INSTRUMENTO DE ENGRAPADO QUIRURGICO QUE INCORPORA UNA BARRA DE DISPARO AHUSADA PARA MAYOR FLEXIBILIDAD ALREDEDOR DE LA UNION DE ARTICULACION
INTERREFERENCIA CON SOLICITUD RELACIONADA

CAMPO DE LA INVENCION
La presente invención se refiere en general a instrumentos engrapadores quirúrgicos que son capaces de aplicar líneas de grapas a tejido mientras cortan el tejido entre las líneas de grapas, y más particularmente a mejoramientos relativos a instrumentos engrapadores y mejoramientos en procedimientos para formar varios componentes de dichos instrumentos engrapadores que incluyen una flecha de articulación.
ANTECEDENTES DE LA INVENCION
Los instrumentos quirúrgicos endoscópicos frecuentemente son preferidos sobre los dispositivos quirúrgicos abiertos tradicionales puesto que una incisión más pequeña tiende a reducir el tiempo de recuperación postoperatorio y las complicaciones. Consecuentemente, un desarrollo significativo ha conducido a una gama de instrumentos quirúrgicos endoscópicos que son adecuados para la colocación precisa de un activador de extremo distante en un sitio quirúrgico deseado, por medio de una cánula de un trocar. Estos activadores de extremo distante acoplan el tejido de varias maneras para lograr un efecto diagnóstico o terapéutico (por ejemplo, cortador de extremo, sujetador, cortador, engrapadores, aplicador de clip, dispositivo de acceso, dispositivo de suministro de terapia de fármaco/gen, y dispositivo de energía que usa ultrasonido, RF, láser, etc.).
La colocación del activador de extremo es restringida por el trocar. Generalmente estos instrumentos quirúrgicos endoscópicos incluyen una flecha larga entre el activador de extremo y una porción de mango manipulada por el clínico, esta flecha larga permite la inserción hasta una profundidad deseada y rotación alrededor del eje longitudinal de la flecha, colocando con ello el activador de extremo hasta un grado. Con la colocación atinada del trocar y el uso de sujetadores, por ejemplo, por medio de otro trocar, frecuentemente este grado de colocación es suficiente. Los instrumentos quirúrgicos de engrapado y separación, como los que se describen en la patente de E.U.A. No. 5,465,895, son un ejemplo de un instrumento quirúrgico endoscópico que coloca exitosamente un activador de extremo por inserción y rotación. Más recientemente, la solicitud de E.U.A. No. Serie 10/443617, "Surgical Stapling Instrument Incorporating an E-beam Firing Mechanism", de Shelton y otros, presentada el 20 de mayo de 2003, describe una barra de disparo en forma de "barra en E" para separar tejido y accionar grapas. Algunas de las ventajas adicionales incluyen espaciar afirmativamente las mordazas del activador de extremo, incluso si muy poco, poco o demasiado tejido es sujetado para una óptima formación de grapa. Además, la barra de disparo en forma de E incluye acoplamiento del activador de extremo y el cartucho de grapas de una manera que permite incorporar varios cierres benéficos. Dependiendo de la naturaleza de la operación, puede ser o^seable-a|ustar--adicionalmente la colocación del activador de extremo de un instrumento quirúrgico endoscopico en lugar de limitarse a inserción y rotación. En particular, frecuentemente es deseable orientar el activador de extremo en un eje transversal al eje longitudinal de la flecha del instrumento. El movimiento transversal del activador de extremo con respecto a la flecha del instrumento es referido convenientemente como "articulación". Esta colocación articulada le permite al clínico acoplar más fácilmente el tejido en algunos casos. Además, la colocación articulada permite ventajosamente colocar un endoscopio detrás del activador de extremo sin ser bloqueado por la flecha del instrumento. Aunque los instrumentos no articulados de engrapado y separación anteriormente mencionados tienen gran utilidad y se pueden emplear exitosamente en muchos procedimientos quirúrgicos, es deseable mejorar su operación con la capacidad de articular el activador de extremo, dando con ello mayor flexibilidad clínica en su uso. Los enfoques para articular engrapado y separación tienden a ser complicados, integrando el control de la articulación junto con el control de cierre del activador de extremo para sujetar tejido y disparar el activador de extremo (es decir, engrapar y separar), dentro de las restricciones de diámetro pequeño de un instrumento endoscopico. Generalmente, los tres movimientos de control son transferidos a través de la flecha como traslaciones longitudinales. Por ejemplo, la patente de E.U.A. No. 5,673,840 describe un mecanismo de articutedóji similar a un acordeón ("cuello flexible") que es rttealatlo-tirafide^ete de dos varillas de conexión por medio de la flecha del instrumento, cada varilla está descentrada respectivamente sobre lados opuestos de la línea de centro de la flecha. Otro ejemplo de control longitudinal de un mecanismo de articulación es la patente de E.U.A. No. 5,865,361 , que incluye un eslabón de articulación descentrado de un pivote levador de tal manera que la traslación longitudinal de impulso o tracción del eslabón de articulación afecta la articulación en un lado respectivo. Similarmente, la patente de E.U.A. No. 5,797,537 describe una varilla similar que pasa a través de la flecha para efectuar la articulación. Aunque estos mecanismos de articulación controlados longitudinalmente han provisto ciertas ventajas a los instrumentos quirúrgicos tales como los de engrapado y separación endoscópica, se considera que un movimiento de articulación alternativo proveería flexibilidad adicional de diseño. En particular, se describen enfoques ventajosos en las cuatro solicitudes copendientes arriba interrelacionadas, en donde un movimiento rotacional con respecto a un eje longitudinal de la flecha transfiere un movimiento de articulación a un mecanismo de articulación, acoplando el activador de extremo a la flecha. Lo que sería deseable también es retener las ventajas de una barra de disparo de tipo barra en E en un instrumento quirúrgico de engrapado y separación en combinación con un mecanismo de articulación controlado F0tacaQnalmente.-jCo se^^^ necesidad significativa de un instrumento tal que incorpore una barra de disparo que separe ventajosamente el tejido sujetado, acople las mordazas del activador de extremo para un engrapado controlado afirmativamente y todavía sea acoplado para movimiento de disparo por medio de un mecanismo de articulación.
