본 발명은 몇몇 태양에서 인터류킨-15(IL-15)의 전부 또는 기능성 부분을 발현하는 천연 살해(NK) 세포, 및 상기와 같은 세포의 생성 방법을 제공한다. 본 발명은 또한 피실험자에서 암을 치료하거나 또는 NK 세포의 증대 및/또는 생존을 증진시키기 위해 인터류킨-15(IL-15)의 전부 또는 기능성 부분을 발현하는 천연 살해(NK) 세포를 사용하는 방법을 제공한다.

본 출원은 2014년 5월 15일자로 출원된 미국 가 출원 제 61/993,494 호의 이점을 청구한다. 상기 출원의 전체 교시는 본 발명에 참고로 인용된다.
생체내에서 NK 세포의 생존 및 증식은 사이토킨, 예를 들어 IL-2 및 IL-15에 의한 자극을 필요로 한다. 예를 들어, 면역결핍 마우스에서 주사 후, 활성화된 NK 세포는 1주일 후에 검출할 수 없게 되지만 인간 IL-2가 또한 투여된 경우에는 한 달까지 지속되었다. 따라서, NK 세포 주입을 사용하는 임상 프로토콜은 전형적으로 환자에서 NK 세포 생존을 연장시키는데 IL-2 투여에 의존한다. 그러나, IL-2는 상당한 부작용을 가질 수 있다. IL-2 투여는 발열 및 오한 외에, 보다 심하고 잠재적으로 치명적인 결과, 예를 들어 모세혈관 누출 증후군을 유발할 수 있다. IL-2의 용량 감소는 부작용의 위험을 감소시키지만, NK 세포 기능을 억제할 수 있고 어쩌면 그의 항암 효과를 무효로 할 수 있는 조절 T 세포를 자극할 수 있다.

천연 살해(NK) 세포의 충분히-확립된 백혈병-억제 활성은 NK 세포 주입에 대한 치료 가능성을 암시한다. 물질 및 방법
종양 세포주

말초 혈액 샘플을 건강한 성인 공여자로부터의 혈소판 콜렉션의 버려진 부산물로부터 수득하였다. 단핵세포를 Accu-Prep 밀도 스텝(애큐레이트(Accurate), 미국 뉴욕주 웨스트베리 소재)상에서 원심분리에 의해 정제하고 RPMI-1640에서 2회 세척하였다. CD56+ CD3-NK 세포를 증대시키기 위해서, 말초 혈액 단핵세포 및 유전자 변형된 K562-mb15-41BBL 세포주를 앞서 문헌[Fujisaki et al., Cancer Res, 69(9):4010-4017 (2009)]; 문헌[Imai et al., Blood, 106:376-383 (2005)]에 개시된 바와 같이 공-배양하였다. 간단히, 말초 혈액 단핵세포를 6-웰 조직 배양 플레이트에서 100 Gy-조사된 K562-mb15-41BBL 세포와, 10% FBS 항생제 및 10 IU/㎖의 재조합 인간 인터류킨-2(IL-2; 롯슈, 독일 만하임 소재)를 함유하는 SCGM(셀제닉스(CellGenix), 독일 프라이부르크 소재)에서 1.5:1 비로 배양하였다. 조직 배양 배지를 2일마다 부분적으로 교환하였다. 7일의 공-배양 후에, 잔여 T 세포를 다이나비즈(Dynabeads) CD3(인비트로젠)으로 제거하여, >95% CD56+ CD3- NK 세포를 함유하는 세포 집단을 생성시켰다.
플라스미드, 바이러스 생산 및 유전자 형질도입
pMSCV-IRES-GFP, pEQ-PAM3(-E) 및 pRDF를 세인트 주드 벡터 디벨로프먼트 앤드 프로덕션 셰어드 리소스로부터 수득하였다. 긴 신호 펩티드를 갖는 인터류킨-15(IL-15)를 주형으로서 사용된 인간 비장 cDNA 라이브러리(닥터 지 닐(Dr G. Neale), 세인트 주드 칠드런스 리써치 호스피틀)로부터 폴리머라제 쇄 반응(PCR)에 의해 서브-클로닝하였다. CD8α의 신호 펩티드를 암호화하는 cDNA, IL-15의 성숙한 펩티드 및 CD8α의 막관통 도메인을, PCR에 의한 중복 연장에 의한 이어맞추기(SOE-PCR)에 의해 조립하여 IL-15의 막-결합된 형태("mbIL15")를 암호화하고; IL-15의 야생형(CD8α 막관통 도메인에 결합되지 않은; "wtIL15")을 또한 시험 제조하였다. 생성된 발현 카세트를 쥐(murine) 줄기-세포 바이러스-내부 리보솜 유입 부위-녹색 형광 단백질(MSCV-IRES-GFP)의 EcRI 및 XhoI 부위에 서브-클로닝하였다.
