボーラス放出に続いて、48時間の時点まで、更にrhPDGF−BBが連続的にゆっくりと放出された。48時間のインキュベーション中に放出されたrhPDGF−BBの累積量は用量依存的であり、ディップコーティング溶液の濃度が高いほど多くのrhPDGF−BBが放出され、第4群(10mg/ml;6495.9±552.6ng/cm)で、第2群(0.3mg/ml;14.0±5.7ng/cm)及び第3群(1.0mg/ml;126.8±18.8ng/cm)と比べて有意に増加していた(p<0.001)(図14B)。第2群と第3群で放出されたrhPDGF−BBの累積量に有意な差はなかった(p>0.05)。放出されたrhPDGF−BBの累積量(Y、ng/cm)は以下の式により初期コーティング濃度(X、mg/ml)に対数比例していた(R2=0.9575)。

全ての動物は手術に好ましい反応を示したが、例外として(組織学的検査に割り当てられた)第3群の1頭は麻酔による合併症により手術後一晩で死亡した。修復に使用された縫合糸の長さは群の間で一致していた(第1群:4.8±0.1cm;第2群:4.7±0.2cm;第3群:4.8±0.1cm;及び第4群:4.8±0.1cm;p=0.94)(図14D)。インビトロで放出されたrhPDGF−BB平均量及び縫合糸の長さに基づいて、各群のインビボでの用量(平均±SEM)は以下のように計算された:第1群:0ng;第2群:66.0±61.2ng;第3群:602.5±190.9ng;及び第4群:31,342.6±6774.5ng。放出されたrhPDGF−BBの累積量同様、インビボで送達される用量(Y、ng)は、以下の式で初期コーティング濃度(X、mg/ml)に対数比例した(R2=0.9859)(図14C)。

5つの検体は、その試験日にインストロン社製試験機でエラーが生じたため、生体力学的分析に使用することができなかった。生体力学的分析に得られた検体の数はn=7(第1群、第2群、及び第3群)及びn=6(第4群)であった。

.1±0.2MPa)並びに0mg/ml群と比べた1.0mg/ml(1.9±0.2MPa)rhPDGF−BB群で見られた。弾性係数は、10.0mg/ml(7.22±1.5MPa)rhPDGF−BB群で他のすべての群(0mg/ml:3.5±0.4MPa;0.3mg/ml:4.4±0.4MPa;1.0mg/ml rhPDGF
−BB:4.9±0.7MPa)と比べて有意に増加していることが観察された(p<0.05)。0、0.3、及び1.0mg/ml rhPDGF−BB群には互いに統計的
な差はなかった。
各群の動物4頭の組織学的検査を行った(第3群はn=3)。各切片を3人の採点者がスコア化し、平均スコアを求めた。平均スコアは中央値(範囲)で表される。組織学的分析は、群間でコラーゲン組織化又は血管形成のスコアの有意な差を示さなかった。しかし、対照群と比べてrhPDGF−BB群で、特にコラーゲン組織化のスコアで、より正常な外観が観察される傾向があった。
本研究で、本発明者らは、rhPDGF−BBをバイクリル縫合糸にコーティングでき、インビトロで縫合糸から放出されるrhPDGF−BBの量が初期コーティング濃度に依存することを実証した。構造的機械的特性(極限荷重及び剛性)又は組織学的試験で有意な差は観察されなかったが、rhPDGF−BB処置群でこれらの特性の改善傾向が見られた。材料の力学的特性(極限引張応力及び弾性係数)の増加に示されるように、材料の生体力学的特性について、rhPDGF−BBコーティング縫合糸に対する用量依存的反応が観察された。
に8頭/群及び組織学に4頭/群でデザインされた。力学的試験日の1日でインストロン社の試験デバイスにエラーが生じたため、5つの検体が分析に含められず、統計的検出力が低下した。その結果、極限荷重、剛性等の特性が対照群と比べて平均的に増加しているように見えたにも関わらず、これらの特性のより小さな差は実験群で有意と見なされなかった。

Claims (21)
- 腱障害の処置に使用するための、1〜2mLの1用量当たり75μg〜7,500μgのPDGF及びバッファーを含む組成物であって、前記組成物が腱又は骨−腱接合部への直接注射により投与される、組成物。
- 前記腱障害が腱症、腱炎、又は腱鞘炎である、請求項1に記載の組成物。
- 前記PDGFが、PDGF−AA、PDGF−BB、PDGF−AB、PDGF−CC、及びPDGF−DDからなる群から選択される、請求項1〜2のいずれか一項に記載の組成物。
- 前記PDGFがPDGF−BBである、請求項3に記載の組成物。
- 前記PDGFが組換えヒト(rh)PDGF−BBである、請求項4に記載の組成物。
- 前記組成物が、1〜2mLの1用量当たり500〜1,000μg、5,000〜7,500μg、450〜3000μg、400〜1000μg、500〜900μg、600〜800μg、650〜750μg、又は700μgのPDGF−BBを含む、請求項4〜5のいずれか一項に記載の組成物。

- 前記組成物が単回投与として投与される、請求項1〜13のいずれか一項に記載の組成物。
- 前記組成物が単回注射によって投与される、請求項1〜14のいずれか一項に記載の組成物。
- 前記組成物が2回以上の投与で投与される、請求項1〜13のいずれか一項に記載の組成物。
- 前記組成物が、週1回の単回注射によって4週間投与される、請求項1〜13又は16のいずれか一項に記載の組成物。
- 前記組成物により、投与から7日以内にベースラインと比べて腱強度が少なくとも60%、少なくとも65%、又は少なくとも70%上昇する、請求項1〜17のいずれか一項に記載の組成物。
- 前記組成物により、腱が、投与から7日以内に、投与の21日後に測定される最終強度の少なくとも80%、少なくとも85%、又は少なくとも90%に達する、請求項1〜18のいずれか一項に記載の組成物。
- (i)1ヶ月、2ヶ月、3ヶ月、4ヶ月、5ヶ月、6ヶ月、又はそれ以上にわたって週1回、