BREVE DESCRIPCION DE LA INVENCION
La invención supera las deficiencias anteriormente mencionadas y otras más de la técnica anterior suministrando una barra de disparo que es efectiva para accionar longitudinalmente un activador de extremo, y también tiene una porción contigua ahusada que es efectiva para doblarse por medio de una flecha de articulación. Con ello son realizadas las ventajas clínicas de un instrumento quirúrgico de articulación, sin degradar la operación consistente del activador de extremo. En un aspecto de la invención, un instrumento quirúrgico tiene una porción de mango que produce un movimiento de articulación y un movimiento de disparo que son transferidos por medio de una flecha que tiene un eje longitudinal. Un mecanismo de articulación acopla la flecha con un activador de extremo y es sensible al movimiento de articulación para girar el activador de extremo desde el eje longitudinal de la flecha. Un mecanismo de disparo responde al movimiento de disparo y está acoplado para movimiento por medio del mecanismo de articulación y el activador de extremo. En arttct^af ^l^meafflSffio-d lls arQ tiene una porción accionadora que tiene un primer grosor y está colocada en el activador de extremo, y una porción de articulación unida contiguamente a la porción accionadora y teniendo un segundo grosor menor que el primer grosor para movimiento de articulación por medio del mecanismo de articulación. Por medio de lo cual el mecanismo de disparo es efectivo tanto para accionar el activador de extremo como para articular por medio del mecanismo de articulación. En otro aspecto de la invención, un instrumento quirúrgico tiene una porción de mango que produce un movimiento de disparo, un movimiento de cierre y un movimiento de articulación, con todo el juego transferido separadamente a una flecha. Un activador de extremo es movido pivotalmente por un mecanismo de articulación sensible al movimiento de articulación. El activador de extremo incluye un canal alargado acoplado con la flecha e incluye una ranura de canal. Un yunque está acoplado pivotalmente con el canal alargado y es sensible al movimiento de cierre de la flecha y tiene un canal de yunque. Un dispositivo de disparo tiene un borde de corte presentado distantemente recibido longitudinalmente entre el canal alargado y el yunque, e incluye una porción de tira adelgazada que cambia por medio del mecanismo de articulación. Una barra de disparo responde al movimiento de disparo longitudinal de la porción de mango. La barra de disparo incluye una tira alargada colocada longitudinalmente para movimiento por medio del mecanismo de articulación, y tiene una cabeza de barra de disparo conectada distantemente a la tira alargada y colocada para movimiento longitudinal en el activador de extremo. Estos y otros objetos y ventajas de la presente invención serán evidentes a partir de los dibujos anexos y la descripción de los mismos.