RD144-위유형(pseudotyped) 레트로바이러스를 생성시키기 위해서, 3.0x106 293T 세포를 X-tremeGENE 9 DNA(롯슈, 독일 만하임 소재)를 사용하여 형질감염시키고, 18시간 동안 3.5 ㎍의 mbIL15 구조물을 암호화하는 cDNA, 3.5 ㎍의 pEQ-PAM3(-E) 및 3 ㎍의 pRDF가 있는 10-㎝ 조직 배양 디쉬에서 유지시켰다. 상기 배지를 24시간째에 10% FBS 및 항생제가 있는 RPMI-1640으로 교체한 후에, 레트로바이러스를 함유하는 순화배지(conditioned medium)를 36 내지 96시간째에 수확하고 레트로넥틴(타카라(Takara), 일본 오쓰 소재)으로 코팅된 폴리프로필렌 튜브에 가하고, 이를 1400 g에서 10분 동안 원심분리시키고 37 ℃ 및 5% CO2에서 4시간 동안 인큐베이션하였다. 추가적인 원심분리 및 상등액 제거 후에, 증대된 NK 세포(0.5-1x106)를 상기 튜브에 가하고 37 ℃에서 12시간 동안 방치하고; 이 단계를 2 내지 3일에 걸쳐 6회 이하로 반복하였다. 이어서 세포를 FBS, 항생제 및 100 IU/㎖의 IL-2가 있는 RPMI-1640에서 유지시켰다. 형질도입된 세포를 형질도입 후 3 내지 29일째에 분석하였다.
mbIL-15의 표면 발현을 항-인간 IL-15 항체(R&D, 미국 미네소타주 미니애폴리스 소재) 및 피코에리쓰린 접합된 염소 항-마우스 IgG1(서던 바이오테크(Southern Biotech), 미국 알라바마주 버밍햄 소재)을 사용하여 유식 세포측정에 의해 분석하였다. 항체 염색을 포르테사(Fortessa) 유식 세포계(벡톤 디킨슨(Becton Dickinson))로 검출하였다. 배양 상등액 중의 IL-15의 수준을 콴티킨 이뮤노어세이(Quantikine Immunoassay)(R&D)로 측정하였다.
시험관내에서 NK 세포의 기능 분석
시험관내에서 NK 세포 생존 및 생육을 평가하기 위해서, 형질도입된 NK 세포(1x106 세포/㎖)를 10% FBS 및 항생제가 있는 RPMI-1640에 재현탁하고, 24- 또는 96-웰 플레이트(코스타(Costar), 미국 뉴욕주 코닝 소재)의 웰에 넣고, IL-2(10 내지 100 IU/㎖)의 존재 또는 부재하에서 배양하였다. 생육성 GFP+ 세포의 수를 프로피디움 요오다이드로 염색 후, 애큐리(Accuri) C6 유식 세포계(벡톤 디킨슨)로 측정하였다. 일부 실험에서, 세포를 배양 전에 중화 항-IL-15 항체(R&D) 또는 아이소타입-합치된 비-반응성 항체와 10분 동안 인큐베이션하였다.