BREVE DESCRIPCION DE LAS FIGURAS

DESCRIPCION DETALLADA DE LA INVENCION
Volviendo a los dibujos, en donde números similares denotan componentes similares en todas las vistas, las figuras 1-3 representan un instrumento quirúrgico, que en la modalidad ilustrativa es más particularmente un instrumento quirúrgico de engrapado y separación, 10, que es capaz de practicar los beneficios únicos de la presente invención. En particular, el instrumento quirúrgico de engrapado y separación, 10, tiene el tamaño adecuado para su inserción, en un estado no articulado como se representa en la figura 1 , a través del pasaje de una cánula de trocar, en un sitio quirúrgico en un paciente para realizar un procedimiento quirúrgico. Una vez que un mecanismo de articulación 1 1 y un activador de extremo 12 unido distantemente_sorijns_ertados a Jrav ss telj>asaje de la cánula, el mecanismo e-¾ ti€ü aGi(k 4^puad ser^artÍculacio remotamente, como se representa en la figura 2, por medio de un control de articulación, 13. Con ello, el activador de extremo, 12, puede llegar detrás de un órgano o puede aproximarse a un tejido desde un ángulo deseado, o por otras razones. Por ejemplo, un mecanismo de disparo, representado ventajosamente como una barra de disparo 14 de tipo de barra en E (representada en la figura 3), que separa tejido sujetado, acopla un canal alargado 16 y un yunque unido pivotalmente, 18. El instrumento quirúrgico de engrapado y separación, 10, incluye una porción de mango, 20, conectada a una porción 22 de instrumento, esta última comprendiendo además una flecha 23 que termina distantemente en el mecanismo de articulación, 11 , y el activador de extremo, 12. La porción de mango 20 incluye un agarro de pistola, 24, hacia la cual el clínico tira pivotalmente un gatillo de cierre, 26, para ocasionar sujeción o cierre del yunque 18 hacia el canal alargado 16 del activador de extremo 12. Un gatillo de disparo, 28, está mas alejado fuera del gatillo de cierre 26, y es tirado pivotalmente por el clínico para ocasionar engrapado y separación del tejido sujetado en el activador de extremo 12. Posteriormente, un botón de liberación 30 es oprimido para liberar el tejido sujetado. Un manguito de cierre más externo, 32, de la flecha 23, se traslada longitudinalmente en respuesta al gatillo de cierre 26 para cerrar pivotalmente el yunque 18. Específicamente, una porción distante, o anillo de
t T^es-^GStef^a^^r^reGtameQíe^-poJ un .marco 34 de la porción 22 de instrumento (visible parcialmente en el mecanismo de articulación 1 1 ). En el mecanismo de articulación, 1 1 , una porción próxima, o tubo de cierre 35, del manguito de cierre 32 se comunica con la porción distante (anillo de cierre) 33. El marco 34 está unido flexiblemente al canal alargado 16 por medio del mecanismo de articulación 1 , habilitando la articulación en un solo plano. El marco 34 también sostiene de manera deslizable longitudinalmente un miembro impulsor de disparo, 36, que comunica un movimiento de disparo del gatillo de disparo 28 a la barra de disparo 14. En la figura 3 se representa solo la barra de disparo 14 del miembro impulsor de disparo 36, pero el miembro impulsor de disparo 36 se describe abajo en mayor detalle con respecto a varias versiones de un mecanismo de articulación, 1 1 , controlado rotacionalmente. Se apreciará que los términos "contiguo" y "distante" se usan aquí con referencia a un clínico que agarra el mango de un instrumento. De esta manera, el activador de extremo 12 es distante con respecto a la porción de mango más próxima, 20. Se apreciará también que por conveniencia y claridad, los términos espaciales como "vertical" y "horizontal" se usan aquí con respecto a los dibujos. Sin embargo, los instrumentos quirúrgicos se usan en muchas orientaciones y posiciones, y estos términos no se consideran limitativos ni absolutos.

En la figura 3, la barra de disparo 14 está colocada contiguamente, permitiendo que un cartucho de grapas sin usar, 37, sea instalado en el canal alargado 16. En particular, un perno superior 38 de la barra de disparo 14 reside dentro de un hueco representado como una cavidad de yunque, 40, permitiendo que el yunque 18 sea abierto y cerrado repetidamente. Con el activador de extremo cerrado como se representa en la figura 4, la barra de disparo 14 se hace avanzar en acoplamiento con el yunque 18, el perno superior 38 teniendo que entrar en la ranura longitudinal del yunque, 42. Un perno más inferior, o casquete de barra de disparo, 44, se acopla con una superficie inferior del canal alargado 16, la barra de disparo 14 teniendo que extenderse a través de una ranura de canal, 45. Un perno medio 46 se acopla deslizablemente con una superficie superior del canal alargado 16, cooperando con el casquete de barra de disparo, 44. Con ello, la barra de disparo 14 separa afirmativamente el activador de extremo 12 durante el disparo, superando la opresión que puede ocurrir con una cantidad mínima de tejido sujetado y superando la formación fallida de grapa con una cantidad excesiva de tejido sujetado. Durante el disparo, un borde de corte presentado distantemente, 48, entre el perno superior 38 y el perno medio 46 de la barra de disparo, ejitra_a^n^ ajiujra^y^j ical^p resentada contiguamente, 49, del cartucho de grapas. 37. separando el tejido sujetado entre el cartucho de grapas 37 y el yunque 18. Como se muestra en la figura 4, el perno medio 46 acciona el cartucho de grapas 37 entrando en una ranura de disparo dentro del cartucho de grapas 37, impulsando un trineo de cuña, 41 , a contacto levador ascendente con impulsores de grapa, 43, que a su vez impulsan una pluralidad de grapas 47 fuera de aberturas de grapas 51 en el cartucho de grapas 37, para hacer contacto con cavidades de grapa, 53, sobre una superficie interna del yunque 18. La figura 5 representa la barra de disparo 14 trasladada distantemente por completo después de completar la separación y engrapado de tejido.
Mango de dos ejes Con referencia a las figuras 6-7, la porción de mango 20 está comprendida de primera y segunda sección de base, 50 y 52, que están moldeadas de un material polimérico tal como policarbonato relleno con vidrio. La primera sección de base 50 está provista con una pluralidad de pernos de forma cilindrica, 54. La segunda sección de base 52 incluye una pluralidad de miembros de extensión, 56, cada uno con una abertura de forma hexagonal, 58. Los pernos cilindricos 54 son recibidos dentro de las aberturas hexagonales 58 y son mantenidos ahí friccionalmente para mantener la primera y segunda sección de base 50 y 52 en ensamble. Un casquete de alojamiento, 60, tiene un calibre 62 que se extiende completamente a través del mismo para acoplar y girar la porción de instrumento. 22, alrededor de su eje longitudinal. El casquete de alojamiento 60 incluye una protuberancia que sobresale internamente, 64, que se extiende a lo largo de por lo menos una porción del calibre 62. La protuberancia 64 es recibida dentro de una ranura longitudinal 66 formada en una porción próxima del manguito de cierre, 32, de tal manera que la rotación del casquete de alojamiento 60 efectúa rotación del manguito de cierre 32. Será apreciado que la protuberancia 64 se extiende además a través del marco 34 y hace contacto con una porción del miembro impulsor de disparo 36 para efectuar también su rotación. De esta manera, el activador de extremo 12 (no mostrado en las figuras 3-4) gira con el casquete de alojamiento 60. Un extremo contiguo 68 del marco 34 pasa contiguamente a través del casquete de alojamiento 60 y está provisto con una muesca circunferencial 70 que es acoplada por miembros de aseguramiento de canal opuestos, 72, que se extienden respectivamente desde las secciones de base 50 y 52. Se muestra solamente el miembro de aseguramiento de canal 72 de la segunda sección de base. Los miembros de aseguramiento de canal 72 que se extienden desde las secciones de base 50, 52, sirven para asegurar el marco 34 a la porción de mango 20, de tal manera que el marco 34 no se mueve longitudinalmente con respecto a la porción de mango 20. El gatillo de cierre, 26, tiene una sección de mango, 74, una sección de segmento de engranaje, 76, y una sección intermedia, 78. Un calibre 80 se extiende a través de la sección intermedia 78. Un miembro de soporte cilindrico, 82, que se extiende desde la segunda sección de base 52 pasa a través del calibre 80 para montar pivotalmente el gatillo de cierre 26 sobre la porción de mango 20. Un segundo miembro de soporte cilindrico, 83, que se extiende desde la segunda sección de base 52, pasa a través de un calibre 81 del gatillo de disparo 28 para montarse pivotalmente sobre la porción de mango 20. Una abertura hexagonal 84 está provista en el miembro de soporte cilindrico 83 para recibir un perno de aseguramiento (no mostrado) que se extiende desde la primera sección de base 50. Un yunque de cierre 86 está alojado dentro de la porción de mango 20 para movimiento alternativo en el mismo y sirve para transferir movimiento del gatillo de cierre 26 al manguito de cierre 32. Miembros de soporte 88 que se extienden desde la segunda sección de base 52 y un miembro de aseguramiento 72, que se extiende a través de un hueco 89 en el yunque 86, sostienen el yunque 86 dentro de la porción de mango 20. Un extremo contiguo 90 del manguito de cierre 32 está provisto con una pestaña 92 que entra a presión en un hueco receptor 94 formado en un extremo distante 96 del yunque 86. Un extremo contiguo 98 del yunque 86 tiene una cremallera 100 que se acopla con la sección de segmento de engranaje, 76, del gatillo de cierre 26. Cuando el disparador de cierre 26 es movido hacia el agarro de pistola 24 de la porción de mango 20, el yunque 86, y por tanto el manguito de cierre 32, se mueven distantemente, comprimiendo un resorte 102 que desvía el yunque 86 contiguamente. El movimiento distante del manguito de cierre 32 efectúa movimiento de traslación pivotal del yunque 18 distantemente y hacia el canal alargado 16 del activador de extremo 12, y el movimiento contiguo efectúa cierre, como se expone mas abajo. El gatillo de cierre 26 es desviado hacia delante a una posición abierta por una superficie frontal 130 que interacciona con una superficie de acoplamiento 128 del gatillo de disparo 28. Un primer gancho sujetador, 104, que se mueve pivotalmente de arriba hacia atrás en la porción de mango 20 alrededor de un perno 106, restringe el movimiento del gatillo de disparo 28 hacia el agarro de pistola 24 hasta que el gatillo de cierre 26 es sujetado a su posición cerrada. El gancho 104 restringe el movimiento del gatillo de disparo 28 acoplando un perno de cierre 107 en el gatillo de disparo 28. El gancho 104 también está en contacto con el gatillo de cierre 26. Conforme el yunque 86 se mueve distantemente en respuesta al movimiento contiguo del gatillo de cierre 26, un brazo superior de traba, 1 18, del botón de liberación 30, se mueve a lo largo de una superficie superior 120 en el yunque 86, hasta caer en un hueco 122 presentado hacia arriba en una porción inferior próxima del yunque 86. El resorte de liberación 112 impulsa el botón de liberación 30 hacia fuera, lo que mueve pivotalmente el brazo superior de traba 118 descendentemente hacia acoplamiento con el hueco 122 presentado hacia arriba, trabando así el gatillo de cierre 26 en una posición de sujeción de tejido. El brazo de traba 118 puede ser movido fuera