NK 세포 면역표현형분석법을 표에 나열된 항체들을 사용하여 수행하고, 포르테스 유식 세포계로 시각화하고 디바(Diva)(벡톤 디킨슨) 및 플로우조(FlowJo)(트리스타(TreeStar), 미국 오리건주 애슐랜드 소재) 소프트웨어에 의해 분석하였다. 인단백질 분석을 위해서, 모의- 및 mbIL15-형질도입된 NK 세포(1x107)를 IL-2 없이 48시간 동안 배양하였다. 세포 용해물을 20 mM 3-(N-모르폴리노) 프로판설폰산, 2 mM EGTA, 5 mM EDTA, 30 mM 나트륨 플루오라이드, 60 mM β-글리세로포스페이트, 20 mM 나트륨 피로포스페이트, 1 mM 나트륨 오쏘바나데이트, 1% 트리톤 X-100, 완전 미니 프로테아제 억제제 칵테일(롯슈, 독일 만하임 소재) 및 1 mM 디티오쓰레이톨을 함유하는 용해 완충제를 사용하여 제조하였다. 초음파 처리 후, 용해물을 -80 ℃에서 동결시키고 키넥스 안티바디 마이크로어레이 분석을 위해 드라이 아이스 중에서 키넥서스(Kinexus)(캐나다 밴쿠버 소재)로 발송하였다.
세포독성 분석을 위해서, 루시페라제-표지된 표적 세포 및 NK 세포(48시간 동안 IL-2 없이 배양)를 96-웰, 편평 바닥 흑색 뷰플레이트(Viewplate)(코닝)에 다양한 효과기:표적(E:T) 비로 도말하고, 4 또는 24시간 동안 배양하였다. 부착성 세포주를 37 ℃ 및 5% CO2에서 4시간 동안 인큐베이션한 후에 NK 세포를 가하여 세포 부착을 허용하였다. 항체-의존성 세포 세포독성 분석을 위해서, 리툭시맙(리툭산, 롯슈; 독일 만하임 소재), 트라스투주맙(허셉틴, 롯슈) 또는 정제된 인간 IgG(R&D 시스템스, 미국 미네소타주 미니애폴리스 소재)를 NK 세포 전에 가하였다(모두 1 ㎍/㎖). 이어서 상기 배양의 끝에서, 동등한 부피의 브라이트 글로(Bright-Glo) 루시페라제 시약(프로메가(Promega), 미국 위스콘신주 매디슨 소재)을 각 시험 웰에 가하고, 5분 후에, 발광을 플레이트 판독기를 사용하여 측정하고 Gen5 2.00 소프트웨어(둘 다 바이오텍(BioTek)으로부터, 미국 애리조나주 투손 소재)로 분석하였다. 각 플레이트에서, 표적 세포 생육력을 표적 세포만을 함유하는 웰로부터의 발광 신호를 사용하여 계산하였다. 모든 실험을 3회 중복 수행하였다.
세포용해성 과립의 방출을 측정하기 위해서, NK 세포(IL-2 부재하에서 48시간 동안 배양)를 4시간 동안 K562, U937 세포, 또는 722.221 세포 및 이들의 Cw6-발현 변체와 함께 공배양하였다. 우리는 상기 배양의 초기에 PE- 또는 PE-Cy7-접합된 항-CD107a 항체(BD 바이오사이언시즈) 및 1시간 후에 골지스탑(GolgiStop)(0.15 ㎕; BD 바이오사이언시즈)을 가하였다. CD107a+ NK 세포의 백분율을 유식 세포측정에 의해 측정하였다.
면역결핍 마우스에서 NK 세포의 증대 및 세포독성
생체내에서 NK 세포 증대를 시험하기 위해, mbIL15로 형질도입되거나 또는 모의-형질도입된 인간 NK 세포(6-9x106 세포/마우스)를 NOD.Cg-PrkdcscidIL2rgtmlWjl/SzJ(NOD/scid IL2RGnull) 마우스(잭슨 레보라토리즈, 미국 메인주 바하버 소재)의 꼬리 정맥에 주사하였다. 일부 마우스에서, 우리는 20000 IU의 IL-2를 주당 3회 복강내(i.p.) 주사하였다. 7일 및 11일째에, 혈액 세포를 세포 계수기(벡크만 쿨터)로 카운트하고; 인간 및 마우스 CD45+ 세포를 적혈구 용해 용액(인비트로젠)으로 처리하고 알로피코시아닌-접합된 마우스-항-인간 CD45 및 피코에리쓰린-접합된 래트 항-마우스 CD45 항체(둘 다 BD 바이오사이언시즈로부터)로 염색한 후에 유식 세포 측정에 의해 세었다. 안락사 후에, 골수, 간, 비장, 신장, 폐 및 뇌 중의 인간 NK 세포를 상기와 같이 세었다. 모든 동물 실험은 싱가포르 국립대학 연구 동물 보호 및 사용 위원회에 의해 승인된 프로토콜에 따라 수행하였다.