del hueco 122 para liberar el yunque 18 empujando el botón de liberación 30 internamente. Específicamente, el brazo superior de traba 118 se mueve pivotalmente hacia arriba alrededor del perno 123 de la segunda sección de base 52. Entonces el yunque 86 se puede mover contiguamente en respuesta al movimiento de retorno del gatillo de cierre 26. Un resorte de retorno de gatillo de disparo, 124, está localizado dentro de la porción de mango 20 con un extremo unido al perno 106 de la segunda sección de base 52, y el otro extremo unido a un perno 126 sobre el gatillo de disparo 28. Cuando está en su posición de disparo, el gatillo de disparo 28 está en un ángulo de aproximadamente 45° con respecto al agarro de pistola 24. Después de disparar la grapa, el resorte 124 ocasiona que el gatillo de disparo 28 retorne a su posición inicial. La sección alternativa 134 comprende un trineo de cuña, o trineo de cuña, en la porción de instrumento 22 (no mostrada en las figuras 6-7), y tiene una varilla impulsora metálica 140. El mecanismo impulsor 138 incluye primera y segunda cremalleras, 141 y 142. Una primera muesca 144 está provista en el miembro impulsor 138 intermedio a la primera y segunda cremallera, 141 , 142. Durante el movimiento de retorno del gatillo de disparo 28, un diente 146 sobre el gatillo de disparo 28 se engrana con la primera muesca 144 para retornar el miembro impulsor 138 a su posición inicial después de disparar la grapa. Una segunda muesca 148 está localizada en un extremo contiguo de la varilla impulsora metálica 140 para trabar la varilla impulsora metálica 140 con el bxazQ_süperior de traba 118 del botón de liberación 30 en su posición no disparada. El multiplicador 136 comprende primer y segundo engranaje con piñón integrales, 150 y 152. El primer engranaje con piñón integral 150 está acoplado con una primera cremallera 154 provista sobre la varilla impulsora metálica 140. El segundo engranaje con piñón integral 152 está acoplado con la primera cremallera 141 sobre el miembro impulsor 138. El primer engranaje con piñón integral 150 tiene un primer diámetro, y el segundo engranaje con piñón integral 152 tiene un segundo diámetro que es más pequeño que el primer diámetro.
Control de articulación rotacional Haciendo referencia a las figuras 6-9, la porción de mango 20 incorpora ventajosamente el control de articulación 13 que hace girar la porción de instrumento 22 alrededor del eje longitudinal del instrumento quirúrgico 10, y articula el activador de extremo 12 en un ángulo con el eje longitudinal. Un tubo hueco de impulso de articulación, 200, está localizado concéntricamente dentro del manguito de cierre 32, y está acoplado operativamente con una palanca de accionamiento, 202, de tal manera que la rotación de la palanca de accionamiento 202 gira el tubo 200 alrededor del eje longitudinal y ocasiona rotación perpendicular o articulación del anillo de cierre 250 y el activador de extremo 12. Esta articulación del anillo de cierre 250 8rrespojide_con el grado y dirección de rotación de la palanca de accionamiento 202 observada y manipulada por el clínico. En la versión ilustrativa, la relación es de uno a uno, con el grado de rotación de la palanca de accionamiento 202 correspondiendo con el grado de articulación del eje longitudinal de la flecha 23, dando así una indicación intuitiva para el clínico. Se apreciará que se pueden seleccionar otras relaciones angulares. El control de articulación 13 incluye un par de alojamientos de transmisión de articulación reflejados, 204, que están unidos al casquete de alojamiento, 60. Además, el alojamiento de transmisión de articulación, 204, incluye apéndices externos alineados longitudinalmente, 206, que un clínico tuerce para efectuar la rotación del alojamiento de transmisión de articulación 204, y por lo tanto del activador de extremo 2, alrededor del eje longitudinal de la porción de instrumento 22. La ventana 222 provee un claro para una protuberancia 224 presentada internamente desde el yunque de articulación 220, que pasa a través de la ventana rectangular 222 para acoplar una ranura 226 en el tubo de impulso de articulación 200, colocando longitudinalmente el tubo de impulso de articulación 200 para movimiento rotacional. El tubo hueco de impulso de articulación 200 se extiende longitudinalmente dentro del manguito de cierre 32 desde el mecanismo de articulación , y termina distantemente antes de los apéndices de trabado 227 del manguito de cierre 32. Los apéndices 227 están doblados internamente detrás de la cara próxima del tubo de impulso de articulación 200, y con ello retienen el tubo de impulso de articulación 200 en la flecha 23. El mecanismo de articulación 240 tiene un tubo hueco de impulso de articulación 242 que está localizado concéntricamente dentro del manguito de cierre 32, y tiene una sección de engranaje que se proyecta distantemente, 244, alrededor de una primera porción de circunferencia, 246. La sección de engranaje 244 engrana con un engrane recto 248 unido a un anillo de cierre 250 y que se proyecta contiguamente desde el mismo, dicho anillo de cierre 250 se mueve pivotalmente alrededor de pernos 253 que se extienden a través de primer y segundo punto de giro, 252, 260, proyectándose distantemente desde el manguito de cierre, 32. La sección de engranaje 256 está acoplada operativamente con un segundo engrane recto 258 unido a, y que se proyecta contiguamente desde, un lado lateral opuesto del anillo de cierre, 250, por