마우스에서 종양 세포 살해를 시험하기 위해서, 우리는 2개의 이종이식 모델을 제조하였다. 먼저, 루시페라제를 발현하는 U937 세포를 NOD.Cg-PrkdcscidIL2rgtmlWjl/SzJ(NOD/scid IL2RGnull) 마우스(1x104 세포/마우스)에 i.p. 주사하였다. 3일 후에, GFP 단독 또는 mbIL15를 함유하는 MSCV 벡터로 형질도입된 NK 세포를 i.p. 주사하고(1x107 세포/마우스); NK 세포 주사를 7일째에 반복하였다. 대조용으로서, 하나의 마우스 그룹에 NK 세포 대신에 조직 배양 배지를 제공하였다. 두 번째 모델에서, 마우스에게 ES8 세포를 생착시킨 다음(i.p.; 1x105 세포/마우스) 3일째에 1회 NK 세포 주사를 상기와 같이 주사하였다. 종양 생착 및 진행을, D-루시페린 칼륨염(3 ㎎/마우스)의 수용액을 i.p. 주사 후 5분째에 영상화를 시작하여, 제노젠(Xenogen) IVIS-200 시스템(캘리퍼 라이프 사이언시즈(Caliper Life Sciences, 미국 매사추세츠주 홉킨톤 소재)을 사용하여 평가하였다. 루시페라제-발현 세포로부터 방출된 광자를 리빙 이미지(Living Image) 4.3.1 소프트웨어 프로그램을 사용하여 정량분석하였다.
결과
IL-15 구조물의 설계 및 NK 세포에서 발현
본 명세서에 기재된 바와 같이, 2가지 형태의 IL15 유전자, 즉 인간 IL15 유전자가 CD8α의 막관통 도메인을 암호화하는 유전자에 결합된 구조물로부터 생성되는 막-결합된 형태("mbIL15"), 및 야생형의 변형되지 않은 형태("wtIL15")를 인간 NK 세포에 발현시켰다. 상기 두 구조물을 모두, 자극 세포주 K562-mb15-41BBL.28와 함께 말초 혈액 단핵세포를 배양한 후에 수득되는 증식성 NK 세포의 형질도입에 사용되는 GFP를 함유하는 MSCV 레트로바이러스 벡터에 삽입하였다(도 1A). 상기 배양의 끝에서, 레트로바이러스 형질도입 전에, 잔류 T-세포를 항-CD3 면역자기 비드로 고갈시켜 >95% 순수한 CD56+ CD3- 세포를 생성시켰다. 중간 GFP 발현은 mbIL15를 함유하는 구조물의 경우 71%(23% - 97%, n=60)였고, wtIL15를 함유하는 경우는 69%(범위, 20% - 91%, n-25)였다. 오직 GFP만을 함유하는 벡터로 또한 형질도입된 동일한 공여자로부터의 NK 세포는 84%(53% - 98%, n=60)의 중간 GFP 발현을 가졌다(도 1B).
mbIL15로 형질도입 후, IL-15를 NK 세포막상에 발현시켰다: 40% - 63%(중간, 52; n=7)의 GFP+ NK 세포는 항-IL15 항체에 의해 검출된 바와 같이 IL-15를 가졌다(도 1B). 대조적으로, wtIL15로 형질도입된 세포(n=4) 또는 모의 형질도입된 NK 세포(n=7)에서 IL-15는 검출할 수 없었다. 상기 형질도입된 NK 세포에 의한 용해성 IL-15의 생산을 24 및 48시간 배양 후 수집된 상등액을 시험함으로써 측정하였다. 도 1C에 나타낸 바와 같이, wtIL15를 발현하는 세포는 상당량의 IL-15를 분비한 반면, 이는 mbIL15-NK 세포에서 최소였으며 모의-형질도입된 NK 세포에서는 검출할 수 없었다.
IL-15를 발현하는 NK 세포는 자율 생존 및 증대 능력을 갖는다
IL-15의 발현이 NK 세포 생존의 유지에서 외인성 IL-2를 대체할 수 있는지를 측정하기 위해서, 15명의 공여자로부터의 NK 세포를 mbIL15 구조물로 형질도입시키고 IL-2의 부재하에서 배양하고; 이어서 배양 후 세포수를 모의-형질도입된 NK 세포와의 평행 배양에서의 경우와 비교하였다. 도 2A에 나타낸 바와 같이, mbIL-15의 발현은 NK 세포 생존을 대단히 증가시켰다: 7일의 배양 후에, 중간 세포 회수율은 85%인 반면 생육성 모의-형질도입된 NK 세포는 실질적으로 검출할 수 없었다(<1%; 대응표본 t 검정에 의해 P<0.0001). mbIL15의 효과는 항-IL-15 중화 항체를 상기 배양물에 첨가한 경우 현저하게 감소하였다(도 6A 내지 6B). 상기 15명의 공여자 중 9명에서, mbIL15 NK 세포의 회수율을 또한 wtIL15를 발현하는 NK 세포의 경우와 비교하였다: 이는 전자보다 현저하게 더 높았다(중간값, 85% 대 56%, P=0.026; 도 2A).
평행 실험에서, 외인성 IL-2의 존재하에서 IL15 발현의 지지 효과를 측정하였다. 배양물이 10 IU/㎖의 IL-2를 함유한 경우, mbIL15 또는 wtIL15를 발현하는 NK 세포의 7-일 회수율은 모의-형질도입된 세포의 경우보다 현저하게 더 높게 남아있었으며; 이러한 조건하에서 상기 2가지 형태의 IL15간에 현저한 차이는 주목되지 않았다(도 6A 내지 6B). 오직 외인성 IL-2가 고농도(100 IU/㎖)로 존재하는 경우에만, 모의-형질도입된 NK 세포의 7-일 회수율은 IL15로 형질도입된 NK 세포의 경우와 합치하였다(도 6A).
상기 9명의 공여자 중 6명으로부터의 증대된 NK 세포에 의한 실험에서, 저 용량 IL-2(10 IU/㎖) 하에 7일 이상 NK 세포 생존을 지지하는 mbIL15의 능력을 측정하였다. 14일째에, mbIL15 NK 세포수는 상기 6개의 배양물 중 4개에서 유지되거나 증가되었으며; 이들 중 2개에서 세포가 21일까지 추가로 증대되었다. 동일한 공여자로부터의 모의-형질도입된 NK 세포를 갖는 6개의 배양물 중 단지 2개만이 14일 및 21일째에 세포수를 유지하였으며, 세포 생육은 관찰되지 않았다; 21일째의 중간 세포 회수율은 mbIL15 NK 세포의 경우 205%이고 모의-형질도입된 NK 세포의 경우 80%였다. 따라서, 저 용량 IL-2의 존재하에서조차, mbIL15 발현은 상당한 생존 및 생육 이점을 부여하였다.
하나의 공여자로부터의 NK 세포의 배양물에서, 특히 높은 세포 회수율은 IL15가 발현된 7일째에 관찰되었다(IL-2의 부재하에서 mbIL15의 경우 261% 및 wtIL15의 경우 161%; 10 IU/㎖ IL-2의 경우 266% 및 188%). 이들 배양물을 2개월 동안 모니터하였으며 mbIL15의 발현에 의해 야기된 세포 증대 및 생존의 현저한 개선이 관찰되었다(도 2C). IL-2의 부재하에서조차, mbIL-15 NK 세포는 21일까지 계속해서 생존하였으며 상기 배양의 개시 후 75째에 여전히 검출될 수 있었던 반면, 모의-형질도입된 세포는 7일째에 검출할 수 없게 되었고 wtIL15-형질도입된 NK 세포는 42일째에 검출할 수 없게 되었다. 저 용량(10 IU/㎖)의 IL-2의 존재하에서, 상기 mbIL15-발현 NK 세포의 수는 상기 배양의 개시 후 2개월째에 최초에 시딩된 경우와 동일한 반면, 생육성 모의-형질도입된 및 wtIL15-형질도입된 NK 세포는 훨씬 더 일찍 감소하였다. 부록 도면 6B에 나타낸 바와 같이, IL-2를 고 용량(100 IU/㎖)으로 상기 배양물에 가한 경우, mbIL155 또는 wtIL15로 형질도입된 NK 세포는 유사한 지속 프로파일을 가졌으며, 상기 두 세포 유형은 모두 이러한 조건하에서조차 모의-형질도입된 NK 세포보다 더 오래 생존하였다.