un engrane de retroceso, 262, sostenido pivotalmente por el marco 34. El engrane de retroceso 262 engrana tanto con la sección de engrane ahuecada que se proyecta distantemente, 256, sobre un lado, como con el segundo engrane recto 258 del anillo de cierre, sobre el otro lado. Cuando se acciona el gatillo de cierre 26, tanto el tubo hueco de impulso de articulación 242 como el tubo de cierre 250 unido pivotalmente del manguito de cierre 32, son movidos distantemente para cerrar el yunque 18. El tubo de cierre 35 del manguito de cierre 32 está separado lejos del anillo de cierre por puntos de giro 252, 260, clavados en agujeros de pivote 264 y 266, centrados en engranes rectos 248, 258, y una abertura de marco 268 que se extiende a través del mismo. La abertura de marco 268 provee un claro de tal manera que los bordes contiguos del anillo de cierre 33 y los bordes distantes del tubo de cierre 35 del manguito de cierre, no chocan durante la articulación. La figura 11 representa en forma desarmada una porción de instrumento 270 que incluye el mecanismo de articulación de engrane recto 240. Un marco 272 es unible longitudinalmente a la porción de mango (representada en las figuras 1 y 2) con un buje 274 en su extremo contiguo para acoplamiento rotativo con el mismo. Una depresión de marco, 276, íormada_p_or una_abejrtura ^2 8_almeada longitudinalmente con el centro del marr.n 279, es mas larga que un conector de disparo 280 que se desliza longitudinalmente dentro de la depresión de marco 276. El extremo contiguo del conector de disparo 280 acopla rotativamente el extremo distante de la barra de impulso metálica 140 (representada en la figura 6). El extremo distante del conector de disparo 280 incluye una ranura 282 que recibe un extremo contiguo de la barra de disparo 14, unida ahí por medio de pernos 284. Una porción más distante de la barra de disparo 14 está colocada dentro de una ranura inferior 286 en una guía ranurada de barra de disparo, 288, que está acoplada distantemente con un miembro de marco articulado 290 y el marco 272. El miembro de marco articulado, 290, tiene un miembro de anclaje de canal 292 que une distantemente un collar de unión 294 de una porción próxima en el canal alargado 16. La barra de disparo 14 pasa a través de una ranura inferior 295 en el miembro de marco articulado 290. El miembro de marco articulado 290 es separado en alejamiento del extremo distante del marco 272 por la guía ranurada de barra de disparo, 288, y unido flexiblemente a la misma para articulación con un conector elástico 296. Un extremo contiguo ensanchado 298 del conector elástico 296 acopla un hueco superior de comunicación distante, 300, en el extremo distante del marco 272, y un extremo distante ensanchado 302 del conector elástico 296 acopla un hueco superior de comunicación próxima, 304, en el miembro de marco articulado 290. Con ello, el canal alargado 16 queda unido a la porción de mango 20, aunque con una porción flexible entre ambas.
El canal alargado 16 también tiene una ranura de leva de yunque, 306, que recibe pivotalmente un pivote de yunque, 308, del yunque 18. El anillo de cierre 250, que abarca el miembro de marco articulado 290, incluye un apéndice presentado distantemente, 310, que acopla un aspecto de yunque 312 contiguo pero distante al pivote de yunque 308 sobre el yunque 18, para así efectuar apertura. Cuando el anillo de cierre 250 es movido hacia delante, su cara de cierre presentada distantemente, 314, hace contacto con una cara de cierre cilindrica inclinada, 316, que es distante al apéndice 312 del yunque 18. Esta acción de leva cierra el yunque 18 hacia abajo hasta que la cara de cierre 314 del anillo de cierre 250 hace contacto con una cara cilindrica plana 318 del yunque 18.
Barra ahusada de disparo La figura 12 representa el mecanismo de articulación 240 a lo largo del eje de giro de articulación, que ilustra estructuras de soporte flexibles entre la flecha 23 y el activador de extremo 12, y una construcción de la barra de disparo 14 que realiza ventajosamente separación, aunque es lo suficientemente flexible para articulación. El tubo hueco de impulso de articulación 242 acopla el engrane recto 248 del anillo de cierre (siendo representado solo el engrane recto 248 del anillo de cierre 33). De esta vista se omite la porción contigua, o tubo de cierre 35, del manguito de cierre 32 que coloca longitudinalmente para articulación el engrane recto 248. El soporte elástico en el mecanismo de articulación 240 permite la articulación alrededor del eje de giro de articulación, incluye un par de placas de soporte, 400, 402, que proveen guías laterales a una porción contigua de la barra de disparo, representada como una tira de barra de disparo ahusada alargada, 404, alineada para flexión alrededor del eje de giro de la articulación. Esta tira de barra de disparo ahusada, 404, cambia a una porción distante más gruesa, representada como una cabeza de la barra de disparo, 406, que incluye el borde de corte 48, perno superior 38, perno medio 46 y casquete de barra disparo 44. La porción más delgada de la tira de barra de disparo 404 es más fácil de doblar que la porción más gruesa 406, reduciendo con ello la fuerza para articular el activador de extremo. Esta cabeza de barra de disparo más gruesa, 406, tiene un grosor más grande para resistir la desviación de cargas de sujeción de tejido durante el disparo, asegurando así una separación efectiva y accionamiento del cartucho de grapas, 37.