생체내에서 mbIL15 NK 세포의 증대 및 귀소(homing)
시험관내에서 수행된 실험은 IL15 발현이 NK 세포의 생존 및 증대를 개선시키고 mbIL15는 전체적으로 보다 양호한 자극을 생성시킴을 가리켰다. 이어서 mbIL15 발현이 NOD/scid IL2RGnull 마우스에서 인간 NK 세포의 증대를 지속시키는지를 측정하였다. 4명의 공여자로부터의 활성화된 NK 세포를 mbIL15로 형질도입시키고(52% - 74% GFP-양성) 4마리의 마우스에 주사하였으며(공여자당 한 마리의 마우스); 4마리의 대조용 마우스에게 동일한 공여자로부터 모의-형질도입된 NK 세포를 주사하였다. mbIL15를 발현하는 NK 세포는 모의-형질도입된 NK 세포보다 훨씬 더 증대되었다: 주사 후 7일째에, mbIL15 NK 세포/혈액 ㎕의 중간수는 44.5(범위, 42-60) 대 모의-형질도입된 NK 세포의 경우 6.5(0-12)였다(P=0.004)(도 3A). 동일한 세포로 평행 실험을 수행하였으며, 이번에는 또한 20,000 IU 인간 IL-2를 2일마다 i.p. 투여하였다(도 3A). 이러한 조건하에서, mbIL15 NK 세포는 훨씬 더 증대된 반면(중간 NK 세포/㎕, 101; 범위, 60-167), 모의-형질도입된 세포는 낮게 남아있었다(중간, 18; 범위, 6-20; P=0.021).
주사 후 11일째에, mbIL15 NK 세포는, IL-2의 부재하에서 168.5 세포/㎕(범위, 94 - 355)의 말초 혈액 단핵 세포 및 IL-2가 또한 투여되었을 때 382 세포/㎕(151 - 710)을 포함하였다(도 3A, B). 대조적으로, 모의-형질도입된 NK 세포가 주사된 마우스에서, 인간 CD45 세포는 IL-2 부재하에서 실질적으로 검출될 수 없었으며, IL-2가 또한 주사된 경우 낮은 수준으로 존재하였다(중간, 27; 범위 9 - 207; P=0.026). 인간 CD45+ 세포는 또한 CD56을 발현하였으며 CD3은 없었다(도시 안 됨). 중요하게, GFP+의 비율은 주사전 66.5%±9.9%에서부터 7일째 93.8%±4.4% 및 11일째 94.8%±3.4%로 증가하였다(상기 두 비교 모두에 대해서 P<0.01).
11일째에 안락사 후, 상기 4마리 마우스 중 3마리를 다양한 조직 중 인간 CD45+ 세포의 존재에 대해 검사하였다. mbIL15가 발현된 경우, 상당수의 인간 NK 세포가 골수, 간, 비장, 신장 및 폐에서 검출되었으며; 모든 조직에서 숫자가 모의-형질도입된 세포에 대해 관찰된 경우보다 현저하게 더 높았다(도 3C): mbIL15를 발현하는 CD45+ 세포의 평균(±SD) 백분율은 모의-형질도입된 세포에 대한 0.04%±0.09% 및 0.4%±0.6%에 비해, IL-2 부재하에서 1.2%±1.5% 및 IL-2 존재하에서 3.0%±4.3%였다(각각 P<0.001 및 P=0.002). 유일한 예외는 mbIL15도 모의-형질도입된 NK 세포도 검출될 수 없었던 뇌였다.
mbIL15 자극의 기전
mbIL15가 트랜스(이웃하는 세포를 자극하는 하나의 NK 세포상에 제공된 IL-15(생리학적으로 발생하는 것으로 보고된 기전)) 또는 시스(동일한 세포에서 발현된 수용체에 대한 mbIL15의 직접적인 결합에 의해)로 세포를 우세하게 자극하는지를 측정하기 위해서, 배양물 중의 GFP+ 및 GFP- NK 세포의 비율을 배양 7일 후에 평가하였다. 상기 트랜스 기전이 우세한 경우, GFP+와 GFP- NK 세포간의 비는 배양 중 변경되지 않은 채로 있을 것이며; 시스가 우세한 경우 GFP+ 세포의 비율이 증가할 것이다. 도 4A는 상기와 같은 분석의 결과를 나타낸다: IL-2 없이 7일의 배양 후 검사된 NK 세포 가운데 GFP+ 세포의 백분율은 mbIL15가 발현된 경우 일관되게 증가한 반면, 모의-형질도입된 세포와의 배양에서는 증가하지 않았다: GFP+ 세포는 0일째에 전체 세포 집단의 71.2%±19.0% 대 80.5%±17.1%에 비해, 7일째에 95.9%±3.3% 대 57.5%±18.6%(P<0.0001)를 차지하였다. 따라서, NK 세포에서 발현된 mbIL-15에 의한 우세한 자극 기전은 자가분비이다.

시험관내 및 생체내에서 NK 세포 항-종양 세포독성에 대한 mbIL-15의 효과
mbIL15의 발현에 의해 유발된 NK 세포 생존 및 증식의 개선은 종양 세포의 NK-매개된 살해가 또한 증가할 듯함을 암시하였다. 이러한 생각을 먼저 mbIL15-NK 세포에 의해 발휘된 종양 세포 세포독성을 동일한 공여자로부터의 모의 형질도입된 NK 세포의 경우와 비교함으로써 시험하였다. 상이한 E:T 비 및 공-배양 지속기간에서, 총 90회의 실험 동안 백혈병 세포주 Nalm-6(B-계통 급성 림프모구성 백혈병), U937 및 K562(급성 골수성 백혈병)뿐만 아니라 다우디(B-세포 림프종), SKBR3(유방 암종) 및 ES8(유잉 육종)을 표적화하는 9명의 공여자로부터의 NK 세포를 사용하는 실험을 수행하였다. 도 5A는 24-시간 분석의 결과를 나타낸다: 중간 세포독성은 모의-형질도입된 NK 세포에 의해서 1:4 E:T에서 22% 및 1:1 E:T에서 54%였고; mbIL15 NK 세포에 의해서는 각각 71% 및 99%였다(P<0.0001). 개별적인 세포주들에 대한 결과를 도 7A 내지 7B에 나타낸다. 상기 증가된 세포독성은 배양물 중의 NL 세포의 생존 증가와 관련될 수도 있지만, K562 또는 U937 세포와의 배양 후 CD107a 염색에 의해 밝혀진 바와 같이, mbIL15-NK에 의한 세포용해 과립의 증가된 방출이 또한 관찰되었다(P=0.0067; 도 5B).

논의
암의 NK 세포-기반 요법의 성공을 결정하는 인자들 가운데 아마도 가장 근본적인 것은 NK 세포가 종양 세포감소를 생성시킬 것 같은 E:T 비를 성취하기에 충분한 수로 지속되는 것이다.32 인간 NK 세포에서 IL-15의 막-결합된 형태의 발현이 IL-2의 부재하에서 상기 세포의 자율적인 증대를 지지하고 생존을 연장하는 것으로 본 명세서에 입증되어 있다. mbIL15를 발현하는 NK 세포는 외인성 IL-2 없이 2개월까지 시험관내에서 유지될 수 있었다. mbIL15를 발현하는 NK 세포는 면역결핍 마우스 및 침윤된 다수의 조직에서 증대될 수 있었으며, 여기에서 모의-형질도입된 세포보다 훨씬 더 큰 수로 발견될 수 있었다. mbIL-15 NK 세포의 증대는 시험관내 및 생체내 모두에서 저농도의 IL-2에 의해 추가로 증가되었다. mbIL15의 발현은 NK 세포의 세포독성 능력을 손상시키지 않았다. 실제로, 이종이식 모델에서, mbIL15 NK 세포는 모의-형질도입된 세포의 경우보다 더 강력한 항암 활성을 발휘하였으며, 이는 상기 접근법이 IL-2 투여의 부작용을 피하면서 NK 세포 주입의 항종양 능력을 개선시킬 수도 있음을 암시한다.