Operación Un activador de extremo cerrado 12 y una flecha 23 de una porción de instrumento 22 de un instrumento quirúrgico de engrapado y separación, 10, se insertan a través del pasaje de una cánula de un trocar a un sitio quirúrgico para un procedimiento endoscópico o laparoscópico. El control de articulación 13 se hace girar a voluntad alrededor del eje longitudinal de la flecha 23 para efectuar una rotación correspondiente del activador de extremo 12. Convenientemente, la palanca de accionador 202 del control de articulación 13 es movido pivotalmente para crear un movimiento de articulación de rotación en un tubo de impulso de articulación 200, 242, que es convertido a un movimiento de articulación en una conexión engranada en un mecanismo de articulación 1 1 , 240, colocando con ello el activador de extremo 12 en una posición deseada. La barra de disparo 14 se hace avanzar a través de la porción de instrumento 22 para accionar el activador de extremo 12, asistido ventajosamente por medio del mecanismo de articulación 11 , 240, incluyendo la tira de barra de disparo ahusada alargada, 404, que se dobla con resistencia reducida. Aunque la presente invención se ha ¡lustrado mediante la descripción de varias modalidades, y aunque las modalidades ilustrativas han sido descritas en detalle considerable, no es la intención del solicitante restringir o limitar en modo alguno el alcance de las reivindicaciones anexas. Ventajas y modificaciones adicionales pueden surgir fácilmente para el experto en la materia. La presente invención ha sido expuesta en términos de procedimientos y aparatos endoscópicos. Sin embargo, se debe considerar que el uso de términos tales como "endoscópico", no limita la presente invención a un instrumento quirúrgico de engrapado y separación para usar solo en conjunto con un tubo endoscópico (es decir, trocar). Por el contrario, se considera que la presente invención puede tener uso en cualquier procedimiento en donde el acceso esté limitado a una incisión pequeña, incluyendo sin limitación procedimientos laparoscópicos, así como también prncadimientos abiertos. Para otro ejemplo, aunque la barra de disparo en forma de barra en E, 14, tiene ventajas para un instrumento quirúrgico de engrapado y separación 10 empleado quirúrgicamente, se puede usar una barra en E similar en otros procedimientos clínicos. Generalmente es aceptado que los procedimientos endoscópicos son más comunes que los procedimientos laparoscópicos. Por consiguiente, la presente invención se ha expuesto en términos de procedimientos y aparatos endoscópicos . Sin embargo, se debe considerar que el uso aquí de términos tales como "endoscópico" no limita la presente invención a un instrumento quirúrgico de engrapado y separación para usar solo en conjunto con un tubo endoscópico (es decir, trocar). Por el contrario, se considera que la presente invención puede tener uso en cualquier procedimiento en donde el acceso esté limitado a una incisión pequeña, incluyendo sin limitación procedimientos laparoscópicos, así como también procedimientos abiertos. Para otro ejemplo, aunque una porción de mango 20 ilustrativa descrita en la presente es operada manualmente por un clínico, es consistente con aspectos de la invención que algunas o todas las funciones de una porción de mango sean propulsadas (por ejemplo, neumáticas, hidráulicas, electromecánicas, ultrasónicas, etc.). Además, los controles de cada una de estas funciones pueden ser presentados manualmente en una porción de mango, o pueden ser controladas remotamente (por ejemplo, control remoto inalámbrico, consola automática remota, etc.).
Gomo jo tro ejemplo adicional, aunque se ¡lustra ventajosamente en la presente un instrumento de engrapado y separación simultánea, sería consistente con aspectos de la invención una articulación controlada rotacionalmente con otros tipos de activadores de extremo, tales como sujetador, cortador, aplicador de clip, dispositivo de acceso, dispositivo de suministro de terapia de fármaco/gen, y un dispositivo de energía que utiliza ultrasonido, RF, láser, etc. Por ejemplo, aunque se ilustra en la presente un mecanismo de articulación de engrane recto, 240, se pueden incluir otros mecanismos de articulación, como los que se describen en las solicitudes copendientes anteriormente mencionadas. Como un ejemplo adicional, para mecanismos de articulación que obstruyen el eje longitudinal de la flecha, las placas de soporte y la tira de barra de disparo ahusada pueden estar desalineadas del eje longitudinal.
Claims (7)
1.- Un instrumento quirúrgico que comprende: una porción de mango configurada operativamente para producir un movimiento de articulación y un movimiento de disparo; una flecha que tiene un eje longitudinal unido a la porción de mango para transferir el movimiento de articulación y el movimiento de disparo; un activador de extremo; un mecanismo de articulación que acopla la flecha con el activador de extremo y es sensible al movimiento de articulación para girar el activador de extremo desde el eje longitudinal de la flecha; y un mecanismo de disparo sensible al movimiento de disparo y acoplado para movimiento por medio del mecanismo de articulación y activador de extremo, el mecanismo de disparo comprendiendo: una porción de accionamiento que tiene un primer grosor y colocada en el activador de extremo, y una porción de articulación unida contiguamente a la porción de accionamiento y teniendo un segundo grosor menor que el primer grosor para movimiento de articulación por medio del mecanismo de articulación.