본 명세서의 발견은 인간 NK 세포에서 IL-15의 엑토픽(ectopic) 발현이, IL-15가 분비된 형태보다 막-결합된 형태로 존재할 때 보다 강한 생존-촉진 효과를 유발하였음을 나타낸다. 그러나, 현저하게는 NK 세포에서 발현된 mbIL15는 IL-15가 다른 세포들에 의해 제공될 때 트랜스에서보다는 오히려 시스에서 우선적으로 자극된다. 즉, mbIL15는 동일한 세포상의 IL-15 수용체와 선택적으로 맞물려 자가분비 자극을 생성시키는 것으로 보인다. 이러한 기전은, 세포의 상당 부분이 강한 GFP 발현을 갖지만 표면상으로는 IL-15가 없는 것으로 보이는 IL-15 발현 패턴을 설명하며, 상기 패턴은 mbIL15-형질도입된 NK 세포를 항-IL-15 항체로 표지한 경우 일관되게 관찰된다(도 1B). 이들 세포에서 IL-15는 그의 수용체에 결합되고/되거나 내면화되기 때문에, 발현은 되지만 항체에 접근하기 쉽지 않은 것으로 가정된다. NK 세포의 임상 투여는 전형적으로는 생체내에서 상기 세포의 생존 및 증대를 지지하는 IL-2에 따라 변한다. 그러나, IL-2 투여와 관련된 다수의 부작용들은 잠재적으로 심각하며 종종 상기 사이토킨의 투여를 불충분하게 허용되게 한다. IL-2 투여의 중지 또는 그의 용량의 감소는 감소된 NK 세포 증대 및 비효율적인 항-종양 효과를 생성시킬 수 있으며, 이는 조절 T 세포의 자극에 의해 더욱 억제될 수 있다. 이 때문에, IL-2를 IL-15로 대체시키는 것은 잠재적으로 매력적이지만 IL-15의 임상 제형은 여전히 시험중이다. 상기는 붉은털 원숭이(rhesus macaques)에 투여될 때 전체적으로 잘 허용되었지만, 일부 동물에서 부작용, 예를 들어 설사, 구토, 체중감소, 일시적인 호중구감소증, 트랜스아미나제의 증가 및 저나트륨혈증이 관찰되었다. NK 세포의 항암 능력을 지지하는 상당한 임상적 증거가 존재한다. NK 세포는 또한 단클론 항체로 치료된 환자에서 항체-의존적인 세포 세포독성을 매개하는데 중요한 역할을 한다. 따라서, NK 세포의 주입은 다수의 환경에서 이로운 듯하다. 생체외에서 다수의 인간 NK 세포의 증대가 가능하며; 상기 목적을 위한 확고한 대규모 방법이 확립되었고 임상 시험에 사용되고 있다. 레트로바이러스 형질도입 또는 일렉트로포레이션(electroporation)에 의한 NK 세포의 유전자 변형이 또한 가능하다. 따라서, 본 명세서에 기재된 접근법의 임상-등급 조건으로의 이동은 현실적이며 mbIL15-NK 세포의 우수한 증대 및 세포독성에 의해 보장된다.

Claims (20)

- 제1항에 있어서,
IL-15가 막-결합된 폴리펩티드로서 및/또는 분비된 폴리펩티드로서 발현되는 NK 세포. - 제1항 또는 제2항에 있어서,
IL-15의 전부 또는 기능성 부분이 막관통 단백질의 전부 또는 일부에 융합되는 NK 세포. - 인터류킨-15(IL-15)의 전부 또는 기능성 부분을 발현하는 천연 살해(NK) 세포의 생성 방법으로서,
a) IL-15의 전부 또는 기능성 부분을 암호화하는 핵산을 상기 NK 세포에 도입시키고;
b) 상기 NK 세포를 상기 IL-15의 전부 또는 기능성 부분이 발현되는 조건하에서 유지시켜, IL-15의 전부 또는 기능성 부분을 발현하는 NK 세포를 생성시킴
을 포함하는 방법. - 제4항에 있어서,
NK 세포에 도입된 핵산이 CD8α의 신호 펩티드, IL-15의 전부 또는 기능성 부분 및 CD8α의 막관통 도메인의 전부 또는 일부를 포함하는 방법. - 제4항 또는 제5항에 있어서,