2.- El instrumento quirúrgico de conformidad con la reivindicación 1 , caracterizado además porque el activador de extremo comprende un par de mordazas pivotalmente opuestas para sujetar tejido, la porción de accionamiento del mecanismo de disparo incluye un borde de corte presentado distantemente para separar el tejido sujetado.
3.- El instrumento quirúrgico de conformidad con la reivindicación 2, caracterizado además porque el activador de extremo también comprende: un canal alargado acoplado con la porción de mango e incluyendo una ranura de canal; un cartucho de grapas recibido por el canal alargado e incorporando un miembro de cuña colocado contiguamente, alineado para elevar un impulsor que sostiene una grapa; un yunque acoplado pivotalmente al canal alargado e incluyendo un canal de yunque; y la porción de articulación del mecanismo de disparo incluyendo el borde de corte presentado distantemente, recibido longitudinalmente entre el canal alargado y el yunque, un miembro superior acoplable al canal de yunque, un miembro inferior que se acopla con la ranura de canal, y un miembro medio operable para accionar el cartucho de grapas trasladando distantemente el miembro de cuña del cartucho de grapas.
4.- El instrumento quirúrgico de conformidad con la reivindicación 3, caracterizado además porque la flecha también comprende un tubo de impulso de articulación sensible al movimiento rotacional de la porción de mango y terminando distantemente en una sección de engranaje, el mecanismo de articulación comprendiendo un engrane recto unido contiguamente al activador de extremo y acoplado por la sección de engranaje.
5.- El instrumento quirúrgico de conformidad con la reivindicación 1, caracterizado además porque la flecha también comprende un tubo de mango y terminando distantemente en una sección de engranaje, el mecanismo de articulación comprendiendo un engrane recto unido contiguamente al activador de extremo y acoplado por la sección de engranaje.
6.- Un instrumento quirúrgico que comprende: una porción de mango operable para producir un movimiento de disparo, un movimiento de cierre y un movimiento de articulación; una flecha acoplada con la porción de mango, operable para transferir separadamente el movimiento de disparo, el movimiento de cierre y el movimiento de articulación; un canal alargado acoplado con la flecha e incluyendo una ranura de canal, un yunque acoplado pivotalmente con el canal alargado sensible al movimiento de cierre de la flecha e incluyendo un canal de yunque; y un dispositivo de disparo que incluye un borde de corte presentado distantemente, recibido longitudinalmente entre el canal alargado y el yunque; y un mecanismo de articulación que mueve pivotalmente el canal alargado desde la flecha en respuesta al movimiento de articulación; en donde el dispositivo de disparo incluye una porción de tira adelgazada que cambia por medio del mecanismo de articulación.
7.- El instrumento quirúrgico de conformidad con la reivindicación 6, caracterizado además porque el dispositivo de disparo está configurado para separar afirmativamente el yunque del canal alargado durante el trayecto ^pjigitudjnaie^t^e^j^unque y el canal alargado. 8 ^EJinstJ imento g ilrúrqico de conformidad con la reivindicación 6, caracterizado además porque comprende un cartucho de grapas acoplado por el canal alargado e incluyendo una ranura contiguamente abierta para recibir el borde de corte del dispositivo de disparo, el cartucho de grapas incluyendo una pluralidad de grapas elevadas por el movimiento longitudinal distante del dispositivo de disparo. 9. - El instrumento quirúrgico de conformidad con la reivindicación 8, caracterizado además porque el cartucho de grapas incluye también una pluralidad de impulsores que sostienen la pluralidad de grapas y un trineo de cuña sensible al movimiento longitudinal distante del mecanismo de disparo, para elevar los impulsores y así formar la pluralidad de grapas contra el yunque. 10. - El instrumento quirúrgico de conformidad con la reivindicación 8, caracterizado además porque el yunque forma una relación desviada internamente con el canal alargado, configurada para ayudar al dispositivo de disparo en la separación afirmativa entre el yunque y el canal alargado durante el accionamiento del cartucho de grapas. 11. - Un instrumento quirúrgico que comprende: una porción de mango configurada operativamente para producir un movimiento de articulación rotacional y un movimiento de disparo longitudinal; una flecha configurada operativamente para transferir separadamente el movimiento de articulación rotacional y el movimiento de disparo longitudinal; un activador de extremo ^cc^jao^^s antemente a los medios de flecha; un mecanismo de activador de extremo; y una barra de disparo sensible al movimiento de disparo longitudinal de la porción de mango, la barra de disparo comprendiendo: una tira alargada colocada longitudinalmente para movimiento por medio del mecanismo de articulación, y una cabeza de barra de disparo conectada distantemente a la tira alargada y colocada para movimiento longitudinal en el activador de extremo. 12. - El instrumento quirúrgico de conformidad con la reivindicación 11 , caracterizado además porque la porción de mango comprende unos medios de mango para producir un movimiento de articulación rotacional y un movimiento de disparo longitudinal, y la flecha comprende unos medios de flecha para transferir separadamente el movimiento de articulación rotacional y el movimiento de disparo longitudinal. 13. - El instrumento quirúrgico de conformidad con la reivindicación 12, caracterizado además porque los medios de mango comprenden también unos medios para producir un movimiento de cierre longitudinal, y los medios de flecha comprenden también unos medios para transferir separadamente el movimiento de cierre longitudinal.
